Abstract:Background: Pregnancies in women with a unicornuate uterus are associated with an increased risk for specific adverse pregnancy and obstetric outcomes. We sought to investigate the obstetric outcomes of singleton pregnancies in nulliparous women with a unicornuate uterus in the third-trimester and to determine safe and appropriate modes of delivery. Methods: This retrospective cohort study included 94 nulliparous women with singleton pregnancies and a unicornuate uterus, along with 278 matched controls with a normally shaped uterus, from March 2009 to March 2019. Results: A total of 124 patients were diagnosed with a unicornuate uterus in the obstetrics department after the 28th week of pregnancy, including 94 nulliparous women. Compared to pregnancies with normal uterine morphology, a unicornuate uterus is associated with an increased risk of preterm delivery (PTD), preterm prelabor rupture of membranes (PPROM), malpresentation, caesarean section (CS), intrauterine growth restriction (IUGR), and neonatal intensive care unit (NICU) admission. Additionally, pregnancies with unicornuate uterus show significantly lower gestational age at delivery, as well as lower rates of term delivery and vaginal delivery. No increased risk was observed for in-labor CS rates or labor duration. In total, 17 pregnant women in the unicornuate uterus group had vaginal deliveries, including 2 who had previously undergone rudimentary horn resection. Conclusions: The presence of a unicornuate uterus is associated with an increased risk of specific adverse obstetric outcomes compared to pregnancies in women with a normal uterus. However, vaginal delivery may be considered in in women with a unicornuate uterus thorough evaluation.

Abstract:To investigate the pregnancy outcomes between women with a didelphus uterus and those with a unicornuate uterus after in vitro fertilisation-embryo transfer (IVF-ET). This was a retrospective analysis of 50 infertile women with a didelphus uterus and 280 cases with a unicornuate uterus from January 2004 to December 2017 at our reproductive centre. All women conceived a singleton pregnancy after IVF-ET at our reproductive centre. The diagnoses of uterine malformations were based on American Fertility Society (AFS) classification system. The outcome measures included pregnancy and obstetric outcomes, which were compared between the didelphus uterine group and the unicornuate uterine group. The didelphus uterine group and the unicornuate uterine group were statistically similar regarding maternal age ( 30.4 ± 4.7 vs. 29.9 ± 4.5 years; P = 0.505), infertility type (P = 0.462), cause of infertility (P = 0.070) and insemination methods (P = 0.261). The didelphus uterine group and the unicornuate uterine group had similar rates of miscarriage (18.0 vs. 20.7%; P = 0.660), ectopic pregnancy (0 vs. 1.1%; P = 1.000), preterm delivery (16.0 vs. 13.6%; P = 0.648), term delivery (66.0 vs. 64.6%; P = 0.853), very preterm birth (2.4 vs. 2.7%; P = 0.913), low live birth weight (12.2 vs. 11.6%; P = 0.918), very low live birth weight (2.4 vs. 0.5%; P = 0.295), Caesarean section (85.4% vs. 84.0%; P = 0.828), live birth (82.0 vs. 76.8%; P = 0.415) and perinatal mortality (0 vs. 1.8%; P = 1.000); additionally, the gestational age at delivery (38.2 ± 2.1 vs. 37.9 ± 2.7 weeks; P = 0.465) and the live birth weight (3026 ± 573 vs. 3072 ± 518 g; P = 0.608) had no significant differences between these two groups. For singleton pregnancy conceived via IVF-ET, women with a didelphus uterus and those with a unicornuate uterus had similar pregnancy and obstetric outcomes.

