Abstract:Background and Aims Guidelines now recommend patients with low-risk adenomas receive colonoscopy surveillance in 7-10 years and those with the previously recommended 5-year interval be reevaluated. We tested three outreach approaches for transitioning patients to the 10-year interval recommendation. Methods Design : 3-arm pragmatic randomized trial comparing telephone, secure messaging, and mailed letter outreach. Setting : Kaiser Permanente Northern California, a large integrated healthcare system. Participants : Patients aged 54-70 years with 1-2 small (<10 mm) tubular adenomas at baseline colonoscopy, due for 5-year surveillance in 2022, without high-risk conditions, and with access to all three outreach modalities. Patients were randomly assigned to outreach arm (telephone [n=200], secure message [n=203], and mailed letter [n=201]) stratified by age, sex, race, and ethnicity. Intervention : Outreach in each arm was performed by trained medical assistants (unblinded) communicating in English with 1 reminder attempt at 2-4 weeks. Participants could change their assigned interval to 10 years or continue their planned 5-year interval. Results 60-day response rates were higher for telephone (64.5%) and secure messaging outreach (51.7%) versus mailed letter (31.3%). Also, more patients adopted the 10-year surveillance interval in the telephone (37.0%) and secure messaging arms (32.0%) compared to mailed letter (18.9%) and rate differences were significant for telephone (18.1%; 97.5% CI: 8.3%, 27.9%) and secure message outreach (13.1%; 97.5% CI: 3.5%, 22.7%) versus mailed letter outreach. Conclusion Telephone and secure messaging were more effective than mailed letter outreach for de-implementing outdated colonoscopy surveillance recommendations among individuals with a history of low-risk adenomas in an integrated healthcare setting.

Abstract:Introduction: Adenoma detection rate (ADR) is an accepted benchmark for screening colonoscopy. Factors driving ADR and its relationship with sessile serrated lesions detection rate (SSLDR) over time remain unclear. We aim to explore patient, physician, and procedural influences on ADR and SSLDR trends. Methods: Using a large healthcare system in northern California from January 2010 to December 2020, a total of 146,818 screening colonoscopies performed by 33 endoscopists were included. ADR and SSLDR were calculated over time using natural language processing. Logistic regression was used to calculate the odd ratios of patient demographics, physician attributes, and procedural details over time. Results: Between 2010 and 2020, ADR rose from 19.4% to 44.4% while SSLDR increased from 1.6% to 11.6%. ADR increased by 2.7% per year (95% confidence interval [CI], 1.9% to 3.4%) and SSLDR increased by 1.0% per year (95% CI, 0.8% to 1.2%). Higher ADR was associated with older age, male sex, higher body mass index, current smoker, higher comorbidities, and high-risk colonoscopy. In contrast, SSLDR was associated with younger age, female sex, white race, and fewer comorbidities. Patient and procedure characteristics did not significantly change over time (P interaction >0.05). Longer years in practice and male physician were associated with lower ADR and SSLDR in 2010, but significantly attenuated over time (P interaction <0.05). Discussion: Both ADR and SSLDR have increased over time. Patient and procedure factors did not significantly change over time. Male endoscopist and longer years in practice had lower initial ADR and SSLDR, but significantly lessened over time.

