Abstract:Objective: To investigate the risk factors influencing the postoperative outcome of arthroscopic rotator cuff repair (ARCR) and develop a nomogram prediction model. Methods: A retrospective study was conducted on 302 patients who underwent ARCR from January 2019 to August 2023. Patients were categorized into two groups: a control group with 150 patients showing good recovery and an observation group with 152 patients exhibiting poor recovery. Relevant clinical data were collected and statistically analyzed. A nomogram model was constructed based on the results of multivariate logistic regression analysis. The model's accuracy, discrimination, and clinical utility were evaluated using calibration charts, AUC, c-index, and decision curve analysis. Internal validation was performed through self-random sampling. Results: Univariate and multivariate regression analysis identified having a frozen shoulder, large rotator cuff tear, increased intraoperative rivet use, diabetes, and traumatic tear as predictive risk factors for poor postoperative outcomes. These factors were utilized to develop a clinical predictive nomogram. The nomogram model demonstrated excellent predictive accuracy for poor postoperative outcomes, both internally and externally. The unadjusted concordance index (C-index) was 0.793 [95% confidence interval (CI), 0.825-0.995]. The AUC for the nomogram was 0.788. Decision curve analysis revealed that the predictive model was clinically useful when the threshold probability ranged from 20 to 60%. Conclusion: The presence of a frozen shoulder, large rotator cuff tear, increased intraoperative rivet use, diabetes, and traumatic tear elevate the risk of suboptimal outcomes following ARCR. Conversely, having a higher preoperative University of California at Los Angeles Shoulder Rating Scale score mitigates this risk. This study introduces a novel nomogram model, exhibiting relatively high accuracy, which enables clinicians to precisely assess the postoperative adverse risk among patients with rotator cuff injuries requiring arthroscopic repair at the outset of treatment.
Arthroscopic Single-Row Versus Double-Row Technique for Repairing Rotator Cuff Tears: a Systematic Review and Meta-Analysis.

Abstract:OBJECTIVE:The purpose of this study was to systematically review published reports that compare the outcomes of single-row and double-row suture anchor fixation in arthroscopic rotator cuff repair. METHODS:Combined data regarding relevant patient characteristics, rotator cuff pathology, surgical techniques, postoperative rehabilitation regimens, University of California at Los Angeles (UCLA) Scores, Constant scores, American Shoulder and Elbow Society (ASES) scores, tendon healing, incidence of recurrent rotator cuff defects, shoulder muscle strength, range of motion, surgical time and patient satisfaction were assessed. RESULTS:Seven eligible randomized controlled studies and four prospective cohort studies were identified. There were no significant differences between the single-row and double-row groups in terms of Constant or ASES scores. However, UCLA scores, tendon healing, abduction shoulder strength index (SSI), external rotation SSI and internal rotation SSI were significantly better for double-row technique than for single-row technique. A statistically significant advantage for double-row repair according to UCLA score and external rotation SSI was found in the subgroup with tears greater than 3 cm. CONCLUSION:No definite conclusion could be drawn about differences in overall outcomes of double- and single-row techniques for repairing for small to medium (<3 cm) or large to massive (>3 cm) rotator cuff tears, even though some measures of clinical outcome showed significant differences between these two techniques.

