Abstract:Sensorized insoles provide a tool for gait studies and health monitoring during daily life. For users to accept such insoles they need to be comfortable and lightweight. Previous work has already demonstrated that estimation of ground reaction forces (GRFs) is possible with insoles. However, these are often assemblies of commercial components restricting design freedom and customization. Within this work, we investigate using four 3D-printed soft foam-like sensors to sensorize an insole. These sensors were combined with system identification of Hammerstein-Wiener models to estimate the 3D GRFs, which were compared to values from an instrumented treadmill as the golden standard. It was observed that the four sensors behaved in line with the expected change in pressure distribution during the gait cycle. In addition, the identified (personalized) Hammerstein-Wiener models showed the best estimation performance (on average RMS error 9.3%, R^2=0.85 and mean absolute error (MAE) 7%) of the vertical, mediolateral, and anteroposterior GRFs. Thereby showing that these sensors can estimate the resulting 3D force reasonably well. These results for nine participants were comparable to or outperformed other works that used commercial FSRs with machine learning. The identified models did decrease in estimation performance over time but stayed on average 11.35% RMS and 8.6% MAE after a week with the Hammerstein-Wiener model seeming consistent between days two and seven. These results show promise for using 3D-printed soft piezoresistive foam-like sensors with system identification to be a viable approach for applications that require softness, lightweight, and customization such as wearable (force) sensors.

Abstract:Wearable robotic devices require sensors and algorithms that can recognize the user state in real-time, in order to provide synergistic action with the body. For devices intended for locomotion-related applications, shoe-embedded sensors are a common and convenient choice, potentially advantageous for performing gait assessment in real-world environments. In this work, we present the development of a pair of pressure-sensitive insoles based on optoelectronic sensors for the real-time estimation of temporal gait parameters. The new design makes use of a simplified sensor configuration that preserves the time accuracy of gait event detection relative to previous prototypes. The system has been assessed relatively to a commercial force plate recording the vertical component of the ground reaction force (vGRF) and the coordinate of the center of pressure along the so-called progression or antero-posterior plane (CoPAP) in ten healthy participants during ground-level walking at two speeds. The insoles showed overall median absolute errors (MAE) of 0.06 (0.02) s and 0.04 (0.02) s for heel-strike and toe-off recognition, respectively. Moreover, they enabled reasonably accurate estimations of the stance phase duration (2.02 (2.03) % error) and CoPAP profiles (Pearson correlation coefficient with force platform ρCoP = 0.96 (0.02)), whereas the correlation with vGRF measured by the force plate was lower than that obtained with the previous prototype (ρvGRF = 0.47 (0.20)). These results confirm the suitability of the insoles for online sensing purposes such as timely gait phase estimation and discrete event recognition.

Abstract:The carbon fiber‐based smart insole system can not only exhibit real‐time high‐spatial‐resolution data of plantar pressure distribution for gait analysis, but also can regionalize and digitize whole plantar pressure distribution, remotely recognize diverse motion states, which provides a convenient and efficient communication platform that satisfies the continuously increasing demand for intelligent human‐machine interactions. Traditional approaches for monitoring human gait have severe spatial and temporal restrictions with complex analysis methods and high cost, which are powerless to promote the development of intelligent life involving fitness, sport training, and healthcare. Herein, a portable smart insole system with high spatial resolution and simple manufacturing process to measure plantar pressure distribution anytime, anywhere for gait analysis is proposed. An insole‐shaped array of 104 piezoresistive sensors with highly robust characteristics is assembled, exhibiting a good pressure‐sensing uniformity. The smart insole not only detects the subtle displacement of the center of gravity of the body, but also exhibits a real‐time, high‐resolution thermodynamic diagram of the plantar pressure distribution during human activities. More importantly, the function of motion intelligence identification can be realized by regionalizing and digitizing the whole plantar pressure distribution, achieving an average recognition accuracy of 83.32% among six predefined motions. These results imply that the carbon fiber‐based smart insole can provide an effective approach for convenient gait analysis and motion identification, which has a great potential in the application of future intelligent life.

