Abstract:Objective To elucidate the difference between choroidal thickness (CT) in primary open-angle glaucoma (POAG) and normal subjects and to compare the CT measured using spectral domain optical coherence tomography (SD-OCT) and swept source optical coherence tomography (SS-OCT). Methods and analysis This cross-sectional observational study included 25 eyes of 17 POAG subjects (cases) and 31 eyes of 20 normal subjects (controls). All the patients underwent complete ophthalmologic examination, enhanced depth imaging, SD-OCT and SS-OCT. In both controls and cases, the CT was measured in seven predetermined points in macular and peripapillary area and were compared. Results Choroid was significantly thicker on SS-OCT compared with SD-OCT in peripapillary and macular area in both cases and controls, except for inferior peripapillary in controls. The CT was not different from glaucoma and controls in peripapillary or macular area on SD-OCT (p>0.05), however, the CT was significantly thicker in glaucoma compared with controls in peripapillary area on SS-OCT (p<0.05) except inferior peripapillary (p=0.13). There was good intraobserver (±20 μm) and interobserver (±55 μm and ±45 μm) agreement on both SD-OCT and SS-OCT respectively. On SD-OCT, choroid was thinnest at the temporal peripapillary and thickest at subfoveal location in controls. In POAG, choroid was thinnest at inferior peripapillary region and thickest (500 μm) nasal to the fovea. On SS-OCT, choroid was thinnest at inferior peripapillary and thickest at the temporal peripapillary area in both cases and controls. Conclusion CT measurements on SS-OCT were higher than the SD-OCT measurements possibly due to better delineation of the sclerochoroidal junction on SS-OCT. CT was significantly thicker in glaucoma subjects compared with controls in peripapillary area on SS-OCT but not in macular area.

Abstract:PURPOSE:To evaluate the quantitative characteristics of choroidal thickness in primary open-angle glaucoma (POAG), normal tension glaucoma (NTG) and in normal eyes using spectral-domain optical coherence tomography (SD-OCT). To evaluate the diagnostic ability of choroidal thickness in glaucoma and to determine the correlation between choroidal thickness and visual field parameters in glaucoma. METHODS:A total of 116 subjects including 40 POAG, 30 NTG and 46 healthy subjects were enrolled in this study. Choroidal thickness measurements were acquired in the macular and peripapillary regions using SD-OCT. All subjects underwent white-on-white (W/W) and blue-on-yellow (B/Y) visual field tests using Humphrey Field Analyzer. The receiver operating characteristic (ROC) curve and the area under curve (AUC) were generated to assess the discriminating power of choroidal thickness for glaucoma. Pearson's correlation coefficients were calculated to assess the structure function correlation for glaucoma patients. RESULTS:No significant differences were observed for macular choroidal thickness among the different groups (all P > 0.05). Regarding the peripapillary choroidal thickness (PPCT), significant differences were observed among the three groups (all P < 0.05). Post hoc tests for multiple comparisons revealed a significant difference in the NTG-normal comparison group (all P < 0.01). The inferior and temporal PPCT in POAG patients were significantly thinner than those in normal subjects (P = 0.007, P = 0.002, respectively). Different parameters of PPCT showed significantly low diagnostic values to detect POAG from normal subjects (AUC: 0.555 to 0.652) and to discriminate NTG from POAG (AUC: 0.462 to 0.702), but moderate diagnostic power to detect NTG from normal subjects (AUC: 0.708 to 0.771). Regarding the diagnosis of early glaucoma, different parameters of PPCT showed relatively low diagnostic power (AUC: 0.606 to 0.698). In all the glaucoma subjects, PPCT was not significantly correlated with W/W mean deviation (MD) (all P > 0.05), but showed significant correlations with B/Y MD (all P < 0.05). In the early glaucomatous eyes, PPCT showed significant correlations with W/W MD and B/Y MD (all P < 0.05). CONCLUSIONS:In our study, peripapillary choroidal thickness measured on OCT showed a low to moderate but statistically significant diagnostic power and a significant correlation with blue-on-yellow visual field indices in glaucoma. This may indicate a potential adjunct for peripapillary choroidal thickness in glaucoma diagnosis.

