Abstract:Objective:This work aims to analyze the clinical features and curative effects of 42 cases with alveolar soft-part sarcoma (ASPS ).Methods:Data of 42 patients with ASPS,who received treatment at this hospital,during the period May 1962 to August 2008,were reviewed.The age of the patients ranged from 9 years to 56 years(median 27 ).There were 23 females and 19 males in the cases.The major site of the primary cancer was the limbs(83.3%,n = 35 ).The median size of the primary tumor was 4.2 cm. Five patients(12%) presented metastatis.Wide local excision of the primary cancer was performed in 38 of the 42 patients.Results: After a median follow-up of 57 months(range:18 months to 187 months),the overall survival rate at 5,10,and 15 years were 81.7%, 63.6%,and 31.8%,respectively.Recurrence or metastatis occurred in 27 patients(64.3%) within the follow-up period.Local recurrence occurred in only 1 of the cases,local recurrence combined with metastasis in 3,and metastatic disease alone in 23.Pulmonary metastasis was found in 21 patients,brain metastasis in 7,and osseous metastasis in 5.The median survival time after diagnosis of distant metastasis was 36 months(ranging from 17 months to 82 months ).Chemotherapy using a variety of regimens was perfonned in 20 of the patients who suffered distant metastasis.Six patients also underwent metastasectomy for pulmonary metastasis.Response to chemotherapy was recorded in 14 patients,among which stable disease was seen in 6 and progressive disease in 8.Conclusion:ASPS displays a relatively indolent clinical course and features a younger age range.Surgery is the mainstay of therapy.The ultimate prognosis of ASPS is dissatisfactory and is often characterized by late metastasis to the lung,bone,or brain.

Abstract:Background: Alveolar soft part sarcoma (ASPS) is a rare type of soft tissue sarcoma that predominantly affects adolescents and young adults. Early diagnosis of ASPS is crucial for optimal therapeutic planning and improving prognosis, but its diagnostic features are not well delineated. This study aimed to retrospectively analyze the imaging features of ASPS with an emphasis on the dynamic contrast-enhanced-magnetic resonance imaging (DCE-MRI) and diffusion-weighted imaging (DWI) findings to identify imaging findings that might suggest the diagnosis to radiologists.Methods: The imaging features of 34 patients with pathologically proven limb ASPS were retrospectively analyzed. A total of 23 underwent DCE-MRI, and 12 underwent DWI.Results: Among the 34 cases of ASPS, 31 tumors were in the lower extremities, and 3 were in the upper extremities. The maximum tumor diameters ranged from 3.0 to 19.4 cm (mean, 8.7±3.96 cm). A total of 28 cases had well-defined borders. The masses demonstrated heterogeneous high signal intensity on T2-weighted imaging (T2WI) and the fat-suppressed (FS) T2WI sequence and slight hyperintensity on T1-weighted imaging (T1WI). A total of 25 lesions had thin hypointense bands on T1WI and T2WI. Intra- and peri-tumoral tubular areas of flow void were exhibited on both T1WI and T2WI in all cases. A total of 12 cases showed a high signal on DWI, and the mean apparent diffusion coefficient (ADC) value was (0.86±0.07)×10-3 mm2/s [range, (0.6-1.4)×10-3 mm2/s]. Persistent remarkable enhancement of the lesion was displayed on contrast-enhanced scans. The time-intensity curves (TICs) of 23 masses showed early arterial enhancement and slow washout of contrast.Conclusions: ASPS most commonly presents in the lower extremities of adolescents or young adults. Hyperintense T1WI, T2WI, and DWI signals, low ADC, flow voids, early arterial enhancement are frequent MRI features.

