Abstract:Purpose: Potential factors influencing stereopsis were investigated in patients with both refractive accommodative esotropia (RAE) and amblyopia. Materials and methods: A retrospective chart review was performed to find out all patients with the diagnosis of both RAE and amblyopia. Patients are classified into two groups: group 1 (with stereopsis) and group 2 (without stereopsis). Onset age of RAE, history of strabismus in family members, time of amblyopia treatment, mean spherical equivalent, anisometropia, ocular movement disorders, especially, overaction of inferior oblique (IO) muscle, visual acuity difference (VAD) between eyes, best-corrected visual acuity (BCVA) levels of amblyopic and normal eyes and the presence of alternation of fixation (AOF) were investigated as possible factors. These factors were compared statistically between groups. Results: Groups 1 and 2 consisted of 21 and 26 patients, respectively. There was no statistical significant difference in terms of onset age of RAE, family history, amblyopia treatment, BCVA of normal eyes and anisometropia. IO overaction and higher VAD were found to be statistically different between groups (p: 0.019, p: 0.022, respectively). Besides, there was significant difference in terms of AOF and better BCVA in amblyopic eyes (p: 0.000, p: 0.009, respectively). Conclusion: IO overaction, BCVA in amblyopic eyes, VAD and AOF were found to be potential risk factors for the development of stereopsis in patients with both RAE and amblyopia.
Nonaccommodative Factors of Refractive Accommodative Esotropia.

Abstract:The clinical characteristics of 45 cases of refractive accommodative esotropia were compared with 31 cases of the control group who remained orthotropic in the presence of uncorrected same degree of hypermetropia. In addition to the general ophthalmologic and orthoptic examinations, the accommodative convergence/accommodation (AC/A) ratio, fusional vergences and random dot stereopsis were investigated. The hypermetropic patients without esotropia were found to have a lower AC/A ratio and normal fusional function, while in cases with refractive accommodative esotropia, their AC/A ratios were normal and their fusional functions were decreased, especially significant being the divergent fusional function. In both groups, their stereopsis was defective or absent. It is suggested that esotropia be not simply caused by excessive accommodation due to uncorrected hypermetropia, and the range of divergent fusional function and the level of AC/A ratio be the two factors which determine the causation of esotropia in cases with uncorrected hypermetropia.

Abstract:Clinical relevance: Acute acquired comitant esotropia (AACE) is becoming increasingly common in young children. Understanding clinical characteristics and treatment outcomes can inform future practice. Background: To evaluate clinical characteristics of myopic children and adolescents with AACE, and the outcomes of varying treatments. Methods: A retrospective review of medical records was conducted to identify patients diagnosed with AACE and diplopia between 2016 and 2020. The duration of smartphone use, angle of deviation, refractive error, stereopsis, and treatment options were analysed. The motor outcomes were evaluated at the earliest 6-month postoperative visit (6-30 months), and orthotropia or esodeviation ≤10 prism dioptres (PD) was defined as success. Successful sensory outcomes were defined as the elimination of diplopia in primary gaze or the presence of stereoacuity. Results: Twelve patients diagnosed with AACE were identified. Ages were between 9 and 19 years (15.6 ± 3.3 years). All patients showed convergent and comitant esotropia ranging from 12 to 40 PD at distance fixation that was nearly equivalent for distance and near fixation. The mean age at the initial visit was 15.6 ± 3.3 years. Post-treatment esodeviations were significantly lower at both distance and near compared to pretreatment (p < 0.001, for both). Eight patients required surgery and were treated with bilateral medial rectus muscle recession, two patients were treated with botulinum toxin, and two patients were followed with prismatic glasses. Nine of 10 patients (%90) treated with either surgery or botulinum toxin had successful motor and sensory outcomes. Conclusions: Surgery is the most efficient treatment option; botulinum toxin and prismatic glasses could be useful treatment options in patients who refuse to have surgery or for whom the amount of deviation is small and variable.
Accommodative esotropia: An outcome analysis from a tertiary center in Oman

