Abstract:OBJECTIVE: The objective of this systematic review was to comprehensively assess the literature regarding the applications, accuracy, advantages and limitations of dynamic navigation in endodontics.METHODS: Case reports and laboratory studies in the English language, which used the Dynamic Navigation System (DNS) for endodontic application and assessed the accuracy of treatment, the time required for treatment and iatrogenic errors were included. PubMed, Scopus, Embase and Web of Science were searched for eligible articles (up to July 2021). Additional hand searching of four peer-reviewed endodontic journals and a grey literature search were also carried out. A risk of bias assessment was done using the Joanna Briggs Institute (JBI) critical appraisal checklists. Data were extracted based on endodontic application of DNS, tooth type, DNS brand, accuracy, iatrogenic errors, and time taken, followed by qualitative analysis.RESULTS: Fourteen articles (three case reports and eleven in-vitro studies) met the eligibility criteria and were included. The quality assessment revealed a low risk of bias, with mean scores of 83.34% for case reports and 84.09% for in-vitro studies. DNS was used for various clinical applications such as access cavity preparation, pulp canal obliteration, endodontic retreatment and microsurgery. The DNS brands used were Navident, X-guide, ImplaNav, and DENACAM. Due to the nature of the component studies, meta-analysis was not possible.CONCLUSION: Challenging clinical situations like pulp canal obliteration, conservative access preparation, endodontic retreatment and microsurgery can be managed efficiently with fewer iatrogenic errors in a shorter time using DNS. However, this systematic review's evidence is low since the included articles are either case reports or in-vitro studies. Clinical studies are needed to test DNS efficacy among operators, including those who are less proficient and compare the accuracy of currently available systems.
Current Applications of Dynamic Navigation System in Endodontics: A Scoping Review

Abstract:This scoping review (SCR) was conducted to map the existing literature on dynamic navigation system (DNS), to examine the extent, range, and nature of research activity. Additionally, this SCR disseminates research findings, determines the value of conducting a full systematic review with meta-analysis, and identifies gaps in the existing literature and future directions. This SCR followed Arksey and O'Malley's five stages framework. The electronic search was performed in PubMed (Medline), Scopus (Elsevier), and Web of Science (Clarivate Analytics) databases using a search strategy. Five themes emerged during the descriptive analysis that captured the DNS application in endodontics. The DNS has been explored for creating access cavities (8/18, 44.44%), locating calcified canals (4/18, 22.2%), microsurgery (3/18, 16.6%), post removal (2/18, 11.1%), and intraosseous anesthesia (1/18, 5.5%). Out of the 18 studies included, 12 are in vitro (66.6%), five are in vivo (case report) (27.7%), and one is ex vivo (5.5%). The DNS demonstrated accuracy and efficiency in performing minimally invasive access cavities, locating calcified canals, and performing endodontic microsurgery, and it helped target the site for intraosseous anesthesia. Dynamic navigation: how far it would go in endodontics?

Abstract:Background No doubt that the great revolution of digital dentistry has opened the door wide for treatment options that would not otherwise exist and the field of endodontics sets no exception for that. The concept of dynamic navigation (DN) has been recently introduced to be applied not only in difficult cases that otherwise lead to catastrophic errors, but would go far till everyday practice. Aim The present study is designed to systematically review the application of DN in endodontics. Furthermore, meta-analysis was conducted to assess the accuracy and feasibility of the use of DN as compared to conventional endodontics. Methods A comprehensive review was performed by two independent reviewers who conducted a literature search using a combination of keywords in four different electronic databases. Results Fifteen eligible articles were included and reviewed in the systematic review; of which 10 were included in the meta-analysis to assess different aspects such as accuracy, efficiency as reflected by time consumed, dentine loss, and effect of operator experience. DN showed statistically significant lower linear and angular deviation as well as dentine loss. Nevertheless, studies that included more than one operator of different levels of experience showed a statistically non-significant difference. Conclusion Dynamic navigation is a valuable, accurate, and effective modality in endodontics that would ensure accuracy even for the less experienced operator and allow for a new era of digitalized endodontic practice.

