Abstract:In the rapidly evolving healthcare landscape, artificial intelligence (AI), particularly the large language models (LLMs), like OpenAI's Chat Generative Pretrained Transformer (ChatGPT), has shown transformative potential in emergency medicine and critical care. This review article highlights the advancement and applications of ChatGPT, from diagnostic assistance to clinical documentation and patient communication, demonstrating its ability to perform comparably to human professionals in medical examinations. ChatGPT could assist clinical decision-making and medication selection in critical care, showcasing its potential to optimize patient care management. However, integrating LLMs into healthcare raises legal, ethical, and privacy concerns, including data protection and the necessity for informed consent. Finally, we addressed the challenges related to the accuracy of LLMs, such as the risk of providing incorrect medical advice. These concerns underscore the importance of ongoing research and regulation to ensure their ethical and practical use in healthcare.

Abstract:Objectives: This review explores the potential applications of large language models (LLMs) such as ChatGPT, GPT-3.5, and GPT-4 in the medical field, aiming to encourage their prudent use, provide professional support, and develop accessible medical AI tools that adhere to healthcare standards. Methods: This paper examines the impact of technologies such as OpenAI's Generative Pre-trained Transformers (GPT) series, including GPT-3.5 and GPT-4, and other large language models (LLMs) in medical education, scientific research, clinical practice, and nursing. Specifically, it includes supporting curriculum design, acting as personalized learning assistants, creating standardized simulated patient scenarios in education; assisting with writing papers, data analysis, and optimizing experimental designs in scientific research; aiding in medical imaging analysis, decision-making, patient education, and communication in clinical practice; and reducing repetitive tasks, promoting personalized care and self-care, providing psychological support, and enhancing management efficiency in nursing. Results: LLMs, including ChatGPT, have demonstrated significant potential and effectiveness in the aforementioned areas, yet their deployment in healthcare settings is fraught with ethical complexities, potential lack of empathy, and risks of biased responses. Conclusion: Despite these challenges, significant medical advancements can be expected through the proper use of LLMs and appropriate policy guidance. Future research should focus on overcoming these barriers to ensure the effective and ethical application of LLMs in the medical field.

Abstract:Large language models (LLMs) are artificial intelligence (AI) tools specifically trained to process and generate text. LLMs attracted substantial public attention after OpenAI's ChatGPT was made publicly available in November 2022. LLMs can often answer questions, summarize, paraphrase and translate text on a level that is nearly indistinguishable from human capabilities. The possibility to actively interact with models like ChatGPT makes LLMs attractive tools in various fields, including medicine. While these models have the potential to democratize medical knowledge and facilitate access to healthcare, they could equally distribute misinformation and exacerbate scientific misconduct due to a lack of accountability and transparency. In this article, we provide a systematic and comprehensive overview of the potentials and limitations of LLMs in clinical practice, medical research and medical education.

Abstract:Since the release of ChatGPT and GPT-4, large language models (LLMs) and multimodal large language models (MLLMs) have garnered significant attention due to their powerful and general capabilities in understanding, reasoning, and generation, thereby offering new paradigms for the integration of artificial intelligence with medicine. This survey comprehensively overviews the development background and principles of LLMs and MLLMs, as well as explores their application scenarios, challenges, and future directions in medicine. Specifically, this survey begins by focusing on the paradigm shift, tracing the evolution from traditional models to LLMs and MLLMs, summarizing the model structures to provide detailed foundational knowledge. Subsequently, the survey details the entire process from constructing and evaluating to using LLMs and MLLMs with a clear logic. Following this, to emphasize the significant value of LLMs and MLLMs in healthcare, we survey and summarize 6 promising applications in healthcare. Finally, the survey discusses the challenges faced by medical LLMs and MLLMs and proposes a feasible approach and direction for the subsequent integration of artificial intelligence with medicine. Thus, this survey aims to provide researchers with a valuable and comprehensive reference guide from the perspectives of the background, principles, and clinical applications of LLMs and MLLMs.

