Abstract:P171A Aim: For technical reasons living donor kidneys are procured without an aortal patch for transplantation. Many centers avoid to obtain kidneys from living donors presenting multiple renal arteries (MRA). At our institution kidneys from living donors with MRA are routinely transplanted by sequential arterial anastomosis technique while others prefer ex situ vascular reconstruction procedures. The aim of this study was to analyze outcome of recipients receiving kidneys from living donors presenting MRA in comparison to kidney grafts with singular renal arteries (SRA). Patients and Materials: Intraoperative details as well as postoperative courses of 196 kidney transplanted patients following living donation between May 1996 to December 2003 were studied prospectively. Postoperative creatinin levels, urea levels, blood presure, initial non function (INF) of kidney grafts, incidence of complications as well as graft survival and patient survival were assessed. Results: Of 196 kidney transplantations 33 kidney grafts revieled MRA (17%). Of those, 2 arteries were present in 27 (14%) and 3 arteries in 6 cases (3%). There was no significant difference in age (SRA: 33 ± 18 years; MRA: 33 ± 19 years), height (SRA: 1,74 ± 0,10m; MRA: 1,71 ± 0,08m), body weight (SRA: 75 ± 16kg; MRA: 72 ± 11kg), and sex of recipients (SRA: 113 males/50 females; MRA: 19 males/14 females). All implantations of MRA were performed by sequential anastomosis to the common and/or external iliac artery. The incidence of surgical complications was 23% for the SRA and 30% for the MRA group, respectively (n.s.). In detail, there was no difference in the incidence of urinary leakage (SRA: n=7, 4% vs. MRA: n= 2, 6%) as well as bleeding (SRA: n= 5, 1% vs. MRA: n=1, 3%) for both groups. In the SRA group 2 vascular complications occurred. Namely one arterial stenosis and one intima lesion were observed, both requiring surgical revision. No vascular complications were seen in the MRA group. The incidence of lymphoceles was significantly higher in the MRA than in the SRA group (SRA: n=5, 3% vs. MRA: n= 4, 12%; p<0,01). In both groups operative revision for lymphocele was required in 3 patients, respectively. The rate of initial non-function of kidney grafts was 3% for the SRA and 3% for the MRA group. The 1- and 5-year patient survival was 99% and 94% for the SRA and 100% and 100% for the MRA group (n.s.) Corresponding death censored 1- and 5-year graft survival was 97% and 92% for the SRA and 100% and 100% for the MRA group (n.s.) Conclusions: Although MRA have been considered a relative contraindication for living donation kidney transplantation in the past, we implanted succesfully kidneys with MRA using sequential anastomosis technique. Thus, results are comparable to transplantation of kidneys with SRA. The higher incidence of lymphoceles in the MRA group might be related to a more extensive preparation of the iliac arteries to allow sequential anastomosis. However, this did neither lead to reduced patient nor graft survival. We suggest that the presence of MRA should not be considered as a contraindication for living donation. This may help to expand the pool of living kidney donors.
The Safety Analysis of Living-Related Kidney Donors in Short Term after Transplantation

Abstract:OBJECTIVE:To evaluate the safety of living related donors in short term after transplantation.METHODS:Two hundred and fifty-one cases of living related donor kidney transplantation from May 2000 to July 2007 were analysed retrospectively. There were 117 male and 134 female aged from 22 to 72 years old, with a mean of 46.6 years old. The indexes were compared including serum creatinine (SCr), creatinine clearance (CCr), glomerular filtration rate (GFR) and quality of life before and after donation. Surgical complications were followed-up.RESULTS:Donors' SCr was (75.9 +/- 17.2) micromol/L before donation, (107.4 +/- 21.2) micromol/L on 7 d after donation, (130.4 +/- 58.2) micromol/L at the 1(st) month and (116.1 +/- 24.1) micromol/L at the 3(rd) month. There were significant difference between any 2 time points (P < 0.01). CCr was (94.4 +/- 17.5) ml/min before donation and (63.5 +/- 17.8) ml/min on 10 d after donation (P < 0.01). In 62 donors, total GFR was (82.4 +/- 21.8) ml/min before donation. On 10 d after donation, GFR of remaining kidney was (57.4 +/- 14.1) ml/min which was 34.7% higher than GFR of this kidney before donation (42.6 +/- 11.8) ml/min. There was no significant difference in quality of life before living related donors and non-donor populations (P = 0.116). Surgical complications included splenic rupture in 1 case, descending colon rupture in 1 case and wound infection in 5 cases.CONCLUSION:Living donor kidney transplantation is safe for donors, although part of indexes would vary within normal range during the early time after donation.