Abstract:Optical coherence tomography (OCT) is a non-contact,good repeatability technique to obtain fastly high-resolution images from the anterior and posterior segments of the eye,and it has been widely used in the field of ophthalmology.Previous studies have shown that the thickness of circumpapillary retinal nerve fibre layer (cpRNFL) and optic disc assessment with OCT are valuable in diagnosing glaucoma.Traditional time domain (TD) OCT can measure macular nerve epithelium thickness.However,spectral-domain(SD) OCT allows a higher image resolution and can measure the thickness of retinal ganglion cell complex,which has less variablity in normal subjects so to be used as better indicator for early diagnosis in glaucoma.In this paper,we review the macular thickness measurement with OCT in glaucoma early diagnosis.

Abstract:PURPOSE To compare the diagnostic ability of macular ganglion cell inner plexiform layer (mGCIPL) and peripapillary retinal nerve fiber layer (pRNFL) thickness on optical coherence tomography (OCT) and macular and peripapillary perfusion changes using OCT angiography (OCTA) in glaucoma suspect and early primary open angle glaucoma (POAG). METHODS Ninety patients (30 normal, 30 glaucoma suspects, and 30 early POAG) were recruited in this cross-sectional, prospective study. The average thickness of mGCIPL and pRNFL on spectral domain-OCT and macular vessel density (VD), optic nerve head (ONH) perfusion, and ONH flux index (FI) on OCTA were evaluated for early diagnosis of glaucoma. RESULTS Macular VD, ONH perfusion, and ONH FI were significantly reduced in early POAG eyes compared to normal. The best correlation was observed between ONH FI and the average RNFL in both glaucoma suspects (r = 0.47, P < 0.01) and early POAG patients (r = 0.53, P < 0.01). Out of all the measured OCTA parameters, only ONH perfusion was significantly lower in glaucomatous eyes compared to glaucoma suspects (P < 0.001). Average GCIPL (0.82) and macular VD (0.76) had the highest area under the receiver operating characteristic (AUROC) curve value among all the OCT and OCTA parameters, respectively, for differentiating glaucoma suspects from controls. Rim area (0.92) and ONH FI (0.81) had the highest AUROC value among all the OCT and OCTA parameters for differentiating early POAG patients from controls. CONCLUSION OCTA vascular parameters had a good correlation with structural damage both at the disc and the macula. OCT parameters were superior to OCTA parameters for diagnosis of glaucoma, although OCTA parameters are deranged very early in the disease.

Abstract:Purpose: To study the subfoveal choroidal thickness (SFCT), macular ganglion cell layer (GCL-IPL) and central macular thickness (CMT) in Caucasian children, and to analyze these optical coherence tomography (OCT) parameters depending on the spherical equivalent (SE). Methods: Cross-sectional study of SFCT, GCL-IPL, and CMT in Caucasian children, analyzed with spectral-domain OCT Cirrus 5000 and Enhanced-depth imaging technique. Correlation between these three OCT parameters, age, sex, and spherical equivalent was analyzed. The eyes were classified into three groups: group 1 included eyes with SE +2.00. Results: Hundred ninety-eight eyes of 121 subjects were studied. The mean age was 9.22 years (range 3–16); 61.1% were female. The mean SFCT was 351.04 ± 84.08 μm, being 310.04 ± 82.84μm in group 1 (n = 62), 373.14 ± 83.16 μm in group 2 (n = 71) and 365.18 ± 73.16 μm in group 3 ( n = 65); statistically significant differences were found between groups 2 and 3, compared with group 1. GCL-IPL thickness was significantly thinner ( p < 0.001) in group 1, compared with group 3. There were no statistically significant differences between the three groups regarding CMT. Correlation with age, and sex was not found. Conclusions: SFCT and GCL-IPL thickness were significantly thinner ( p < 0.001) in myopic children when compared with a non-myopic pediatric population. However, it seems that there is not a correlation among the three OCT parameters studied, age and sex, when they are analyzed depending on refractive error.

