A hidden cause of middle-aged onset heart failure with preserved ejection fraction: a GTPBP3 variant

Abstract:A 47-year-old man with heart failure with preserved ejection fraction (HFpEF) and without hearing loss, diabetes, intellectual disability, or seizure was referred to our hospital. Blood tests showed mild elevation in brain natriuretic peptide and lactate concentrations. Magnetic resonance imaging showed mild concentric left ventricular hypertrophy ( Panel A ; Supplementary data online , Video S1 ). Electron microscopy of myocardial biopsy specimens revealed characteristic findings of mitochondrial cardiomyopathy 1 : disruption of the myofibrillar structure by a marked increase in mitochondria and appearance of malformed mitochondria (white arrow) and large autophagic vacuoles containing mitochondria ( Panels B–D ). Complex I activities of myocardium were modestly reduced (29% of citrate synthetase) together with reduced immunohistochemical signals of NDUFA9 ( Panel E ). 2 The maximum respiration rates of dermal fibroblasts isolated from the patient in glucose medium ( Panel F ) and galactose medium (not shown) were reduced to 52% and 42%, respectively, of that in a normal control, similar to other cases of mitochondrial respiratory chain disorders (MRCDs). 3 Results of targeted re-sequencing showed a novel homozygous missense variant, NM_001128855.2: c.136T>G (p.Cys46Gly), in GTPBP3 , a nuclear DNA-encoded gene, which was predicted as damaging by bioinformatics analyses (SIFT and MutationTaster prediction). The mRNA and protein levels of GTPBP3 were reduced in dermal fibroblasts from the patient ( Panels G and H ). GTPBP3 variants are associated with a mitochondrial translational defect, leading to infancy/childhood-onset MRCDs. 4,5 This case indicates that mitochondrial cardiomyopathy without characteristic manifestation of MRCDs may be latent in adult-onset HFpEF. Ultra-structural/immunohistochemical analyses of myocardial biopsy samples provide a clue to proceed to further analyses including genetic testing. 1,2
