Abstract:Background Conventional breath-held gadolinium-based contrastenhanced MR angiography (CE-MRA) provides excellent definition of extra-cardiac anatomy, but does not provide diagnostic image quality for intra-cardiac anatomy because it is not gated to ECG due to time constraints associated with breath-holding and the need to capture the first-pass of gadolinium. To address this issue, we propose a 4D non-breath-held multiphase steady-state imaging sequence (MUSIC) using ferumoxytol, which is an FDA approved iron-oxide particle for treating irondeficiency anemia, as an intravascular contrast agent and demonstrate its feasibility in children with congenital heart disease (CHD).

Abstract:Purpose To develop an MR multitasking-based multidimensional assessment of cardiovascular system (MT-MACS) with electrocardiography-free and navigator-free data acquisition for a comprehensive evaluation of thoracic aortic diseases. Methods The MT-MACS technique adopts a low-rank tensor image model with a cardiac time dimension for phase-resolved cine imaging and a T-2-prepared inversion-recovery dimension for multicontrast assessment. Twelve healthy subjects and 2 patients with thoracic aortic diseases were recruited for the study at 3 T, and both qualitative (image quality score) and quantitative (contrast-to-noise ratio between lumen and wall, lumen and wall area, and aortic strain index) analyses were performed in all healthy subjects. The overall image quality was scored based on a 4-point scale: 3, excellent; 2, good; 1, fair; and 0, poor. Statistical analysis was used to test the measurement agreement between MT-MACS and its corresponding 2D references. Results The MT-MACS images reconstructed from acquisitions as short as 6 minutes demonstrated good or excellent image quality for bright-blood (2.58 +/- 0.46), dark-blood (2.58 +/- 0.50), and gray-blood (2.17 +/- 0.53) contrast weightings, respectively. The contrast-to-noise ratios for the three weightings were 49.2 +/- 12.8, 20.0 +/- 5.8 and 2.8 +/- 1.8, respectively. There were good agreements in the lumen and wall area (intraclass correlation coefficient = 0.993, P < .001 for lumen; intraclass correlation coefficient = 0.969, P < .001 for wall area) and strain (intraclass correlation coefficient = 0.947, P < .001) between MT-MACS and conventional 2D sequences. Conclusion The MT-MACS technique provides high-quality, multidimensional images for a comprehensive assessment of the thoracic aorta. Technical feasibility was demonstrated in healthy subjects and patients with thoracic aortic diseases. Further clinical validation is warranted.
Accelerated Ferumoxytol-Enhanced 4d Multiphase, Steady-State Imaging with Contrast Enhancement (music) Cardiovascular Mri: Validation in Pediatric Congenital Heart Disease

Abstract:The purpose of this work was to validate a parallel imaging (PI) and compressed sensing (CS) combined reconstruction method for a recently proposed 4D non-breath-held, multiphase, steady-state imaging technique (MUSIC) cardiovascular MRI in a cohort of pediatric congenital heart disease patients. We implemented a graphics processing unit accelerated CS-PI combined reconstruction method and applied it in 13 pediatric patients who underwent cardiovascular MRI after ferumoxytol administration. Conventional breath-held contrast-enhanced magnetic resonance angiography (CE-MRA) was first performed during the first pass of ferumoxytol injection, followed by the original MUSIC and the proposed CS-PI MUSIC during the steady-state distribution phase of ferumoxytol. Qualities of acquired images were then evaluated using a four-point scale. Left ventricular volumes and ejection fractions calculated from the original MUSIC and the CS-PI MUSIC were also compared with conventional multi-slice 2D cardiac cine MRI. The proposed CS-PI MUSIC reduced the imaging time of the MUSIC acquisition to 4.6 +/- 0.4 min from 8.9 +/- 1.2 min. Computationally intensive image reconstruction was completed within 5 min without interruption of sequential clinical scans. The proposed method (mean 3.3-4.0) provided image quality comparable to that of the original MUSIC (3.2-4.0) (all P = 0.42), and better than conventional breath-held first-pass CE-MRA (1.1-3.3) for 13 anatomical structures (all P = 0.0014) with good inter-observer agreement (kappa > 0.46). The calculated ventricular volumes and ejection fractions from both original MUSIC (r > 0.90) and CS-PI MUSIC (r > 0.85) correlated well with 2D cine imaging. In conclusion, PI and CS were successfully incorporated into the 4D MUSIC acquisition to further reduce scan time by approximately 50% while maintaining highly comparable image quality in a clinically practical reconstruction time.

