Abstract:OBJECTIVE:To study the clinicopathologic features and differential diagnosis of alveolar soft part sarcoma (ASPS).METHODS:The clinical data and pathologic features of 48 cases of ASPS were evaluated. Immunohistochemical study, PAS staining and fluorescence in-situ hybridization (FISH) were carried out in selected examples. Relevant literature was reviewed.RESULTS:Amongst the 48 cases studied, there were 17 males and 31 females, with male-to-female ratio of 1.0∶1.8. The age of patients ranged from 2 to 60 years (median=26 years). The tumor was most commonly located in deep soft tissue, especially that of lower extremities. Histologically, the tumor cells were arranged in alveolar or solid patterns and separated by sinusoidal vessels. They were large and contained abundant eosinophilic granules or crystals in cytoplasm. The nuclei were round to polygonal and vesicular, often with prominent nucleoli. Intravascular tumor extension was common. Some cases showed necrosis, hemorrhage and cystic changes. Immunohistochemical study showed that the tumor cells were positive for TFE3 (100%, 33/33). FISH assay was carried out in 4 cases and all of them had TFE3-ASPL gene fusion.CONCLUSIONS:ASPS is a rare malignant neoplasm, often occurs in young patients. TFE3 is a useful immunohistochemical marker for diagnosis. The diagnosis is further confirmed by other markers.
Analysis of Clinical Characteristics and Curative Effects on Alveolar Soft-Part Sarcoma

Abstract:Objective:This work aims to analyze the clinical features and curative effects of 42 cases with alveolar soft-part sarcoma (ASPS ).Methods:Data of 42 patients with ASPS,who received treatment at this hospital,during the period May 1962 to August 2008,were reviewed.The age of the patients ranged from 9 years to 56 years(median 27 ).There were 23 females and 19 males in the cases.The major site of the primary cancer was the limbs(83.3%,n = 35 ).The median size of the primary tumor was 4.2 cm. Five patients(12%) presented metastatis.Wide local excision of the primary cancer was performed in 38 of the 42 patients.Results: After a median follow-up of 57 months(range:18 months to 187 months),the overall survival rate at 5,10,and 15 years were 81.7%, 63.6%,and 31.8%,respectively.Recurrence or metastatis occurred in 27 patients(64.3%) within the follow-up period.Local recurrence occurred in only 1 of the cases,local recurrence combined with metastasis in 3,and metastatic disease alone in 23.Pulmonary metastasis was found in 21 patients,brain metastasis in 7,and osseous metastasis in 5.The median survival time after diagnosis of distant metastasis was 36 months(ranging from 17 months to 82 months ).Chemotherapy using a variety of regimens was perfonned in 20 of the patients who suffered distant metastasis.Six patients also underwent metastasectomy for pulmonary metastasis.Response to chemotherapy was recorded in 14 patients,among which stable disease was seen in 6 and progressive disease in 8.Conclusion:ASPS displays a relatively indolent clinical course and features a younger age range.Surgery is the mainstay of therapy.The ultimate prognosis of ASPS is dissatisfactory and is often characterized by late metastasis to the lung,bone,or brain.

Abstract:Background and purpose:Alveolar soft part sarcoma(ASPS)is a rare malignant neoplasm characterized by slow growth and indolent behavior,and most of the patients had metastases in diagnoses.This study was to def ine clinical features and assess prognostic factors of ASTS.Methods:Between January 2003 and December 2008,there were 15 patients with ASTS who received treatment and were followed up in the Department of Orthopedic Oncology of Peking University People's Hospital.We retrospectively analyzed the clinical factors of the disease related to prognosis:gender,age,tumor size,primary tumor site,metastatic sites,local recurrence,the roles of postoperative radiotherapy and postoperative chemotherapy.Results:At a follow-up from 7 to 76 months,only 2 patients had local recurrences after excision,but there was a high incidence of pulmonary metastases.Only 6 patients achieved disease free survival,5 patients died of metastases or other complications.The fi ve-year overall survival rates was 41.7%,and the median survival time was 42.0 months.Univariate analysis and Cox regression analysis revealed that tumor size,tumor primary site and metastases at diagnoses were prognostic factors,but not gender and age.Conclusion:Alveolar soft part sarcoma is a high grade malignant soft tissue neoplasm with frequent metastases to lung,bone or brain.Tumor size,primary tumor site and metastases at diagnoses are some of the prognostic factors.Chemotherapy or radiotherapy after surgery have no benefi t to the survival in our analysis.Further prospective studies are needed to clarify the best strategy for the treatment of ASPS.