A Comparative Evaluation of Real-Time Guided Dynamic Navigation and Conventional Techniques for Post Space Preparation During Post Endodontic Management: An In Vitro Study

Abstract:Introduction: The three-dimensional (3D) dynamic navigation system (DNS; Navident, ClaroNav Technology, Toronto, ON) is a revolutionary technique in endodontics that offers superior precision and efficiency compared to existing techniques for post space preparation. Aim: The aim is to evaluate and contrast the efficacy and efficiency of the DNS with conventional post space preparation techniques. This assessment considers several parameters, notably canal deviation (global coronal and apical deviation and angular deflection), duration of the procedure, and total volumetric loss of instrumented root canal and volumetric loss of instrumented root canal above 4 mm from the apex. Materials and methods: Freshly extracted maxillary central incisors were chosen for this study. A total sample size of 60 (n) was included. The extracted teeth samples were divided into two groups: 3D DNS (group I; n = 30) and conventional techniques (group II; n = 30). The samples were taken, and 50% of the crown structure was reduced for post space preparation to ensure standardization between the two groups. The samples were root canal-treated and mounted in a 3D-printed maxillary cast. Preoperative micro-computed tomography (micro-CT) and cone-beam computed tomography (CBCT) were taken for both groups. For group I, post space preparation was conducted with the DNS, which provided comprehensive guidance. The procedure was stopped when post space preparation was 4 mm short of the apex, as indicated in the system display. For group II, post space preparation was done without the guidance of DNS. Time taken for the procedure was assessed using a timer; canal deviation was evaluated using CBCT analysis, and volumetric loss was estimated using micro-CT analysis. Results: The dynamic navigation group achieves significantly more precise outcomes in post space preparation than the conventional technique. The DNS group has significantly lesser global coronal and apical deviation and angular deflection compared to the conventional group (p < 0.05). The DNS group has reduced the volumetric loss of instrumented root canals compared to the conventional group (p < 0.05). Furthermore, the DNS group requires significantly less time than the conventional method, with a mean difference of about 10.567 minutes (p < 0.05). Conclusion: Implementing dynamic navigation improves precision in post space preparation, with a notable reduction in canal deviation and volumetric loss and a decrease in procedure time compared to the conventional method.
Clinical and radiological outcomes of dynamic navigation in endodontic microsurgery: a prospective study

Abstract:Objectives: This study was aimed at evaluating the clinical and radiological outcomes of novel dynamic navigation (DN)-aided endodontic microsurgery (EMS), with an analysis of potential prognostic factors. Materials and methods: Forty-six teeth from 32 patients who received DN-aided EMS were included. Clinical and radiographic assessments were performed at least 1 year postoperatively. Two calibrated endodontists assessed radiological outcomes according to two-dimensional (2D) periapical radiography (PA) and three-dimensional (3D) cone-beam computed tomography (CBCT) imaging using Rud's and Molven's criteria and modified PENN 3D criteria, respectively. Fisher's exact test was used for statistical analysis of the predisposing factors. Results: Of the 32 patients with 46 treated teeth, 28 with 40 teeth were available for follow-up. Of the 28 patients, four (five teeth) refused to undergo CBCT and only underwent clinical and PA examinations, and the remaining 24 (35 teeth) underwent clinical, PA, and CBCT examinations. Combined clinical and radiographic data revealed a 95% (38/40) success rate in 2D healing evaluations and a 94.3% (33/35) success rate in 3D healing evaluations. No significant effect was found in sex, age, tooth type, arch type, preoperative lesion volume, preoperative maximum lesion size, presence/absence of crown and post, and the root canal filling state on the outcome of DN-aided EMS. Conclusions: DN-aided EMS has a favorable prognosis and could be considered an effective and reliable treatment strategy. Further investigations with larger sample sizes are required to confirm these results. Clinical relevance: DN-aided EMS could be considered an effective and reliable treatment strategy.