Abstract:Background: Sessile serrated adenoma/polyp is notorious for its malignancy potential and the difficulty to be detected and distinguished under endoscopy. This systematic review and meta-analysis aimed to evaluate endoscopic characteristics of SSA/P and delineate features that inform distinction between SSA/P and other types of lesions, including hyperplasitc polyp (HP) and conventional adenoma. Methods: MEDLINE, Embase and Cochrane Library from the inception to September 9, 2020 were searched for cohort, cross-sectional or case-control studies comparing endoscopic characteristics of SSA/P and other polyps. The primary outcome measure was the odds of finding specific endoscopic characteristics in patients with SSA/P compared with other types of polys in patients undergoing colorectal cancer screening. Findings: We included 74 studies from 16 countries comprising 34,535 SSA/P. Compared with HP, SSA/P was more likely to be in the right colon (OR 5.45; 95% CI 4.13-7.17; P 5mm vs. <=5mm OR 5.60; 95% CI 3.82-8.22; P<0.01); in Paris-0-II morphology (OR 2.33; 95%CI 1.45-3.70; P<0.01); having mucus cap (OR 8.48; 95% CI 4.86-14.8; P<0.01); vague margin under white light endoscopy (OR 2.71; 95% CI 1.88-3.92; P<0.01); having expended crypt opening (OR 5.09; 95% CI 1.87-13.9; P<0.01), varicose microvascular vessels (OR 6.17; 95% CI 1.57-24.3; P<0.01) and thick branched vessel (OR 5.17; 95% CI 1.81-15.8; P<0.01) under magnified narrow band imaging and type II-O pit pattern (OR 18.56; 95% CI 8.45-40.7; P<0.01) under magnified chromoendoscopy. Interpretations: We systematically synthesised current evidence on the discriminating endoscopic features of SSA/P. These findings could enhance the detection of SSA/P in endoscopy practice. Abstract:Prompt diagnosis and correct management of the so called “serrated lesions” (SLs) of the colon-rectum are generally considered of crucial importance in the past years, mainly due to their histological heterogeneity and peculiar clinical and molecular patterns; sometimes, they are missed at conventional endoscopy and are possibly implicated in the genesis of interval cancers. The aim of this review is to focus on the diagnostic challenges of serrated lesions, underlying the role of both conventional endoscopy and novel technologies. We will show how an accurate and precise diagnosis should immediately prompt the most appropriate therapy other than defining a proper follow-up program. It will be emphasized how novel endoscopic techniques may provide better visualization of mucosal microsurface structures other than enhancing the microvascular architecture, in order to better define and characterize specific patterns of mucosal lesions of the gastrointestinal tract. Standard therapy of SLs of the colon-rectum is still very debated, also due to the relatively lack of studies focusing on treatment issues. The high risk of incomplete resection, together with the high rate of postcolonoscopy interval cancers, suggests the need of an extra care when facing this kind of lesions. Given this background, we will outline useful technical tips and tricks in the resection of SLs, taking aspects such as the size and location of the lesions, as well as novel available techniques and technologies, other than future perspectives, including confocal laser endomicroscopy into consideration. Follow-up of SLs is another hot topic, also considering that their clinical impact has been misunderstood for a long time. The incidence of the so called interval colorectal cancer underlines how some weaknesses exist in current screening and follow-up programs. Considering the lack of wide consensus for the management of some SLs, we will try to summarize and clarify the best strategies for their optimal management.

Abstract:Background and aims: Colonoscopy screening can substantially reduce colorectal cancer incidence and mortality. Colonoscopies may achieve maximum benefit when they are performed with high quality and accompanied by follow-up recommendations that adhere to clinical guidelines. This study aimed to determine to what extent endoscopists met targets for colonoscopy quality from 2016 through 2019 (the most recent years before the COVID-19 pandemic). Methods: We examined measures of colonoscopy quality and recommended follow-up intervals in the GI Quality Improvement Consortium, a large nationwide endoscopy registry. The analysis included >2.5 million outpatient screening colonoscopies in average-risk adults aged 50 to 75 years. Results: At least 90% of endoscopists met performance targets for adequate bowel preparation, cecal intubation rate, and adenoma detection rate. However, nonadherence to guidelines for follow-up intervals was common. For patients with no colonoscopy findings, 12.0% received a follow-up interval recommendation of ≤5 years instead of the guideline-recommended 10 years. For patients with 1 to 2 small tubular adenomas, 13.5% received a follow-up interval recommendation of ≤3 years instead of the guideline-recommended 5 to 10 years. For patients with small sessile serrated polyps, 30.7% received a follow-up interval recommendation of ≤3 years instead of the guideline-recommended 5 years. Some patients with higher risk findings received a follow-up interval recommendation of ≥5 years instead of the guideline-recommended 3 years, including 18.2% of patients with advanced serrated lesions. Conclusions: Additional attention may be needed to achieve more consistent adherence to guidelines for colonoscopy follow-up recommendations.

Abstract:Background: A low adenoma detection rate (ADR) increases risks of interval cancers (ICs). Proximal colon flat polyps, e.g. serrated lesions (SLs), are difficult to find. Missed proximal colon flat lesions likely contribute to IC.Aims: We compared chromoendoscopy with water exchange (CWE), water exchange (WE) and air insufflation (AI) in detecting adenomas in screening colonoscopy.Methods: After split-dose preparation, 480 veterans were randomized to AI, WE and CWE.Results: Primary outcome of proximal ADR (55.6% vs 53.4% vs 52.2%, respectively) were similar in all groups. Adenoma per colonoscopy (APC) and adenoma per positive colonoscopy (APPC) were comparable. Detection rate of proximal colon SLs was significantly higher for CWE and WE than AI (26.3%, 23.6% and 11.3%, respectively, <i>p</i> = 0.002). Limitations: single operator; SLs only surrogate markers of but not IC.Conclusions: When an endoscopist achieves high-quality AI examinations with overall ADR twice (61.6%) the recommended standard (30%), use of WE and CWE does not produce further improvement in proximal or overall ADR. Comparable APC and APPC confirm equivalent withdrawal inspection techniques. WE alone is sufficient to significantly improve detection of proximal SLs. The impact of increased detection of proximal SLs by WE on prevention of IC deserves to be studied. This study is registered at ClinicalTrial.gov (NCT#01607255).