Abstract:Aim: The aim of this study is to determine the outcome of accommodative esotropia (ET) and influencing factors in young Omani children. Subjects and methods: In this retrospective cohort, children diagnosed with accommodative ET who had followed up in a tertiary hospital from 2006 to 2011 were identified. Parameters studied included cycloplegic refraction and its change with time, ocular alignment, binocularity, visual acuity (VA), amblyopia, and requirement for surgery. Results: A total of 51 patients were identified. Twenty-four patients were diagnosed with fully accommodative ET (FAET) and 27 with partially accommodative ET (PAET). The mean (± standard deviation [SD]) age of onset and reporting were 2.6 (±1.58) and 3.2 (±1.84) years in the two groups, respectively. The mean (SD) cycloplegic refraction at presentation was 4.50 (±1.66) in the FAET group and 3.65 (±1.67) in the PAET group. Anisometropia was present in 28% of patients. The mean follow-up period was 4.9 years. The following were detected in the final visit. A reduction in amblyopia from 43% to 6% of patients, binocularity in 75% of patients, and a mean increase of 0.64 (±1.3) D in cycloplegic refraction from the first visit (P = 0.005). The mean angle of deviation at near and distance was 29.86 (±15.21) and 17.80 (±10.14) prism diopters, respectively, in FAET patients and 30.15 (±14.83) and 29.53 (±15.53), respectively, in PAET patients. Thirty-seven percent of the PAET patients underwent surgery within 5 years from diagnosis. All participants in this cohort continued to wear glasses in the last follow-up visit. Conclusion: Most children with refractive accommodative ET have an excellent outcome in terms of VA and binocular vision. The PAET group was characterized by delayed reporting, the presence of anisometropia, and lower hypermetropia. Further study is required to determine the possibility of weaning glasses in FAET patients.
Comparison of Long-term Stereoacuity Improvement Between Patients With Initial Subnormal Stereopsis and Nil Stereopsis in Refractive Accommodative Esotropia

Abstract:Purpose: To compare improvement in long-term stereoacuity between patients with refractive accommodative esotropia (RAET) with initial subnormal stereopsis (between 120 and 1,980 arcsec of stereoacuity) and nil stereopsis. Methods: The medical records of patients 4 years and older who had RAET with initial subnormal stereopsis and nil stereopsis and a minimum follow-up period of 5 years were retrospectively reviewed. Improvement in stereoacuity at the last follow-up visit and the factors that could influence it were compared between the initial subnormal stereopsis and the nil stereopsis groups. Results: A total of 79 patients (mean age: 6.3 ± 1.9 years) were included: 31 patients with initial subnormal stereopsis and 48 patients with nil stereopsis. The mean follow-up time was 11.7 ± 1.8 years (range: 5 to 21 years). At the last follow-up visit, a statistically significantly greater number of patients with initial subnormal stereopsis demonstrated improvement in stereoacuity and also achieved 60 arcsec of stereoacuity compared with those with nil stereopsis. Age at onset, duration of esodeviation, mean hyperopia, amblyopia, anisometropia, and follow-up duration were not significantly different between the initial subnormal stereopsis and the nil stereopsis groups. The initial mean near and distance deviations with hyperopic correction were significantly smaller in patients with initial subnormal stereopsis. A significantly greater number of patients with initial sub-normal stereopsis had fusion at distance. Conclusions: Patients with RAET with initial subnormal stereopsis have greater chances of stereoacuity improvement and recovery of 60 arcsec of stereoacuity than those with nil stereopsis. Patients who initially have nil stereopsis may develop normal stereoacuity. Smaller initial deviations with hyperopic correction and fusion at distance indicate a favorable prognosis for stereoacuity improvement. [J Pediatr Ophthalmol Strabismus. 2022;59(4):248-253.].
Factors affecting postoperative stereopsis in acquired nonaccommodative esotropia

