Abstract:Real-time polymerase chain reaction (PCR) is a highly sensitive method that can be used for the detection and quantification of microbial populations without cultivating them in anaerobic processes and environmental samples. This work was conducted to design primer and probe sets for the detection of methanogens using a real-time PCR with the TaqMan system. Six group-specific methanogenic primer and probe sets were designed. These sets separately detect four orders (Methanococcales, Methanobacteriales, Methanomicrobiales, and Methanosarcinales) along with two families (Methanosarcinaceae and Methanosaetaceae) of the order Methanosarcinales. We also designed the universal primer and probe sets that specifically detect the 16S rDNA of prokaryotes and of the domain Bacteria and Archaea, and which are fully compatible with the TaqMan real-time PCR system. Target-group specificity of each primer and probe set was empirically verified by testing DNA isolated from 28 archaeal cultures and by analyzing potential false results. In general, each primer and probe set was very specific to the target group. The primer and probe sets designed in this study can be used to detect and quantify the order-level (family-level in the case of Methanosarcinales) methanogenic groups in anaerobic biological processes and various environments.

Abstract:Aim: To explore whether propofol could in vivo, suppress intestinal permeability and proinflammatory cytokines in the intestine after traumatic brain injury (TBI). Methods: The rats were randomly divided into four groups including control group with right parietal bone window alone and no brain injury, TBI group, TBI + propofol (5 mg/kg/h), and propofol alone group. All animals were killed 72 h later. Plasma endotoxin was assayed by the chromogenic limulus amebocyte lysate test. Intestinal permeability was quantified using the lactulose/mannitol (L/M) test. TNF-α and IL-6 in the intestine were measured using commercially available enzyme-linked immunoassay Kits. Nuclear factor kappa B (NF-κB) was analyzed by Electrophoretic mobility shift assay (EMSA). Results: No rats died before sacrifice. Plasma endotoxin, L/M ratio, intestinal level of NF-κB, TNF-α, and IL-6 remained at baseline level in controlled animals. TBI promoted a significant increment in plasma endotoxin, L/M ratio, intestinal level of NF-κB, TNF-α, and IL-6. Propofol inhibited those parameters at the dose of 5 mg/kg/h. Conclusion: Propofol has the protective effect on the intestine following traumatic brain injury in rats.
Cystatin activity in gingival crevicular fluid from periodontal disease patients measured by a new quantitative analysis method.

Abstract:Cystatins are protein inhibitors of cysteine proteinases, which are believed to play an important role in the pathogenesis of periodontal disease. In this study, we report a new sensitive method for the quantitative analysis of cystatin activity in a small amount of crude sample such as gingival crevicular fluid. Cystatin activity in the crude sample was determined by using active site-titrated papain, which is a cysteine proteinase from the plant Carica papaya. Crude samples usually contain endogenous cysteine proteinases. These competed with the added papain for the active sites of the cystatins. The cystatin-cysteine proteinase complex was able to be dissociated by the addition of papain. This competition and dissociation could interfere with the determination of cystatin activity, since some of the cysteine proteinases, such as cathepsin B, hydrolyzed the specific substrate for papain during titration with the papain. In order to exclude this interference and measure total cystatin activity, the crude sample must be alkalinized (pH 11.0) for 5 min at 4 degrees C followed by 10 min at 40 degrees C before titration with papain. The minimum detectable amount of cystatins was 20 fmol/assay when it was calculated per mole of papain inhibitory sites. Using this method, significant levels of cystatin activity were detected in all the samples of gingival crevicular fluid taken from periodontal disease patients. These results suggest that cystatins could regulate the cysteine proteinases in gingival crevicular fluid and that this new method could be useful to clarify the role of cystatins in the pathogenesis of periodontal disease.

