Abstract:To study the characteristics of common myocardial tomography artifacts so as to increase the diagnosis accuracy, 132 patients with myocardial perfusion were reviewed. With careful recognition of artifacts, the patients were re-diagnosed and compared with previous results. It was found that attenuation artifacts and motion artifacts were found frequently in thallium SPECT images. Artifacts caused by thoracic wall were frequent in female and showed a fixed defect in anterior or lateral wall; attenuation artifacts caused by left hemidiaphragm were always found in male and showed that the inferior wall became thinner gradually from apex to bottom in vertical long axis slices. The coexistence of defects and hot areas was the characteristics of motion artifacts. Artifacts caused by non-target organs, liver and/or gastrointestinal tracts, were frequently seen in 99mTc- sestamibi SPECT image. By recognizing the artifacts, misdiagnosis rate decreased significantly from 13.6% to 6.8%. Our studies show that all kinds of artifacts have their prevalence rate and imaging feature, the artifacts could be eliminated effectively by careful analysis and some correction measures.

Abstract:Following a moving hot spot in the projections of raw images and profound perfusion defects in myocardial perfusion single-photon emission computed tomography (SPECT) imaging of a patient, a hypothesis was postulated that the perfusion defects were artifactual, and the high activity concentration of the gallbladder may be a culprit for this phenomenon, owing to flawed event positioning function of the gamma camera due to a malfunctioning digital event processor electronics board. To depict the characteristics of this artifact, a point source containing an activity of 3 mCi of pertechnetate is placed on the scanning table with the detector facing the table (at a distance of 30 cm), and then, in other detector positions and 1-min static images are acquired accordingly. The ratio is calculated as follows: count of the artifactual focus: 1860, count of the index focus: 705,727, and artifactual-to-index focus ratio: 0.003. In testing the uniformity of gamma camera based on the National Electrical Manufacturers Association protocol, a nonuniform response was detected, seemingly, a smaller field of view (FOV) is reproduced in the main FOV causing nonuniformity more than the acceptable level. The smaller flood image lies in the upper right corner of the main flood image. In essence, the extremely bright gallbladder was the source of error, and its image was reproduced in the FOV, which was superimposed on the left ventricular myocardium in some of the projections and was propagated to SPECT images.

Abstract:Cardiac motion and partial volume effects (PVE) are two of the main causes of image degradation in cardiac PET. Motion generates artifacts and blurring while PVE lead to erroneous myocardial activity measurements. Newly available simultaneous PET-MR scanners offer new possibilities in cardiac imaging as MRI can assess wall contractility while collecting PET perfusion data. In this perspective, we develop a list-mode iterative reconstruction framework incorporating both tagged-MR derived non-rigid myocardial wall motion and position dependent detector point spread function (PSF) directly into the PET system matrix. In this manner, our algorithm performs both motion 'deblurring' and PSF deconvolution while reconstructing images with all available PET counts. The proposed methods are evaluated in a beating non-rigid cardiac phantom whose hot myocardial compartment contains small transmural and non-transmural cold defects. In order to accelerate imaging time, we investigate collecting full and half k-space tagged MR data to obtain tagged volumes that are registered using non-rigid B-spline registration to yield wall motion information. Our experimental results show that tagged-MR based motion correction yielded an improvement in defect/myocardium contrast recovery of 34-206% as compared to motion uncorrected studies. Likewise, lesion detectability improved by respectively 115-136% and 62-235% with MR-based motion compensation as compared to gating and no motion correction and made it possible to distinguish non-transmural from transmural defects, which has clinical significance given the inherent limitations of current single modality imaging in identifying the amount of residual ischemia. The incorporation of PSF modeling within the framework of MR-based motion compensation significantly improved defect/myocardium contrast recovery (5.1-8.5%, p < 0.01) and defect detectability (39-56%, p < 0.01). No statistical difference was found in PET contrast and lesion detectability based on motion fields obtained with half and full k-space tagged data.

