Abstract:BACKGROUND Dual energy computed tomography (DECT) is a new method of computed tomography (CT) imaging which allows to determine stone composition in addition to assessing stone morphology. The purpose of this study was to evaluate the role of dual energy CT (DECT) preoperatively to assess the composition of urinary stones and to compare it with post-operative in vitro qualitative chemical analysis as reference standard. METHODS Forty patients (18 male and 22 female) who presented with symptoms of renal stones in the department of urology were included in the study. All 40 patients who were diagnosed to have renal stones clinically and by ultrasonography (USG) kidney, ureter and bladder (KUB) region were subjected to dual energy CT. The stone composition assessed in vivo using DECT preoperatively and in vitro by chemical analysis post operatively after stone extraction by surgical procedure. The results were compared by statistical analysis. Sensitivity, specificity and positive predictive value (PPV) were calculated and descriptive study done using Statistical Package for Social Sciences (SPSS) 20.00 version. Data was analysed by comparing it with correlative qualitative chemical analysis. RESULTS In our study, in vivo analysis using DECT showed most common type of stone was calcium oxalate seen in 20 cases compromising 50 % of total cases. Next common stone type was uric acid stone (22.5 %) followed by cysteine (17.5 %) and calcium hydroxyapatite (10 %) respectively. When the same stones were subjected to ex vivo chemical analysis, one of the calcium oxalate stone came out to be calcium phosphate and one of the cysteine stone came out to be mixed stone. Thus, out of 40 stones, 38 stones were found to have the same result in ex vivo chemical analysis as that of in vivo analysis by dual energy CT. Hence, accuracy of dual energy CT in diagnosis of renal stones was found to be 100 % with CI 91.19 % - 100 %. CONCLUSIONS With dual energy CT, it is possible to determine the composition of renal calculi in vivo non-invasively (with specificity of 100 % in our present study). Therefore, this helps in deciding the modality of treatment pre-operatively whether the stone is amenable to medical management (e.g., Uric acid stones) or requires extracorporeal shock wave lithotripsy (ESWL) or surgical intervention can be determined preoperatively. This helps to reduce the unnecessary financial burden and is found to be time saving. Renal stone assessment with dual-energy multidetector CT and advanced postprocessing techniques: improved characterization of renal stone composition--pilot study

Abstract:Purpose: To prospectively evaluate the capability of noninvasive, simultaneous dual-energy (DE) multidetector computed tomography (CT) to improve characterization of human renal calculi in an anthropomorphic DE renal phantom by introducing advanced postprocessing techniques, with ex vivo renal stone spectroscopy as the reference standard. Materials and methods: Fifty renal calculi were assessed: Thirty stones were of pure crystalline composition (uric acid, cystine, struvite, calcium oxalate, calcium phosphate, brushite), and 20 were of polycrystalline composition. DE CT was performed with a 64-detector CT unit. A postprocessing algorithm (DECT(Slope)) was proposed as a pixel-by-pixel approach to generate Digital Imaging and Communications in Medicine dataset gray-scale-encoding ratios of relative differences in attenuation values of low- and high-energy DE CT. Graphic analysis, in which clusters of equal composition were identified, was performed by sorting attenuation values of color composition-encoded calculi in an ascending sequence. Multivariate general linear model analysis was used to determine level of significance to differentiate composition on native and postprocessed DE CT images. Results: Graphic analysis of native DE CT images was used to identify clusters for uric acid (453-629 HU for low-energy CT, 443-615 HU for high-energy CT), cystine (725-832 HU for low-energy CT, 513-747 HU for high-energy CT), and struvite (1337-1530 HU for low-energy CT, 1007-1100 HU for high-energy CT) stones; high-energy clusters showed attenuation value overlap. Polycrystalline calcium oxalate and calcium phosphate calculi were found throughout the entire spectrum, and dense brushite had attenuation values of more than 1500 HU for low-energy CT and more than 1100 HU for high-energy CT. The DE CT algorithm was used to generate specific identifiers for uric acid (77-80 U(Slope), one outlier), cystine (70-71 U(Slope)), struvite (56-60 U(Slope)), calcium oxalate and calcium phosphate (17-59 U(Slope)), and brushite (4-15 U(Slope)) stones. Statistical analysis showed that all compositions were identified unambiguously with the DECT(Slope) algorithm. Conclusion: DE multidetector CT with advanced postprocessing techniques improves characterization of renal stone composition beyond that achieved with single-energy multidetector CT acquisitions with basic attenuation assessment.
Evaluation of low-dose dual energy computed tomography for in vivo assessment of renal/ureteric calculus composition