Abstract:Objective To analyze the clinical outcomes and evaluate the safety of living related live donors in kidney transplantation.Methods One hundre and thirty-two cases of living related donors were studied retrospectively for psychological and physiological parameters. The parameters including life quality, urinalysis, serum biochemistry tests, glomerular filtration rate (GFR) and endogenous creatinine clearance rate (CCr) were analyzed.Results There was no significant difference between living related donors and normal people in the aspect of life quality. In living related donors,SCr was (78.33±15.94)μmol/L before operation and was (108.49±19.88)μmol/L at 7 days postoperation, P＜0.05. SCr was (112.47±20. 38)μmol/L at 6 months post-operation. There was no significant difference in SCr levels between 7 days and 6 months post-operation (P=0. 109). CCr was (95.80±20.92)ml/min in pre-operation and was (57.36±14. 92)ml/min at 7 days post-operation,P＜0.05. CCr was (65. 49±8. 25) ml/min at 6 months post-operation. There was no significant difference in CCr between 7 days and 6 months post-operation.The pre-operative total GFR was(74.08±18.51)ml/min. Of which, the right kidney GFR was (38.43±10.33)ml/min. The residual right kidney GFR was (56. 49±13. 01 ) ml/min 6 months after operation, which decreased 17. 59 ml/min (23.8%) compared with pre-operative total GFR (P＜0.05) and increased 18. 06 ml/min (47.0%) compared to the pre-operative right kidney GFR. Surgical complications included 1 case of splenectomy, 1 case of descending colon rupture and 5 cases of wound fat liquefactions.Conclusion Pre-operatively systemic psychological and physiological evaluation on living related donors, detailed consent consultation, standardized operating techniques, careful perioperative management and strict follow-up can improve the safety of living related donors.

Abstract:Objective To assess the physical impact on the donors and recipients of the live related renal transplantation.Methods Six live donor renal transplantations were taken from Jan.2000 to Dec.2002 and the donors and recipients were all followed up.The perioperation complications and the renal functions were dynamically observed.Results Among the 6 recipients,1 case failed in the cadaveric donor renal transplantation occurred the hyperacute rejection this time and continued in the hemodialysis status.5 recipients got the triple immunosuppressive therapy with normal renal functions.1 case occurred the acute rejection and another case occurred subclinical rejection,both recovered after antirejection therapy.No other complications occurred.Conclusion Live related renal transplantation provides significant advantages in terms of improved patients and graft survival with little impacts on donors.We should think highly of the preoperation physical evaluation and the postoperation follow-up.

Abstract:Kidney transplant has become the preferred renal replacement therapy for children with end-stage renal disease. The results of kidney transplant have improved enormously due to advances in organ procurement, organ preservation, surgical techniques, and immunosuppressive regimens. Renal transplant is a more cost-effective method versus hemodialysis and provides better quality of life. Kidney allografts with multiple renal arteries are not uncommon and have been associated with a higherrisk to develop vascular and urologic complications. We report a case of a pediatric transplant recipient of donor kidney with 2 renal arteries. A 14-year-old female child (16 kg) diagnosed with end-stage renal disease presented to our hospital for renal transplant. The child's mother agreed to donate one of her kidneys. The mother's renal angiogram revealed 2 bilateral renal arteries. End-to-side anastomosis of the renal artery to the common iliac artery was performed. Postoperative recovery was normal. Postoperative color Doppler ultrasonography revealed normal blood flow in both the renal arteries. Double renal arteries in the donor kidney should not be a contraindication for transplant in a child. The outcome in such cases is excellent and similar to cases with a single renal artery.

