Abstract:Hereditary myopathy with early respiratory failure (HMERF) is a rare but geographically widespread disorder. This study was initiated to clarify the phenotypic and molecular spectrum in Chinese neuromuscular cohort. Next-generation sequencing of all coding regions of genes related to neuromuscular disorders (Gene table of monogenic neuromuscular disorders Version 2018) was used for screening patients with characteristic features of HMERF. Seven different titin mutations were identified in 11 patients from 10 unrelated families. Four mutations have been reported while three are novel, all located exclusively in the FN3 119 domain (A150) of A-band titin, except for one novel variant. The novel variant TTN c.95962G>A p.V31988M located in the exon 345, which is not related to HMERF before. The patient carrying heterozygous p.V31988M mutation had clinicopathologic hallmarks, including exertional dyspnea, lower limb weakness, typical muscle MR findings, and cytoplasmic bodies. The other two novel mutations are small deletions without interruption of the opening reading frame. The p.C31712R missense mutation was found in 3 familial cases, while p.P31732L was only seen in sporadic cases. One of the patients carries homozygous p.P31732L mutations with subclinical myopathy in the heterozygous mother, suggesting a semi-recessive inheritance pattern. Cardiac abnormalities were identified in 2 cases: one with I° atrioventricular block, the other had imaging evidence of otherwise unexplained hypertrophic cardiomyopathy. We report a Chinese cohort of patients with HMERF with seven different mutations, which extended the primary causative region to TTN exon 345. Our cohort suggested that HMERF is not extremely rare, and should not be overlooked in undetermined myogenic respiratory failure.