Abstract:Gait phase monitoring wearable sensors play a crucial role in assessing both health and athletic performance, offering valuable insights into an individual's gait pattern. In this study, we introduced a simple and cost-effective capacitive gait sensor manufacturing approach, utilizing a micropatterned polydimethylsiloxane dielectric layer placed between screen-printed silver electrodes. The sensor demonstrated inherent stretchability and durability, even when the electrode was bent at a 45-degree angle, it maintained an electrode resistance of approximately 3 Ω. This feature is particularly advantageous for gait monitoring applications. Furthermore, the fabricated flexible capacitive pressure sensor exhibited higher sensitivity and linearity at both low and high pressure and displayed very good stability. Notably, the sensors demonstrated rapid response and recovery times for both under low and high pressure. To further explore the capabilities of these new sensors, they were successfully tested as insole-type pressure sensors for real-time gait signal monitoring. The sensors displayed a well-balanced combination of sensitivity and response time, making them well-suited for gait analysis. Beyond gait analysis, the proposed sensor holds the potential for a wide range of applications within biomedical, sports, and commercial systems where soft and conformable sensors are preferred.

Effects of Cyclic Mechanical Stretching on the mRNA Expression of Tendon/Ligament-Related and Osteoblast-Specific Genes in Human Mesenchymal Stem Cells

Abstract:The purpose of this study was to explore the influences of cyclic mechanical stretching on the mRNA expressions of tendon/ligament-related and osteoblast-specific marker genes in human MSCs seeded onto a collagen type I-coated surface. The stretch-induced mRNA expressions of mesenchymal stem cell protein (MSCP), matrix metalloproteinase-3 (MMP-3), and marker genes related to tendon/ligament cells (type I collagen, type III collagen, and tenascin-C) and those typical of osteoblasts (core binding factor alpha 1 (Cbfa1), alkaline phosphatase (ALP), and osteocalcin (OCN)) were analyzed by quantitative real-time PCR. The results revealed significant downregulation of MSCP and upregulation of MMP-3 genes in MSCs subjected to mechanical loading, regardless of the magnitude of the stretching (high or low). Moreover, the typical marker genes of the osteoblast lineage were upregulated by low-magnitude stretching, whereas tendon/ligament-related genes were upregulated by high-magnitude stretching for a long period. Cbfa1 and ALP were upregulated starting as early at 8 hr, followed by a downward trend and no significant change in expression at the other time points. The mRNA expressions of type I collagen, type III collagen, and tenascin-C significantly increased in MSCs subjected to 10% stretching for 48 hr, and this effect still existed after the stretched cells had rested for 48 hr. This study demonstrated the effect of cyclic mechanical stretching on differential transcription of marker genes related to different cell lineages. Low-magnitude stretching increased mRNA expressions of Cbfa1 and ALP and was possibly involved in the early osteoblastic differentiation of MSCs, whereas high-magnitude stretching upregulated the mRNA expressions of tendon/ligament-related genes.

Abstract:Gait analysis is a systematic study of human locomotion, which can be utilized in various applications, such as rehabilitation, clinical diagnostics and sports activities. The various limitations such as cost, non-portability, long setup time, post-processing time etc., of the current gait analysis techniques have made them unfeasible for individual use. This led to an increase in research interest in developing smart insoles where wearable sensors can be employed to detect vertical ground reaction forces (vGRF) and other gait variables. Smart insoles are flexible, portable and comfortable for gait analysis, and can monitor plantar pressure frequently through embedded sensors that convert the applied pressure to an electrical signal that can be displayed and analyzed further. Several research teams are still working to improve the insoles’ features such as size, sensitivity of insoles sensors, durability, and the intelligence of insoles to monitor and control subjects’ gait by detecting various complications providing recommendation to enhance walking performance. Even though systematic sensor calibration approaches have been followed by different teams to calibrate insoles’ sensor, expensive calibration devices were used for calibration such as universal testing machines or infrared motion capture cameras equipped in motion analysis labs. This paper provides a systematic design and characterization procedure for three different pressure sensors: force-sensitive resistors (FSRs), ceramic piezoelectric sensors, and flexible piezoelectric sensors that can be used for detecting vGRF using a smart insole. A simple calibration method based on a load cell is presented as an alternative to the expensive calibration techniques. In addition, to evaluate the performance of the different sensors as a component for the smart insole, the acquired vGRF from different insoles were used to compare them. The results showed that the FSR is the most effective sensor among the three sensors for smart insole applications, whereas the piezoelectric sensors can be utilized in detecting the start and end of the gait cycle. This study will be useful for any research group in replicating the design of a customized smart insole for gait analysis.

