Abstract:The aim of this study is to assess the relationship between somatosensory functional changes and inferior alveolar nerve (IAN) exposure after impacted mandibular third molars (M3M) removal. We recruited 35 patients who underwent impacted M3M extraction near the IAN. The M3Ms were extracted by combined endoscopy, piezosurgery, and contra-angle high-speed turbine handpiece. All IAN canal perforations and exposed regions were recorded and measured by endoscopy after extraction and on cone-beam computed tomography (CBCT) images before extraction. The patients were followed up 1, 7, and 35 days after surgery. A standardized quantitative sensory testing (QST) battery was performed on the lower lip skin. All of 35 cases had exposed IAN on CBCT images, 5 of which had no exposed IAN under endoscopy. For the other 30 cases, the endoscopy-measured IAN length and width were shorter than the CBCT measurements (P < 0.001). The warm and mechanical detection thresholds (MDT) on the operation side were significantly higher than the contralateral side after surgery (P < 0.05). Thermal sensory limen, MDT, and cold pain threshold were strongly correlated with the exposed IAN length and MDT also with the exposed IAN width one day after surgery. In conclusion, it was found that not all exposed IAN in CBCT images were real exposure after surgery. The intraoperative exposed IAN endoscopic measurements were smaller than by CBCT and strongly correlated with some QST parameters.
Retrospective evaluation of sensory neuropathies after extraction of mandibular third molars with confirmed "high-risk" features on cone beam computed topography scans

Abstract:Objectives: The objective of this study was to identify the incidence of inferior alveolar nerve (IAN) injury after extraction of mandibular third molars with confirmed "high-risk" features on cone beam computed topography scans and establish any risk factors that significantly increase the chances of sustaining an IAN injury. Study design: This study is a retrospective analysis of outcomes after surgical extraction of 500 mandibular third molars over a 5-year period. All teeth showed signs of contact with or compression of the IAN on cone beam computed topography scans. Results: The overall incidence of IAN injury was 6.6%, with permanent neuropathies accounting for 1.8% within this high-risk cohort. Statistically significant factors shown to increase the risk of nerve injury included increasing age (P = .002), compression of the nerve evident on cone beam computed topography scan (P = .005), and buccal or interradicular position of the nerve (P = .042). Conclusions: Results indicate a low incidence of IAN injury, particularly given the high-risk nature of all teeth that were extracted. It highlights the benefits of cone beam computed topography scans in surgical planning. These data are an important contribution to the existing literature and valuable in the consent procedure for patients undergoing surgical removal of mandibular third molars in contact with the IAN.
Removal of Long-Term Broken Roots Displaced into the Maxillary Sinus by Endoscopic Assistant

Abstract:PURPOSE:This report presented a patient with 2 long-term broken roots displaced in left maxillary sinus. The residual root fragments made the patient uncomfortable in both mind and body and interfered with prosthodontics work. The application of endoscope combined with piezoelectric device both helps in removing the broken roots successfully with minimally surgical injury and preserves the residual alveolar bone.METHODS:Computed tomography scans and 3-dimensional reconstructions located the broken roots. A 1.0 cm × 1.5 cm rectangle bone window on anterolateral sinus wall was opened by a piezoelectric device to place the endoscope and forcep into sinus. Two broken roots could be observed clearly via a endoscopic screen. They were removed by a mini goblet forcep completely and efficiently. A whole bone lid was replaced with a biological membrane to help repair bone defect after removing procedure.RESULTS:The operation is about 20 minutes with endoscope and piezoelectric device helped to save a lot of time and provided excellent visual surgical field. Main postoperative adverse effects were swelling, numbness, and temporal no-vitality for the first premolar (24). Three months later, computed tomography shows the Schneiderian membrane thinned to around 0.8 mm. The bone lid is on its position and starts to perform synostosis. The 24 tooth is still dysesthetic and needs time to recover.CONCLUSIONS:Endoscopic surgery combined with a piezoelectric device has obvious advantage of minimizing surgical injury and providing excellent visibility of surgical field when removing long-term foreign bodies in maxillary sinus. It is efficient and protects the residual alveolar bone.
Evaluating the Risk of Post-Extraction Inferior Alveolar Nerve Injury Through the Relative Position of the Lower Third Molar Root and Inferior Alveolar Canal.

