Abstract:Objective: This study aimed to explore the most effective nonsurgical therapy to treat stress urinary incontinence (SUI). Methods: Data were pooled and the Bayesian random effects model was utilized to evaluate the inconsistencies between indirect and direct proof. Network sub-analyses, sensitivity and subgroup analysis, and standard pairwise comparisons were utilized in preparation of the main analysis. Results: A total of 17 RCTs (852 participants) were enrolled. With regard to therapy, pelvic floor muscle training (PFMT) was more often recommended than biofeedback (BF). The combination of PFMT and BF was remarkably promoted than PFMT alone or PFMT with vaginal cone (VC) or electrical stimulation (ES). The QL scores of patients who underwent PFMT were noticeably improved compared with those of patients who underwent ES. The combination of BF and PFMT was remarkably promoted compared with BF alone. VC was superior to ES with regard to QL scores. By contrast, ES alone was the least preferred method to decrease urine leakage and to promote QL. Conclusion: The combination of BF and PFMT was proven to be an optimal method to treat SUI which aims to promote urine leakage and LQ compared with either PFMT alone, ES, VC, or circular muscle (CME), while ES was the least preferred method to promote LQ.

Abstract:INTRODUCTION: Overactive bladder (OAB) symptoms (urge, frequency, nocturia) are not generally considered surgically curable by learning institutions. The Integral Theory hypothesizes that OAB is a prematurely activated, but normal micturition reflex caused by loose suspensory ligaments and potentially curable surgically by repairing such ligaments. To test this hypothesis by surgical repair of loose cardinal and uterosacral ligaments in patients with 2nd degree or greater uterine/apical prolapse.MATERIAL AND METHODS: Multicenter prospective case control audit. 611 females, mean age 70. Inclusions: symptomatic apical prolapse of 2nd or greater degree, (POPQ stages 2-4), and at least two pelvic symptoms. Exclusions: Comorbid medical problems known to cause chronic pelvic pain (e.g., infection), sphincter tears, neurological bladder conditions. Surgery: minimally invasive cardinal/uterosacral ligament repair using the TFS (Tissue Fixation System). Primary outcome: Uterine prolapse cure; Secondary outcomes; bladder, bowel, and pain symptoms improvement.RESULTS: 90% prolapse cure in 611 patients. Symptom incidence (% Cure at 12 months in brackets) was: urge incontinence: n = 310 (85%); frequency: n = 317 (83%); nocturia: n = 254 (68%); chronic pelvic pain (CPP): n = 194 (77%); fecal incontinence: n = 93 (65%). Statistics: McNemar x2-tests to test for significant changes in the symptoms' incidence-frequency from baseline (preoperative) to the postoperative phase. For each symptom the null hypothesis H0: P(baseline) = P(12 months after surgery)versus H1: P(baseline) ≠ P(12 months after surgery) was tested, with P indicating prevalence or incidence rate.CONCLUSIONS: Bladder & bowel incontinence and chronic pelvic pain occur in predictable groupings and are associated with apical prolapse. OAB symptom improvement with the TFS ligament repair provides a good alternative to anticholinergics, especially when considering their association with dementia causation. Application of the Integral Theory System has the potential to significantly improve clinical practice, QoL for ageing women, delaying entry into Nursing Homes and creating new scientific research directions. The take home message is that symptoms of chronic pelvic pain, bladder and bowel dysfunction occur in relatively predictable groups, caused by lax suspensory ligaments and can be cured or improved by TFS mini sling ligament repair.
Symptoms of defective emptying and raised residual urine may arise from ligamentous laxity in the posterior vaginal fornix