Abstract:The quantification of plantar pressure distribution is widely used, such as diagnosis of lower limb deformity, gait analysis, and so on. Here, an insole based on warp-knit spacer fabric and graphene was proposed, which is lightweight, wireless, affordable, and user-friendly. The insole with five sensing areas greatly optimizes the number of sensors required to be used. For the insole as a whole, the insole has a large pressure detection range, that is, 5–120 kPa. The pressure detection limit can reach 5 kPa with a 0.08-s response time and a 0.07-s recovery time, and the gauge factor (GF) can be up to 63.500. It also has good pressure response durability. It can maintain stability after it is used continuously for more than 30 min (more than 200 tests). Surely, the insole's sensing performance under compression, bending, and tensile deformation that must bear when walking is excellent and timely, though the deformation degree and frequency changes applied on it. When placed in the shoe, the insole does not interfere with the user's gait. It can recognize different human movements, such as normal walking, fast walking, sitting, and standing. It provides a feasible solution for low-cost, accurate, real-time, and high-user comfort foot movement/exercise biomechanical information acquisition, injury prevention, and ulcer prediction.

Abstract:BackgroundWearable sensor systems can provide data for at-home gait analyses and input to controllers for rehabilitation devices but they often have reduced estimation accuracy compared to laboratory systems. The goal of this study is to evaluate a portable, low-cost system for measuring ground reaction forces and ankle joint torques in treadmill walking and calf raises.MethodsTo estimate the ground reaction forces and ankle joint torques, we developed a custom instrumented insole and a tissue force sensor. Six healthy subjects completed a collection of movements (calf raises, 1.0 m/s walking, and 1.5 m/s walking) on two separate days. We trained artificial neural networks on the study data and compared the estimates to a multi-camera motion system and an instrumented treadmill. We evaluated the relative strength of each sensor by testing each sensor’s ability to predict the ankle joint torque calculated from a reference inverse kinematics algorithm. We assessed model accuracy through root mean squared error and normalized root mean square error. We hypothesized that the estimation of the models would have normalized root mean square error measures less than 10 %.ResultsFor walking at 1.0 and walking at 1.5 m/s, the single-task, intra-day and multi-task, intra-day predictions had normalized root mean square error less than 10 % for all three force components and both center of pressure components. For the calf raise task, the single-task, intra-day and multi-task, intra-day predictions had normalized root mean square error less than 10 % for only the anterior-posterior center of pressure. The multi-task, intra-day model had similar predictions to the single-task, intra-day model. The normalized root mean square error of predictions from the insole sensor alone were less than 10 % for walking at 1.0 m/s and 1.5 m/s. No sensor was sufficient for the calf raise task. The combination of the insole sensor and the tendon sensor had lower normalized root mean square error than the individual sensors for all three tasks.ConclusionsThe proposed sensor system provided accurate estimates for five of the six components of the ground reaction kinetics during walking at 1.0 and 1.5 m/s and one of the six components during the calf raise task. The normalized root mean square error of the predictions of the ground reaction forces were similar to published studies using commercial devices. The proposed system of low-cost sensors can provide useful estimations of ankle joint torque for both walking and calf raises for future studies in mobile gait analysis.

Abstract:Wireless pressure insoles may enable the assessment of movement biomechanics in a real-world setting, and thus play an important role in the recommendation of clinical management, but they are not yet a gold standard due to the unknown accuracy and reliability with respect to different functional activities. Here, we compare novel wireless pressure insoles with force plates and examine the test–retest reliability of the insoles for measuring vertical ground reaction forces (vGRFs) and trajectories of the center of pressure (COP). In this observational study, healthy adults underwent two data collection sessions during one day. The Bland–Altman analysis was used to compare the outcomes measured with the two instruments during squats, jumps, and the sit-to-stand test. Test–retest reliability was assessed by the interclass correlation coefficient and the standard error of measurement for the outcomes during squats, jumps, walking, and stair ambulation. Trajectories of the COP in the anterior–posterior direction were comparable between the two systems during all activities. The insoles consistently measured shorter trajectories of the COP in the medial–lateral direction (except jumps) and lower vGRFs than the force plates. Test–retest reliability of the insoles was fair to high or excellent for all outcomes during all activities. In conclusion, the insoles provide reliable measures of vGRFs and trajectories of the COP during multiple functional activities in healthy adults. Although the insoles do not produce identical results to the force plate, the qualitative similarity and consistency between the two systems confirm the insoles can be used to measure these outcomes, based on the purpose and accuracy required.