Abstract:The aim of this study was to introduce a method to evaluate the risk of inferior alveolar nerve (IAN) injury following the extraction of impacted lower third molars. Two hundred impacted lower third molars adjacent to the IAN were evaluated. These were divided into four classification groups according to preoperative cone beam computed tomography (CBCT) findings: AR, apical region; LT, lateral region of the tapered root; LE, lateral region of the enlarged root; AE, adjacent to the enlarged root. All teeth were dislocated along the long axis or arc of the root by tooth sectioning technique and extracted by a single surgeon. The primary outcome variable was postoperative neurosensory impairment of the IAN. The χ2 test was used to evaluate differences in postoperative IAN injury between the classifications. Logistic regression analysis was used to evaluate the risk factors for postoperative IAN injury. The overall incidence of postoperative IAN injury was 7%. Specifically, most injuries involved classification AE (AE 36%, LE 8.6%, LT 3.6%, AR 0%), and the difference was statistically significant (P< 0.05). Logistic regression showed that classification AE was the only risk factor for postoperative IAN injury (P< 0.001). According to preoperative CBCT, the risk of postoperative IAN injury is higher when the IAN is adjacent to the enlarged part of the root.

Abstract:The present study aimed to evaluate the degree of nerve injury on inferior alveolar nerve (IAN) by different implant drills resulting from direct canal intrusion into inferior alveolar canal (IAC). A cadaveric study involving 7 human mandibles was performed to evaluate mechanical injury of canal enclosed IAN resulting from different drills. In group 1, osteotomies were created using different drills with 1mm of intracanal intrusion, simulating accidental drill intrusion into canal. In group 2, drilling was stopped when the tip has breached into IAC, limited by tactile feedback of operator. The depth and width of nerve defects were assessed using optical coherence tomography. A significant difference in defect depth was found ( p <0.001) in group 1. A sinus lift reamer inflicted the least damage (0.068±0.022mm). It was also found that the mean defect depth was significantly different when a twist drill was used ( p =0.016). Sinus lift reamer can be used safely for osteotomy preparation in mandible when bone height is limited or when radiographic visualization of canal is poor. Bone corticalization around IAC does not provide adequate protection for IAN in the event of accidental intracanal intrusion.

Abstract:The aim of this study is to provide reliable support for the choice of extraction of high inverted mesiodens, and minimize the operation injury for patients. The operation statistics, life quality of postoperative patients, and the operative injury/recovery were compared and analyzed. The laser Doppler blood flowmetry, laser speckle contrast imaging, and electric pulp testing were explored to detect the postoperative pulp and gingiva blood supply of adjacent teeth. For the clinician's primary concerns, the surgical time, the volume of osteotomy, and the amount of bleeding in the labial approach group (The p values are 0.0001, <0.0001, and 0.0131, respectively.) and intranasal approach group (All p values were <0.0001.) were significantly less than that in the palatal approach group. However, from the patient's perspective, the postoperative swelling in the labial approach was far more than that in the intranasal approach group (p = 0.0044), with unsurprisingly lower satisfaction (p < 0.0001). There were no significant differences in pulp and gingival blood supply of adjacent teeth and jaw development. Trauma was manageable in all patients. Under the inclusion and exclusion criteria of this study, the extraction of mesiodens by the intranasal approach achieved a delicate balance between reducing surgical trauma and optimizing postoperative recovery, which was of concern and merited further study.

