Impacting GDMT With Story Health - 1 Year Later

Abstract:Introduction Heart failure causes significant morbidity & mortality. Maximally tolerated GDMT improves both. Inpatient titration is often difficult in the setting of acute illness & outpatient titration of GDMT continues to pose a challenge. Story Health is a company designed to interface with electronic medical record (EMR) & provide a link between clinicians & patients to optimize GDMT faster. Objective In the past, with a smaller sample size & a shorter time period, Story Health demonstrated effective titration of maximally tolerated GDMT. We aim to assess the reproducibility & effectiveness of Story Health in maximally optimizing GDMT in patients with ACC Stage C, HFrEF over the course of 1 year. Methods HFrEF patients identified during hospitalization & in advanced heart failure clinic were enrolled & followed for 1 year. At the time of enrollment, patients received BP cuffs & weight scales that transmitted their vital signs to a secure server. A titration schedule was built by enrolling HF cardiologists. Medication adjustments were made biweekly with prompts by a cardiac nurse to the clinician per the titration schedule. They contacted patients regularly for symptoms, vitals and for labs as scheduled. Clinicians reviewed patients’ progress in a Story Health integrated EMR site. Clinicians then approved next GDMT optimization steps or made further adjustments as necessary. The data was collected, and titration point scheme was used as shown in Table 1. Results 245 patients were recruited between 6/2021 & 3/2023. 151/245 patients were still enrolled at the 1-year mark. At enrollment, average titration points were 4. After 1-year, the average points increased to 7. Though majority of titration was performed in the first 90 days, optimization continued for the entire year (Table 2). Conclusions Story Health GDMT titration is effective in instituting maximally tolerated GDMT & the data is reproducible with a larger sample size over a longer period. Further data analysis is needed to assess effectiveness to reduce re-admissions & mortality.
