Abstract:Urinary tract stone disease is common in the Western world. Nonenhanced computed tomography (CT) is the method of choice for stone diagnosis; however, it has a limited role in predicting stone type. Knowledge of stone composition is currently determined by postoperative assessment of stone fragments. A noninvasive tool for accurate characterization of stone composition would improve patient management. Dual-energy CT aids in the differentiation among calcium, uric acid, and cystine stones. In this review we describe the role of dual-energy CT for in vitro and in vivo analysis of urinary stones. We illustrate the technique, review the literature, and discuss future directions.

Abstract:To investigate the optimal scanning parameters of dual-energy computed tomography (DECT), which can accurately determine sensitivity (the detectability of urinary stones) and accuracy (the composition matching of urinary stones), and to apply them to clinical trials. Fifteen urinary stones were chemically analyzed, and their chemical compositions were considered a reference standard with which we compared the uric acid (UA) and non-UA compositions determined using DECT. The urinary stones were placed inside a bolus and scanned with a dual-source CT scanner under various selected dual-energy conditions (A to X) using various solid water phantom thicknesses. These datasets were analyzed using the Siemens syngo .via software tool (integrated into the CT system) for matching the sensitivity and accuracy assessments. This study showed that 80% of the highest sensitivity (detection of urinary stones) and 92% of the highest accuracy (composition matching of urinary stones) were achieved under condition A (a collimation beam width setting of 2 × 32 × 0.6 mm, an automatic exposure control setting of 80/sn140 peak kilovoltage, and a slice thickness of 0.5/0.5 mm) ( P < 0.05). Application of the DECT energy parameters presented in the study will help identify the sensitivity and accuracy of UA and non-UA stone analysis, even in patients with small-sized urinary stones and in conditions difficult for analysis.

Teaching hypothesis-oriented thinking to medical students: the University of Florida's clinical investigation program.

Abstract:Recent studies show alarming decreases in the proportions of physicians applying for federal resources and of graduating medical students who declare strong interest in pursuing careers as physician-scientists. To expose medical students in their formative years to hypothesis-driven experimental investigations in a clinical setting, the first-year curriculum at the University of Florida has involved students as both investigators and study subjects in patient-oriented research conducted in the General Clinical Research Center (GCRC). Each year a hypothesis-driven experiment is conceived by first-year medical students in the university's MD-PhD program. Later in the year, the protocol is implemented in the GCRC by the entire freshman class, whose members serve as volunteer study subjects or as investigators. The experimental data are analyzed by the MD-PhD students, who report their findings at national biomedical research meetings and submit a manuscript on their project to a peer-reviewed journal. The authors describe students' research projects over the first six years of this GCRC-based program. They also describe the responses of former students to a questionnaire about their perceptions of the value of the research program. Most respondents considered the GCRC research exercise to have been useful and relevant to their overall education, and many more declared a current interest in pursuing research careers compared with the number who had declared such interest as freshmen. The authors conclude that early integration of hands-on, patient-oriented research into the medical school curriculum is a positive educational experience for students, and may contribute to their ultimate pursuit of academic research careers.