Abstract:Large language models (LLMs), such as ChatGPT, have received substantial attention due to their capabilities for understanding and generating human language. While there has been a burgeoning trend in research focusing on the employment of LLMs in supporting different medical tasks (e.g., enhancing clinical diagnostics and providing medical education), a review of these efforts, particularly their development, practical applications, and outcomes in medicine, remains scarce. Therefore, this review aims to provide a detailed overview of the development and deployment of LLMs in medicine, including the challenges and opportunities they face. In terms of development, we provide a detailed introduction to the principles of existing medical LLMs, including their basic model structures, number of parameters, and sources and scales of data used for model development. It serves as a guide for practitioners in developing medical LLMs tailored to their specific needs. In terms of deployment, we offer a comparison of the performance of different LLMs across various medical tasks, and further compare them with state-of-the-art lightweight models, aiming to provide an understanding of the advantages and limitations of LLMs in medicine. Abstract:Large language models (LLMs), such as GPT series models, have received substantial attention due to their impressive capabilities for generating and understanding human-level language. More recently, LLMs have emerged as an innovative and powerful adjunct in the medical field, transforming traditional practices and heralding a new era of enhanced healthcare services. This survey provides a comprehensive overview of Medical Large Language Models (Med-LLMs), outlining their evolution from general to the medical-specific domain (i.e, Technology and Application), as well as their transformative impact on healthcare (e.g., Trustworthiness and Safety). Abstract:Large language models (LLMs) are increasingly recognized for their advanced language capabilities, offering significant assistance in diverse areas like medical communication, patient data optimization, and surgical planning. Our survey meticulously searched for papers with keywords such as "medical," "clinical," "healthcare," and "LLMs" across various databases, including ACM and Google Scholar. It sought to delve into the latest trends and applications of LLMs in healthcare, analyzing 175 relevant publications to support both practitioners and researchers in the field. We have compiled 56 experimental datasets, various evaluation methods and reviewed cutting-edge LLMs across tasks. Our comprehensive analysis of LLMs in healthcare applications, including medical question-answering, dialogue summarization, electronic health record generation, scientific research, medical education, medical product safety monitoring, clinical health reasoning, and clinical decision support. Furthermore, we have identified the challenges, including data security, inaccurate information, fairness and bias, plagiarism, copyrights, and accountability, and the potential solutions, namely de-identification framework, references,counterfactually fair prompting,opening and ending control codes, and establishing normative standards,to address these open issues,respectively. The findings of this survey exert a profound impact on spurring innovation in practical applications and addressing inherent challenges within the academic and medical communities.

Abstract:Large language models (LLMs) represent a transformative class of AI tools capable of revolutionizing various aspects of healthcare by generating human-like responses across diverse contexts and adapting to novel tasks following human instructions. Their potential application spans a broad range of medical tasks, such as clinical documentation, matching patients to clinical trials, and answering medical questions. In this primer paper, we propose an actionable guideline to help healthcare professionals more efficiently utilize LLMs in their work, along with a set of best practices. This approach consists of several main phases, including formulating the task, choosing LLMs, prompt engineering, fine-tuning, and deployment. We start with the discussion of critical considerations in identifying healthcare tasks that align with the core capabilities of LLMs and selecting models based on the selected task and data, performance requirements, and model interface. We then review the strategies, such as prompt engineering and fine-tuning, to adapt standard LLMs to specialized medical tasks. Abstract:Within the domain of Natural Language Processing (NLP), Large Language Models (LLMs) represent sophisticated models engineered to comprehend, generate, and manipulate text resembling human language on an extensive scale. They are transformer-based deep learning architectures, obtained through the scaling of model size, pretraining of corpora, and computational resources. The potential healthcare applications of these models primarily involve chatbots and interaction systems for clinical documentation management, and medical literature summarization (Biomedical NLP). The challenge in this field lies in the research for applications in diagnostic and clinical decision support, as well as patient triage. Therefore, LLMs can be used for multiple tasks within patient care, research, and education. Throughout 2023, there has been an escalation in the release of LLMs, some of which are applicable in the healthcare domain. This remarkable output is largely the effect of the customization of pre-trained models for applications like chatbots, virtual assistants, or any system requiring human-like conversational engagement. As healthcare professionals, we recognize the imperative to stay at the forefront of knowledge. However, keeping abreast of the rapid evolution of this technology is practically unattainable, and, above all, understanding its potential applications and limitations remains a subject of ongoing debate. Consequently, this article aims to provide a succinct overview of the recently released LLMs, emphasizing their potential use in the field of medicine. Perspectives for a more extensive range of safe and effective applications are also discussed. The upcoming evolutionary leap involves the transition from an AI-powered model primarily designed for answering medical questions to a more versatile and practical tool for healthcare providers such as generalist biomedical AI systems for multimodal-based calibrated decision-making processes. Abstract:Over the past year, the emergence of state-of-the-art large language models (LLMs) in tools like ChatGPT has ushered in a rapid acceleration in artificial intelligence (AI) innovation. These powerful AI models can generate tailored and high-quality text responses to instructions and questions without the need for labor-intensive task-specific training data or complex software engineering. As the technology continues to mature, LLMs hold immense potential for transforming clinical workflows, enhancing patient outcomes, improving medical education, and optimizing medical research. In this review, we provide a practical discussion of LLMs, tailored to gastroenterologists. We highlight the technical foundations of LLMs, emphasizing their key strengths and limitations as well as how to interact with them safely and effectively. We discuss some potential LLM use cases for clinical gastroenterology practice, education, and research. Finally, we review critical barriers to implementation and ongoing work to address these issues. This review aims to equip gastroenterologists with a foundational understanding of LLMs to facilitate a more active clinician role in the development and implementation of this rapidly emerging technology.