Comparative Evaluation of a Dynamic Navigation System versus a Three-dimensional Microscope in Retrieving Separated Endodontic Files: An In Vitro Study

Abstract:Introduction: This study aimed to compare the effectiveness of a dynamic navigation system and a three-dimensional microscope in retrieving broken rotary Nickel-Titanium files when using trepan burs and the extractor system. Materials and methods: Thirty maxillary first bicuspids with 60 separate roots were split into 2 comparable groups based on a comprehensive cone beam computed tomography analysis of the root length and curvature. After standardized access opening, glide paths, and patency attainment with the K file (sizes 10 and 15), the teeth were arranged on 3D models (three per quadrant, six per model). Subsequently, controlled-memory heat-treated Nickel-Titanium rotary files (#25/0.04) were notched 4 mm from the tips and fractured at the apical third of the roots. The C-FR1 Endo file removal system was employed under both guidance to retrieve the fragments, and the success rate, canal aberration, treatment time, and volumetric changes were measured. The statistical analysis was performed using IBM SPSS software at a significance level of 0.05. Results: The microscope-guided group had a higher success rate than the dynamic navigation system guidance, but the difference was insignificant (P > .05). In addition, the microscope-guided drills resulted in a substantially lower proportion of canal aberration, shorter time to retrieve the fragments and less change in the root canal volume (P < .05). Conclusion: Although dynamically guided trephining with the extractor can retrieve separated instruments, it is inferior to three-dimensional microscope guidance regarding treatment time, procedural errors, and volume change.

Abstract:OBJECTIVES: To evaluate the accuracy of dynamic computer-aided implant surgery (dCAIS) and compare it with static computer-aided implant surgery (sCAIS) and freehand implant placement (FH) in partially or fully edentulous patients.DATA: Studies that analyzed the accuracy of dynamic computer-assisted implant surgery in partially or fully edentulous patients.SOURCES: This meta-analysis included studies published in English and Mandarin Chinese from January 2013 to February 2023 from MEDLINE/PubMed, Embase, CENTRAL (Cochrane Central Register of Controlled Trials), and CNKI (China National Knowledge Infrastructure).STUDY SELECTION: Only clinical studies were included. Accuracy was the primary outcome. Seventeen studies met the inclusion criteria. A total of 2,025 implants were analyzed. Meta-regression was conducted to compare the six different navigation systems. GRADE (Grading of Recommendations Assessment, Development, and Evaluation) assessment was adopted as a collective grading of the evidence.CONCLUSIONS: Dynamic navigation is a clinically reliable method for implant placement. Significantly lower angular deviation was observed for dCAIS compared to both sCAIS and FH, while significantly lower global platform and apex deviations were displayed between dCAIS and FH. Overall, dynamic navigation allowed for higher accuracy compared to both sCAIS and FH in a clinical setting; however, additional large sample RCT studies should be conducted, and patient-reported outcome measures (PROMs) reported.CLINICAL SIGNIFICANCE: This systematic review analyzed the accuracy of dynamic computer-assisted implant surgery in partially or fully edentulous patients compared with static navigation. The results demonstrated that dynamic navigation could decrease implant placement angular deviations.

Comparison of Static and Dynamic Navigation in Root End Resection Performed by Experienced and Inexperienced Operators: An In Vitro Study

Abstract:Introduction: This study aimed to compare the effects of static navigation (SN), a dynamic navigation system (DNS), and the freehand (FH) technique in root end resection and the differences between these effects according to the level of experience of the operator. Methods: Maxillary models reconstructed with Mimics software (Materialise, Leuven, Belgium) were 3-dimensionally printed and divided according to the experimental technique (FH, SN, or DNS) and the operator (experienced or inexperienced). SN was designed using 3-matic Medical software (Materialise) and printed, and a surgical approach plan for DNS was established and performed using DCARER (Suzhou, China) software. The accuracy, efficiency, and safety of the resections were assayed. Results: The length, angle, volume, and depth deviations of the root end resections were significantly lower in the SN and DNS group compared with the FH group. SN significantly improved the efficiency of both operators, whereas DNS only improved the efficiency of the inexperienced operator. No difference between the SN and DNS groups was found, except for the time required for the surgery. No mishaps occurred during surgery in the SN or DNS group. The number of mishaps with the FH technique when used by the inexperienced operator was significantly higher than that registered for the rest of the groups. No interaction effect between technique and operator experience level was detected. Conclusions: Regardless of operator experience, both SN and DNS could improve the accuracy and safety of root end resection. SN significantly improved the chairside efficiency of both operators, whereas DNS was more helpful for the inexperienced operator.
Evaluation of a dynamic navigation system for endodontic microsurgery: study protocol for a randomised controlled trial

