Abstract:Background: The aim of this study was to characterize the contrast-enhanced ultrasound imaging features of focal splenic tuberculosis. Material/Methods: We retrospectively analyzed the conventional ultrasound (US) and contrast-enhanced ultrasound (CEUS) imaging features of 22 patients with splenic TB confirmed by surgical histopathology or biopsy. Results: Conventional US demonstrated that 15 of the 22 patients had a single lesion, while 7 had multiple lesions. The maximum diameter of the lesions ranged from 1.0 to 3.7 cm. Of the 22, 17 were detected with hypoecho and 5 were detected with complex echo by conventional US. Seven (7/22) were detected with blood flow signals by color Doppler flow image (CDFI). CUES demonstrated that 18 cases (81.8%, 18/22) began to enhance in the arterial phase, mostly followed by slow wash-out in the intermediate or late parenchymal phase, and 4 (18.2%, 4/22) presented with non-enhancement during all phases. The enhancement patterns were categorized into 4 types: Type I, homogeneous enhancement (2/22); Type II, rim-like enhancement (12/22); Type III, septation-like enhancement (4/22); and Type IV, non-enhancement (4/22). Conclusions: CEUS showed that splenic TB lesions were enhanced in the arterial phase, followed by slow washed out or persistent enhancement in the intermediate and late parenchymal phases. The rim- or septation-like enhancement may be helpful for diagnosing splenic TB. The splenic lesions presenting round hypoecho by conventional US and complete non-enhancement by CEUS are highly suspicious of splenic TB.
Splenic cysts: clues in sonographic differential diagnosis and a new role for twinkling artifact

Abstract:Aim: Our aim was to investigate the frequency of various splenic cysts, to define the sonographic differential diagnostic clues and to introduce the value of twinkling artefact in the diagnosis of epidermoid splenic cysts.Material and methods: All the splenic cysts imaged by ultrasound in 3 university hospitals during the period of 2005 to 2022 were recorded, followed-up and analyzed.Results: One hundred seventy-one patients with splenic cysts were detected and these were classified and 73% of the cysts were simple. Ten cysts were epidermoid cysts as proven by post-operative final histology.Conclusion: Cystic splenic lesions are rare. Most of them are small simple cysts. Epidermoid cysts are larger in volume, constitute 6% of the total and can be differentiated by the help of twinkling artefact by Doppler ultrasound.

Abstract:Objective To explore the CT features of splenic cystic lesions(SCL)in order to improve the diagnostic ability of these diseases.Methods The CT findings of 32 patients with SCL proved by surgical pathology were reviewed retrospectively.Of the patients,10 underwent plain CT scan,8 underwent enhanced CT scan,and 14 underwent both plain and enhanced CT scan.Results Non-tumor splenic cystic lesions were 19,including 7 congenital cysts,1 pseudocyst,1 hydatid cyst,2 abscesses and 8 hematomas.Spleen tumor lesions were 13,including 5 angiomas,2 splenic cystic lymphangiomas and 6 cystic metastases.The CT characteristics in size,shape, cyst wall,density and enhancement feature can help to discriminate non-neoplasm and neoplasm cyst.Conclusion With the plain and enhanced CT manifestations of some SCL,we could make correct preoperative diagnosis,and provide theoretical basis for selection of clinic treatment and prognostic judgment.

Abstract:Objective To investigate the value of contrast-enhanced ultrasonography(CEUS) for differential diagnosis of focal splenic solid lesions.Methods Thirty-two patients with 32 focal splenic lesions were examined with CEUS.The enhancement phases and perfusion were observed.Each group has 16 patients.Results The contrast media extincted more quickly in malignant lesions than in benign ones.The peak time and initial decreasing time of the malignant lesions were (19.7±4.5)s and (29.7±10.9)s.The opposite time of the benign lesions were (24.2±4.4)s and (50.9±22.8)s.The initial decreasing time of the malignant lesions was earlier than that of the benign ones.The enhancement pattern was similar between the two groups.More lesions can be detected after CEUS.The number of the lesions was 111 before CEUS and 142 after CEUS.Conclusions CEUS is useful in the differential diagnosis of splenic lesions.

