Abstract:BACKGROUND:Lung cancer is one of the most prevalent malignancies worldwide. The number of anatomic lung cancer resections performed via video-assisted thoracoscopic surgery (VATS) is growing rapidly. Staplers are widely used in VATS procedures, but there is limited clinical data regarding how they might affect performance and postoperative outcomes, including air leak. This clinical trial assessed the use of a powered stapler in VATS lung resection, with a primary study endpoint being occurrence and duration of air leak and prolonged air leak (PAL).METHODS:Data was collected from a single arm, multi-center study in Chinese patients receiving VATS wedge resection or lobectomy. Intra-operative data included surgery duration; cartridge selection for ligation/transection of bronchus, major vessels, and lung parenchyma; staple line interventions; blood loss; and device usage. Post-operative data included air leak assessments, chest tube duration, length of hospital stay, and adverse events (AEs).RESULTS:A total of 94 procedures across four institutions in China were included in the final analysis: 15 wedge resections, 74 lobectomies, and five wedge resections followed by lobectomies. Post-operative air leak occurred in five (5.3%) patients who had lobectomy procedures, with PAL in one (1.1%) patient. Sites were generally consistent relative to cartridge use by tissue type. The incidence of stapler firings requiring surgical interventions was seven out of 550 (1.3%). Surgeons participating in the study were satisfied with the articulation and overall usability of the stapler.CONCLUSIONS:The powered staplers make the VATS procedure easier for the surgeons and have achieved intra- and post-operative patient outcomes comparable to those previously reported.

Abstract:BackgroundAlthough thoracoscopic stapled bullectomy is a standard procedure for primary spontaneous pneumothorax (PSP), the postoperative recurrence rate is high. We investigated whether using a Vicryl (Ethicon, Somerville, NJ) mesh to cover the staple line after bullectomy reduces the postoperative recurrence rate.MethodsOur single-blind, parallel-group, prospective, randomized controlled trial at 2 medical centers in Taiwan studied patients with PSP who were aged 15 to 50 years and required thoracoscopic bullectomy. On the day of operation, patients were randomly assigned (1:1) to receive Vicryl mesh (mesh group) or not (control group) after thoracoscopic bullectomy with linear stapling and mechanical apical pleural abrasion. Randomization was achieved using computer-generated random numbers in sealed envelopes. Our primary end point was the pneumothorax recurrence rate within 1 year after the operation (clinicaltrials.gov number, NCT01848860.)ResultsBetween June 2013 and March 2016, 102 patients were assigned to the mesh group and 102 to the control group. Within 1 year after operation, recurrent pneumothorax was diagnosed in 3 patients (2.9%) in the mesh group compared with 16 (15.7%) in the control group (P = .005). The short-term postoperative results and hospitalization duration were comparable between the groups.ConclusionsFor thoracoscopic bullectomy with linear stapling and mechanical apical pleural abrasion, the use of a Vicryl mesh to cover the staple line is effective for reducing the postoperative recurrence of pneumothorax. Vicryl mesh coverage can be considered an optimal adjunct to the standard surgical procedure for PSP.

Abstract:Objective To Evaluate the feasibility of video-assisted thoracoscopic surgery(VATS) reoperation for recurrent primary spontaneous pneumothorax.Methods The clinical and follow-up date of 35 recurrent pneumothorax patients,who received reoperation from January 2004 to November 2013 at the Beijing Haidian Hospital and Peking University People's Hospital were retrospectively reviewed.Chi square,t test and Kaplan-Meier curve was used to evaluate risk factors.Results The relapsed 35 patients included male 30,female 5.The median age was 22 years old(range 16-50 years).The median operating time was 100 min (range 40-190 min),the median blood loss was 50 ml (range 5-200 ml) and the median postoperative stay in the hospital was 7 days(range 3-13 days).VATS was successfully performed for all patients,without conversion.No morbidity or mortality occurred.The median follow-up time was 49months(range 12-133 months).Adhesions were observed from 34 patients,wherein,the severity ranged from mild to dense.New bullae or blebs were found in 26 patients (74.29％),9 (34.62％) of which were adjacent to the stapled incisal margin.No bullae or blebs were found on the 9 patients (26.71％)that underwent reoperation within 18 months from their initial operation.No recurrence was observed during the follow-up.Conclusion The recurrence of spontaneous pneumothorax was mostly caused by the formation of new bullae or blebs near the staple lines.Reoperation for recurrent pneumothorax was safe and advocated.VATS operation includes the dissection of pleural adhesions,wedge resection of neoformative bullae or blebs and elimination of the residual apical space.
Safety and efficacy of video-assisted thoracic surgical techniques for the treatment of spontaneous pneumothorax