Abstract:Due to the progress that has been made in intensive care, more and more patients survive shock. It is a clinical observation that their condition improves substantially with the restoration of intestinal function. Different experimental models have been developed to investigate the pathophysiology of the intestine in shock. As noxious periods, both the phase of ischemia and the phase of reperfusion have been identified. Since the experimental models are methodologically very different, the results of the various studies may only be compared with reservations. Nevertheless, it can be stated that reduced intestinal blood flow leads to ischemia and hypoxia of the villous tips. Reperfusion may lead to further mucosal injury. During this period oxygen free radicals and their derivates seem to play an essential role. Xanthine oxidase is thought to be the major source of these radicals in the small intestine. During ischemia ATP is catabolized to hypoxanthine, which is enzymatically transformed to xanthine, a process generating oxygen free radicals. Moreover oxygen free radicals seem to be produced by activated neutrophilic granulocytes. At present, only hypotheses exist concerning the interactions between granulocytes and these radicals. The mechanism of injury produced by oxygen free radicals is based on the peroxidation of the lipid components of the cellular membrane system. The small intestine represents a very vulnerable shock organ: apart from its very important nutritive functions, it provides the necessary barrier between the intestinal pool of endotoxin and the circulation. The loss of these vital functions due to ischemic lesions dramatically worsens the chances for the patient to survive. Therefore, it is necessary to develop therapeutic principles to maintain or restore intestinal function in shock.

Abstract:In clinical practice, intestinal autologous diseases, ailments and organ transplants can cause severe congestive damage to the intestinal tract. However, after the etiological factor is gotten rid of and blood flow is free without any hinderance, further damage to the intestinal wall often occurs, causing other related organ dysfunctions. This ultimately results in intestinal congestion reperfusion injury (ICRI). When the structure and function of the intestine are destroyed, bacteria, metabolites and endotoxins in the intestinal tract perfuse and enter the portal vein through the already compromised intestinal mucosa, to the other organs via the liver. Nevertheless, this gives rise to further aggravation of the injury, and reperfusion injury syndrome occurs. ICRI is a very common complication encountered by clinicians, and its harm is more severe and serious as compared with that caused by ischemia-reperfusion. Quite a few number of studies on ICRI have been reported to date. The exact mechanism of the injury is still idiopathic, and effective treatment strategies are still limited. Based on recent studies, this article is aimed at reviewing the destruction, damage mechanisms resulting from ICRI to the intestinal anatomical sites and distant organs. It is geared towards providing new ideas for the prevention and therapeutic approaches of ICRI.
REPLY TO ANGELO : CLIMATE CHANGE AND SPECIES LOSS IN

Abstract:Our research has identified that climate change has been responsible for shaping the pattern of species loss in Concord, Massachusetts, USA (Willi s et al. 2008). In particular, we identified that closely related species (i.e., clades) that are less phenologically responsive to both longand shortterm changes in climate have declined in abundance. Phenologic al change has long been known to be a key indicator of species response to climate change (F itt r & Fitter 2002; Walther et al. 2002; Parmesan & Yohe 2003; Visser & Both 2005) and is believed to ha ve important consequences for the success of species (Cleland et al. 2012; Cahill et al. 2013) . In a recent critique of our work, Angelo (2014) called into question the main conclusion of our study on tw grounds: i) our estimates of species loss in Concord are inaccurate, and ii) it is unlikely that climate change has significantly impacted species loss given the myriad of additional facto rs hat effect species in suburban communities like Concord (e.g., reforestation, land-use c hange, non-native species, and increased deer herbivory). We believe that Angelo’s critique of our stud y is based on a misunderstanding of our methods and conclusions. Here, we explicitly address his crit que and reiterate that our results and conclusions are sound. Our response is focused specifically on the critique of Willis et al. (2008) because this is the study for which we performed the bulk of the analyses, interpretation, and writing.
Levels of antibodies to endotoxin and cytokine release in patients with severe trauma: does posttraumatic dysergy contribute to organ failure?