Abstract:While drastic improvements in CT technology have occurred in the past 25 years, spatial resolution is one area where progress has been limited until recently. New photon counting CT systems, are capable of much better spatial resolution than their (energy integrating) predecessors. These improvements have the potential to improve the evaluation obstructive coronary artery disease by enabling more accurate delineation between calcified plaque and coronary vessel lumen. A new set of vessel phantoms has been designed and manufactured for quantifying this improvement. Comparisons are made between an existing clinical CT system, a prototype photon counting system, with images from a micro CT system being used as the gold standard. Scans were made of the same objects on all three systems. The resulting images were registered and the luminal cross section areas were compared. Luminal cross-sections near calcified plaques were reduced due to blooming, but this effect was much less pronounced in images from the prototype photon counting system as compared to the images from the clinical CT system.

Abstract:The combination of Technetium-99m-methoxyisobutylisonitrile (Tc-99m-MIBI) single-photon emission computed tomography (SPECT) and Fluorine-18-fluorodeoxyglucose (F-18-FDG) Positron Emission Tomography/Computed Tomography (PET/CT) for myocardial perfusion/metabolic imaging is commonly used to assess myocardial viability, guide treatment strategies, and predict outcomes in patients with ischemic heart failure. [ 1 , 2 ] Hibernating myocardium (HM) is a chronic and potentially reversible myocardial dysfunction caused by ischemia. In myocardial viability imaging, perfusion-metabolism mismatch is considered a characteristic feature of HM, whereas a decrease or lack of perfusion/metabolism match indicates the presence of infarct scar tissue. [ 3 , 4 ] Research evidence suggests that the extent of functional improvement in the heart after revascularization is positively correlated with the proportion of hibernating myocardium. [ 5 ] Hence, the presence and proportion of HM and scars are critical for determining the suitability for revascularization, where more HM and fewer scars correlate with a better potential for recovery and prognosis. Therefore, an accurate assessment of HM and scars is crucial for clinical decision-making. Clinical studies [ 6 ] have shown that timely coronary revascularization to restore blood flow to the HM can improve the symptoms and prognosis of patients with ischemic heart failure. While PET/CT myocardial perfusion imaging (MPI) generally provides high-quality imaging with reduced impact from soft tissue attenuation, making it ideal for evaluating myocardial blood flow, it is not entirely free from attenuation effects. Moreover, its widespread clinical application is limited by factors such as the availability of PET/CT facilities, accessibility, and the cost of positron-emitting radiopharmaceuticals. Consequently, most clinical practice studies [ 7–9 ] have employed SPECT MPI. However, SPECT MPI is known to suffer from attenuation artifacts caused by uneven absorption of low-energy gamma photons by the breast, diaphragm, and chest wall muscles, which can lead to false positives or exacerbate perceived reductions in perfusion. [ 10 , 11 ] Our search revealed that some large-scale multicenter clinical studies, such as the STICH trial, [ 12 ] used SPECT MPI without combining it with PET/CT metabolic imaging or applying attenuation correction (AC). Furthermore, recent studies [ 8 , 13–18 ] assessing myocardial viability using SPECT MPI combined with F-18-FDG PET/CT did not analyze AC-corrected SPECT data. In patients with ischemic heart failure, the enlarged heart could further exacerbate the range and severity of attenuation artifacts, potentially leading to greater misestimation of perfusion deficits, which could, in turn, affect the calculation results of HM, Scar, and other normal myocardial areas when combined with PET/CT metabolic imaging, thereby affecting the accuracy of myocardial viability and type calculations.

Quantitative assessment of motion effects in dual-source dual energy CT and dual-source photon-counting detector CT

