Abstract:We report highly pathogenic avian influenza A(H5N1) virus in dairy cattle and cats in Kansas and Texas, United States, which reflects the continued spread of clade 2.3.4.4b viruses that entered the country in late 2021. Infected cattle experienced nonspecific illness, reduced feed intake and rumination, and an abrupt drop in milk production, but fatal systemic influenza infection developed in domestic cats fed raw (unpasteurized) colostrum and milk from affected cows. Cow-to-cow transmission appears to have occurred because infections were observed in cattle on Michigan, Idaho, and Ohio farms where avian influenza virus–infected cows were transported. Although the US Food and Drug Administration has indicated the commercial milk supply remains safe, the detection of influenza virus in unpasteurized bovine milk is a concern because of potential cross-species transmission. Continued surveillance of highly pathogenic avian influenza viruses in domestic production animals is needed to prevent cross-species and mammal-to-mammal transmission.

Abstract:In February and March of 2024, an unusual illness began affecting dairy herds primarily in Texas and neighboring states. The causative agent of this illness was ultimately confirmed in late March 2024 to be a strain of highly pathogenic avian influenza H5N1 belonging to clade 2.3.4.4b. In the months following the emergence of this viral disease in cattle, infections have spread to at least 191 herds in 13 states at the time of this writing in August 2024, primarily through cattle and human movement. Surprisingly, early examination of raw milk samples from clinically affected animals indicated that the virus had an affinity for the mammary tissue, and viral shedding into raw milk occurred at high levels, exceeding 108 log10 50% tissue culture infectious dose (TCID50) in some cases. These high viral loads coupled with evidence that farm cats who consumed raw milk from clinically ill animals were infected and exhibited high mortality rates, raised concerns about the safety of the US milk supply for human consumption. To date, 4 cow-associated human infections have been reported, all from farm employees with direct contact with infected animals. Several parameters ultimately affect the theoretical public health risk from consumption of dairy products manufactured from a milk supply containing H5N1, namely (1) initial viral load, (2) persistence of H5N1 in raw milk, (3) viral inactivation through processing practices including pasteurization, and (4) human susceptibility and infectious dose. In the short period since the emergence of this disease in dairy cattle in the United States, research has begun to answer these critical questions, although our knowledge is still quite limited at this time. Here we review the literature available from the current H5N1 outbreak in US dairy cattle, as well as selected relevant literature from previous research in other animal agriculture sectors, that affect our current understanding of the parameters associated with the food safety risk of this disease in the US dairy supply chain.

Abstract:The relentless march of a highly pathogenic avian influenza virus (HPAIV) strain, known as H5N1, to become an unprecedented panzootic continues unchecked. The leap of H5N1 clade 2.3.4.4b from Eurasia and Africa to North America in 2021 and its further spread to South America and the Antarctic have exposed new avian and mammalian populations to the virus and led to outbreaks on an unrivaled scale. The virus has infected wild birds across vast geographic regions and caused wildlife deaths in some of the world's most biodiverse ecosystems. Hundreds of millions of poultry have died or been culled, affecting global food security in some of the world's poorest regions. Numerous mammalian species, including sea lions and fur animals, have been infected. Outbreaks in dairy cows in the United States have been occurring for months, seemingly unchecked in most affected states. Why is there not a greater sense of urgency to control these infections?

Abstract:On March 24, 2024, the US Department of Agriculture (USDA) confirmed the first case of highly pathogenic avian influenza (HPAI) H5N1 clade 2.3.4.4b in dairy cattle. Following this initial detection, the virus was found to have spread to other species, including humans. Since then, at least 192 dairy herds across 13 states have tested positive for the virus. The emergence of HPAI H5N1 in dairy cattle poses a serious threat to the dairy industry, requiring an approach that integrates human, animal, and environmental health. Enhancing biosecurity, improving surveillance, and advancing research are critical to managing this challenge. Swift adaptation is needed to protect both public health and dairy production. Through this work, we discuss the epidemiological and clinical aspects of HPAI H5N1 in cattle by reviewing the spatiotemporal situation of the disease across the United States, the clinical signs and testing leading to a confirmed case, and the surveillance and control measures implemented at the national and state levels. Further, considering pathogenesis and risk factors, we discuss surveillance and control measures to implement within herds, and note areas where research is needed to understand how to control this emerging disease.

Abstract:The emergence and growth of highly pathogenic avian influenza(HPAI) A(H5N1) in dairy cows poses a growing threat for both the food supply and onward zoonosis to humans. Despite ongoing surveillance and control measures recommended by animal and public health authorities to limit viral spread, new herds continue to report infections. We show here that the continued spread between farms could be explained by the rapid pace of pathogen spread reported within farms, which greatly limits the potential effectiveness of these recommendations. Under reasonable surveillance strategies, we show that the time farms have to mobilize interventions is extremely limited, as few as a couple days and typically less than a week. Our findings suggest that passive surveillance measures, such as detection of H5N1 via weekly bulk milk testing, comes too late such that most infections have already occurred. For current interventions to be valuable, more sensitive and extensive surveillance is needed and an emphasis should be placed on biosecurity practices rather than reactive practices.

