Abstract:Despite the increasing adoption of insulin pumps and continuous glucose monitoring devices, most people with type 1 diabetes do not achieve their glycemic goals<a href="/articles/s41591-020-1045-7#ref-CR1">1</a>. This could be related to a lack of expertise or inadequate time for clinicians to analyze complex sensor-augmented pump data. We tested whether frequent insulin dose adjustments guided by an automated artificial intelligence-based decision support system (AI-DSS) is as effective and safe as those guided by physicians in controlling glucose levels. ADVICE4U was a six-month, multicenter, multinational, parallel, randomized controlled, non-inferiority trial in 108 participants with type 1 diabetes, aged 10–21 years and using insulin pump therapy (ClinicalTrials.gov no. NCT03003806). Participants were randomized 1:1 to receive remote insulin dose adjustment every three weeks guided by either an AI-DSS, (AI-DSS arm, n = 54) or by physicians (physician arm, n = 54). The results for the primary efficacy measure—the percentage of time spent within the target glucose range (70–180 mg dl−1 (3.9–10.0 mmol l−1))—in the AI-DSS arm were statistically non-inferior to those in the physician arm (50.2 ± 11.1% versus 51.6 ± 11.3%, respectively, P < 1 × 10−7). The percentage of readings below 54 mg dl−1 (<3.0 mmol l−1) within the AI-DSS arm was statistically non-inferior to that in the physician arm (1.3 ± 1.4% versus 1.0 ± 0.9%, respectively, P < 0.0001). Three severe adverse events related to diabetes (two severe hypoglycemia, one diabetic ketoacidosis) were reported in the physician arm and none in the AI-DSS arm. In conclusion, use of an automated decision support tool for optimizing insulin pump settings was non-inferior to intensive insulin titration provided by physicians from specialized academic diabetes centers.

Abstract:An estimated 425 million people globally have diabetes, accounting for 12% of the world's health expenditures, and yet 1 in 2 persons remain undiagnosed and untreated. Applications of artificial intelligence (AI) and cognitive computing offer promise in diabetes care. The purpose of this article is to better understand what AI advances may be relevant today to persons with diabetes (PWDs), their clinicians, family, and caregivers. The authors conducted a predefined, online PubMed search of publicly available sources of information from 2009 onward using the search terms "diabetes" and "artificial intelligence." The study included clinically-relevant, high-impact articles, and excluded articles whose purpose was technical in nature. A total of 450 published diabetes and AI articles met the inclusion criteria. The studies represent a diverse and complex set of innovative approaches that aim to transform diabetes care in 4 main areas: automated retinal screening, clinical decision support, predictive population risk stratification, and patient self-management tools. Many of these new AI-powered retinal imaging systems, predictive modeling programs, glucose sensors, insulin pumps, smartphone applications, and other decision-support aids are on the market today with more on the way. AI applications have the potential to transform diabetes care and help millions of PWDs to achieve better blood glucose control, reduce hypoglycemic episodes, and reduce diabetes comorbidities and complications. AI applications offer greater accuracy, efficiency, ease of use, and satisfaction for PWDs, their clinicians, family, and caregivers.

Abstract:Type 1 diabetes (T1D) is a chronic health condition resulting from pancreatic beta cell dysfunction and insulin depletion. While automated insulin delivery systems are now available, many people choose to manage insulin delivery manually through insulin pumps or through multiple daily injections. Frequent insulin titrations are needed to adequately manage glucose, however, provider adjustments are typically made every several months. Recent automated decision support systems incorporate artificial intelligence algorithms to deliver personalized recommendations regarding insulin doses and daily behaviors. This paper presents a comprehensive review of computational and artificial intelligence-based decision support systems to manage T1D. Articles were obtained from PubMed, IEEE Xplore, and ScienceDirect databases. No time period restrictions were imposed on the search. After removing off-topic articles and duplicates, 562 articles were left to review. Of those articles, we identified 61 articles for comprehensive review based on algorithm evaluation using real-world human data, in silico trials, or clinical studies. We grouped decision support systems into general categories of (1) those which recommend adjustments to insulin and (2) those which predict and help avoid hypoglycemia. We review the artificial intelligence methods used for each type of decision support system, and discuss the performance and potential applications of these systems.