Abstract:Objective: An altered sensation during endodontic treatment can occur due to the extrusion of endodontic materials. This study aims to discuss intentional replantation to address paresthesia resulting from an endodontic file penetrating the inferior alveolar nerve canal and provide a protocol for managing nerve injuries in such incidents. Case presentation: A 12-year-old girl developed paresthesia when an endodontic file separated and was inadvertently pushed through the apical foramen into the inferior alveolar nerve canal during root canal treatment of the mandibular left first molar. A CBCT scan revealed the file penetrating the canal towards the inferior border of the mandible. After considering the treatment options, intentional replantation was deemed suitable. The tooth was atraumatically extracted and preserved in sterile saline. The surgeon then carefully cleaned and irrigated the socket. The radiographic assessment confirmed successful file removal from the socket. The root ends were resected, and retrograde preparation and obturation were conducted using ultrasonic tips and mineral trioxide aggregate. The tooth was then replanted into the socket. Successful replantation was confirmed by tooth stability and an audible click. The patient was prescribed antibiotics and steroids. After completion of the endodontic treatment, a stainless steel crown was cemented. The successful intentional replantation procedure resulted in rapid improvement in the patient's condition. The normal sensation had been restored, indicating nerve recovery. At the 15-month follow-up, periapical bone healing and the eruption of the adjacent second molar were observed, affirming the treatment protocol's overall success. Conclusion: Prompt intervention and immediate intentional replantation facilitated direct inspection of the separated file within the socket. Collaboration between an oral maxillofacial surgeon and an endodontist ensures expedited and targeted treatment, leading to favorable outcomes.
Risk Factors Associated with Inferior Alveolar Nerve Injury after Extraction of Impacted Lower Mandibular Third Molars: A Prospective Cohort Study

Abstract:BackgroundInferior alveolar nerve (IAN) injury is one of the complications of impacted lower mandibular third molar (LM3) extraction. Given the unknown prognosis of IAN injuries and limited treatment options, it is critical to understand the risk factors of IAN injury before LM3 extraction.PurposeThe study purpose was to identify risk factors associated with IAN injury after LM3 extraction.Study design, setting, sampleThis was a prospective cohort study including patients who underwent LM3 extraction from May to December in 2021 at authors’ institution. Patients with systemic diseases, previous maxillofacial surgeries or sensory abnormalities were excluded.Predictor variable(s)The predictor variable is composed of several risk factors. The variables were grouped into four categories: demographic, radiographic, procedure-related, and surgeon experience.Main outcome variable(s)The outcome variable was postoperative neurosensory disturbance coded as present or absent and was measured as one-month (transient) and one-year (permanent).CovariatesNot applicable.AnalysesThe measurement data were represented by mean and standard deviation (SD). The association of each variable with the presence of IAN injury was tested by the chi-square test. Statistical significance was accepted at p < 0.05.ResultsThe study sample was consisted of 705 patients (37.0% male) with an average age of 28.51 ± 6.51 years. A total of 17/705 (2.4%) and 4/705 (0.57%) patients developed transient and permanent IAN injury. The results demonstrated that the following factors were associated with higher rates of transient injury: use of chisels during surgeries (6.4%; 95% CI: 2.7-12.3; p=0.02; RR=11.4), LM3s located below the IAN canal (8.7%; 95% CI: 4.3-15.7; p<0.01; RR=7.3), compressed contact between LM3s and the IAN canal (36.4%; 95% CI: 12.3-78.2; p<0.001; RR=25.4), and not using corticosteroids after surgeries (3.8%; 95% CI: 1.9-6.5; p=0.03; RR=3.1). The only factor associated with permanent injury was compressed contact between LM3s and the IAN canal (18.2%; 95% CI: 2.2-62.3; p<0.001; RR=48.2).Conclusions and RelevanceClose proximity between LM3s and IAN canal, and the use of chisels, increase the risk of transient IAN injury. Corticosteroid treatment may promote nerve recovery. Compressed contact between LM3s and IAN canal is the only risk factor for permanent injury.

Abstract:In case of traumatic injuries to the anterior teeth, fragment reattachment is considered a minimally invasive technique. Re-attaching the same tooth fragment, possesses the advantage, of biomimicry by complete restoration of form, function, texture, lustre, and translucency. Due to advancements in adhesive dentistry, simple re-attachment to complex fragment rehabilitation have become predictable and reliable. Various techniques have been proposed for fragment reattachment based on the extent of injury, type of injury, condition of the fragment, and time- lapse. This case report demonstrates the endo-aesthetic rehabilitation of a complicated crown root fracture using a fiber post on lateral incisor with fracture line on the cervical 1/3of the labial aspect, running obliquely and apically towards the palatal surface after 1 year follow-up.
A Novel Approach for Orthodontic Extrusion Prior to Intentional Replantation: A Case Report