Abstract:During the past century, reductions in animal diseases have resulted in a safer, more uniform, and more economical food supply. In the United States, the passage of the 1906 Federal Meat Inspection Act mandated better sanitary conditions for slaughter and processing, as well as inspection of live animals and their processed products. Following World War II, Congress passed the Poultry Products Inspection Act. Both acts are regulated by the Food Safety and Inspection Service (FSIS) of the US Department of Agriculture (USDA). The USDA's Animal and Plant Health Inspection Service (APHIS) is responsible for regulations governing the health of live animals prior to slaughter. This article is a brief overview of the ways in which the current predominance of zoonotics among emerging diseases underscores the importance of veterinary health professionals and the need for continued coordination between animal-health and public-health officials. Examples of intersections between animal- and public-health concerns include bovine spongiform encephalopathy (BSE) and Johne's disease, as well as extending beyond food safety to diseases such as avian influenza (AI). In the United States, we have in place an extensive public and private infrastructure to address animal-health issues, including the necessary expertise and resources to address animal-health emergencies. However, many challenges remain, including a critical shortage of food-animal veterinarians. These challenges can be met by recruiting and training a cadre of additional food-supply veterinarians, pursuing new technologies, collaborating with public-health officials to create solutions, and sending a clear and consistent message to the public about important animal-health issues.

Abstract:Highly pathogenic avian influenza (HPAI) viruses with H5 hemagglutinin (HA) genes (clade 2.3.4.4b) are causing an ongoing panzootic in wild birds. Circulation of these viruses is associated with spillover infections in multiple species of mammals, including a large, unprecedented outbreak in American dairy cattle. With limited testing of dairy herds, there is an unmet need for genomic surveillance. Infected cattle can shed high amounts of HPAI H5N1 viruses in milk, allowing detection in pasteurized retail dairy samples. Over a 2-month sampling period in one Midwestern city, we obtained dairy products processed in 20 different states. Here we demonstrate that a tiled-amplicon sequencing approach produced over 90% genome coverage at greater than 20x depth from 5 of 13 viral RNA positive samples. A combination of RT-qPCR testing and sequencing from retail dairy prod- ucts can be a useful component of a One Health framework for responding to the avian influenza outbreak in cattle.

Abstract:Infections with the highly pathogenic avian influenza (HPAI) H5N1 clade 2.3.4.4b virus have resulted in the death of millions of domestic birds and thousands of wild birds in the U.S. since January, 2022. Throughout this outbreak, spillovers of the virus to mammals have been frequently documented. Here, we report the detection of HPAI H5N1 virus in dairy cattle herds across several states in the U.S. The affected cows displayed clinical signs encompassing decreased feed intake, altered fecal consistency, respiratory distress, and decreased milk production with abnormal milk. Infectious virus and RNA were consistently detected in milk collected from affected cows. Viral staining in tissues revealed a distinct tropism of the virus for the epithelial cells lining the alveoli of the mammary gland in cows. Analysis of whole genome sequences obtained from dairy cows, birds, domestic cats, and a racoon from affected farms indicated multidirectional interspecies transmissions. Epidemiologic and genomic data revealed efficient cow-to-cow transmission after healthy cows from an affected farm were transported to a premise in a different state. These results demonstrate the transmission of HPAI H5N1 clade 2.3.4.4b virus at a non-traditional interface and to a new and highly relevant livestock species, underscoring the ability of the virus to cross species barriers.

Abstract:Context: The risk of possible transmission of bovine spongiform encephalopathy (BSE) in the United States is a substantial public health concern. Objective: To systematically review the current scientific literature and discuss legislation and regulations that have been implemented to prevent the disease. Methods: Literature review using the MEDLINE, EMBASE, and Lexis/Nexis databases for 1975 through 1997 on the terms bovine spongiform encephalopathy, prion diseases, prions, and Creutzfeldt-Jakob syndrome. The Internet was used to identify regulatory actions and health surveillance. Data extraction: MEDLINE, EMBASE, and Lexis/Nexis databases were searched from 1975 through 1997 for English-language articles that provided information on assessment of transmission risk. Results: Unique circumstances in the United Kingdom caused the emergence and propagation of BSE in cattle, including widespread use of meat and bonemeal cattle feed derived from scrapie-infected sheep and the adoption of a new type of processing that did not reduce the amount of infectious prions prior to feeding. Many of these circumstances do not exist in the United States. In the United Kingdom, new variant Creutzfeldt-Jakob disease probably resulted from the ingestion of BSE-contaminated processed beef. The United Kingdom and the European Union now have strong regulations in place to stop the spread of BSE. While BSE has not been observed in the United States, the US government has surveillance and response plans in effect. Conclusions: Current risk of transmission of BSE in the United States is minimal because (1) BSE has not been shown to exist in this country; (2) adequate regulations exist to prevent entry of foreign sources of BSE into the United States; (3) adequate regulations exist to prevent undetected cases of BSE from uncontrolled amplification within the US cattle population; and (4) adequate preventive guidelines exist to prevent high-risk bovine materials from contaminating products intended for human consumption.