Abstract:Background:Rotator cuff injury is the most common clinical shoulder joint disease, accounting for 50%-85% of all shoulder joint diseases. At present, arthroscopic rotator cuff repair has replaced the traditional surgical plan of incision and suture, becoming the first choice of surgeons. Its effectiveness and safety have been recognized in the industry. However, there are also occasional cases with poor postoperative effects and insufficient pain relief.Objective:To analyze the prognostic factors of arthroscopy rotator cuff repair.Methods:From February 2019 to February 2020, the primary data of 117 patients who received arthroscopic rotator cuff repair in the department of orthopedics of tenth people's hospital of Tongji university were retrospectively analyzed, and the surgical efficacy was followed up for one year after surgery.Results:There were 95 patients with a good prognosis, accounting for 81.20%. Multivariate analysis showed that age, preoperative course of the disease, degree of tear, and the use of platelet-rich plasma (PRP) were independent factors affecting prognosis.Conclusion:The age, preoperative course of the disease, rotator cuff tear degree, use of PRP during operation are important factors affecting the prognosis of arthroscopy rotator cuff repair.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Retear after arthroscopic rotator cuff repair (ARCR) consistently challenges medical staff and patients, and the incidence of retear after surgery is 10-94%. The purpose of this study was to identify the risk factors that cause retear after ARCR and provide theoretical guidance for clinical intervention to reduce the occurrence of postoperative rotator cuff retear.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Study Design</h3><p>Systematic review and meta-analysis.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The protocol for this meta-analysis was registered with PROSPERO (CRD42021225088). PubMed, Web of Science and Embase were searched for observational studies on risk factors for rotator cuff retear after arthroscopic repair. Meta-analytical methods were used to determine the odds ratio (OR) or weighted mean difference (WMD) of potential risk factors related to postoperative rotator cuff retear. Stata 15.1 was used to quantitatively evaluate the publication bias of the statistical results.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Fourteen studies from 6 countries with a total of 5693 patients were included. The meta-analysis revealed that the risk factors for retear after rotator cuff repair were age, body mass index (BMI), diabetes, subscapularis and infraspinatus fatty infiltration, symptom duration, bone mineral density, tear length, tear width, tear size area, amount of retraction, critical shoulder angle (CSA), acromiohumeral interval (AI), distance from the musculotendinous junction to the glenoid, operative duration, biceps procedure, and postoperative University of California Los Angeles (UCLA) score.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>These findings can help clinical medical staff identify patients who are prone to retear early after arthroscopic repair and develop targeted prevention and treatment strategies for modifiable risk factors, which are of great significance for reducing the occurrence of rotator cuff retear after ARCR.</p>

Risk Factors Analysis and Prediction of Rotator Cuff Tears: A Retrospective Study

Abstract:Aim: Rotator cuff tears (RCTs) are a major cause of shoulder pain and disability, affecting millions worldwide. Understanding the risk factors and developing reliable predictive measures for RCTs is essential for early diagnosis, targeted prevention, and effective treatment of this patient population. This study seeks to enhance our understanding by analyzing the acromiohumeral distance (AHD) and Constant-Murley Score (CMS) in patients with and without RCTs, thereby aiding the development of a predictive model aimed at improving clinical outcomes and prevention strategies in rotator cuff pathology. Methods: This retrospective analysis involved 201 patients with shoulder pain, categorized into RCT (n = 72) and no RCTs (N-RCTs, n = 129) groups based on Magnetic Resonance Imaging (MRI) findings. We compared demographics, AHD, CMS, and rotator cuff status between groups and utilized logistic regression for identifying RCT predictors, leading to the development of a multifactorial predictive model. Results: The mean AHD was 6.60 ± 1.12 mm. The RCT group showed a marginally higher AHD than the N-RCT group (p = 0.669). CMS scores were significantly lower in the RCT group (p < 0.001). Dominant side involvement (Odds Ratio (OR) 2.244), type III acromion (OR 6.106), and lower CMS (OR 0.938) significantly correlated with RCTs. The predictive model demonstrated an area under the curve (AUC) of 0.701 for RCT diagnosis. Conclusions: Reduced CMS, dominance of the affected side, and type III acromion emerged as key risk factors for RCTs. Our predictive model, incorporating these factors, holds promise for RCT diagnosis, with future studies needed for further validation.

Abstract:Objective: Rotator cuff tear is a common shoulder injury that often leads to serious limitations in daily life. Herein, a network Meta-analysis using frequency theory was performed to evaluate the clinical outcomes of five rotator cuff repair techniques, including single-row repair, double-row repair, suture bridge repair, platelet-rich plasma therapy, and bone marrow stimulation, thus guiding clinical decision-making on rotator cuff repair. Methods: PubMed, EMbase, The Cochrane Library, and Web of Science were searched for randomized controlled trials and cohort studies comparing rotator cuff repair techniques published from inception to May 2022. Combined analysis and quality assessment were performed using software STATA15.1 and Review Manager5.3. Results: A total of 51 articles were finally included, including 27 randomized controlled trials and 24 cohort studies. Results from the network Meta-analysis showed that: (1) In terms of the American Shoulder and Elbow Surgeons score, platelet-rich plasma therapy, double-row repair, bone marrow stimulation, and single-row repair were significantly better than suture bridge repair. (2) In terms of Constant score, bone marrow stimulation was significantly better than double-row repair, single-row repair, and suture bridge repair. (3) In terms of visual analog scale score, platelet-rich plasma therapy was significantly better than double-row repair and suture bridge repair. (4) In terms of the Shoulder Rating Scale of the University of California at Los Angeles score, platelet-rich plasma therapy and double-row repair were relatively better but not significantly different from the other treatments. (5) In terms of the risk of re-tear, the re-tear rate of platelet-rich plasma therapy and double-row repair was significantly lower than that of single-row repair and suture bridge repair. Conclusion: Based on the results of network Meta-analysis and surface under the cumulative ranking, platelet-rich plasma therapy, bone marrow stimulation, and double-row repair have good overall rehabilitation effects. It is recommended to choose appropriate repair techniques as per the actual clinical situation.

