Abstract:To investigate the clinical value of three-dimensional ultrasonography (3DUS) in obstetrics, various 3DUS rendering methods including surface mode, transparent mode and multiplanar mode were employed to scan 30 fetuses in second and third trimester by using the transabdominal volume transducer. The results showed that surface mode could vividly demonstrate the surface morphologic features of the fetuses, as well as the stereo-shape and the spatial relationship among the surface structures. The face, limbs, umbilical cord and outer genitalia of the fetus could be well displayed by surface mode. Transparent mode could reveal the bony structures under the surface, such as ribs, vertebrae, crania, etc. The result was not affected by the sophisticated curvature of these bony structures and the success rate was up to 100 %. When rendered by multiplanar mode, the region of interest (ROI) could be viewed from different directions. It should be concluded that 3DUS could serve as a supplement to two-dimensional ultrasonography (2DUS). 3DUS might play an important role in prenatal diagnosis and enhance the diagnostic confidence level of the physicians.

Clinical application value of three-dimensional ultrasonography in obstetrics

Abstract:Objective To evaluate the clinical application value of three-dimensional ultrasonography (3DUS) in obstetrics.Methods Thirty pregnant women from 18 to 40 weeks of gestation were studied with transabdominal volume transducer,of which the frequency range was 3.0～5.0 MHz.The reconstruction methods included surface mode,transparent mode and multiplanar mode.Results Surface mode could clearly demonstrate the surface features of the fetus,such as the fetal face,fetal genitalia,umbifical cord,etc.Transparent mode could display the inner structures (mainly the bony structures) of the fetus,and their 3D images were not affected by their curvature.The successful rats was up to 100%.On the other hand,the region of interest could be viewed from various directions by using multiplanar mode.Conclusions 3DUS has special advantages and clinical value in obstetrics.It can serve as a beneficial supplement to two-dimensional ultrasonography,but some work still should be done to further develop it.

Abstract:Objective To search for an effective imaging method for the morphological observation of female pelvic support by studying 3D ultrasound image features of nulliparous vagina and perivaginal structures at different level.Methods Fifty nulliparas were examined by 3D pelvic floor ultrasonography.The appearance of vagina and perivaginal structures was observed from vaginal vault to orifice in transversal 3D reconstruction image of pelvic floor.Results Three-dimensional pelvic floor ultrasound showed cross-sectional imaging of the whole vagina clearly.The vaginal transversal morphologies were different at different level.It was lip-like in proximal vagina,butterfly-like in middle vagina and crescent-like in distal vagina.The perivaginal structures at different level were different.Perineal body was at distal vagina level,urethrovaginal septum,rectovaginal septum and paravaginal support at middle vagina level,perivaginal connective tissue at proximal vagina level.All were uniform,dense and complete.Conclusions The 3D pelvic floor ultrasound images of vagina and perivaginal structures at different level are distinct,which is valuable for the study of 3D ultrasound in female pelvic support and pelvic floor dysfunction.
Can three-dimensional transvaginal sonography replace office hysteroscopy in detecting uterine abnormalities in infertility patients?

Abstract:BACKGROUND: Evaluation of uterine cavity is an important step during investigation of infertile women. The presence of uterine pathology causes impaired receptivity, failed implantation and poor pregnancy outcomes. Various investigative modalities though available; have their limitations. Hysteroscopy considered the gold standard diagnostic modality is invasive; thus, an investigation which could overcome its limitations was required. 3-Dimensional transvaginal sonography (3D TVS), which non-invasively visualizes uterine morphology, registers all three imaging planes simultaneously could be an alternative to hysteroscopy.AIM: To compare 3-D TVS with the gold standard office hysteroscopy for evaluating uterine cavity in sub fertile women.SETTING AND DESIGN: It was a comparative study carried out at the Reproductive Medicine Centre of a tertiary care hospital.MATERIALS AND METHODS: Over 12 months , 154 women with infertility underwent both hysteroscopy and 3D TVS. The primary outcome was to compare the sensitivity and specificity of 3D TVS in diagnosing endouterine abnormalities and assess if 3-DTVS could replace hysteroscopy for evaluation of uterine cavity.STATISTICAL ANALYSIS: The sensitivity, specificity, positive and negative predictive value and diagnostic accuracy of the 3-D TVS was calculated.RESULTS: The 3-D TVS had a sensitivity of 88.46% ; specificity of 99.21% for detection of uterine abnormalities with hysteroscopy as gold standard. The Positive Predictive Value was 95.83%; Negative Predictive Value 97.69% and diagnostic accuracy was 97.4%. It was seen that of 24 lesions diagnosed by 3D TVS, hysteroscopy was in agreement with 21 lesions.CONCLUSION: 3D TVS with advantages of giving better spatial orientation, non-invasive, pain free, can replace hysteroscopy for screening endouterine lesions. With no statistically significant superiority of hysteroscopy over 3D TVS as seen in this study it may be deemed unnecessary in 50% patients having normal uterine cavity.