Nurse-in-the-Loop Artificial Intelligence for Precision Management of Type 2 Diabetes in a Clinical Trial Utilizing Transfer-Learned Predictive Digital Twin

Abstract:Background: Type 2 diabetes (T2D) is a prevalent chronic disease with a significant risk of serious health complications and negative impacts on the quality of life. Given the impact of individual characteristics and lifestyle on the treatment plan and patient outcomes, it is crucial to develop precise and personalized management strategies. Artificial intelligence (AI) provides great promise in combining patterns from various data sources with nurses' expertise to achieve optimal care. Methods: This is a 6-month ancillary study among T2D patients (n = 20, age = 57 +- 10). Participants were randomly assigned to an intervention (AI, n=10) group to receive daily AI-generated individualized feedback or a control group without receiving the daily feedback (non-AI, n=10) in the last three months. The study developed an online nurse-in-the-loop predictive control (ONLC) model that utilizes a predictive digital twin (PDT). The PDT was developed using a transfer-learning-based Artificial Neural Network. The PDT was trained on participants self-monitoring data (weight, food logs, physical activity, glucose) from the first three months, and the online control algorithm applied particle swarm optimization to identify impactful behavioral changes for maintaining the patient's glucose and weight levels for the next three months. The ONLC provided the intervention group with individualized feedback and recommendations via text messages. The PDT was re-trained weekly to improve its performance. Findings: The trained ONLC model achieved >=80% prediction accuracy across all patients while the model was tuned online. Participants in the intervention group exhibited a trend of improved daily steps and stable or improved total caloric and total carb intake as recommended.

We are no longer confined to descriptive data analysis because we may acquire a larger by recognizing and anticipating patterns coming from inductive reasoning, owing to medical learning. AI applications have the potential to revolutionize diabetic care by assisting millions of people with diabetes in achieving better blood glucose management, reducing hyperglycemic episodes, and lowering diabetic morbidity and complications. We can see the evolution of closed-loop insulin delivery systems with inbuilt AI algorithms to safeguard both hypoglycemia and hyperglycemia in Type 1 diabetes, which have been relatively few attempts in management techniques for diabetes excursions. The medication you choose and how much you take depend on a variety of factors, including your body mass index, which influences beta-cell activity and insulin resistance, among other things. There are great assessments of research that have employed an AI approach to treat diabetes. The only way to deal with vast, diverse datasets is to rely solely on quantitative data. At this point, there are numerous issues with absolute reliance on quantitative data, depending on the frequently poor quality of this sort of information, as well as the necessity to complement combining a quantitative and qualitative approach. Attempting toward the transformation of unstructured data into digitally processed information data is a domain of cognitive computing that is predicted to grow in importance. To make a substantial contribution to AI aside from the purpose, there are 48 other factors to consider.
Real-time Artificial Intelligence Assisted Insulin Dosage Titration System for Glucose Control in Type 2 Diabetic Patients: a Proof of Concept Study