Abstract:The utilization of large language models (LLMs) in the Healthcare domain has generated both excitement and concern due to their ability to effectively respond to freetext queries with certain professional knowledge. This survey outlines the capabilities of the currently developed LLMs for Healthcare and explicates their development process, with the aim of providing an overview of the development roadmap from traditional Pretrained Language Models (PLMs) to LLMs. Specifically, we first explore the potential of LLMs to enhance the efficiency and effectiveness of various Healthcare applications highlighting both the strengths and limitations. Secondly, we conduct a comparison between the previous PLMs and the latest LLMs, as well as comparing various LLMs with each other. Then we summarize related Healthcare training data, training methods, optimization strategies, and usage. Finally, the unique concerns associated with deploying LLMs in Healthcare settings are investigated, particularly regarding fairness, accountability, transparency and ethics. Our survey provide a comprehensive investigation from perspectives of both computer science and Healthcare specialty. Besides the discussion about Healthcare concerns, we supports the computer science community by compiling a collection of open source resources, such as accessible datasets, the latest methodologies, code implementations, and evaluation benchmarks in the Github. Summarily, we contend that a significant paradigm shift is underway, transitioning from PLMs to LLMs. This shift encompasses a move from discriminative AI approaches to generative AI approaches, as well as a shift from model-centered methodologies to data-centered methodologies. Also, we determine that the biggest obstacle of using LLMs in Healthcare are fairness, accountability, transparency and ethics.

Abstract:Objective: Large language models (LLMs) like ChatGPT are powerful algorithms that have been shown to produce human-like text from input data. A number of potential clinical applications of this technology have been proposed and evaluated by biomedical informatics experts. However, few have surveyed healthcare providers for their opinions about whether the technology is fit for use. Materials and Methods: We distributed a validated mixed-methods survey to gauge practicing clinicians’ comfort with LLMs for a breadth of tasks in clinical practice, research and education, which were selected from the literature. Results: A total of 30 clinicians fully completed the survey. Of the 23 tasks, 16 were rated positively by more than 50% of the respondents. Based on our qualitative analysis, healthcare providers considered LLMs to have excellent synthesis skills and efficiency. However, our respondents had concerns that LLMs could generate false information and propagate training data bias. Discussion: Our survey respondents were most comfortable with scenarios that allow LLMs to function in an assistive role, like a physician extender or trainee. Conclusion: In a uniquely rigorous and comprehensive mixed-methods survey of clinicians about LLM use, healthcare providers were encouraging of having LLMs in healthcare for many tasks, and especially in assistive roles. There is a need for continued human-centered development of both LLMs and artificial intelligence (AI) in general.

Abstract:Recent years have witnessed notable progress in Language Model (LM) technology, especially with the introduction of Large Language Models (LLMs). With the use of complex neural networks and enormous volumes of data, these models—like the GPT (Generative Pre-trained Transformer) series—have completely changed the field of natural language processing. This study offers a thorough analysis of LLMs, examining their uses as well as the difficulties and moral issues that come with implementing LLMs in practical settings. Moreover, it investigates the wide range of uses of LLMs in many fields, such as question answering, drug discovery, language translation, and text production. By means of this review, we hope to offer ideas on future directions in this quickly developing field as well as insights into the state-of-the-art in LLM research today. Abstract:This study systematically reviewed the application of large language models (LLMs) in medicine, analyzing 550 selected studies from a vast literature search. LLMs like ChatGPT transformed healthcare by enhancing diagnostics, medical writing, education, and project management. They assisted in drafting medical documents, creating training simulations, and streamlining research processes. Despite their growing utility in assisted diagnosis and improving doctor-patient communication, challenges persisted, including limitations in contextual understanding and the risk of over-reliance. The surge in LLM-related research indicated a focus on medical writing, diagnostics, and patient communication, but highlighted the need for careful integration, considering validation, ethical concerns, and the balance with traditional medical practice. Future research directions suggested a focus on multimodal LLMs, deeper algorithmic understanding, and ensuring responsible, effective use in healthcare.