Abstract:Aim: This case report aimed to present a novel surgical technique involving orthodontics luxation of the offended tooth using miniscrew prior to performing intentional replantation (IR) to remove an extruded separated instrument that injured the inferior alveolar nerve (IAN). Background: Intentional replantation is a dental procedure that involves extracting a tooth, managing the root canal, and then reinserting the tooth back into its socket. This procedure is typically used as a last resort when other treatment options have failed or are not possible. Intentional replantation can be successful in preserving the tooth and preventing tooth loss, but it is important to note that there are risks involved. Maintaining the viability of the periodontal ligament (PDL) is a pivotal step to achieve a favorable outcome. Case description: A 15-year-old female patient came to the clinic complaining of electric-like pain in the left mandibular posterior area that radiated to the left ear and sometimes caused a headache. Upon clinical and radiographic evaluation, extruded endodontic file from the mesial root of the left first molar that penetrated the IAN canal was noticed. Nonsurgical root canal retreatment was performed, which failed to retrieve the separated file. Orthodontics luxation of the offended tooth was done 2 weeks before the surgical intervention using a miniscrew to induce PDL inflammation, which increased the tooth mobility and PDL volume, facilitating the atraumatic extraction and reduced the risk of complications, such as root resorption and ankylosis. Then, IR was performed, and the extruded file was successfully retrieved. Three months follow-up showed complete recovery of the endodontics-related IAN injury symptoms. Preapical radiographic evaluation and cone-beam computed tomography scan showed complete healing of the periapical radiolucency/area of low density and complete formation of the PDL space and lamina dura around the resected roots. Conclusion: This novel approach using a miniscrew suggests a noninvasive technique that minimizes the damage to the offended tooth surrounding tissues as well as minimizing the morbidity of the adjacent teeth and the vital anatomical structures. Clinical significance: Multidisciplinary comprehensive preplanning of complicated cases is essential to maximize treatment efficiency. The orthodontic extrusion facilitates the extraction process that helps in preserving the PDL, and ultimately increases the survivability of the teeth. ' Modified Technique for Preservation of Inferior Alveolar Nerve During Mandibulectomy

Abstract:Background: The purpose of this article is to introduce the modified technique of preservation of the inferior alveolar nerve (IAN) during mandibulectomy for a benign lesion. Methods: Five cases of osteofibrous hyperplasia and 3 cases of centricity osteomyelitis were included. During surgery, the IAN was marked using a planned cutting guide. Using an oscillating saw, the depth of the osteotomy along the IAN was controlled until the bone cortex was cut through. After splitting, the bony section was removed, leaving the neurovascular bundle intact. The sensation of the lower lip was evaluated using current perceptive threshold testing during follow-up. Results: After follow-up for 6-27 months, no recurrence or secondary deformity was found. One patient had severe sensory disturbance. Conclusion: With the use of a cutting guide and osteotomy tricks, mandibulectomy with preservation of the IAN can be accurately performed.
Outcome Assessment of Three Different Methods of Root-end Preparation and Filling Materials in Endodontic Surgery: A Comparative Clinical Prospective Study

Abstract:Introduction: The technique of endodontic surgery had evolved tremendously in the recent years with introduction of new instruments and materials. Aim: This study aims to compare the clinical outcome of endodontic microsurgery using three different techniques with three different root-end filling materials for 16 months. Materials and methods: A total of 45 maxillary incisors indicated for root-end surgery were selected for the study. They were enrolled into three groups, i.e. Group A (traditional [TRS]/heat burnished gutta-percha), Group B (concave [CON]/Retroplast), and Group C (cavity/DiaRoot BioAggregate), of 15 teeth each. The clinical and radiographic outcome was recorded at 1, 6, 12, and 16 months using various criteria. Statistical analysis: The data were analyzed by one-way analysis of variance and Tukey's post hoc test using SPSS V. 21 software (IBM Corp., Somers, NY, USA). Results: All patients had uneventful healing at the final follow-up. Radiological intratime analysis concluded a highly high significant (P < 0.0001) decrease in the size of radiolucency between the three groups at the third recall visit. Intertime analysis recorded no significant decrease in radiolucency between Groups A and B, a significant decrease in B and C, and a highly significant decrease between Groups A and C at 12 months. Conclusions: There was no significant difference in the clinical outcome after endodontic surgery when comparing TRS/heat burnished gutta-percha, CON/Retroplast, and cavity/DiaRoot BioAggregate techniques at 16 months. However, cavity/DiaRoot BioAggregate resulted in significantly rapid and predictable healing at 12 months.
Retrieval of Root Fragment in Maxillary Sinus Via Anterolateral Wall of the Sinus to Preserve Alveolar Bone