The effect of unicornuate uterus on reproductive outcomes in infertile patients

Abstract:Objective: Unicornuate uterus, which is a rare congenital anomaly, has been associated with obstetric and perinatal complications. The aim of this study is to evaluate the reproductive outcomes of patients with unicornuate uterus, and also to compare them with outcomes of infertile patients without a Müllerian anomaly. Methods: Retrospective analyses of the data of 18-40 year old infertile patients with unicornuate uterus diagnosed in between January 2012 and December 2022, and a control group with the same number of patients, with age and infertility durations matched were performed. Demographic data, cycle parameters and reproductive outcomes of the unicornuate uterus and control groups were compared. Results: A total of 75 patients with unicornuate uterus and a control group of 75 infertile patients without a Müllerian anomaly were included in the study. Totally, 116 and 91 pregnancies were achieved at unicornuate uterus and the control groups, respectively. Ectopic pregnancy rates (10.3 % vs 2.2 %, OR = 5.53, 95 %CI [1.17-26.21]) and malpresentation rates (29 % vs 0 %, OR = 1.40, 95 %CI [1.12-1.76]) were significantly higher and newborn birth weights in singleton pregnancies were significantly lower in the unicornuate group (3000 g vs 3455 g, p = 0.005). No significant difference was found in other obstetric parameters. Conclusion: Unicornuate uterus constitutes a small portion of all congenital uterine anomalies. Although increased ectopic pregnancy and malpresentation rates with diminished birth weights were found in the unicornuate group in our study, live birth rates and preterm deliveries did not differ significantly between the two groups. However, patients should be counseled on this issue both during the reproductive treatment process and during pregnancy, and should be followed closely.

Abstract:Background Unicornuate uterus, a congenital uterine malformation resulting from unilateral maldevelopment of Mullerian duct, is more prevalent in women with infertility. Owing to relative rarity of the condition, the evidence on the associated reproductive outcomes is derived from small heterogeneous studies that report different clinical endpoints and often do not account for the anatomical variations of unicornuate uterus. The aim of this study was to evaluate the embryological and clinical outcomes following IVF-ICSI treatment in women with unicornuate uterus without rudimentary functional cavity (ESHRE-ESGE class IVb). Methods Retrospective nested case-control study comprised 342 women with unicornuate uterus and 1026 matched controls who underwent IVF-ICSI cycles between October 2012 and October 2016. Cumulative live birth rate upon one complete IVF cycle, including transfers of all resulting embryos was considered as a primary outcome measure. Results Baseline characteristics were comparable between the unicornuate uterus and control groups except for higher rate of primary infertility in unicornuate uterus. Ovarian response to stimulation did not differ between the groups. Transfer of day-3 embryos in fresh cycle resulted in lower clinical pregnancy rate (35.9% vs. 43.9%, p = 0.028) and live-birth rate (26.9% vs. 35.2%, p = 0.017) per transfer, but the difference was not observed when either cleavage frozen-thaw embryos or blastocysts were transferred. Implantation rate was lower and miscarriage rate was higher in women with unicornuate uterus but the difference between the groups did not reach statistical significance. Transfer of cleavage embryos resulted in significantly higher miscarriage rate and lower live-birth rate in fresh versus frozen-thaw cycles in each group, whereas fresh and frozen-thaw blastocyst embryos had comparable outcomes. Upon completion of one IVF-ICSI cycle, the cumulative pregnancy rate (53.1% vs. 65.7, p < 0.001) and cumulative live birth rate (42.4% vs. 54.6%, p < 0.001) were significantly lower in women with unicornuate uterus compared to those in women with normal uterus. Cumulative outcomes were superior when embryos were cultured to blastocyst stage. Conclusions Women with unicornuate uterus have lower clinical pregnancy and live birth rate after IVF-ICSI treatment compared to women with normal uterus. The treatment outcomes are improved with blastocyst culture, which warrants evaluation in prospective setting.
The Impact of Unicornuate Uterus on Perinatal Outcomes after IVF/ICSI Cycles: a Matched Retrospective Cohort Study.

Abstract:Objective: To evaluate the impact of unicornuate uterus on perinatal outcomes after in vitro fertilization and/or intracytoplasmic sperm injection (IVF/ICSI) cycles. Methods: We performed a retrospective cohort study including 160 women with unicornuate uterus and 1:1 matched controls with normally shaped uterus. They received IVF/ICSI treatment during January 2009 and December 2011. The perinatal outcomes were followed up till December 2014. Results: There were no significant differences in pregnancy rate, clinical pregnancy rate or live birth rate (53.6 versus 52.7, 41.4 versus 43.5, 33.8% versus 31.8%) between unicornuate uterus group and controls. Their biochemical pregnancy rate (22.8 versus 17.5%) and miscarriage rate (16.0 versus 18.8%) were similar. No significant differences were identified in preterm birth rate (18.3 versus 11.8%), birthweight (3.24 +/- 0.60 versus 3.33 +/- 0.54 kg) or birth length (50.47 +/- 2.33 versus 50.06 +/- 2.40 cm) among the singletons. However, lower gestational age (35.56 +/- 2.68 versus 36.71 +/- 1.73, p = .019), higher preterm birth rate (55.0 versus 34.4%, p = .038), lower birthweight (2.33 +/- 0.58 versus 2.69 +/- 0.38 kg, p = .001), lower birth length (45.33 +/- 2.46 versus 48.88 +/- 2.06 cm, p = .000), as well as higher rate of very low birthweight (13.2% versus 0, p = .007) were found for the twins from unicornuate uterus group. Conclusions: The results indicated unimpaired pregnancy and perinatal outcomes for women with unicornuate uterus conceiving one fetus. However, close attention should be paid to twin pregnancy in unicornuate uterus owing to increased risks of prematurity and low birthweight. Selected single embryo transfer is recommended for women with unicornuate uterus undergoing IVF/ICSI cycles.

