Abstract:Stroke is becoming a widely concerned social problem, and robot-assisted devices have made considerable contributions in the training and treatment of rehabilitation. Due to the compliance and continuous deformation capacity, rehabilitation devices driven by soft actuators are attached to widespread attention. Considering the large output force of pneumatic artificial muscle (PAM) and the biological musculoskeletal structure, an antagonistic PAM-driven rehabilitation robotic device is developed. To fulfill the need for control of the proposed device, a knowledge-guided data-driven modeling approach is used and an adaptive feedforward–feedback control approach is presented to ensure the motion accuracy under large deformation motion with high frequency. Finally, several simulations and experiments are carried out to evaluate the performance of the developed system, and the results show that the developed system with the proposed controller can achieve expected control performance under various operations.

Abstract:Upper limb functionality is essential to perform activities of daily living. It is critical to investigate neurorehabilitation therapies in order to improve upper limb functionality in post-stroke patients. This paper presents a soft-robotic bilateral system to provide rehabilitation therapy for hand and wrist joints. A sensorized glove that tracks finger and wrist joint movements is worn on the healthy limb, which guides the movement of the paretic limb. The input of sensors from the healthy limb is provided to the soft robotic exoskeleton attached to the paretic limb to mimic the motion. A proportional derivative flow-based control algorithm is used to perform bilateral therapy. To test the feasibility of the developed system, two different applications are performed experimentally: (1) Wrist exercise with a dumbbell, and (2) Object pick-and-place task. The initial tests of the developed system verified its capability to perform bilateral therapy.

Abstract:The preliminary test results of a novel robotic hand rehabilitation device aimed at treatment for the loss of motor abilities in the fingers and thumb due to stroke are presented. This device has been developed in collaboration with physiotherapists who regularly treat individuals who have suffered from a stroke. The device was tested on healthy adults to ensure comfort, user accessibility, and repeatability for various hand sizes in preparation for obtaining permission from regulatory bodies and implementing the design in a full clinical trial. Trials were conducted with 52 healthy individuals ranging in age from 19 to 93 with an average age of 58. A comfort survey and force data ANOVA were performed to measure hand motions and ensure the repeatability and accessibility of the system. Readings from the force sensor (p < 0.05) showed no significant difference between repetitions for each participant. All subjects considered the device comfortable. The device scored a mean comfort value of 8.5/10 on all comfort surveys and received the approval of all physiotherapists involved. The device has satisfied all design specifications, and the positive results of the participants suggest that it can be considered safe and reliable. It can therefore be moved forward for clinical trials with post-stroke users.

An Upper Limb Rehabilitation Exercise Status Identification System Based on Machine Learning and IoT

Abstract:Rapid increase in stroke incidence coupled with the high cost and limited availability of healthcare professionals has made stroke rehabilitation process inaccessible to many patients in developing countries. Stroke rehabilitation exercises are of critical importance in ensuring quick and lasting recovery. A machine learning-based system that can automatically identify the completion status of upper limb rehabilitation exercises is presented in this study. Proposed system can detect completion status of twelve hand and arm rehabilitation exercises: six hand, four forearm and two shoulder exercises. Accelerometers record the movement of the upper limb part being exercised in the form of data sequences. Six curve-fitting-based machine learning model types are trained and evaluated for their effectiveness in modelling these sequences. This resulted in a total of 216 models (i.e. 12 exercises × 3 axes of motion × 6 model types) being evaluated on three performance measures: SSE, RMSE and R2. Best-performing model for each exercise is plugged into the proposed exercise completion status identification algorithm. System effectiveness is validated using four performance measures: accuracy, precision, recall and F1-score. Proposed system is demonstrated to be capable of detecting the exercise completion status with up to 90% accuracy. The proposed system is connected to cloud via an IoT interface with a dashboard type visualization system that allows for the healthcare professionals to remotely monitor the progress for each patient as well as carry out offline analysis. A game interaction interface is also provided to enable interaction with video games, helping in higher patient engagement while performing exercises.

Abstract:This paper presents a novel hand-grip system for evaluation and rehabilitation of individuals with stroke. This system contains a pair of hand dynamometers, which are sensors for the input of grip force, and LabVIEW VI software to translate the grip force into meaningful data. The hand-grip system requires the subjects to move a small ball on a computer screen toward a target, using the hand grip sensor to control the movement of the ball. It goes far beyond measuring maximum grip strength, a common clinical assessment, to adapt grip force and coordinate hands to perform daily functional activities. This system also provides advancement in home-rehabilitation options to improve the hand-grip control of individuals with stroke.