Abstract:Objective: To identify and examine factors that affect postoperative binocularity in children diagnosed with acquired nonaccommodative esotropia (ANAET). Design: Retrospective chart review. Participants: Thirty-four pediatric patients who received surgery for ANAET by 1 surgeon (I.M.) at the Ivey Eye Institute, University of Western Ontario (London, Ont.). Methods: A multivariate logistic regression analysis was performed to evaluate the effect of the following factors on achieving stereopsis postoperatively: age at onset of esotropia (AO), duration of misalignment (DOM), presence of A/V pattern, recent intermittency of esotropia, and presence of dense amblyopia. Results: Forty-one percent of all patients achieved stereoacuity of 100 seconds of arc or better. AO was found to be the only factor significantly associated with postoperative stereopsis (adjusted odds ratio = 1.063, P<0.05). None of the children with AO who were younger than 30 months regained bifoveal fixation regardless of their DOM. Conclusions: Age of onset appears to play a crucial role on postoperative functional outcome in children with ANAET. Delayed surgery in children with an age of onset beyond 30 months did not seem to be detrimental to regaining bifoveal fixation.
Factors Influencing Stereopsis Outcomes in Adults Following Strabismus Surgery

Abstract:Objectives: The aim of the study was to evaluate binocular vision after adult strabismus surgery and to investigate the predictive factors on improvement stereoacuity. Methods: Patients aged upper from 16 years who underwent strabismus surgery in our hospital reviewed retrospectively. Age, existence of amblyopia, pre-operative and postoperatively fusion ability, stereoacuity, and deviation angle were recorded. Patients were divided into two groups according to final stereoacuity; 200 sn/arc and lower: Good stereopsis (Group 1), upper 200 sn/arc: Poor stereopsis (Group 2). Characteristics were compared between groups. Results: A total of 49 patients, who were 16-56 years of age, were included in the study. The mean follow-up time was 37.8 months (range 12-72 months). Of patients, 26 had improvement in stereopsis scores after surgery (53.0%). Group 1 includes 200 sn/arc and lower (n=18, 36.7%) and Group 2 includes higher than 200 sn/arc (n=31, 63.3%). The presence of amblyopia and higher refraction error was frequent significantly in Group 2 (p=0.01 and p=0.02, respectively). The existence of fusion postoperatively was significantly frequent in Group 1 (p=0.02). Type of strabismus and the amount of deviation angle were not found in a relationship with good stereopsis. Conclusion: In adults, surgical correction of horizontal deviation improves stereoacuity. Having no amblyopia, having fusion after surgery, and low refraction error are predictive for the improvement in stereoacuity.
The Surgical Management of Esotropia with High AC/A Ratio

Abstract:Objective To determine the effectiveness of strabismus surgery as an altemative treatment for high accommodative convergence/accommodation (AC/A) ratio (convergence excess) esotropia.Methods Sixty-five consecutive children with an AC/A ratio of distance-near disparity of 10 prism diopters (PD) or more were studied in a prospective design.Each patient was treated by recession of both medial rectus muscles for the full amount of esotropia measured at near fixation through the full distance hyperopic correction.The magnitude of esodeviation,the AC/A ratio,and the grade of binocularity were measured before and after strabismus surgery.Results The majority of patients were able to maintain satisfactory ocular alignment after surgery,the alignment rates were 76.3％ (after lweek),82.8％ (after 1 month),85.7％ (after 6 months),92.1％ (after 1 year).The grade of binocular fusion or stereopsis improved significantly in all of the patients.Conclusions These results demonstrate that strabismus surgery may be more effective in restoring normal stereopsis and binocular fusion for high AC/A patients.
Acquired comitant esotropias - comparison of surgical outcomes of accommodative vs non-accommodative types