Abstract:Introduction Endodontic microsurgery is a very important technique for preserving the natural teeth. The outcomes of endodontic microsurgery largely depend on the skill and experience of the operators, especially for cases in which the apices are located far away from the labial/buccal cortical bone. A dynamic navigation system (DNS) could provide a more accurate and efficient way to carry out endodontic microsurgery. This study is devoted to comparing the clinical outcomes of the DNS technique with those of the freehand technique. Methods and analysis Sixteen patients will be randomly assigned to one of two groups. For the experimental group, the osteotomy and root-end resection will be performed under the guidance of dynamic navigation. For the control group, these procedures will be performed freehand by an endodontist. The required time to perform these procedures will be used to evaluate the efficiency of the DNS technique. A Visual Analogue Scale will be used to evaluate pain at 1, 3 and 7 days after endodontic microsurgery. Preoperative and postoperative cone beam CT scans will be obtained to evaluate the accuracy of the DNS technique. The global coronal deviations, the apical deviations and the angular deflection will be measured. The root-end resection length deviation, the root-end resection angle deviations, the extent of the osteotomy and the volume change of the buccal cortical bone will also be measured. Periapical radiographs will be obtained to evaluate the outcome at 1 year after microsurgery. The time to execute the study, including follow-ups, will last from 1 June 2022 to 31 December 2025. Ethics and dissemination The present study has received approval from the Ethics Committee of Peking University School and Hospital of Stomatology. The results will be disseminated through scientific journals. Trial registration number ChiCTR2200059389.

Abstract:Dynamic navigation (DN) is a promising technology designed to guide the placement of drills/implants in real time, based on information generated from the patient’s cone-beam computed tomography (CBCT). In endodontics, dynamic navigation is used for localization of calcified canals as well root-end resection surgeries. Treatment planning and surgery can be performed in the same appointment, based on data from patient’s cone-beam computed tomography. This technology allows safe, minimal invasive and predictable access opening, osteotomy as well as root-end resection.
A laboratory study comparing the static navigation technique using a bur with a conventional freehand technique using ultrasonic tips for the removal of fibre posts

Abstract:Aim There are currently no high‐quality studies comparing the static navigation technique with conventional methods of fibre post removal. The aim of this ex vivo study was to compare the effectiveness of fibre post removal between a static navigation technique and a conventional freehand technique using ultrasonics by experienced and inexperienced operators. Methodology Forty‐eight extracted single‐rooted human premolars were root‐filled. A fibre post was cemented in all 48 teeth, which were then divided randomly into the following groups: static navigation group using burs; static navigation‐ultrasonic group; and non‐guided group using ultrasonic tips. The following parameters were evaluated for both experienced operators and inexperienced operators: reaching the gutta‐percha root filling successfully, the time required to remove the entire post, the occurrence of lateral root perforations, and the amount of root dentine removed. The Kolmogorov–Smirnov test was used to examine the normality of the data; the anova test was used to compare the significant differences among groups; and Tukey tests were used for all two‐by‐two comparisons. The significance level was set at 0.05. Results In the static navigation group, the gutta‐percha was reached significantly more frequently than in the non‐guided group (p .05). More perforations were associated with the non‐guided group than with the other two groups. The total mean loss of dentine in the non‐guided group in all directions was 0.39 (±0.17) mm, with 0.25 (±0.09) mm for experienced, and 0.42 (±0.16) mm for inexperienced operators. Conclusion When compared to a conventional ultrasonic technique for the removal of fibre posts, the static navigation method using burs resulted in less dentine removal, more rapid access to the gutta‐percha root filling, less overall time to remove the posts, and fewer complications. When using static navigation, there was no difference in performance between experienced and inexperienced operators.