Abstract:Background: Alveolar soft part sarcoma (ASPS) is a rare sarcoma that has been shown to be highly effective to antiangiogenic agents and immune checkpoint inhibitors, but most reported studies about ASPS were concentrated on adult population. In this study, we aimed to describe the clinical features and therapeutic outcomes of ASPS in children. Methods: We retrospectively reviewed the records of patients with ASPS in our institution since Jan 2015. All patients included in this study were pathologically confirmed ASPS and aged under 12 years at the time of initial diagnosis. Demographic characteristics, tumor sizes, primary tumor sites, metastasis, treatments used, therapeutic responses and survivals were evaluated. Results: We identified a total of 56 patients to be initially diagnosed as ASPS since Jan 2015. A predisposition of high occurrence in head and neck (32.1%) was observed (versus 41.1% in limbs and 21.4% in trunk). 26 (46.4%) patients developed metastasis at the time of diagnosis or during follow-up. Tumors in tongue, pharynx and larynx had the least likelihood to metastasize (7.7%, P<0.05). Observation was recommended for 15 stage IV patients with only pulmonary metastasis. 7 (46.7%) patients remained stable until last follow up. The 1-year PFS rate was 83.3% and median progression-free survival time (PFS) was 29.4 months. 15 patients with progressive disease received mono or combined therapy. 11 patients received PD-1 monotherapy. 2 patients achieved partial response and 5 stable disease. The overall response rate was 18.2%. The median PFS of this group was 22.0 months, and the 1-year PFS rate was 70.0%. 4 patients received a combination therapy of PD-1 inhibitors plus tyrosine kinase inhibitors. All of them remained stable. No disease-related death occurred during follow-up. Conclusions: ASPS exhibits a higher occurrence in head and neck in children. ASPS originating from glossopharyngeal region tends to have a lower metastasis rate. ASPS displays a more indolent growth pattern in children, which makes observation a preferable choice for children with sole pulmonary metastasis. Pediatric ASPS appears to be less effective to targeted therapy and immunotherapy than adults. The treatment of progressive ASPS in children remains challenging.

Abstract:Alveolar soft-part sarcoma involving the lung is mostly metastatic in nature, while primary alveolar soft-part sarcoma involving the lung occurs more rarely. Herein, we report a rare case of a patient with primary alveolar soft-part sarcoma of the lung, which may represent the earliest onset of this condition reported thus far. In this patient, surgery was performed to excise the lesion to the greatest extent possible, and the combination of surgery with chemoradiotherapy and an antiangiogenic agent may provide an important reference for the development of standard or first-line treatment for such pediatric patients.

Abstract:OBJECTIVE:To study the clinicopathologic features and differential diagnosis of alveolar soft part sarcoma (ASPS).METHODS:The clinical data and pathologic features of 48 cases of ASPS were evaluated. Immunohistochemical study, PAS staining and fluorescence in-situ hybridization (FISH) were carried out in selected examples. Relevant literature was reviewed.RESULTS:Amongst the 48 cases studied, there were 17 males and 31 females, with male-to-female ratio of 1.0∶1.8. The age of patients ranged from 2 to 60 years (median=26 years). The tumor was most commonly located in deep soft tissue, especially that of lower extremities. Histologically, the tumor cells were arranged in alveolar or solid patterns and separated by sinusoidal vessels. They were large and contained abundant eosinophilic granules or crystals in cytoplasm. The nuclei were round to polygonal and vesicular, often with prominent nucleoli. Intravascular tumor extension was common. Some cases showed necrosis, hemorrhage and cystic changes. Immunohistochemical study showed that the tumor cells were positive for TFE3 (100%, 33/33). FISH assay was carried out in 4 cases and all of them had TFE3-ASPL gene fusion.CONCLUSIONS:ASPS is a rare malignant neoplasm, often occurs in young patients. TFE3 is a useful immunohistochemical marker for diagnosis. The diagnosis is further confirmed by other markers.

