Abstract:With recent advances in both knowledge and available technology, therapeutic strategies for gastric cancer have become diversified.1 Surgical instruments and devices have improved, and chemotherapeutic drugs and targeted agents have become more advanced as we gain a more comprehensive understanding of gastric cancer biology. The epidemiology of gastric cancer has also been changing due to mass screening programs and improved survival rates.2 However, these collective achievements have not reached the point of making patients satisfied. The relationship between doctors and patients is a very sensitive and hot topic in Chinese medical fields. It is like complex hair knots that can not easily be smoothed without suffering and pain. There are numerous problems involved in this issue. It is important for all of us to take into consideration the system and rules when considering this issue. Not only do doctors have overwhelming work hours but our needs also give all medical professionals too many burdens to carry. Advanced technology will aid doctors in their work but a solution to the whole problem of gastric cancer has yet to be found. The NCCN is a set of guidelines for Gastric Cancer treatment.3 But even though the NCCN is universal for anyone who is of the medical profession, the approach to treatment of a gastric cancer patient varies greatly from doctor to doctor in different departments. For instance a surgeon may respond to the patient's problems in an extreme and unnecessary way. On the other hand, the radiologist and chemo-therapist might not do enough. The doctors need to realize that patients are not carbon copies of one another and that each needs to be approached with an acute awareness of what exactly is going on with the patient in order to remedy the problems as best as possible. In the past decade, advances in high tech medical technologies have developed so fast that we have gone from ignorant to becoming very knowledgeable about complex diseases, one example being stomach cancer. While we were limited in what we could do many years ago, we are now implementing complex treatment; from traditional open operation to minimally invasive therapy, radiotherapy, chemotherapy, biotherapy and targeted therapy, etc.4 However new techniques keep coming and the advances in all scientific areas make doctors confused. Chinese people always say “five separate fingers cannot make a fist without a palm,” which means those separate remedies cannot be integrated for an individual who is succumbing to gastric cancer. A dedicated multidisciplinary team (MDT) of specialists needs to be formed to consider and discuss the treatment of gastric cancer patients. Although recently some hospitals or departments have tried to structure a reasonable cooperation between multiple disciplines, only a few can offer a real multidisciplinary team that can direct the patients' comprehensive treatment during the entire period of disease. The goal is clear but the means to achieve it seems to be completely out of reach. There are different reasons in the real clinical world; for instance, old-fashioned classification of modern clinical medicine branches, outdated internal operation system of the hospital, jaundiced minds, opinion, outlook of doctors, and so on. Ideal hospital departments should be classified by organ and disease. To put it simply, gastric cancer should be treated in a gastric cancer center. This gastric center should be composed by surgeon, gastroenterologists, physician, pathologists, image doctor, interventional doctor, radiologist, chemo-therapist, nutritionist psychiatrist, etc. The real practical pattern of MDT was consummated when the gastric center was set up. In reality, there is a common issue hidden behind these problems, paying less attention to medical humanism. Some doctors keep their eyes on the sickness and the technical index but forget about the integrity of “humanity”. It does not matter how advanced and helpful a piece of equipment is, it still needs the human touch. In China, there is a slogan: “technique is originating from Daoism” which is more advanced than the technique. Here Daoism is referred to as some kind of art. Under the influence of modern technology techniques have changed rapidly; however, Daoism has remained stagnant. A people-oriented management model is the most important Daoism of clinical medicine. Humans are not biological objects whose body parts can be divided into certain biological sections. If someone has a headache, you can not just examine the head: there could be tight muscles in the shoulders or back muscles that could be the cause. Modern medical education and research has focused too much on specific issues, and they forget the body as a whole. Those in the medical field need to realize they are not curing a disease; they are curing one human being at a time, and as a whole. The person presents a unique, diverse, and difficult set of problems for each individual. But this kind of thought process is often ignored as much of today's medical technique seems to have a template to which we try to fit each patient: “Oh you have this disease? Then follow steps a, b…” and so forth. We were warned 100 years ago by Dr. Osler saying “By far the most dangerous foe we have to fight is apathy - indifference from whatever cause, not from a lack of knowledge, but from carelessness, from absorption in other pursuits, from contempt bred of self-satisfaction”.5 The problem is the decline of the doctor-patient relationship. What makes the situation worrisome is that the conflict is about to reach its worst point. Numerous problems could have been solved or prevented if the doctors had just been more compassionate. Instead, the patients were treated like another subject waiting in line to be dealt with, not a real human being with feelings and emotions. Hospitals need more departments that focus on checking up on patients and making sure they are doing well after treatments: Hire more social workers, counselors, and physiologists. It is time for everyone, doctors, patients, citizens, government officials, medical professionals, and especially the media, to wake up and face the problem that plagues China. In order to cure a disease, one needs knowledge and the technical skills to carry out the task. A medical problem cannot be solved without one or the other. When doctors take advantage of their knowledge and technical abilities and combine those two with compassion for their patients, the possibilities for good things will be endless.