Abstract:Purpose: To compare the motor and sensory outcomes of strabismus surgery and the factors affecting surgical success in acquired acute non-accomodative esotropia (ANAET) and partially accommodative refractive esotropia (pARET). Methods: A retrospective chart review of patients with ANAET and pARET who underwent unilateral or bilateral horizontal rectus muscle surgery between January 2020 and December 2021 was conducted. Patients with postoperative follow-up of at least six weeks were included. Patients with pattern deviation, lateral incomitance, and near-distance disparity were excluded. Motor success was defined as a postoperative deviation within eight prism diopters of orthophoria. Sensory success was defined as presence of binocular single vision (BSV) for both distance and near (Worth four dot test). The effect of factors like age at onset, age at surgery, amblyopia before surgery, duration of squint before treatment, presence or absence and magnitude of vertical deviation, preoperative angle of deviation, and spherical equivalent on the motor and sensory success in each group were analyzed and compared. Results: 38 patients with ANAET and 33 patients with pARET were included. The mean age of onset of esotropia was 8.55 ± 4.65 years and 4.39 ± 2.27 years (p < .001) and the mean age at surgery was 10.62 ± 4.99 years and 7.89 ± 2.84 years (p = .006) in the ANAET and the pARET group respectively. The mean duration of the final follow-up was 38.51 weeks in the ANAET and 48.68 weeks in the pARET group (p = .089). Patients were successfully aligned for both distance and near in 81.5% of patients in the ANAET and 78.9% of patients in the pARET group at the final follow-up (p. 0.775). A BSV for both distance and near at the final follow-up was seen in 81.2% vs 66.6% of patients in the ANAET and the pARET group respectively (p = .25). A good near stereoacuity (<120 arcsecs) was seen in 60.6% and 41.9% of the ANAET and the pARET groups respectively (p = .175). The percentage of patients in the ANAET group who had orthophoria, any esodeviation, and any exodeviation for distance at the final follow-up was 63.1%, 34.2% and, 2.6%. The percentage of patients in the pARET group in similar categories was 36.3%, 42.4% and, 21.2%. None of the demographic and preoperative factors were found to affect the surgical outcomes in the two groups. Conclusions: The motor and sensory outcomes were similar in the two groups. A higher proportion of ANAET patients remained orthophoric during the follow-up. The patients in the pARET group showed a tendency toward exodrift.
Characteristics and Surgical Outcomes of Comitant Esotropia in an Adult Population Between 18 and 60 Years Old

Abstract:Purpose: To describe the clinical characteristics and surgical outcomes of adults with comitant nonaccommodative esotropia. Design: Retrospective case series. Methods: Retrospective review of medical records of patients 18 to 60 years old with comitant esotropia who underwent strabismus surgery at a tertiary eye care center between 2014 and 2023. The etiology of esotropia was categorized into three groups based on the disparity between near-distance angles of deviation: (1) basic esotropia (ETBA); (2) esotropia divergence insufficiency pattern; or (3) esotropia convergence excess pattern. The main outcome measures were strabismus characteristics and motor and sensory surgical outcomes. Surgical motor success was defined as a deviation that measured ≤10 prism diopters (PD). Results: Of the 219 that met the inclusion criteria, most patients were female (140, 64%) and had a mean age of 36.7 ± 12.3 years (range 18-60 years). The majority were myopic (157, 72%) and reported diplopia (176/219, 80.3%). The esotropia convergence excess pattern group had the largest mean deviations at both distance (45.5 ± 11.5 PD) and near (64 ± 12.3 PD) while the ETBA group had the largest ranges at distance (31 ± 13.5 PD, range 3-90) and near (30 ± 15 PD, range 2-85). Bilateral medial rectus recession and unilateral recess-resect procedures were performed with equal frequency (both 48%). Motor and sensory success were achieved more often with recess-resect than bilateral medial rectus recession, although only motor success was statistically significant (87.8% vs. 73.2%, P = .0375 and 93.3% vs. 85.5%, P = 15, respectively). At the last encounter, 88.1% (119/135) of patients with preoperative diplopia achieved single binocular vision. Conclusions: Regardless of the pattern of esotropia, strabismus surgery in adults with comitant nonaccommodative esotropia resulted in good motor and sensory outcomes.
Predictive factors of stereopsis outcomes following strabismus surgery