Abstract:Ultrasound (US) evaluation of the spleen is mandatory in the assessment of patients with chronic liver disease, and splenomegaly can be a sign of systemic diseases. However, due to the lack of distinctive ultrasound findings in specific splenic pathologies, clinical diagnosis can be very challenging. Splenomegaly, defined by increased splenic dimensions, can indicate underlying systemic conditions and is a common manifestation of portal hypertension (PH). Ultrasound and Doppler techniques help assessing splenic involvement in PH. Splenic stiffness measurement, using elastography, offers additional diagnostic accuracy, especially when liver stiffness measurements are inconclusive. CEUS enhances the diagnostic capability for focal splenic lesions, differentiating between benign and malignant lesions by their distinct enhancement patterns, and plays also a critical role in the context of splenic traumatic pathology. Overall, CEUS significantly improves the characterization of splenic pathology, reducing the need for invasive procedures and ensuring appropriate patient management. This review article describes the normal US findings of the spleen and examines the role of multiparametric US in the evaluation of the most common splenic pathologies encountered in the daily clinical practice.

Abstract:Splenic cysts are rare benign lesions of the spleen, often asymptomatic and incidentally discovered during imaging studies. While many splenic cysts remain asymptomatic and do not require intervention, surgical management becomes essential in cases of symptomatic cysts, large cysts, or when malignancy cannot be ruled out. Laparoscopic surgery has emerged as a minimally invasive and effective approach for treating splenic cysts, offering advantages such as shorter hospital stays, reduced postoperative pain, and faster recovery. In this case report, we describe our experience with laparoscopic surgery for a symptomatic splenic cyst in a young patient.

Comparison of enhancement features of splenic solid tumors on contrast-enhanced ultrasonography and ;contrast-enhanced CT

Abstract:Objective: To compare the enhancement features of splenic solid tumors on contrast-enhanced ultrasonography (CEUS) and contrast-enhanced CT (CECT). Methods:Twenty-one patients with focal splenic lesions were enrolled, including 5 cases of hemangioma, 5 cases of lympho-hemangioma, 4 cases of lymphoma and 7 cases of metastases. Their enhancement appearances were compared. Results:The vascular displaying rates of CEUS and CECT were both 100%. The two contrast methods were more sensitive than color Doppler ultrasound in displaying the perfusion of the splenic lesions (71.4%) (P<0.05). Hemodynamic characteristics of the splenic lesions were accordant. The detection rate of special performance of CEUS was 57.1% (12/21) in the enhancement process. Compared to that of CECT, the enhancement on CEUS was more diversiifed. The difference was statistically signiifcant between CEUS and CECT images (P<0.05). Conclusion:Both CEUS and CECT can demonstrate hemodynamic features of splenic tumors sensitively, which is helpful in differential diagnosis of splenic tumors.

Abstract:Abstract Background Focal splenic lesions are usually incidentally discovered on radiological assessments. Although percutaneous tissue acquisition (TA) under trans-abdominal ultrasound guidance is a well-established technique for obtaining cyto-histological diagnosis of focal splenic lesions, endoscopic ultrasound (EUS)-guided TA has been described in several studies, reporting different safety and outcomes. The aim was to assess the pooled safety, adequacy, and accuracy of EUS-TA of splenic lesions. Methods A comprehensive review of available evidence was conducted at the end of November 2021. All studies including more than five patients and reporting about the safety, adequacy, and accuracy of EUS-TA of the spleen were included. Results Six studies (62 patients) were identified; all studies have been conducted using fine-needle aspiration (FNA) needles. Pooled specimen adequacy and accuracy of EUS-TA for spleen characterization were 92.8% [95% confidence interval (CI), 86.3%–99.3%] and 88.2% (95% CI, 79.3%–97.1%), respectively. The pooled incidence of adverse events (six studies, 62 patients) was 4.7% (95% CI, 0.4%–9.7%). Conclusion EUS-FNA of the spleen is a safe technique with high diagnostic adequacy and accuracy. The EUS-guided approach could be considered a valid alternative to the percutaneous approach for spleen TA.