Abstract:Background: We conducted a prospective study in patients with multiple injuries investigating the time course of trauma-related changes of systemic immunologic defense mechanisms. Methods: Patients with multiple injuries with Injury Severity Scores of more than 20 were included if they survived for more than 4 days after injury. Further inclusion criteria were no local or systemic infection (pneumonia, sepsis, soft-tissue infection, acquired immunodeficiency syndrome, tuberculosis, etc.) at the time of injury and no history of liver disease, bowel disease, or abdominal surgery. Serum endotoxin levels were measured from peripheral venous blood, as were the immunoglobulin G (IgG) and immunoglobulin M (IgM) antibodies against lipid A and against the core polysaccharide of endotoxin (lipopolysaccharide [LPS]), during the course of intensive care management. Serial central venous levels of interleukin-6 were determined as a marker of the inflammatory response. Results: The patients were grouped according to their survival, with the survivors belonging to group S (48 patients) and the nonsurvivors belonging to group N (16 patients). The time of death for the nonsurvivors was between days 10 and 32 after the initial trauma. Thirteen of these patients (81%) died of multiple organ failure between days 12 and 17, two died of head trauma, and one died of sepsis. In patients who died of multiple organ failure, a significantly lower production of the IgM and IgG antibodies (AB) against lipid A and LPS was found before death (lipid A IgM-AB, day 11: group N, 29 +/- 11 U/mL; group S, 106 +/- 16 U/mL; p = 0.008; lipid A IgG-AB, day 11: group N, 18 +/- 9 U/mL; group S, 57 +/- 18 U/mL; p = 0.007; LPS IgM-AB, day 11: group N, 36 +/- 14 U/mL; group S, 122 +/- 23 U/mL; p = 0.009; LPS IgG-AB, day 11: group N, 17 +/- 12 U/mL; group S, 56 +/- 19 U/mL; p = 0.03). Interleukin-6 levels were significantly increased in the nonsurvivors (day 1: group N, 1,095 +/- 112 pg/mL; group S, 393 +/- 67 U/L; p = 0.008). Conclusion: In patients who died of severe trauma and in whom the cause of death was multiple organ failure, a significantly lower production of antiendotoxin antibodies was measured shortly before death. An insufficient immune defense (dysergy) may be involved in the pathomechanisms leading to the development of organ dysfunction.

Abstract:AIM:To investigate the intestinal barrier function damage induced by trauma and infection in rats.METHODS:Experimental models of surgical trauma and infection were established in rats. Adult Sprague-Dawley rats were divided into 4 groups: control group (n = 8), EN group (n = 10), PN group (n = 9) and Sep group (n = 8). The rats in PN and Sep groups were made into PN models that received isonitrogenous, isocaloric and isovolumic TPN solution during the 7-d period. Rats in EN and Sep groups received laparotomy and cervical catheterization on day 1 and received lipopolysaccharide injection intraperitoneally on d 7. On the 7(th) day all the animals were gavaged with lactulose and mannitol to test the intestinal permeability. Twenty-four hours later samples were collected and examined.RESULTS:The inflammatory responses became gradually aggravated from EN group to Sep group. The mucosal structure of small intestine was markedly impaired in PN and Sep groups. There was a low response in IgA level in Sep group when compared with that of EN group. Lipopolysaccharide injection also increased the nitric oxide levels in the plasma of the rats. The intestinal permeability and bacterial translocation increased significantly in Sep group compared with that of control group.CONCLUSION:One wk of parenteral nutrition causes an atrophy of the intestinal mucosa and results in a moderate inflammatory reaction in the rats. Endotoxemia aggravates the inflammatory responses that caused by laparotomy plus TPN, increases the production of nitric oxide in the body, and damages the intestinal barrier function.
Atrial Natriuretic Peptide: A Potential Early Therapy for the Prevention of Multiple Organ Dysfunction Syndrome Following Severe Trauma