Abstract:BACKGROUND: Alveolar soft part sarcoma (ASPS) is a rare soft tissue tumor that usually affects young patients. Because of the rarity of the disease, most reports relating to ASPS are in the form of case reports or small series.METHODS: We performed a retrospective study to evaluate the clinicopathologic features, treatment, outcome and pattern of treatment failure in a consecutive series of patients with localized or metastatic ASPS between 1996 and 2011. Demographics, tumor sizes, sites and extent of disease, treatments provided, progression-free survival, and overall survival were evaluated.RESULTS: A total of 19 patients were identified. The clinical assumptive diagnosis of the first medical examination doctor was benign soft tissue tumor in 5 cases (26%) and benign hemangioma in 4 cases (21%), delaying treatment. The most common location of primary tumor was the thigh. The median diameter of the mass was 55 mm (range, 10 to 130 mm). An R0 resection was obtained in 11 cases. Adjuvant radiotherapy was delivered in 8 cases; postoperative systemic chemotherapy was delivered in 10 cases. Eight out of 15 patients (53%) exhibited metastases either at presentation or later. Median overall follow-up was 54 months.CONCLUSIONS: The treatment principle for alveolar soft tissue sarcoma is massive resection, and when the surgical margin is questionable, radiation treatment can be added for prevention of local recurrence. Also, due to discovery of metastases and local recurrence, even after 5 years of general treatment, outpatient department follow-up is needed, and we need to keep in mind that lung, intracranial, and bone metastases are common.

Abstract:Purpose Alveolar Soft Part Sarcoma (ASPS) is an exceedingly rare and aggressive cancer in children. Our objective was to conduct a population-based cohort study to forecast overall survival (OS) in pediatric ASPS patients. Methods We utilized the Surveillance, Epidemiology, and End Results (SEER) database to identify all pediatric ASPS patients diagnosed between 1975 and 2019. Kaplan-Meier estimations were employed to construct survival curves based on various criteria. Survival curves were compared using the log-rank test. Cox proportional-hazards regression was utilized to determine variables associated with OS. Additionally, we constructed a nomogram to predict overall survival in pediatric ASPS patients. Results A total of 103 pediatric ASPS patients were identified. Predominantly, the tumors affected females (62.2 %), and most of them located in the extremities (53.4 %). The majority of patients underwent surgery (83.5 %). Survival rates declined with increasing tumor size, and patients with localized tumors exhibited significantly better prognoses than those with distant tumors. Surgery conferred superior survival outcomes compared to no surgery. Cox proportional hazard regression analysis identified SEER stage and surgery as important independent predictors of survival. Conclusions Our study highlights SEER stage and surgery as key predictors of OS in pediatric ASPS, offering crucial epidemiological insights for clinical management.

However, limited information is available regarding its clinical characteristics. The present study presents the clinical manifestations, imaging findings, pathological features, treatment strategies and prognosis records of 8 cases of orbital ASPS over the last 15 years in a single hospital, along with a review of the literature. Included were 3 male and 5 female patients, with the median age at presentation being 9.5 years. The mean average largest tumour diameter was 3.6 cm. A total of 5 patients underwent surgical excision of the tumour, with 2 undergoing orbital exenteration and 1 undergoing partial orbital exenteration. In total, 6 patients received postoperative radiotherapy and 2 received chemotherapy. Upon follow-up, 6 patients were doing well with no evidence of recurrence or metastasis. Local recurrence developed in 2 patients, of whom 1 succumbed following withdrawal from treatment. According to the present series and the cases mentioned in the literature, orbital alveolar soft part sarcoma has characteristics distinct from those of alveolar soft part sarcoma which arises in other locations. Orbital alveolar soft part sarcoma presents itself in a younger population with a shorter course of disease, smaller tumour size, improved prognosis, a marked association with the extraocular muscles and with the Ki-67 proliferation index possibly associated with prognosis of the disease.