Abstract:Background Vessel density (VD) of the elderly ocular hypertension patients measured by optical coherence tomography angiography (OCTA) have been reported. However, the studies of VD in juvenile ocular hypertension (JOHT) are limited. We wished to evaluate VD changes using OCTA in JOHT. We also investigated the potential risk parameters of intraocular pressure (IOP) and vertical cup/disc ratio (CDR) with OCTA for observing the development of JOHT. Methods We examined 86 eyes in 45 control (Ctrl) subjects and 65 eyes in 34 patients with JOHT using OCTA at the glaucoma clinic of the Eye, Ear, Nose, and Throat Hospital of Fudan University. The VD of radial peripapillary capillaries (RPC) and the perifoveal superficial vascular plexus (SVP) was compared between the Ctrl and JOHT groups. Other basic study factors such as age, sex, blood pressure, best-corrected visual acuity, central corneal thickness, IOP, CDR, the thickness of the retinal nerve fiber layer, ganglion cell complex, visual field mean deviation, and pattern standard deviation were also recorded. Results Bare difference was found in the nasal-inferior and temporal RPC-VD between the Ctrl and JOHT groups ( P = 0.042 and P = 0.033, respectively) while SVP-VD was not (all P > 0.05). In the mixed linear regression model analysis, temporal RPC-VD was marginally negatively associated with high IOP (r = − 1.379, P = 0.043). Five additional sections of nasal, inferior-nasal, inferior-temporal, superior-temporal, and superior-nasal RPC-VD showed positive correlation with large CDR (all P < 0.05). SVP-VD in the superior and nasal regions was marginally negatively correlated with high IOP (r = − 1.877, P = 0.023; r = − 1.693, P = 0.049). No other regions were found statistical different of relationship between IOP, CDR and VD. Conclusions Nasal-inferior and temporal peripapillary VD was marginally lower in JOHT subjects. Regarding parameters of IOP and CDR, peripapillary temporal VD had a borderline level of negative correlation with IOP more than 21 mmHg while additional five regions were strongly positively correlated with large CDR. Some macular regions only found marginal positive correlation with parameter of high IOP. We conclude that OCTA can be used as a potential technique to evaluate the VD in JOHT and peripapillary scans should be analyzed individually based on different levels of CDR.
Correlation Analysis between Macular Choroidal Thickness and Visual Field Mean Defect in Primary Open-Angle Glaucoma

Abstract:Purpose. To evaluate the differences in macular choroidal thickness and volume between patients with primary open-angle glaucoma (POAG) and healthy controls to assess the correlation between macular choroidal thickness and visual field mean defect (MD). Methods. Case-control study. A total of 101 patients (101 eyes) with POAG who were hospitalized in our hospital and 102 healthy subjects (102 eyes) matched by age, sex, and axial length were consecutively selected as the POAG group and the control group, respectively. The macular choroidal thickness and volume in nine regions were measured in all subjects by enhanced-depth imaging optical coherence tomography (EDI-OCT). Results. The mean thicknesses of the choroid in the macular area in the POAG group and healthy group were 207.97 ± 62.83 μm and 208.24 ± 47.97 μm, and the mean volumes were 0.63 ± 0.19 μm3 and 0.64 ± 0.14 μm3. There were no significant differences in macular choroidal thickness, volumes of various macular regions, or mean choroidal thickness or volume between the POAG and healthy groups (all p > 0.05 ). The macular choroidal thickness of various macular regions was not correlated with visual field MD in the POAG group (all p > 0.05 ). Conclusion. The macular choroidal thicknesses and volumes in POAG patients were not significantly different from those in healthy individuals. The macular choroidal thickness was not correlated with MD in POAG patients. Therefore, macular choroidal thickness is not an appropriate parameter to evaluate damage caused by POAG, and the role of the macular choroid thickness in POAG needs to be further investigated.