Abstract:Purpose Alveolar Soft Part Sarcoma (ASPS) is an exceedingly rare and aggressive cancer in children. Our objective was to conduct a population-based cohort study to forecast overall survival (OS) in pediatric ASPS patients. Methods We utilized the Surveillance, Epidemiology, and End Results (SEER) database to identify all pediatric ASPS patients diagnosed between 1975 and 2019. Kaplan-Meier estimations were employed to construct survival curves based on various criteria. Survival curves were compared using the log-rank test. Cox proportional-hazards regression was utilized to determine variables associated with OS. Additionally, we constructed a nomogram to predict overall survival in pediatric ASPS patients. Results A total of 103 pediatric ASPS patients were identified. Predominantly, the tumors affected females (62.2 %), and most of them located in the extremities (53.4 %). The majority of patients underwent surgery (83.5 %). Survival rates declined with increasing tumor size, and patients with localized tumors exhibited significantly better prognoses than those with distant tumors. Surgery conferred superior survival outcomes compared to no surgery. Cox proportional hazard regression analysis identified SEER stage and surgery as important independent predictors of survival. Conclusions Our study highlights SEER stage and surgery as key predictors of OS in pediatric ASPS, offering crucial epidemiological insights for clinical management.

Abstract:Abstract Context Alveolar soft part sarcoma (ASPS) is a rare soft tissue tumor most commonly occurring in deep intramuscular plane of lower extremities of adolescents and young adults. It is a highly vascular, slow growing tumor with malignant potential having lung as the most common site of metastases at the time of presentation. Aims The aim is to review the imaging findings of ASPS and determine characteristic imaging features of this rare tumor. Materials and Methods Sixteen patients having histopathological diagnosis and preoperative imaging of ASPS attending the dedicated sarcoma clinic at our institute were included in the study. The demographic, clinical, and imaging data were retrieved from the case records and then evaluated for characteristic imaging features which may raise suspicion of ASPS. Results The patients ranged from 3 to 72 years of age and with a slight male preponderance. Of the eight CECTs evaluated, 62.5% tumors showed well-defined lobulated margins, 87.5% cases showed intense enhancement with presence of feeder vessels. On CEMRI of 10 patients, 70% had well circumscribed lobulated margins with intense enhancement and tortuous flow voids in most of them. All cases showed T2 hyperintense signal. Fourteen of 16 (87.5%) patients had metastatic disease with lung as the most common site (92.8%). Conclusion ASPS is a rare soft tissue sarcoma seen in children and young adults. Imaging may mimic a vascular malformation due to the presence of tortuous feeders. Misdiagnosis at an early stage may lead to later metastatic presentation of the disease, thus emphasizing the need to suspect it on imaging.

Abstract:OBJECTIVES:To summarize the clinical features, diagnosis, treatment strategies, and prognosis of alveolar soft part sarcoma (ASPS) of the oral and maxillofacial region.STUDY DESIGN:We performed a retrospective study in a consecutive series of 18 patients with ASPS of the oral and maxillofacial region between 1995 and 2013. Demographic characteristics, tumor sizes, sites, tumor metastasis, diagnosis, treatments, and overall follow-ups were documented.RESULTS:The 18 patients were diagnosed pathologically with primary tumor developed on the tongue (10), the cheek (5), the pharynx (1), and the gingiva (2) with an average tumor size of 4 cm. At the latest follow-up, 1 patient with lung metastases survived for 23 months; 1 died 3 months after the confirmation of local recurrence and multiple pulmonary metastases; the rest of the patients were disease free and remained in good health.CONCLUSIONS:ASPS of the oral and maxillofacial region appears to have special clinical characteristics.

