Cancer Storytelling: A Study of Well-Being Expressions Made by Patients

Abstract:Many cancer patients tell their story to help others. This paper reports a study of 160 such stories, which were processed using a grounded theory methodology. The stories were rich in emotional words and in metaphors and these, along with self-talk and meaning-for-life, provided the commonest indications of the patients' well-being. Cancer patients, through the process of telling their story, help themselves and enhance their well-being. The method of active listening is described, with associated risks for professionals.

Abstract:Communication is an integral part of nursing practice—with patients and their relatives, other nurses and members of the healthcare team, and ancillary staff. Through interaction with the 'other', language and silence creates and recreates social realities. Acceptance, rejection or modification of social realities depends on what is expressed and by whom. Narratives that are offered can tell of some experiences and not others. Some nurses choose to be silent while others are silenced. In nursing situations recognising and allowing silence to speak is a challenging but uniquely personal experience that embraces reflection in and on experiences, practice and self as a person and a professional. If enabled and truly heard, silence can speak more loudly than the hubbub of daily practice, allowing us to collectively question and challenge inherent assumptions and biases as professionals, and as a profession. Through a microcosm of Newly Graduated Nurses' lived experiences of nursing situations and expressions of silence individuals' discomfort and private efforts to ascribe meaning to experiences are reflected on. Returning to silence is to return to a constant process of professional transformation that can enable ways of knowing and being that can reform our profession from within and enable us to cast off shackles that bind us to a shameful cultural underbelly.

Abstract:OMEGA - Journal of Death and Dying, Ahead of Print. This study examines adult children's loss of a parent through final conversations and communal coping. By looking at survivors' experiences through the lens of the opportunity model for presence (OMP) during the end-of-life (EOL) process, a model that provides a path of engagement leading to a good or bad death and consequent bereavement processes, we quantitatively test the relationships between two communication constructs before and after death of a parent. While final conversations as one construct did not lead to appraisal or action coping, results illustrate that final conversation topics of identity, instrumental, love, and everyday talk, were significantly related to shared appraisal. Instrumental talk and love were significantly related to joint action. Joint action and shared appraisal together led to participants engaging in all three types of coping. Final conversations as a whole, was important for bereavement outcomes, whereas communal coping's role in these relationships was convoluted.

Abstract:We all long for a sense of community in our professional lives where we can bring our spirit, passion, and whole selves to work. When we work in environments of tribal warfare and clans carrying out subversive activities, we will choose to leave our souls at home to protect them from the toxicity of the work environment. For anyone who has been cared for by nurses who have left their soul at home, you have experienced the outcome of negative environments that cannot have effective conversations. Recently a nurse told about her experience of realizing one day that she had the right to be happy at work so that she could more effectively take care of her 5-year-old daughter when she returned home. When she saw this as a right, she made a decision to leave her organization and look for a place that valued conversations and harmony rather than aggression and lack of trust. The only way to improve the world is through relationships, and conversations are the prelude to creating that change. Having the courage to reach out and start the dialogue knowing full well that it might be very messy is the only way to end the fragmentation and loss of community we experience in our organizations. Wheatley (2002) describes the experience of "...sacred as a feeling that I belong here" (p. 133). In the process of conversations, we can explore and appreciate our differences but also feel more connected and one with each other as a result of having the courage to converse. When we see each other as equals, we also free ourselves to open up to new learning and relationships which can lead to excellence in clinical outcomes. It's all about the courage of conversations.$

Opening the Door: Rethinking "Difficult Conversations" about Living and Dying with Dementia

Abstract:This essay looks closely at metaphors and other figures of speech that often feature in how Americans talk about dementia, becoming part of cultural narratives: shared stories that convey ideas and values, and also worries and fears. It uses approaches from literary studies to analyze how cultural narratives about dementia may surface in conversations with family members or health care professionals. This essay also draws on research on a notable social effect of legalizing medical aid in dying: patients may find it easier to bring up a range of concerns, regardless of whether they have any interest in hastening their own death. The essay proposes that health care professionals rethink an idea prominent within their own culture: that conversations about the end of a person's life are inherently difficult. This framing may make it hard for people facing dementia to bring up their concerns about what lies ahead. We suggest a different way to think about these conversations, using the metaphor of "opening the door" to represent inviting a patient to bring up issues of deepest concern.
Stories as integrated patterns of knowing in nursing education

