Abstract:The foot progression angle is an important measurement related to knee loading, pain, and function for individuals with knee osteoarthritis, however current measurement methods require camera-based motion capture or floor-embedded force plates confining foot progression angle assessment to facilities with specialized equipment. This paper presents the validation of a customized smart shoe for estimating foot progression angle during walking. The smart shoe is composed of an electronic module with inertial and magnetometer sensing inserted into the sole of a standard walking shoe. The smart shoe charges wirelessly, and up to 160h of continuous data (sampled at 100Hz) can be stored locally on the shoe. For validation testing, fourteen healthy subjects were recruited and performed treadmill walking trials with small, medium, and large toe-in (internal foot rotation), small, medium, and large toe-out (external foot rotation) and normal foot progression angle at self-selected walking speeds. Foot progression angle calculations from the smart shoe were compared with measurements from a standard motion capture system. In general, foot progression angle values from the smart shoe closely followed motion capture values for all walking conditions with an overall average error of 0.1±1.9deg and an overall average absolute error of 1.7±1.0deg. There were no significant differences in foot progression angle accuracy across the seven different walking gait patterns. The presented smart shoe could potentially be used for knee osteoarthritis or other clinical applications requiring foot progression angle assessment in community settings or in clinics without specialized motion capture equipment.

Abstract:The quantification of plantar pressure distribution is widely done in the diagnosis of lower limbs deformities, gait analysis, footwear design, and sport applications. To date, a number of pressure insole layouts have been proposed, with different configurations according to their applications. The goal of this study is to assess the validity of a 16-sensors (1.5 × 1.5 cm) pressure insole to detect plantar pressure distribution during different tasks in the clinic and sport domains. The data of 39 healthy adults, acquired with a Pedar-X® system (Novel GmbH, Munich, Germany) during walking, weight lifting, and drop landing, were used to simulate the insole. The sensors were distributed by considering the location of the peak pressure on all trials: 4 on the hindfoot, 3 on the midfoot, and 9 on the forefoot. The following variables were computed with both systems and compared by estimating the Root Mean Square Error (RMSE): Peak/Mean Pressure, Ground Reaction Force (GRF), Center of Pressure (COP), the distance between COP and the origin, the Contact Area. The lowest (0.61%) and highest (82.4%) RMSE values were detected during gait on the medial-lateral COP and the GRF, respectively. This approach could be used for testing different layouts on various applications prior to production.

Center of Pressure Measurement Accuracy via Insoles with a Reduced Pressure Sensor Number during Gaits

Abstract:The objective was to compare simplified pressure insoles integrating different sensor numbers and to identify a promising range of sensor numbers for accurate center of pressure (CoP) measurement. Twelve participants wore a 99-sensor Pedar-X insole (100 Hz) during walking, jogging, and running. Eight simplified layouts were simulated, integrating 3-17 sensors. Concordance correlation coefficients (CCC) and root mean square errors (RMSE) between the original and simplified layouts were calculated for time-series mediolateral (ML) and anteroposterior (AP) CoP. Differences between layouts and between gait types were assessed via ANOVA and Friedman test. Concordance between the original and simplified layouts varied across layouts and gaits (CCC: 0.43-0.98; χ(7)2 ≥ 34.94, p < 0.001). RMSEML and RMSEAP [mm], respectively, were smaller in jogging (5 ± 2, 15 ± 9) than in walking (8 ± 2, 22 ± 4) and running (7 ± 4, 20 ± 7) (ηp2: 0.70-0.83, p < 0.05). Only layouts with 11+ sensors achieved CCC ≥ 0.80 in all tests across gaits. The 13-sensor layout achieved CCC ≥ 0.95 with 95% confidence, representing the most promising compromise between sensor number and CoP accuracy. Future research may refine sensor placement, suggesting the use of 11-13 sensors. For coaches, therapists, and applied sports scientists, caution is recommended when using insoles with nine or fewer sensors. Consulting task-specific validation results for the intended products is advisable.