Abstract:Alveolar soft part sarcoma (ASPS) is an extremely rare and aggressive soft-tissue sarcoma (STS) subtype with poor prognosis and limited response to radiation therapy and chemotherapy. Prompt recognition and referral to sarcoma centers for appropriate management are crucial for patients’ survival. The purpose of this study was to report ASPS pre-treatment imaging features and to examine the existing literature on this topic. Twelve patients (7 women, 5 men—mean age 27.1 ± 10.7 years) were included from our single-center experience. Ultrasonography (US), computed tomography (CT), and magnetic resonance imaging (MRI) available were reviewed according to an analysis grid incorporating features from the latest research on STS. Clinical, histological, and outcome data were collected. MRI was available in 10 patients (83.3%), US in 7 patients (58.3%), and CT in 3 patients (25%). Mean longest tumor diameter was 7.6 ± 2.9 cm, and all tumors were deeply seated. Large peritumoral feeding vessels were systematically found and identified on ultrasonography (7/7), MRI (10/10), and CT (3/3). US revealed a well-defined heterogeneous hypoechoic pattern, with abundant flow signals in all patients (7/7). In all patients, MRI showed mildly high signal intensity (SI) on T1-WI and high SI on T2-WI and peritumoral edema. Moreover, flow-voids (due to arteriosus high-flow) into the peritumoral/intratumoral feeding vessels were detected in the MRI fluid-sensitive sequences of all patients. At baseline, whole-body contrast-enhanced CT revealed metastases in 8/12 (66.7%) patients. A pre-treatment longest diameter > 5 cm was significantly associated with distant metastases at diagnosis (p = 0.01). A maximum diameter > 5 cm represents a risk of metastatic disease at diagnosis (odds ratio = 45.0000 (95% CI: 1.4908—1358.3585), p = 0.0285). In the comprehensive literature review, we found 14 articles (case series or original research) focusing on ASPS imaging, with a total of 151 patients included. Merging our experience with the data from the existing literature, we conclude that the hallmark of ASPS imaging at presentation are the following characteristics: deep location, a slight hyperintense MRI SI on T1-WI and a hyperintense SI on T2-WI, numerous MRI flow voids, high internal vascularization, and large peritumoral feeding vessels.

Abstract:BACKGROUND: Alveolar soft part sarcoma (ASPS) is an extremely rare malignant sarcoma, accounting for less than 1% of all soft-tissue sarcomas. However, limited information is available on multimodal imaging [computed tomography (CT), magnetic resonance imaging (MRI), and positron emission computed tomography/computed tomography (PET/CT)] of ASPS.CASE SUMMARY: This study reports a case of a 35-year-old female patient with ASPS of the left thigh with lung metastasis. The patient presented with a 1-year history of a palpable mass in the lower extremity, which exhibited rapid growth for 3 wk. CT, MRI, and F-deoxyglucose PET/CT examinations were performed. CT showed a slightly hypodense or isodense mass with patchy calcifications. On MRI examination, the mass manifested hyperintensity on T1-weighted, T2-weighted, and diffusion-weighted images with some signal voids. PET/CT images demonstrated an intensely hypermetabolic mass in the left thigh and hypermetabolic nodules in lungs.CONCLUSION: ASPS should be considered as a possible diagnosis when a slow-growing mass is detected in the soft tissue of the extremities, with hyperintensity and numerous signal voids on T1-weighted, T2-weighted, and diffusion-weighted images and intense F-deoxyglucose uptake on PET/CT. ASPS can have calcifications on CT.

Treatment of a case of primary alveolar soft part sarcoma of the lung: Case report and literature review

Abstract:Alveolar soft part sarcoma (ASPS) is an extremely rare type of soft tissue sarcoma. The primary sites of ASPS are mostly located in the extremities and trunk. Primary pulmonary ASPS is extremely rare. A search of the PubMed® database identified only five cases of primary pulmonary ASPS. This current case report describes the sixth case of ASPS in a 15-year-old male that presented with recurrent headaches. Head computed tomography showed space-occupying lesions in the left parietal lobe. Positron emission tomography-computed tomography confirmed the space-occupying lesions in the left parietal lobe and showed multiple nodules and masses in the two lungs and pleura, which were considered to be low-grade malignant mesenchymal tumours. The case report presents the clinical characteristics, diagnosis and treatment process. Programmed cell death protein 1 monoclonal antibody (sintilimab) combined with a tyrosine kinase inhibitor (anlotinib hydrochloride) achieved a good therapeutic effect, indicating that this combination therapy is worth exploring further. Large-scale prospective studies are needed to explore and develop standardized treatments for ASPS.