Abstract:Ground reaction force (GRF) data is often collected for the biomechanical analysis of running, due to the performance and injury risk insights that GRF analysis can provide. Traditional methods typically limit GRF collection to controlled lab environments, recent studies have looked to combine the ease of use of wearable sensors with the statistical power of machine learning to estimate continuous GRF data outside of these restrictions. Before such systems can be deployed with confidence outside of the lab they must be shown to be a valid and accurate tool for a wide range of users. The aim of this study was to evaluate how accurately a consumer-priced sensor system could estimate GRFs whilst a heterogeneous group of runners completed a treadmill protocol with three different personalised running speeds and three gradients. Fifty runners (25 female, 25 male) wearing pressure insoles made up of 16 resistive sensors and an inertial measurement unit ran at various speeds and gradients on an instrumented treadmill. A long short term memory (LSTM) neural network was trained to estimate both vertical ( G R F v ) and anteroposterior ( G R F a p ) force traces using leave one subject out validation. The average relative root mean squared error (rRMSE) was 3.2% and 3.1%, respectively. The mean ( G R F v ) rRMSE across the evaluated participants ranged from 0.8% to 8.8% and from 1.3% to 17.3% in the ( G R F a p ) estimation. The findings from this study suggest that current consumer-priced sensors could be used to accurately estimate two-dimensional GRFs for a wide range of runners at a variety of running intensities. The estimated kinetics could be used to provide runners with individualised feedback as well as form the basis of data collection for running injury risk factor studies on a much larger scale than is currently possible with lab based methods.
Flexible Insole Sensors with Stably Connected Electrodes for Gait Phase Detection

Abstract:Gait analysis is an important assessment tool for analyzing vital signals collected from individuals and for providing physical information of the human body, and it is emerging in a diverse range of application scenarios, such as disease diagnosis, fall prevention, rehabilitation, and human-robot interaction. Herein, a kind of surface processed conductive rubber was designed and investigated to develop a pressure-sensitive insole to monitor planar pressure in a real-time manner. Due to a novel surface processing method, the pressure sensor was characterized by stable contact resistance, simple manufacturing, and high mechanical durability. In the experiments, it was demonstrated that the developed pressure sensors were easily assembled with the inkjet-printed electrodes and a flexible substrate as a pressure-sensitive insole while maintaining good sensing performance. Moreover, resistive signals were wirelessly transmitted to computers in real time. By analyzing sampled resistive data combined with the gait information monitored by a visual-based reference system based on machine learning method (k-Nearest Neighbor algorithm), the corresponding relationship between plantar pressure distribution and lower limb joint angles was obtained. Finally, the experimental validation of the ability to accurately divide gait into several phases was conducted, illustrating the potential application of the developed device in healthcare and robotics.

Abstract:The trends of wearable health monitoring systems have led to growing demands for gait-capturing devices. However, comfortability and durability under repeated stress are still challenging to achieve in existing sensor-enabled footwear. Herein, a flexible textile piezoresistive sensor (TPRS) consisting of a reduced graphene oxide (rGO)-cotton) fabric electrode and an Ag fabric circuit electrode is proposed. Based on the mechanical and electrical properties of the two fabric electrodes, the TPRS exhibits superior sensing performance, with a high sensitivity of 3.96 kPa-1 in the lower pressure range of 0–36 kPa, wide force range (0–100 kPa), fast response time (170 ms), remarkable durability stability (1000 cycles) and detection ability in different pressures ranges. For the prac-tical application of capturing plantar pressure, six TPRSs were mounted on a flexible printed circuit board and integrated into an insole. The dynamic plantar pressure distribution during walking was derived in the form of pressure maps. The proposed fully-textile piezoresistive sensor is a strong candidate for next-generation plantar pressure wearable monitoring devices.

Abstract:Gait analysis refers to the systematic study of human locomotion and finds numerous applications in the fields of clinical monitoring, rehabilitation, sports science and robotics. Wearable sensors for real-time gait monitoring have emerged as an attractive alternative to the traditional clinical-based techniques, owing to their low cost and portability. In addition, 3D printing technology has recently drawn increased interest for the manufacturing of sensors, considering the advantages of diminished fabrication cost and time. In this study, we report the development of a 3D-printed capacitive smart insole for the measurement of plantar pressure. Initially, a novel 3D-printed capacitive pressure sensor was fabricated and its sensing performance was evaluated. The sensor exhibited a sensitivity of 1.19 MPa−1, a wide working pressure range (<872.4 kPa), excellent stability and durability (at least 2.280 cycles), great linearity (R2=0.993), fast response/recovery time (142–160 ms), low hysteresis (DH<10%) and the ability to support a broad spectrum of gait speeds (30–70 steps/min). Subsequently, 16 pressure sensors were integrated into a 3D-printed smart insole that was successfully applied for dynamic plantar pressure mapping and proven able to distinguish the various gait phases. We consider that the smart insole presented here is a simple, easy to manufacture and cost-effective solution with the potential for real-world applications.