Abstract:BackgroundWearable measurement of center of pressure (COP) coordinates is the key of obtaining the measurement of natural gait. Plantar pressure insole is the right sensing unit for plantar pressure monitoring for long-term outdoor measurements and the control of walking assisting exoskeleton robot. It’s necessary to study the configuration of pressure sensing cells.MethodsThis study explored the sensing cell configuration for the plantar pressure insole. The data of plantar pressure of walking is collected for layout variants. The RMSE of COP coordinates estimations are used as the evaluation criteria.ResultsThe RMSE of COP coordinates decreases from 8.00 to 3.20 mm as the amount of pressure sensing cells increases from 2 to 7. The size of pressure sensing cells contribute to reduce the RMSE of COP coordinates and 7 pressure sensing cells, with the size of 2.0–2.5 cm have the satisfying performance. Adding pressure sensing cell in the heel and hallux area increase the accuracy of estimating COP coordinates.ConclusionComparison results indicate that the configuration of 7 pressure sensing cells has a satisfying measurement performance.

Estimating ground reaction force with novel carbon nanotube-based textile insole pressure sensors

Abstract:This study presents a new wearable insole pressure sensor (IPS), composed of fabric coated in a carbon nanotube-based composite thin film, and validates its use for quantifying ground reaction forces (GRFs) during human walking. Healthy young adults (n = 7) walked on a treadmill at three different speeds while data were recorded simultaneously from the IPS and a force plate (FP). The IPS was compared against the FP by evaluating differences between the two instruments under two different assessments: (1) comparing the two peak forces at weight acceptance and push-off (2PK) and (2) comparing the absolute maximum (MAX) of each gait cycle. Agreement between the two systems was evaluated using the Bland-Altman method. For the 2PK assessment, the group mean of differences (MoD) was -1.3 ± 4.3% body weight (BW) and the distance between the MoD and the limits of agreement (2S) was 25.4 ± 11.1% BW. For the MAX assessment, the average MoD across subjects was 1.9 ± 3.0% BW, and 2S was 15.8 ± 9.3% BW. The results of this study show that this sensor technology can be used to obtain accurate measurements of peak walking forces with a basic calibration and consequently open new opportunities to monitor GRF outside of the laboratory.
Customized Textile Capacitive Insole Sensor for Center of Pressure Analysis

Abstract:Center of pressure refers to the centroid of the ground reaction force vector detected underneath the walking foot, which is a summary measure representing body segment movements during human locomotion. In this study, we developed a cost-effective, lightweight insole-type textile capacitive sensor (I-TCPs) to analyze plantar pressure (PP) distribution and center of pressure (COP) trajectory. To test the accuracy of I-TCPs, the measured pressure data was compared with that of F-scan. The sensor performance test was divided into a static baseline test and a dynamic gait experiment, both at two different gait speeds self-selected by the subjects. Static gait results showed that I-TCPs were capable of recognizing PP segments at different gait speeds. Dynamic gait results showed an average RMSE of 1.29 ± 0.47 mm in COPx (mediolateral shift) and 12.55 ± 5.08 mm in COPy (anteroposterior shift) at a comfortable gait speed. The COP correlation between I-TCPs and F-scan was 0.54 ± 0.09 in COPx and 0.92 ± 0.04 in COPy in comfortable gait speed conditions, in which COPy values presented a stronger correlation. RMSE and correlation in fast gait speed conditions also showed similar results. The findings of this study can be the basis for future research, including rehabilitation engineering, developing ICT devices, and creating smart wearable sensors to improve quality of life for patients and healthy individuals.
Evaluation of Different Pressure-Based Foot Contact Event Detection Algorithms across Different Slopes and Speeds

Abstract:If validated, in-shoe pressure measuring technology allows for the field-based quantification of running gait, including kinematic and kinetic measures. Different algorithmic methods have been proposed to determine foot contact events from in-shoe pressure insole systems, however, these methods have not been evaluated for accuracy, reliability against a gold standard using running data across different slopes, and speeds. Using data from a plantar pressure measurement system, seven different foot contact event detection algorithms based on pressure signals (pressure sum) were compared to vertical ground reaction force data collected from a force instrumented treadmill. Subjects ran on level ground at 2.6, 3.0, 3.4, and 3.8 m/s, six degrees (10.5%) inclined at 2.6, 2.8, and 3.0 m/s, and six degrees declined at 2.6, 2.8, 3.0, and 3.4 m/s. The best performing foot contact event detection algorithm showed maximal mean absolute errors of only 1.0 ms and 5.2 ms for foot contact and foot off, respectively, on level grade, when compared to a 40 N ascending and descending force threshold from the force treadmill data. Additionally, this algorithm was unaffected by grade and had similar levels of errors across all grades.