Abstract:Conventional dual-source CT scanners can be used to either provide better temporal resolution or dual-energy imaging, but not both at the same time. This presents a dilemma in cardiac CT as both high temporal resolution and multi-energy imaging are desirable. The current study evaluated a dual-source photon-counting-detector (DS-PCD) CT which can acquire multi-energy images at high temporal resolution. A cardiac motion phantom with a 3-mm diameter iodinated rod, mimicking the right coronary artery, was scanned 25 times using a DS-PCD CT (66 ms resolution) and a dual-source dual-energy (DS-DE, 125 ms resolution) CT. Low/high energy images and iodine maps were reconstructed at 40% and 75% cardiac phases. To quantify the impact of motion on image quality, dice similarity coefficient was computed between the low/high energy images while the circularity and effective diameter of the iodinated rod were computed on the iodine maps. The dice coefficients were higher for DS-PCD with a mean of 0.89 and 0.91 at the 40% and 70% phases, while DS-DE had a lower mean of 0.20 and 0.78, respectively. The circularity was excellent for DS-PCD with a mean of 0.97 and 0.98 at the 40% and 75% phases, while DS-DE had a mean of 0.71 and 0.98, respectively. The effective diameter was accurate for DS-PCD with a mean of 2.9 mm (true size of 3 mm) at both phases, while DS-DE had a mean of 4.0 mm and 3.2 mm at the 40% and 75% phases, respectively. These results indicate that DS-PCD CT enables simultaneous high temporal resolution and multi-energy cardiac imaging with minimal motion artifacts.

Abstract:Purpose Photon counting CT (PCCT) holds promise for mitigating metal artifacts and can produce virtual mono‐energetic images (VMI), while maintaining temporal resolution, making it a valuable tool for characterizing the heart. This study aimed to evaluate and optimize PCCT for cardiac imaging in patients during left ventricular assistance device (LVAD) therapy by conducting an in‐depth objective assessment of metal artifacts and visual grading. Methods Various scan and reconstruction settings were tested on a phantom and further evaluated on a patient acquisition to identify the optimal protocol settings. The phantom comprised an empty thoracic cavity, supplemented with heart and lungs from a cadaveric lamb. The heart was implanted with an LVAD (HeartMate 3) and iodine contrast. Scans were performed on a PCCT (NAEOTOM Alpha, Siemens Healthcare). Metal artifacts were assessed by three objective methods: Hounsfield units (HU)/SD measurements (DiffHU and SDARTIFACT), Fourier analysis (AmplitudeLowFreq), and depicted LVAD volume in the images (BloomVol). Radiologists graded metal artifacts and the diagnostic interpretability in the LVAD lumen, cardiac tissue, lung tissue, and spinal cord using a 5‐point rating scale. Regression and correlation analysis were conducted to determine the assessment method most closely associated with acquisition and reconstruction parameters, as well as the objective method demonstrating the highest correlation with visual grading. Results Due to blooming artifacts, the LVAD volume fluctuated between 27.0 and 92.7 cm3. This variance was primarily influenced by kVp, kernel, keV, and iMAR (R2 = 0.989). Radiologists favored pacemaker iMAR, 3 mm slice thickness, and T3D keV and kernel Bv56f for minimal metal artifacts in cardiac tissue assessment, and 110 keV and Qr40f for lung tissue interpretation. The model adequacy for DiffHU SDARTIFACT, AmplitueLowFreq, and BloomVol was 0.28, 0.76, 0.29, and 0.99 respectively for phantom data, and 0.95, 0.98, 1.00, and 0.99 for in‐vivo data. For in‐vivo data, the correlation between visual grading (VGSUM) and DiffHU SDARTIFACT, AmplitueLowFreq, and BloomVol was −0.16, −0.01, −0.48, and −0.40 respectively. Conclusion We found that optimal scan settings for LVAD imaging involved using 120 kVp and IQ level 80. Employing T3D with pacemaker iMAR, the sharpest allowed vascular kernel (Bv56f), and VMI at 110 keV with kernel Qr40 yields images suitable for cardiac imaging during LVAD‐therapy. Volumetric measurements of the LVAD for determination of the extent of blooming artifacts was shown to be the best objective method to assess metal artifacts.

