Abstract:Purpose: To determine the feasibility of percutaneous microwave coagulation (PMC) for the treatment of small solitary breast cancers. Materials and Methods: With approval of the institutional ethics committee and written informed consent, 41 patients with core-needle-biopsy-proved breast cancers 3.0 cm or less in diameter accessed by using ultrasonography (US) were recruited. US-guided PMC was performed with general anesthesia, followed immediately by mastectomy. Histochemical staining with a-nicotinamide adenine dinucleotide, reduced (NADH)-diaphorase was used to determine cell viability and the extent of PMC lesions. Results: The mean tumor volume was 5.26 cm(3) +/- 3.80 (standard deviation), with a range from 0.09 to 14.14 cm(3). PMC was successfully performed in all cases, with complete tumor ablation as assessed by using US. The mean time to reach complete ablation was 4.48 minutes, ranging from 3 to 10 minutes. With microscopic examination, 37 of 41 cases (90%; 95% confidence interval [CI]: 76.9%, 97.3%) showed complete tumor coagulation, as observed by using a-NADH-diaphorase staining. Of 38 cases diagnosed with invasive ductal carcinoma, 36 cases (95%; 95% CI: 82.3%, 99.4%) showed complete tumor coagulation. Slight thermal injuries to the skin and pectoralis major muscle, which proved reversible, were found in three cases. Conclusion: US-guided PMC of small solitary breast cancers is feasible. Nevertheless, larger-scale clinical trials are still needed to validate PMC for adoption into a standard clinical practice. (C) RSNA, 2012

Abstract:To test the feasibility, safety, and efficacy of microwave (MW) ablation for primary hyperparathyroidism (pHPT) in patients who are unsuited or unwilling to undergo surgery.Fifteen patients with benign parathyroid nodules were treated with MW ablation. Ultrasound, laboratory data, and clinical symptoms were evaluated before treatment; 1 week and 1, 3, 6, and 12 months after treatment; and every 6-12 months thereafter.All patients were followed up for more than 1 year, with an average duration of 32.8 months ± 17.9. Eleven patients underwent successful ablation in a single session, and two patients with bilateral disease and two patients with residual disease were treated with two sessions each. The rate of complete nodule disappearance was 17.6%. Nodule volume and serum parathyroid hormone (PTH) and calcium levels were significantly lower at the last follow-up than before treatment (volume, 0.39 cm(3) ± 0.69 vs 2.62 cm(3) ± 3.32; PTH, 54.5 pg/mL ± 24.1 vs 592.5 pg/mL ± 579.1; and calcium, 2.32 mmol/L ± 0.12 vs 2.93 mmol/L ± 0.47; P < .01). Treatment was well tolerated. Minor complications included transient voice change in one patient.MW ablation is a safe and effective technique for the treatment of pHPT. It is a good alternative for patients who do not meet surgery criteria or decline surgery.
Study on Ultrasound-guided Percutaneous Microwave Ablation and Surgical Resection for Papillary Thyroid Microcarcinoma

Abstract:Objective To evaluate the clinical efficacy and compare with these of safety of Ultrasound-guided percutaneous microwave ablation and surgical resection for papillary thyroid microcarcinoma.Methods 65 patients with ultrasound guided fine-needle aspiration biopsy(FNAB)proven thyroid papillary carcinoma,nodal diameter<1.0cm,have divided into two groups according to their preference,30 cases were enrolled in microwave ablation treatment group and 35 cases in surgical treatment group.Operation time,amount of blood loss,complications,postoperative changes of thyroid-related hormone levels and tumor shrinkage after microwave ablation were observed in two groups.Results In the microwave ablation treatment group,the operation time was shorter,the amount of blood loss was less and the hospitalization time the thyroid-related hormone levels was changed small after the treatment.The differences between the two groups were statistically significant(P>0.05).While the postoperative complications and efficiency between the two groups were not statistically significant(P>0.05).Conclusions Ultrasound-guided percutaneous microwave ablation therapy of thyroid papillary carcinoma has advantage than surgical resection,it′s a new minimally invasive treatment for patient who have surgical contraindications or reluctant to surgery.
Safety and Effectiveness Analysis of Percutaneous Microwave Thermal Ablation Therapy Combined Brachytherapy for Lung Malignant Lesion