Predicting vertical and shear ground reaction forces during walking and jogging using wearable plantar pressure insoles

Abstract:Background: The development of plantar pressure insoles has made them a potential replacement for force plates. These wearable devices can measure multiple steps and might be used outside of the lab environment for rehabilitation and evaluation of sport performance. However, they can only measure the vertical force which does not completely represent the vertical ground reaction force. In addition, they are not able to measure shear forces which play an import role in the dynamic performance of individuals. Indirect approaches might be implemented to improve the accuracy of the force estimated by plantar pressure systems. Research question: The aim of this study was to predict the vertical and shear components of ground reaction force from plantar pressure data using recurrent neural networks. Methods: Ground reaction force and plantar pressure data were collected from 16 healthy individuals during 10 trials of walking and five trials of jogging using Bertec force plates at 1000 Hz and FScan plantar pressure insoles at 100 Hz. A long short-term memory neural network was built to consider the time dependency of pressure and force data in predictions. The data were split into three subsets of train, to train the model, evaluate, to optimize the model hyperparameters, and test sets, to assess the accuracy of the model predictions. Results: The results of this study showed that our long short-term memory model could accurately predict the shear and vertical force components during walking and jogging. The predictions were more accurate during walking compared to jogging. In addition, the predictions of mediolateral force had higher error and lower correlation compared to vertical and anteroposterior components. Significance: The long short-term memory model developed in this study may be an acceptable option for accurate estimation of ground reaction force during outdoor activities which can have significant impacts in rehabilitation, sport performance, and gaming.

Abstract:BackgroundWearable sensor systems can provide data for at-home gait analyses and input to controllers for rehabilitation devices but they often have reduced estimation accuracy compared to laboratory systems. The goal of this study is to evaluate a portable, low-cost system for measuring ground reaction forces and ankle joint torques in treadmill walking and calf raises.MethodsTo estimate the ground reaction forces and ankle joint torques, we developed a custom instrumented insole and a tissue force sensor. Six healthy subjects completed a collection of movements (calf raises, 1.0 m/s walking, and 1.5 m/s walking) on two separate days. We trained artificial neural networks on the study data and compared the estimates to a multi-camera motion system and an instrumented treadmill. We evaluated the relative strength of each sensor by testing each sensor’s ability to predict the ankle joint torque calculated from a reference inverse kinematics algorithm. We assessed model accuracy through root mean squared error and normalized root mean square error. We hypothesized that the estimation of the models would have normalized root mean square error measures less than 10 %.ResultsFor walking at 1.0 and walking at 1.5 m/s, the single-task, intra-day and multi-task, intra-day predictions had normalized root mean square error less than 10 % for all three force components and both center of pressure components. For the calf raise task, the single-task, intra-day and multi-task, intra-day predictions had normalized root mean square error less than 10 % for only the anterior-posterior center of pressure. The multi-task, intra-day model had similar predictions to the single-task, intra-day model. The normalized root mean square error of predictions from the insole sensor alone were less than 10 % for walking at 1.0 m/s and 1.5 m/s. No sensor was sufficient for the calf raise task. The combination of the insole sensor and the tendon sensor had lower normalized root mean square error than the individual sensors for all three tasks.ConclusionsThe proposed sensor system provided accurate estimates for five of the six components of the ground reaction kinetics during walking at 1.0 and 1.5 m/s and one of the six components during the calf raise task. The normalized root mean square error of the predictions of the ground reaction forces were similar to published studies using commercial devices. The proposed system of low-cost sensors can provide useful estimations of ankle joint torque for both walking and calf raises for future studies in mobile gait analysis.

Heel and toe clearance estimation for gait analysis using wireless inertial sensors

Abstract:Tripping is considered a major cause of fall in older people. Therefore, foot clearance (i.e., height of the foot above ground during swing phase) could be a key factor to better understand the complex relationship between gait and falls. This paper presents a new method to estimate clearance using a foot-worn and wireless inertial sensor system. The method relies on the computation of foot orientation and trajectory from sensors signal data fusion, combined with the temporal detection of toe-off and heel-strike events. Based on a kinematic model that automatically estimates sensor position relative to the foot, heel and toe trajectories are estimated. 2-D and 3-D models are presented with different solving approaches, and validated against an optical motion capture system on 12 healthy adults performing short walking trials at self-selected, slow, and fast speed. Parameters corresponding to local minimum and maximum of heel and toe clearance were extracted and showed accuracy ± precision of 4.1 ± 2.3 cm for maximal heel clearance and 1.3 ± 0.9 cm for minimal toe clearance compared to the reference. The system is lightweight, wireless, easy to wear and to use, and provide a new and useful tool for routine clinical assessment of gait outside a dedicated laboratory.
Effect of sensor location for modifying center of pressure during gait using haptic feedback in people with chronic ankle instability