Abstract:Objectives: To prospectively test the hypothesis that laxity in the posterior ligaments of the vagina may cause raised residual urine and abnormal emptying symptoms. Methods: Eighteen patients, 16 multiparous, and 2 nulliparous, were referred with symptoms of abnormal bladder emptying. All were assessed with standard cystometry and standing resting and straining lateral X-ray films, with the bladder containing a Foley balloon catheter containing 10 ml of radio-opaque dye. Three patients were also assessed with videocystourethrograms, and these were compared to 4 normal controls. A posterior fornix repair was performed. A full thickness horizontal vaginal incision was made using a scalpel. The incision was stretched antero-posteriorly, and the utero-sacral and cardinal ligaments tightened by suturing them side to side. Results: Specific symptoms of defective opening were reduced from a total of 43 pre-operatively to 9 post-operatively. Mean residual urine was reduced from 98.7 to 31.7 ml (p < OR = 0.028). Peak flow increased from 29.7 to 34.2 ml/s, p < OR = 0.08. Mean flow pre-operative was 8.18 ml/s, and post-operative flow 8.6 ml/s (p < OR = 0.88). Mean emptying time decreased from 65.3 to 60.4 s. Conclusions: Reference to radiographs in normal patients demonstrates that bladder funnelling is associated with a powerful downward force transmitted to the coccyx via the utero-sacral ligaments. Correction of laxity in the utero-sacral ligaments, the effective insertion points of the downward force, improved bladder emptying. This appears to sustain the hypothesis that abnormal bladder emptying in the female may be at least partly caused by ineffectiveness of the opening muscles, because of laxity in their insertion points.

Abstract:Underactive bladder (UAB) is a major component of lower urinary tract symptoms. The physiopathology mechanisms underlying UAB include detrusor underactivity, acontractile detrusor, and reduced or absent bladder sensation. UAB can influence lower and upper urinary tract function and lead to serious consequences; however, the symptom syndrome is poorly understood and incompletely researched at present. Moreover, there is no agreement among authorities regarding the terminology, definition, epidemiology, classification, risk factors, animal model, pathogenesis, diagnosis, and treatment of UAB. In this review, we discuss the current terminology, epidemiology, physiopathology, evaluation, and management of UAB.

Abstract:Aims To provide a brief anatomical pathogenesis of the Working Group SUI publication recommendations. Methods and Results The anatomical science and surgical practice presented here formed the original basis for the MUS and other bladder dysfunctions, updated to 2024 with videos: https://atm.amegroups.org/issue/view/1400. Bladder control is binary, from outside the bladder, with ONLY two modes, EITHER closed (continence) OR open (micturition). The key concept for surgical repair of stress (SUI) and other types of urinary dysfunctions, is the role of three balanced oppositely‐acting muscle forces which contract against PUL (pubourethral ligament) and USL (uterosacral ligaments) to close urethra, open it before evacuation and stretch vagina to prevent premature activation of micturition, perceived as "urge to go." Collagen‐induced weakness in PUL or USL may cause dysfunction in all three of these activities, which can be improved or cured by collagen‐creating ligament repair techniques (e.g., slings). It is important to diagnose Tethered Vagina Syndrome (TVS), iatrogenic scarring at bladder neck, much more frequent since large vaginal mesh implantation. TVS is often confused with SUI, as its cardinal symptom is massive urine loss with the "effort" of getting out of bed. Sling surgery worsens TVS as it adds more scar. Vaginal skin graft is required to restore elasticity and continence. Conclusions "Anatomical defects in binary control may cause SUI, retention or urge," and are potentially repairable. With regard to SUI, "a normal PUL does not lengthen during effort."

Abstract:Underactive bladder (UAB) is a prevalent but under-researched lower urinary tract symptom that typically occurs alongside detrusor underactivity (DU). Unlike UAB, DU is a urodynamic diagnosis which the International Continence Society (ICS) defines as “a contraction of reduced strength and/or duration, resulting in prolonged bladder emptying and/or a failure to achieve complete bladder emptying within a normal time span”. Despite the widespread prevalence of UAB/DU, there are significant gaps in our understanding of its pathophysiological mechanisms, diagnosis, and treatment compared with overactive bladder (OAB) and detrusor overactivity (DO). These gaps are such that clinicians regard UAB/DU as an incurable condition. In recent years, the understanding of UAB has increased. The definition of UAB has been clarified, and the diagnostic criteria for DU have been considered more comprehensively. Meanwhile, a number of non-invasive diagnostic methods have also been reported. Clinical trials involving novel drugs, electrical stimulation, and stem cell therapy have shown promising results. Therefore, this review summarizes recent reports on UAB and DU and highlights the latest advances in their diagnosis and treatment.