Abstract:Purpose: This study aimed to assess the accuracy of low-dose dual-energy computed tomography (DECT) in predicting the composition of urinary calculi. Materials and methods: A total of 52 patients with urinary calculi were scanned with a 128-slice dual-source DECT scanner by use of a low-dose protocol. Dual-energy (DE) ratio, weighted average Hounsfield unit (HU) of calculi, radiation dose, and image noise levels were recorded. Two radiologists independently rated study quality. Stone composition was assessed after extraction by Fourier transform infrared spectroscopy (FTIRS). Analysis of variance was used to determine if the differences in HU values and DE ratios between the various calculus groups were significant. Threshold cutoff values to classify the calculi into separate groups were identified by receiver operating characteristic curve analysis. Results: A total of 137 calculi were detected. FTIRS analysis differentiated the calculi into five groups: uric acid (n=17), struvite (n=3), calcium oxalate monohydrate and dihydrate (COM-COD, n=84), calcium oxalate monohydrate (COM, n=28), and carbonate apatite (n=5). The HU value could differentiate only uric acid calculi from calcified calculi (p<0.001). The DE ratio could confidently differentiate uric acid, struvite, calcium oxalate, and carbonate apatite calculi (p<0.001) with cutoff values of 1.12, 1.34, and 1.66, respectively, giving >80% sensitivity and specificity to differentiate them. The DE ratio could not differentiate COM from COM-COD calculi. No study was rated poor in quality by either of the observers. The mean radiation dose was 1.8 mSv. Conclusions: Low-dose DECT accurately predicts urinary calculus composition in vivo while simultaneously reducing radiation exposure without compromising study quality.
Dual-energy computed tomography with advanced postimage acquisition data processing: improved determination of urinary stone composition

Abstract:Introduction: The characterization of urinary calculi using noninvasive methods has the potential to affect clinical management. CT remains the gold standard for diagnosis of urinary calculi, but has not reliably differentiated varying stone compositions. Dual-energy CT (DECT) has emerged as a technology to improve CT characterization of anatomic structures. This study aims to assess the ability of DECT to accurately discriminate between different types of urinary calculi in an in vitro model using novel postimage acquisition data processing techniques. Methods: Fifty urinary calculi were assessed, of which 44 had >or=60% composition of one component. DECT was performed utilizing 64-slice multidetector CT. The attenuation profiles of the lower-energy (DECT-Low) and higher-energy (DECT-High) datasets were used to investigate whether differences could be seen between different stone compositions. Results: Postimage acquisition processing allowed for identification of the main different chemical compositions of urinary calculi: brushite, calcium oxalate-calcium phosphate, struvite, cystine, and uric acid. Statistical analysis demonstrated that this processing identified all stone compositions without obvious graphical overlap. Conclusion: Dual-energy multidetector CT with postprocessing techniques allows for accurate discrimination among the main different subtypes of urinary calculi in an in vitro model. The ability to better detect stone composition may have implications in determining the optimum clinical treatment modality for urinary calculi from noninvasive, preprocedure radiological assessment.

Abstract:Background: Dual-energy computed tomography (DECT) scan has been proposed as an effective modality for determining the chemical composition of renal stone subtypes. Objectives: This study aimed to evaluate and compare the efficacy of DECT in the identification of renal stone type and composition versus biochemical analysis as the gold standard. Materials and Methods: The type of 146 renal stones, removed from patients by surgery, was determined in a medical laboratory. The stones were then inserted into the kidney of an anthropomorphic Rando phantom, and DECT scans were acquired. The stone type, specified by the scanner software, was compared with the results of biochemical analysis as the gold standard. Results: The DECT scans showed acceptable accuracy in identifying the chemical composition of renal stone subtypes. Based on the comparison of biochemical analysis and DECT results, the accuracy values for cystine, uric acid, and calcium-oxalate stones were 97%, 100%, and 97%, respectively. Conclusion: Based on the present results, DECT could detect pure uric acid, cystine, and calcium-oxalate stone types with high sensitivity. However, for mixed stones, the stone type, directly identified by DECT scan, was not reliable.