Abstract:Background Gait retraining using haptic biofeedback medially shifts the center of pressure (COP) while walking in orthopedic populations. However, the ideal sensor location needed to effectively shift COP medially has not been identified in people with chronic ankle instability (CAI). Research questions Can a heel sensor location feasibly be employed in people with CAI without negatively altering kinematics? Does a heel sensor placement relative to the 5th metatarsal head (5MH) impact COP location while walking in people with CAI? Methods In this exploratory crossover study, 10 participants with CAI walked on a treadmill with vibration feedback for 10 minutes with a plantar pressure sensor under the heel and 5MH. Separate 2×2 repeated measures analyses of covariances (rmANCOVAs) were used to compare the averaged COP location and 3-D lower extremity kinematics from the first 10% of stance before and after training and between sensor locations. Baseline measures served as covariates to adjust for baseline differences. Results Feedback triggered by a heel sensor resulted in 40% of participants avoiding a heel strike. There were no significant main effects or interactions between time and sensor location on COP location when controlling for baseline COP (p>0.05). However, with the 5MH placement, participants displayed less ankle internal rotation(IR) (5MH/Heel: −4.12±0.00o/ −6.43±0.62o), less forefoot abduction (-4.29±0.00o/ −5.14±1.01o), more knee flexion (3.40±0.32o/ 0.14±0.57o), less knee external rotation (-10.95±0.00o/-11.24±1.48o), less hip extension (-0.20±0.00o/-1.42±1.05o), and less hip external rotation (3.12±0.00o/3.75±1.98o). Significance A 5MH location may be more feasible based on difficulties maintaining heel strike when the sensor was under the heel. While no sensor location was statistically better at changing the COP, the 5MH location decreased proximal transverse plane motions making participants' gait more like controls. Individual response variations support comprehensive lower extremity assessments and the need to identify responder profiles using sensory feedback in people with CAI.

Abstract:Wireless pressure insoles may enable the assessment of movement biomechanics in a real-world setting, and thus play an important role in the recommendation of clinical management, but they are not yet a gold standard due to the unknown accuracy and reliability with respect to different functional activities. Here, we compare novel wireless pressure insoles with force plates and examine the test–retest reliability of the insoles for measuring vertical ground reaction forces (vGRFs) and trajectories of the center of pressure (COP). In this observational study, healthy adults underwent two data collection sessions during one day. The Bland–Altman analysis was used to compare the outcomes measured with the two instruments during squats, jumps, and the sit-to-stand test. Test–retest reliability was assessed by the interclass correlation coefficient and the standard error of measurement for the outcomes during squats, jumps, walking, and stair ambulation. Trajectories of the COP in the anterior–posterior direction were comparable between the two systems during all activities. The insoles consistently measured shorter trajectories of the COP in the medial–lateral direction (except jumps) and lower vGRFs than the force plates. Test–retest reliability of the insoles was fair to high or excellent for all outcomes during all activities. In conclusion, the insoles provide reliable measures of vGRFs and trajectories of the COP during multiple functional activities in healthy adults. Although the insoles do not produce identical results to the force plate, the qualitative similarity and consistency between the two systems confirm the insoles can be used to measure these outcomes, based on the purpose and accuracy required.