Abstract:Renal artery aneurysms (RAAs) may occur in patients with transplanted kidneys, either through de novo development or as a preexisting feature of the donor kidney. How this vascular condition progresses in patients on immunosuppressive therapy after transplantation is poorly understood, and to our knowledge, consensus guidelines for treating transplant patients with RAA have not been developed. We present the case of a kidney allograft recipient on triple immunosuppressive therapy in whom postoperative imaging revealed a 13-mm renal artery aneurysm in the renal hilum not amenable to endovascular intervention. We review systemic influences on aneurysm formation and how matrix metalloproteinases may interact with immunosuppressive medications. Surveillance imaging over 5 years has shown a stable aneurysm, and the patient has maintained stable renal function with adequate creatinine levels and no adverse symptoms.

Abstract:Objective. Renal transplantation is generally accepted as one of the most effective treatment options for the end-stage renal disease. However, donor kidneys with multiple renal arteries represent a surgical challenge. For these cases, the surgical strategy is to directly anastomose multiple renal arteries to the internal iliac artery or the external iliac artery. However, this approach is complicated by the difficulty in performing the anastomoses, bleeding, stenosis, and a high incidence of false aneurysms.Methods. By applying the method of ex vivo reconstruction of the donor renal artery with the recipient internal iliac artery, we repaired five cases of donor kidneys with more than three renal arteries and excessively short renal arteries during transplantation and treated three cases of renal aneurysms with renal autotransplantation.Results. In all the cases, the surgery was without complications. At a minimum follow-up of 4 years, all patients showed normal renal function, and color Doppler ultrasonography indicated no thrombus or aneurysm formation or obstruction in the renal artery or its branches.Conclusions. The ex vivo reconstruction of the donor renal artery with the recipient internal iliac artery is a feasible and effective surgical procedure for the transplantation of donor kidneys with multiple renal arteries and the autotransplantation of kidneys for the treatment of renal artery aneurysms.

Abstract:Objective To evaluate the efficacy of living related kidney transplants (LRKT). Methods Retrospectively clinical results of 25 LRKT were reviewed and analyzed. Results Living related donors underwent nephrectomy without any complications and the donors have had normal renal function on follow up studies.All the renal grafts survived.Two recipients experienced delayed graft function recovery,Both the patients and the grafts have been surviving. Conclusions The patient and graft survival rates of LRDT are better than cadaveric donor transplantation.Living related donor serves as an alternate way of kidney source.
Long-Term Follow-up of Over 600 Living Related Kidney Donors: Single-Center Experience

Abstract:Objectives: Kidney transplant is the treatment of choice in patients with end-stage renal disease because it offers improved survival and better quality of life. Although most epidemiologic studies have suggested that living kidney donors have a minimal lifetime risk of developing end-stage renal disease, long-term complications and physiologic and psychologic sequelae resulting from donation remain unclear. Here, we examined the long-term results of living-related kidney donors who donated kidneys at the Başkent University Ankara Hospital over the past 25 years. Materials and methods: We were able to examine 607 kidney transplant donors (mean age of 52.03 ± 11.54 years) who were seen at our center from 1986 to 2021 and who agreed to a general health evaluation. Collected data included donor age, sex, blood type, body mass index, duration after donation, blood pressure measurements, biochemical parameters, abdominal ultrasonograph for size, structure, and renal blood flow of the solitary kidney, comorbid conditions, chronic drug use, and surgical procedures after donation. Results: Mean time after donation was 10.4 ± 3.2 years. Twenty-four donors (3.9%) were diagnosed with diabetes and 21 (3.4%) with thyroid disease, 64 (10.5%) developed hypertension, and 48 (8.8%) developed atherosclerotic cardiovascular disease. Obesity was found to be an increasing problem in our donor population, with 174 (28.6%) developing mild to moderate obesity (body mass index >25 kg/m2). Older age, obesity, smoking, and hyperlipidemia were found to be the major and independent risk factors of both hypertension and atherosclerotic cardiovascular disease in donors. None of our donors developed endstage renal disease. Conclusions: Obesity and hypertension were the most common comorbidities that developed in our kidney donor population. Our principle is to avoid unrelated and nondirected donors because of the possible long-term complications. Unrelated donors may be desperate if a family member needs donation in the future.