Abstract:Objective To investigate the clinicopathological characteristics,immunophenotype,molecular genetic feature and differential diagnosis of alveolar soft part sarcoma(ASPS) of cervix.Methods Two cases of cervical ASPS were reported and studied with routine HE stain,MaxVision immunohistochemical stain and fluorescence in situ hybridization(FISH).Results The two cases of cervical ASPS showed a local slowly growing neoplasm.Microscopically,the tumor cells arranged in alveolar pattern and nests,separated by fibrovascular trabeculare.The cells were polygonal to round,and larger with abundant finely granular eosinophilic cytoplasm.Immunohistochemically,the tumor cells expressed TFE3.FISH assay showed that the tumor cells had TFE3-ASPL gene fusion.Conclusions The cervical ASPS is extremely rare,with a histopathological features similar to those occurred in other locations.TFE3 is known as a specific immuno-marker of ASPS and the genetics shows TFE3-ASPL gene fusion.
A retrospective analysis of lung metastasis in 64 patients with alveolar soft part sarcoma

Abstract:Objectives To analyse the lung metastasis and possible factors influencing lung metastasis in alveolar soft part sarcoma (ASPS) patients. Methods The medical records of 64 consecutive ASPS patients were reviewed to analyse their treatments, features of lung metastasis, and possible factors influencing lung metastasis. Results Thirty-six females and 28 males with a median age of 27 years were included. The primary disease sites were the extremities in 51 patients and other locations in 13 patients. The median primary tumour size was 5 cm. Wide local excision of the primary tumour was performed on 56 patients (87.5 %). Thirteen patients (20.3 %) received postoperative adjuvant radiotherapy, and nine patients (14.1 %) underwent adjuvant chemotherapy. Twelve patients (18.8 %) presented with metastatic lung disease. Twenty-nine patients (45.3 %) developed metastatic lung disease during follow-up. Lung metastasis occurred in 64.1 % of the patients. Lung metastasis was detected at a median interval of 20 months after primary ASPS diagnosis. Being male, >20 years of age, having a primary tumour size ≥5 cm, and local recurrence were associated with a greater rate of lung metastasis. Median survival after the diagnosis of lung metastasis was 34 months. The 5-year survival rates were 64.1 and 95.2 % for patients with and without lung metastasis ( P < 0.001). Thirty-seven patients with metastatic lung disease received anthracycline- and ifosfamide-based chemotherapy. One patient experienced a partial remission. Conclusions ASPS patients have a high prevalence of lung metastasis. Sex, age, primary tumour size, and local recurrence are major factors influencing lung metastasis. Chemotherapy is not efficacious in ASPS patients with lung metastasis.

Abstract:OBJECTIVE: Alveolar soft part sarcoma (ASPS) is characterized by distinctive histomorphology of variably discohesive epithelioid cells arranged in nests and translocation of t(x;17) (p11.2;q25) resulting in ASPSCR1-TFE3 fusion. The aim of the present study is to review the clinical, histopathological, and immunohistochemical profile of ASPS with a focus on unusual histological features.MATERIAL AND METHOD: The present study is retrospective and descriptive. All cases with a diagnosis of ASPS were retrieved with clinical and radiology details.RESULTS: 22 patients of ASPS were identified. The most common site was the lower extremity and the size range was 3-22 cm. 54.5% of the patients had metastasis, with the lung as the most common site. Metastasis preceded detection of primary tumour in two cases. All cases showed similar histopathology of monomorphic epithelioid cells arranged in nests encircled by sinusoidal vasculature. Architecturally, the organoid pattern (81.8%) was followed by the alveolar pattern. 68.2% of the cases showed apple bite nuclei as the predominant nuclear feature. Rare nuclear features included binucleation (n=13), multinucleation (n=8), pleomorphism (n=4), nuclear grooves in three cases and intranuclear inclusion in one case, mitosis (n=5), and focal necrosis (n=6). All cases were positive for TFE3 and negative for AE1/AE3, EMA, HMB45, PAX8, MyoD1, SMA, synaptophysin, and chromogranin. Only two cases showed focal S100 positivity while one showed focal desmin positivity.CONCLUSION: Diffuse strong nuclear TFE3 positivity is sensitive for ASPS in an appropriate clinicoradiological context. Due to the high propensity for early metastasis, complete metastatic work-up and long term follow up is recommended.