Evaluation of in-shoe plantar pressure and shear during walking for diabetic foot ulcer prevention

Abstract:Objective: To investigate reliability and changes of in-shoe plantar pressure and shear during walking at three cadences with two insole designs. This was a precursor to the investigation of plantar loading in people with diabetes for potential foot ulcer prevention. Method: A sensorised insole system, capable of measuring plantar pressure and shear at the heel, fifth metatarsal head (5MH), first metatarsal head (1MH) and hallux, was tested with ten healthy participants during level walking. Reliability was evaluated, using intra-class correlation coefficient (ICC), while varying the cadences and insole types. Percentage changes in pressure and shear relative to values obtained at self-selected cadence with a flat insole design were investigated. Results: Mean±standard deviation of maximum pressure, medial-lateral and anterior-posterior shear of up to 380±24kPa, 46±2kPa and -71±4kPa, respectively, were measured. The ICC in ranges of 0.762-0.973, 0.758-0.987 and 0.800-0.980 were obtained for pressure, anterior-posterior and medial-lateral shear, respectively. Opposite anterior-posterior shear directions between 5MH and 1MH (stretching), and between 1MH and hallux (pinching) were observed for some participants. Increasing cadence increased pressure and anterior-posterior shear (by up to +77%) but reduced medial-lateral shear at the heel and hallux (by up to -34%). Slower cadence increased anterior-posterior shear (+114%) but decreased medial-lateral shear (-46%) at the hallux. The use of a flexible contoured insole resulted in pressure reduction at the heel and 5MH but an increase in anterior-posterior shear at the heel (+69%) and hallux (+75%). Conclusion: The insole system demonstrated good reliability and is comparable to reported pressure-only systems. Pressure measurements were sensitive to changes in cadence and insole designs in ways that were consistent with the literature. However, our plantar shear showed localised shear changes with cadences and insoles for the first time, as well as stretching and pinching effects on plantar tissue. This opens new possibilities to investigate plantar tissue viability, loading characteristics and orthotic designs aimed towards foot ulcer prevention.
Design of a Plantar Pressure Insole Measuring System Based on Modular Photoelectric Pressure Sensor Unit

Abstract:Accurately perceiving and predicting the parameters related to human walking is very important for man–machine coupled cooperative control systems such as exoskeletons and power prostheses. Plantar pressure data is rich in human gait and posture information and is an essential source of reference information as the input of the exoskeleton control system. Therefore, the proper design of the pressure sensing insole and validation is a big challenge considering the requirements such as convenience, reliability, no interference and so on. In this research, we developed a low-cost modular sensing unit based on the principle of photoelectric sensing and designed a plantar pressure sensing insole to achieve the purpose of sensing human walking gait and posture information. On the one hand, the sensor unit is made of economy-friendly commercial flexible circuits and elastic silicone, and the mechanical and electrical characteristics of the modular sensor unit are evaluated by a self-developed pressure-related calibration system. The calibration results show that the modular sensor based on the photoelectric sensing principle has fast response and negligible hysteresis. On the other hand, we analyzed the area where the plantar pressure is densely distributed. One benefit of the modular sensing unit design is that it is rather convenient to fabricate different insole solutions, so we fabricated and compared several pressure-sensitive insole solutions in this preliminary study. During the dynamic locomotion experiments of wearing the pressure-sensing insole, the time series signal of each sensor unit was collected and analyzed. The results show that the pressure sensing insole based on the photoelectric effect can sense the distribution of the plantar pressure by capturing the deformation of the insole caused by the foot contact during locomotion, and provide reliable gait information for wearable applications.

Plantar pressure during running in subjects with chronic ankle instability

Abstract:Background: It has been suggested that dynamic foot and ankle mechanics predispose individuals with CAI to repetitive episodes of the ankle ``giving way.'' Plantar pressure variations during a walking gait have been detected in those with CAI, but more dynamic conditions for analysis are needed. The purpose of this study was to evaluate plantar pressure distributions during a running gait in individuals with CAI, individuals who suffered a lateral ankle sprain, but did not develop CAI (AS), and subjects with no history of a lateral ankle sprain (CON). Materials and methods: Forty-five subjects [15 in each group, healthy males (18) and females (27), age 18 to 45] were recruited from University communities to participate in this study. Plantar pressure distributions were analyzed on a Tekscan© plantar pressure mat at 66 frames per second during a running gait at a controlled speed. The following variables were obtained: rearfoot medial/lateral (M/L) pressure ratio at foot strike (FS) and center-of-pressure (COP) trajectory during the initial loading response (heel strike to initial peak GRF). Separate one-way ANOVA with Tukey's post-hoc were used to test for group differences. The significance level was defined as p < 0.05. Results: The CAI group had a significantly more lateral ratio (0.97 ± 0.12) at FS when compared to the CON (1.01 ± 0.13) and AS (1.11 ± 0.13) groups. The CAI subjects had a lateral COP trajectory during the loading phase (7.97 degrees ± 11.02), while both the AS (-3.68 degrees ± 10.24) and CON groups (-6.27 degrees ± 9.86) had medial trajectories. The difference was significant between the CAI group and both the AS and CON groups (all significant {\it p} values were less than 0.05). Conclusion: Our results confirm that CAI subjects have a more lateral foot positioning and loading pattern during a barefoot running gait when compared to both the CON and LAS groups. Clinical relevance: Clinicians treating patients with CAI should consider providing interventions to decrease the amount of rearfoot inversion at FS and during loading in order to create a more medial COP trajectory upon impact.
Simplified versus geometrically accurate models of forefoot anatomy to predict plantar pressures: A finite element study

