Abstract:China adopted rigorous lockdowns and restrictions to contain the spread of COVID-19 from 23 January to 8 April 2020. Although the quarantine severely limited people's freedom and caused multiple secondary disasters, most Chinese citizens tolerated it. Based on an online survey conducted at the beginning of the lockdown (from 31 January to 4 February 2020), we argue that local governments in many parts of the country gained more trust than usual, narrowing the trust gap with the central government. In the early stage of the pandemic, effective implementation of anti-COVID policies, official media propaganda, and public expectation all contributed to the public's increased confidence in local governments.

Abstract:Using an online panel, we surveyed a representative sample of 500 each in Australia and New Zealand during July 2020, in the middle of the Covid‐19 pandemic. We find trust in government has increased dramatically, with around 80% of respondents agreeing government was generally trustworthy. Around three quarters agreed management of the pandemic had increased their trust in government. Over 85% of respondents have confidence that public health scientists work in the public interest. Testing four hypotheses, we find that income and education predict trust in government and confidence in public health scientists, as does voting for the political party in government. Trust in government and confidence in public health scientists strongly predict Covid‐19 phone application use, largely through convincing people the App is beneficial. Trust in government then is both an outcome and antecedent of government effectiveness. Building trust is important for governments implementing difficult policy responses during a crisis.

Abstract:Background: Mistrust in science and scientists may adversely influence the rate of COVID-19 vaccination and undermine public health initiatives to reduce virus transmission. Methods: Students, staff and faculty responded to an email invitation to complete an electronic survey. Surveys included 21-items from the Trust in Science and Scientists Inventory questionnaire. Responses were coded so higher scores indicated a higher trust in science and scientists, A linear regression model including sex, age group, division, race and ethnicity, political affiliation, and history of COVID-19, was used to determine variables significantly associated with trust in science and scientists scores at the p < 0.05 level. Results: Participants were mostly female (62.1%), Asian (34.7%) and White (39.5%) and students (70.6%). More than half identified their political affiliation as Democrat (65%). In the final regression model, all races and ethnicities had significantly lower mean trust in science and scientists scores than White participants [Black ([Formula: see text]= -0.42, 95% CI: -0.55, -0.43, p < 0.001); Asian ([Formula: see text]= -0.20, 95% CI: -0.24, -0.17, p < 0.001); Latinx ([Formula: see text]= -0.22, 95% CI: -0.27, -0.18, p < 0.001); Other ([Formula: see text]= -0.19, 95% CI: -0.26, -0.11, p < 0.001)]. Compared to those identifying as Democrat, all other political affiliations had significantly lower mean scores. [Republican ([Formula: see text] =-0.49, 95% CI: -0.55, -0.43, p < 0.0001); Independent ([Formula: see text] =-0.29, 95% CI: -0.33, -0.25, p < 0.0001); something else ([Formula: see text] =-0.19, 95% CI: -0.25, -0.12, p < 0.0001)]. Having had COVID-19 ([Formula: see text]= -0.10, 95% CI: -0.15, -0.06, p < 0.001) had significantly lower scores compared to those who did not have COVID-19. Conclusion: Despite the setting of a major research University, trust in science is highly variable. This study identifies characteristics that could be used to target and curate educational campaigns and university policies to address the COVID19 and future pandemics.