Non-Invasive characterisation of renal stones using dual energy CT: A method to differentiate calcium stones

Abstract:Background: Non-invasive DECT based characterization of renal stones using their effective atomic number (Zeff) and the electron density (ρe) in patients. Aim: This paper aims to develop a method for in-vivo characterization of renal stone. Differentiation of renal stones in-vivo especially sub types of calcium stones have very important advantage for better judgement of treatment modality. Materials and methods: 50 extracted renal stones were scanned ex-vivo using dual energy CT scanner. A method was developed to characterize these renal stones using effective atomic number and electron density obtained from dual energy CT data. The method and formulation developed in ex-vivo experiments was applied in in-vivo study of 50 randomly selected patients of renal stones who underwent dual energy CT scan. Results: The developed method was able to characterize Calcium Oxalate Monohydrate (COM) and the combination of COM and Calcium Oxalate Dihydrate (COD) stones non-invasively in patients with a sensitivity of 81% and 83%respectively. The method was also capable of differentiating Uric, Cystine and mixed stones with the sensitivity of 100, 100 and 85.71% respectively. Conclusion: The developed dual energy CT based method was capable of differentiating sub types of calcium stones which is not differentiable on single energy or dual energy CT images.

Abstract:Urinary tract stone disease is common in the Western world. Nonenhanced computed tomography (CT) is the method of choice for stone diagnosis; however, it has a limited role in predicting stone type. Knowledge of stone composition is currently determined by postoperative assessment of stone fragments. A noninvasive tool for accurate characterization of stone composition would improve patient management. Dual-energy CT aids in the differentiation among calcium, uric acid, and cystine stones. In this review we describe the role of dual-energy CT for in vitro and in vivo analysis of urinary stones. We illustrate the technique, review the literature, and discuss future directions.
Renal stones composition in vivo determination: comparison between 100/Sn140 kV dual-energy CT and 120 kV single-energy CT

Abstract:The objective of this study is to compare in vivo, the accuracy of single-energy CT (SECT) and dual-energy CT (DECT) in renal stone characterization. Retrospective study approved by the IRB. 30 patients with symptomatic urolithiasis who underwent CT on a second-generation dual-source scanner with a protocol that included low-dose 120 kV scan followed by 100/Sn140 kV dual-energy scan have been included. Stone composition was classified as uric acid, cystine or calcium oxalates, and phosphates according to attenuation values at 120 kV and to 100/Sn140 kV attenuation ratios and compared with the infrared spectroscopy analysis. 50 stones were detected in 30 patients. SECT correctly assessed stone composition in 52 % of the cases, DECT in 90 %. Sensitivity, specificity, positive predictive value, and negative predictive value in differentiating uric acid vs. non-uric acid stones were 0.94, 0.72, 0.64, and 0.96 for SECT and 1.00, 0.94, 1.00, and 0.96 for DECT, respectively. DECT significantly performs better than SECT in characterising renal stones in vivo, and may represent a useful tool for treatment planning.
Characterization of renal stone composition by using fast kilovoltage switching dual-energy computed tomography compared to laboratory stone analysis: a pilot study

Abstract:Purpose: To prospectively examine the diagnostic performance of fast kilovoltage switching dual-energy computed tomography (DECT) in characterization of in vivo renal stone composition compared with postoperative stone analysis. Methods: Consecutive consenting patients scheduled for endoscopic kidney stone surgery in a tertiary referral hospital from June 2015 to January 2016 were enrolled. Patients were preoperatively scanned with single-source, fast kilovoltage switching DECT. Stone compositions were determined regarding the effective atomic number measurements. Results of the stone compositions from DECT were compared to postoperative infrared spectroscopy stone analysis as the standard reference. Results: For the 39 patients enrolled in the study, DECT was able to detect uric acid stone with sensitivity of 88.9% and specificity of 100%. There was 100% positive predictive value, 96.8% negative predictive value, and 97.4% accuracy. For non-contrast CT scan, sensitivity was 88.9%, specificity was 96.7%, positive predictive value was 88.9%, negative predictive value was 96.7%, and accuracy was 94.8%. Of the 39 samples examined, 21 (54%) were single composition, whereas 18 (46%) were combined. Single composition stones were correctly characterized by DECT in 100% (8/8) for uric acid. Whereas the result of uric acid stone containing stone discrimination in mixed composition was not so good with Zeff alone, iodine imaging can compensated this fault. Conclusions: DECT provides excellent accuracy in characterizing uric acid stone compositions. With the addition of iodine image, all of uric acid-containing stones can be determined by the DECT.
Dual-energy computed tomography: A reliable and established tool for In vivo differentiation of uric acid from nonuric acid renal Stones