Abstract:Integration of patient-specific biomechanical measurements into the design of therapeutic footwear has been shown to improve clinical outcomes in patients with diabetic foot disease. The addition of numerical simulations intended to optimise intervention design may help to build on these advances, however at present the time and labour required to generate and run personalised models of foot anatomy restrict their routine clinical utility. In this study we developed second-generation personalised simple finite element (FE) models of the forefoot with varying geometric fidelities. Plantar pressure predictions from barefoot, shod, and shod with insole simulations using simplified models were compared to those obtained from CT-based FE models incorporating more detailed representations of bone and tissue geometry. A simplified model including representations of metatarsals based on simple geometric shapes, embedded within a contoured soft tissue block with outer geometry acquired from a 3D surface scan was found to provide pressure predictions closest to the more complex model, with mean differences of 13.3kPa (SD 13.4), 12.52kPa (SD 11.9) and 9.6kPa (SD 9.3) for barefoot, shod, and insole conditions respectively. The simplified model design could be produced in <1h compared to >3h in the case of the more detailed model, and solved on average 24% faster. FE models of the forefoot based on simplified geometric representations of the metatarsal bones and soft tissue surface geometry from 3D surface scans may potentially provide a simulation approach with improved clinical utility, however further validity testing around a range of therapeutic footwear types is required.

Abstract:Background: Three-dimensional motion analysis using optoelectronic cameras and force platforms is typically used to quantify gait disorders. However, these systems have various limitations, particularly when assessing patients in an ecological environment. To address these limitations, several wearable devices have been developed. However, few studies have reported metrological information regarding their repeatability and sensitivity to change. Methods: A healthy adult performed 6 min walking tests with FeetMe® system insoles under different walking conditions overground and on a treadmill. The standard error of measurement (SEM), the minimum detectable differences (MDDs), and the effect size (ES) were calculated for spatio-temporal parameters, and the ground reaction force was calculated from the 16,000 steps recorded. Results: SEM values were below 3.9% for the ground reaction force and below 6.8% for spatio-temporal parameters. ES values were predominantly high, with 72.9% of cases between overground and treadmill conditions with induced asymmetry, and 64.5% of cases between treadmill conditions with and without induced asymmetry exhibiting an ES greater than 1.2. The minimum detectable differences ranged from 4.5% to 10.7% for ground reaction forces and 2.1% to 18.9% for spatio-temporal parameters. Conclusion: Our study demonstrated that the FeetMe® system is a reliable solution. The sensitivity to change showed that these instrumented insoles can effectively reflect patient asymmetry and progress.