Abstract:Video-assisted thoracic surgery has been widely used in the treatment of spontaneous pneumothorax despite a paucity of data regarding the relative safety and long-term efficacy for this procedure. We reviewed 113 consecutive patients (68 male and 45 female patients, aged 15 to 92 years, mean 35.1) who underwent 121 video-assisted thoracic surgical procedures during 119 hospitalizations from 1991 through 1993. Recurrent ipsilateral pneumothorax was the most frequent indication for surgery and occurred in 77 patients (65%). The most common method of management was stapling of an identified bleb in the lung, which was undertaken in 105 (87%) patients. No operative deaths occurred. Complications included an air leak lasting longer than 5 days in 10 (8%) patients, two of whom required second procedures for definitive management. No episodes of postoperative bleeding or empyema occurred. The postoperative stay ranged from 1 day to 39 days (median 3 days, average 4.3 days) and 99 patients (84%) were discharged within 5 days. Mean follow-up was 13.1 months and ranged from 1 to 34 months. Eleven patients (10%) were lost to follow-up. Ipsilateral pneumothorax recurred after five of 121 procedures (4.1%). Twelve perioperative parameters (age, gender, race, smoking history, site of pneumothorax, severity of pneumothorax, operative indications, number of blebs, site of blebs, bleb ablation, method of pleurodesis, and prolonged postoperative air leak) were entered into univariate and multivariate analysis to identify significant independent predictors of recurrence. The only independent predictor of recurrence was the failure to identify and ablate a bleb at operation, which resulted in a 23% recurrence rate versus a 1.8% rate in those with ablated blebs (p < 0.001). These data suggest that video-assisted thoracic surgery is a viable alternative to thoracotomy for the treatment of recurrent spontaneous pneumothorax. It results in a short hospital stay, low morbidity, high patient acceptance, and a low rate of recurrence.

Abstract:Background: The development of minimally invasive surgical approaches has revolutionized surgical care and greatly improved surgical outcomes. This study aimed to evaluate the clinical effectiveness of a powered vascular stapler (PVS) during video-assisted thoracoscopic surgery (VATS) lobectomy. Methods: This prospective, multi-center, post-market clinical study in China enrolled 50 patients with either a suspected or formal diagnosis of clinical stage IA to IIB non-small cell lung cancer (NSCLC) scheduled for VATS lobectomy. The clinical effectiveness of the PVS for successful pulmonary artery (PA)/ pulmonary vein (PV) transection was evaluated. In addition, the surgeon's stress, device usability, and surgeon satisfaction were measured using multiple questionnaires. Results: A total of 167 PAs/PVs were transected with 3 (1.8%) requiring intra-operative intervention. Fourteen of the 50 patients (28.0%) experienced at least one adverse event (AE), among whom 5 (10.0%) suffered from serious AEs. There were no postoperative hemorrhagic complications related to transection of the PA/PV with PVS. Surgeon satisfaction was surveyed by questionnaire after each of the 50 procedures resulting in a 96% reported satisfaction with device usability, specifically related to a low stress load and an increase in work efficiency. Conclusions: For VATS lobectomy, the PVS demonstrated a positive surgical effectiveness and value in cognitive and physical distress reduction. Complications following VATS lobectomy to treat NSCLC were generally low and as expected. Intraoperative complications were few and there were no postoperative complications related to the transection of the PA and PV during VATS lobectomy. Favorable results were reported on the surgeon satisfaction questionnaire regarding usability and surgeon stress.
Uniportal approach as an alternative to the three-portal approach to video-assisted thoracic surgery for primary spontaneous pneumothorax