Abstract:Background: Alveolar soft part sarcoma (ASPS) is a rare sarcoma that has been shown to be highly effective to antiangiogenic agents and immune checkpoint inhibitors, but most reported studies about ASPS were concentrated on adult population. In this study, we aimed to describe the clinical features and therapeutic outcomes of ASPS in children. Methods: We retrospectively reviewed the records of patients with ASPS in our institution since Jan 2015. All patients included in this study were pathologically confirmed ASPS and aged under 12 years at the time of initial diagnosis. Demographic characteristics, tumor sizes, primary tumor sites, metastasis, treatments used, therapeutic responses and survivals were evaluated. Results: We identified a total of 56 patients to be initially diagnosed as ASPS since Jan 2015. A predisposition of high occurrence in head and neck (32.1%) was observed (versus 41.1% in limbs and 21.4% in trunk). 26 (46.4%) patients developed metastasis at the time of diagnosis or during follow-up. Tumors in tongue, pharynx and larynx had the least likelihood to metastasize (7.7%, P<0.05). Observation was recommended for 15 stage IV patients with only pulmonary metastasis. 7 (46.7%) patients remained stable until last follow up. The 1-year PFS rate was 83.3% and median progression-free survival time (PFS) was 29.4 months. 15 patients with progressive disease received mono or combined therapy. 11 patients received PD-1 monotherapy. 2 patients achieved partial response and 5 stable disease. The overall response rate was 18.2%. The median PFS of this group was 22.0 months, and the 1-year PFS rate was 70.0%. 4 patients received a combination therapy of PD-1 inhibitors plus tyrosine kinase inhibitors. All of them remained stable. No disease-related death occurred during follow-up. Conclusions: ASPS exhibits a higher occurrence in head and neck in children. ASPS originating from glossopharyngeal region tends to have a lower metastasis rate. ASPS displays a more indolent growth pattern in children, which makes observation a preferable choice for children with sole pulmonary metastasis. Pediatric ASPS appears to be less effective to targeted therapy and immunotherapy than adults. The treatment of progressive ASPS in children remains challenging.

Abstract:Alveolar soft part sarcoma (ASPS) occurs most often in the deep muscles or fascia of the extremities in adults, with only 3.4% of these tumours originating from the head, face and neck. To date, only 17 cases of buccal ASPS have been reported, including the case presented here. Only one case of ASPS recurrence at the primary site, similar to our case, has been reported thus far. Immune checkpoint inhibitors (ICPis)-associated diabetes, with an estimated incidence of 0.43%, is usually seen in older cancer patients and has not been reported in younger people or in patients with ASPS.

Treatment of a case of primary alveolar soft part sarcoma of the lung: Case report and literature review

Abstract:Alveolar soft part sarcoma (ASPS) is an extremely rare type of soft tissue sarcoma. The primary sites of ASPS are mostly located in the extremities and trunk. Primary pulmonary ASPS is extremely rare. A search of the PubMed® database identified only five cases of primary pulmonary ASPS. This current case report describes the sixth case of ASPS in a 15-year-old male that presented with recurrent headaches. Head computed tomography showed space-occupying lesions in the left parietal lobe. Positron emission tomography-computed tomography confirmed the space-occupying lesions in the left parietal lobe and showed multiple nodules and masses in the two lungs and pleura, which were considered to be low-grade malignant mesenchymal tumours. The case report presents the clinical characteristics, diagnosis and treatment process. Programmed cell death protein 1 monoclonal antibody (sintilimab) combined with a tyrosine kinase inhibitor (anlotinib hydrochloride) achieved a good therapeutic effect, indicating that this combination therapy is worth exploring further. Large-scale prospective studies are needed to explore and develop standardized treatments for ASPS.