Abstract:PurposeTo develop a free‐breathing, non‐electrocardiogram technique for simultaneous myocardial T1, T2, T2*, and fat‐fraction (FF) mapping in a single scan.MethodsThe MR Multitasking framework is adapted to quantify T1, T2, T2*, and FF simultaneously. A variable TR scheme is developed to preserve temporal resolution and imaging efficiency. The underlying high‐dimensional image is modeled as a low‐rank tensor, which allows accelerated acquisition and efficient reconstruction. The accuracy and/or repeatability of the technique were evaluated on static and motion phantoms, 12 healthy volunteers, and 3 patients by comparing to the reference techniques.ResultsIn static and motion phantoms, T1/T2/T2*/FF measurements showed substantial consistency (R > 0.98) and excellent agreement (intraclass correlation coefficient > 0.93) with reference measurements. In human subjects, the proposed technique yielded repeatable T1, T2, T2*, and FF measurements that agreed with those from references.ConclusionsThe proposed free‐breathing, non‐electrocardiogram, motion‐resolved Multitasking technique allows simultaneous quantification of myocardial T1, T2, T2*, and FF in a single 2.5‐min scan.

Abstract:Background Our recently proposed MUSIC approach acquires ferumoxytol-enhanced 4D images using ECG and airway pressure signal for motion gating. It has been routinely used in our institution and provides detailed information on vascular anatomy. However, the airway pressure signal is only available in patients with general anesthesia (GA) and ECG is often problematic in high field strength. Therefore, we propose the self-gated MUSIC (SGMUSIC) where cardiac and respiratory motion is compensated by retrospective data sorting base on derived SG signals. In this study, we tested SG-MUSIC on pediatric patients under GA so that the derived SG signal can be validated against the recorded physiological signal and the images compared with the original MUSIC.

Abstract:Cardiac MRI is a crucial tool for assessing congenital heart disease (CHD). However, its application remains challenging in young children when performed at 3T. The aim of this retrospective single center study was to compare a non-contrast free-breathing 2D CINE T1-weighted TFE-sequence with compressed sensing (FB 2D CINE CS T1-TFE) with 3D imaging for diagnostic accuracy of CHD, image quality, and vessel diameter measurements in sedated young children. FB 2D CINE CS T1-TFE was compared with a 3D non-contrast whole-heart sequence (3D WH) and 3D contrast-enhanced MR angiography (3D CE-MRA) at 3T in 37 CHD patients (20♂, 1.5±1.4 years). Two radiologists independently assessed image quality, type of CHD, and diagnostic confidence. Diameters and measures of contrast and sharpness of the aorta and pulmonary vessels were determined. A non-parametric multi-factorial approach was used to estimate diagnostic accuracy for the diagnosis of CHD. Linear mixed models were calculated to compare contrast and vessel sharpness. Krippendorff’s alpha was determined to quantify vessel diameter agreement. FB 2D CINE CS T1-TFE was rated superior regarding image quality, diagnostic confidence, and diagnostic sensitivity for both intra- and extracardiac pathologies compared to 3D WH and 3D CE-MRA (all p<0.05). FB 2D CINE CS T1-TFE showed superior contrast and vessel sharpness (p<0.001) resulting in the highest proportion of measurable vessels (740/740; 100%), compared to 3D WH (530/620; 85.5%) and 3D CE-MRA (540/560; 96.4%). Regarding vessel diameter measurements, FB 2D CINE CS T1-TFE revealed the closest inter-reader agreement (Krippendorff’s alpha: 0.94–0.96; 3D WH: 0.78–0.94; 3D CE-MRA: 0.76–0.93). FB 2D CINE CS T1-TFE demonstrates robustness at 3T and delivers high-quality diagnostic results to assess CHD in sedated young children. Its ability to function without contrast injection and respiratory compensation enhances ease of use and could encourage widespread adoption in clinical practice.