Abstract:BACKGROUND: In recent years, uniportal video-assisted thoracic surgery (U-VATS) has been used for primary spontaneous pneumothorax (PSP). This study compared the perioperative outcomes of U-VATS and three-port VATS (3P-VATS) and sought to determine the risk factors for postoperative recurrence.METHODS: From October 2010 to February 2017, 232 patients with PSP undergoing surgical treatment were enrolled in this study. The patients were divided into two groups: U-VATS (n=161) and 3P-VATS (n=71) depending on the period of surgery. Retrospective analysis of the perioperative results and the risk factors for recurrence was performed.RESULTS: Both the operation time and duration of postoperative drainage were initially longer in the U-VATS group, but the difference gradually decreased such that ultimately there was no significant difference compared to the 3P-VATS group (P=0.10 and P=0.12, respectively). The duration of postoperative hospital stay and postoperative recurrence rate were not different between the two groups (P=0.084 and P=0.44, respectively). By multivariate analysis, the age (HR, 0.42, 95% CI: 0.24-0.72, P<0.01) and number of bullae (single vs. multiple: HR, 0.03, 95% CI: 0.002-0.54, P=0.02) were risk factors for recurrence.CONCLUSIONS: The perioperative results and recurrence rate did not differ between the U-VATS and 3P-VATS groups, thereby demonstrating the non-inferiority of U-VATS. Postoperative risk factors for PSP recurrence were patient age and the number of bullae. Additional treatment may be needed to reduce recurrence in young patients with multiple bullae. Abstract:Background: Spontaneous pneumothorax (SP) remains a significant health problem because of the high recurrence rate during thoracic drainage and conservative treatments. SP can appear under two forms: primary SP occurs in otherwise healthy patients, most commonly in tall, young, lean men, and particularly in those who smoke. Secondary SP occurs as a complication of an underlying lung disease, which most often is chronic obstructive pulmonary disease. Objective: The aim of the study was to compare outcome in patient who undergo thoracoscope and patients who undergo thoracotomy in primary spontaneous pneumothorax. Patients and Methods: This randomized clinical trial study was conducted at Cardiothoracic Surgery Department, Zagazig University Hospitals. We included 30 patients with primary spontaneous pneumothorax in a randomly selected (allocation sealed envelope). Patients were randomly divided into two groups; Group A: 15 cases were subjected to video-assisted thoracic surgery (VATS) Bullectomy in primary spontaneous pneumothorax. Group B: 15 cases were subjected to open thoracotomy in primary spontaneous pneumothorax. Results: This study showed that there was significant difference between both groups regarding to intraoperative blood loss, chest tube duration and hospital stay that was higher in thoracotomy group than VATS group. The complications were higher in open thoracotomy than VATS group but it did not reach significant difference. Also, there was no significant difference regarding to recurrence. Conclusion: VATS is associated with shorter length of hospital stay and less blood loss with no difference of recurrence, and postoperative outcome compared to open thoracotomy in primary spontaneous pneumothorax patients.
Secondary Video-assisted Thoracoscopic Surgery for Recurrent Pneumothorax after Thoracoscopic Surgery

Abstract:Objective To discuss the feasibility of secondary video-assisted thoracoscopic surgery(VATS) in the treatment of recurrent spontaneous pneumothorax after thoracoscopic surgery.Methods From January 2009 to November 2012,77 cases of postoperative recurrent spontaneous pneumothorax underwent secondary VATS.Adhesion lysis of pleura,bullectomy,pleurodesis and closed drainage were performed.Results The procedures were completed in all of the patients successfully.Severe adhesion could be observed during the surgery.No intraoperative hemorrhage or perioperative death occurred.The operation time was 35-87 min,with a mean of(59±12)min.Intraoperative blood loss ranged from 30 to 160 ml,with a mean of(92±33)ml.77 patients were followed up for 1-46 months,with a mean of(22±13) months,and no patients had recurrence.Conclusions Secondary video-assisted thoracoscopic surgery for postoperative recurrent spontaneous pneumothorax is safe and feasible.Pleura adhesion should be selectively released in operation.