Abstract:The development of dynamic navigation system (DNS) has facilitated the development of modern digital medicine. In the field of dentistry, the cutting-edge technology is garnering widespread recognition. Based on the principles of 3-dimensional visualization, virtual design, and precise motion tracking, DNS is mainly composed of a computer, a tracking system, specialized tracer instruments, and navigation software. DNS employs a workflow that begins with preoperative data acquisition and imaging data reconstruction, followed by surgical instrument calibration and spatial registration, culminating in real-time guided operations. Currently, the system has been applied in a broad spectrum of dental procedures, encompassing dental implants, oral and maxillofacial surgery (such as tooth extraction, the treatment of maxillofacial fractures, tumors, and foreign bodies, orthognathic surgery, and temporomandibular joint ankylosis surgery), intraosseous anesthesia, and endodontic treatment (including root canal therapy and endodontic surgery). These applications benefit from its enhancements in direct visualization, treatment precision, efficiency, safety, and procedural adaptability. However, the adoption of DNS is not without substantial upfront costs, required comprehensive training, additional preparatory time, and increased radiation exposure. Despite challenges, the ongoing advancements in DNS are poised to broaden its utility and substantially strengthen digital dentistry.
Real-Time Three-dimensional Dynamic Navigation for Post Space Preparation in Root Canal-Treated Teeth: An In vitro Study

Abstract:Introduction The aim of this study was to investigate the feasibility of a real-time three-dimensional dynamic navigation system (3D-DNS) for post space preparation (PSP) in root canal-treated teeth and to compare the accuracy and efficiency of 3D-DNS to freehand (FH) for PSP. Methods Fifty-four maxillary molars were divided into two groups: 3D-DNS ( n = 27) and FH group ( n = 27). Cone beam computed tomography (CBCT) scans were taken preoperatively and postoperatively. The drilling path for the PSP was virtually planned in the preoperative CBCT scan in the X-guide software (X-Nav Technologies, Lansdale, PA). For the 3D-DNS group, the PSP drilling was conducted under dynamic navigation. The 3D deviations and angular deflections were calculated. The residual dentin thickness (RDT) was determined after PSP. The operation time and the total number of mishaps were recorded. Shapiro-Wilk, t -test or Mann-Whitney rank sum, weighted Cohen's kappa, and Fisher exact tests were used ( P < .05). Results The PSP was completed in all samples (54/54). The 3D-DNS was more accurate than the FH, with significantly fewer 3D deviations and angular deflections (all, P < .05). The 3D-DNS required less operating time than the FH ( P < .05). For the 3D-DNS, no teeth had RDT < 1 mm, whereas 6/27 in the FH showed RDT < 1 mm after the PSP. There was no difference in the total number of mishaps ( P > .05). Conclusion Within the limitations of this in vitro study, the 3D-DNS is feasible for PSP. The 3D-DNS improved the accuracy and efficiency of PSP. The dynamic navigation system can potentially become a safe and reliable technology for PSP.