Abstract:Objective To evaluate the early clinical results after arthroscopic rotator cuff repair using a suture-bridge technique for patients with full-thickness rotator cuff tears.Methods A retrospective analysis of 44 patients(22 males;22 females;mean age 52.3 years) with full-thickness rotator cuff tears,who were treated consecutively between 2010 and 2011 at our clinic,was carried out to evaluate the outcome of arthroscopic suture-bridge technique.The visual analog pain scale scores(VAS),the range of motion(ROM),the rating scale of the University of California at Los Angeles(UCLA),and the shoulder index of the American Shoulder and Elbow Surgeons(ASES) were determined as the evaluation criteria.Results The mean follow-up period was 9.7 months.The follow-up rate was 100%.The study included 14(31.8%) small,21(47.7%) medium,five(11.4%) large and four(9.1%) massive rotator cuff tears.At the last follow-up,the VAS score decreased significantly from preoperative(6.4±3.4) to(0.7±1.5) at rest,and from preoperative(7.9±2.1) to(1.8±1.8) during motion.The ROM improved significantly.The atisfactory rate was 93%.The UCLA score improved significantly from(15.3±4.9) preoperatively to(30.7±4.0) at follow-up.The ASES shoulder index improved significantly from(30.5±17.8) preoperatively to(84.2±12.6) at follow-up.There were no complications.Conclusions The early results of this study indicate that the arthroscopic suture-bridge technique is an effective and reliable treatment for patients with full-thickness rotator cuff tears.The long-term outcomes remain to be determined.

Abstract:Purpose: To use a finite element method to construct a patch-bridge repair model for massive rotator cuff tears (MRCTs) and investigate the effects of different suture methods and knot numbers on postoperative biomechanics. Methods: A finite element model based on intact glenohumeral joint data was used for a biomechanical study. A full-thickness defect and retraction model of the supraspinatus tendon simulated MRCTs. Patch, suture, and anchor models were constructed, and the Marlow method was used to assign the material properties. Three suturing models were established: 1-knot simple, 1-knot mattress, and 2-knot mattress. The ultimate failure load, failure mode, stress distribution of each structure, and other biomechanical results of the different models were calculated and compared. Results: The ultimate failure load of the 1-knot mattress suture (71.3 N) was 5.6 % greater than that of the 1-knot simple suture (67.5 N), while that (81.5 N) of the 2-knot mattress was 14.3 % greater than that of the 1-knot mattress. The stress distribution on the patch and supraspinatus tendon was concentrated on suture perforation. Failure of the bridging reconstruction mainly occurred at the suture perforation of the patch, and the damage forms included cutting-through and isthmus pull-out. Conclusion: A finite element model for the patch-bridging reconstruction of MRCTs was established, and patch-bridging restored the mechanical integrity of the rotator cuff. The 2-knot mattress suture was optimal for patch-bridging reconstruction of MRCTs.