Abstract:Trauma remains a tremendous medical burden partly because of increased expenditure for the management of multiple organ dysfunction syndrome (MODS) developed during hospital stay. The intestinal barrier injury continues to be a second insult resulting in MODS which currently lacks efficient strategies for prevention. Recent studies have uncovered multi-organ protective benefits of atrial natriuretic peptide (ANP) in cardiovascular disease. However, the role of ANP in the prevention of MODS following severe trauma has not been understood. In our laboratory study, 1-h infusion of exogenous ANP during hemorrhagic shock following severe trauma induced high-level expression of endogenous serum ANP after 24 h, this effect was related to the improved level of functional biomarkers in multiple organs. Such phenomenon has not been found in other laboratories. A thorough literature review consequently was performed to uncover the potential mechanisms, to appraise therapy safety, and to propose uncertainties. In severe trauma, short-term exogenous ANP therapy during hemorrhagic shock may promote sustained endogenous expression of ANP from intestinal epithelium through activating a positive feedback loop mechanism involving phospholipase C-γ1 and reactive oxygen species crosstalk. This feedback loop may prevent MODS through multiple signaling pathways. Administration of ANP during hemorrhagic shock is thought to be safe. Further studies are required to confirm our proposed mechanisms and to investigate the dose, duration, and timing of ANP therapy in severe trauma.

The model is to be validated by measurements performed at 2 GHz with the use of a remote-controlled airship in the area of Prague, the Czech Republic. The first level of the model, accounting for the trunk and main branches, is based on Physical Optics techniques applied to dielectrics and is presented in this paper and compared with one set of experimental data. The model will be completed with the inclusion of the contributions, coherent and incoherent, of the leafy canopy. As the work is still ongoing, only the preliminary results are shown.
Vibrational Spectroscopy of Gas Phase Functional Molecules and Their Complexes Cooled in Supersonic Beams