Abstract:Over the last few decades technological advancements have led to a significant impact over dentistry and the field of endodontics is no exception to it. It has covered a long way from apex locators and microscopes to digital radiography and intraoral scanners and finally navigation. “Guided Dentistry” term is used explicitly for navigation in dentistry. In the 19th century, navigation in the medical sciences first appeared and it was used to perform stereotactic surgery on the human brain. It was first used in dentistry in the year 2000 to facilitate the placement of dental implants. Navigation in endodontics is based on the concept of minimally invasive endodontics. Guided endodontics can also be called as “Targeted endodontic treatment”. Compared to standard treatment methods, guided endodontics can produce more predictable treatment results. Guided navigation in endodontics is of two types-Static and Dynamic. In Static Guided Endodontics (SGE) the CBCT scan is merged with optical scan to design a virtual drill path using a planning software following which a template is fabricated using 3-D printing. In Dynamic Navigation System (DNS), the virtual drill path is converted into a real time drill path using navigation software. Abstract:Background/purpose: Quantitative in vitro research was conducted on the learning process of a dynamic navigation system. This study provides guidance for the promotion and application of dynamic navigation technology in the endodontic apical surgery field. Materials and methods: Standardized models were designed and 3D printed to form the approach operation of endodontic apical surgery. 6 clinicians with no experience in dynamic navigation performed the operation. The distance deviation tolerance was set as 0.6 mm, and the angle deviation tolerance was set as 5°. Fifteen mm deep approach operation was completed using dynamic navigation. Each operator performed 10 consecutive exercises on the models. The positioning deviation and operation time of each operator for each practice were recorded. Based on this, the learning curve of the dynamic navigation of every operator was mapped. The learning difficulty of dynamic navigation was evaluated. Results: The learning curves of all operators reached a stable level after the 7th practice, which can ensure that the distance and angle deviations are maintained within the deviation tolerances (0.6 mm, 5°). Conclusion: Operators with no experience in dynamic navigation technology need practice to master dynamic navigation operations. For this navigation system, operators with no operational experience can master dynamic navigation operations after 7 exercises.
Comparison of the Accuracy and Efficiency of Two Dynamic Navigation System Workflow for Fiber-post Removal: Small versus Large Field-of-view Registration Workflows

Abstract:Introduction: This study investigates the feasibility of a dynamic navigation system (DNS) small field of view workflow (DNS-SFOVw) for fiber-post removal and compares its accuracy and efficiency to the conventional large field of view workflow (DNS-LFOVw). Methods: Fifty-six extracted human maxillary molars were divided into DNS-SFOVw (n = 28) and DNS-LFOVw (n = 28). The palatal canal was restored with an intraradicular RelyX fiber post and luted with RelyX Unicem; a core buildup was used. Teeth were mounted in a 3D-printed surgical jaw. A preoperative cone-beam computed tomography (CBCT) scan was taken with a 40 × 40 mm FOV for the DNS-SFOVw and a single arch CBCT scan for the DNS-LFOVw. The drilling entry point, trajectory, angle, and depth were planned in the X-guide software. The DNS registration method for the DNS-SFOVw was virtual-based registration on teeth, and the marker point-based method was used for the DNS-LFOVw. The fiber posts were drilled out under DNS guidance. A postoperative CBCT scan was taken. Three-dimensional deviations, angular deflection, number of mishaps, registration, and total operation time were calculated. Results: The DNS-SFOVw was as accurate as DNS-LFOVw (P > .05). The DNS-LFOVw registration time was less than DNS-SFOVw (P < .05). There was no difference in the number of mishaps (P > .05). Both DNS-SFOVw and DNS-LFOVw were time-efficient, with DNS-LFOVw taking less total operational time (P < .05). Conclusion: Within the limitations of this in-vitro study, the DNS-SFOVw was as accurate as the DNS-LFOVw for fiberpost removal. Both DNS-LFOVw and DNS-SFOVw were time-efficient in removing fiber-posts.
Accuracy of dynamic navigation compared to static surgical guide for dental implant placement

Abstract:Abstract Objectives To compare the accuracy of dynamic navigation (DN) with a static surgical guide (SSG) for dental implant placement and the influence factors such as the experience of the surgeon and the implant sites. Methods and materials A total of 38 implants, which underwent the dynamic navigation , and 57 implants which underwent a static surgical guide were enrolled in the retrospective study. Coronal deviation, apical deviation, and angular deviation were compared between the DN and SSG groups, along with the different experience level of surgeons and implant sites in the DN group. Results There were no statistically significant differences between the DN and SSG groups, and the experience level of the surgeons and implant sites in the DN group. However, the apical deviation of the DN was slightly higher than the SSG group in the anterior teeth ( P = 0.028), and the angular deviation of DN was smaller than the SSG group in the molar. Conclusion Dynamic navigation can achieve accurate implant placement as well as the static surgical guide. Additionally, the experience level of the surgeon and implant site do not influence the accuracy of dynamic navigation, while the accuracy of DN seems higher than the SSG in molar.