Abstract:Objective To discuss the characters,diagnosis and treatment of oblique vaginal septum syndrome.Methods Ten cases of oblique vaginal septum syndrome in the latest 6 year(from April 2000 to May 2006) were collected and analyzed retrospectively.Results The patients aged 13-30 including 2 cases of type Ⅰ,7 cases of type Ⅱ and 1 case of type Ⅲ,had frequent symptoms of dysmenorrheal and abdominal pain(7 cases),vaginal puruloid secretion(3 cases) and infertility(3 cases);10 of them were complicated with double uterus,2 with unicornous uterus,3 with endometrial cystis;8 of them were complicated with ipsilatera renal agenesis,2 with horseshoe kidney;2 of them were misdiagnosised.The main treatments were septile incisions and sometimes together with essential cystectomy and unicornous hysterectomy.Conclusion The symptoms of oblique vaginal septum syndrome were diverse,commonly with companion of ungenitalsy stem malformation.It should be diagnosed as early as possible by clinical manifestation,vaginal examination and ultrasound reports,and the misdiagnosis rate can be reduced.

Pregnancy in a Unicornuate Uterus with Non-Communicating Rudimentary Horn: Diagnostic and Therapeutic Challenges

Abstract:Unicornuate uterus with non-communicating rudimentary horn is a type of congenital uterine abnormality that occurs as a consequence of the arrested development of one of the two Müllerian ducts. Patients with unicornuate uterus have increased incidence of obstetric and gynaecological complications. We present a report of a clinical case of a 28-years-old female, who was referred to the hospital for evaluation of her infertility. The patient reported primary infertility and inability to conceive after 3-year period of regular unprotected intercourse. Transvaginal ultrasound, along with the preoperative evaluation were completed; however, no anomalies or irregularities were reported. Combined diagnostic simultaneous laparoscopy and hysteroscopy were performed to establish the diagnosis of unicornuate uterus with non-communicating rudimentary horn. The patient conceived spontaneously after diagnostic laparoscopy and hysteroscopy. During and after pregnancy, our patient and her child experienced numerous complications (cervical incompetence, acute chorioamnionitis, acute fetal distress, pneumonia, septic shock) and procedures (cervical cerclage, urgent cesarean section, intensive care unit treatment) without significant fetal or maternal compromise.

Abstract:Unicornuate uteri are caused by non-development of one Müllerian duct. The objective of this study was to investigate pregnancy outcomes of singleton and/or twin pregnancies in women with unicornuate uterus after IVF–embryo transfer (IVF–ET). The study group comprised 238 patients with a unicornuate uterus and the control group 818 patients with normal uterus. Compared with pregnancies with normal uterine morphology, a unicornuate uterus was associated with an increased risk of early pregnancy loss [adjusted odds ratio (aOR) 1.88, 95% confidence interval (CI) 1.25–2.83; P = 0.002], premature delivery (aOR 2.11, 95% CI 1.45–3.07; P < 0.001), perinatal mortality (aOR 3.35, 95% CI 1.89–5.94; P < 0.001), low birthweight (LBW, aOR 1.43, 95% CI 1.04–1.79; P = 0.005) and very low birthweight (VLBW, aOR 2.58, 95% CI 1.53–4.34; P < 0.001). Additionally, significantly lower rates of term delivery (aOR 0.43, 95% CI 0.31–0.58; P < 0.001) and live birth (aOR 0.53, 95% CI 0.37–0.76; P < 0.001) were observed. These findings indicate that the presence of unicornuate uterus is associated with significantly increased risk of some adverse pregnancy outcomes compared with pregnancies with a normal uterus in women who undergo IVF–ET.