Abstract:BACKGROUND:Microwave ablation and radioactive seed implantation are therapeutic options for patients with advanced lung cancer and lung metastases lesion who are not surgical candidates. However, reports on therapeutic effectiveness and safety of the two combination treatments methods have been limited to small case series. This research was to assess the clinical outcome and safety of radioactive seed implantation combined with computed tomography (CT)-guided microwave ablation for the treatment of advanced lung cancer and lung metastases lesion.METHODS:The clinical data of 21 advanced lung cancer and lung metastases patients who received treatment at author hospital during the period from May 2018 to December 2018 were retrospectively analyzed. The patients were divided into two groups by whether received extra percutaneous microwave thermal ablation therapy. The short-term effectiveness and safety were analyzed.RESULTS:The efficacy of control group was 28.75%, and the efficacy of experiment group was 14.28%. The rate of adverse reaction between two groups has no significantly difference (P>0.05).CONCLUSIONS:Seed implantation combined with CT-guided microwave ablation for advanced lung cancer and lung metastases is safety and effective.

Abstract:Breast cancer is the most common malignancy in women worldwide. Recent developments in minimally invasive interventional radiology techniques have significantly improved breast cancer treatment. This study aimed to develop a novel technique for the local management of breast cancers using radiofrequency heat (RFH). We performed both in vitro experiments using human breast cancer cells and in vivo validation in xenograft animal models with magnetic resonance imaging (MRI) and pathological correlation to investigate the feasibility of our approach. Four treatment groups, including (1) no treatment (control), (2) RFH-only, (3) chemo (doxorubicin)-only, and (4) combination therapy with both doxorubicin and RFH, were conducted in each experiment. In vitro combination therapy significantly decreased breast cancer cell proliferation while increased their apoptosis index compared to the other three groups. MRI demonstrated a significant tumor size reduction in animals treated with combination therapy compared to those receiving other treatments in vivo. Such result was further confirmed by pathological examination. In conclusion, our findings suggests that RFH can enhance the therapeutic efficiency of doxorubicin on breast cancers, thus establishing the basis for future development of interventional molecular image-guided local chemotherapy for breast malignancies.

Abstract:AbstractBackgroundComputed tomography (CT)‐guided percutaneous microwave ablation (MWA) is a very common ablation method that shows a good local tumor control rate in primary and secondary lung tumors. At present, few reports have explored the safety and efficacy of MWA for lung metastases from breast cancer.MethodsFrom January 2012 to January 2018, 32 breast cancer patients with 46 pulmonary metastases received CT‐guided percutaneous MWA. The study was approved by the local institutional review board. The clinical efficacy and complications of MWA were investigated.ResultsThe median follow‐up time was 32 months and the main effective rate was 97.8% (45/46). Five of 46 lesions had local progression (10.9%), with a median progression time of 10 months. The 1‐, 3‐, and 5‐year overall survival (OS) rates were 96.9%, 53.3%, and 17.8%, respectively. The median OS time was 36 months. Among 46 MWA treatments, 11 (23.9%) had massive pneumothorax, two (4.3%) had massive pleural effusion, and two (4.3%) had a pulmonary infection.ConclusionCT‐guided percutaneous MWA may be safe and effective for treating lung metastases from breast cancer.

Abstract:Objective To investigate the method of ultrasound guided percutaneous microwave coagulation therapy(PMCT),and to evaluate the efficacy of PMCT for patients with liver carcinoma. Methods Puncture frame combined with free-handed method was used,32 patients with 47 tumor nodules underwent sonographically guided PMCT with 60～80W,10～20min one or many coagulation. Results Among the 47 nodules in 32 patients with liver carcinoma ,a follow-up of 8～56 months demonstrated that the lesions became smaller in 95.64%, and among them 31.91% became smaller more than 50%.35 tumors (74.47%) showed no enhancement on contrast-enhanced. AFP level decreased in 17 and became normal in 13(76.48%). A post-treated biopsy on 19 lesions showed complete destruction of tumor in 73.7%. The survival rate of 1,2,3 and 5 years were 93.94%,80.99%,72.23% and 72.23%.There were no serious clinical side effects or complications. Conclusions Ultrasound guided percutaneous microwave coagulation therapy for liver carcinoma is effective and safe. The location is accurate.
Clinical Assessment of Percutaneous Microwave Ablation for Liver Tumors Abutting Special Locations under B Ultrasound Guidance