Abstract:BACKGROUND: Three dimensional, whole-heart (3DWH) MRI is an established non-invasive imaging modality in patients with congenital heart disease (CHD) for the diagnosis of cardiovascular morphology and for clinical decision making. Current techniques utilise diaphragmatic navigation (dNAV) for respiratory motion correction and gating and are frequently limited by long acquisition times. This study proposes and evaluates the diagnostic performance of a respiratory gating-free framework, which considers respiratory image-based navigation (iNAV), and highly accelerated variable density Cartesian sampling in concert with non-rigid motion correction and low-rank patch-based denoising (iNAV-3DWH-PROST). The method is compared to the clinical dNAV-3DWH sequence in adult patients with CHD.METHODS: In this prospective single center study, adult patients with CHD who underwent the clinical dNAV-3DWH MRI were also scanned with the iNAV-3DWH-PROST. Diagnostic confidence (4-point Likert scale) and diagnostic accuracy for common cardiovascular lesions was assessed by three readers. Scan times and diagnostic confidence were compared using the Wilcoxon-signed rank test. Co-axial vascular dimensions at three anatomic landmarks were measured, and agreement between the research and the corresponding clinical sequence was assessed with Bland-Altman analysis.RESULTS: The study included 60 participants (mean age ± [SD]: 33 ± 14 years; 36 men). The mean acquisition time of iNAV-3DWH-PROST was significantly lower compared with the conventional clinical sequence (3.1 ± 0.9 min vs 13.9 ± 3.9 min, p < 0.0001). Diagnostic confidence was higher for the iNAV-3DWH-PROST sequence compared with the clinical sequence (3.9 ± 0.2 vs 3.4 ± 0.8, p < 0.001), however there was no significant difference in diagnostic accuracy. Narrow limits of agreement and mean bias less than 0.08 cm were found between the research and the clinical vascular measurements.CONCLUSIONS: The iNAV-3DWH-PROST framework provides efficient, high quality and robust 3D whole-heart imaging in significantly shorter scan time compared to the standard clinical sequence.

Abstract:To evaluate parallel-imaging methods in free-breathing whole-heart 3D coronary magnetic resonance angiography and assess the navigator techniques and visualization rates of the major coronary arteries. We compared key parameters of the generalized autocalibrating partially parallel acquisition and modified sensitive encoding images in vitro phantom MRI; performed the MRA with GRAPPA parallel imaging in healthy volunteers; compared 1D- and 2D-prospective acquisition correction and analyzed the differences; and evaluated the visualization of major coronary arterial branches. GRAPPA images had higher signal-to-noise ratio and contrast-to-noise ratio and fewer aliasing artifacts. The coronary arteries were adequately visualized in 38 volunteers. 2D-PACE had a higher navigator efficiency, shorter scan time, and gave clearer reconstructed images in comparison with 1D-PACE. GRAPPA images were superior to mSENSE images. Whole-heart 3D coronary MRA along with parallel-imaging technique is a potential clinical method, and 2D-PACE is a better navigation technique than 1D-PACE.