Abstract:Diabetic foot ulceration is driven by peripheral neuropathy, resulting in abnormal foot biomechanics and elevated plantar load. Plantar load comprises normal pressure and tangential shear stress. Currently, there are no in-shoe devices measuring both components of plantar load. The STAMPS (STrain Analysis and Mapping of the Plantar Surface) system was developed to address this and utilises digital image correlation (DIC) to determine the strain sustained by a plastically deformable insole, providing an assessment of plantar load at the foot-surface interface during gait. STAMPS was developed as a multi-layered insole, comprising a deformable mid-layer, onto which a stochastic speckle pattern film is applied. A custom-built imaging platform is used to obtain high resolution pre- and post-walking images. Images are imported into commercially available DIC software (GOM Correlate, 2020) to obtain pointwise strain data. The strain and displacement data are exported and post-processed with custom analysis routines (MATLAB, Mathworks Inc.), to obtain the resultant global and regional peak strain (SMAG), antero-posterior strain (SAP) and medio-lateral strain (SML). To validate the core technique an experimental test process used a Universal Mechanical Tester (UMT) system (UMT TriboLab, Bruker) to apply controlled vertical and tangential load regimes to the proposed multi-layer insole. A pilot study was then conducted to assess the efficacy of using the STAMPS system to measure in-shoe plantar strain in three healthy participants. Each participant walked 10 steps on the STAMPS insole using a standardised shoe. They also walked 10 m in the same shoe using a plantar pressure measurement insole (Novel Pedar®) to record peak plantar pressure (PPP) as a gold-standard comparator. The results of the experimental validation tests show that with increased normal force, at a constant shear distance, SMAG increased in a linear fashion. Furthermore, they showed that with increased shear distance, at a constant force, SMAG increased. The results of the pilot study found participant 1 demonstrated greatest SMAG in the region toes 3-5 (15.31%). The highest mean SMAG for participant 2 was at the hallux (29.31%). Participant 3 exhibited highest strain in the regions of the first and second metatarsal heads (58.85% and 41.62% respectively). Increased PPP was strongly associated with increased SMAG with a Spearman's correlation coefficient 0.673 (p < 0.0001). This study has demonstrated the efficacy of a novel method to assess plantar load across the plantar surface of the foot. Experimental testing validated the sensitivity of the method to both normal pressure and tangential shear stress. This technique was successfully incorporated into the STAMPS insole to reliably measure and quantify the cumulative degree of strain sustained by a plastically deformable insole during a period of gait, which can be used to infer plantar loading patterns. Future work will explore how these measures relate to different pathologies, such as regions at risk of diabetic foot ulceration.
Barefoot plantar pressure measurement in Chronic Exertional Compartment Syndrome

Abstract:Background: Patients with Chronic Exertional Compartment Syndrome (CECS) have exercise-limiting pain that subsides at rest. Diagnosis is confirmed by intramuscular compartment pressure (IMCP) measurement. Accompanying CECS, subjective changes to gait (foot slap) are frequently reported by patients. This has not previously been investigated. The aim of this study was to investigate differences in barefoot plantar pressure (BFPP) between CECS cases and asymptomatic controls prior to the onset of painful symptoms. Methods: 40 male military volunteers, 20 with symptoms of CECS and 20 asymptomatic controls were studied. Alternative diagnoses were excluded with rigorous inclusion criteria, magnetic resonance imaging and dynamic IMCP measurement. BFPP was measured during walking and marching. Data were analysed for: Stance Time (ST); foot progression angle (FPA); centre of force; plantarflexion rate after heel strike (IFFC-time); the distribution of pressure under the heel; and, the ratio between inner and outer metatarsal loading. Correlation coefficients of each variable with speed and leg length were calculated followed by ANCOVA or t-test. Receiver operating characteristic (ROC) curves were constructed for IFFC-time. Results: Caseshad shorter ST and IFFC-times than controls. FPA was inversely related to walking speed (WS) in controls only. The area under the ROC curve for IFFC-time ranged from 0.746 (95%CI: 0.636-0.87) to 0.773 (95%CI: 0.671-0.875) representing 'fair predictive validity'. Conclusion: Patients with CECS have an increased speed of ankle plantarflexion after heel strike that precedes the onset of painful symptoms likely resulting from a mechanical disadvantage of Tibialis Anterior. These findings provide further insight into the pathophysiology of CECS and support further investigation of this non-invasive diagnostic. The predictive value of IFFC-time in the diagnosis of CECS is comparable to post-exercise IMCP but falls short of dynamic IMCP measured during painful symptoms.

Abstract:Reliable computational foot models offer an alternative means to enhance knowledge on the biomechanics of human foot. Model validation is one of the most critical aspects of the entire foot modeling and analysis process. This paper presents an in vivo experiment combining motion capture system and plantar pressure measure platform to validate a three-dimensional finite element model of human foot. The Magnetic Resonance Imaging (MRI) slices for the foot modeling and the experimental data for validation were both collected from the same volunteer subject. The validated components included the comparison of static model predictions of plantar force, plantar pressure and foot surface deformation during six loading conditions, to equivalent measured data. During the whole experiment, foot surface deformation, plantar force and plantar pressure were recorded simultaneously during six different loaded standing conditions. The predictions of the current FE model were in good agreement with these experimental results.