Abstract:Alveolar soft-part sarcoma involving the lung is mostly metastatic in nature, while primary alveolar soft-part sarcoma involving the lung occurs more rarely. Herein, we report a rare case of a patient with primary alveolar soft-part sarcoma of the lung, which may represent the earliest onset of this condition reported thus far. In this patient, surgery was performed to excise the lesion to the greatest extent possible, and the combination of surgery with chemoradiotherapy and an antiangiogenic agent may provide an important reference for the development of standard or first-line treatment for such pediatric patients.
Alveolar Soft-Part Sarcoma of the Prostate: a Case Report and Review of the Literature.

Abstract:Alveolar soft-part sarcoma (ASPS) is a rare malignant soft tissue tumor of uncertain cellular origin. We reported the case of a 21-year-old man with ASPS presenting itself as a markedly vascular tumor of the prostate. Immunohistochemistry showed positive nuclear staining for TFE3, positive cytoplasm staining for MyoD1 and neuron-specific enolase, and negative for S100, CK, synaptophysin, chromogranin A, myogenin and PSA. A dual-color, break-apart fluorescence in situ hybridization (FISH) assay identified the presence of a TFE3 gene fusion in the tumor cells. RT-PCR was performed to confirm the ASPSCR1 (ASPL)/TFE3 fusion transcript product in the tumor tissue. The patient suffered bone metastases 8 months after surgery and died of cachexia 14 months later. ASPS of the prostate should be discussed in terms of differential diagnosis from clinicopathological characteristics, immunophenotypes, and molecular genetic features.
Alveolar Soft-Part Sarcoma in Olfactory Cleft: A Case Report and Clinicopathological Review

Abstract:Sinonasal alveolar soft-part sarcoma (ASPS) is a rare malignant disease that comprises 0.4% to 1.2% of all soft-tissue sarcomas. Alveolar soft-part sarcoma is usually difficult to diagnose because it has many clinical and pathological mimickers. In this case study, ASPS occurred in the olfactory cleft, representing the second case in this location in the literature. This article presents the clinical presentation, radiologic, and histopathological case, and reviews the literature regarding the differentials.

Abstract:Alveolar soft part sarcoma (ASPS) is a rare tumor of uncertain histogenesis, accounting for only 0.5% to 1% of all soft tissue tumors. [ 1 ] The overall 2-year survival rate is 82%, and 5-year survival rate is 56%. [ 2 ] ASPS is characterized by der(17)t(X;17)(p11;q25) resulting in ASPSCR1–TFE3 fusion. [ 3 ] Demonstration of this particular gene rearrangement is the most desirable diagnostic criterion of ASPS. Conventional chemotherapy and radiation therapy are not beneficial in most previous studies. [ 4 ] Surgical excision is the main treatment method for ASPS, and has been reported to improve the prognosis of both patients with localized disease and distant metastases. [ 2 ] Primary pulmonary ASPS is extremely rare. Literature to date reported only 6 instances of primary pulmonary ASPS. [ 5–10 ] Due to financial and technical considerations, only 2 of the 6 cases confirmed the presence of ASPSCR1–TFE3 fusion. [ 6 , 7 ] This study reported a 27-year-old male case of primary pulmonary ASPS with confirmed ASPSCR1–TFE3 fusion and reviewed the currently published literature.

Abstract:Alveolar soft part sarcoma (ASPS) is a tumor of unknown histogenesis, composed of large, epithelioid cells with eosinophilic cytoplasm, having an alveolar pattern. Primary ASPS of uterine cervix is very rare. In this report, we present a 21-aged-old female with primary ASPS in the uterine cervix and discuss the clinicopathological characteristics, immunophenotype, molecular genetic feature and differential diagnosis of ASPS of cervix.
