Abstract:Objective To explore an advanced and effective method of hospital cost management so as to improve the effectiveness of hospital economic management. Methods A variety of methods were used, including field research, expert consultation, activity-based costing management, business process reengineering, and satisfaction survey combined with statistics and computer network technology. Results A system for implementing activity-based costing was established and remarkable results were achieved in lowering the cost of patient care, optimizing business process, raising the staff's awareness of cost, and assisting in development decision-making. Conclusion Activity-based costing, an advanced method of cost management, is of great significance to Chinese hospitals in reducing operational cost, optimizing work process, and improving the effectiveness of management.

Abstract:BACKGROUND:With market-oriented economic and health-care reform, public hospitals in China have received unprecedented pressures from governmental regulations, public opinions, and financial demands. To adapt the changing environment and keep pace of modernizing healthcare delivery system, public hospitals in China are expanding clinical services and improving delivery efficiency, while controlling costs. Recent experiences are valuable lessons for guiding future healthcare reform. Here we carefully study three teaching hospitals, to exemplify their experiences during this period.METHODS:We performed a systematic analysis on hospitalization costs, health-care quality and delivery efficiencies from 2006 to 2010 in three teaching hospitals in Beijing, China. The analysis measured temporal changes of inpatient cost per stay (CPS), cost per day (CPD), inpatient mortality rate (IMR), and length of stay (LOS), using a generalized additive model.FINDINGS:There were 651,559 hospitalizations during the period analyzed. Averaged CPS was stable over time, while averaged CPD steadily increased by 41.7% (P<0.001), from CNY 1,531 in 2006 to CNY 2,169 in 2010. The increasing CPD seemed synchronous with the steady rising of the national annual income per capita. Surgical cost was the main contributor to the temporal change of CPD, while medicine and examination costs tended to be stable over time. From 2006 and 2010, IMR decreased by 36%, while LOS reduced by 25%. Increasing hospitalizations with higher costs, along with an overall stable CPS, reduced IMR, and shorter LOS, appear to be the major characteristics of these three hospitals at present.INTERPRETATIONS:These three teaching hospitals have gained some success in controlling costs, improving cares, adopting modern medical technologies, and increasing hospital revenues. Effective hospital governance and physicians' professional capacity plus government regulations and supervisions may have played a role. However, purely market-oriented health-care reform could also misguide future healthcare reform.

Abstract:BackgroundThe increasing cost on healthcare exposes China’s healthcare budgets and system to financial crisis. To control the excessive growth of healthcare expenditure, China’s healthcare reforms emphasize the control of the global budget for healthcare, which leads to the release of relevant policy and a series of cost-control actions implemented by different hospitals. This work aims to identify the effects brought by the cost-control policy and actions via surveying and analysing feedback from clinicians.MethodsQuestionnaires on the cost-control policy and actions were designed for surveying 110 clinicians in hospitals from different regions of China. The data on the implementation of the cost-control actions and doctors’ feedback on these actions were analysed using descriptive statistics. Pearson’s chi-squared tests were performed to detect associations between doctors’ opinions and specific cost-control actions. A value of p < 0.05 was considered statistically significant. Association relationships between doctors’ opinions and cost-control actions were modelled into network models, and key factors were identified in a multi-variate framework. Last, we visualized our resultant data using a network model, and further multi-variate analysis was performed.ResultsThere were three main findings. (1) The cost-control policy has been widely implemented in the sampled hospitals in different regions of China, with more than 80% of those surveyed acknowledging that their hospitals take actions of reducing average prescription fees for outpatients, drug costs, and in-hospitalization durations. (2) Most doctors have a negative view of some cost-control actions; this is mainly due to concerns about the effects of these actions on the doctors’ own healthcare performance and patient satisfaction. (3) Cost-control actions that had a significant impact on doctors’ performance included limiting average prescription fees for outpatients and limiting the use of examinations/drugs/surgeries. Decreased patient satisfaction was associated with fewer admissions of critically ill patients, reduced use of brand-name drugs, and increased total costs to patients due to increased frequencies of visits to the hospitals.ConclusionsCost-control actions implemented in hospitals in response to the government’s policy to reduce its national healthcare budget affect both doctors and patients in several ways. Moreover, the cost-control policy and actions can be improved.

Abstract:Based on the information construction and taking a hospital as an example, this paper analyzes the current situation, existing problems, and reasons for financial management. This paper puts forward the scheme of optimizing hospital financial management and points out that accounting, asset management, cost management, and performance are all based on accounting. Even though with the continuous improvement and development of hospital informatization, various infrastructures have been built, the problems are more prominent. This can be solved by improving the focus on the products, and processes by reducing waste, or by increasing the quality. In hospitals, the core layer of hospital information technology is the financial management construction and development, and it is also an indispensable factor. It is better to improve the hospital's financial fine management level and promote the hospital's sustainable development. It is not enough to improve the assessment management and comprehensive budget management. It is necessary to integrate the perfect Health Information System, enterprise resource planning system, scientific research system, and structured query language database, establish a comprehensive financial management platform, and obtain data information and indicators of various economic operations in hospitals. Experiments show that the consumables sorting optimization algorithm in this paper can improve the equipment management of medical finance to some extent. In other aspects, this system also incorporates information technology to make enhancements to the original financial management.

Effects and problems in the global budget management of health insurance in Shanghai

Abstract:Since 2002,the global budget management of health insurance in Shanghai has gone through three stages: exploration,development,and maturity.The main practices include global budget,prepaid by month,postponement of payment by categories,settling accounts by season,circulating information by season,half-yearly evaluation,year-end liquidation,assessment and cost overruns sharing,and the implementation of"giving four items publicity"and "three rounds of negotiation."The reforms have controlled medical expenses,encouraged hospital selfmanagement,created a good environment for hospital reform,and strengthened the monitoring mechanism.However,there are still some problems with reform,including lack of standardization in the internal management of medical institutions,contradictions between cost control and health service,and lack of effective supporting measures.We recommend a three-pronged approach to perfecting global budget management: optimizing indicators,strengthening supervision,and developing payment reform.