Abstract:Objective To evaluate the value of three dimensional(3D)steady state free precession sequence(SSFP)in diagnosis of congenital heart disease(CHD).Methods 71 patients with CHD[mean age(7.27±4.25)years]underwent both 3D SSFP and conventional MR imaging:including two dimensional(2D)SSFP and contrast enhanced magnetic resonance angiography(CE-MRA).The image quality was compared,and statistical analysis was used to evaluate consistency of 3D SSFP and CEMRA for diameter measurements of ascending aorta(AAO),main pulmonary artery(MPA)and superior vena cava(SVC).Results The image quality of 3D SSFP MRI for intra cardiac structure(4.15±0.62)was better than that of CE-MRA(3.70±0.65)(P<0.05),while it was similar with that of 2D SSFP(3.98±0.53)(P=0.21).The diagnostic sensitivity,specificity of 3D SSFP for the detection of atrial site,atria-ventricular connection and ventricular-arterial connection were all 100%.Diameter measurements of AAO,MPA and SVC at 3D SSFP showed great agreement with CE-MRA(P<0.05).Conclusions 3D SSFP MRI could provide better image quality than conventional MRI for diagnosis of intra cardiac anomaly,and atria-ventricular connection,ventricular-arterial connection could be more directly seen by 3D SSFP MRI,which can make it ideally suited to the evaluation of complicated CHD.
Magnetization Transfer BOOST Noncontrast Angiography Improves Pulmonary Vein Imaging in Adults With Congenital Heart Disease

Abstract:Background: Cardiac MRI plays an important role in the diagnosis and follow-up of patients with congenital heart disease (CHD). Gadolinium-based contrast agents are often needed to overcome flow-related and off-resonance artifacts that can impair the quality of conventional noncontrast 3D imaging. As serial imaging is often required in CHD, the development of robust noncontrast 3D MRI techniques is desirable. Purpose: To assess the clinical utility of noncontrast enhanced magnetization transfer and inversion recovery prepared 3D free-breathing sequence (MTC-BOOST) compared to conventional 3D whole heart imaging in patients with CHD. Study type: Prospective, image quality. Population: A total of 27 adult patients (44% female, mean age 30.9 ± 14.8 years) with CHD. Field strength/sequence: A 1.5 T; free-breathing 3D MTC-BOOST sequence. Assessment: MTC-BOOST was compared to diaphragmatic navigator-gated, noncontrast T2 prepared 3D whole-heart imaging sequence (T2prep-3DWH) for comparison of vessel dimensions, lumen-to-myocardium contrast ratio (CR), and image quality (vessel wall sharpness and presence and type of artifacts) assessed by two experienced cardiologists on a 5-point scale. Statistical tests: Mann-Whitney test, paired Wilcoxon signed-rank test, Bland-Altman plots. P < 0.05 was considered statistically significant. Results: MTC-BOOST significantly improved image quality and CR of the right-sided pulmonary veins (PV): (CR: right upper PV 1.06 ± 0.50 vs. 0.58 ± 0.74; right lower PV 1.32 ± 0.38 vs. 0.81 ± 0.73) compared to conventional T2prep-3DWH imaging where the PVs were not visualized in some cases due to off-resonance effects. MTC-BOOST demonstrated resistance to degradation of luminal signal (assessed by CR) secondary to accelerated or turbulent flow conditions. T2prep-3DWH had higher image quality scores than MTC-BOOST for the aorta and coronary arteries; however, great vessel dimensions derived from MTC-BOOST showed excellent agreement with standard T2prep-3DWH imaging. Data conclusion: MTC-BOOST allows for improved contrast-free imaging of pulmonary veins and regions characterized by accelerated or turbulent blood flow compared to standard T2prep-3DWH imaging, with excellent agreement of great vessel dimensions. Evidence level: 1 TECHNICAL EFFICACY: Stage 2.
Preliminary study of three dimensional dual-phase steady state free precession imaging technique in the diagnosis of congenital heart diseases

