Abstract:Single photon emission computed tomography (SPECT) is a cost-effective, widely-used diagnostic modality for the assessment of cardiac and cerebral functions. However, most SPECT protocols for clinical use support only qualitative reconstruction. Its lack of quantitative capability compromises the accurate evaluation of diseases. Quantitative SPECT by simultaneous compensation for different degradation factors is in great demand and should improve the diagnosis greatly. The variety of the existing SPECT algorithm can be split into a family of analytical methods and a wide class of iterative techniques. By reviewing the research work published in the past decades, this paper tries to make a state of arts and comprehensive outline of efficient, analytical solutions to SPECT reconstruction problem, which could compensate scatter, photon attenuation, and detector response, as well as suppression of Poisson noise, respectively.

Abstract:Abstract Attenuation correction has been one of the main methodological challenges in the integrated positron emission tomography and magnetic resonance imaging (PET/MRI) field. As standard transmission or computed tomography approaches are not available in integrated PET/MRI scanners, MR-based attenuation correction approaches had to be developed. Aspects that have to be considered for implementing accurate methods include the need to account for attenuation in bone tissue, normal and pathological lung and the MR hardware present in the PET field-of-view, to reduce the impact of subject motion, to minimize truncation and susceptibility artifacts, and to address issues related to the data acquisition and processing both on the PET and MRI sides. The standard MR-based attenuation correction techniques implemented by the PET/MRI equipment manufacturers and their impact on clinical and research PET data interpretation and quantification are first discussed. Next, the more advanced methods, including the latest generation deep learning-based approaches that have been proposed for further minimizing the attenuation correction related bias are described. Finally, a future perspective focused on the needed developments in the field is given.

Abstract:A wrong diagnosis will occur in cardiac SPECT imaging if the attenuation and scattering of photons by body's soft tissues are not considered. To correct the attenuation, the simultaneous emission and transmission projections are obtained from the same SPECT system, and attenuation correction algorithms must be used in the emission image reconstruction. This paper compares the quality of various image reconstruction algorithms in SPECT, such as conventional filtered back projection, weighted back projection, iterative Chang's method, ML EM, OS EM, RAMLA, ML EM with Gibbs smoothing, Bayesian reconstruction, etc.. A mathematical phantom is used to test the varias algorithms. The results suggest some consideration to choose the most appropriate algorithm. A new algorithm, the Multi object Optimization Method is proposed to improve image reconstruction and attenuation correction.

Abstract:Tying Sulzer's vein does not alter the enhanced metabolic response to noradrenaline of cold-acclimated rats, neither immediately after the operation nor 1, 2, or 5 days later. In contrast, removal of the interscapular brown adipose tissue, while having no marked immediate effect upon the enhanced metabolic response to noradrenaline, causes a delayed loss of the enhancement during the subsequent 2 days. These findings argue against the hypothesis that the local direction of heat from the interscapular brown adipose tissue via Sulzer's vein to the spinal cord has importance in determining the capacity of the rat to respond to noradrenaline or to use nonshivering thermogenesis. The difference between the effect of complete removal of the interscapular brown adipose tissue and the effect of tying Sulzer's vein would suggest that the reduced circulation through the interscapular brown adipose tissue when Sulzer's vein is tied is still sufficient for it to exert its influence upon other tissues. The effects of attenuation can lead to artifacts near high-activity structures, e.g., the liver, spleen, kidneys, or urinary bladder. Without effective attenuation correction, these artifacts may reduce diagnostic utility. This study compares three processing, methods: 1) zeroth-order Chang's attenuation correction (ZOCAC) with three-dimensional (3-D) Butterworth filtering; 2) quasi-optimal attenuation correction (QOAC) with 3-D Butterworth filtering; and 3) QOAC with a new adaptive filter. A physical phantom is used to determine effective attenuation coefficients to use with Tc-99m and with In-111. Computer simulation studies are used to optimize the parameters of the filters and to compare the three processing methods using bias versus variance plots. The results indicate that QOAC and the adaptive filter both improve the bias-variance tradeoff. Differences between the ZOCAC and the QOAC reconstructions are seen in clinical studies. QOAC noticeably reduces artifacts which have artificially low activity concentration near high-activity structures, e.g-, near the urinary bladder and great vessels in Tc-99m labeled red blood cell images. QOAC, combined with the new adaptive filter, holds promise for improving Tc-99m/In-111 SPECT reconstructions.