Abstract:To evaluate the value of contrast-enhanced ultrasonography (CEUS) in the diagnosis of benign cystic renal masses. Twenty-four patients were included in this study. Sonographic examinations were performed with Philips IU22 ultrasound instruments and ultrasound contrast agent SonoVue. The images of conventional ultrasound (US) and CEUS were analyzed retrospectively. Size, inner echoes, boundary and color flow signals of the masses were observed by conventional US. The enhanced features of the internal structure of lesions including the wall, septa and solid component, were also observed with CEUS. All cases were confirmed to be benign cystic renal masses by pathology. There were 11 cases of simple cysts, 5 cases of complex cysts (4 cases with hemorrhage or infection), 5 cases of renal parenchymal and pelvis inflammation, 2 cases of renal hematoma and 1 case of calyceal diverticulum. The conventional US showed 8 cases of cystic masses, 16 cases of solid –cystic masses, including septa of 12 cases, calcification of 6 cases, thicker wall or solid component of 4 cases. Color flow signals were showed in the wall or septa in 5 cases. Fifteen masses appeared as regular enhancement in small portion of septa in 7 cases, merely enhancement in the wall in 5 cases and honeycomb-like enhancement in 3 cases respectively, while 9 cases showed absence of enhancement. Cystic renal masses which show absence of enhancement or enhancement merely in the wall or regular enhancement in small portion of septa on CEUS should be regarded as benign lesions.
A Large Splenic Epidermoid Cyst Initially Misdiagnosed as a Hydatid Cyst

Abstract:BACKGROUND Splenic cysts are classified as either parasitic or non-parasitic cysts, with both types sharing similarities in their clinical presentation and imaging findings. Most splenic cysts are of parasitic origin, while non-parasitic splenic cysts are less common. Splenic epidermoid cysts (SECs) are a rare subtype of non-parasitic cysts and compromise only around 10% of them. CASE REPORT In this paper we present a case of 22-year-old man with no significant past clinical history, who presented with non-specific, vague symptoms, including persistent left upper-quadrant pain and discomfort for the last 2 years. A physical examination and extensive laboratory tests were inconclusive. Subsequently, the patient underwent multiple imaging studies including ultrasonography and computed tomography (CT) scan of the abdomen. His ultrasonographic findings were consistent with the diagnosis of hydatid cyst, which was further emphasized by its frequent occurrence in clinical practice, as our country is considered an endemic region. In light of this, he underwent laparoscopic splenectomy following percutaneous cyst drainage. The consequent histopathological examination revealed the diagnosis of splenic epidermoid cysts. CONCLUSIONS When encountering splenic cysts in regions where parasitic infections are endemic, special attention is needed, as physical examination, laboratory tests, and imaging studies alone are insufficient to differentiate among the types of cysts. Histopathological examination remains the diagnostic tool of choice, particularly when imaging findings are inconclusive. Splenectomy, with either a laparoscopic or open approach, is the treatment of choice for splenic cysts to prevent recurrence as well as other potential catastrophic complications.
23-year old man with a long history of abdominal pain, nausea and vomiting: Case report of a splenic cyst

Abstract:Introduction: Splenic cysts are rare. They are usually incidentally diagnosed and there is no harmonised treatment pathway. We report a case of a large splenic epidermoid type cyst without history of previous abdominal trauma. Presentation of case: A 23-year old male patient presented with symptoms of upper abdominal pain, nausea and vomiting. Except for a tenderness in the upper and lower left quadrant of the abdomen, the initial examination showed no extraordinary findings. A contrast enhanced computed tomography revealed a large singular splenic cyst displacing neighbouring structures. Echinococcus serology was tested negative. A laparoscopic fenestration of the superficially located splenic cyst was performed. Perioperative course was free of complications. Histopathological analysis of the excisate showed a squamous lining indicating the cyst as epidermoid type. Discussion: Non-parasitic cyst types include traumatic, neoplastic, degenerative and congenital cysts. Due to its considerable size, our patients splenic cyst was diagnosed after occurring symptoms lead to further examination (CT scan). Laparoscopic fenestration of the cyst was chosen as the optimal surgical approach because of the superficial location of the cyst and to preserve residual splenic parenchyma. In the present case, recurrence of the splenic cyst appeared, which left the patient with a total splenectomy as the final treatment choice. Conclusion: Due to the unspecific symptoms, the diagnosis of a splenic cyst can be prolonged. Choosing the adequate surgical technique to avoid complications is crucial. By deepening the understanding of the condition and surgical approaches, we can improve diagnostic and therapeutic management for affected patients.