Abstract:Importance: Trust in physicians and hospitals has been associated with achieving public health goals, but the increasing politicization of public health policies during the COVID-19 pandemic may have adversely affected such trust. Objective: To characterize changes in US adults' trust in physicians and hospitals over the course of the COVID-19 pandemic and the association between this trust and health-related behaviors. Design, setting, and participants: This survey study uses data from 24 waves of a nonprobability internet survey conducted between April 1, 2020, and January 31, 2024, among 443 455 unique respondents aged 18 years or older residing in the US, with state-level representative quotas for race and ethnicity, age, and gender. Main outcome and measure: Self-report of trust in physicians and hospitals; self-report of SARS-CoV-2 and influenza vaccination and booster status. Survey-weighted regression models were applied to examine associations between sociodemographic features and trust and between trust and health behaviors. Results: The combined data included 582 634 responses across 24 survey waves, reflecting 443 455 unique respondents. The unweighted mean (SD) age was 43.3 (16.6) years; 288 186 respondents (65.0%) reported female gender; 21 957 (5.0%) identified as Asian American, 49 428 (11.1%) as Black, 38 423 (8.7%) as Hispanic, 3138 (0.7%) as Native American, 5598 (1.3%) as Pacific Islander, 315 278 (71.1%) as White, and 9633 (2.2%) as other race and ethnicity (those who selected "Other" from a checklist). Overall, the proportion of adults reporting a lot of trust for physicians and hospitals decreased from 71.5% (95% CI, 70.7%-72.2%) in April 2020 to 40.1% (95% CI, 39.4%-40.7%) in January 2024. In regression models, features associated with lower trust as of spring and summer 2023 included being 25 to 64 years of age, female gender, lower educational level, lower income, Black race, and living in a rural setting. These associations persisted even after controlling for partisanship. In turn, greater trust was associated with greater likelihood of vaccination for SARS-CoV-2 (adjusted odds ratio [OR], 4.94; 95 CI, 4.21-5.80) or influenza (adjusted OR, 5.09; 95 CI, 3.93-6.59) and receiving a SARS-CoV-2 booster (adjusted OR, 3.62; 95 CI, 2.99-4.38). Conclusions and relevance: This survey study of US adults suggests that trust in physicians and hospitals decreased during the COVID-19 pandemic. As lower levels of trust were associated with lesser likelihood of pursuing vaccination, restoring trust may represent a public health imperative.

Abstract:Abstract The worldwide COVID-19 pandemic puts countries and their governments in an unprecedented situation. Strong countermeasures have been implemented in most places, but how much do people trust their governments in handling this crisis? Using data from a worldwide survey, conducted between March 20th and April 22nd, 2020, with more than 100,000 participants, we study people’s perceptions of government reactions in 57 countries. We find that media freedom reduces government trust directly as well as indirectly via a more negative assessment of government reactions as either insufficient or too strict. Higher level of education is associated with higher government trust and lower tendency to judge government reactions as too extreme. We also find different predictors of perceived insufficient reactions vs. too-extreme reactions. In particular, number of COVID-19 deaths significantly predicts perceived insufficient reactions but is not related to perceived too-extreme reactions. Further survey evidence suggests that conspiracy theory believers tend to perceive government countermeasures as too strict.

Abstract:A cross-sectional survey study from 1496 respondents was conducted to investigate how interpersonal risk communication and media risk communication foster individuals' interpersonal trust and trust in scientists during the ongoing COVID-19 pandemic. Findings reveal that interpersonal risk communication is significantly related to interpersonal trust and trust in scientists, but have no direct association with preventive behaviors. Media risk communication is significantly associated with preventive behaviors and increases individuals' trust in scientists while having no effect on interpersonal trust. Preventive behaviors are significantly influenced by both interpersonal trust and trust in scientists. In addition, trust mediated the association between risk communication and preventive behaviors.

Abstract:The devastating COVID-19 pandemic has cast a light on the question of why trust in scientists is important. This mixed-method study aimed to explore the study participants' trust in scientists about getting the COVID-19 vaccine. It also aimed to reveal which key sources of information participants followed about the pandemic. Participants' health-related behaviors and justifications for their behaviors as a response to the pandemic were also explored to understand how trust in scientists and sources of information played a role in the fight against COVID-19. The responses of 1233 participants across Turkey were analyzed. The findings indicated that trust in scientists and compliance with the scientists' suggested behaviors among the participants were quite high. It was also revealed that, for both trust and non-trust groups, participants questioned the reliability of information and the sources. Participants justified their behaviors mainly by referring to policies, e.g., masks, distance, and hygiene, developed and implemented with the collaboration of government, scientists, and the World Health Organization. Moreover, trust issues regarding scientists, government authorities, or other citizens were mentioned in participants' justifications. The current study also shed light on the importance of building trust through an open and healthy dialogue among scientists, government authorities, and the public to overcome the COVID-19 pandemic. The study highlighted the need for scientifically informed citizens to deal with the problems, i.e., misinformation and disinformation, in the post-truth era such as not jeopardizing effective solutions to eliminate the pandemic, e.g., taking the vaccine.