Abstract:ObjectiveHybrid PET/MRI presents many advantages in comparison with its counterpart PET/CT in terms of improved soft-tissue contrast, decrease in radiation exposure, and truly simultaneous and multi-parametric imaging capabilities. However, the lack of well-established methodology for MR-based attenuation correction is hampering further development and wider acceptance of this technology. We assess the impact of ignoring bone attenuation and using different tissue classes for generation of the attenuation map on the accuracy of attenuation correction of PET data.MethodsThis work was performed using simulation studies based on the XCAT phantom and clinical input data. For the latter, PET and CT images of patients were used as input for the analytic simulation model using realistic activity distributions where CT-based attenuation correction was utilized as reference for comparison. For both phantom and clinical studies, the reference attenuation map was classified into various numbers of tissue classes to produce three (air, soft tissue and lung), four (air, lungs, soft tissue and cortical bones) and five (air, lungs, soft tissue, cortical bones and spongeous bones) class attenuation maps.ResultsThe phantom studies demonstrated that ignoring bone increases the relative error by up to 6.8 % in the body and up to 31.0 % for bony regions. Likewise, the simulated clinical studies showed that the mean relative error reached 15 % for lesions located in the body and 30.7 % for lesions located in bones, when neglecting bones. These results demonstrate an underestimation of about 30 % of tracer uptake when neglecting bone, which in turn imposes substantial loss of quantitative accuracy for PET images produced by hybrid PET/MRI systems.ConclusionConsidering bones in the attenuation map will considerably improve the accuracy of MR-guided attenuation correction in hybrid PET/MR to enable quantitative PET imaging on hybrid PET/MR technologies.

Abstract:The combination of Technetium-99m-methoxyisobutylisonitrile (Tc-99m-MIBI) single-photon emission computed tomography (SPECT) and Fluorine-18-fluorodeoxyglucose (F-18-FDG) Positron Emission Tomography/Computed Tomography (PET/CT) for myocardial perfusion/metabolic imaging is commonly used to assess myocardial viability, guide treatment strategies, and predict outcomes in patients with ischemic heart failure. [ 1 , 2 ] Hibernating myocardium (HM) is a chronic and potentially reversible myocardial dysfunction caused by ischemia. In myocardial viability imaging, perfusion-metabolism mismatch is considered a characteristic feature of HM, whereas a decrease or lack of perfusion/metabolism match indicates the presence of infarct scar tissue. [ 3 , 4 ] Research evidence suggests that the extent of functional improvement in the heart after revascularization is positively correlated with the proportion of hibernating myocardium. [ 5 ] Hence, the presence and proportion of HM and scars are critical for determining the suitability for revascularization, where more HM and fewer scars correlate with a better potential for recovery and prognosis. Therefore, an accurate assessment of HM and scars is crucial for clinical decision-making. Clinical studies [ 6 ] have shown that timely coronary revascularization to restore blood flow to the HM can improve the symptoms and prognosis of patients with ischemic heart failure. While PET/CT myocardial perfusion imaging (MPI) generally provides high-quality imaging with reduced impact from soft tissue attenuation, making it ideal for evaluating myocardial blood flow, it is not entirely free from attenuation effects. Moreover, its widespread clinical application is limited by factors such as the availability of PET/CT facilities, accessibility, and the cost of positron-emitting radiopharmaceuticals. Consequently, most clinical practice studies [ 7–9 ] have employed SPECT MPI. However, SPECT MPI is known to suffer from attenuation artifacts caused by uneven absorption of low-energy gamma photons by the breast, diaphragm, and chest wall muscles, which can lead to false positives or exacerbate perceived reductions in perfusion. [ 10 , 11 ] Our search revealed that some large-scale multicenter clinical studies, such as the STICH trial, [ 12 ] used SPECT MPI without combining it with PET/CT metabolic imaging or applying attenuation correction (AC). Furthermore, recent studies [ 8 , 13–18 ] assessing myocardial viability using SPECT MPI combined with F-18-FDG PET/CT did not analyze AC-corrected SPECT data. In patients with ischemic heart failure, the enlarged heart could further exacerbate the range and severity of attenuation artifacts, potentially leading to greater misestimation of perfusion deficits, which could, in turn, affect the calculation results of HM, Scar, and other normal myocardial areas when combined with PET/CT metabolic imaging, thereby affecting the accuracy of myocardial viability and type calculations.

Anatomical-based Partial Volume Correction for Low-Dose Dedicated Cardiac SPECT/CT