Abstract:The uterosacral ligaments (USLs) are important anatomical structures that support the uterus and apical vagina within the pelvis. As these structures are over-stretched, become weak, and exhibit laxity, pelvic floor disorders such as pelvic organ prolapse occur. Although several surgical procedures to treat pelvic floor disorders are directed toward the USLs, there is still a lot that is unknown about their function. This manuscript presents a review of the current knowledge on the mechanical properties of the USLs. The anatomy, microstructure, and clinical significance of the USLs are first reviewed. Then, the results of published experimental studies on the in vivo and ex vivo, uniaxial and biaxial tensile tests are compiled. Based on the existing findings, research gaps are identified and future research directions are discussed. The purpose of this exhaustive review is to help new researchers navigate scientific literature on the mechanical properties of the USLs. The use of these structures remains very popular in reconstructive surgeries that restore and augment the support of pelvic organs, especially as synthetic surgical mesh implants continue to be highly controversial.

[Colposuspension at the lateral abdominal wall using fascia strips--an adaptable elevation of the bladder neck in urinary incontinence]

Abstract:Modifications of classical vesico-urethral suspension, according to Marshall-Marchetti-Krantz, are in widespread use among numerous operations for urinary incontinence, on account of their mild direct effect on both the urethra and bladder neck region. However, dysuria and dyspareunia may be quite frequent consequences later on because of retrosymphysial adhesions and rigid fixation of the vagina. We have, therefore, adopted an approach by which the advantages of mobile sling suspension are combined with indirect elevation of the urethrovesical junction, in that the vagina is fixed to strips obtained from the aponeurosis of the abdominal wall or from lyophilised dura mater. Fascial origin and dura anchorage in the abdominal wall are conducive to achieving a pronounced ventrocranial pull, and involvement of the rectus musculature in bridle action was found to add to the elevation effect in stress situations. A detailed account is given of the surgical technique. The method is contra-indicated or at least not fully sufficient only in cases of severe pelvic floor insufficiency or descensus of the vagina. Twenty-eight women have undergone this operation, within one year, with 20 of them (71 per cent) being relieved from complaints and five (18 per cent) substantively improved. Stress quotient and transmission factor are favourably affected, in terms of urodynamics, while ventral urethrovesical fixation is improved, in terms of radiography. Postoperative uroflow parameters have stayed normal. Problems of co-habitation did not occur. The small number of cases and lack of long-time results, therefore, cannot prevent the authors from recommending the technique for verification by re-use.
Overactive bladder (OAB): a failed concept needing revision to accommodate an external anatomical control system

Abstract:Thesis and aims: In 45 years, the definitions and practice of the urodynamically based overactive bladder (OAB)/detrusor overactivity (DO) system have failed to adequately address pathogenesis and cure of urinary urge incontinence, frequency and nocturia. Methods: We analysed the OAB syndrome with reference to the Integral Theory paradigm's (ITS) binary feedback system, where OAB in the female is viewed as a prematurely activated, but otherwise normal micturition caused mainly, but not entirely, by ligament damage/laxity. The ITS Clinical Assessment Pathway which details the relationships between structural damage (prolapse), ligaments and dysfunction (symptoms) is introduced. Results: The ITS was able to "better explain" OAB pathophysiology in anatomical terms with reference to the binary model. The phasic patterns diagnostic of "detrusor overactivity" are explained as a struggle for control by the closure and micturition reflexes. The exponentially determined relationship between urethral diameter and flow explains why obstructive patterns occur, why they do not and why urine may leak with no recorded pressure. Mechanically supporting ligaments ("simulated operations") during urodynamic testing can improve low urethral pressure, negative pressure during coughing with SUI and diminish urge sensation or even DO patterns, transforming urodynamics from non-predictive test to accurate predictor of continence surgery results. High cure rates for OAB by daycare repair of damaged ligaments is a definitive test of the binary system's validity. Conclusion: Conceptual progression of OAB to the Integral Theory paradigms's prematurely activated micturition validates OAB component symptoms as a syndrome, explains pathogenesis, and unlocks a new way of understanding, diagnosing, treating and researching OAB.
Late-occurring pain/other dysfunctions in midurethral sling class actions are likely caused by uterosacral ligament weakness, not implant or surgeon