Abstract:U.S.-based research suggests conservatism is linked with less concern about contracting coronavirus and less preventative behaviors to avoid infection. Here, we investigate whether these tendencies are partly attributable to distrust in scientific information, and evaluate whether they generalize outside the U.S., using public data and recruited representative samples across three studies (Ntotal = 34,710). In Studies 1 and 2, we examine these relationships in the U.S., yielding converging evidence for a sequential indirect effect of conservatism on compliance through scientific (dis)trust and infection concern. In Study 3, we compare these relationships across 19 distinct countries. Although the relationships between trust in scientific information about the coronavirus, concern about coronavirus infection, and compliance are consistent cross-nationally, the relationships between conservatism and trust in scientific information are not. These relationships are strongest in North America. Consequently, the indirect effects observed in Studies 1-2 only replicate in North America (the U.S. and Canada) and in Indonesia. Study 3 also found parallel direct and indirect effects on support for lockdown restrictions. These associations suggest not only that relationships between conservatism and compliance are not universal, but localized to particular countries where conservatism is more strongly related to trust in scientific information about the coronavirus pandemic.

Abstract:Objectives: This study aimed to examine the levels of trust in information provided by public authorities 2 years after the COVID-19 outbreak and to examine factors associated with trust. Methods: Using a cross-national approach, online survey data was collected from four Western countries-Australia, Norway, the United Kingdom, and the United States of America. Differences in reports of very low or low levels of trust were examined by age, gender, area of residence, and the highest level of education in the four countries. Results: Levels of trust in the public authorities' information were highest among Norwegian respondents and lowest among U.K. respondents. Lower levels of trust in public authorities were found among males, individuals living in rural or remote areas, and those with lower levels of education. Conclusion: The outcomes contribute to knowledge regarding differences between socio-demographic groups and countries regarding the levels of trust people have in public authorities' information concerning a crisis, such as COVID-19. Strategies to promote trust in societies in different countries could consider these socio-demographic differences.
Public trust in the Government to control the spread of COVID-19 in England after the first wave—a longitudinal analysis

Abstract:Abstract Background To control the spread of coronavirus disease 2019 (COVID-19), governments are increasingly relying on the public to voluntarily manage risk. Effectiveness is likely to rely in part on how much the public trusts the Government’s response. We examined the English public’s trust in the Conservative Government to control the spread of COVID-19 after the initial ‘crisis’ period. Methods We analyzed eight rounds of a longitudinal survey of 1899 smartphone users aged 18–79 in England between October 2020 and December 2021. We fitted a random-effects logit model to identify personal characteristics and opinions associated with trust in the Conservative Government to control the spread of COVID-19. Results Trust was lowest in January 2021 (28%) and highest in March 2021 (44%). Being older, having lower educational attainment and aligning with the Conservative Party were predictors of higher levels of trust. Conversely, being less deprived, reporting that Government communications were not clear and considering that the measures taken by the Government went too far or not far enough were predictors of being less likely to report a great deal or a fair amount of trust in the Government to control the pandemic. Conclusion Trust in the Government’s response was found to be low throughout the study. Our findings suggest that there may be scope to avoid losing trust by aligning Government actions more closely with scientific advice and public opinion, and through clearer public health messaging. However, it remains unclear whether and how higher trust in the Government’s response would increase compliance with Government advice.