Abstract:Due to the limited spatial resolution, partial volume effect has been a major degrading factor on quantitative accuracy in emission tomography systems. This study aims to investigate the performance of several anatomical-based partial volume correction (PVC) methods for a dedicated cardiac SPECT/CT system (GE Discovery NM/CT 570c) with focused field-of-view over a clinically relevant range of high and low count levels for two different radiotracer distributions. These PVC methods include perturbation geometry transfer matrix (pGTM), pGTM followed by multi-target correction (MTC), pGTM with known concentration in blood pool, the former followed by MTC and our newly proposed methods, which perform the MTC method iteratively, where the mean values in all regions are estimated and updated by the MTC-corrected images each time in the iterative process. The NCAT phantom was simulated for cardiovascular imaging with (99m)Tc-tetrofosmin, a myocardial perfusion agent, and (99m)Tc-red blood cell (RBC), a pure intravascular imaging agent. Images were acquired at six different count levels to investigate the performance of PVC methods in both high and low count levels for low-dose applications. We performed two large animal in vivo cardiac imaging experiments following injection of (99m)Tc-RBC for evaluation of intramyocardial blood volume (IMBV). The simulation results showed our proposed iterative methods provide superior performance than other existing PVC methods in terms of image quality, quantitative accuracy, and reproducibility (standard deviation), particularly for low-count data. The iterative approaches are robust for both (99m)Tc-tetrofosmin perfusion imaging and (99m)Tc-RBC imaging of IMBV and blood pool activity even at low count levels. The animal study results indicated the effectiveness of PVC to correct the overestimation of IMBV due to blood pool contamination. In conclusion, the iterative PVC methods can achieve more accurate quantification, particularly for low count cardiac SPECT studies, typically obtained from low-dose protocols, gated studies, and dynamic applications.

Abstract:Image degradation during single photon emission computed tomography (SPECT) due to attenuation and Compton scatter of photons can cause clinical image artifacts and will also result in inaccurate quantitative data. Therefore attenuation correction methods recently received wide interest. Transmission imaging can be performed to obtain the attenuation coefficients of a nonhomogeneous attenuating medium accurately. The aim of this study was firstly to evaluate the imaging characteristics of the scanning line source assembly. The results obtained with Tc-99m and Ce-139 were compared. Secondly the calculated attenuation coefficients were compared with known values from literature, using Tc-99m and Ce-139 as transmission sources. Lastly the method of acquiring simultaneous transmission and emission data was investigated. This study shows that an attenuation coefficient map can be obtained using a scanning line source for transmission imaging with a dual opposing detector camera. The imaging characteristics of Tc-99m and Ce-139 as transmission sources are similar. The resolution obtained with the Ce-139 line source was poorer than that obtained with the Tc-99m line source. A linear relationship was found between CT numbers and attenuation coefficients for transmission images using both Tc-99m and Ce-139 line sources. The attenuation coefficient value for water was underestimated by 1% using the Tc-99m transmission source and underestimated by 10% using Ce-139 as transmission source. This underestimation of attenuation coefficient values was also obtained in the human study. A myocardial perfusion study processed without and with attenuation correction clearly demonstrated the effect of the attenuation correction in the inferior myocardial region. The potential of using a scanning line source as transmission source with a dual opposing detector camera has been demonstrated in this study. The transmission source, Ce-139 was successfully introduced in this investigation for simultaneous acquisition of transmission and emission data.

Attenuation Correction of Pet Cardiac Data with Low-Dose Average Ct in Pet/Ct

Abstract:We proposed a low-dose average computer tomography (ACT) for attenuation correction (AC) of the PET cardiac data in PET/CT. The ACT was obtained from a cine CT scan of over one breath cycle per couch position while the patient was free breathing. We applied this technique on four patients who underwent tumor imaging with F18-FDG in PET/CT, whose PET data showed high uptake of F18-FDG in the heart and whose CT and PET data had misregistration. All four patients did not have known myocardiac infarction or ischemia. The patients were injected with 555-740MBq of F18-FDG and scanned 1h after injection. The helical CT (HCT) data were acquired in 16s for the coverage of 100cm. The PET acquisition was 3min per bed of 15cm. The duration of cine CT acquisition per 2cm was 5.9s. We used a fast gantry rotation cycle time of 0.5s to minimize motion induced reconstruction artifacts in the cine CT images, which were averaged to become the ACT images for AC of the PET data. The radiation dose was about 5mGy for 5.9s cine duration. The selection of 5.9s was based on our analysis of the respiratory signals of 600 patients; 87% of the patients had average breath cycles of less than 6s and 90% had standard deviations of less than 1s in the period of breath cycle. In all four patient studies, registrations between the CT and the PET data were improved. An increase of average uptake in the anterior and the lateral walls up to 48% and a decrease of average uptake in the septal and the inferior walls up to 16% with ACT were observed. We also compared ACT and conventional slow scan CT (SSCT) of 4s duration in one patient study and found ACT was better than SSCT in depicting average respiratory motion and the SSCT images showed motion-induced reconstruction artifacts. In conclusion, low-dose ACT improved registration of the CT and the PET data in the heart region in our study of four patients. ACT was superior than SSCT for depicting average respiration motion in a patient study.