Abstract:The most common cause of permanent vision loss or blindness in the world is glaucoma. Primary Open Angle Glaucoma (POAG) develops when the eye drainage canals clog over time, increasing intraocular pressure and damaging the optic nerve. In POAG, an ophthalmology examination usually shows an open anterior chamber and chronic, irreversible multifactorial optic neuropathy followed by central visual field loss. OCT is specifically employed to investigate the elasticity of tissue, particularly in Schlemm's canal. This study aims to analyzed the morphological parameters of Schlemm's canal among patients with POAG and normal healthy control. In this observational study, we included one hundred patients, among them 50 patients had POAG, and 50 healthy controls, and both eyes were assessed. We measured the study cohort's and the healthy cohort's eyes' elevated IOP and the Schlemm's canal cross-sectional area (CSA). RT-Vue OCT was used to analyze both eyes' temporal, superior, inferior, and nasal regions. The data were analyzed using both univariate and bivariate methods. The characteristics of the morphological measurement of Schlemm's canal results were similar to those of Kagemann et al. There was a significant difference between age and SC measurement, but no correlation was observed between SC measurement and IOP or visual acuity. OCT is a useful way to evaluate the morphological status of Schlemm Canal in glaucoma patients. The eyes with POAG had smaller Schlemm's Canal- CSA than the normal eyes. The Schlemm canal-CSA in Chinese people had no differences from that of peoples of other races.

Subfoveal Choroidal Thickness in Diabetic Patients without Retinopathy versus Normal Individuals Using Optical Coherence Tomography

Abstract:Background: Diabetes mellitus (DM) is a prevalent systemic ailment. The incidence of this condition varies from 8.3% to 11.6% among different ethnic groups in the general population. Objective: The objective is to compare the subfoveal choroidal thickness between diabetic patients without retinopathy and healthy individuals, utilizing Optical Coherence Tomography (OCT) as the primary diagnostic tool. Individuals and Methods: This study was conducted on 40 individuals who attended the Ophthalmology outpatient Clinic at Al-Zahraa University Hospital. Results: The mean age of the cases ranged between 18 and 53 year (mean ± SD was 37.65 ± 6.91 years. Twenty-five were females (62.5%) and 15 were males (37.5%). The mean subfoveal choroidal thickness (SFCT) in healthy individual was 272±13.79 (μm), while in diabetic patients without retinopathy it was 269±12.1 (μm). In our study there was non-statistically significant difference in right, left and average SFCT found between the patients group and the normal group (p-values were 0.428, 0.805 and 0.529 (μm) respectively). Conclusion: Diabetes mellitus is amicrovascular systemic disease, so it affects choroidal circulation lately with development of diabetic retinopathy. Diabetic patients without retinopathy had no significant changes in the subfoveal choroidal thickness. Spectral domain optical coherence tomography with enhanced depth imaging is an objective, fast, reliable, high sensitive, and a non-invasive technique used for measurement of subfoveal choroidal thickness.

Abstract:AIM: To compare the three-dimensional choroidal vascularity index (CVI) and choroidal thickness between fellow eyes of acute primary angle-closure (F-APAC) and chronic primary angle-closure glaucoma (F-CPACG) and the eyes of normal controls. METHODS: This study included 37 patients with unilateral APAC, 37 with asymmetric CPACG without prior treatment, and 36 healthy participants. Using swept-source optical coherence tomography (SS-OCT), the macular and peripapillary choroidal thickness and three-dimensional CVI were measured and compared globally and sectorally. Pearson's correlation analysis and multivariate regression models were used to evaluate choroidal thickness or CVI with related factors. RESULTS: The mean subfoveal CVIs were 0.35±0.10, 0.33±0.09, and 0.29±0.04, and the mean subfoveal choroidal thickness were 315.62±52.92, 306.22±59.29, and 262.69±45.55 μm in the F-APAC, F-CPACG, and normal groups, respectively. All macular sectors showed significantly higher CVIs and choroidal thickness in the F-APAC and F-CPACG eyes than in the normal eyes (P&#x003C;0.05), while there were no significant differences between the F-APAC and F-CPACG eyes. In the peripapillary region, the mean overall CVIs were 0.21±0.08, 0.20±0.08, and 0.19±0.05, and the mean overall choroidal thickness were 180.45±54.18, 174.82±50.67, and 176.18±37.94 μm in the F-APAC, F-CPACG, and normal groups, respectively. There were no significant differences between any of the two groups in all peripapillary sectors. Younger age, shorter axial length, and the F-APAC or F-CPACG diagnosis were significantly associated with higher subfoveal CVI and thicker subfoveal choroidal thickness (P&#x003C;0.05). CONCLUSION: The fellow eyes of unilateral APAC or asymmetric CPACG have higher macular CVI and choroidal thickness than those of the normal controls. Neither CVI nor choroidal thickness can distinguish between eyes predisposed to APAC or CPACG. A thicker choroid with a higher vascular volume may play a role in the pathogenesis of primary angle-closure glaucoma.