Abstract:<span>During international crises, trust in government is expected to increase irrespective of the wisdom of the policies it pursues. This has been called a 'rally-round-the-flag' effect. This article examines whether the COVID-19 crisis has resulted in such a rally effect. Using multi-wave panel surveys conducted in Austria and France starting from March 2020, in the article it is examined how government trust was affected by the perceived threats to the nation's health and economy created by the pandemic as well as by the perceived appropriateness of the government's crisis response. A strong rally effect is shown in Austria, where trust was closely tied to perceived health risks, but faded away quickly over time. Perceptions of government measures mattered, too, while perceived economic threat only played a minor role. Abstract:This study shows that social and political trust may diverge in the face of shared threats, and that this pattern is driven by negative information about crisis management. Leveraging a three-wave panel survey and an information-provision experiment in the USA during the COVID-19 crisis, our research reveals that negative perceptions of pandemic management lead to a decline in political trust and a parallel increase in social trust. This dynamic is pronounced among government supporters, who, confronted with COVID-19 challenges, experience a substantial erosion of political trust. Simultaneously, there is a notable rise in social trust within this group. Our analysis suggests that, as government supporters attributed more responsibility for the crisis to their political leader, political trust was supplanted by social trust. Disenchanted voters, feeling let down by institutions, sought support in society. Both the survey and the experiment underscore that societal shocks can prompt individuals to shift from relying on formal institutions to informal ones as a coping strategy. This research contributes a generalizable framework explaining how negative perceptions of crisis management can lead societies to substitute political trust with social trust, advancing our understanding of societal responses to shared threats and adaptive strategies during crises.

Methods: This study utilizes data from two large-scale surveys: the global behaviors and perceptions at the onset of COVID-19 pandemic and the Oxford COVID-19 Government Response Tracker (OxCGRT). Linear regression models were used to estimate the effects of public trust and strictness of restriction measures on people's compliance level. The model accounted for individual and daily variations in country-level stringency of preventative measures. Differences in the dynamics between public trust, the stringent level of government health guidelines and policy compliance were also examined among countries based on political regimes. Results: We find strong evidence of the increase in compliance due to the imposition of stricter government restrictions. The examination of heterogeneous effects suggests that high public trust in government and the perception of its truthfulness double the impact of policy restrictions on public compliance. Among political regimes, higher levels of public trust significantly increase the predicted compliance as stringency level rises in authoritarian and democratic countries. Conclusion: This study highlights the importance of public trust in government and its institutions during public health emergencies such as the COVID-19 pandemic. Our results are relevant and help understand why governments need to address the risks of non-compliance among low trusting individuals to achieve the success of the containment policies. Abstract:Scandinavian Journal of Public Health, Ahead of Print. Aims:To explore variations in trust in science and scientific institutions in Finland during the COVID-19 pandemic, and the factors influencing trust in experts of a key institution in the management of the pandemic, the Finnish Institute for Health and Welfare (THL). These results are contrasted with trust in the Ministry of Social Affairs and Health.Methods:Five surveys were conducted between April 2020 and March 2022 (n=5448). The changes were tested with the chi-square test. Predictors of trust in THL and the ministry was examined with binary logistic regression.Results:Trust in science and key scientific institutions remained consistently high throughout this period. In the early pandemic, trust in the ministry declined. The most significant explanatory factors for trust in THL and the ministry were being a supporter of some other party than the right-wing Finns Party and belonging to the age groups over 50 years.Conclusions:Our findings indicate that the pandemic, marked by unpredictability, did not weaken trust in science and THL, whereas trust in the ministry responsible for managing the pandemic and university experts weakened. In increasingly politically polarised societies, the impact of political sentiments on health-related perceptions and choices should be analysed more in future public health studies.

Abstract:This study examines public trust in government and public belief in its truthfulness in respect of the measures it is taking to combat COVID‐19. Analysing global data from the International Coronavirus Survey of 178 countries between 20 March and 8 April 2020, we establish that integrated government response policies, underpinned by containment health measures and economic reliefs, are crucial to winning public trust and support. We find that a one standard deviation increase in composite government response measures leads to a 0.353% increase in public trust in government and a 0.414% increase in public belief in its truthfulness. The impacts vary according to legal systems, whose political ethos determines the quality of welfare services and their ability to respond to citizens’ needs during a public health emergency. Further, public trust in government measures differs in relation to how a country's system of governance and institutional culture respond to meet public expectations, with citizens’ attitudes influenced by the fairness, effectiveness and accountability of government agencies. Most importantly, our evidence consistently demonstrates that the provision of impartial, transparent and truthful government communications is vital for maintaining public trust. Moreover, experience gained from previous pandemics, reinforcing a nation's preparedness and responsiveness to future public health crises, is crucial to ensuring citizens’ confidence in government competence. Overall, our original investigation reveals a contention between the exigencies of government policies and public expectations in a global health emergency, and has profound implications for public management and business and economic regeneration in the aftermath of the pandemic, laying the foundations for future research.