Abstract:This chapter focuses on a patient self-controlled rehabilitation system using our developed exoskeleton-type hand-motion-assist robot and tele-rehabilitation. The virtual reality-enhanced new hand rehabilitation support system, which we have developed for stroke patients in the acute stage, is aiming to allow such patients to conduct every day exercises by themselves without supervisors. This system features a multi-DOF motion assistance device, a virtual reality interface for patients, and a symmetrical master-slave motion assistance training strategy called ”self-motion control”, in which the stroke patients' healthy hand on the master side creates the assistance motion for the impaired hand on the slave side. Moreover, a tele-rehabilitation system consisting of a hand rehabilitation support system for the patients, an anthropomorphic robot hand for the therapist, and a remote monitoring system for diagnosing the degree of recovery is explained.

Abstract:Robotic devices for rehabilitation and training is a promising and challenging research topic with a potentially huge social impact. The availability of tools for autonomously performing physiotherapy exercises increases their efficiency, provides supplementary information about results and progress, reduces physiotherapists’ efforts and the need of their physical presence during exercise sessions, and encourages autonomy and independence in people with disabilities. Nevertheless, supportive technologies developed without the inputs and feedback of the end-user throughout the design process are less likely to be adopted for their intended purpose and use case. In this article, we propose a modular hand/wrist exoskeleton that actuates the wrist flexion/extension and adduction/abduction motions and hand fingers flexion/extension motions. It is designed to be wearable and easy to control and manage and can be used by the patient in collaboration with the physiotherapist or autonomously. A user-centered design perspective has been employed in all the design and development phases. This article introduces the main features of the device and presents some tests conducted with a user having limited hand and wrist mobility.

Abstract:As stroke is one of the most morbidity disease, some rehabilitation devices has been developed to solve the problem, but there isn’t maturation in hand. This paper presents a new hand exoskeleton rehabilitation device to improve the result of rehabilitation and help doctors to perfect theory about hand rehabilitation. We set 8 DOF in this three fingers device, with 2 active DOF and 1 coupled DOF in index or middle finger, 1 active DOF and 1 passive DOF in thumb. The 5 active DOF is driven by motors. The transmission consists of screw nut and multi-link mechanism. We control the device through a typical secondary control system. As this device is designed to help patients not only regain the range of flexion/extension but also complete some typical simple grasping movements. To substantiate the feasibility, an experiment has been carried out. Finally, Suggestions for improvement are given.

Abstract:Due to the ageing population and an increasing number of stroke patients, we see the potential future of rehabilitation in telerehabilitation, which might alleviate the workload of physiotherapists and occupational therapists. In order to enable the use of telerehabilitation, devices aimed for home and independent use need to be developed. This paper describes the design of a robotic device for post-stroke wrist and finger rehabilitation and evaluates the movement it can perform. Six healthy subjects were tested in three experimental conditions: performing a coupled movement of wrist and fingers from flexion to extension without the device, with a passive device, and with an active device. The kinematics of the hand were captured using three Optotrak Certus motion capture systems and tracking 11 infrared active light-emitting diode (LED) markers. The results are presented in the form of base-line trajectories for all middle finger (MF) joints. In addition, the deviations of trajectories between conditions across all subjects were computed for the metacarpophalangeal (MCP) joint and fingertip of the MF and pinkie (PF) finger. Deviations from the base-line trajectory between measurement protocols and the root-mean-square deviation (RMSD) values indicate that the motion of the hand, imposed by the developed device, is comparable to the unconstrained motion of the healthy subjects, especially when moving into the extension, opening the hand.

Abstract:One of the most difficult parts of stroke therapy is hand mobility recovery. Indeed, stroke is a serious medical disorder that can seriously impair hand and locomotor movement. To improve hand function in stroke patients, new medical technologies, such as various wearable devices and rehabilitation therapies, are being developed. In this study, a new design of electromyography (EMG)-controlled 3D-printed hand exoskeleton is presented. The exoskeleton was created to help stroke victims with their gripping abilities. Computer-aided design software was used to create the device’s 3D architecture, which was then printed using a polylactic acid filament. For online classifications, the performance of two classifiers—the support vector machine (SVM) and the K-near neighbor (KNN)—was compared. The Robot Operating System (ROS) connects all the various system nodes and generates the decision for the hand exoskeleton. The selected classifiers had high accuracy, reaching up to 98% for online classification performed with healthy subjects. These findings imply that the new wearable exoskeleton, which could be controlled in accordance with the subjects’ motion intentions, could aid in hand rehabilitation for a wider motion range and greater dexterity.