Abstract:A BSTRACT Background: The cuff is a complex structure with many factors affecting it. How much it is affected after repair is still being investigated. Aim: The aim of this study was to analyze the functional results of individuals who received arthroscopic rotator cuff repair and assess the various factors that could impact these outcomes. Methods: The study included 57 patients with a mean age of 58.8 years (range: 39–71) who underwent arthroscopic rotator cuff repair between 2013 and 2020, with a minimum of 6 months after the operation. Functional and clinical outcomes of the patients were evaluated using preoperative and postoperative scores (QDASH, ASES, and VAS). The study also analyzed how demographic factors, type and duration of the tear, comorbidities such as diabetes mellitus (DM) and hypertension (HT), and other pathologies may have affected the patients’ scores. Results: The effect of the operation on the scores in rotator cuff tears was highly significant ( P < 0.05). Age, gender, tear size, fatty degeneration, anchor configuration, comorbidities, and additional procedures presented variable values on postoperative scores, but were not statistically significant. The effect of DM and HT on VAS scores was statistically significant ( P < 0.05). Conclusion: The results of rotator cuff repair appear to be influenced by several factors, including the sex of the patient, type and duration of tear, comorbidities, and surgical procedures used. Although these factors had an effect on the scores, they were not statistically significant.

Abstract:Intertrochanteric femoral fractures are also known as "last fractures." [ 1 ] As the life expectancy of the population increases and the population ages, the number of intertrochanteric femoral fractures that occur each year increases significantly. [ 2–4 ] The current treatment options for intertrochanteric fractures of the femur can be generally categorized into 2 treatment modalities: surgical treatment and conservative treatment. However, conservative treatment has many postoperative complications, [ 5 , 6 ] and intertrochanteric femoral fractures are generally treated surgically. Proximal femoral anti-spin intramedullary nailing (PFNA) is the most mainstream treatment for intertrochanteric fractures, and the main nail can perfectly match the anatomical shape of the femur, and the design is in accordance with human mechanics. [ 7 , 8 ] Although PFNA has good efficacy in the treatment of intertrochanteric fractures, there are some postoperative complications, such as postoperative lung infection, [ 9 ] postoperative deep vein thrombosis in the lower limbs, [ 10 ] and especially postoperative internal fixation failure, which is catastrophic for patients. [ 11 ] There are few studies on the risk factors for postoperative internal fixation failure in intertrochanteric femoral fractures and the findings are inconsistent. [ 12 , 13 ] The nomogram has been used in many fields, not only for predicting lower extremity deep vein thrombosis after hip fracture, [ 14 ] but also for predicting postfracture pneumonia [ 15 ] and postfracture delirium, [ 16 ] and all of them have achieved good predictive results. The aim of the present study was to investigate the independent risk factors for postoperative internal fixation failure in intertrochanteric femur fractures and to establish a nomogram prediction model accordingly.

Abstract:Abstract Background: Single-row suture anchor technique is a well-established, economical, and relatively easier method of arthroscopic rotator cuff repair that has been recently tripped over by double-row technique which is proven to be biomechanically superior. However, this advantage is not reflected in the functional outcome scores which are reported to be identical with both single- and double-row techniques. Materials and Methods: A prospective cohort study was performed on 67 patients with degenerative rotator cuff tear, diagnosis was confirmed on radiological magnetic resonance imaging, treated surgically by single-row suture anchor method, and evaluated for functional outcome at minimum 1-year follow-up. Pre- and postoperative Visual Analog Scale (VAS) and American Shoulder and Elbow Surgeons (ASES) scores were calculated at initial evaluation and the end of 1-year follow-up. Results: Eighty-six percentage of the patients showed a clinically significant improvement in VAS and ASES scores. Conclusion: Consistent acceptable success rates in terms of outcome functional scores, including patient reported outcomes, depict single-row suture anchor technique as a reliable option for the management of degenerative rotator cuff tears 3 cm or less.

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Repair of rotator cuff tear is not always feasible, depending on the severity. Although several studies have investigated factors related to reparability and various methods to predict it, inconsistent scoring methods and a lack of validation have hindered the utility of these methods.Purpose:To develop machine learning models to predict the reparability of rotator cuff tears, compare them with previous scoring systems, and provide an accessible online model.Study Design:Cohort study; Level of evidence, 3.Methods:Arthroscopic rotator cuff repairs for tears with both anteroposterior and mediolateral diameters >1 cm on preoperative magnetic resonance imaging were included and divided into a training set (70%) and an internal validation set (30%). For external validation, rotator cuff repairs performed by 2 different surgeons were included in a test set. Machine learning models and a newly adjusted scoring system were developed using the training set. The performance of the models including the adjusted scoring system and 2 previous scoring systems were compared using the test set. The performance was assessed using metrics such as the area under the receiver operating characteristic curve (AUROC) and compared using the net reclassification improvement based on the adjusted scoring system.Results:A total of 429 patients were included for the training and internal validation set, and 112 patients were included for the test set. An elastic-net logistic regression demonstrated the best performance, with an AUROC of 0.847 and net reclassification improvement of 0.071, compared with the adjusted scoring system in the test set. The AUROC of the adjusted scoring system was 0.786, and the AUROCs of the previous scoring systems were 0.757 and 0.687. The elastic-net logistic regression was transformed into an accessible online model.Conclusion:The performance of the machine learning model, which provides a probability estimation for rotator cuff reparability, is comparable with that of the adjusted scoring system. Nevertheless, when deploying prediction models beyond the original cohort, regardless of whether they rely on machine learning or scoring systems, clinicians should exercise caution and not rely solely on the output of the model.