Quantitative evaluation of beam-hardening artefact correction in dual-energy CT myocardial perfusion imaging

Abstract:Objectives: To assess quantitatively the impact of a novel reconstruction algorithm ("kernel") with beam-hardening correction (BHC) on beam-hardening artefacts of the myocardium at dual-energy CT myocardial perfusion imaging (DE-CTMPI). Methods: Rest-series of DE-CTMPI examinations from 14 patients were retrospectively analyzed. Six image series were reconstructed for each patient: a) 100 kV, b) 140 kV, and c) linearly blended MIX0.5, each with BHC (D33f kernel) and without (D30f kernel). Seven hundred and fifty-six myocardial regions were assessed. Seven equal regions of interest divided the myocardium in the axial section. Three subdivisions were created within these regions in areas prone to BHA. Reports of SPECT studies performed within 30 days of CT examination were used to confirm the presence and location of true perfusion defects. Paired student t-test was used for statistical evaluation. Results: Overall mean myocardial attenuation was lower using BHC (D30f: 87.3 ± 24.1 HU; D33f: 85.5 ± 21.5 HU; p = 0.009). Overall relative difference from average myocardial attenuation (RDMA) was more homogeneous using BHC (D30f: -0.3 ± 11.4 %; D33f: 0.1 ± 10.1 %; p < 0.001). Changes in RDMA were greatest in the posterobasal myocardium (D30f: -16.2 ± 10.0 %; D33f: 3.4 ± 10.7 %; p < 0.001). Conclusions: A dedicated reconstruction algorithm with BHC can significantly reduce beam-hardening artefacts in DE-CTMPI. Key points: • Beam-hardening artefacts (BHA) cause interference with attenuation-based CT myocardial perfusion assessment (CTMPI). • BHA occur mostly in the posterobasal left ventricular wall. • Beam-hardening correction homogenized and decreased mean myocardial attenuation. • BHC can help avoid false-positive findings and increase specificity of static CTMPI.

Abstract:Quantitative PET attenuation correction (AC) for combined cardiac PET/MR is a challenging problem. We propose and evaluate an AC approach that uses coincidences from a relatively weak and physically fixed sparse external source, in combination with that from the patient, to correct for PET attenuation based on physics principles alone. The low 30 ml volume of the source makes it easy to fill and place, and the method does not use prior image data or attenuation map assumptions. Our supplemental transmission aided maximum likelihood reconstruction of attenuation and activity (sTX-MLAA) algorithm contains an attenuation map update that maximizes the likelihood of terms representing coincidences originating from tracer in the patient and a weighted expression of counts segmented from the external source alone. Both external source and patient scatter and randoms are fully corrected. We evaluated performance of sTX-MLAA compared to reference standard CT-based AC with FDG PET/CT phantom studies; including modeling a patient with myocardial inflammation. Through an ROI analysis we measured less than 5% bias in activity concentrations for PET images generated with sTX-MLAA relative to CT-AC. PET background variability (from noise and sparse sampling) was substantially reduced with sTX-MLAA compared to using coincidences segmented from the transmission source alone for AC. The study suggests that sTX-MLAA will produce PET images on PET/MR with quantification comparable to PET/CT results during human cardiac exams.
Myocardial Characterization with Extracellular Volume Mapping with a First-Generation Photon-counting Detector CT with MRI Reference

Abstract:Background Photon-counting detector (PCD) CT provides comprehensive spectral data with every acquisition, but studies evaluating myocardial extracellular volume (ECV) quantification with use of PCD CT compared with an MRI reference remain lacking. Purpose To compare ECV quantification for myocardial tissue characterization between a first-generation PCD CT system and cardiac MRI. Materials and Methods In this single-center prospective study, adults without contraindication to iodine-based contrast media underwent same-day cardiac PCD CT and MRI with native and postcontrast T1 mapping and late gadolinium enhancement for various clinical indications for cardiac MRI (the reference standard) between July 2021 and January 2022. Global and midventricular ECV were assessed with use of three methods: single-energy PCD CT, dual-energy PCD CT, and MRI T1 mapping. Quantitative comparisons among all techniques were performed. Correlation and reliability between different methods of ECV quantification were assessed with use of the Pearson correlation coefficient (r) and the intraclass correlation coefficient. Results The final sample included 29 study participants (mean age ± SD, 54 years ± 17; 15 men). There was a strong correlation of ECV between dual- and single-energy PCD CT (r = 0.91, P < .001). Radiation dose was 40% lower with dual-energy versus single-energy PCD CT (volume CT dose index, 10.1 mGy vs 16.8 mGy, respectively; P < .001). In comparison with MRI, dual-energy PCD CT showed strong correlation (r = 0.82 and 0.91, both P < .001) and good to excellent reliability (intraclass correlation coefficients, 0.81 and 0.90) for midventricular and global ECV quantification, but it overestimated ECV by approximately 2%. Single-energy PCD CT showed similar relationship with MRI but underestimated ECV by 3%. Conclusion Myocardial tissue characterization with photon-counting detector CT-based quantitative extracellular volume analysis showed a strong correlation to MRI. © RSNA, 2023 Supplemental material is available for this article.
Post-reconstruction Attenuation Correction for SPECT Myocardium Perfusion Imaging Facilitated by Deep Learning-Based Attenuation Map Generation