Abstract:Despite the shortage of cadaver kidneys, there is a general reluctance to accept donors < 5 years of age. A major factor for centers declining to use these small organs is the high rate of surgical complications. From January 1986 through December 1990, 50 kidneys from donors aged 11 to 48 months were transplanted as single units into adult recipients. No allografts were used unless there was an ample aortic cuff to minimize arterial stenosis and thrombosis. All renal veins were anastomosed with interrupted 6–0 silk sutures. Previous studies attributed the high incidence of ureteral complications to the fragile blood supply of the pediatric ureter. To alleviate this problem, a stented high uretero‐ureterostomy or uretero‐pyelostomy was done. The surgical complications in these 50 recipients from donors ≤4 years of age were compared with 319 recipients of adult kidneys: Ped vs Adult, renal artery thrombosis 0/50 vs 2/319 (0.63%) (p = ns); renal artery stenosis 2/50 (4%) vs 1/319 (0.31%) (p = ns); renal vein thrombosis 0 vs 0; ureteral fistula 2/50 (4%) vs 7/319 (2.2%) (p = ns); and ureteral obstruction 0 vs 0 (p = ns). The ureteral blood supply was studied in the discarded organs with methylene blue. These studies demonstrated a rich blood supply to the renal pelvis and upper ureter. The blood supply to the distal lumbar ureter was scanty. It was concluded that kidneys from donors <48 months can be safely transplanted as single units into adult recipients with an incidence of surgical complications that is comparable to adults receiving adult cadaver kidneys.

Abstract:Transplant renal artery stenosis (TRAS) is a well-recognized vascular complication after kidney transplantation, with an incidence ranging from 1% to 23%. TRAS often presents with clinical features such as refractory hypertension, de novo hypertension, allograft dysfunction, and the presence of a bruit over the graft. A rare manifestation of TRAS is flash pulmonary edema. Here, we present a case of a 37-year-old male who received a living donor kidney. Four years after the transplant, he presented with acute kidney injury, hypertensive crisis, and flash pulmonary edema. Initially, methylprednisolone pulses were administered due to suspicion of acute rejection, which was later ruled out after a kidney graft biopsy. Computed tomography angiography showed findings suggesting stenosis or thrombus in the renal artery. The patient developed sudden acute pulmonary edema, requiring hemodialysis, with notable clinical improvement. Subsequently, stent placement was performed without complications, resulting in the complete recovery of renal function and effective blood pressure control. The incidence of renal artery stenosis is higher in living donor kidney transplantation, mainly due to technical complexities during surgery. Acute presentations, such as flash edema, are exceptionally rare but can occur years after transplantation. Prompt intervention can lead to favorable outcomes.

Do Multiple Renal Arteries in the Remnant Kidney Have a Negative Influence on Kidney Donors after Kidney Donation?