Abstract:Background: Dual-energy computed tomography (DECT) utilises the technique of simultaneous image acquisition of a body part at two different energy levels of the X-ray tubes. We used the DECT in this project to study its accuracy in non-invasive in vivo differentiation of uric acid (UA) calculi from non-UA calculi using the 3rd generation dual-source computed tomography (CT) scanner. Materials and methods: The DECT ratios of the various stones were noted and compared with the postextraction analysis of stones for composition. Sixty stones from 53 patients were included in the study, and their DECT results were compared with the post-extraction analysis (by infrared spectroscopy as the standard comparative method). Results: DECT was highly sensitive and specific in the diagnosis of various types of calculi based on their dual-energy ratio. It was found to be 100% sensitive and specific for differentiating UA stones from the non-UA stones. The DE ratio for the UA stones was found to be 0.98-1.13. Most of the stones belonged to non-UA category (85%), and only 15% were UA calculi. The sensitivity and specificity in differentiating a calcium oxalate from non-calcium oxalate calculus was 97.8% and 92.3%, respectively. Conclusion: DECT using the 3rd generation CT scanners is highly sensitive in the determination of urinary calculi composition which will be very helpful to the treating surgeon in selecting the optimal modality of treatment as UA stones are amenable to medical therapy while most of the non-UA stones require surgical intervention.

Abstract:Background: The urinary tract stone disease is a common problem. Dual energy CT with advanced post processing techniques gives anatomical information as size, site of the stone as well as its chemical composition which alter the clinical management. Objective: This study was aimed at studying the role of dual energy computed tomography in characterization of renal stones. Patients and methods: This study was carried out at the private radiology center throughout the period from January 2020 to January 2021 included 48 patients with renal stones. In this study, renal stones were more common in men (66.7 %) than in women (30.3%). The age of our selected patients ranged from 24 to 55 years with a mean of 42 years. The most affected age group was from 24-44 years (24 cases) represented 50% of the cases. Results: The average age of the studied group was (42.1±7.6), the most common presentation symptoms was loin pain (75.0%) followed by hematuria (18.8 %) then nausea& vomiting (4.2%) and fever (2.0%).Dual energy CT had 88.9% ability to truly detect uric from non-uric acid stones with (97.9%) accuracy, (100.0%) ability to exclude non uric stones (specificity), (100.0%) predictive value positive and (97.5%) predictive value negative with (50.0%) prevalence and (35.0%) likelihood positive ratio. Conclusion: It could be concluded that dual energy CT has been shown to be effective for characterizing chemic composition of the urinary stones, dual energy CT not well established enough to be used in clinical practice, but it will replace helical non contrast CT as the standard imaging modality for urinary calculi.
In vivo characterization of urinary calculi on dual-energy CT: going a step ahead with sub-differentiation of calcium stones

Abstract:Background: The role of dual-energy computed tomography (DECT) in characterization of urinary calculi is evolving and literature regarding differentiation of calcium calculi is sparse and confounding. Purpose: To evaluate the capability of DECT in assessing the urinary calculi composition in vivo, especially in differentiating various types of calcium calculi. Material and methods: One hundred and twenty patients underwent DECT for characterization of urinary calculi. Seventy patients with 114 calculi, including 93 calcium stones, were retrospectively analyzed. DE ratios and attenuation differences were compared using ANOVA and receiver-operating-characteristic (ROC) analysis was done to predict cut-off values, in particular for detecting calcium-oxalate-monohydrate (COM) stones. Results: DE ratio ≤1.14 accurately detected uric acid calculi, ≥1.29 was definitive for calcium and intermediate values were characteristic of cystine stones. DE ratios were significantly different between group 1 (COM [n = 32]; mean 1.376 ± 0.041), group 2 ([calcium oxalate dihydrate (COD) + COM] [n = 51]; 1.416 ± 0.048), and group 3 ([carbonate apatite (CaP) + COD + COM] [n = 10]; 1.468 ± 0.038) (group 1 vs. 2, P = 0.001; 1 vs. 3, P = 0.000; 2 vs. 3, P = 0.004). More importantly, pure COM calculi (group 1) had significantly lower DE ratio compared with mixed calcium calculi (groups 2 and 3) (P = 0.000). Attenuation differences (between low and high kV images) could not distinguish between COM and mixed calculi. ROC analysis for detection of COM calculi yielded AUC of 0.770 with cut-off DE ratio 1.385 (sensitivity 65.6%, specificity 82%) and value <1.335 was seen only with COM calculi (100% specificity). Conclusion: DECT can be employed for in vivo differentiation of various types of calculi and for detection of relatively lithotripsy-resistant COM calculi.
Identification of Urinary Stone Composition with Dual-energy Acquisition of Dual-source CT in Vivo