Abstract:Journal of Health Services Research &Policy, Ahead of Print. ObjectivesTo explore how the use of, and trust in, different sources of advice and information on COVID-19 differed across the four UK nations and between different sociodemographic groups and their associations with COVID-19 vaccination status.MethodsWe used a UK-wide representative survey conducted in July 2021, which included data on uptake of COVID-19 vaccination, trust in information sources, use of sources and geographical and sociodemographic variables. We used multivariate logistic regression to identify factors associated with completed or planned COVID-19 vaccination.ResultsTrust in the NHS, followed by trust in scientists, were the strongest predictors of vaccination intention. NHS websites were the most used (56% across the UK); only the Scottish government website had a higher level of reported use (58%). Using either source was associated with a positive vaccination status as were use of the GP and television as sources of advice. Use of social media, family and friends, and 'none' of the sources enquired about, were all linked to a lower likelihood of being or intending to get vaccinated. Compared to those in England, respondents in other UK nations were less likely to trust the central UK government for advice on COVID-19. There was considerable variation by age in trust and use of some, but not all, sources of advice, with predicted probabilities ranging from 35% among the youngest age group to 62% among those aged 65 years or older. There were also significant differences by annual household income and by occupational class for trust in government, with higher incomes correlating with greater likelihood of trust.ConclusionsThis study demonstrates high levels of trust in the key sources of public health advice and there was a positive association between using official sources of advice and vaccination intentions, even in the context of overall high vaccination rates. Our findings highlight the need for the UK and devolved governments to value the importance of public trust in the health system and take appropriate measures to avoid undermining such trust.

Abstract:Introduction: Negative trends of trust in governments have been described around the world. This study aimed to describe the distrust level in the National Health Service (NHS) and in governmental management of the pandemic, one year after the start of the COVID-19 vaccination campaign. Methods: A survey was distributed in February 2022 among a convenience sample. Outcomes were measured through validated tools: Revised Health Care System Distrust Scale, and the COVID-SCORE-10 questionnaire. Associations were assessed using multiple linear regression models. Results: A total of 2111 questionnaires were collected (54.8% female, median age 43 years [IQR = 34-50]). Distrust in the NHS had a median level of 12 [IQR = 10-14], while trust in the government had a level of 47 [IQR = 35-60]. COVID-19 vaccine hesitancy, high educational attainment, worse economic status, low conventional and digital health literacy, as well as the presence of conspiracy thoughts and distrust in the NHS were significantly associated with lower trust in government during the COVID-19 pandemic. Lack of trust in governmental actions, along with low education and health literacy, as well as the presence of conspiracy thoughts and worse perceptions of one's own health, were associated with greater distrust in the NHS. Conclusions: Overall, low levels of trust in both the government and the NHS have emerged. Since the determinants of reduced trust in institutions can be very diverse and that such levels of distrust after a pandemic can last for an entire generation, tailored interventions are needed to rebuild adequate levels of trust in institutions among the population.

Abstract:The COVID-19 pandemic jeopardized individuals' health and economic stability, and the associated shocks might have decreased individuals' trust. In this paper, we study the relationship between subjective perceptions of the pandemic and individuals' institutional and interpersonal trust (e.g., trust towards the government or health representatives), while considering objective health and economic shocks due to the pandemic as drivers. We collected data across Australia, France, Germany, and South Africa during a later stage of the COVID-19 pandemic (i.e., from mid-April to early-June 2021) when individuals had time to personally experience the pandemic and its effects. COVID-19 illness perception was associated with lower institutional and interpersonal trust. The health shock of having experienced COVID-19 was associated with higher interpersonal trust, while economic shocks were associated with lower institutional trust when they were due to the pandemic. The results suggest that public policy interventions in a later stage of a pandemic should consider objective economic and health outcomes as well as subjective ones, such as individual's perceptions. Authorities should communicate in a way that helps concerned people understand that they can take control of their health and the possibility of infection, and reassure them that health measures such as vaccination can help prevent the spread of the virus.