Abstract:Purpose: To comparatively analyze differences in macular choroidal thickness and volume in primary chronic angle-closure glaucoma (PACG) eyes. Methods: Thirty-one PACG patients were sequentially selected for this case-control study. Thirty-one eyes with PACG were included in group A, 31 fellow eyes were included in group B, and group C included 67 normal eyes. Enhanced-depth imaging optical coherence tomography (EDI-OCT) was used to measure choroidal thickness and volume. Results: The choroidal thicknesses and volumes of the central subfield macula (CSM), nasal inner macula (NIM), temporal inner macula (TIM), inferior inner macula (IIM), temporal outer macula (TOM), inferior outer macula (IOM), and mean macula (MM) in group A were all higher than those in group C (P < 0.05). The choroidal thicknesses and volumes of the NIM, superior inner macula (SIM), IIM, nasal outer macula (NOM), and MM in group B were all higher than those in group C (P < 0.05). No statistically significant differences were found between groups A and B (P > 0.05). The choroidal thicknesses of different macular regions in group A were not correlated with the mean defect (MD). Conclusion: Increased macular choroidal thickness may be a common anatomical characteristic of PACD eyes. Macular choroidal thickness is not a good marker for assessing PACG severity.
Glaucomatous Maculopathy: Thickness Differences on Inner and Outer Macular Layers between Ocular Hypertension and Early Primary Open-Angle Glaucoma Using 8 × 8 Posterior Pole Algorithm of SD-OCT

Abstract:The purpose of this study was to compare the thickness of all inner and outer macular layers between ocular hypertension (OHT) and early primary open-angle glaucoma (POAG) using spectral domain optical coherence tomography (SD-OCT) 8 × 8 posterior pole algorithm (8 × 8 PPA). Fifty-seven eyes of 57 OHT individuals and fifty-seven eyes of 57 early POAG patients were included. The thickness of macular retinal nerve fiber layer (mRNFL), ganglion cell layer (GCL), inner plexiform layer (IPL), inner nuclear layer (INL), outer plexiform and nuclear layer, photoreceptor layer (PRL) and retinal pigment epithelium were obtained in 64 cells for each macular layer and mean thickness of superior and inferior hemispheres was also calculated. Thinning of superior and inferior hemisphere mean thickness in mRNFL, GCL and IPL and thickening of superior and inferior hemisphere mean thickness in PRL and inferior hemisphere in INL were found in early GPAA group. Otherwise, heatmaps representing cell-to-cell comparisons showed thinning patterns in inner retinal layers (except for INL) and thickening patterns in outer retinal layers in GPAA group. We found that 8 × 8 PPA not only allows the detection of significant thinning patterns in inner retinal layers, but also thickening patterns in outer retinal layers when comparing early POAG eyes to OHT eyes.