Abstract:Objective To evaluate the efficacy and safety of percutaneous microwave ablation(PMB) for liver tumor abutting special locations such as gallbladder,diaphragm,heart,hepatic port,great vessels(with the shortest distance of tumor from the organ ≤ 0.5 cm) under B ultrasound guidance.Methods Twenty-one patients with advanced liver tumors abutting special locations received PMB were enrolled and served as treatment group.Meanwhile,comparative studies were also performed using controls(20 cases with advanced liver tumors abutting common locations) by stratified random selection.All the patients followed ultrasound(or contrast-enhanced ultrasound) and enhanced CT scan or MRI before and after the treatment,and underwent alpha-fetoprotein(AFP) detection.Adverse reactions and complications in the two groups were observed.Results According to the results of theCT scan after 1 month,there were 19 cases with complete tumor necrosis in the treatment group and 18 in the control group,and necrosis rates was 90.5%(19 /21) and 90.0%(18 /20),respectively.The negative rates of AFP were all 78.6%(11 /14) in the two groups after 1 month treatment.The whole survival rates of years 0.5,1,2,3 were 100.0%(21 /21),95.2%(20 /21),81.0%(17 /21),71.4%(15 /21) in the treatment group and 100.0%(20 /20),95.0%(19 /20),90.0%(18 /20),80.0%(16 /20) in the controls,respectively.Conclusion The efficacy of a proper PMB treatment for the liver tumors adjacent to special locations is closed to that of PMB for common locations.
Microwave Ablation Versus Nipple Sparing Mastectomy for Breast Cancer ≤5 cm: A Pilot Cohort Study

Abstract:Objectives: Compared with nipple sparing mastectomy (NSM), microwave ablation (MWA) is one relatively new modality indicated for selected breast cancer with nipple sparing and with little of evidence-based medical research for decision-making. The objective of this study was to compare the effect of ultrasound-guided percutaneous MWA and NSM for breast cancer.Materials and Methods: A retrospective cohort study was conducted in a single institution from 2014 to 2020. Women with invasive ductal carcinoma of the breast ≤ 5cm treated by MWA or NSM were enrolled. The primary end point was tumor progression and secondary end points included survival, cosmetic results, and complications.Results: 21 patients in the MWA group and 43 in the NSM group were evaluated. The mean tumor size was 2.3 cm (range, 0.3–5.0 cm). Median follow-up was 26.7 months (range, 14.6–62.5 months). The mean age of MWA was 24 years older than that of the NSM group. All the patients achieved technique effectiveness. One local tumor progression and one ipsilateral breast recurrence occurred at 42 and 28 months after MWA, respectively. One ipsilateral breast recurrence and two bone metastasis occurred at 31.2, 34, and 30.5 months after NSM. Two groups had no significant difference in tumor progression (P = 0.16). No participants in both groups developed cancer related death (P > 0.99) and major complications (P > 0.99). However, MWA needed less hospitalization time (P < 0.001) and achieved better cosmetic results (P < 0.001).Conclusions: MWA achieved similar short term effect for breast cancer control and better cosmetic satisfaction compared with NSM in selected patients. MWA provides appropriate option for elderly patients who are unfit for surgery.

Abstract:Objective: This study aims to assess the feasibility, effectiveness, and safety of image-guided percutaneous microwave ablation (PMWA) for unresectable pancreatic cancer. Methods: In this retrospective study, 72 patients from four hospitals were enrolled between November 2009 and October 2022. Descriptive statistics were employed to describe the patients' characteristics and prognostic factors. The primary endpoint compassed the complete ablation rate (CAR), incidence of complications and the pain relief rate (PRR). Results: The median age of the 72 patients was 61 (interquartile range (IQR) 52.5-67.0) years, with 62.5 % (45/72) being male. 26 cases received computed tomography (CT) guidance; 46 cases received ultrasound guidance. A total of 74 tumors were identified (2 in 2 patients), with 56.8 % (42/74) at the body and tail, and the rest at the head and neck. Overall, 73 ablation sessions were carried out, achieving a technical success rate (TSR) of 100 %. The CAR was 40.5 % (30/74). The median follow-up time was 4.6 (1-43.4) months. 50 % (36/72) of patients had died with a median overall survival (OS) of 5.6 (1-27) months. Regarding complications, 18.1 % (13/72) of cases were classified as grade I and II, and 9.8 % (7/72) as grade IIIa. Before surgery, 33 patients experienced pain symptoms, and the postoperative PRR was 96.7 % (32/33). The average pain score decreased from 6.3 (4-10) before surgery to 2.0 (0-8) after ablation (P<0.001). Conclusions: Image-guided PMWA for unresectable pancreatic cancer is safe and feasible, effectively relieving cancer pain and improving patients' the quality of life.