Abstract:Background: Large sums of money have been awarded against manufacturers of midurethral slings (MUS) because of complaints of pain and other complications, even though pelvic pain is rarely seen at the 6-12 weeks review. Hypothesis and aims: Pain/other posterior fornix symptoms (urge, frequency, nocturia, and abnormal emptying) may appear weeks or months after MUS surgery due to dislocation of already weakened uterosacral ligaments (USL), a consequence of diversion of pelvic forces from pubourethral ligaments strengthened by the sling. Methods: Review for prevalence, pathogenic pathway from damaged USLs to pain, OAB, emptying, and other late complications by reference to data from functional anatomy imaging, mechanical support of USLs (speculum test), and post-USL surgical repair. Results: Pelvic pain and other pelvic symptoms frequently co-exist pre-operatively with SUI, but are not volunteered because patients complain of one main pelvic symptom, others being "under the surface" (Pescatori Iceberg). Late de novo occurrence of symptom complications beyond perioperative MUS surgery may occur: pain (5.7%), retention (5.4%), UTI (9.3%), and OAB (10.2%). Xray/ultrasound evidence of pelvic forces acting on USLs support the hypothesis of diversion of forces. Improvement of pain and urgency by the "speculum test" indicates USL causation, as do cure of pain and other pelvic symptoms by USL slings. Conclusions: Late-occurring PFS symptoms are the fault of neither implant, nor surgeon, but more likely consequences of pelvic forces acting on USLs already weakened by childbirth/age. Bladder/bowel/pain symptoms need to be sought out preoperatively and discussed before MUS surgery. Brief summary: Late MUS complications, OAB, pain, retention subject to class actions, may be caused by uterosacral dislocation from pre-existing structural weakness, not surgeon or device.

Abstract:Stress urinary incontinence (SUI) is a common condition that has a significant impact on quality of life. The integral theory showed that defective mid urethral support and the resultant low urethral pressure are the main mechanism behind SUI. Concerns have been raised about safety of vaginal mid urethral mesh tapes with a move towards native tissue treatments. Rectus fascial sling has high success rate. It is done through a laparotomy, and is associated with high rates of voiding dysfunction. More recently, sling on a string was introduced using a laparotomy, with blind introduction of the strings using a special curved needle. In this video, a novel technique of laparoscopic mid urethral fascial sling insertion under vision using rectus sheath fascia is shown in managing recurrent stress incontinence.

Abstract:Background Sacral fractures can cause lower urinary tract symptoms (LUTS) due to damage to the cauda equina. While several studies have reported on sacral fractures due to high-energy trauma, those due to fragility fractures have only been reported in case reports, and their clinical differences are not well known. This study aimed to investigate the clinical characteristics of LUTS caused by fragility sacral fractures and propose a novel treatment strategy. Methods This study is retrospective, uncontrolled, clinical case series. The inclusion criteria were sole sacral fractures due to low-energy trauma and appearance of LUTS after injury. Patients with additional spinal fractures or combined abdominal or pelvic organ injuries that could cause LUTS were excluded. Improvement in LUTS, period from onset to improvement, and imaging findings were recorded. Results Eight patients met the inclusion criteria (four surgical and four conservative treatment cases). Six patients showed improvement in LUTS. In surgical cases, the mean period from onset of LUTS to surgery and from onset of LUTS to improvement was 14.5 and 21.5 days, respectively. Intraoperative rupture or laceration of the dural sac was not observed. In two conservatively improved cases, the period from onset to improvement of LUTS was 14 and 17 days. Conclusions LUTS can improve even with conservative treatment and should be utilized as the primary choice. LUTS caused by severe sacral canal deformity and stenosis can be reversible, and the decision to perform surgical treatment is still timely if LUTS do not improve with conservative treatment for several weeks.

Abstract:Background: Treating an underactive bladder (UAB) is challenging. Previously, we introduced a more precise method of transvaginal ultrasound-guided botulinum toxin A (BoNT-A) injection into the external urethral sphincter as a treatment option for patients with UABs. Although many patients experience good results, those with an UAB and excessive residual urine still require catheterization. Therefore, we developed a new method that combines transvaginal ultrasound-guided BoNT-A injection with a transurethral bladder neck incision. Methods: A prospective study was conducted on 16 patients who experienced symptoms of UAB and chronic urine retention. The treatment consisted of a combination of transvaginal ultrasound-guided BoNT-A injection and a transurethral incision of the bladder neck (TUI-BN). The primary objective was to assess the efficacy of this combined treatment in improving symptoms in women with UABs. Results: Our study demonstrated significant improvements after treatment, including increased voiding volume, decreased post-void residual (PVR) urine, and improved voiding efficiency. The frequency of clean intermittent catheterization (CIC) decreased at 1 and 3 months post-surgery, along with improvements in the AUA symptoms score and the Patient Perception of Bladder Condition (PPBC) score. Conclusions: Our study showed significant improvements in the surgical treatment of UABs using a combination of transvaginal ultrasound-guided BoNT-A and TUI-BN.