Abstract:Objective This study aims to develop an insulin dosage adjustment model using machine learning of high quality electronic health records (EHRs) notes and then to form an artificial intelligence-based insulin clinical decision support workflow (iNCDSS) implemented in the HIS system to give a real-time recommendation of insulin dosage titration. The efficacy and safety in clinical practice is evaluated in this proof-of-concept study. Research design and methods We extracted patient-specific and time-varying features from the original EHRs data and performed machine learning analysis through 5-fold cross validation. In the patient-blind, single-arm interventional study, insulin dosage was titrated according to iNCDSS in type 2 diabetic inpatients for up to 7 d or until hospital discharge. The primary end point of the trial was the difference in glycemic control as measured by mean daily blood glucose concentration during the intervention period. Results A total of 3275 type 2 diabetic patients with 38,406 insulin counts were included for the model analysis. The XGBoost model presented the best performance with root mean square error (RMSE) of 1.06 unit and mean absolute relative difference (MARD) of 6.0% in the training dataset, and RMSE of 1.30 unit and MARD of 6.9% in the testing dataset. Twenty-three patients with T2DM (male 14, 60.9%; age 58.8 ± 10.7 years; duration of diabetes 11.8 ± 8.8 years, HbA1c 9.1 ± 1.1%) were enrolled in the proof of concept trial. The duration of iNCDSS intervention was 7.0 ± 0.1 d. The insulin dose recommended by iNCDSS was accepted by physicians in 97.8%. The mean daily capillary blood glucose was markedly improved during the intervention period, with a reduction of mean daily capillary BG from 11.3(8.0, 13.9) mmol/L in the first 24 h to 7.9(6.5,8.9) mmol/L in the last 24 h of the trial ( P < 0.001). In addition, the time range below 3.9 mmol/L was decreased from 1.1% to 0.5%. Conclusions The clinical decision support system of insulin dosage titration developed using a machine learning algorithm based on the EHRs data was effective and safe in glycemic control in in type 2 diabetic inpatients. Abstract:Introduction: Insulin therapy is most effective if patients learn how to properly adjust insulin to achieve glycaemic targets. There is a need for methods and tools that can assist these processes in clinical practice. The purpose of this feasibility study was to evaluate an approach to support insulin dose adjustment in individual patients using a mobile titration application (app). Methods: A cohort of adults (N=36) with type 2 diabetes with suboptimal glycaemia who were starting basal insulin self-titration were trained by a diabetes care and education specialist to use a mobile titration app to guide adjusting insulin doses. Glycaemia, diabetes distress and patient and provider satisfaction were assessed during the first 3 months after initiating basal insulin titration using the mobile app. Results: Mean haemoglobin type A1c (HbA1c) was significantly reduced by an average of 2.1 ± 2.2% from baseline to 3 months (p<0.001). Diabetes distress significantly decreased from baseline to follow-up with scores going down (or improving) across all scales. Both patients and providers reported high levels of satisfaction and positive experiences. Conclusion: The model offers a promising solution to streamline insulin dosage adjustments to achieve specific clinical and self-management goals with high expectations for long-term benefits and warrants further investigation.
Simulation-driven optimization of insulin therapy profiles in a commercial hybrid closed-loop system

Abstract:Background: Automated insulin delivery (AID) has represented a breakthrough in managing type 1 diabetes (T1D), showing safe and effective glucose control extensively across the board. However, metabolic variability still poses a challenge to commercial hybrid closed-loop (HCL) solutions, whose performance depends on customizable insulin therapy profiles. In this work, we propose an Identification-Replay-Optimization (IRO) approach to optimize gradually and safely such profiles for the Control-IQ AID algorithm. Methods: Closed-loop data are generated using the full adult cohort of the UVA/Padova T1D simulation platform in diverse glycemic scenarios. For each subject, daily records are processed and used to estimate a personalized model of the underlying insulin-glucose dynamics. Every two weeks, all identified models are integrated into an optimization procedure where daily basal and bolus profiles are adjusted so as to minimize the risks for hypo- and hyperglycemia. The proposed strategy is tested under different scenarios of metabolic and behavioral variability in order to evaluate the efficacy and convergence of the proposed strategy. Finally, glycemic metrics between cycles are compared using paired t-tests with p<0.05 as the significance threshold. Results: Simulations reveal that the proposed IRO approach was able to improve glucose control over time by safely mitigating the risks for both hypo- and hyperglycemia. Furthermore, smaller changes were recommended at each cycle, indicating convergence when simulation conditions were maintained. Conclusions: The use of reliable simulation-driven tools capable of accurately reproducing field-collected data and predicting changes can substantially shorten the process of optimizing insulin therapy, adjusting it to metabolic changes and leading to improved glucose control.
Intermittent Control for Safe Long-Acting Insulin Intensification for Type 2 Diabetes: In-Silico Experiment