Abstract:With a gradual increase in breast cancer incidence and mortality rates and an urgent need to improve patient prognosis and cosmetology, magnetic resonance imaging (MRI)-guided radiofrequency ablation (RFA) therapy has attracted wide attention as a new treatment method for breast cancer. MRI-RFA results in a higher complete ablation rate and extremely low recurrence and complication rates. Thus, it may be used as an independent treatment for breast cancer or adjuvant to breast-conserving surgery to reduce the extent of breast resection. Furthermore, with MRI guidance, accurate control of RFA can be achieved, and breast cancer treatment can enter a new stage of minimally invasive, safe, and comprehensive therapy. With progress in MR thermometry technology, the applications of MRI are expected to broaden.

A Preliminary Study on the Microwave Ablation of Multifocal Papillary Thyroid Microcarcinoma

Abstract:Rationale and Objectives To evaluate the feasibility, efficiency, and safety of microwave ablation (MWA) for multifocal papillary thyroid microcarcinoma (PTMC). Methods This was a retrospective study, and the data of patients who underwent MWA for multifocal PTMC from October 2016 to December 2021 were reviewed. After ablation, the changes in tumor size and volume, as well as the rates of technical success, tumor disappearance, disease progression, and complications, were assessed. According to the tumor location, the cases were further divided into a unilateral multifocal disease (UMD) subgroup and a bilateral multifocal disease (BMD) subgroup. Further analyses were carried out. Results There was a total of 94 cases enrolled in the present study, which included 24 males and 70 females. The median age was 40 years (22–66 years); the median follow-up time was 14 months (6–48 months). Complete ablation was achieved in all enrolled cases. Therefore, the technical success rate was 100%. Due to expanding ablation, the MD and volume of the ablation zone increased at the 1st and 3rd months after ablation and decreased from the 12th month after ablation (p < 0.05 for all). The total complete tumor disappearance rates were 45/94 (47.87%) overall, 40.625% (13/32) in the UMD subgroup and 51.61% (32/62) in the BMD subgroup (p = 0.312). The total disease progression rates were 4.26% (4/94) overall, 6.25% (2/32) in the UMD subgroup and 3.23% (2/62) in the BMD subgroup (p = 0.881). The overall complication rate was 4.26% (4/94). Conclusion This preliminary study indicates that MWA is a safe and effective treatment for multifocal PTMC.

Abstract:When microwave hyperthermia (MWH) array antenna technology is used to treat breast cancer, how to effectively target and heat deep tumors and reduce thermal damage to healthy tissues is still a challenge in clinical application. In this study, the synergistic MWH effect of Conformal-Array antenna (CAA) and novel Microwave-Thermal-Sensitive nanomaterial (MTSN) was investigated for the treatment of subcutaneous deep in situ breast cancer. At the beginning of the study, the thermal damage score was used to evaluate the therapeutic efficacy of CAA. It was found that although array antenna technology can achieve effective heating of deep tumors, its damage to healthy tissues is unacceptable. Consequently, we developed a novel MTSN, ZIF-8@HA, whose unique structure significantly enhanced the absorption of MW energy and MW thermal conversion efficiency in the local tumor region. The MW thermal conversion efficiency of ZIF-8@HA achieved a high of 46.46% in vivo MW heating experiments. In the phantom that simulates the electromagnetic environment of the human body, the microwave-thermal sensitization (MTS) effect is also significant, and the reduction in the average thermal damage score of healthy tissue by more than 10% was verified through measurements using the coaxial probe method and COMSOL simulations. Cellular experiments confirmed that the combination of ZIF-8@HA and MW irradiation could significantly reduce the survival rate of tumor cells. In addition, cross-tissue MW heating experiments revealed the advantages of ZIF-8 @ HA combined with CAA. Finally, phantom experiments confirmed that the synergistic use of CAA with ZIF-8@HA significantly accelerated the local heating rate of deep tumors, reduced the time required for the tumor region to achieve 100% thermal damage, and effectively minimized thermal damage to healthy tissues.