Abstract:In recent years, advances in wearable medical devices have been driven by the development of flexible sensors and wireless IoT technologies, with research and applications in this field rapidly increasing. Plantar pressure analysis is a gait method that can be used to analyze foot pressure during human movement to identify potential health problems, but most of the devices based on this method are currently single-functional, poorly portable, and expensive. This work proposes a smart insole using meshless conductive rubber sensors, as well as a multi-channel data acquisition and transmission system based on these sensors, deep learning networks, and Bluetooth technology. The smart insole sensing unit and circuit board were made of flexible materials and powered by rechargeable lithium batteries. The system can be used to collect plantar pressure data from the sensors in real time, analyze the pressure in different regions of the foot through a neural network, and infer the gait cycle and abnormal gait of the user. The effective communication range can be up to 70 meters under barrier-free conditions. This smart insole has a bright future thanks to the pressure detection range, fast response time, and good durability.

However, typical gait analysis has to be performed in a gait laboratory, which is inaccessible for a large population and cannot provide natural gait measures. In this paper, we present a novel sensor device, namely, Smart Insole, to tackle the challenge of efficient gait monitoring in real life. An array of electronic textile (eTextile)-based pressure sensors are integrated in the insole to fully measure the plantar pressure. Smart Insole is also equipped with a low-cost inertial measurement unit including a three-axis accelerometer, a three-axis gyroscope, and a three-axis magnetometer to capture the gait characteristics in motion. Smart Insole can offer precise acquisition of gait information. Meanwhile, it is lightweight, thin, and comfortable to wear, providing an unobtrusive way to perform the gait monitoring. Furthermore, a smartphone graphic user interface is developed to display the sensor data in real-time via Bluetooth low energy. We perform a set of experiments in four real-life scenes including hallway walking, ascending/descending stairs, and slope walking, where gait parameters and features are extracted. Finally, the limitation and improvement, wearability and usability, further work, and healthcare-related potential applications are discussed.

Abstract:Monitoring gait patterns in daily life will provide a lot of biological information related to human health. At present, common gait pressure analysis systems, such as pressure platforms and in-shoe systems, adopt rigid sensors and are wired and uncomfortable. In this paper, a biomimetic porous graphene–SBR (styrene-butadiene rubber) pressure sensor (PGSPS) with high flexibility, sensitivity (1.05 kPa−1), and a wide measuring range (0–150 kPa) is designed and integrated into an insole system to collect, process, transmit, and display plantar pressure data for gait analysis in real-time via a smartphone. The system consists of 16 PGSPSs that were used to analyze different gait signals, including walking, running, and jumping, to verify its daily application range. After comparing the test results with a high-precision digital multimeter, the system is proven to be more portable and suitable for daily use, and the accuracy of the waveform meets the judgment requirements. The system can play an important role in monitoring the safety of the elderly, which is very helpful in today’s society with an increasingly aging population. Furthermore, an intelligent gait diagnosis algorithm can be added to realize a smart gait monitoring system.

Abstract:The quantification of plantar pressure distribution is widely done in the diagnosis of lower limbs deformities, gait analysis, footwear design, and sport applications. To date, a number of pressure insole layouts have been proposed, with different configurations according to their applications. The goal of this study is to assess the validity of a 16-sensors (1.5 × 1.5 cm) pressure insole to detect plantar pressure distribution during different tasks in the clinic and sport domains. The data of 39 healthy adults, acquired with a Pedar-X® system (Novel GmbH, Munich, Germany) during walking, weight lifting, and drop landing, were used to simulate the insole. The sensors were distributed by considering the location of the peak pressure on all trials: 4 on the hindfoot, 3 on the midfoot, and 9 on the forefoot. The following variables were computed with both systems and compared by estimating the Root Mean Square Error (RMSE): Peak/Mean Pressure, Ground Reaction Force (GRF), Center of Pressure (COP), the distance between COP and the origin, the Contact Area. The lowest (0.61%) and highest (82.4%) RMSE values were detected during gait on the medial-lateral COP and the GRF, respectively. This approach could be used for testing different layouts on various applications prior to production.