Abstract:BACKGROUND Dual energy computed tomography (DECT) is a new method of computed tomography (CT) imaging which allows to determine stone composition in addition to assessing stone morphology. The purpose of this study was to evaluate the role of dual energy CT (DECT) preoperatively to assess the composition of urinary stones and to compare it with post-operative in vitro qualitative chemical analysis as reference standard. METHODS Forty patients (18 male and 22 female) who presented with symptoms of renal stones in the department of urology were included in the study. All 40 patients who were diagnosed to have renal stones clinically and by ultrasonography (USG) kidney, ureter and bladder (KUB) region were subjected to dual energy CT. The stone composition assessed in vivo using DECT preoperatively and in vitro by chemical analysis post operatively after stone extraction by surgical procedure. The results were compared by statistical analysis. Sensitivity, specificity and positive predictive value (PPV) were calculated and descriptive study done using Statistical Package for Social Sciences (SPSS) 20.00 version. Data was analysed by comparing it with correlative qualitative chemical analysis. RESULTS In our study, in vivo analysis using DECT showed most common type of stone was calcium oxalate seen in 20 cases compromising 50 % of total cases. Next common stone type was uric acid stone (22.5 %) followed by cysteine (17.5 %) and calcium hydroxyapatite (10 %) respectively. When the same stones were subjected to ex vivo chemical analysis, one of the calcium oxalate stone came out to be calcium phosphate and one of the cysteine stone came out to be mixed stone. Thus, out of 40 stones, 38 stones were found to have the same result in ex vivo chemical analysis as that of in vivo analysis by dual energy CT. Hence, accuracy of dual energy CT in diagnosis of renal stones was found to be 100 % with CI 91.19 % - 100 %. CONCLUSIONS With dual energy CT, it is possible to determine the composition of renal calculi in vivo non-invasively (with specificity of 100 % in our present study). Therefore, this helps in deciding the modality of treatment pre-operatively whether the stone is amenable to medical management (e.g., Uric acid stones) or requires extracorporeal shock wave lithotripsy (ESWL) or surgical intervention can be determined preoperatively. This helps to reduce the unnecessary financial burden and is found to be time saving. Abstract:Background: The urinary tract stone disease is a common problem. Dual energy CT with advanced post processing techniques gives anatomical information as size, site of the stone as well as its chemical composition which alter the clinical management. Objective: This study was aimed at studying the role of dual energy computed tomography in characterization of renal stones. Patients and methods: This study was carried out at the private radiology center throughout the period from January 2020 to January 2021 included 48 patients with renal stones. In this study, renal stones were more common in men (66.7 %) than in women (30.3%). The age of our selected patients ranged from 24 to 55 years with a mean of 42 years. The most affected age group was from 24-44 years (24 cases) represented 50% of the cases. Results: The average age of the studied group was (42.1±7.6), the most common presentation symptoms was loin pain (75.0%) followed by hematuria (18.8 %) then nausea& vomiting (4.2%) and fever (2.0%).Dual energy CT had 88.9% ability to truly detect uric from non-uric acid stones with (97.9%) accuracy, (100.0%) ability to exclude non uric stones (specificity), (100.0%) predictive value positive and (97.5%) predictive value negative with (50.0%) prevalence and (35.0%) likelihood positive ratio. Conclusion: It could be concluded that dual energy CT has been shown to be effective for characterizing chemic composition of the urinary stones, dual energy CT not well established enough to be used in clinical practice, but it will replace helical non contrast CT as the standard imaging modality for urinary calculi.
Comparison of Four Dual-Energy CT Scanner Technologies for Determining Renal Stone Composition: A Phantom Approach.

Abstract:Background Studies have investigated the value of various dual-energy CT (DECT) technologies for determining renal stone composition. However, sparse multivendor comparison data exist. Purpose To compare the performance of four DECT technologies in determining renal stone composition at standard- and low-dose acquisitions. Materials and Methods This was an in vitro phantom study. Seventy-one urinary stones (size: 2.7-14.1 mm) of known chemical composition (51 calcium, four struvite, four cystine, and 12 urate) were placed in a custom-made cylindrical phantom. Consecutive scans with manufacturer-recommended protocols and dose-optimized institutional protocols (up to 80% reduction in volumetric CT dose index) were obtained with rapid kilovolt peak switching DECT (rsDECT) ( = 2), dual-source DECT ( = 2), twin-beam DECT (tbDECT) ( = 1), and dual-layer detector-based CT (dlDECT) ( = 1) scanners. The image data sets were analyzed using effective atomic number and dual-energy ratio indexes of maximally available and comparable spectra. The performance of each combination of scanner technology, method, and acquisition was assessed. Logistic regression models were used to calculate the area under the receiver operating characteristic curve (AUC). Results After image analysis, all scanners except tbDECT had an AUC greater than 0.95 in at least one acquisition in distinguishing urate from other stones. All DECT techniques were able to help differentiate calcium oxalate monohydrate stones with moderate accuracy (AUC: 0.70-0.83), and brushite was differentiated from urate with AUC greater than 0.99. There was no correlation between performance and acquisition with dose-optimized and/or vendor-recommended settings. Conclusion All four dual-energy CT (DECT) technologies enabled accurate determination of stone composition at standard- and low-dose acquisitions; however, performance varied based on the scanner parameters, DECT technique, and stone type. © RSNA, 2022 See also the editorial by Ringl and Apfaltrer in this issue.