Abstract:Functional molecules and supramolecules are the assemble of molecules, which are bound by noncovalent interactions, such as dispersion force, coordinate-bonding, hydrogenbonding, etc. They exhibit special functions by forming regular high dimensional structures controlled by those weak interactions. The concept of the supramolecule was first proposed by Jean-Marie Lehn, who succeeded in synthesizing cryptand (Lehn, 1995). In the early stage, many studies have been carried out for host-guest complexes of crown ether (Gokel, 1991; Izatt et al., 1969; Pedersen, 1967; Pedersen & Frensdorff, 1972), calixarene (Atwood et al., 2002; Gutsche, 1998; Purse et al., 2005; Thallapally et al., 2005), and cyclodextrin (Brocos et al., 2010; Szejtli, 1988). These studies are extended to larger size systems built by several units, such as protein, LangmuirBlodgett (LB) film, self assembled monolayer (SAM), and liquid crystal. In addition, more complicated molecular assemblies, rotaxane, catenane, and molecular capsules, are synthesized. Also, many functional groups have been used to applications such as sensing and basic chemical research, by covalently linking fluorescent dyes, nanoparticles, proteins, DNA, and other compounds of interest. Biomolecules may also be categorized to the functional molecules. In the biomolecules, many units form high dimensional structure through noncovalent interactions and exhibit special functions which are not possible for each unit. Among many experimental studies on the functional molecules, vibrational spectroscopy is one of the most general methods. By examining the frequency shift and intensity change of the specific vibrations sensitive to the local interaction, we can study which part or site of the molecule bound for the complexation. In this sense, it is necessary to investigate vibrational spectra not only of the complexes but also of each unit under the isolated condition. By the comparison of the spectra at different conditions, we understand how the molecules change their initial structures or which conformer is preferred for the complexation. A problem for this study is that in most cases they have flexible structures so that the structures are affected by many factors, such as temperature, local environment, solvent molecules as well as the phases. These effects result in the homogenous and inhomogeneous broadening of the spectra, which leads to the difficulty to analyze the spectra. Abstract:IntroductionAbdominal distension is common in critical illness. There is a growing recognition that intra-abdominal hypertension (IAH) may complicate nonsurgical critical illness as well as after abdominal surgery. However, the pathophysiological basis of the injury to the intestinal mucosal barrier and its influence on the onset of abdominal compartment syndrome (ACS) and multiorgan dysfunction syndrome (MODS) remain unclear. We measured intestinal microcirculatory blood flow (MBF) during periods of raised intra-abdominal pressure (IAP) and examined how this influenced intestinal permeability, systemic endotoxin release, and histopathological changes.MethodsTo test different grades of IAH to the injury of intestinal mucosa, 96 New Zealand white rabbits aged 5 to 6 months were exposed to increased IAP under nitrogen pneumoperitoneum of 15 mmHg or 25 mmHg for 2, 4 or 6 hours. MBF was measured using a laser Doppler probe placed against the jejunal mucosa through a small laparotomy. Fluorescein isothiocyanate (FITC)-conjugated dextran was administered by gavage. Intestinal injury and permeability were measured using assays for serum FITC-dextran and endotoxin, respectively, after each increase in IAP. Structural injury to the intestinal mucosa at different levels of IAH was confirmed by light and transmission electron microscopy.ResultsMBF reduced from baseline by 40% when IAP was 15 mmHg for 2 hours. This doubled to 81% when IAP was 25 mmHg for 6 hours. Each indicator of intestinal injury increased significantly, proportionately with IAP elevation and exposure time. Baseline serum FITC-dextran was 9.30 (± SD 6.00) μg/ml, rising to 46.89 (±13.43) μg/ml after 15 mmHg IAP for 4 hours (P <0.01), and 284.59 (± 45.18) μg/ml after 25 mmHg IAP for 6 hours (P <0.01). Endotoxin levels showed the same pattern. After prolonged exposure to increased IAP, microscopy showed erosion and necrosis of jejunal villi, mitochondria swelling and discontinuous intracellular tight junctions.ConclusionsIntra-abdominal hypertension can significantly reduce MBF in the intestinal mucosa, increase intestinal permeability, result in endotoxemia, and lead to irreversible damage to the mitochondria and necrosis of the gut mucosa. The dysfunction of the intestinal mucosal barrier may be one of the important initial factors responsible for the onset of ACS and MODS.

Abstract:When including clinical mastitis in the breeding goal, it is useful to know what measure of the trait is most appropriate and its relationship to the primary production traits and indicator traits in the relevant population. In this paper, genetic and phenotypic parameters for clinical mastitis, somatic cell production deviance, and protein yield were estimated for the dairy breed Danish Red. In preliminary analyses, the heritability for clinical mastitis was found to be highest in early lactation, and its genetic correlation to clinical mastitis at other stages of lactation were high. Therefore, clinical mastitis defined in early lactation was the measure of clinical mastitis used in subsequent analyses. Two bivariate analyses were performed. Each analysis fitted clinical mastitis and either somatic cell production deviance or protein yield as a continuous trait. The bivariate model was composed of a Gaussian model for the continuous trait and a threshold model for mastitis. The analyses were performed in a Bayesian setting, using the Gibbs sampler. Point estimates (mean of marginal posterior densities) of heritability for mastitis on the underlying scale were estimated to be 0.10 and 0.12 in the two analyses. The genetic correlation between mastitis and protein yield was 0.43 and between mastitis and somatic cell production deviance was 0.80. These results make clear the importance of including clinical mastitis in the breeding goal and the usefulness of somatic cell production deviance as the indicator trait for clinical mastitis. The best measure of clinical mastitis was to consider only cases in early lactation.
Pulmonary microvascular injury following intestinal reperfusion