Abstract:Objective:To evaluate the value of MRI dual phase three dimensional steady state free precession (dual-phase 3D SSFP)sequence in the diagnosis of congenital heart disease (CHD).Methods:60 pediatric patients of CHD with the average heart rate as 108 beats/min underwent MRI dual-phase 3D SSFP sequence,the acquired images were at the end systolic phase and mid-late diastolic phase.The image quality and contrast-to-noise ratio (CNR)were analyzed and com-pared.Results:The ECG trigger delay time was 180 ~300ms [mean= (235.41 ±35.59 )ms]in systole and 384 ~550ms [m=(443.77±50.81)ms]in diastole.The image qualities of intra-cardiac structures,pulmonary veins,superior and inferior vena cava in 3D SSFP systole were significantly better than that in diastole,with statistical difference (P <0.05).However, the distinctness of aorta with no stenosis and main pulmonary artery was better in diastole compared with that of systole, yet with no statistical difference (P >0.05).The CNR of intra-cardiac structures was significantly higher in 3D SSFP systo-le than in diastole,with statistic difference (P <0.05).The CNR of SVC and IVC was higher in systole,but only the CNR of IVC showed statistical difference (P <0.05).The CNR of aorta and main pulmonary artery was lower in systole than in diastole,but there was no statistical difference (P >0.05).In patients with pulmonary stenosis and subvalvuular pulmonary stenosis,the percentage of visibility of pulmonary artery branches in systole was only 39.1%,but was higher in diastole (73.9%).Conclusion:Dual-phase 3D SSFP sequence had the advantages of showing images in systole and diastole and ulti-mately can provide more accurate diagnosis of intra-/extra-cardiac great vessel abnormalities in pediatric patients with CHD.

Abstract:Abstract Background To evaluate a non-contrast respiratory- and electrocardiogram-gated 3D cardiovascular magnetic resonance angiography (CMRA) based on magnetization-prepared Dixon method (relaxation-enhanced angiography without contrast and triggering, REACT) for the assessment of the thoracic vasculature in congenital heart disease (CHD) patients. Methods 70 patients with CHD (mean 28 years, range: 10–65 years) were retrospectively identified in this single-center study. REACT-CMRA was applied with respiratory- and cardiac-gating. Image quality (IQ) of REACT-CMRA was compared to standard non-gated multi-phase first-pass-CMRA and respiratory- and electrocardiogram-gated steady-state-CMRA. IQ of different vessels of interest (ascending aorta, left pulmonary artery, left superior pulmonary vein, right coronary ostium, coronary sinus) was independently assessed by two readers on a five-point Likert scale. Measurements of vessel diameters were performed in predefined anatomic landmarks (ascending aorta, left pulmonary artery, left superior pulmonary vein). Both readers assessed artifacts and vascular abnormalities. Friedman test, chi-squared test, and Bland-Altman method were used for statistical analysis. Results Overall IQ score of REACT-CMRA was higher compared to first-pass-CMRA (3.5 ± 0.4 vs. 2.7 ± 0.4, P < 0.001) and did not differ from steady-state-CMRA (3.5 ± 0.4 vs. 3.5 ± 0.6, P = 0.99). Non-diagnostic IQ of the defined vessels of interest was observed less frequently on REACT-CMRA (1.7 %) compared to steady-state- (4.3 %, P = 0.046) or first-pass-CMRA (20.9 %, P < 0.001). Close agreements in vessel diameter measurements were observed between REACT-CMRA and steady-state-CMRA (e.g. ascending aorta, bias: 0.38 ± 1.0 mm, 95 % limits of agreement (LOA): − 1.62–2.38 mm). REACT-CMRA showed high intra- (bias: 0.04 ± 1.0 mm, 95 % LOA: − 1.9–2.0 mm) and interobserver (bias: 0.20 ± 1.1 mm, 95 % LOA: − 2.0–2.4 mm) agreements regarding vessel diameter measurements. Fat-water separation artifacts were observed in 11/70 (16 %) patients on REACT-CMRA but did not limit diagnostic utility. Six vascular abnormalities were detected on REACT-CMRA that were not seen on standard contrast-enhanced CMRA. Conclusions Non-contrast-enhanced cardiac-gated REACT-CMRA offers a high diagnostic quality for assessment of the thoracic vasculature in CHD patients.