Macular Choroidal Thickness in Children: The Shandong Children Eye Study

Abstract:PURPOSE To examine the thickness of the macular choroid and its associations in school children aged 6 to 18 years. METHODS The school-based cross-sectional Shandong Children Eye Study included 6026 (94.7%) of 6364 eligible children fulfilling the inclusion criterion of an age from 4 to 18 years. Spectral-domain optical coherence tomography (SD-OCT) was performed for a subgroup of 972 school children aged 6+ years. All participants underwent ocular examinations, including measurement of visual acuity, cycloplegic refractometry, biometry, and SD-OCT (enhanced depth imaging mode) for measurement of choroidal thickness. RESULTS The study included 972 children (501 girls) with a mean age of 11.3 ± 3.3 years (range, 6-18 years) and mean axial length of 24.10 ± 1.56 mm (range, 16.57-28.82 mm). Mean choroidal thickness was thicker (P < 0.001) at 500 μm temporal to the foveola (290 ± 67 μm) than in the subfoveal region (283 ± 67 μm; range, 113-507 μm) and the region 500 μm superior to the fovea (283 ± 66 μm), where it was thicker (P < 0.001) than at 500 μm inferior of the foveola (281 ± 66 μm), and it was thinnest (P < 0.001) at 500 μm nasal of the foveola (268 ± 67 μm). In multivariate analysis, thicker SFCT was (overall correlation coefficient r: 0.51) associated with shorter axial length (P < 0.001; standardized correlation coefficient β: -0.48; B: -23.7; 95% confidence interval [CI]: -27.2 to -20.3), male sex (P = 0.006; β: -0.08; B: -10.7; 95% CI: -18.3 to -3.11), and younger age (P = 0.04; β: -0.07; B: -1.46; 95% CI: -2.85 to -0.07). CONCLUSIONS As in adults, thicker SFCT in children and teenagers was markedly associated with shorter axial length, and to a lesser degree with male sex and older age. As in adults, increasing axial myopia in teenagers is associated with choroidal thinning and development of a leptochoroid.

Abstract:PURPOSE:To evaluate in vivo features of Schlemm's canal (SC) in patients with primary open-angle glaucoma (POAG) with spectral-domain optical coherence tomography (SD-OCT) and to investigate the relationship of SC size with intraocular pressure (IOP) and glaucoma severity. DESIGN:Prospective, comparative study. PARTICIPANTS:Fifty Chinese patients with newly diagnosed POAG who had not undergone surgery and 50 normal Chinese subjects from a population-based, cross-sectional study in Shanghai. METHODS:All participants underwent SD-OCT. The diameter and area of SC were examined in the temporal and nasal sections and measured with customized software. MAIN OUTCOME MEASURES:Patient demographics, repeatability and reproducibility assessed with the coefficient of variation (CV) and the intraclass correlation coefficient (ICC), SC parameters and their correlation with IOP, and the mean deviation (MD) of the visual field were analyzed. RESULTS:The percentage of sections in which SC was observable was similar between eyes with POAG and normal eyes, and ranged from 78% to 86%. For intraobserver repeatability, the CV and ICC values were 7.9% and 0.97 for diameter, and 13.8% and 0.83 for area, respectively. For interobserver repeatability, the CV and ICC values were 13.6% and 0.89 for diameter, and 13.4% and 0.80 for area, respectively. Significant differences between the 2 groups were found for the average SC area (11332 ± 2015 μm(2) vs. 13991 ± 1357 μm(2); P<0.001), but not for the SC diameter (40.2 ± 7.1 μm vs. 45.2 ± 4.0 μm; P = 0.195). In addition, the mean IOP values correlated well only with the SC area (ρ = -0.674, P<0.001), not with the SC diameter (ρ = -0.103, P = 248). No significant correlations were found between the MD values and the SC parameters. CONCLUSIONS:Eyes with POAG have a decreased SC area compared with normal eyes. A correlation between the SC area and the IOP also was observed. However, the degree of glaucoma damage was not consistently associated with the SC area. Spectral-domain OCT could be used for investigating SC changes in patients with glaucoma.