The role of three-dimensional imaging in the investigation of IUD malposition

Abstract:Objectives: The aim of this study was to present a systematic review of the use of three-dimensional ultrasound for the detection and evaluation of intrauterine device malposition in a patient of our department. Background: Transvaginal sonography failed to detect IUD misplacement in about 9% of cases. Whereas the three-dimensional ultrasound (3D), a new emerging technology, that could provide precise evaluation of IUD malposition. Methods: The data were extracted from the literature using computerised Medline system. The use-effectiveness and acceptance of three-dimensional imaging in the detection of IUD malposition was examined as an alternative method to two-dimensional ultrasound. Results: Three-dimensional ultrasound was more accurate than two-dimensional for the identifying IUD device. Three-dimensional technique enables assessment of IUD length in the longitudinal section synchronically with imaging arms of the device. Examination with 2D ultrasound is limited to transverse views of the shaft. The arms or other smaller parts cannot be investigated completely because of the frontal view of an IUD is rarely presented. Conclusion: Three-dimensional ultrasound is a new and promising imaging tool which provides much better view of the endometrial cavity. This is especially useful in uterus examination and in the detection of IUDs because structures that are not located in one single plane can be imaged simultaneously. Three-dimensional ultrasound is considered to be more secure and safer diagnostic technique to determine the location of IUDs than hysteroscopic evaluation (Fig. 2, Ref. 17). Abstract:Background: Three-dimensional sonographic technology has gained increased acceptance in clinical practice. This technique involves the gathering of a large amount of data and the quick reconstruction of pictures in the coronal, sagittal and transverse planes. The purpose of this work was to compare the diagnostic accuracy of the three-dimensional trans vaginal ultra-sonographic (3-DTVUS) and hysteroscopy in the detection of cavitary lesions in the uterus. Methods: This study was comparative cross-sectional and included 60 patients who had 2D ultrasonography or hysterosalpingography for suspected intrauterine abnormalities. Each patient underwent a thorough history taking, clinical assessment, investigations, imaging [2D transvaginal ultrasound and 3D vaginal ultrasound] and hysteroscopy. Results: 3DTVUS had a (95%) sensitivity in detecting uterine abnormalities compared to hysteroscopic examination. Also, the specificity was (88%) which indicates that only (12%) of the truly negative cases will be missed. The positive predictive values (PPV) of 3D TVUS were (99%) with an accuracy of (94%). The lowest parameter calculated was the negative predictive value (NPV) (58%). 3DTVUS had a specificity and PPV of 100% regarding the detection of all of the mentioned uterine abnormalities. Furthermore, the sensitivity was 100% for all abnormalities except the polyps and adhesions which were 91% and 67% respectively. The NPV was 100% for all abnormalities except for the polys and intrauterine adhesions (IUAs) which recorded 97%. Finally, the accuracy of 3DTVUS in comparison to hysteroscopy was 100% for all intrauterine abnormalities except the polyps and IUAS which were 98% and 97% respectively. Conclusions: 3DTVUS can be utilized in diagnosing focal lesions of the uterus with comparable outcomes to hysteroscopy.