Abstract:Vascular pulsation and cardiac motion compromise image quality in contrast enhanced magnetic resonance angiography (CE-MRA) in the thorax, resulting in blurring and ghosting artifacts. The use of ECG gating has been proposed in the past to mitigate these artifacts but previous methods suffered from increased scanner time because only a fraction of the cardiac cycle was used for image acquisition and from loss of the study when gating failed. We propose a robust ECG ordering of k-space for breath hold CE-MRA that acquires the central part of k-space in a motion-free portion of diastole and fills in from the periphery of k-space at all other times. To make maximal use of the contrast enhancement, data is acquired continuously even when the ECG signal is lost. The proposed sequence is shown to allow thoracic and pulmonary MRA with a higher resolution when compared to the conventional gated sequence.

Abstract:Purpose To develop and evaluate a highly efficient free‐breathing and contrast‐agent‐free three‐dimensional (3D) whole‐heart Cardiac Magnetic Resonance Angiography (CMRA) sequence at 0.55T. Methods Free‐breathing whole‐heart CMRA has been previously proposed at 1.5 and 3T. Direct application of this sequence to 0.55T is not possible due to changes in the magnetic properties of the tissues. To enable free‐breathing CMRA at 0.55T, pulse sequence design and acquisition parameters of a previously proposed whole‐heart CMRA framework are optimized via Bloch simulations. Image navigators (iNAVs) are used to enable nonrigid respiratory motion‐correction and 100% respiratory scan efficiency. Patch‐based low‐rank denoising is employed to accelerate the scan and account for the reduced signal‐to‐noise ratio at 0.55T. The proposed approach was evaluated on 11 healthy subjects. Image quality was assessed by a clinical expert (1: poor to 5: excellent) for all intrapericardiac structures. Quantitative evaluation was performed by assessing the vessel sharpness of the proximal right coronary artery (RCA). Results Optimization resulted in an imaging flip angle of 110∘ , fat saturation flip angle of 180∘ , and six k‐space lines for iNAV encoding. The relevant cardiac structures and main coronary arteries were visible in all subjects, with excellent image quality (mean 4.9/5.0 ) and minimal artifacts (mean 4.9/5.0 ), with RCA vessel sharpness (50.3%±9.8% ) comparable to previous studies at 1.5T. Conclusion The proposed approach enables 3D whole‐heart CMRA at 0.55T in a 6‐min scan (5.9±0.7 min ), providing excellent image quality, minimal artifacts, and comparable vessel sharpness to previous 1.5T studies. Future work will include the evaluation of the proposed approach in patients with cardiovascular disease.

Abstract:Current myocardial perfusion MRI acquisitions are performed with a saturation recovery sequence, in large part to minimize sensitivity to arrhythmia. A new approach is proposed here where the images are acquired ungated at steady state without use of a saturation pulse. The data are acquired continuously and reach steady state after the first few images. A confluence of advances has made this new paradigm of an ungated steady-state acquisition possible-very rapid undersampled readouts with new reconstruction technologies permit enough measurements that continuous acquisition becomes a feasible approach. Gating can be applied retrospectively from a logged electrocardiogram (ECG) or with self-gating methods. In this work, simulations and measurements in a concentration phantom are used to demonstrate that similar contrast and signal can be obtained with the standard saturation recovery and the proposed spoiled gradient echo (SPGR) acquisition. Specifically, for a flip angle of 14° and a saturation recovery time of 80 ms, similar signals are acquired over a range of T(1) s that reflect realistic myocardial tissue concentrations. Preliminary results in one subject are presented to show the potential of this new approach. The method may allow for cine cardiac perfusion and more signal-to-noise ratio-efficient acquisitions.