Abstract:Video-assisted thoracoscopy has used for lots of thoracic disorders. Spontaneous pneumothorax may be ideally suited for thoracoscopic management. Stapling of apical blebs and pleurodesis can now be performed thoracoscopically. We compared our results with thoracoscopic management of spontaneous pneumothorax in 12 patients (group I) with a group of 8 patients (group II) previously subjected to lateral thoracotomy. Operative time and hospital stay were less in group I (P < 0.05), as the use of parenteral narcotics after 24 hours (P < 0.05). There have been no recurrences to date in group I. Video-assisted thoracoscopic management is safe and offers the benefits of shorter hospital stays and less pain.

Abstract:Objectives Primary spontaneous pneumothorax (PSP) is a common disease in young and thin male. Operation has been regarded as definitive treatment for it. However, the operative methods for those patients are under dispute. This study aims to directly compare the outcomes of uniportal VATS vs. needlescopic VATS in the treatment of PSP, focusing on postoperative pain and safety outcomes. Methods From July 2013 to December 2017, the patients who underwent video-assisted thoracic surgery for pneumothorax in National Taiwan University Hospital were retrospectively collected. The preoperative condition, surgical results, and postoperative outcomes was analyzed. Results There were 60 patients undergoing needlescopic VATS and 91 undergoing uniportal VATS during the study period. There was no significant difference between the patients who underwent needlescopic VATS and those who underwent uniportal VATS in their demographic and clinical characteristics. The post-operative pain score was significantly lower in the uniportal VATS group compared to the needlescopic VATS group at day 1 (2.65 ± 1.59 vs. 1.74 ± 1.35, p = 0.001). Conclusion Uniportal VATS offers an effective, safe alternative for PSP treatment, with benefits including reduced post-operative pain. Our findings support the use of uniportal VATS, supplemented by a wound protector, as a viable option for PSP patients.

Abstract:Objective To explore operative way and time as well as postoperative reccurence in the Video-assisted thoracoscopic surery (VATS) of primary spontaneous pneumothorax.Methods To analysize clinical materials of 18 cases in our hospital from October 2008 to April 2009 retropcctively.Results All of the patients made the chest CT examination before VATS and found the pulmonary bullae in the the pneumothoracic lung without emphysema signs.operative time among 18 cases is from 45 minutes to 60 minutes,the average time is 50 minutes.The hospitalization time is from 4 days to 10 days,the average day is 6 days.Thoracic drainage tube time is from 3 dys to 9 days,the average is 5 days.One cases is recurrent in the six months follow-up period,the recurrence rate is 5.6%.Conclusion primary spontaneous pneumothorax procedure by VATS is marked safe,less postoperative pain and minimally invisive technique.the patients recover after operation quickly in the clinical practice.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.

Abstract:Video-assisted thoracic surgery (VATS) is the treatment of choice for recurrence prevention in patients with spontaneous pneumothorax (SP). Although the optimal surgical technique is uncertain, bullous resection using staplers in combination with mechanical pleurodesis, chemical pleurodesis and/or staple line coverage is usually undertaken. Currently, patient satisfaction, postoperative pain and other perioperative parameters have significantly improved with advancements in thoracoscopic technology, which include uniportal, needlescopic and nonintubated VATS variants. Ipsilateral recurrences after VATS occur in less than 5% of patients, in which case a redo-VATS is a feasible therapeutical option. Randomized controlled trials are urgently needed to shed light on the best definitive management of SP.

Abstract:【Objective】 To investigate the feasibility and efficiency of video-assisted thoracoscopic surgery(VATS) for bilateral primary spontaneous pneumothorax(BPSP).【Methods】 Eleven patients with BPSP diagnosed and underwent one-stage VATS were reviewed retrospectively.【Results】 All patients were successfully treated by VATS.The mean operation time was 108.7 ± 20.3 min.The mean hospital stay after operation was 7.4 ± 1.7 days.There was no intraoperative or postoperative deaths.One patient had reexpansion pulmonary edema.The follow up period ranged from 6 months to 36 months and one patient experienced left recurrence a month after the operation.【Conclusions】 One-stage VATS is a safe and effective procedure for BPSP.It avoids subsequent operative procedures and is recommended.

Video-assisted Thoracic Surgery for Pulmonary Sequestration: a Safe Alternative Procedure.