Abstract:Aim: To investigate whether the presence of multiple renal arteries in the remnant kidney has implications for lower renal function or increased incidence of hypertension. Methods: We reviewed the intraoperative and follow-up data of 101 live kidney donors who underwent nephrectomies at our institution. Sixty-nine donors (68.3%) had single artery in the remnant kidney (Group A), while 32 donors (31.7%) had multiple renal arteries in the remnant kidney (Group B). We compared the demographic and intraoperative data between the two groups. The follow-up data of donors in each group were divided into three subgroups based on the length of the follow-up period (12-24 months, 24-48 months and >= 48 months). Subgroups were created based on blood pressure and serum creatinine level. The delta blood pressure (follow-up blood pressure minus preoperative blood pressure) and delta serum creatinine (follow-up serum creatinine minus preoperative serum creatinine) in each subgroup in Group A were compared with the counterparts in Group B. Results: Renal arterial stenosis and calcification of renal arterial wall were not observed in all donors. There were no significant differences in the intraoperative characteristics (e. g. age, body mass index, operative duration and estimated blood loss) between the two groups. In addition, the blood pressure and serum creatinine level among subgroups within each group were similar. Furthermore, significant differences in dblood pressure and dserum creatinine were not observed between subgroups within the same follow-up period. Recipient survival rate and serum creatinine level were similar and acceptable in both groups. Conclusions: The presence of multiple renal arteries in the remnant kidney does not have additional negative influence on kidney donors after kidney donation.

Abstract:Since 1966 we have performed 16 renal transplants from unrelated living donors (ULD) (14 were “emotionally related”). The recipients were non‐diabetic, ABO‐compatible (mean age 39.3 yr) and had a negative T‐cell crossmatch with their donor. We compared their results with those of 16 recipients of cadaver donor kidneys, matched for age, sex, immunosuppressive regimen and year of transplant. In addition to the standard immunosuppressive regimen, 15 ULD recipients were given a 3‐d pre‐transplant course of ALG. All recipients were given pretransplant blood transfusions, 10 underwent thoracic duct drainage, 11 were splenectomized, and 4 received donor‐specific transfusions. The results of transplantation from poorly‐matched ULD were as good as those obtained with well‐matched cadaveric grafts, perhaps because of the better preparation of the ULD recipients.

Abstract:Living kidney donors are screened for transmissible diseases including cancer. Outcomes following donation are excellent, but concern exists regarding development of chronic kidney disease, and cancer risk is unknown. We used linked transplant and cancer registry data to identify incident cancers among 84,357 kidney donors in the United States (1995‐2017). We compared risk to the general population using standardized incidence ratios (SIRs). For selected cancers, we used Poisson regression to compare donors to 47,451 Adventist Health Study 2 (AHS‐2) participants, who typically have healthy lifestyles. During follow‐up, 2843 cancers were diagnosed in donors, representing an overall deficit (SIR 0.79, 95%CI 0.76‐0.82). None of 46 specified cancer sites occurred in excess relative to the general population, and 15 showed significant deficits (SIR<1.00). Compared with AHS‐2 participants, donors had similar incidence of liver cancer, melanoma, breast cancer, and non‐Hodgkin lymphoma but, starting seven years after donation, elevated incidence of colorectal cancer (adjusted incidence rate ratio 2.07, 95%CI 154‐2.79) and kidney cancer starting (2.97, 1.58‐5.58, accounting for the presence of a single kidney in donors). Elevated kidney cancer incidence may reflect adverse processes in donors' remaining kidney. Renal biopsy findings of living-related donors and their correction with post-surgery recovery