Abstract:The primary aim of this qualitative study was to explore the scholarly benefits of storytelling as a means to promote students' understanding and integration of both the art and science of nursing. As one of the oldest methods of communication, storytelling has been used in a variety of ways in nursing education. Despite this myriad of literature, there were no studies found that analyzed the actual content of students' stories. Using Carper's (1978) Fundamental Patterns of Knowing as a guiding framework for narrative analysis, twenty-five personal stories written by junior level nursing students were examined for evidence of empirics, ethics, esthetics, and personal knowing. Results indicated students were able to not only integrate art and science within their experiences but were also able to see themselves one day as caring nurses. Moreover, Carper's patterns add credibility to storytelling as a worthwhile teaching strategy with benefits to both students and faculty.
Storytelling near the end-of-life: Contingency modes in conversations with relatives of patients receiving palliative care: A qualitative study

Abstract:This article explores the role of end-of-life conversations between palliative home-care patients and their relatives in shaping the bereavement process. Through interviews with 17 bereaved relatives, four distinct modes of contingency were identified: denial, acknowledgment, accepting, and receiving. While some individuals remained in a single mode, others transitioned through all four, suggesting a deeply personal process independent of external influences. The findings underscore the importance of healthcare providers facilitating these conversations, as they can help relatives process emotions and support their grieving. By addressing these modes, caregivers can aid relatives in becoming compassionate advocates for patients nearing death. The study highlights the need for further research into strategies for supporting relatives in navigating these modes, potentially enhancing bereavement care practices.

Abstract:Storytelling is a deeply human act-ancient, universal, and familiar. But in this data-driven "Information Age," is there still room for narrative? As oncologists, we often feel overclocked, increasingly forced to microtask and live by the business world's mantra of "faster, cheaper, better," while our carefully honed humanistic clinical skills are coldly commoditized into Relative Value Units, our aptitude measured with tools that emphasize form over substance. In such an environment, can we still afford the inefficiency of anecdotes? Some might argue that we no longer need patient and physician narratives to inform care; instead, accurate molecular subtyping of tumors, good pharmaceutical agents, and rigorous clinical trial results are enough. Yet stories and reflections about being a doctor or being a patient remain important and instructive even in this era of molecular cancer medicine, even in the face of our increasing reliance on high-tech diagnostics and narrowly targeted smart therapeutics. Amid the pressures of rapidly changing practice patterns and the daily emotional challenges of working with seriously ill and dying patients, the acts of reflection and storytelling can also help keep us sane. Here I discuss the persistent importance of narrative in the context of the "Art of Oncology" forum of the Journal of Clinical Oncology.
Caring in Creative Writing—A Case Study

Abstract:This study focuses on identifying natural caring for the human being through creative writing. It offers increased understanding of creative writing through a case study. It illustrates caring in written texts created by one participant during 17 years and in an interview. The results of the study show three themes of natural caring in an analysis of creative writing materials: caring in relation to oneself, caring in relation to the world one lives in, and caring in relation to one's own time perspective in life. These findings may help to identify caring through creative writing within or outside clinical practice.

Abstract:Background Teaching nursing students to become relational practitioners requires theoretical approaches and strategies that engender personal and aesthetic knowing. These qualities closely parallel those that define relational practice. The use of creative self-expression in supporting the development of student capacity for relational practice offers a viable approach. Purpose To learn how nursing students’ engagement in creative self-expression activities may impact the construction of their professional identity and capacity for relational practice as novice nurses. Method Clandinin and Connelly's narrative inquiry approach was used to explore nursing students’ experiences of learning how to become relational practitioners. Four new nurse graduates engaged in a follow-up focus group using Schwind's narrative reflective process to discuss the impact of a relational practice workshop series. Findings Bronwyn's story became the composite of the four participants. Two key narrative patterns emerged: being in service of others and being knowledgeable collaborative leaders. These entailed an intentional engagement in relationships with patients, which required attention to the co-constructed relational space. The creative approaches used to facilitate students’ learning informed their awareness that led to their transformation. Implications Educating future nurses who are relational, person-centered practitioners requires a holistic approach to teaching/learning which also includes creative self-expression.