Abstract:European Journal of Ophthalmology, Ahead of Print. BackgroundThe aim of the study is to utilize swept-source OCT for imaging the rear sclera near the ONH, calculating the thickness of the SL and lamina, and examining how these measurements correspond to various metrics.DesignCross-sectional design with a prospective approach.Materials and methodsThe study included 96 glaucoma patients and 39 control subjects, all with axial lengths greater than 23 mm. Swept-source OCT was used to visualize the subfoveal and ONH regions. Thicknesses of SL and lamina were then calculated from the "B-scan" visuals collected. In order to confirm the consistency of the calculation, "intraclass correlation coefficients" were computed from specific "B-scans". The study compared the thickness of the SL and lamina in subjects with NTG to those with POAG. A "Pearson correlation" was conducted to analyze the connections between the thickness of the SL and lamina with various ocular parameters.ResultsSwept-source OCT enabled the measurement of posterior SL thickness in 65.9% and laminar thickness in 89.8% of the subjects. The consistency of measurements taken by different observers and by the same observer varied from moderate to excellent. There was a notable variation among the NTG and POAG sets in terms of subfoveal SL thickness, with measurements of 680.84 ± 170.60 mm and 486.55 ± 125.21 mm, respectively. The connection among subfoveal SL thickness and axial-length was discovered to be negative in subjects with NTG, but this was not observed in subjects with POAG. The variation in connection among the 2 subject groups was statistically important (r [ L0.687, P < 0.002).Conclusions"Swept-source OCT" identified variations in posterior SL thickness between NTG and POAG eyes. The subfoveal scleral thickness showed a negative association with axial-length, particularly in eyes with normal-tension glaucoma.

Abstract:Purpose: The aim of this study was to assess the correlation of hemoglobin A1c (HbA1c) levels with choroidal thickness in patients with type 2 diabetes mellitus (DM) using spectral domain optical coherence tomography (SD-OCT). Methods: In this prospective case series, 180 eyes from 90 patients with type 2 DM were classified into three study groups based on HbA1c values: group 1 included patients with good glycemic control (HbA1c ≤ 7%), group 2 included patients with moderate glycemic control (HbA1c between 7% and 8%), and group 3 included patients with poor glycemic control (HbA1c ≥ 8%). Additionally, 50 eyes from 25 non-diabetic subjects were enrolled to group 4 as a control group. Sub-foveal, nasal, and temporal choroidal thickness were measured and compared. Results: Mean central, nasal, and temporal choroidal thicknesses in diabetic patients (247.80, 238.63, and 239.30.

Abstract:Background To assess quantitatively the microvascular changes in the retina and choroid of healthy eyes, eyes of subjects with primary angle-closure glaucoma (PACG), primary open-angle glaucoma (POAG) and neuromyelitis optica spectrum disorder (NMOSD) using optical coherence tomography angiography (OCTA). Methods A series of healthy individuals and subjects with PACG, POAG and NMOSD were recruited in this cross-sectional study. OCTA was performed to capture the optic nerve head and macula images and the vessel density (VD) and retinal nerve fiber layer (RNFL) thickness were quantified. The choriocapillary flow density (CFD) was calculated as the percentage of flow area to the whole selected area. Results A total of 68 PACG subjects, 25 POAG subjects, 51 NMOSD subjects and 37 healthy controls were enrolled. Significant decreases in peripapillary VD and RNFL thickness were observed in PACG ( p <0.001) and POAG ( p <0.001) eyes and also in NMOSD subjects with an optic neuritis history ( p <0.001) compared with healthy controls. Unaffected eyes in PACG and POAG subjects had lower baseline peripapillary VD than healthy controls ( p =0.002 and p =0.011, respectively). PACG eyes showed a lower baseline CFD than POAG ( p =0.0027) and the CFD in early and advanced PACG eyes showed a significantly much more decrease than POAG ( p =0.002 and p <0.001,respectively). Conclusions The peripapillary vessel density and RNFL thickness was reduced in glaucomatous and NMOSD eyes compared with healthy controls. PACG eyes showed a lower CFD than POAG and the distinct peripapillary and choriocapillaris microvasculature changes may be correlated to different PACG and POAG pathogenesis.