Outcomes of Mid-Urethral Sling for Urodynamic Stress Incontinence Following Extensive Pelvic Reconstructive Surgery

Abstract:Introduction and hypothesis: To assess the outcomes of mid-urethral sling (MUS) procedures for urodynamic stress incontinence (USI) following extensive pelvic reconstructive surgery (PRS) and identify risk factors for persistent USI (P-USI). Methods: This retrospective study analyzed 84 women who underwent a staged approach to MUS for USI after PRS for advanced pelvic organ prolapse (Pelvic Organ Prolapse Quantification III and IV). The primary outcome was objective cure rate, defined by negative urine leakage on urodynamic study and a 1-h pad test weight of < 2 g. Subjective cure rate was through a negative response to question 3 of UDI-6. Results: The overall objective cure rate was 81.0%. The highest cure rate was observed in de novo USI (MUS-D; 89.7%) compared with women with persistent USD (MUS-P). Patients with overt SUI exhibited lower cure rates than those with occult SUI. Predictive factors for persistent USI were lower pre-operative maximum urethral closure pressure (MUCP; p = 0.031) and higher BMI in the MUS-P group than in the MUS-D group (p = 0.008). Subjective improvement was noted, especially in the MUS-D group, with a subjective cure rate of 78.6%. Those with MUS-D reported a higher impact on patient well-being post-surgery. No complications were observed after MUS surgery at follow-up. Conclusions: Overt USI, low MUCP and high BMI are independent predictors of persistent USI after a staged MUS approach after pelvic reconstructive surgery.
[An in Vivo Biomechanical Study of Chinese Female with Uterosacral Ligament Suspension].

Abstract:Objective: To measure in vivo biomechanical properties of pelvic support structures associated with uterosacral ligament suspension (ULS) . Methods: Ten Chinese female cadavers (death within 48 hours) without embalming were carefully dissected to expose the ligament. After dissection, ligaments were sewn according to standard ULS surgical technique. They were stretched at a steady rate to breakage during constant electronic registration of load and displacement. In vivo biomechanical properties were evaluated by a load-displacement curve. Results: Ultimate load of uterosacral ligament, ring around cervix were (37.3±23.5) , (49.3±28.4) N, respectively. The differences between the two groups was not significant (P>0.05) . Ligament stiffness were (1.26±1.22) , (1.45±0.92) N/mm, respectively. There was no significant difference between groups (P>0.05) . Conclusions: The uterosacral ligament, ring around cervix with sufficient strength are not easy to deform. ULS is an effective procedure for treating middle pelvic defects from a biomechanics perspective.

Abstract:OBJECTIVE:To establish the finite element model of uterosacral ligament (USL) and cardinal ligament (CL) and analyze the stress distribution and deformation with USL and CL under different working conditions.METHODS:Patients with stage Ⅲ-Ⅳ pelvic organ prolapse (POP) and healthy female volunteers were selected for research subject, and divided into anterior uterus group and posterior uterus group. Two POP patients and two volunteers were selectd into the anterior uterus group and posterior uterine group respectively. Pelvic MRI scan was performed in two groups. Based on the original MRI data sets, the finite element model of USL and CL was constructed by using the software such as the Mimics, and the stress distribution and deformation of USL and CL were simulated.RESULTS:Under the premise of the elastic modulus fixed and three different working conditions such as 60 cmH2O, 99 cmH2O and 168 cmH2O (1 cmH2O=0.098 kPa) with abdominal pressure generated by maximum Valsalva maneuver, according to the present conditions and the simulation, the trend was analyzed: the stress and deformation of the uterus, anterior vaginal wall, USL and CL in two groups were mainly distributed in the middle and lower part of the anterior vaginal wall or the ligament and the cervix-vagina junction, the maximum stress and the maximum displacement were mainly concentrated in the lower region of the anterior vaginal wall. With increasing of abdominal pressure generated by the maximum Valsalva maneuver, the maximum stress values of the POP patient in anterior uterus group under three different working conditions were: 0.027 9, 0.046 0, 0.078 0 MPa, and the maximum displacement values were: 9.145 5, 15.090 0, 25.607 0 mm. The maximum stress values of the volunteer in anterior uterus group under three different working conditions were: 0.012 6, 0.020 8, 0.035 3 MPa, and the maximum displacement values were: 1.816 7, 2.997 5, 5.086 7 mm. The maximum stress values of the POP patient in posterior uterine group under three different conditions were: 0.069 4, 0.114 6, 0.194 5 MPa, and the maximum displacement values were: 11.658 0, 19.236 0, 32.643 0 mm. The maximum stress values of the volunteer in posterior uterus group under three different working conditions were: 0.009 1, 0.015 1, 0.025 6 MPa, and the maximum displacement values were: 2.581 6, 4.259 6, 7.228 4 mm. The maximum stress values and the maximum displacement values were all increased with increasing of abdominal pressure in the two groups. The maximum stress values and the maximum displacement values of the POP patients were greater than those of volunteers. Under different working conditions, the maximum stress values and maximum displacement values of the posterior uterus POP patient were all greater than those of the anterior uterus POP patient.CONCLUSIONS:The finite element model of USL and CL is completely based on the MRI technology and the model is real and reliable. The increase of abdominal pressure will produce a larger stress and deformation of USL and CL, which is one of the reasons causing the injury of the ligament.
Comparative Analysis of the Efficacy of Different Surgical Modalities for the Treatment of Female Stress Urinary Incontinence: A Multicenter Retrospective Study