Abstract:Attenuation correction can improve the quantitative accuracy of single-photon emission computed tomography (SPECT) images. Existing SPECT-only systems normally can only provide non-attenuation corrected (NC) images which are susceptible to attenuation artifacts. In this work, we developed a post-reconstruction attenuation correction (PRAC) approach facilitated by a deep learning-based attenuation map for myocardial perfusion SPECT imaging. In the PRAC method, new projection data were estimated via forwardly projecting the scanner-generated NC image. Then an attenuation map, generated from NC image using a pretrained deep learning (DL) convolutional neural network, was incorporated into an offline reconstruction algorithm to obtain the attenuation-corrected images from the forwardly projected projections. We evaluated the PRAC method using 30 subjects with a DL network trained with 40 subjects, using the vendor-generated AC images and CT-based attenuation maps as the ground truth. The PRAC methods using DL-generated and CT-based attenuation maps were both highly consistent with the scanner-generated AC image. The globally normalized mean absolute errors were 1.1% ± .6% and .7% ± .4% and the localized absolute percentage errors were 8.9% ± 13.4% and 7.8% ± 11.4% in the left ventricular (LV) blood pool, respectively, and − 1.3% ± 8.0% and − 3.8% ± 4.5% in the LV myocardium for PRAC methods using DL-generated and CT-based attenuation maps, respectively. The summed stress scores after PRAC using both attenuation maps were more consistent with the ground truth than those of the NC images. We developed a PRAC approach facilitated by deep learning-based attenuation maps for SPECT myocardial perfusion imaging. It may be feasible for this approach to provide AC images for SPECT-only scanner data.
Visual patterns of breast attenuation artifacts in women and men with an upright and supine cadmiun-zinc-telluride camera

Abstract:Background: Breast attenuation artifacts occurring with upright cadmium-zinc-telluride (CZT) cardiac imaging systems have not been well characterized. Methods: 216 consecutive patients with Single Photon Emission Computerized Tomography myocardial perfusion imaging and no angiographically significant obstructive coronary artery disease were identified. All upright and supine SPECT images as well as coronary angiograms were reviewed and analyzed in blinded fashion. Results: In women imaged upright, more visual false positive defects were noted in the inferior wall compared to the anterior wall (26 vs. 10 at rest, p = 0.006, and 33 vs. 13 at stress, p < 0.001). Visual inferior wall defects were more common in the upright than supine position at stress (33 vs. 23, p = 0.018) and rest (26 vs. 14, p = 0.011), and most apparent in non-obese women (13 vs. 8, at stress, p = 0.059 and 11 vs. 5, at rest, p = 0.014). Conclusions: With upright CZT myocardial perfusion imaging, women often have visible inferior wall attenuation artifact defects, likely from pendant breast tissue. These inferior wall attenuation artifacts may be seen in non-obese female patients.
Implementation of a Tc-99m and Ce-139 scanning line source for attenuation correction in SPECT using a dual opposing detector scintillation camera

