Rigid fixation of the sternum: a comparative biomechanical study

Abstract:Rigid fixation of the sternum is becoming increasingly common among sternal closure techniques as a result of the positive findings obtained from clinical studies. However, experimental and numerical studies examining the mechanical behaviour of sternal closures made with plates are very few. In this study, a comprehensive numerical investigation is presented on the mechanical behaviour of sternal rigid fixation techniques. For the first time in the literature, numerical models are validated with experimental results and a total of 11 different closure techniques using x-shaped, star-shaped and straight-shaped plates are examined by finite element analyses. Lateral distraction and longitudinal shear loads are applied to the models and the allowable loads and stiffness values of the techniques are determined and compared. Considering both loading conditions, the 4X method is determined as the optimum closure technique, and the closures made with only straight-shaped plates are found to be the most inadequate methods. In addition, it is determined that the plates should be applied as close to the xiphoid region as possible and attention should be paid to the bending effect on the sternum.