Abstract:Ultrasound is the most used interdisciplinary imaging technique in clinical routine for assessment of renal pathologies. This includes the monitoring of cystic renal lesions, which can be classified as non-complicated or complicated and by means of occurrence as solitary or multifocal lesions. The Bosniak-classification (I-IV) classifies renal cysts in 5 different categories and is used for decisions of further clinical treatment. This classification was developed for computed tomography and has been adopted for magnetic resonance imaging as well as contrast-enhanced ultrasound. In the following review article, cystic kidney lesions and their differentiation using contrast-enhanced ultrasound are presented and an overview of the therapy options is given. In interventional procedures, CEUS can make a valuable contribution in histological sampling, reduce radiation exposure and, under certain circumstances, the number of interventions for the patient.
Ultrasound diagnosis of splenic hemangiolyphangioma

Abstract:Objective To investigate the diagnostic value of ultrasonography in spleen hemangiolyphangioma and the sonographic feature.Methods Sixteen patients with spleen hemangiolyphangioma were reviewed by routine ultrasound and color Doppler flow imaging(CDFI).The diagnosis of spleen hemangiolyphangioma was confirmed by pathology after operation.Contrast-enhanced ultrasonography(CEUS) was performed in five patients.Results There were 25 lesions totally.The lesions were cystic,honeycomb-shaped,well-demarcated(80.0%,20/25),clear margin(80.0%,20/25),and homogeneous(72.0%,18/25).CDFI showed blood flow in 56.0%(14/25) lesions.CEUS showed the lesions were mainly enhanced synchronously with the spleen parenchyma.The lesions were hypoechoic in comparison with the spleen parenchyma with branch-shaped diffuse enhancement pattern.The lesions also showed persistent enhancement.The detection rate of color flow before and after CEUS were 62.5%(5/8) and 100%(8/8),respectively.Conclusions Homogeneous hyperechoic leision with major solid portion is the characteristic sonographic feature of spleen hemangiolyphangioma.Ultrasound examination is helpful for the diagnosis of spleen hemangiolyphangioma.

Abstract:This narrative review aims to describe the current status of contrast-enhanced ultrasound (CEUS) in characterizing renal cystic lesion. The imaging techniques usually performed for their evaluation are ultrasonography (US), computed tomography (CT), and magnetic resonance imaging (MRI) with different criteria of application based on the individual case and the purpose of the examination. Generally, US, as a non-ionizing examination, is the first imaging modality performed and therefore the one that incidentally detects cystic lesions. CT is the most performed imaging modality for cystic lesion assessment before MRI evaluation. It provides better characterization and management and has been introduced into the Bosniak classification. In this context, CEUS is making its way for its characteristics and represents the emerging technique in this field. With these premises, the authors analyze the role of CEUS in the evaluation of renal cysts, starting with an explanation of the technique, describe its main advantages and limitations, and end with a discussion of its application in the Bosniak classification and management, following the current major guidelines.

Abstract:Objective: To discuss the value of contrast-enhanced ultrasound (CEUS) in the spleen. Methods: The spleens in 28 patients including 10 healthy cases and 18 cases with abnormal lesion were examined with 2D and CDFI. Then contrast-enhanced ultrasound with a blot injection of SonoVue was performed in 28 patients. Results: After injection of the contrast agent, the microbubble signals were seen well-distributed in 10 healthy cases and 2 suspected abnormal lesion cases. Contrast-enhanced ultrasound diagnosis was: hemangioma in 3 cases, spleen infarction in 2 cases, malignant tumour in 4 cases, accessory spleen in 2 cases and misdiagnosis in 3 cases including hematomas in 2 cases and spleen infarction in 1 case. Conclusion: Contrast-enhanced ultrasound has great value in the diagnosis of spleen diseases.
Large splenic epithelial cyst: A rare presentation