Abstract:BACKGROUND: Alveolar soft part sarcoma (ASPS) is a rare soft tissue tumor that usually affects young patients. Because of the rarity of the disease, most reports relating to ASPS are in the form of case reports or small series.METHODS: We performed a retrospective study to evaluate the clinicopathologic features, treatment, outcome and pattern of treatment failure in a consecutive series of patients with localized or metastatic ASPS between 1996 and 2011. Demographics, tumor sizes, sites and extent of disease, treatments provided, progression-free survival, and overall survival were evaluated.RESULTS: A total of 19 patients were identified. The clinical assumptive diagnosis of the first medical examination doctor was benign soft tissue tumor in 5 cases (26%) and benign hemangioma in 4 cases (21%), delaying treatment. The most common location of primary tumor was the thigh. The median diameter of the mass was 55 mm (range, 10 to 130 mm). An R0 resection was obtained in 11 cases. Adjuvant radiotherapy was delivered in 8 cases; postoperative systemic chemotherapy was delivered in 10 cases. Eight out of 15 patients (53%) exhibited metastases either at presentation or later. Median overall follow-up was 54 months.CONCLUSIONS: The treatment principle for alveolar soft tissue sarcoma is massive resection, and when the surgical margin is questionable, radiation treatment can be added for prevention of local recurrence. Also, due to discovery of metastases and local recurrence, even after 5 years of general treatment, outpatient department follow-up is needed, and we need to keep in mind that lung, intracranial, and bone metastases are common.

Abstract:Objective To improve the knowledge,diagnosis and treatment level of pulmonary artery sarcoma (PAS).Methods The clinical data of four patients with PAS confirmed in Sir Run Run Shaw Hospital,Zhejiang University School of Medicine from September 2013 to February 2016 were retrospectively analyzed,and relative literatures were reviewed.Results There were two males and two females,39-58 years old.They all showed chest distress and shortness of breath in their chronic course of the disease.All the patients had different degrees of high D-dimer level.Color doppler ultrasonography showed no abnormity in their deep veins of lower limbs.Echocardiography of three patients revealed echo of clumps in their pulmonary arteries.CT pulmonary angiography examination showed filling defect in the related pulmonary arteries.PET-CT examination of three patients showed that fluorodeoxyglucose intake in the lesions was high.All the patients were misdiagnosed as pulmonary thromboembolism at first,but were confirmed PAS through histopathology after operation finally.Conclusions PAS is a high malignancy with insidious onset.The clinical features and imaging examination have no obvious specificity.The understanding of the disease should be strengthened in clinic,early diagnosis and treatment can prolong the survival time of PAS patients.
Clinical features and prognostic factors of alveolar soft part sarcoma in 15 patients

Abstract:Background and purpose:Alveolar soft part sarcoma(ASPS)is a rare malignant neoplasm characterized by slow growth and indolent behavior,and most of the patients had metastases in diagnoses.This study was to def ine clinical features and assess prognostic factors of ASTS.Methods:Between January 2003 and December 2008,there were 15 patients with ASTS who received treatment and were followed up in the Department of Orthopedic Oncology of Peking University People's Hospital.We retrospectively analyzed the clinical factors of the disease related to prognosis:gender,age,tumor size,primary tumor site,metastatic sites,local recurrence,the roles of postoperative radiotherapy and postoperative chemotherapy.Results:At a follow-up from 7 to 76 months,only 2 patients had local recurrences after excision,but there was a high incidence of pulmonary metastases.Only 6 patients achieved disease free survival,5 patients died of metastases or other complications.The fi ve-year overall survival rates was 41.7%,and the median survival time was 42.0 months.Univariate analysis and Cox regression analysis revealed that tumor size,tumor primary site and metastases at diagnoses were prognostic factors,but not gender and age.Conclusion:Alveolar soft part sarcoma is a high grade malignant soft tissue neoplasm with frequent metastases to lung,bone or brain.Tumor size,primary tumor site and metastases at diagnoses are some of the prognostic factors.Chemotherapy or radiotherapy after surgery have no benefi t to the survival in our analysis.Further prospective studies are needed to clarify the best strategy for the treatment of ASPS.