Abstract:PURPOSE:To develop a free-running framework for 3D isotropic simultaneous myocardial T1/T2 mapping and cine imaging.METHODS:Continuous data acquisition with 3D golden angle radial trajectory is used in conjunction with T2 preparation of varying echo times and inversion recovery (IR) pulses to enable simultaneous myocardial T1/T2 mapping and cine imaging. Data acquisition is retrospectively synchronized with ECG signal, and 1D respiratory self-navigation signal is extracted from the k-space center of all radial spokes. Respiratory binning is performed based on the estimated respiratory signal, enabling estimation and correction of 3D translational respiratory motion. Using high-dimensionality patch-based undersampled reconstruction with dictionary-based low-rank inversion, whole-heart T1/T2 maps and cine images can be generated with 2 mm isotropic spatial resolution. The proposed technique was validated in a standardised phantom and ten healthy subjects in comparison to conventional 2D imaging techniques.RESULTS:Phantom T1 and T2 measurements demonstrated good agreement with 2D spin echo techniques. Septal T1 estimated with the proposed technique (1185.6 ± 49.8 ms) was longer than with a conventional breath-hold 2D IR-prepared sequence (1044.3 ± 26.7 ms), whereas T2 measurements (47.6 ± 2.5 ms) were lower than a breath-hold 2D gradient spin echo sequence (52.0 ± 1.8 ms). Precision of the proposed 3D mapping was higher than conventional 2D mapping techniques. Ejection fraction measured with the proposed 3D approach (63.8 ± 6.8%) agreed well with conventional breath-held multi-slice 2D cine (62.3 ± 6.4%).CONCLUSIONS:The proposed technique provides co-registered 3D T1/T2 maps and cine images with isotropic spatial resolution from a single free-breathing scan, thereby providing a promising imaging tool for whole-heart myocardial tissue characterization and functional evaluation.

Abstract:Background: Contemporary advances in low-field magnetic resonance imaging systems can potentially widen access to cardiovascular magnetic resonance (CMR) imaging. We present our initial experience in building a comprehensive CMR protocol on a commercial 0.55 T system with a gradient performance of 26 mT/m amplitude and 45 T/m/s slew rate. To achieve sufficient image quality, we adapted standard imaging techniques when possible, and implemented compressed-sensing (CS) based techniques when needed in an effort to compensate for the inherently low signal-to-noise ratio at lower field strength. Methods: A prototype CMR exam was built on an 80 cm, ultra-wide bore commercial 0.55 T MR system. Implementation of all components aimed to overcome the inherently lower signal of low-field and the relatively longer echo and repetition times owing to the slower gradients. CS-based breath-held and real-time cine imaging was built utilizing high acceleration rates to meet nominal spatial and temporal resolution recommendations. Similarly, CS 2D phase-contrast cine was implemented for flow. Dark-blood turbo spin echo sequences with deep learning based denoising were implemented for morphology assessment. Magnetization-prepared single-shot myocardial mapping techniques incorporated additional source images. CS-based dynamic contrast-enhanced imaging was implemented for myocardial perfusion and 3D MR angiography. Non-contrast 3D MR angiography was built with electrocardiogram-triggered, navigator-gated magnetization-prepared methods. Late gadolinium enhanced (LGE) tissue characterization methods included breath-held segmented and free-breathing single-shot imaging with motion correction and averaging using an increased number of source images. Proof-of-concept was demonstrated through porcine infarct model, healthy volunteer, and patient scans. Results: Reasonable image quality was demonstrated for cardiovascular structure, function, flow, and LGE assessment. Low-field afforded utilization of higher flip angles for cine and MR angiography. CS-based techniques were able to overcome gradient speed limitations and meet spatial and temporal resolution recommendations with imaging times comparable to higher performance scanners. Tissue mapping and perfusion imaging require further development. Conclusion: We implemented cardiac applications demonstrating the potential for comprehensive CMR on a novel commercial 0.55 T system. Further development and validation studies are needed before this technology can be applied clinically.