Abstract:Purpose: To determine the predictive factors of post-operative stereopsis in patients with strabismus. Method: In this retrospective study, records of patients who received surgical treatment for strabismus were reviewed. All types of strabismus were included. Pre- and post-treatment stereoacuity were measured using the Titmus Stereo Fly test, and predictive factors of stereopsis were evaluated. Results: A total of 194 patients (132 females and 62 males) with a mean age of 14.8 ± 8.4 years were included. There was a statistically significant improvement in stereopsis following surgery (p value < 0.001). Patients with a higher amount of deviation at baseline had poorer stereopsis on the final examination (p value < 0.001). Stereopsis improvement was more prominent in the pure horizontal strabismus group, compared to combined horizontal and vertical deviations. Baseline and the final stereopsis were higher in the "exotropia" group as compared to the "esotropia" group (p value = 0.003 and 0.0155, respectively); however, the within group change of stereopsis was not significantly different between these two groups (p value = 0.144). Post surgical residual deviation was associated with a poorer stereopsis (p value = 0.002, r = 0.251). A longer duration of strabismus before surgery was associated with poorer final stereopsis levels (p value = 0.026). The presence of amblyopia before surgery was associated with poorer stereopsis on last examination (p value < 0.001 for both correlations). Conclusion: Based on the result of this study, final stereopsis after strabismus surgery could be affected by the type, duration, and the amount of deviation before surgery, amblyopia, and post operative ocular deviation.
Acquired distance esotropia associated with myopia

Abstract:BACKGROUND: Von Graefe (1864)(1) and Bielschowsky (1922)(2) described acquired progressive esotropia, with typically a larger angle for distance than near, associated with moderate myopia. This is unlike the esotropia seen in patients with high myopia.METHODS: We reviewed our patient records, finding 26 cases: 14 were female, 12 male. Age range at first presentation was 37-74 with a mean of 55.5. The myopia was from -0.75 to -10.00, with a mean of -5.50 in each eye. Eighteen patients mostly wore glasses, eight mainly wore contact lenses. All complained of horizontal diplopia. Seventeen (65%) had been treated with prisms with a gradual increase in power.RESULTS: Six patients (23%) continued with prisms although surgery has been offered in three cases. Seven patients (27%) had botulinum toxin to one medial rectus muscle. In two cases this proved to be successful maintenance therapy, the remaining five have gone on to have surgery. Seventeen (65%) have undergone surgery (mainly unilateral medial rectus recession and lateral rectus resection) with relief of diplopia and discarding of prisms. Three patients required further surgery for a recurrence after 10-12 years and another is using prisms for a recurrence after 11 years. One patient has suffered an early recurrence 6 months following surgery and is having his neurological work-up repeated.CONCLUSION: This unusual sub-type of strabismus is a benign entity, which responds well to prism correction or surgery in cases who wish to wear contact lenses or whose prisms become inconveniently strong.
Long-term Ocular Alignment and Sensory Outcomes After Medial Rectus Recession for High AC/A Ratio Esotropia

Abstract:Purpose: To report long-term ocular alignment and sensory outcomes after medial rectus recession for high accommodative convergence/accommodation (AC/A) ratio esotropia. Methods: The medical records of consecutive patients who had undergone unilateral or bilateral medial rectus recession for high AC/A ratio esotropia and were observed postoperatively for a minimum of 5 years were reviewed retrospectively. Results: A total of 34 patients were included. Twenty-three patients (68%) used bifocals preoperatively. The mean age at surgery was 11.5 ± 4.4 years (range: 2.5 to 19.0 years). The mean postoperative follow-up was 7.5 ± 2.3 years (range: 5.0 to 15.25 years). Overall, 21 patients (62%) had surgical success at their last follow-up visit. Age at surgery, preoperative angle of distance and near deviation, distance-near disparity, and preoperative bifocal wear did not predict motor outcome after surgery. Preoperative presence of peripheral binocular single vision was a significant favorable factor for surgical success. At the last follow-up visit, 21% of patients had a recurrence of high AC/A ratio esotropia and 9% each had consecutive exotropia (intermittent [3%] and constant [6%]) and basic esotropia. Peripheral binocular single vision was achieved in 64% of patients and stereopsis in 28%. Bifocal segment was eliminated postoperatively in 70% of patients. Conclusions: Nearly two-thirds of patients with high AC/A ratio esotropia achieved a successful long-term motor outcome and peripheral binocular single vision, and nearly one-fourth achieved stereopsis. Recurrence of high AC/A ratio esotropia occurred in some patients, and consecutive exotropia and basic esotropia in a few. Bifocal segment was eliminated postoperatively in 70% of patients. Long-term development of refractive error in refractive, nonrefractive and partially accommodative esotropia