Abstract:When microwave hyperthermia (MWH) array antenna technology is used to treat breast cancer, how to effectively target and heat deep tumors and reduce thermal damage to healthy tissues is still a challenge in clinical applications. In this study, the synergistic MWH effect of conformal-array antennas (CAA) and a novel microwave-thermal-sensitive nanomaterial (MTSN) was investigated for the treatment of subcutaneous deep in situ breast cancer. At the beginning of the study, the thermal damage score was used to evaluate the therapeutic efficacy of the CAA. It was found that although array antenna technology can achieve effective heating of deep tumors, its damage to healthy tissues is unacceptable. Consequently, we developed a novel MTSN, ZIF-8@HA, whose unique structure significantly enhanced the absorption of MW energy and MW thermal conversion efficiency in the local tumor region. The MW thermal conversion efficiency of ZIF-8@HA achieved was as high as 46.46% in in vivo MW heating experiments. In the phantom that simulates the electromagnetic environment of the human body, the microwave-thermal sensitization (MTS) effect is also significant, and the reduction in the average thermal damage score of healthy tissues by more than 10% was verified through measurements using the coaxial probe method and COMSOL simulations. Cellular experiments confirmed that the combination of ZIF-8@HA and MW irradiation could significantly reduce the survival rate of tumor cells. In addition, cross-tissue MW heating experiments revealed the advantages of ZIF-8@HA combined with the CAA. Finally, phantom experiments confirmed that the synergistic use of the CAA with ZIF-8@HA significantly accelerated the local heating rate of deep tumors, reduced the time required for the tumor region to achieve 100% thermal damage, and effectively minimized the thermal damage to healthy tissues.

Preliminary Study of Microwave Ablation for Multifocal Papillary Thyroid Microcarcinoma in Nonoperative Candidates

Abstract:PURPOSE: To study the feasibility, safety, and effectiveness of microwave ablation (MWA) in patients with multifocal papillary thyroid microcarcinoma (PTMC).MATERIALS AND METHODS: This retrospective study included patients who underwent MWA for multifocal PTMC (number of nodules ≤3). A total of 44 patients were included, and the mean age was 43 years (SD ± 11). After ablation, progression-free survival (PFS) at 6, 12, 24, 36, and 48 months; disease progression; change in tumor size and volume; tumor disappearance rate; and adverse events (AEs) were assessed, and the feasibility, safety, and effectiveness of MWA for PTMC were evaluated on the basis of statistical analysis.RESULTS: The median follow-up period was 18 months (interquartile range, 12-33 months). The PFS rates at 6, 12, 24, 36, and 48 months were 100.0%, 96.4%, 96.4%, 70.3%, and 52.7%, respectively. The disease progression rate was 11.4% (5 of 44 patients). The maximum diameter (MD) and volume of the ablation zone were larger at the 3-month follow-up than before ablation (median MD, 13.0 vs 7.0 mm; P < .001; median volume, 503.8 vs 113.0 mm3; P < .001). Subsequently, the tumors exhibited a reduction in both size and volume after 18 months (median MD, 4.0 vs 7.0 mm; P = .04; median volume, 12.6 vs 113.0 mm3; P = .055). At the end of the follow-up period, the complete response rate was 59% (26 of 44 patients). The overall AE rate was 6.8%.CONCLUSIONS: MWA is a feasible treatment for PTMC (number of nodules ≤3), and this study preliminarily demonstrated the safety and effectiveness of this technique.

Abstract:Objective To investigate percutaneous removal of benign breast masses using Mammotome vacuum device with three-dimensional(3D) ultrasound guidance. Methods One hundred and sixty-one breast benign masses in 83 patients were removed using Mammotome vacuum device under 3D ultrasound guidance. The maximum diameter of the masses was 0.6-3.5 cm (mean 1.7±0.5 cm). Results All masses were completely and successfully removed. The cut-probe was clearly visualized under real-time 3D ultrasound guidance with accurate localization,but the 3D imaging was not demonstrated smoothly and good enough for real-time image. Follow-ups of 1-18 months showed no recurrence in all cases. Conclusions Real-time 3D ultrasound-guided Mammotome vacuum device is suitable for minimally invasive removal of benign breast lesions (the maximal longitudinal diameter was ≤3.5 cm),which is safe and effective without recurrence in short-time follow-ups. This method is also useful for operation planning,but real-time image should be improved.