Abstract:Introduction: The RobHand (Robot for Hand Rehabilitation) is a robotic neuromotor rehabilitation exoskeleton that assists in performing flexion and extension movements of the fingers. The present case study assesses changes in manual function and hand muscle strength of four selected stroke patients after completion of an established training program. In addition, safety and user satisfaction are also evaluated. Methods: The training program consisted of 16 sessions; two 60-minute training sessions per week for eight consecutive weeks. During each session, patients moved through six consecutive rehabilitation stages using the RobHand. Manual function assessments were applied before and after the training program and safety tests were carried out after each session. A user evaluation questionnaire was filled out after each patient completed the program. Results: The safety test showed the absence of significant adverse events, such as skin lesions or fatigue. An average score of 4 out of 5 was obtained on the Quebec User Evaluation of Satisfaction with Assistive Technology 2.0 Scale. Users were very satisfied with the weight, comfort, and quality of professional services. A Kruskal-Wallis test revealed that there were not statistically significant changes in the manual function tests between the beginning and the end of the training program. Discussion: It can be concluded that the RobHand is a safe rehabilitation technology and users were satisfied with the system. No statistically significant differences in manual function were found. This could be due to the high influence of the stroke stage on motor recovery since the study was performed with chronic patients. Hence, future studies should evaluate the rehabilitation effectiveness of the repetitive use of the RobHand exoskeleton on subacute patients. Abstract:SURVEY of the l i terature on the repair of nerve defects in per ipheral nerve graft ing emphasizes the difficulty of inhibiting the immune response of the host to the graft . Medawar , 33'34 in 1944 and 1945, stressed the impor tance of the immune reaction. Since the first autogenous nerve t ransplanta t ions on the dog by Phil l ipeaux and Vulpian in 18703s and the first homograf t in the h u m a n by Albert , in 1878,1 various techniques have been continued until the present t ime. Abstract:There are many hemiplegic patients with hand dysfunction caused by stroke. Restoring hand function can effectively improve the quality of life of patients. Traditional rehabilitation training can improve hand strength and flexibility, but there are few medical staff and high workload, which may easily lead to unstable training effect. Therefore, this letter designed a soft hand rehabilitation robot with multiple training modes. By analyzing the natural movements of human fingers, the flexible exoskeleton finger and driving unit are designed. The kinematics of the finger are derived to achieve uniform changes in the angle of the finger joints. The hand rehabilitation robot can support multiple training modes such as individual finger training, five-finger collaborative training, and left-right hand symmetry training. Experiments show that the hand rehabilitation robot can carry out continuous and stable finger joint training, and has a good rehabilitation training effect.

Abstract:Hand rehabilitation is critical to the recovery of post-stroke patients. However, designing a modular mechanism of the hand to improve the human-machine compatibility and precision of operation is still a challenge. This paper proposes a new type of hand exoskeleton rehabilitation robot with nine degrees of freedom. With the flexible rods, the passive range of motion for finger adduction/abduction is extended under the premise of independent flexion/extension of the metacarpophalangeal and proximal interphalangeal joint. Based on hand anatomy, the relationship between the offset of the metacarpophalangeal joint and the body height in the process of flexion and extension is discussed, and it is applied to the structure optimization and control system. The genetic algorithm is employed to achieve the size optimization, and the kinematics is analyzed. Finally, a prototype is built and preliminary experiments are carried out, including the range of motion and the grasping ability of the robot. The experimental results show that the robot can realize the patients' hand rehabilitation function and has certain adaptability.

Abstract:Common disabilities like stroke and spinal cord injuries may cause loss of motor function in hands. They can be treated with robot assisted rehabilitation techniques, like continuously opening and closing the hand with help of a robot, in a cheaper, and less time consuming manner than traditional methods. Hand exoskeletons are developed to assist rehabilitation, but their bulky nature brings with it certain challenges. As soft robots use elastomeric and fabric elements rather than heavy links, and operate with pneumatic, hydraulic or tendon based rather than traditional rotary or linear motors, soft hand exoskeletons are deemed a better option in relation to rehabilitation.

Abstract:It is well understood that motor dysfunctions in patients can be restored through a certain amount of repetitive training. Robots and virtual reality offer an excellent solution to incentivize and optimize training intensity while also reducing the workload for therapists. In this paper, leveraging virtual reality technologies and haptic robots, we have developed a cost-effective robot-assisted training system and three training exercises for hand rehabilitation. The Unity game engine was utilized to create training scenarios where hand avatars interact with virtual objects. Off-the-shelf haptic robots were employed to manipulate the hand avatars, enabling completion of training tasks with a realistic tactile experience. Haptic feedback is rendered using impedance control laws. Additionally, to address the needs of both patients and therapists we propose to employ robot-assisted force and feedback force. These mechanisms are designed to provide assistance to patients during tasks if needs and to transmit the patient's force feedback to the therapist, facilitating comprehensive interaction and communication between the two parties. Several experiments were conducted involving six healthy subjects, who used electrode stimulation to simulate impaired limbs, to demonstrate the effectiveness of the proposed training frameworks.