Abstract:Accommodative esotropia (AE) is a convergent deviation due to an excess of the convergence linked to the innervational stimulus for accommodation and it is usually associated to the presence of hyperopia. The development of hyperopia over time has been widely described but the lack of comparative analysis among accommodative esotropia subtypes, does not allow to verify the existence of different developmental patterns. In this study we aimed to describe the long term changes of refractive error in patients affected by accommodative esotropia as a function of the strabismus type: refractive (RAE), non-refractive (NRAE) and partially accommodative esotropia (PAE). The medical records of 66 patients (24 RAE, 22 PAE, 20 NRAE), who wore the full correction of their cycloplegic refraction error during the entire follow up period, were retrospectively reviewed. Mean outcome was the analysis of the variations, among groups, of refractive error over time; differences between mild (≤3.00D) and high (≥5.00D) hyperopia, effects of amblyopia and relationship with AC/A ratio were also investigated. All patients were followed up from approximately 4 years of age to 20, with mean follow up of 16.06±0.29 years. Our results described similar non-linear trend of refractive error development among groups. The initial increase of hyperopia (up to 7-8 years) was followed by a decreasing trend persisting up to adulthood (approximately 20 years of age). During this period, spherical equivalent decreased at a lower mean annual rate in patients affected by RAE (-0.07D) compared to other groups (NRAE -0.11D, p = 0.003 and PAE 0.13D, p = 0.002). In all groups, however, significant amount of hyperopia was found at last examination; indeed the observed difference in SE values from baseline to the end of follow up, was not significant (RAE: +0.27D, 95% C.I. -0.49 to +1.04D, p = 0.472; PAE: -0.69D, 95% C.I. -1.67 to +0.28D, p = 0.154; NRAE: -0.39D, 95% C.I. -1.11 to +0.34D, p = 0.278). AC/A ratio, amblyopia and amount of initial hyperopia appeared to have no significant role in longitudinal change of the refractive error. In conclusion, this study provides a complete overview, from childhood to adulthood, of refractive error development in different form of accommodative esotropia.
Clinical characteristics of acute acquired concomitant esotropia among different age groups