Abstract:Purpose: This study aimed to assess the accuracy of low-dose dual-energy computed tomography (DECT) in predicting the composition of urinary calculi. Materials and methods: A total of 52 patients with urinary calculi were scanned with a 128-slice dual-source DECT scanner by use of a low-dose protocol. Dual-energy (DE) ratio, weighted average Hounsfield unit (HU) of calculi, radiation dose, and image noise levels were recorded. Two radiologists independently rated study quality. Stone composition was assessed after extraction by Fourier transform infrared spectroscopy (FTIRS). Analysis of variance was used to determine if the differences in HU values and DE ratios between the various calculus groups were significant. Threshold cutoff values to classify the calculi into separate groups were identified by receiver operating characteristic curve analysis. Results: A total of 137 calculi were detected. FTIRS analysis differentiated the calculi into five groups: uric acid (n=17), struvite (n=3), calcium oxalate monohydrate and dihydrate (COM-COD, n=84), calcium oxalate monohydrate (COM, n=28), and carbonate apatite (n=5). The HU value could differentiate only uric acid calculi from calcified calculi (p<0.001). The DE ratio could confidently differentiate uric acid, struvite, calcium oxalate, and carbonate apatite calculi (p<0.001) with cutoff values of 1.12, 1.34, and 1.66, respectively, giving >80% sensitivity and specificity to differentiate them. The DE ratio could not differentiate COM from COM-COD calculi. No study was rated poor in quality by either of the observers. The mean radiation dose was 1.8 mSv. Conclusions: Low-dose DECT accurately predicts urinary calculus composition in vivo while simultaneously reducing radiation exposure without compromising study quality.
Dual-energy computed tomography: A reliable and established tool for In vivo differentiation of uric acid from nonuric acid renal Stones

Abstract:Background: Dual-energy computed tomography (DECT) utilises the technique of simultaneous image acquisition of a body part at two different energy levels of the X-ray tubes. We used the DECT in this project to study its accuracy in non-invasive in vivo differentiation of uric acid (UA) calculi from non-UA calculi using the 3rd generation dual-source computed tomography (CT) scanner. Materials and methods: The DECT ratios of the various stones were noted and compared with the postextraction analysis of stones for composition. Sixty stones from 53 patients were included in the study, and their DECT results were compared with the post-extraction analysis (by infrared spectroscopy as the standard comparative method). Results: DECT was highly sensitive and specific in the diagnosis of various types of calculi based on their dual-energy ratio. It was found to be 100% sensitive and specific for differentiating UA stones from the non-UA stones. The DE ratio for the UA stones was found to be 0.98-1.13. Most of the stones belonged to non-UA category (85%), and only 15% were UA calculi. The sensitivity and specificity in differentiating a calcium oxalate from non-calcium oxalate calculus was 97.8% and 92.3%, respectively. Conclusion: DECT using the 3rd generation CT scanners is highly sensitive in the determination of urinary calculi composition which will be very helpful to the treating surgeon in selecting the optimal modality of treatment as UA stones are amenable to medical therapy while most of the non-UA stones require surgical intervention.
Diagnostic performance and feasibility of dual-layer detector dual-energy CT for characterization of urinary stones in patients of different sizes