Materials and Methods: A total of 243 SUI patients who were admitted to Minda Hospital of Hubei Minzu University, were chosen and split into four groups based on various surgical techniques: the AFS group (59 cases), TVT-O group (61 cases), epithelial flap group (62 cases) and ASIS group (61 cases). A comparison of four patient groups' quality-of-life scores, urodynamic indices, urethral structure-related indices before and a year after treatment, complication rates and perioperative-related indices. Results: There was no discernible difference in the treatment outcomes among the four groups(P> 0.05). However, the ASIS group exhibited significantly reduced intraoperative hemorrhage, hospitalization duration and operative time compared to the mucosal flap and TVT-O groups(P< 0.05). Both the TVT-O and ASIS groups demonstrated significantly higher hospitalization expenditures than the mucosal flap and AFS groups (P 0.05). The TVT-O group showed a significantly higher incidence of postoperative medial thigh pain compared to the AFS, mucosal flap, and ASIS groups(P< 0.05). Similarly, the AFS group had a significantly greater incidence of postoperative urine retention compared to the mucosal flap, TVT-O, and ASIS groups(P< 0.05). Conclusion: While all four surgical techniques demonstrated good efficacy and improved patients' quality of life, our study suggests that TVT-O had a significantly higher incidence of post-treatment medial thigh pain and ASIS may be a safer and less problematic surgical approach for the treatment of female SUI. This has important therapeutic implications. Based on survey, 1 the global prevalence of urine incontinence in women is up to 38%. Individuals suffering from SUI have depressive, anxious or insomniac feelings, which can significantly lower their standard of living. The mechanism of SUI may be due to downward displacement of the bladder neck and proximal urethra, decreased closure of the urethral mucosa, or decreased function of the intrinsic urethral sphincter. The main treatment options for SUI encompass both conservative and surgical approaches. Conservative treatments include pelvic floor muscle exercise, pelvic floor magnetic wave stimulation therapy and intravaginal laser therapy. Surgery is recommended when the patient has severe SUI or when there is no change in symptoms after 3 months of conservative treatments. There are several surgical options for treating SUI. Multiple studies have demonstrated 2 that tension-free vaginal tape-obturator (TVT-O) is a procedure with definitive efficacy. However, concerns over complications associated with mesh implantation, such as infections, inner thigh discomfort, and pain during sexual activity, have fueled growing interest in researching alternatives. Autologous fascial midurethral sling (AFS) can prevent the the risks associated with mesh implantation. 3 However, this procedure has drawbacks including long operating times, high operator requirements, and increased intraoperative bleeding that limit its widespread application. The urethral suspension in anterior vaginal wall epithelial flap is more common for the treatment of anterior pelvic prolapse. 4 In recent years, research 5,6 has demonstrated the effectiveness of urethral suspension in anterior vaginal wall epithelial flap for the treatment of SUI. Abstract:Abstract Background Most bladder tumors are derived from the urothelium. Benign mesenchymal tumors are rare. Leiomyoma of the bladder is the most common benign neoplasm. We present a case of leiomyoma of the bladder presenting with acute urinary retention in a female patient and report on the post-operative change in urodynamic findings. To our knowledge, few cases of this kind have been reported. Case Presentation A 56-year-old woman presented with acute urinary retention. Evaluations including ultrasound, magnetic resonance imaging, cystoscopy, and urodynamics contributed to a diagnosis of leiomyoma of the bladder. Various medications were ineffective for solving her lower urinary tract symptoms; therefore, a transurethral resection was performed. The final pathological report was leiomyoma. After the operation, her symptoms resolved; this improvement was confirmed by an urodynamic analysis. The postoperative urodynamics demonstrated a lower frequency of detrusor overactivity during filling cystometry and an increase in the uroflow rate, with reduced detrusor pressure in a pressure flow study. Conclusions Leiomyoma of the bladder can cause female outlet obstruction. A review of the literature and disease management is discussed.