This study aims to use meta-analysis to compare the clinical and anatomical outcomes between the two techniques.Methods: The Pubmed, Embase, and Cochrane library databases were searched for relevant studies published before November 1, 2012. Studies clearly reporting a comparison of the single-row and double-row techniques were selected. The Constant, ASES, and UCLA scale systems and the rotator cuff integrity rate were evaluated. The weighted mean differences and relative risks were calculated using a fixed-effects or random-effects model.Results: Eight studies were included in this meta-analysis. The weighted mean differences of the ASES (-0.84; P = 0.04; I-2 = 0%) and UCLA (-0.75; P = 0.007; I-2 = 0%) scales were significantly low in the single-row group for full-thickness rotator cuff tears. For tear sizes smaller than 3 cm, no significant difference was found between the groups no matter in Constant (P = 0.95; I-2 = 0%), ASES (P = 0.77; I-2 = 0%), or UCLA (P = 0.24; I-2 = 13%) scales. For tear sizes larger than 3 cm, the ASES (-1.95; P = 0.001; I-2 = 49%) and UCLA (-1.17; P = 0.006; I-2 = 0%) scales were markedly lower in the single-row group. The integrity of the rotator cuff (0.81; P = 0.0004; I-2 = 10%) was greater and the partial thickness retear rate (1.93; P = 0.007; I-2 = 10%) was less in the double-row group. Full-thickness retears showed no difference between the groups (P = 0.15; I-2 = 0%).Conclusion: The meta-analysis suggests that the double-row fixation technique increases post-operative rotator cuff integrity and improves the clinical outcomes, especially for full-thickness rotator cuff tears larger than 3 cm. For tear sizes smaller than 3 cm, there was no difference in the clinical outcomes between the two techniques.

Abstract:Few studies have focused on the risk factors leading to postoperative blood transfusion after open reduction and internal fixation (ORIF) of proximal humeral fractures (PHFs) in the elderly. Therefore, we designed this study to explore potential risk factors of blood transfusion after ORIF for PHFs. We have also established a nomogram model to integrate and quantify our research results and give feedback.

Clinical Problems of Structural Failure after Rotator Cuff Repairs

Abstract:Arthroscopic rotator cuff repairs are preformed due to growing knowledge of rotator cuff tears among surgeons as well as patients.However,it is unavoidable and may sometimes very common that some patients develop structural failures or retears after surgical repairs because of unique blood supply and healing procedure of the rotator cuff,on which there is still much debate focusing on problems that one needs to deal with in clinical practice.Retears often occur,if without new trauma,during early postoperative period,or more specifically within the first 6 months after repair and are more frequently observed in those with old ages,diabetes or osteoporosis.In addition,risk factors of retears consist of longer preoperative duration of symptoms,larger primitive tear sizes,higher grades of fatty infiltration of muscles on preoperative imaging examinations,excessive tension within reconstructed structures,deteriorated quality of rotator cuff tissues and special shapes of the acromion,etc.Diagnosis is recently based on results of postoperative magnetic resonance imaging or echogram examination.MR/CT arthrography is also used.Each instrument has its advantages and limitations.However,the lack of a golden standard when establishing a diagnosis of rotator cuff retears makes integrating evaluations and comparisons of different methods a difficult question.It has been reported by many that the onset of retear leads to obvious shoulder pain or weakness,whereas more than a few studies demonstrated that very limited influences of retears in spite of generally unaffected shoulder functions,like decreased muscle strength on certain motions.The disagreement is possibly caused by specific methods of diagnosis,function instruments and time point of assessments used by different researchers as studies have displayed that feelings of patients after surgery and results attained by objective examinations are not always parallel.Moreover,some long-time follow-ups studies indicated that the impact posted by retears on shoulder functions evolves over a period of time.Owing to the varied opinions on actual symptoms and final functional outcomes caused by structural failure,the mainstay of treatment for rotator cuff tears are still conservative protocols,including observation and symptoms relief instead of second surgery.Second surgery could be an effective choice for certain patients,especially for those who have apparent symptoms.
Prognostic factors affecting structural integrity after arthroscopic rotator cuff repair: a clinical and histological study