Abstract:Purpose:To investigate the clinical value of the determination of renal stone composition in vivo with dual-source dual-energy CT.Methods:One hundred and four cases with 113 renal stones were collected.All cases were scanned with dual-energy acquisition by dual-source CT.The data were sent to the workstation for the analysis of the stone composition.The urinary stones were analyzed by infrared spectroscopy after surgery.These two results were compared and the infrared spectroscopy result was taken as the reference standard.Results:The accuracy,sensitivity,specificity,positive and negative predictive value of the dual-energy CT for the diagnosis of uric acid stone were 97.3%,88.2%,99.0%,93.8%,and 97.9% respectively;those of oxalate were 92.9%,100%,66.7%,91.7%,100%,and those of apatite were 38.1%,9.6%,90.0%,63.6%,35.3%.All cystine stones were with false positive results.Conclusion:Dual source dual-energy CT has high accuracy for identifying uric acid and oxalate stone in vivo and can be used clinically.
Comparison of Four Dual-Energy CT Scanner Technologies for Determining Renal Stone Composition: A Phantom Approach.

Abstract:Background Studies have investigated the value of various dual-energy CT (DECT) technologies for determining renal stone composition. However, sparse multivendor comparison data exist. Purpose To compare the performance of four DECT technologies in determining renal stone composition at standard- and low-dose acquisitions. Materials and Methods This was an in vitro phantom study. Seventy-one urinary stones (size: 2.7-14.1 mm) of known chemical composition (51 calcium, four struvite, four cystine, and 12 urate) were placed in a custom-made cylindrical phantom. Consecutive scans with manufacturer-recommended protocols and dose-optimized institutional protocols (up to 80% reduction in volumetric CT dose index) were obtained with rapid kilovolt peak switching DECT (rsDECT) ( = 2), dual-source DECT ( = 2), twin-beam DECT (tbDECT) ( = 1), and dual-layer detector-based CT (dlDECT) ( = 1) scanners. The image data sets were analyzed using effective atomic number and dual-energy ratio indexes of maximally available and comparable spectra. The performance of each combination of scanner technology, method, and acquisition was assessed. Logistic regression models were used to calculate the area under the receiver operating characteristic curve (AUC). Results After image analysis, all scanners except tbDECT had an AUC greater than 0.95 in at least one acquisition in distinguishing urate from other stones. All DECT techniques were able to help differentiate calcium oxalate monohydrate stones with moderate accuracy (AUC: 0.70-0.83), and brushite was differentiated from urate with AUC greater than 0.99. There was no correlation between performance and acquisition with dose-optimized and/or vendor-recommended settings. Conclusion All four dual-energy CT (DECT) technologies enabled accurate determination of stone composition at standard- and low-dose acquisitions; however, performance varied based on the scanner parameters, DECT technique, and stone type. © RSNA, 2022 See also the editorial by Ringl and Apfaltrer in this issue.

Focused Dual-energy CT Maintains Diagnostic and Compositional Accuracy for Urolithiasis Using Ultralow-dose Noncontrast CT

Abstract:Objective: To evaluate the diagnostic accuracy of a new energy and radiation dose-reduced protocol for noncontrast computed tomography (NCCT) with dual-energy CT (DECT) analysis and its potential for the compositional analysis of uric acid (UA)- or non-uric acid (N-UA)-containing calculi. Methods: A retrospective evaluation was carried out in 61 patients who underwent dose-reduced DECT (tube A: 140 kV/55 mAs; tube B: 80 kV/303 mAs) with a tube current 38.8% lower than that set by the manufacturer. A protocol combining low-dose CT and targeted DE scans was used. Urinary stones were detected and classified as UA- or N-UA-containing or mixed based on DE software results. The accuracy of the compositional analysis was controlled by correlation with conventional infrared-based analysis. Results: The compositional stone differentiation was correct in 58 of 61 (95.1%) patients. The sensitivity of detecting pure UA-containing and pure N-UA-containing stones was 100%. The specificity of detecting UA- and pure N-UA-containing stones was 100% and 78.57%, respectively, as 3 of 7 mixed urinary stones (small fragments <4 mm) were classified as N-UA calculi. The total radiation dose in patients with body mass index <25 and >25 kg/m(2) was 1.2 and 2.5 mSv, respectively. Conclusion: Lowering the DECT tube current by up to 38% of the manufacturer's recommendations allows a reduced radiation dose without impairing detection accuracy and stone compositional analysis. Compared with previous studies, this protocol might significantly decrease patient radiation exposure without affecting the quality of results.
In Vitro Differentiation of Renal Stone Composition Using Dual-Source, Dual-Energy CT