Abstract:AIM:To analyze the clinical characteristics of acute acquired concomitant esotropia(AACE)in patients among different age groups. METHODS:Retrospective analysis of clinical data.A total of 112 non-Swan type AACE patients who underwent surgery at Peking University People's Hospital from January 2015 to December 2022 were retrospectively analyzed.Clinical data were collected and the characteristics were compared,including gender,age,diopter,duration of disease,daily time spent on near work,angles of deviation before and after surgery,stereopsis,etc.According to age,patients were divided into three groups:＜18 years old group(22 cases),18-45 years old group(67 cases),and＞45 years old group(23 cases).The clinical characteristics of patients were compared in each group. RESULTS:A total of 112 patients were included in the study,comprising 56 males and 56 females,with a median age of 29.50(19.25,41.75)years old.Among them,97 patients had myopia(86.6％).There were 93 patients(83.0％)who spent more than 8 h on near work.The age group＜18 years old had the shortest duration before surgery,with a median time of 1.00(0.50,1.00)a,the minimum negative diopter,with a median diopter of-0.75(-3.19,-0.56)D in the right eye and the diopter of-1.25(-2.81,-0.75)D in the left eye,and the maximum preoperative near angle of deviation,with a median angle of 30.00(18.50,80.00)PD,and the maximum preoperative distant angle of deviation,with a median angle of 35.00(23.75,80.00)PD.All these differences were statistically significant compared with other two groups(both P＜0.05).For the age group from 18 to 45 years old,the median near angle of deviation was 20.00(14.00,30.00)PD,and the median distant angle of deviation was 25.00(20.00,35.00)PD,both of which were higher than those in the age group＞45 years old(both P＜0.05).For the age group＞45 years old,the median near angle of deviation after surgery was-4.50(-7.50,0)PD,and the median distant angle of deviation after surgery was 4.50(0,9.50)PD,which were smaller than those in other two groups(all P＜0.05).The age group＞45 years old had the hiughest surgical success rate(100％).The preoperative stereopsis was better in age group＞45 years old than the group＜18 years old(P＜0.05).The postoperative stereopsis of the age group of 18 to 45 years old and the age group＞45 years old was better than age group＜18 years old(both P＜0.05). CONCLUSION:Surgical patients with AACE are mainly in the age group from 18 to 45 years old.The characteristic of angle of deviation is that distant angle of deviation is greater than near angle of deviation.The patients＜18 years old have larger preoperative angles of deviation than adults,while their stereoacuity is worse than adults in the early postoperative period.It is recommended that augmented-dose surgery should be performed in AACE patients who are in the age group of 18 to 45 years old(5-10 PD).A conservative surgery should be designed for hyperopia young children without established binocular vision.
Age-related distance esotropia--fusional amplitudes and clinical course

Abstract:Background: Age-related distance esotropia (ARDET) is a form of acquired strabismus that affects elderly patients and manifests as an esotropia greater with distance fixation. Limited information exists regarding fusional amplitudes and deficient divergence in this disorder. Methods: In this retrospective study, patient characteristics, ocular alignment and motility, and clinical course of patients with ARDET were recorded. Fusional amplitudes were analyzed to assess whether deficient divergence was present in patients with ARDET. Results: Twenty patients with ARDET were identified. Median age was 77.5 years (range, 59 to 89 years) and median best-corrected visual acuity was 20/25. Median esotropia angle with distance fixation was 5.5Δ (range, 2 to 18Δ); median angle with near fixation was esotropia 2.5Δ (range, exotropia 3Δ to esotropia 13Δ). Fusional divergence amplitudes were decreased in all but two patients. The median amplitude with distance fixation was 4.5Δ for break (range, 1 to 11Δ) and 2Δ for recovery (range, 0 to 9Δ). In 5 patients, the fusional divergence amplitude was as large or larger than the esotropia angle. Most patients remained stable throughout a mean follow-up of 18 months (range, 3 to 37 months). Nineteen patients were managed with prisms. Strabismus surgery was performed in one patient. Conclusions: Fusional divergence amplitude was deficient in most but not all patients with ARDET. In this study, most patients with ARDET remained stable and free of diplopia with prism treatment.

Abstract:Background/objectives: To analyze the surgical and sensory outcomes of intermittent exotropia according to refractive errors and the factors associated with surgical success. Subjects/methods: A total of 326 children were divided into three groups according to preoperative refractive error; hyperopic eyes with SE ≥+2D (hyperopic group), eyes with SE between -1D and +2D (emmetropic group), and myopic eyes with SE ≤-1D (myopic group). The surgical outcomes and the sensory outcomes measured by near and distant stereoacuity were compared among the three groups. Results: The surgical success rate in hyperopic group was significantly lower compared to myopic group at last follow-up (P = 0.012). Preoperative near stereopsis was not different among three groups, however, the distance stereopsis was significantly deteriorated in the hyperopic group compared to the other two groups (Titmus; P = 0.168, FD2; P < 0.001, DR; P = 0.048). There was postoperative improvement in both near and distant stereopsis in all three groups (Titmus; P = 0.009, FD2; P = 0.021, DR; P = 0.036) and no significant difference was found in the postoperative distant stereopsis among the three groups. Conclusions: Preoperative refractive error is a prognostic factor of surgical success in patients with intermittent exotropia. Patients with hyperopia achieved less favorable surgical outcome compared to myopic patients. The preoperative distant stereoacuity was decreased in hyperopic patients compared to myopic patients, which eventually improved after surgery and showed no significant difference at postoperative measurements.
The role of stereopsis and early postoperative alignment in long-term surgical results of intermittent exotropia