Abstract:Background: Tendon degeneration contributes to rotator cuff tears; however, its role in postoperative structural integrity is poorly understood. The purpose of this study was to investigate the factors associated with postoperative structural integrity after rotator cuff repair, particularly focusing on the histology of tendons harvested intraoperatively. Methods: A total of 56 patients who underwent primary arthroscopic rotator cuff repair between 2009 and 2011 were analyzed. A 3-mm-diameter sample of supraspinatus tendons was harvested en bloc from each patient after minimal debridement of the torn ends. Tendon degeneration was assessed using seven histological parameters on a semi-quantitative grading scale, and the total degeneration score was calculated. One-year postoperative magnetic resonance imaging was used to classify the patients based on retear. Results: The total degeneration scores in the healed and retear groups were 13.93±2.03 and 14.08±2.23 (P=0.960), respectively. Arthroscopically measured anteroposterior (AP) tear sizes in the healed and retear groups were 24.30±12.35 mm and 36.42±25.23 mm (P=0.026), respectively. Preoperative visual analog scale pain scores at rest in the healed and retear groups were 3.54±2.37 and 5.16±2.16 (P=0.046), respectively. Retraction sizes in the healed and retear groups were 16.02±7.587 mm and 22.33±13.364 mm (P=0.037), respectively. The odds of retear rose by 4.2% for every 1-mm increase in AP tear size (P=0.032). Conclusions: The postoperative structural integrity of the rotator cuff tendon was not affected by tendon degeneration, whereas the arthroscopically measured AP tear size of the rotator cuff tendon was an independent predictor of retear.
Efficacy of platelet-rich plasma and platelet-rich fibrin in arthroscopic rotator cuff repair: A systematic review and meta-analysis

Abstract:Objective: Basic scientific studies have demonstrated positive effects of platelet-rich therapies, such as platelet-rich plasma (PRP) and platelet-rich fibrin (PRF), on tendon repair. However, clinical evidence indicating improved prognosis is controversial. In this study, we aimed to determine whether augmentation of arthroscopic rotator cuff repair with PRP and PRF improves outcomes compared to arthroscopic repair alone. Literature survey: PubMed, Embase, and Cochrane library databases were comprehensively searched for randomized controlled trials (RCTs) published until June 2022 according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. No language restriction was applied. Methodology: The primary outcomes were the rate of repeat tears after arthroscopic rotator cuff repair (retear rate) and clinical function scores (Constant-Murley Score, University of California, Los Angeles Score), and the extracted data were assessed for quality. Statistical analyses were performed using Review manager 5.3, and p < .05 was considered statistically significant. Synthesis: Ten RCTs with 628 patients were included. The results showed that augmenting surgery with PRP reduces retear rates compared to surgery alone (risk ratio [RR] = 0.40, 95% confidence interval [CI] 0.23-0.69, p = .001), whereas PRF has no effect on retear rates (p = .92). Regarding clinical function, PRP improves Constant-Murley scores (mean difference [MD] = 2.03, 95% CI 0.13-3.93, p = .04) and University of California, Los Angeles scores (MD = 1.30, 95% CI 0.36-2.24, p = .007), whereas PRF improves only Constant-Murley scores (MD = 3.93, 95% CI 1.50-6.36, p = .002). However, these differences were small and below the minimum clinically important difference threshold. Conclusions: This study showed that compared to arthroscopic rotator cuff repair alone, the application of PRP in arthroscopic rotator cuff repair reduces retear rate and improves clinical function scores, whereas the application of PRF has no clinically meaningful benefit. The small number and heterogeneity of studies as well as methodological limitations and risk of bias limit confidence in the true effect.