Abstract:Post stroke rehabilitation consumes a huge amount of health care resources in terms of costs related to hospital and home assistance. Recently, robot-assisted rehabilitation has been adapted to support physiotherapists in providing high-intensity and repetitive rehabilitation sessions. It has been observed that robotics offers an objective and reliable instrumented tool to monitor patient’s progress and accurately assess their motor function. Each finger is attached to an instrumented mechanism which allowing force control and a mostly linear displacement. This paper presents a novel finger rehabilitation approach for acute paralyzed stroke survivors using a wearable robotic interface for hand motor function recovery. The device designed based on biomechanics measurements, able to assist the subject in opening and closing movements. It capable to adapt with various hand shapes and finger sizes. Main features of the interface include a differential system, and a lead screw mechanism which allows independent movement of the five fingers with actuators. The device is safe, easily transportable, and offers multiple training possibilities. The prototype deployment was carried out to determine the requirements for a finger rehabilitation device, the design and characterization of the complete system. Offering ease of use and affordability, the device has great potential to be deployed for individualized rehabilitation session for patients who have to undergo therapy in their home.

Abstract:Stroke and related complications such as hemiplegia and disability create huge burdens for human society in the 21st century, which leads to a great need for rehabilitation and daily life assistance. To address this issue, continuous efforts are devoted in human–machine interaction (HMI) technology, which aims to capture and recognize users’ intentions and fulfil their needs via physical response. Based on the physiological structure of the human hand, a dimension-adjustable linkage-driven hand exoskeleton with 10 active degrees of freedom (DoFs) and 3 passive DoFs is proposed in this study, which grants high-level synergy with the human hand. Considering the weight of the adopted linkage design, the hand exoskeleton can be mounted on the existing up-limb exoskeleton system, which greatly diminishes the burden for users. Three rehabilitation/daily life assistance modes are developed (namely, robot-in-charge, therapist-in-charge, and patient-in-charge modes) to meet specific personal needs. To realize HMI, a thin-film force sensor matrix and Inertial Measurement Units (IMUs) are installed in both the hand exoskeleton and the corresponding controller. Outstanding sensor–machine synergy is confirmed by trigger rate evaluation, Kernel Density Estimation (KDE), and a confusion matrix. To recognize user intention, a genetic algorithm (GA) is applied to search for the optimal hyperparameters of a 1D Convolutional Neural Network (CNN), and the average intention-recognition accuracy for the eight actions/gestures examined reaches 97.1% (based on K-fold cross-validation). The hand exoskeleton system provides the possibility for people with limited exercise ability to conduct self-rehabilitation and complex daily activities.

Abstract:Abstract Background Hand rehabilitation is core to helping stroke survivors regain activities of daily living. Recent studies have suggested that the use of electroencephalography-based brain-computer interfaces (BCI) can promote this process. Here, we report the first systematic examination of the literature on the use of BCI-robot systems for the rehabilitation of fine motor skills associated with hand movement and profile these systems from a technical and clinical perspective. Methods A search for January 2010–October 2019 articles using Ovid MEDLINE, Embase, PEDro, PsycINFO, IEEE Xplore and Cochrane Library databases was performed. The selection criteria included BCI-hand robotic systems for rehabilitation at different stages of development involving tests on healthy participants or people who have had a stroke. Data fields include those related to study design, participant characteristics, technical specifications of the system, and clinical outcome measures. Results 30 studies were identified as eligible for qualitative review and among these, 11 studies involved testing a BCI-hand robot on chronic and subacute stroke patients. Statistically significant improvements in motor assessment scores relative to controls were observed for three BCI-hand robot interventions. The degree of robot control for the majority of studies was limited to triggering the device to perform grasping or pinching movements using motor imagery. Most employed a combination of kinaesthetic and visual response via the robotic device and display screen, respectively, to match feedback to motor imagery. Conclusion 19 out of 30 studies on BCI-robotic systems for hand rehabilitation report systems at prototype or pre-clinical stages of development. We identified large heterogeneity in reporting and emphasise the need to develop a standard protocol for assessing technical and clinical outcomes so that the necessary evidence base on efficiency and efficacy can be developed.