Abstract:Objective To evaluate the ability of dual-source,dual-energy CT in differentiating uric acid stones from non-uric acid stones with infrared spectroscopy as reference standard.Materials and Methods Urinary calculus from 308 patients were scanned in first generation dual-source CT with dual-energy mode between July 2011 and June 2012.Renal Stone application was used to analyze their composition.The uric acid stones color were coded red and non-uric acid stones were blue.CT values were measured in 60 selective urinary calculus including 30 uric acid stones and 30 non-uric acid stones.The accuracy of dual energy CT to differentiate uric acid and no-uric acid stones was calculated.Results Of 308 patients,60 patients had uric acid stones and 248 non-uric acid stones.No difference was found for uric acid stone at 80 kV and 140 kV(375.8±69.2 HU vs.374.1±69.4 HU;t=-0.217,P=0.830),while CT values of non-uric acid stones were higher at 80 kV than those at 140 kV(1 455.1±312.4 HU vs.1 039.6±194.4 HU;t=-12.16,P0.001).CT values of non-uric acid stones at 80 kV,140 kV,and average weighted images(1 455.1±312.4 HU,1 039.6±194.4 HU,and 882.0±176.4 HU,respectively) were higher than those of uric acid stones(375.8±69.2 HU,374.1±69.4 HU,and 366.3±80.1 HU,respectively;P0.001).With infrared spectrum findings as reference standard,the accuracy of dual energy CT in differentiating uric acid stones from non-uric acid stones was 100%.Conclusions Dual-source,dual-energy CT can accurately differentiate uric acid stones from non-uric acid stones,and plays an important role in treatment planning of renal stones.
Determination of renal stone composition in phantom and patients using single-source dual-energy computed tomography

Abstract:Purpose: This study aimed to characterize the urinary tract stones in phantom and patients using single-source dual-energy computed tomography. Materials and methods: Twenty stones of pure crystalline composition (uric acid [UA], struvite, cystine, and calcium oxalate monohydrate) were assessed in a phantom and 11 patients (age 39-67 years) with urinary tract stones were evaluated. An initial low-dose unenhanced CT (tube potential, 120 kilovolts [peak]; milliampere range, 150-450; noise index, 26; section thickness, 5 mm) followed by a targeted dual-energy computed tomography acquisition on a single-source dual-energy computed tomography (Discovery CT 750 HDCT, GE) was performed. Uric acid and non-UA stones were defined using a 2-material decomposition (material density-iodine/water) algorithm. The stone effective atomic number (Zeff) was used to subclassify non-UA stones. The stone attenuation (Hounsfield unit) was also studied to determine its performance in predicting the composition. Ex vivo chemical analysis of the stone served as a criterion standard. Results: Of the 59 verified stones (phantom, 20; patients, 39; mean size, 6 mm), there were 16 UA and 43 non-UA type. The material density images were 100% sensitive and accurate in detecting UA and non-UA stones. The Zeff accurately stratified struvite, cystine, and calcium (calcium oxalate monohydrate) stones in the phantom. In patients, Zeff identified 83% of calcium stones (n = 24), and in stones of mixed type, it resembled dominant composition. The Hounsfield unit measurements alone were 71% sensitive and 69% accurate in detecting the UA stones. Conclusions: Single-source dual-energy computed tomography can accurately predict UA and non-UA stone composition in vitro and in vivo. Substratification of non-UA stones of pure composition can be made in vitro and in vivo. In stones of mixed composition, the Zeff values reflect the dominant composition.
Diagnostic performance and feasibility of dual-layer detector dual-energy CT for characterization of urinary stones in patients of different sizes