Abstract:Introduction Bladder outlet obstruction (BOO) in women includes functional and anatomic etiologies. Primary bladder neck obstruction (PBNO), Fowler's syndrome (FS), and dysfunctional voiding (DV) are some examples of functional obstructions, whereas pelvic organ prolapse (POP), periurethral masses, and intragenic causes are some of the anatomic causes. Methods This literature review describes the etiologies of female BOO, unique aspects of the workup and diagnosis, and the data for the standard surgical treatments and newer surgical techniques to treat women. Urethral stenosis and sling‐related obstruction are treated in the other articles of this series. Where possible the focus is the efficacy and outcomes. Results Treatment of PBNO using a transurethral incision of the bladder neck and injection of botulinum toxin in the bladder neck decreases the BOO. After the failure of conservative approaches, sacral neuromodulation (SNM) is effective for FS, while DV may benefit from SNM or botulinum toxin injections. Concerning POP, most surgeries have been reported to significantly improve a pre‐existent BOO but the level of evidence is low. Benign urethral and periurethral masses may provoke BOO, and surgical excision usually resolves this condition. Conclusion Although most surgical treatments of BOO for functional and benign anatomical etiologies in women seem to be effective, data are scarce even for more common conditions like POP. Further studies are required to give better advice on the choice of surgical technique for these patients.

The fragile urethra: what to do next?-a narrative review

Abstract:Background and objective: Although the artificial urinary sphincter (AUS) has demonstrated successful outcomes in treating male stress urinary incontinence (SUI) for the past five decades, this procedure also carries inherent risks, including recurrent SUI, device malfunction, local tissue compromise, and infection/erosion, all of which may require revision surgery with or without device replacement. Patients that are at the highest risk for such untoward events often possess unhealthy urethral tissue (termed a "fragile urethra") that is compromised and unable to provide optimal cuff coaptation and continence. Accordingly, there are several techniques to address recalcitrant SUI in the setting of a fragile urethra to afford an improved chance of return to continence. Here, we review characteristics of patients that are at higher risk for an untoward outcome following AUS implantation and further define strategies to promote optimal success with device implantation. The aim of this paper is to review the available literature and describe surgical options for male SUI in patients with known or anticipated urethral tissue compromise. Methods: A thorough literature review was completed by querying PubMed for relevant articles. Search terms included artificial urinary sphincter, failure, recalcitrant, urethral atrophy, fragile urethra, revision, radiation, cystectomy, incontinence, and/or urethroplasty published between 1975 and 2022. Key content and findings: Options for management of the fragile urethra include cuff relocation, cuff downsizing, tandem cuff placement, transcorporal cuff placement, pressure regulating balloon exchange with increased or decreased pressure, bulbospongiosus preservation, sub-cuff ventral capsulotomy, urethral wrapping with graft, and in select cases, urinary diversion, or complete device removal with a return to SUI. Proper patient selection is paramount to optimize outcomes. Advantages and disadvantages of each strategy are reviewed. Conclusions: Numerous techniques are viable options for patients with recalcitrant SUI in the setting of a fragile urethra, but high-quality evidence with reproducible outcomes for many of these strategies remain limited. Proper patient selection as well as adequate counseling by experienced implant surgeons may help optimize outcomes. Further multi-institutional investigations with longer term outcomes are needed to improve patient selection and counseling with shared decision-making prior to any intervention.