A performative and poetical narrative of critical social theory in nursing education: an ending and threshold of social justice

Abstract:In this article, a poetical and performative narrative is shared to examine how the use of stories to critically self-reflect on oppression facilitates an understanding of critical social theory in nursing education and impacts social justice. A fusion of prose with a poetical narrative is employed; the latter is reserved to capture the immediacy of personal, emotive, and embodied storied experiences. This deeply intimate and dialogical story begins with a pedagogical experiment created to facilitate nursing students' understanding of critical social theory. Drawing upon Paulo Freire's work, the nursing teacher in a professional development course attempted to deconstruct power relations and cultivate an open and safe learning environment by sharing a poem that depicts her oppression. Students then anonymously wrote a word/statement about their oppression. The teacher created a composite poem from students' words and shared it with the class; it was a powerful moment that highlighted their shared humanity. As a way to further explore stories and consider how to preserve these words, a small group of students and the teacher formed the 'the oppression group'. Towards the end, we conclude an unfinished story by realizing that the chains of oppression are loosening and humanity is surfacing. There is still a camouflaging of an authentic self. There are still stories to be told. The group is not yet certain if a social representation of an authentic self is possible and if all stories can be told. It has become apparent that the personal can play out in social justice as enacted in the classroom between teacher and students and provides an entry point into the development of the capacity to be social agents in nursing. The group simultaneously concludes the story with both an ending and a threshold of social justice.
"Losing My Self": A Poet's Ironies and a Daughter's Reflections on Dementia

Abstract:What can we learn about the experience of dementia and about ways of being human when a poet describes her forgetting? My mother, the poet Shirley Kaufman, died in 2016 at the age of 93. She had dementia for many years. By her mid-80s, many of her poems probed the experience of forgetting, including questions about the nature of self, memory, and thought, and the location of the past and the future. Her experience of forgetting revealed in her ninth and last collection, the widely acclaimed Ezekiel's Wheels (2009), serves as my entry point for an exploration of the self as it becomes dislocated in time yet remains attuned to and consciously aware of that dislocation. That dual quality of my mother's experience is the central, ironic tension explored in this essay. An intimate ethnography, this essay connects my mother's late-life poetry with my own experience, both as a daughter who watched her mother's state of mind unfold and as an anthropologist with extensive experience studying aging, late-life identity, illness, and the culture of medicine. Through the insights of my mother's poetic language I show how dementia can expose ironic features of selfhood, communication, and life itself that are worth our discernment. My hope is that my analysis may focus our gaze on the inherent tension in this form of life and teach us something new about identity, memory, and what is shared among us, those with and without the condition we call dementia.

Abstract:Arts-based learning in the context of clinical placement is rarely discussed in the literature, yet storytelling is potentially a valuable tool for students to reflect on the multi-dimensional experience. Clandinin and Connolly’s approach to narrative inquiry provided the methodology to explore students’ experiences of storytelling as a learning strategy. Narrative inquiry is a relationship-based methodology, used to study the nature of people’s experience. The approach is like that of clinical facilitators who work collaboratively with students to understand and provide mentorship through placement experiences. Creative writing and storytelling sessions with students were conducted throughout placement. Data included creative writing, oral stories, and researcher journal entries, collected over six placement blocks with 26 nursing students. Analysis identified that students engaged enthusiastically with storytelling; stories highlighted moments of student-patient interactions; the value of a positive clinical facilitator-student relationship; the complexity of registered nurse-student relationships; and students from cultural and linguistic diverse backgrounds experienced feelings of isolation from staff. The simple tools of pen and paper were a powerful medium for students to reflect on their interactions with patients. Facilitating the process of storytelling provided the researcher with insight into intrinsic and extrinsic obstacles and opportunities which may impact students’ engagement with patients.