Abstract:Abstract A 24-year-old man presented with an asymptomatic mass on his left thigh that had been present for 1 month. On examination, a well-defined subcutaneous nodule measuring approximately 2.1 × 2.1 cm was observed on his left thigh, with no apparent epidermal change. Magnetic resonance image showed a hyperintense lesion over the left thigh with contrast enhancement. An incisional biopsy revealed nests of eosinophilic cells displaying nuclear pleomorphism, prominent nucleoli, and positive transcription factor enhancer (TFE)3 expression. A diagnosis of alveolar soft part sarcoma (ASPS) was made. ASPS, a rare sarcoma primarily occurring in young patients, typically affects deep soft tissues of the buttock and thigh. While it shows a relatively indolent clinical progression, ASPS often indicates a poor prognosis due to late-stage metastases upon diagnosis. Microscopically, ASPS is characterized by uniform nests of polygonal tumour cells within fibrovascular septa and vascular channels, displaying distinctive cellular dyscohesion, resulting in the characteristic ‘pseudoalveolar pattern’. The cells have defined borders, abundant eosinophilic to granular cytoplasm, and an epithelioid appearance. ASPS is caused by a specific unbalanced translocation, der(17)t(X:17)(p11;p25), forming an ASPSCR1–TFE3 fusion gene. In this patient, further imaging was arranged for tumour staging, which confirmed localized disease without lymph node or distant metastasis. The patient underwent wide excision and local flap surgery and has remained recurrence free during a 4-year follow-up. This case emphasizes the importance of recognizing the clinical features of ASPS in young patients and considering skin biopsy and image studies for long-standing subcutaneous nodules, as this can provide an early diagnosis and improve patient outcomes.

Abstract:Objective To investigate the clinicopathological characteristics,immunophenotype,molecular genetic feature and differential diagnosis of alveolar soft part sarcoma(ASPS) of cervix.Methods Two cases of cervical ASPS were reported and studied with routine HE stain,MaxVision immunohistochemical stain and fluorescence in situ hybridization(FISH).Results The two cases of cervical ASPS showed a local slowly growing neoplasm.Microscopically,the tumor cells arranged in alveolar pattern and nests,separated by fibrovascular trabeculare.The cells were polygonal to round,and larger with abundant finely granular eosinophilic cytoplasm.Immunohistochemically,the tumor cells expressed TFE3.FISH assay showed that the tumor cells had TFE3-ASPL gene fusion.Conclusions The cervical ASPS is extremely rare,with a histopathological features similar to those occurred in other locations.TFE3 is known as a specific immuno-marker of ASPS and the genetics shows TFE3-ASPL gene fusion.

Abstract:BackgroundAlveolar soft part sarcoma (ASPS) is a rare mesenchymal malignancy. ASPS usually occurs most commonly in the deep soft tissues of the thigh and buttock or the head and neck regions.ASPS that originate from the uterine corpus are even more rare, with only 10 previous cases reported in the English literature.Case presentationIn our case, the alveolar features were completely lost and the tumour shows a solid, non-alveolar pattern and the nuclei have marked variation in nuclear size, and multinucleation. The correct pathological diagnosis has been made by immuno- histochemical and ultrastructural features, which rvealed overexpression of TFE3 and peculiar cytoplasmic crystalline inclusions.In this paper, an additional case of primary ASPS of uterine corpus is reported with immunohistochemical, ultrastructural study and review of literature in the effort to delineate its clinical and pathological features. In this unusual site, the diagnosis can be problematic because ASPS can mimic other primary or metastatic uterine neoplasms.ConclusionsThus, in this unusual presentation an essential diagnostic marker is the nuclear over-expression of TFE3 as well as ultrastructural study, which reveals the presence of peculiar cytoplasmic crystalline inclusions.
