Palliative Care Consultation And Outcomes Of Heart Failure Hospitalizations With Covid Infection During The 2020 Pandemic. Insight From The 2020 Nationwide Inpatient Sample Database

Abstract:Introduction There is a paucity of data regarding individuals with heart failure (HF) and active COVID-19 infection who got a palliative care (PC) consultation during their hospitalization and their outcomes during the 2020 global pandemic. Hypothesis There is low utilization of PC consultation for in-hospital HF patients despite evidence of PC improving quality of life (QoL). Method We analyzed the 2020 Nationwide Inpatient Sample (NIS). We used the ICD-10 code to select hospitalizations with active COVID-19 infection (U071) and HF. We examined HF hospitalizations with COVID-19 infection. We represented continuous variables as means with standard deviations (SD) and categorical variables as frequencies and percentages. Result There were 1,678,995 COVID-19 hospitalizations, 4,174,024 HF hospitalizations, and 224,246 PC consultations in 2020. Of these, 58,151 (24.4%) PC consultations were for HF hospitalizations (p<0.0001).There was a significant increase in PC consultations from the previous years for HF hospitalizations despite an overall decreased total number of all hospitalizations during the pandemic (14% in 2016 vs. 22.9% in 2019 vs. 24.4% in 2020 (p<0.0001). There were 6,107 HF hospitalizations with active COVID infection who received a palliative consultation (CovHF), a mean age of 77.6 (SD=11.9), and 47.7% were females. Regarding ethnicity, 68.3% were Whites, 15.7% were Blacks, 10.3% were Hispanics, and 2.2% were Asians (p<0.0001). The mean length of stay (LOS) was 11.4 days (SD=11) (p<0.0001), with in-hospital mortality of 65.2% (p<0.0001) and a mean total hospital charge of $145,572 (SD=208,824) (p<0.001). There was a Do Not Resuscitate (DNR) order for 4,931 (80.7%) CovHF hospitalizations. Conclusion There was an overall improvement in palliative care consultation for patients with heart failure during the 2020 pandemic. This, however, pales to the high mortality rates and DNR in this patient population. More steps to include palliative consultation in managing these patients are needed.