Abstract:Objective To evaluate the potential pathological changes of living-related donor kidney and the correlation between pathological changes and post-surgery recovery.Methods From May 2007.to July 2008 kidney biopsy were performed in 59 living-related donors during surgeries.None of the donors has abnormal findings at preoperative examinations.Relationship between the kidney biopsy findings and recovery of donors were analyzed.Results Fifty nine renal biopsies were performed during operations,and pathological changes were found in 14 cases,including mesangial proliferative glomerulonephritis in 1 case,renal small artery intimal hyperplasia and hyaline degeneration in 1 case,glomerular basement membrane thickening or mesentery proliferative lesions in 6 cases and glomerulosclerosis in 6 cases respectively.For donors without pathological changes in their kidneys,the percentage of donors with high-than-normal serum creatinine level after operation was 28.9﹪,and the creatinine level became normal after 1.22 ± 2.11 days.Also the incidence of proteinuria after operation was about 77.8﹪and it lasted 3.67 ± 2.45 days.For donors with pathological changes in their kidneys,the percentage of donors with high-than-normal serum creatinine level after operation was about 50.0﹪ and he creatinine level became normal after 4.20 ± 6.71 days.The incidence of proteinuria was about 71.4﹪ and it lasted 4.20 ± 4.76 days.In 33 donors younger than 55 years,7 were found to have pathological changes in kidney,while in 26 donors older than 55 years,7 were found to have pathological changes in kidney.Conclusions There is no significant differences in the incidence of abnormal Scr level and proteinuria,and the duration of abnormal Scr level and proteinuria between donors with or without pathologic changes.

Abstract:Objective To study the techniques and outcomes of ex situ reconstruction of damaged or malformed renal blood vessels before kidney transplantation.Methods 11 patients treated with transplantation of allogenetic kidneys with damaged or malformed blood vessels from April 2004 to April 2007 were retrospectively reviewed.20 patients transplanted with normal kidney during the same period were selected as control group.Bench surgery with microsurgical techniques was employed for the repair of the vessels,which included 3 cases of end to end anastomosis of arteries,3 cases of end to side anastomosis of arteries,2 cases of side to side conjoined anastomosis of arteries,and 3 cases of renal artery lengthening with an interposition of donor or recipient iliac artery.The renal vein was anastomosed to the external iliac vein and the renal artery to the internal iliac artery.The ureter was implanted using an extravesical technique.Triple immunosupression was used.Results Average follow-up was 2 years.There were no death occurred in both groups.No arterial stenosis occurred during the follow-up.The incidence of delayed graft function was 27.3% and 20% respectively(P=0.484).The incidence of acute rejection was 9.1% and 10.0% in vascular reconstruction group and control group respectively(P=0.719).Mean serum creatinine at 1 year was (110±21) and(107±23)μmol/L,and the difference was not significant between the 2 groups(P=0.664).Conclusion Donor kidneys with damaged or malformed vascular can be used for transplantation after bench reconstruction,and it will not influence the outcome of kidney transplantation.Different techniques can be used according to the vascular condition.

Abstract:Objectives To investigate the safety and feasibility of living relative kidney transplantation(LRKT)and evaluate the clinical effects of LRKT.Methods The clinical data of 12 LRKT were retrospectively analyzed.Results Living relative donors underwent nephrectomy without any complications.They were discharged at 7 to 9 days postoperation.One recipient experienced delayed graft function recovery.Acute rejection occurred in one recipient.The renal function in all recipients recovered well.In 8recipients with 6 months follow up,BUN,Cr and creatinine clearance were 8.2 mmol/L,97.2μmol/Lkidney from living relative donor is higher than that from cadaveric kidney.

Abstract:Objective To compare clinical efficacy of living related renal transplantation with single renal artery and multiple renal arteries.Methods One hundred and thirty cases of renal transplantation performed in Zhongshan Hospital were retrospectively reviewed.Their demographic information was summarized by basic statistical method.All recipients were followed up for one year to five years.χ2 test was used to compare clinical efficacy between single and multiple renal arteries.Results Donors were assessed by CT angiography(CTA) before surgery.Accessory renal artery was found in 32 donors(24.6%),while 10 cases were bilateral(7.7%).Multiple veins in right kidney were found in 15 donors(11.5%).The post-transplantation one-year survival rates of patients and grafts were 94.3% and 91.5%,respectively.In this study,there was no statistical difference in clinical efficacy between single renal artery group and multiple renal arteries group.Conclusions There was no statistical difference in clinical efficacy between living related renal transplantation with single renal artery and multiple renal arteries.The use of these grafts was safe for both recipient and donor.