Abstract:Background: Urinary stones are frequently encountered in urology and are typically identified using non-contrast CT scans. Dual-energy CT (DECT) is a valuable imaging technique that produces material-specific images and allows for precise assessment of stone composition by estimating the effective atomic number (Zeff), a capability not achievable with the conventional single-energy CT's attenuation measurement method. Purpose: To investigate the diagnostic performance and image quality of dual-layer detector DECT (dlDECT) in characterizing urinary stones in patients of different sizes. Methods: All consecutive dlDECT examinations with stone protocol and presence of urinary stones between July 2018 and November 2019 were retrospectively evaluated. Two radiologists independently reviewed 120 kVp and color-overlay Zeff images to determine stone composition (reference standard = crystallography) and image quality. The objective analysis included image noise and Zeff values measurement. Results: A total of 739 urinary stones (median size 3.7 mm, range 1-35 mm) were identified on 177 CT examinations from 155 adults (mean age, 57 ± 15 years, 80 men, median weight 82.6 kg, range 42.6-186.9 kg). Using color-overlay Zeff images, the radiologists could subjectively interpret the composition in all stones ≥ 3 mm (n = 491). For stones with available reference standards (n = 74), dlDECT yielded a sensitivity of 80% (95%CI 44-98%) and a specificity of 98% (95%CI 92-100%) in visually discriminating uric acid from non-uric acid stones. Patients weighing > 90 kg and ≤ 90 kg had similar stone characterizability (p = 0.20), with 86% of stones characterized in the > 90 kg group and 87% in the ≤ 90 kg group. All examinations throughout various patients' weights revealed acceptable image quality. A Zeff cutoff of 7.66 accurately distinguished uric acid from non-uric acid stones (AUC = 1.00). Zeff analysis revealed AUCs of 0.78 and 0.91 for differentiating calcium-based stones from other non-uric stones and all stone types, respectively. Conclusion: dlDECT allowed accurate differentiation of uric acid and non-uric acid stones among patients with different body sizes with acceptable image quality. Clinical impact: The ability to accurately differentiate uric acid stones from non-uric acid stones using color-overlay Zeff images allows for better tailored treatment strategies, helping to choose appropriate interventions and prevent potential complications related to urinary stones in patient care.

Abstract:Introduction. The idea of using dual-energy computed tomography (DECT) originated in the early development of computed tomography (CT). However, only recently, advances in radiation diagnosis have made it possible to use dual-energy CT for routine clinical use. We describes the characteristic features of dual-energy CT scanners, as well as the results of a study of 245 patients with urolithiasis, the identification of urinary stones in vivo and the subsequent comparative characteristics with mineralogical studies of uroliths. Purpose. Evaluate the possibility of using DECT in the diagnosis of urolithiasis with the determination of the chemical composition of urinary stones in vivo. Materials and methods. A group of patients (n=245) aged 18 to 84 years was examined. All patients with the established diagnosis-urolithiasis-were treated with DECT (Somatom Definition, Siemens, Forchheim, Germany) with data processing, then in-vitro infrared spectrometry (IR-Alpha-P spectrometer) to determine the true composition of the calcu lus. Results. After conducting the DECT and then ROC analysis and comparing the results with the IR-spectrometry data, it was established that stones with an average density of less than 500 HU according to DECT can be attributed to urate, with a uric acid content of more than 50% with a sensitivity of 91,1% (34 stones of 35) and specificity of 100% — the content of uric acid is also determined in polymineral calculi with a content of less than 50%. The knowledge gained on the composition and structure of the stone in vivo can subsequently be used in pathogenetic treatment and prevention of complications in patients with urolithiasis, and influence the choice of the tactics of removing the uroliths. Conclusions. The obtained results give the right to apply DECT in the diagnosis of urolithiasis, and with high sensitivity to identify urate stones in vivo, thereby influencing the choice of the tactics of removal of uroliths and pathogenetic treatment, as well as the prevention of complications in patients with urolithiasis.

Abstract:The use of dual energy computed tomography (DECT) has become widespread in urology, including in the diagnosis of urolithiasis. DECT makes it possible to visualize and differentiate urinary stones of different chemical density and composition from each other. Studies have shown the advantages of DECT not only in the detection, but also in the differentiation of the main groups of urinary stones. At the same time, a number of in vivo studies evaluate DECT as a technique for the high-accuracy differential diagnosis of urate stones. The accuracy of diagnosing urate stones with DECT reaches 92-100%, as confirmed by sensitivity (84.6-98.4%) and specificity (100%).
