Abstract:Sellers and colleagues 1 have published a study on the US experience with normothermic regional perfusion (NRP), a technology used in organ donation after circulatory determination of death (DCD). They surveyed organ procurement organizations (OPOs) that have recovered DCD organs in the US and, with a 100% survey response rate, found that 49 OPOs (89%) had experience with NRP. The OPO median (range) case volume was only 8 (1-52). Of deep concern is that nearly half of the OPOs (47%) who have carried out NRP did so without a policy or even a policy pending approval. NRP is a machine perfusion technology that reperfuses and reoxygenates organs inside the body of the donor in DCD. NRP can make organs suitable for transplant that would otherwise not be accepted; it can reduce complications in transplant recipients (particularly ischemic cholangiopathy post liver transplantation), provide time for functional organ assessment before organ removal, allow for a more controlled surgical technique for organ recovery, and reduce cost compared with the organ-specific ex-situ machine perfusion devices. 2 The regional aspect of NRP is that the resumed circulation can be isolated to the abdominal organs only (A-NRP) or the thoracic and abdominal organs (TA-NRP). Both types of NRP rely on surgical techniques to prevent recirculation of oxygenated blood reaching the brain and potentially invalidating the determination of death. 3 In TA-NRP this involves, as a minimum, clamping of the supra-aortic arch vessels and, in the UK and Spain, the additional step of venting the cephalad ends of these vessels to atmospheric pressure. 4 ,5 NRP has proven ethically controversial with the American College of Physicians issuing a statement of concern in 2021. 6 The concerns of this group and other authors can be categorized around conceptual, theoretical, and practice safety. Restoring circulation in the body of the donor appears, at first glance, not to satisfy the Uniform Determination of Death Act (UDDA) requirement for irreversible cessation of circulatory-respiratory function nor satisfy the ethical principle of the dead donor rule. At 5 minutes following circulatory arrest (typical in death determination protocols for DCD), NRP will restore circulation and organ function. In TA-NRP, the objective is to restore cardiac circulation so that the heart recommences beating in the body of the donor. But it is vital that this is done without resuscitating the patient, which could happen if circulation reaches the brain. Defenders of NRP emphasize that it is the brain circulation that matters, and that accepted medical standards for death determination rely on permanence (will not be restored) rather than a strict interpretation of irreversible (cannot be restored). 3 Permanence is maintained if circulation does not reach the brain. Even if NRP can be considered conceptually safe, a different concern is whether it is theoretically possible that brain circulation could restart in TA-NRP, despite efforts to isolate it. In standard DCD (without NRP) when the circulation ceases, brain function is rapidly lost and once the period when autoresuscitation of the heart could occur has passed (5 minutes), this state will permanently persist, as nothing will be done to restore any circulation to any part of the body. The anatomical considerations in NRP are complex, though empirical support is slowly emerging (but still only in very small patient numbers) that brain circulation can be safely isolated. However, there is a worldwide split on the best technique to achieve this in TA-NRP with the US adopting the simplified technique of clamping the supra-aortic arch vessels, while the UK and Spain have moved away from this technique on inadequate safety grounds. 4 ,5 In the UK, the TA-NRP program is currently paused while this issue is resolved, but the number of A-NRP cases increases each year. Very little in donation and transplantation is currently such that if a single error occurs, it threatens public trust and thereby the whole transplant program. In NRP, if the correct surgical technique is not applied each and every single time, brain function risks being restored. Detailed and exacting national surgical protocols for NRP are required and can be written. 7 In Europe TA-NRP has been built off extensive experience with A-NRP, whereas in the US, TA-NRP has led the way. 1 Inadvertently restoring brainstem blood flow in TA-NRP poses the most risk owing to anomalous vertebrobasilar vessels (which may not be surgically recognized) and the potential for direct medullary perfusion via retrograde collateral flow from the anterior spinal artery. 4 Although such low-level blood flow may not be sufficient to restore full consciousness, it could—let us pray never—prove enough for the donor to t -Abstract Truncated-