Abstract:Background: Urinary stones are frequently encountered in urology and are typically identified using non-contrast CT scans. Dual-energy CT (DECT) is a valuable imaging technique that produces material-specific images and allows for precise assessment of stone composition by estimating the effective atomic number (Zeff), a capability not achievable with the conventional single-energy CT's attenuation measurement method. Purpose: To investigate the diagnostic performance and image quality of dual-layer detector DECT (dlDECT) in characterizing urinary stones in patients of different sizes. Methods: All consecutive dlDECT examinations with stone protocol and presence of urinary stones between July 2018 and November 2019 were retrospectively evaluated. Two radiologists independently reviewed 120 kVp and color-overlay Zeff images to determine stone composition (reference standard = crystallography) and image quality. The objective analysis included image noise and Zeff values measurement. Results: A total of 739 urinary stones (median size 3.7 mm, range 1-35 mm) were identified on 177 CT examinations from 155 adults (mean age, 57 ± 15 years, 80 men, median weight 82.6 kg, range 42.6-186.9 kg). Using color-overlay Zeff images, the radiologists could subjectively interpret the composition in all stones ≥ 3 mm (n = 491). For stones with available reference standards (n = 74), dlDECT yielded a sensitivity of 80% (95%CI 44-98%) and a specificity of 98% (95%CI 92-100%) in visually discriminating uric acid from non-uric acid stones. Patients weighing > 90 kg and ≤ 90 kg had similar stone characterizability (p = 0.20), with 86% of stones characterized in the > 90 kg group and 87% in the ≤ 90 kg group. All examinations throughout various patients' weights revealed acceptable image quality. A Zeff cutoff of 7.66 accurately distinguished uric acid from non-uric acid stones (AUC = 1.00). Zeff analysis revealed AUCs of 0.78 and 0.91 for differentiating calcium-based stones from other non-uric stones and all stone types, respectively. Conclusion: dlDECT allowed accurate differentiation of uric acid and non-uric acid stones among patients with different body sizes with acceptable image quality. Clinical impact: The ability to accurately differentiate uric acid stones from non-uric acid stones using color-overlay Zeff images allows for better tailored treatment strategies, helping to choose appropriate interventions and prevent potential complications related to urinary stones in patient care.
Characterization of urinary stones with dual-energy CT: improved differentiation using a tin filter

Abstract:Purpose: To assess image quality and capability of stone differentiation between UA-containing and non-UA-containing uroliths with the latest dual-energy (DE) computed tomography (CT) system equipped with a tin filter (TF) using various data acquisition parameters in a work bench model. Methods and materials: One hundred ten urinary stones (4.2 +/- 3.0 mm, 0.4-12 mm) of 15 compositions were examined in an ex vivo phantom, using DE dual-source CT (Definition Flash, Siemens Healthcare) equipped with a TF. Phantom was scanned in a water tank and contained stones in acrylic elliptic spheres filled with a parenchyma substitute. Scans were performed at 3 different settings: at 80 and 140 kVp without TF, at 80 and 140 kVp with TF, and at 100 and 140 kVp with TF. Tube current time products were adapted to yield constancy in CT dose indices (CTDIvol = 18.84 mGy, 18.95 mGy, and 18.90 mGy, respectively). CT numbers of urinary stones and image noise were electronically measured by placing regions of interest. DE indices (DEI) were calculated and compared using analysis of variances for repeated measures and paired t tests; image noise (IN) using the Friedman test. The stones were classified as UA-containing or non-UA-containing on color-coded images based on the DEI. Diagnostic accuracy was calculated using crystallographic analysis as standard of reference. Results: Of the 110 stones (60%), 65 contained UA; 45 stones (40%) contained no UA. DEI was greatest at 80 and 140 kVp when using the TF (DEI80 kVp/TF140 kVp = 0.038 vs. DEI80 kVp/140 kVp = 0.028, DEI100 kVp/TF140 kVp = 0.025; P < 0.01). IN of high kVp acquisitions were similar (P = 0.15), whereas IN of low kVp acquisitions were significantly (P < 0.001) different being lowest at 100 kVp. The semiautomated DE software correctly classified all stones at all settings with a diagnostic accuracy of 100% (95% confidence interval: 97%-100%). Conclusion: DECT with TF and 80-140 kVp tube voltage settings significantly improves the discrimination between UA-containing and non-UA containing urinary stones as compared with DECT without using the TF on the basis of DEI. The 100/140 kVp setting with TF is associated with lower IN but demonstrates similar discrimination abilities as compared with 80/140 kVp setting without the use of the TF.