Abstract:Despite the presence of good stereopsis, surgically corrected intermittent exotropes may show a gradual outward shift with time. The purpose of this study was to identify early factors that influence the success of corrective surgery. Sixty-seven consecutive patients treated surgically for intermittent exotropia (age range 1.9 to 76 years) constituted our final sample. Sensory status was evaluated with the Titmus stereotest. A successful surgical result was defined as phoria or intermittent tropia of +/- 10 prism dioptres or less for near and distance with maintained or improved stereopsis, 6 months or more after surgery. The average postoperative follow-up period was 3.3 (range 0.5 to 11) years. Good stereopsis (60 seconds of arc or better) was present in only 42% of the patients preoperatively and 61% postoperatively. The overall surgical success rate was 60%. Early overcorrection, defined as an esodeviation present within the first 4 weeks after surgery, caused a significant improvement in the success rate, to 79% (p = 0.03). Poor preoperative stereopsis combined with early overcorrection resulted in a further significant improvement in the surgical success rate, to 92% (p = 0.07). This suggests that poor preoperative stereopsis does not necessarily preclude, and may enhance, long-term alignment stability postoperatively. We suggest that it is peripheral rather than central fusion that keeps the eyes well aligned after surgery and prevents recurrence of the exodeviation.

Abstract:Management of patients with intermittent esotropia having either normal or high accommodative convergence to accomodation (AC:A) ratios is discussed. Treatment should be based on liberal use of miotics to build up fusional reserves and use of hyperopic corrections or bifocals or both whenever specifically indicated. Divergent fusional reserve training is time-consuming and the cost/benefit ratio so low that most patients and doctors prefer the former treatment.

Abstract:We investigate the changes in refractive error and clinical evaluation in partially accommodative esotropia(PAET) after surgery. A total of 68 patients PAET who received at least 2 years of follow-up after surgery were enrolled in this study. We performed a retrospective study in patients who underwent unilateral or bilateral medial rectus recession for a non-accommodative component of PAET between January 2005 and March 2013. Patients were divided into groups according to the presence of dominancy (dominant, non-dominant, alternative eye), and presence of amblyopia (amblyopic, fellow, normal eye). Changes and changing pattern in SE refractive error were analyzed in all patients and compared between groups. Patients were divided into two groups, those weaned off of hyperopic glasses and those who continued using them, then factors that significantly influenced the continued use of glasses were analyzed. The changes and changing pattern in SE refractive error according to time after operation and presence of amblyopia or dominancy. The mean length of follow-up was 4.89±1.74 years after surgery and the mean change in SE refractive error rate per year was -0.284±0.411 diopters (D). The pattern of changes in the mean SE refractive error for those with dominant, non-dominant, and alternative eyes was not significantly different (p = 0.292). The pattern of changes in the mean SE refractive error for those with amblyopic, fellow, and normal eyes was significantly different (p = 0.0002). Patients were successfully weaned off of hyperopic glasses at an average age of 9.41±2.74 years. The average SE refractive error in the group weaned off of hyperopic glasses was significantly lower than that in the group maintained on hyperopic glasses (p = 0.0002). The change of SE refractive error in amblyopic eyes decreased less than that in fellow or normal eyes, which may be correlated with the presence of amblyopia. Patients with a smaller esodeviated angle without hyperopic correction, a lower degree of hyperopia, and who were older at the time of disease onset were discontinued from hyperopic glasses sooner after surgery.