Abstract:Introduction and importance: Splenic cysts are infrequent and most of them are related to parasitic diseases. Splenic epithelial cyst (SEC) is the most common type of primary non-parasitic splenic cysts. Its pathogenesis is not yet clear. Splenic cysts are usually asymptomatic and are detected incidentally during imaging exams or an exploratory laparotomy. Case presentation: Our case is about a primary SEC discovered incidentally. An 18-year-old male patient without any personal and family medical history, presented with symptoms of upper urinary tract infection. Renal ultrasound was performed and found incidentally a solitary cystic lesion in the spleen. A laparoscopic partial splenectomy was made then. The specimen was sent for pathological examination and a diagnosis of primary (epidermoid) epithelial splenic cyst was given. Clinical discussion: SEC is a rare pathology that could mimic other splenic cysts like hydatid cyst. The preoperative diagnosis of SEC can be suspected by ultrasonography, computed tomography or magnetic resonance imaging. However, histopathological examination is mandatory to confirm the diagnosis. A wide range of differential diagnoses is considered when we suspect clinically SEC because of its lower incidence. Conclusion: Actually, spleen-preserving surgery with minimally invasive methods such as laparoscopy is the gold standard for the treatment of SEC despite the risk of recurrence encountered with these techniques. However, different surgical modalities are discussed depending on the size and the location of the cyst and the patient's age.
Contrast-enhanced Ultrasonic Evaluation of Mild Splenic Trauma

Abstract:Objective:The purpose of this study is to locate and characterize splenic trauma with contrast-enhanced ultrasound(CEUS).Methods:A defined mild intraparenchymal lesion was done in 2 pigs.Ultrasound features after trauma were observed by baseline ultrasound,color Doppler flow images(CDFI) and CEUS with a contrast agent(Sono Vue),respectively.Results:In 2 pigs,focal intraparenchymal lesions on spleen with diameters ranging from 10.0 to 10.0 mm could be identified by CDFI and CEUS but not by the baseline ultrasound.Hyperechoic rift in spleen was visible along trauma area as enhancement in splenic parenchyma disappeared on CEUS.Hyperechoic line could be kept in a long time.The hyperechoic rift also appeared at the sub-capsular region beside a traumatic lesion.Hyperechoic rift in traumatic region could be seen even as CEUS was converted to conventional B-mode.Enhancement in the traumatic lesion was clearly seen in CDFI.Conclusion:Hyperechoic rift in spleen was visible along trauma area as a contrast agent is used.CEUS is useful in identifying the grade,range and location of splenic injury.

Abstract:Epidermoid cyst of the spleen is a rare, benign cystic lesion of congenital origin. Although most epidermoid cysts of the spleen are asymptomatic and detected incidentally on ultrasonography, the entity needs to be included in the differential diagnosis of cystic lesion mimics of the spleen, especially in patients with left hypochondriac pain. This case report describes the typical sonological appearance of epidermoid cyst of the spleen in a 31-year-old male.
Conventional and Contrast-Enhanced Ultrasound Features in Sclerosing Adenosis and Correlation with Pathology

Abstract:OBJECTIVES: To evaluate the efficacy of conventional ultrasound (US) and contrast-enhanced ultrasound (CEUS) in differential diagnosis of sclerosing adenosis (SA) from malignance and investigate the correlated features with pathology. METHODS: We retrospectively enrolled 103 pathologically confirmed SA. All lesions were evaluated with conventional US while 31 lesions with CEUS. Lesions were divided into SA with or without benign lesions (Group 1, n = 81) and SA with malignancy (Group 2, n = 22). Performance of two methods were analyzed. The ultrasonographic characteristics were compared between two groups with Student’s t-test for measurement and chi-squared or Fisher’s exact test for count data. RESULTS: There were 22 lesions complicated with malignancy, and the mean age of Group 2 was higher than Group 1 (55.27 vs. 41.57, p < 0.001). The sensitivity, specificity and accuracy of conventional US and CEUS were 95.45%, 46.91%, 57.28% and 100%, 62.5%, 70.97%. Angularity (p < 0.001), spicules (p = 0.023), calcification (p = 0.026) and enlarged scope (p = 0.012) or crab claw-like enhancement (p = 0.008) in CEUS were more frequent detected in SA with malignancy. CONCLUSIONS: Though CEUS showed an improved accuracy, the performance of ultrasound in the diagnosis of SA was limited. Awareness and careful review of the histopathologically related imaging features can be helpful in the diagnosis of SA.