Abstract:Purpose: The objective of this study was to present a functional surgery for retrieval of displaced root fragments from the maxillary sinus while preserving the alveolar bone.Materials and Methods: Twenty-one patients with associated root fragments displaced into the maxillary sinus after tooth extraction were collected and reviewed retrospectively. All patients included in the study were subjected to surgical removal of the displaced root by the use of piezosurgery. In case the root fragment was barely detectable, endoscopy would be helpful.Results: Eleven roots were removed via small windows and 10 roots via large windows. Endoscopy was used in 2 patients. Granulomatous tissues were found around 6 root fragments, and cyst formation was found in 1 patient. Only 1 patient had sinusitis temporarily. No patients developed complications such as facial paresthesia, facial asymmetry, or infection.Conclusions: The functional surgery for the retrieval of a displaced root from the maxillary sinus is recommended owing to quick recovery, preservation of alveolar bone, and minimal complications.

Abstract:The extraction of wisdom teeth with mandibular impact frequently results in complications including damage to the inferior alveolar nerve (IAN) and malformations of the bone. The objective of this research endeavour was to assess the efficacy of low‐level laser therapy and concentrated growth factor (CGF) in facilitating nerve recovery and wound healing in such instances. A total of thirty‐one patients (mean age 27.52 ± 5.79 years) who presented with IAN injury after extraction were randomly assigned to one of three groups: control group (which received oral mecobalamin), CGF group (which received CGF gel applied to the extraction sockets) and laser group (which received low‐level lasers (808 nm, 30 mW, 10 J/cm2)) at the extraction site. Patients' recovery from IAN paresthesia was evaluated seven times over the course of 14 days utilizing visual analogue scale (VAS) and the pinprick test (PP). At multiple intervals following surgery, periodontal probing and bone level measurements were utilized to assess the recovery of both soft and hard tissues. The findings revealed that, compared with the control group, both the CGF and laser treatment groups exhibited a markedly greater improvement in VAS scores and wound healing of soft tissues, as well as in PP results (p

Abstract:A fractured instrument (FI) in the root canal is a common complication during root canal therapy. Under current medical conditions, instrument separation cannot be completely avoided because of the complex morphology of root canals and the limited surgical field and operating space of the surgeon. FIs, especially those broken in the apical third of the canal, render it difficult to completely remove infection in the root canal, where the residual infection can easily develop into reinfection. The removal of FIs is therefore the preferred option for the majority of clinicians in such cases. However, root canal preparation instruments are frequently fractured during treatment because of the complex root canal morphology, such as curvature or severe calcification, which further increases the difficulty of instrument removal. In the present case, a 41-year-old female patient complained of worsening pain in the left maxillary first molar for 3 days. This patient had been treated at another hospital 2 years earlier, but the discomfort persisted after treatment. Preoperative periapical radiography revealed suspected FIs at the apical third of the mesiobuccal (MB) root and the middle third of the distal buccal (DB) root, underfilling of the palatal (P) root canal and large hypodense areas surrounding the periapical region of all roots. Next, the micro-ultrasound technique was used to remove the FI in the DB canal; a bypass through the second MB canal (MB2) was created to fill the apical stop of the MB root and the P canal was retreated. The therapeutic effect of the bypass technique was assessed by comparing bypass treatment and removal treatment for endodontic FIs. The 9-month and 27-month follow-ups revealed that the periapical inflammation surrounding the DB root treated after removal of the FI and the MB root treated by bypass was significantly controlled compared with that before the operation. In addition, the present report reviewed the research progress in bypass and removal techniques, focusing on the difficulties and key points of successful root canal therapy.
Minimally invasive extraction of impacted mandibular third molar compressing the inferior alveolar nerve: a comparison of three techniques