Abstract:Quantitative PET attenuation correction (AC) for combined cardiac PET/MR is a challenging problem. We propose and evaluate an AC approach that uses coincidences from a relatively weak and physically fixed sparse external source, in combination with that from the patient, to correct for PET attenuation based on physics principles alone. The low 30 ml volume of the source makes it easy to fill and place, and the method does not use prior image data or attenuation map assumptions. Our supplemental transmission aided maximum likelihood reconstruction of attenuation and activity (sTX-MLAA) algorithm contains an attenuation map update that maximizes the likelihood of terms representing coincidences originating from tracer in the patient and a weighted expression of counts segmented from the external source alone. Both external source and patient scatter and randoms are fully corrected. We evaluated performance of sTX-MLAA compared to reference standard CT-based AC with FDG PET/CT phantom studies; including modeling a patient with myocardial inflammation. Through an ROI analysis we measured less than 5% bias in activity concentrations for PET images generated with sTX-MLAA relative to CT-AC. PET background variability (from noise and sparse sampling) was substantially reduced with sTX-MLAA compared to using coincidences segmented from the transmission source alone for AC. The study suggests that sTX-MLAA will produce PET images on PET/MR with quantification comparable to PET/CT results during human cardiac exams.

Abstract:Recently, our group proposed a self-collimating SPECT system with multi-layer interspaced mosaic detectors to achieve high resolution and sensitivity simultaneously. In this study, we investigated the effect of attenuation on image quality and applied an accurate attenuation correction during reconstruction for self-collimating SPECT based on a half-ring and a full-ring system, respectively. The attenuation factor was calculated using a fast ray-tracing algorithm. The attenuation correction was performed during OSEM reconstruction by multiplying the attenuation factor by the system matrix. Noise-free and noisy analytical simulations were performed using an XCAT phantom with 1 slice. GATE simulations were performed using an XCAT phantom with 20 slices. The reconstructed image quality was evaluated quantitatively using the absolute relative bias between the reconstructed images and the phantom image. The simulation results showed that attenuation correction greatly improved the image quality qualitatively and quantitatively. The results also showed that the full-ring system was preferable than the half-ring system to avoid attenuation-caused artifact when statistical noise was introduced. The GATE simulation results of the full-ring system showed that the absolute quantification biases were reduced after attenuation correction from 82.0% to 4.2%, from 87.7% to 7.7%, from 24.4% to 2.2%, from 95.0% to 9.5% for 4 ROIs, respectively. In conclusion, attenuation correction is crucial for accurate quantification of the self-collimating SPECT images and the full-ring system is preferable than the half-ring system when both statistical noise and attenuation effect exist in practical applications.

Abstract:Recent developments in artificial intelligence technology have enabled new developments that can improve attenuation and scatter correction in PET and SPECT. These technologies will enable the use of accurate and quantitative imaging without the need to acquire a CT image, greatly expanding the capability of PET/MRI, PET-only, and SPECT-only scanners. The use of AI to aid in scatter correction will lead to improvements in image reconstruction speed, and improve patient throughput. This paper outlines the use of these new tools, surveys contemporary implementation, and discusses their limitations.

Abstract:Using simulation and patient studies, we compare the lesion detection in whole-body PET oncology with and without performing attenuation correction (AC and non-AC). In addition, we discuss the prediction of lesion distortion in non-AC PET images. In all of the simulation studies, the OSEM algorithm is used to reconstruct 2D noise-free data. Lesions of different size, density, and uptake are simulated in the lungs. Lesions with the density of soft tissue and different uptake are simulated in the mediastinum and abdomen. The patient studies are performed using an ADAC dual-head hybrid SPECT-PET system. The lesion-to-background ratio (LBR) in the reconstructed images is used for quantitative analyses. Simulation studies and a total of 40 patient studies show that the LBR in AC images is higher in lungs but lower in the mediastinum and abdomen than in non-AC images. In addition, simulation studies show that, in non-AC images, for given lesion and lung uptakes, the LBR increases when the lesion density decreases and spherical lesions appear to be elongated in the direction with minimal attenuation and squeezed in the direction with maximal attenuation.

Abstract:This work was part of the feasibility study of performing simultaneous PET and SPECT imaging with a microPET scanner. SPECT data were acquired with slit-slat collimation by inserting a slit-slat collimator into microPET's detector ring but were contaminated by 511 keV photons' down-scattered events from PET tracer. In this study, contribution of down-scattered events in Tc-99m energy window (135-145 keV) was characterized through Monte Carlo simulation and measurements. With relative ratio of Tc-99m and F-18 activity being 1:1, ratio of recorded events from Tc-99m and F-18 source was 1:18.31. Nevertheless, simulation results revealed that the majority of down-scattering effects occurred in the detector crystals and thus were less object dependent. We investigated two subtraction-based compensation methods. The first method used an auxiliary energy window (AEW) and the second method employed extra shielding measurement (ESM) to acquire down-scatter- only data. Simulated and experimental studies showed improved image quality after applying the correction methods. With the activity ratio of SPECT tracer to PET tracer being 5:1, good SPECT images were obtained after correction, which were comparable to those from pure SPECT imaging.