Abstract:Image degradation during single photon emission computed tomography (SPECT) due to attenuation and Compton scatter of photons can cause clinical image artifacts and will also result in inaccurate quantitative data. Therefore attenuation correction methods recently received wide interest. Transmission imaging can be performed to obtain the attenuation coefficients of a nonhomogeneous attenuating medium accurately. The aim of this study was firstly to evaluate the imaging characteristics of the scanning line source assembly. The results obtained with Tc-99m and Ce-139 were compared. Secondly the calculated attenuation coefficients were compared with known values from literature, using Tc-99m and Ce-139 as transmission sources. Lastly the method of acquiring simultaneous transmission and emission data was investigated. This study shows that an attenuation coefficient map can be obtained using a scanning line source for transmission imaging with a dual opposing detector camera. The imaging characteristics of Tc-99m and Ce-139 as transmission sources are similar. The resolution obtained with the Ce-139 line source was poorer than that obtained with the Tc-99m line source. A linear relationship was found between CT numbers and attenuation coefficients for transmission images using both Tc-99m and Ce-139 line sources. The attenuation coefficient value for water was underestimated by 1% using the Tc-99m transmission source and underestimated by 10% using Ce-139 as transmission source. This underestimation of attenuation coefficient values was also obtained in the human study. A myocardial perfusion study processed without and with attenuation correction clearly demonstrated the effect of the attenuation correction in the inferior myocardial region. The potential of using a scanning line source as transmission source with a dual opposing detector camera has been demonstrated in this study. The transmission source, Ce-139 was successfully introduced in this investigation for simultaneous acquisition of transmission and emission data.
The Experiment Study of Coronary Artery Imaging by 64 Slice Multidetector Computed Tomography Angiography with Pulsating Cardiac Phantom

Abstract:Objective To evaluate the diagnostic accuracy and reliability for detection of coronary artery stenosis and restenosis in stent using 64 slice multidetector computed tomography angiography(MDCTA).Materials and Methods A pulsating cardiac phantom(GE China CT Lab) with three simulated coronary arteries was scanned on 64 slice MDCT(GE Light Speed VCT) at static state and 3 different sinus rhythms of 50、70、90 beats per minute(bpm).Two simulated arteries were 3 mm and 5 mm respectively in lumen diameter with stenoses of 25%、50%、75%.A stent with stenoses of 50%、75% was placed inside the another artery with 4 mm in lumen diameter.Images were analyzed on the GE AW4.2 workstation and the results were compared with those true dimensions of simulated arteries.Results(1) The average measured values of the 25%、50%、75% stenoses and luminal diameters for the 3 mm and 5 mm simulated arteries were(0.61±0.02) mm、(1.11±0.06) mm、(1.68±0.05) mm、(2.28±0.06) mm(P values of 0.001、0.001、0.000、0.000 with one sample t test) and(1.14±0.04) mm,(2.30±0.04)mm,(3.50±0.04) mm,(4.71±0.04) mm(P values of 0.010、0.003、0.001、0.001 with one sample t test) respectively;The average measured percentage values of the 25%、50%、75% stenoses were(26.8±0.6)%、(48.6±0.7)%、(73.8±0.9)%(P values of 0.009、0.145、0.075 with one sample t test) and(24.1±0.6)%、(48.9±0.8)%、(74.4±0.7)%(P values of 0.066、0.071、0.139 with one-sample t test).(2) The measured values of the 3 mm diameter simulated artery were more susceptible to high heart rate with artifact,especially when they were more than 70 bpm.(3) 64 slice VCT had a potential but limited capability to depict in stent restenosis due to the artifacts caused mainly by high density metal stent and motions at high heart rate;for the same reason the diameter of lumen at stent segment was less than that of normal segment,which was statistical significant.(4) Measurements revealed significantly better results on the gantry rotation time of 0.42 s compared to that of 0.35 s(P=0.036) at heart rate of 90 bpm.Conclusion 64 slices VCT is reliable in detecting coronary artery stenosis,it is limited to depict in stent restenosis,VCT is promising in the cases when vessel caliber is relatively large and heart rate is relatively low.Improving the spatial resolution through software kernels or hardware can increase the accuracy in measuring in stent restenosis.Making fully usage of matched scan parameters can also do much help to imaging quality.
“Virtual” attenuation correction: improving stress myocardial perfusion SPECT imaging using deep learning