Abstract:Around a third of type 2 diabetes patients (T2D) are escalated to basal insulin injections. Basal insulin dose is titrated to achieve a tight glycemic target without undue hypoglycemic risk. In the standard of care (SoC), titration is based on intermittent fasting blood glucose (FBG) measurements. Lack of adherence and the day-to-day variabilities in FBG measurements are limiting factors to the existing insulin titration procedure. We propose an adaptive receding horizon control strategy where a glucose-insulin fasting model is identified and used to predict the optimal basal insulin dose. This algorithm is evaluated in \textit{in-silico} experiments using the new UVA virtual lab (UVlab) and a set of T2D avatars matched to clinical data (NCT01336023). Compared to SoC, we show that this control strategy can achieve the same glucose targets faster (as soon as week 8) and safer (increased hypoglycemia protection and robustness to missing FBG measurements). Specifically, when insulin is titrated daily, a time-in-range (TIR, 70--180 mg/dL) of 71.4$\pm$20.0\% can be achieved at week eight and maintained at week 52 (72.6$\pm$19.6%) without an increased hypoglycemia risk as measured by time under 70 mg/dL (TBR, week 8: 1.3$\pm$1.9% and week 52: 1.2$\pm$1.9%), when compared to the SoC (TIR at week 8: 59.3$\pm$28.0% and week:52 72.1$\pm$22.3%, TBR at week 8: 0.5$\pm$1.3% and week 52: 2.8$\pm$3.4%). Such an approach can potentially reduce treatment inertia and prescription complexity, resulting in improved glycemic outcomes for T2D using basal insulin injections.

Conclusion Artificial intelligence is transforming diabetes research in many technical and organizational aspects. Although its deployment is still limited and is facing many challenges, it will likely impact future patient care by providing automated and personalized healthcare for patients.

Abstract:Artificial intelligence (AI) algorithms can provide actionable insights for clinical decision-making and managing chronic diseases. The treatment and management of complex chronic diseases, such as diabetes, stands to benefit from novel AI algorithms analyzing the frequent real-time streaming data and the occasional medical diagnostics and laboratory test results reported in electronic health records (EHR). Novel algorithms are needed to develop trustworthy, responsible, reliable, and robust AI techniques that can handle the imperfect and imbalanced data of EHRs and inconsistencies or discrepancies with free-living self-reported information. The challenges and applications of AI for two problems in the healthcare domain were explored in this work. First, we introduced novel AI algorithms for EHRs designed to be fair and unbiased while accommodating privacy concerns in predicting treatments and outcomes. Then, we studied the innovative approach of using machine learning to improve automated insulin delivery systems through analyzing real-time information from wearable devices and historical data to identify informative trends and patterns in free-living data. Application examples in the treatment of diabetes demonstrate the benefits of AI tools for medical and health informatics.

Abstract:In type 2 diabetes (T2D) treatment, finding a safe and effective basal insulin dose is a challenge. The dose-response is highly individual and to ensure safety, people with T2D titrate by slowly increasing the daily insulin dose to meet treatment targets. This titration can take months. To ease and accelerate the process, we use short-term artificial pancreas (AP) treatment tailored for initial titration and apply it as a diagnostic tool. Specifically, we present a method to automatically estimate a personalized daily dose of basal insulin from closed-loop data collected with an AP. Based on AP-data from a stochastic simulation model, we employ the continuous-discrete extended Kalman filter and a maximum likelihood approach to estimate parameters in a simple dose-response model for 100 virtual people. With the identified model, we compute a daily dose of basal insulin to meet treatment targets for each individual. We test the personalized dose and evaluate the treatment outcomes against clinical reference values. In the tested simulation setup, the proposed method is feasible. However, more extensive tests will reveal whether it can be deemed safe for clinical implementation.
An Online Stochastic Optimization Approach for Insulin Intensification in Type 2 Diabetes with Attention to Pseudo-Hypoglycemia

Abstract:In this paper, we present a model free approach to calculate long-acting insulin doses for Type 2 Diabetic (T2D) subjects in order to bring their blood glucose (BG) concentration to be within a safe range. The proposed strategy tunes the parameters of a proposed control law by using a zeroth-order online stochastic optimization approach for a defined cost function. The strategy uses gradient estimates obtained by a Recursive Least Square (RLS) scheme in an adaptive moment estimation based approach named AdaBelief. Additionally, we show how the proposed strategy with a feedback rating measurement can accommodate for a phenomena known as relative hypoglycemia or pseudo-hypoglycemia (PHG) in which subjects experience hypoglycemia symptoms depending on how quick their BG concentration is lowered. The performance of the insulin calculation strategy is demonstrated and compared with current insulin calculation strategies using simulations with three different models.
Recent progress in artificial intelligence and machine learning for novel diabetes mellitus medications development