Abstract:The use of dual energy computed tomography (DECT) has become widespread in urology, including in the diagnosis of urolithiasis. DECT makes it possible to visualize and differentiate urinary stones of different chemical density and composition from each other. Studies have shown the advantages of DECT not only in the detection, but also in the differentiation of the main groups of urinary stones. At the same time, a number of in vivo studies evaluate DECT as a technique for the high-accuracy differential diagnosis of urate stones. The accuracy of diagnosing urate stones with DECT reaches 92-100%, as confirmed by sensitivity (84.6-98.4%) and specificity (100%).

Abstract:Objective: To evaluate the diagnostic accuracy of a new energy and radiation dose-reduced protocol for noncontrast computed tomography (NCCT) with dual-energy CT (DECT) analysis and its potential for the compositional analysis of uric acid (UA)- or non-uric acid (N-UA)-containing calculi. Methods: A retrospective evaluation was carried out in 61 patients who underwent dose-reduced DECT (tube A: 140 kV/55 mAs; tube B: 80 kV/303 mAs) with a tube current 38.8% lower than that set by the manufacturer. A protocol combining low-dose CT and targeted DE scans was used. Urinary stones were detected and classified as UA- or N-UA-containing or mixed based on DE software results. The accuracy of the compositional analysis was controlled by correlation with conventional infrared-based analysis. Results: The compositional stone differentiation was correct in 58 of 61 (95.1%) patients. The sensitivity of detecting pure UA-containing and pure N-UA-containing stones was 100%. The specificity of detecting UA- and pure N-UA-containing stones was 100% and 78.57%, respectively, as 3 of 7 mixed urinary stones (small fragments <4 mm) were classified as N-UA calculi. The total radiation dose in patients with body mass index <25 and >25 kg/m(2) was 1.2 and 2.5 mSv, respectively. Conclusion: Lowering the DECT tube current by up to 38% of the manufacturer's recommendations allows a reduced radiation dose without impairing detection accuracy and stone compositional analysis. Compared with previous studies, this protocol might significantly decrease patient radiation exposure without affecting the quality of results.

Abstract:Background: Dual-energy computed tomography (DECT) scan has been proposed as an effective modality for determining the chemical composition of renal stone subtypes. Objectives: This study aimed to evaluate and compare the efficacy of DECT in the identification of renal stone type and composition versus biochemical analysis as the gold standard. Materials and Methods: The type of 146 renal stones, removed from patients by surgery, was determined in a medical laboratory. The stones were then inserted into the kidney of an anthropomorphic Rando phantom, and DECT scans were acquired. The stone type, specified by the scanner software, was compared with the results of biochemical analysis as the gold standard. Results: The DECT scans showed acceptable accuracy in identifying the chemical composition of renal stone subtypes. Based on the comparison of biochemical analysis and DECT results, the accuracy values for cystine, uric acid, and calcium-oxalate stones were 97%, 100%, and 97%, respectively. Conclusion: Based on the present results, DECT could detect pure uric acid, cystine, and calcium-oxalate stone types with high sensitivity. In Vitro Differentiation of Renal Stone Composition Using Dual-Source, Dual-Energy CT