Abstract:Abstract Background To improve detection of mucosal lesions during colonoscopy a number of imaging modalities have been suggested, including high definition and virtual chromoendoscopy. Given the theoretical advantage of these new imaging techniques, we aimed to investigate their use for the detection of polyps in patients referred for colonoscopy in a large tertiary hospital. Methods Demographic, endoscopic, and histological data from 1855 consecutive patients undergoing colonoscopy were collected prospectively. Patients were randomly assigned to three endoscopy systems (Fujinon, Olympus, or Pentax) in combination with four modalities: conventional white-light colonoscopy (n = 505), high definition white-light colonoscopy (n = 582), virtual chromoendoscopy (n = 285) and high definition virtual chromoendoscopy (n = 483). Results The mean adenoma detection rate (ADR) was 34.9 %, and the adenoma per colonoscopy rate (APCR) was 2.1. No significant differences were noted between the three endoscopy systems. Moreover, no differences in ADR or APCR were observed between the four imaging modalities. High definition white-light colonoscopy resulted in a significantly higher detection of sessile serrated adenomas (8.2 % vs. 3.8 %; P < 0.01) and adenocarcinomas (2.6 % vs. 0.5 %; P < 0.05) compared with the conventional procedure. Conclusions No significant differences in ADR or APCR between different endoscopy systems, high definition, and/or virtual chromoendoscopy could be observed in routine colonoscopies in the general population. High definition endoscopy was associated with a significantly higher detection rate of serrated adenomas and adenocarcinomas.

Abstract:Background and AimsSessile serrated lesions (SSLs) have been increasingly recognized as precursors to colorectal cancer. Unlike adenoma detection rate (ADR), there is currently no agreed-upon benchmark for SSL detection rate (SSLDR), and data on factors that impact SSL detection are limited. We aimed to identify patient, endoscopist, and procedural factors associated with SSL and adenoma detection.MethodsWe used a single-center electronic endoscopy database to identify all patients ages ≥ 50 years who underwent outpatient screening colonoscopy from January 1, 2012, to June 30, 2018. Univariable Chi-square analysis was used to determine patient, endoscopist, and procedure-related factors associated with SSL or adenoma detection. We used logistic regression with generalized estimating equations, accounting for clustering by individual endoscopist, to determine factors independently associated with ADR and SSLDR.ResultsWe identified 10,538 unique patients who underwent colonoscopy performed by 28 endoscopists. Overall SSLDR was 2.2%, and overall ADR was 29.1%. On multivariable analysis, patient age, sex, BMI, smoking, endoscopist withdrawal time, and year of colonoscopy were independent predictors of ADR. Smoking and year of colonoscopy were independent predictors of SSLDR. Sub-optimal bowel preparation was inversely associated with SSL detection but not ADR.ConclusionsIn this large study of patients undergoing average-risk screening colonoscopy, overall SSLDR was low, indicating that methods for increasing SSLDR are needed. Our findings suggest that endoscopists may take into account risk factors for SSLs, such as smoking history, and recognize that the detection of such lesions, even more so than for adenomas, is dependent on optimal bowel preparation.

Abstract:With the recommendation of new guidelines to start colorectal cancer screening in average-risk individuals at age 45, gastroenterologists and colorectal surgeons are performing an increasing number of screening colonoscopies. However, certain polyps such as large flat polyps or carpet lesions may need advanced procedures such as EMR (endoscopic mucosal resection), ESD (endoscopic submucosal dissection), or EFTR (endoscopic full-thickness resection) for removal which may necessitate referral to an advanced endoscopist 1 . Endoscopically unresectable or malignant polyps are also referred to surgeons for resection. This study aimed to determine the prevalence of large polyps requiring EMR, ESD, EFTR, or surgery found during screening colonoscopies.
