Radiation-induced Vaginal Injury after Treatment for Cervical Cancer

Abstract:Vagina is both a target organ and an organ at risk (OAR) for locally advanced cervical cancer radiotherapy (RT). Its radiation injury is inevitable. In this multicenter study, we sought to evaluate possible predictors of severity in women with radiation-induced vagina injury (VI) after treatment for cervical cancer (NCT03257454). Women with cervical cancer were eligible if they had FIGO 2009 stage IB2–IIIB (except IIIA); were aged 18 to 60 years. 90 cervical cancer patients who underwent chemoradiotherapy (CRT) at 6 different institutions in Xi'an between 11/2016 and 10/2018 were enrolled in this study. External beam RT (EBRT) was delivered to the entire pelvis at 2.0 Gy per fraction, for a total of 25 fractions and 50 Gy concurrent weekly administration of cisplatin (25 mg/m2) or cisplatin (25 mg/m2, day 1 - day 3) combined with liposome paclitaxel (135 mg/m2, day 1). CT-based high-dose rate (HDR) brachytherapy was administered at doses of 24 to 36 Gy in 4 to 6 fractions after EBRT. The planning aim at point A was 82-90 Gy in EQD2. Age, body Mass Index (BMI), vaginal reference length (VRL) and radiation doses at points A, posterior-inferior border of symphysis (PIBS), PIBS+ 2 cm, and International Commission on Radiation Units (ICRU) R-V were measured. VI was evaluated according to the Common Terminology Criteria for Adverse Events (CTCAE), version 4. Risk factors/predictors for VI were assessed using univariate and multivariate ordinal logistic regression. Statistical analyses were performed with statistical software. The median follow-up duration was 25 months (12-36months). VI grade 2 or worse adverse events were observed for 19 (21 %) of 90 patients. The median total doses at points PIBS, PIBS+ 2 cm, and ICRU R-V were 1.49 (0.64-10.50) Gy, 4.30 (1,21-16.20) Gy and 4.40 (2.35-11.10) Gy, respectively. The mean vaginal reference length (VRL) was 4.62 cm (median, 4.70 cm). Factor analysis with univariate and multivariate analysis showed that increasing age, VRL and radiation doses at PIBS+ 2 cm and ICRU R-V to be significant predictors for severity of VI at last follow-up, especially increasing age (P = 0.0009), VRL (P = 0.0099) and the dose at ICRU R-V (P = 0.0011). There was no statistically significant correlation when comparing FIGO stage, BMI, radiation doses at points A and PIBS with VI severity. There is a high probability of radiation injury above moderate vagina. The doses at ICRU R-V and PIBS+ 2 cm points, and VRL are significant to predict the extent of radiation-induced VI.