Abstract:BACKGROUND:Pulmonary sequestration (PS), a rare congenital anatomic anomaly of the lung, is usually treated through resection by a conventional thoracotomy procedure. The efficacy and safety of video-assisted thoracic surgery (VATS) in PS treatment has seldom been evaluated. To address this research gap, we assessed the efficacy and safety of VATS in the treatment of PS in a large Chinese cohort.METHODS:We retrospectively reviewed 58 patients with PS who had undergone surgical resection in our department between January 2003 and April 2014. Of these patients, 42 (72.4%) underwent thoracotomy, and 16 (27.6%) underwent attempted VATS resection. Clinical and demographic data, including patients' age, sex, complaints, sequestration characteristics, approach and procedures, operative time, resection range, blood loss, drainage volume, chest tube duration, hospital stay, and complications were collected, in addition to short-term follow-up data.RESULTS:Of the 58 participating patients, 55 accepted anatomic lobectomy, 2 accepted wedge resection, and 1 accepted left lower lobectomy combined with lingular segmentectomy. All lesions were located in the lower lobe, with 1-4 aberrant arteries, except one right upper lobe sequestration. Three cases (18.8%) in the VATS group were converted to thoracotomy because of dense adhesion (n=1), hilar fusion (n=1), or bleeding (n=1). No significant differences in operative time, postoperative hospital stay, or perioperative complications were observed between the VATS and thoracotomy groups, although the VATS patients had less blood loss (P=0.032), a greater drainage volume (P=0.001), and a longer chest tube duration (P=0.001) than their thoracotomy counterparts.CONCLUSIONS:VATS is a viable alternative procedure for PS in some patients. Simple sequestration without a thoracic cavity or hilum adhesion is a good indication for VATS resection, particularly for VATS anatomic lobectomy. Thoracic cavity and hilum adhesion remain a challenge for VATS.

Abstract:Objective:To explore the cause and operative results of surgical treatment for spontaneous pneumothorax. Method: The clinical data of 318 cases of spontaneous pneumothorax that underwent surgical treatment were analyzed. The results of different surgical methods were compared. Result: All patients recovered and the rate of complication and recurrence was low. The cases underwent video-assisted thoracoscopic surgery (VATS) had the shortest post-operative drainage time and hospital stay compared with those underwent classic thoracotomy and subaxillary mini thoracotomy. Bullae were the principal cause of spontaneous pneumothorax in the group. Conclusion: Operative method is safe and effective in the treatment of spontaneous pneumothorax. VATS is minimally invasive and is the first preference upon other methods.
Incidence and risk factors for recurrent primary spontaneous pneumothorax after video-assisted thoracoscopic surgery: a systematic review and meta-analysis

Abstract:Background: The incidence and risk factors for recurrent primary spontaneous pneumothorax (PSP) after video-assisted thoracoscopic surgery (VATS) remain controversial. A systematic review and meta-analysis were conducted to determine the incidence and risk factors for recurrence of PSP after VATS. Methods: A systematic search of PubMed, Web of Science, Embase, and Cochrane Library databases was conducted to identify studies that reported the rate and risk factors for recurrence of PSP after VATS published up to December 2023. The pooled recurrence rate and odds ratio (OR) with 95% confidence interval (CI) were calculated using a random-effects model. In addition, risk factors were similarly included in the meta-analysis, and sources of heterogeneity were explored using meta-regression analysis. Results: A total of 72 studies involving 23,531 patients were included in the meta-analysis of recurrence. The pooled recurrence rate of PSP after VATS was 10% (95% CI: 8-12%). Male sex (OR: 0.61; 95% CI: 0.41-0.92; P=0.02), younger age [mean difference (MD): -2.01; 95% CI: -2.57 to -1.45; P<0.001), lower weight (MD: -1.57; 95% CI: -3.03 to -0.11; P=0.04), lower body mass index (BMI) (MD: -0.73; 95% CI: -1.08 to 0.37; P<0.001), and history of contralateral pneumothorax (OR: 2.46; 95% CI: 1.56-3.87; P<0.001) were associated with recurrent PSP, whereas height, smoking history, affected side, stapling line reinforcement, and pleurodesis were not associated with recurrent PSP after VATS. Conclusions: The recurrence rate of PSP after VATS remains high. Healthcare professionals should focus on factors, including sex, age, weight, BMI, and history of contralateral pneumothorax, that may influence recurrence.