Abstract:PurposeMyocardial perfusion imaging (MPI) using single-photon emission computed tomography (SPECT) is widely used for coronary artery disease (CAD) evaluation. Although attenuation correction is recommended to diminish image artifacts and improve diagnostic accuracy, approximately 3/4ths of clinical MPI worldwide remains non-attenuation-corrected (NAC). In this work, we propose a novel deep learning (DL) algorithm to provide "virtual" DL attenuation–corrected (DLAC) perfusion polar maps solely from NAC data without concurrent computed tomography (CT) imaging or additional scans.MethodsSPECT MPI studies (N = 11,532) with paired NAC and CTAC images were retrospectively identified. A convolutional neural network–based DL algorithm was developed and trained on half of the population to predict DLAC polar maps from NAC polar maps. Total perfusion deficit (TPD) was evaluated for all polar maps. TPDs from NAC and DLAC polar maps were compared to CTAC TPDs in linear regression analysis. Moreover, receiver-operating characteristic analysis was performed on NAC, CTAC, and DLAC TPDs to predict obstructive CAD as diagnosed from invasive coronary angiography.ResultsDLAC TPDs exhibited significantly improved linear correlation (p < 0.001) with CTAC (R2 = 0.85) compared to NAC vs. CTAC (R2 = 0.68). The diagnostic performance of TPD was also improved with DLAC compared to NAC with an area under the curve (AUC) of 0.827 vs. 0.780 (p = 0.012) with no statistically significant difference between AUC for CTAC and DLAC. At 88% sensitivity, specificity was improved by 18.9% for DLAC and 25.6% for CTAC.ConclusionsThe proposed DL algorithm provided attenuation correction comparable to CTAC without the need for additional scans. Compared to conventional NAC perfusion imaging, DLAC significantly improved diagnostic accuracy.

Abstract:ABSTRACT: Noninvasive cardiac imaging has rapidly evolved during the last decade owing to improvements in computed tomography (CT)-based technologies, among which we highlight the recent introduction of the first clinical photon-counting detector CT (PCD-CT) system. Multiple advantages of PCD-CT have been demonstrated, including increased spatial resolution, decreased electronic noise, and reduced radiation exposure, which may further improve diagnostics and may potentially impact existing management pathways. The benefits that can be obtained from the initial experiences with PCD-CT are promising. The implementation of this technology in cardiovascular imaging allows for the quantification of coronary calcium, myocardial extracellular volume, myocardial radiomics features, epicardial and pericoronary adipose tissue, and the qualitative assessment of coronary plaques and stents. This review aims to discuss these major applications of PCD-CT with a focus on cardiac and myocardial characterization.

Abstract:Background: Soft-tissue attenuation remains a major limitation of SPECT-MPI which interferes with the diagnosis of CAD. The current study aims to evaluate the pattern of attenuation artifacts in supine and semi-reclining positions on CZT cardiac camera and their interaction with gender, BMI and stress protocols. Methods: We prospectively analysed 150 patients acquired in supine and semi-reclining positions on CZT camera. The images were evaluated for severity and extent of defect using 17-segment model. An additional CT scan was acquired to generate AC image in the first 50 patients studied to assist investigator learning for comparison of artifact vs true defects in the two SPECT systems. The defects present in one position or showing change in severity within two positions were considered as positional artifacts and further validated using CTAC supine image. Results: In overall analysis, higher extent and severity of positional artifacts were observed more in semi-reclining position affecting the apex, apico-inferior, inferolateral and inferoseptal segments. Females showed more positional artifacts than males with inferior wall attenuation in the semireclining position and anterior wall attenuation in the supine position. A positive correlation of the extent and severity of positional artifacts was noted with an increasing BMI. In patients with BMI > 30, mid inferior and inferolateral segments were most affected followed by anterior wall segments. Highest correction of artifactual perfusion defects by CTAC was noted in inferior wall followed by inferolateral segments. Conclusion: The incidence of positional artifacts was greater in semi-reclining position in females, higher BMI groups and adenosine stress subsets. Knowledge of the pattern of positional artifacts appears to be a reliable alternative of CTAC for correct interpretation of myocardial perfusion images.