Abstract:Objective To evaluate the ability of dual-source,dual-energy CT in differentiating uric acid stones from non-uric acid stones with infrared spectroscopy as reference standard.Materials and Methods Urinary calculus from 308 patients were scanned in first generation dual-source CT with dual-energy mode between July 2011 and June 2012.Renal Stone application was used to analyze their composition.The uric acid stones color were coded red and non-uric acid stones were blue.CT values were measured in 60 selective urinary calculus including 30 uric acid stones and 30 non-uric acid stones.The accuracy of dual energy CT to differentiate uric acid and no-uric acid stones was calculated.Results Of 308 patients,60 patients had uric acid stones and 248 non-uric acid stones.No difference was found for uric acid stone at 80 kV and 140 kV(375.8±69.2 HU vs.374.1±69.4 HU;t=-0.217,P=0.830),while CT values of non-uric acid stones were higher at 80 kV than those at 140 kV(1 455.1±312.4 HU vs.1 039.6±194.4 HU;t=-12.16,P0.001).CT values of non-uric acid stones at 80 kV,140 kV,and average weighted images(1 455.1±312.4 HU,1 039.6±194.4 HU,and 882.0±176.4 HU,respectively) were higher than those of uric acid stones(375.8±69.2 HU,374.1±69.4 HU,and 366.3±80.1 HU,respectively;P0.001).With infrared spectrum findings as reference standard,the accuracy of dual energy CT in differentiating uric acid stones from non-uric acid stones was 100%.Conclusions Dual-source,dual-energy CT can accurately differentiate uric acid stones from non-uric acid stones,and plays an important role in treatment planning of renal stones.

Abstract:BACKGROUND The numerous adverse effects of ethanol abuse and ethanol withdrawal on biological systems are well documented. Conversely, the understanding of the molecular mechanisms underlying these pathological effects is still incomplete. This study was undertaken to investigate the effects of short-term chronic ethanol administration and ethanol withdrawal on the molecular structure and function of hippocampal tissue, a brain region important for mnemonic processes and known to be highly susceptible to ethanol intoxication. METHODS Ethanol was administered to adult Wistar rats by intragastric intubation for 15 days with a stepwise increase in the daily dose from 6 to 12 g/kg body weight, with the highest dose delivered for the last 2 days only. The total daily dose of ethanol was divided into 3 equal portions administered 4 hours apart. Animals were sacrificed by decapitation at 4, 24, and 72 hours after the last ethanol administration to examine potential effects of ethanol intoxication and ethanol withdrawal. Ethanol-related molecular changes were monitored by Fourier transform infrared (FT-IR) spectroscopy. RESULTS Significant changes in the hippocampal content, structure, and function of lipids, proteins, and nucleic acids were recorded under ethanol intoxication. Seventy-two hours after the cessation of ethanol administration, during the late phase of withdrawal, alterations in the macromolecules' content and conformational changes in protein and nucleic acid structure ameliorated, while the changes in macromolecular ratios, lipid order, and dynamics aggravated. CONCLUSIONS Our results suggest that 15 days of binge-like drinking resulting in the high blood alcohol concentration (varying in the dose-dependent manner between 253 and 606 mg/dl) produced a strong physical dependence manifested mainly by the changes in lipid profiles pointing toward withdrawal-induced oxidative stress. These results show that ethanol withdrawal may cause equal to or even more severe brain damage than the ethanol itself, which should be considered when designing antialcohol therapies.

Abstract:The resaturation curve, a noninvasive indicator-dilution test using an ear oximeter to detect rates of change in arterial oxygen saturation during breathing of various concentrations of oxygen, was used to assess cardiac performance in normal subjects and in 108 patients with cardiac valvular disease. Measurements made during exercise included the time constant of resaturation (tau) and beat-to-beat changes in arterial oxygen saturation (the left heart clearance fraction). At maximum rates of voluntary work, patients had a significantly reduced clearance fraction and longer tau than normal subjects. Clearance fraction and tau improved in patients after aortic valve replacement; deterioration occurred in tau and clearance fraction over time in patients treated medically, as compared to normal subjects who showed little change during a ten-year period. Clearance fraction and tau correlated with hemodynamic data obtained during cardiac catheterization. The resaturation curve provides an objective measure of cardiac impairment that can be readily repeated during follow-up of patients with heart disease.