Abstract:PURPOSE:Extraction of impacted mandibular third molar compressing inferior alveolar nerve (IAN) is highly related to inferior alveolar nerve injury.The aim of this study was to compare the incidences of IAN injury in orthodontic traction,coronectomy and piezosurgery for the impacted mandibular third molar removal.METHODS:After examination of panoramic radiography and cone-beam computed tomography (CBCT),sixty patients with impacted mandibular third molar compressing the IAN were enrolled in this study.Orthodontic traction,coronectomy and piezosurgery were carried out in 20 patients,respectively.Signs of postoperative neurosensory impairment were recorded.RESULTS:No sign of neurosensory impairment was observed in patients undergoing orthodontic traction or coronectomy.One patient (5％) undergone piezosurgery had postoperative lower lip numbness,and recovered within 1 month after drug treatment.CONCLUSIONS:Orthodontic traction,coronectomy and piezosurgery can reduce the incidence of nerve injury during removal of impacted mandibular third molar compressing IAN.
Use of an intraoperative navigation system for retrieving a broken dental instrument in the mandible: a case report

Abstract:BackgroundA fracture of root canal instruments, with a fractured piece protruding beyond the apex, is a troublesome incident during an endodontic treatment. Locating and retrieving them represents a challenge to maxillofacial surgeons because it is difficult to access due to the proximity between the foreign body and vital structures. Although safe and accurate for surgery, radiographs and electromagnetic devices do not provide a precise three-dimensional position. In contrast, computer-aided navigation provides a correlation between preoperatively collected data and intraoperatively encountered anatomy.However, using a navigation system for mandible treatment is difficult as the mobile nature of the mandible complicates its synchronization with the preoperative imaging data during surgery.Case presentationThis report describes a case of a dental instrument breakage in the mandible during an endodontic treatment for a restorative dental procedure in a 65-year-old Japanese woman. The broken dental instrument was removed using a minimally invasive approach with a surgical navigation system and an interocclusal splint for a stable, identically repeatable positioning of the mandible. Using the three-dimensional position of the navigation probe, a location that best approximated the most anterior extent of the fragment was selected. A minimally invasive vestibular incision was made at this location, a subperiosteal reflection was performed, and the foreign body location was confirmed using a careful navigation system. The instrument was carefully visualized and extruded from the apical to the tooth crown side and was then removed using mosquito forceps through the medullary cavity of the crown side of the tooth. Follow-up was uneventful; her clinical course was good.ConclusionsThe use of a surgical navigation system together with an interocclusal splint enabled the retrieval of a broken dental instrument in a safe and minimally invasive manner without damaging the surrounding vital structures.
Prediction Of Inferior Alveolar Nerve Injury In Complicated Mandibular Wisdom Teeth Extractions: A New Classification System

Abstract:Purpose: To introduce and validate a new classification system for the inferior alveolar nerve (IAN) injury based on the proximity and positional relation between the tooth root and the inferior alveolar canal (IAC). Methods: In this retrospective study, consecutive patients, who underwent piezosurgery extractions of complicated impacted mandibular third molars from March to November 2014, were reviewed. Only teeth in close proximity to the IAC in orthopantomogram (OPG) were included. Then, teeth were classified according to the proximity of the root to the IAC (grade I: contacting; grade II: compressing; and grade III: penetrating) in both OPG and Cone-beam CT (CBCT), as well as, the relation to the IAC (class I: on the lingual side; class II: above the IAC; class III: on the buccal side; and class IV: roots surrounding the IAC) in CBCT. The incidence of the IAN injury in all patients was recorded. chi(2) test and Fisher test was used to analyze the data. Results: 92 teeth (84 patients) in close proximity to the IAC were extracted and recorded. Of these, 33.70%, 28.26% and 38.04% were in grades I, II, and III, respectively, based on OPG; while, 36.95%, 33.70% and 29.35% were in grades I, II, and III, respectively, based on CBCT. Furthermore, the percentages of teeth in classes I, II, III and IV were 31.52%, 55.43%, 11.96%, and 1.09%. 7.61% of all the extractions led to the IAN injury. There was a positive correlation between the degree of root proximity to the IAC and IAN injury (P<0.05) with the highest incidence being in class III (P<0.05). Conclusion: The novel classification system seems well reproducible, facilitating the recognition of patients with higher risk of IAN injury during extraction of complicated lower wisdom teeth.
