Abstract:Large language models (LLMs) like OpenAI's ChatGPT are powerful generative systems that rapidly synthesize natural language responses. Research on LLMs has revealed their potential and pitfalls, especially in clinical settings. However, the evolving landscape of LLM research in medicine has left several gaps regarding their evaluation, application, and evidence base.

What problem does this paper attempt to address?
The main problems that this paper attempts to solve are the current application status, major challenges and opportunities of large - language models (LLMs) in the clinical environment. Specifically, the research objectives include: 1. **Summarize current research evidence**: Evaluate the accuracy and effectiveness of LLMs in medical applications. 2. **Discuss ethical, legal, logistical and socio - economic impacts**: Explore the ethical, legal, logistical and socio - economic issues that may be brought about when LLMs are used in clinical settings. 3. **Explore implementation barriers and facilitators**: Analyze the barriers and facilitators to the implementation of LLMs in the healthcare environment. 4. **Propose a standardized evaluation framework**: Suggest a standardized evaluation framework for evaluating the clinical utility of LLMs in future research. 5. **Identify evidence gaps and propose future research directions**: Point out the evidence gaps in the current literature and propose directions for further research on LLMs in clinical applications in the future. Through these objectives, the paper aims to comprehensively understand the application potential of LLMs in the medical field and the challenges they face, and provide guidance and suggestions for future clinical practice.