Abstract:Diabetes mellitus, stemming from either insulin resistance or inadequate insulin secretion, represents a complex ailment that results in prolonged hyperglycemia and severe complications. Patients endure severe ramifications such as kidney disease, vision impairment, cardiovascular disorders, and susceptibility to infections, leading to significant physical suffering and imposing substantial socio-economic burdens. This condition has evolved into an increasingly severe health crisis. There is an urgent need to develop new treatments with improved efficacy and fewer adverse effects to meet clinical demands. However, novel drug development is costly, time-consuming, and often associated with side effects and suboptimal efficacy, making it a major challenge. Artificial Intelligence (AI) and Machine Learning (ML) have revolutionized drug development across its comprehensive lifecycle, spanning drug discovery, preclinical studies, clinical trials, and post-market surveillance. These technologies have significantly accelerated the identification of promising therapeutic candidates, optimized trial designs, and enhanced post-approval safety monitoring. Recent advances in AI, including data augmentation, interpretable AI, and integration of AI with traditional experimental methods, offer promising strategies for overcoming the challenges inherent in AI-based drug discovery. Despite these advancements, there exists a notable gap in comprehensive reviews detailing AI and ML applications throughout the entirety of developing medications for diabetes mellitus. This review aims to fill this gap by evaluating the impact and potential of AI and ML technologies at various stages of diabetes mellitus drug development. It does that by synthesizing current research findings and technological advances so as to effectively control diabetes mellitus and mitigate its far-reaching social and economic impacts. The integration of AI and ML promises to revolutionize diabetes mellitus treatment strategies, offering hope for improved patient outcomes and reduced healthcare burdens worldwide.

Abstract:As all people with type 1 diabetes (T1D) and some with type 2 diabetes (T2D) require insulin, there is a need to develop management methods that not only achieve glycemic targets but also reduce the burden of living with diabetes. After insulin pumps and continuous glucose monitors, the next step in the evolution of diabetes technology is automated insulin delivery (AID) systems, which have transformed intensive insulin management over the past decade, as these systems address the shortcomings of previous management options. However, AID use remains fairly limited, and access represents a major barrier to use for many people with diabetes, despite these systems being standard of care. Therefore, the future of AID will necessitate addressing barriers related to social determinants of health, finances, and an expansion of the number and type of health care professionals (HCPs) prescribing AID systems. These crucial steps will be essential to ensure that everyone with intensively managed diabetes can use AID systems. The impact of implementing these changes will create a shift in the future of diabetes care that will result in achievement of more targeted glycemia and psychosocial outcomes for all people with diabetes and an expansion of the role of all HCPs in AID-related diabetes care. Even more importantly, by addressing social determinants of health and clinical inertia related to AID, the field can address disparities in outcomes across countries, race, gender, socioeconomic status, and insurance status. Furthermore, the increased use of AID system will provide more time during appointments for a shift in the discussion away from fine tuning insulin dosing and toward a focus on more topics related to behavior and conversations about general health. This will include psychosocial outcomes, and quality of life. In addition, these changes can hopefully allow for time to discuss more general issues, such as cardiovascular health, obesity prevention, diabetes-related complications, and other health-related concerns.

Abstract:Artificial intelligence is growing quickly, and its application in the global diabetes pandemic has the potential to completely change the way this chronic illness is identified and treated. Machine learning methods have been used to construct algorithms supporting predictive models for the risk of getting diabetes or its complications. Social media and Internet forums also increase patient participation in diabetes care. Diabetes resource usage optimisation has benefited from technological improvements. As a lifestyle therapy intervention, digital therapies have made a name for themselves in the treatment of diabetes. Artificial intelligence will cause a paradigm shift in diabetes care, moving away from current methods and toward the creation of focused, data-driven precision treatment.