Abstract:Multiple organ failure is the most common cause of death in critically ill patients in the United States. Acute respiratory failure is the most important single component of this clinical scenario, with a mortality risk > 50%. Key pathophysiologic events occur in the pulmonary microvasculature at the interface between circulating elements and the external environment. In particular, the response of the alveolar capillary endothelial cell is of fundamental importance in this injury process. A variety of clinical stimuli initiate a systemic inflammatory response that contributes to acute microvascular lung injury. Sepsis, trauma, thermal injury, acute pancreatitis, and ischemia-reperfusion injury are among these stimuli. The particular emphasis of this review is on events associated with intestinal ischemia-reperfusion, a common and important clinical event. The pathogenic mechanisms that lead to acute lung injury in this setting are not completely understood, although it is clear that neutrophil-endothelial interactions regulated by both humoral and local mediators are crucial. Oxygen-derived free radicals, proteases, cytokines, eicosanoids, endotoxin, complement activation products, and probably platelet activating factor and nitric oxide are involved as either signalling or effector molecules. The key cellular participants during the acute phase of injury are the polymorphonuclear neutrophil (PMN) and the microvascular endothelial cell. Each of these participants is considered with regard to phlogistic behavior and the potential for therapeutic intervention. Adherence of the neutrophil to the endothelium creates a microenvironment in which PMN-derived oxidants, proteases, and cationic proteins are discharged under conditions that lead to cellular injury. Loss of microvascular integrity results and pulmonary dysfunction follows. At present, we offer only nonspecific supportive care for patients with this problem. However, investigations into relevant molecular and cellular regulatory events offer important opportunities for directed therapy. We are now approaching the threshold for utilization of several new and specific approaches. While no single pharmacologic therapy is likely to be curative for this complex problem, it is probable that certain approaches will be of clinical benefit in the near future. This review is designed to provide a basis for understanding this evolution.

Abstract:AIM: To retrospectively evaluate the preoperative diagnostic approaches and management of colonic injuries following blunt abdominal trauma.METHODS: A total of 82 patients with colonic injuries caused by blunt trauma between January 1992 and December 2005 were enrolled. Data were collected on clinical presentation, investigations, diagnostic methods, associated injuries, and operative management. Colonic injury-related mortality and abdominal complications were analyzed.RESULTS: Colonic injuries were caused mainly by motor vehicle accidents. Of the 82 patients, 58 (70.3%) had other associated injuries. Laparotomy was performed within 6 h after injury in 69 cases (84.1%), laparoscopy in 3 because of haemodynamic instability. The most commonly injured site was located in the transverse colon. The mean colon injury scale score was 2.8. The degree of faecal contamination was classified as mild in 18 (22.0%), moderate in 42 (51.2%), severe in 14 (17.1%), and unknown in 8 (9.8%) cases. Sixty-seven patients (81.7%) were treated with primary repair or resection and anastomosis. Faecal stream diversion was performed in 15 cases (18.3%). The overall mortality rate was 6.1%. The incidence of colonic injury-related abdominal complications was 20.7%. The only independent predictor of complications was the degree of peritoneal faecal contamination (P = 0.02).CONCLUSION: Colonic injuries following blunt trauma are especially important because of the severity and complexity of associated injuries. A thorough physical examination and a combination of tests can be used to evaluate the indications for laparotomy. One stage management at the time of initial exploration is most often used for colonic injuries. (c) 2007 The WJG Press. All rights reserved.

Abstract:Abdominal trauma (AT) is of major global importance, particularly because the civil, terroristic, and military traumatic potential of blast injuries has increased. The consequences of blunt abdominal injuries are highly variable and frequently underestimated or even overlooked. However, the underlying path mechanisms and subsequent innate immune response remain poorly understood. Therefore, we investigated the spatiotemporal local and systemic effects of a standardized blast-induced blunt AT on the intestine and innate immune response. In an established AT model, 66 male C57Bl6 mice were anesthetized and exposed to either a single blast wave centered on the epigastrium or control treatment (sham). At 2, 6, or 24 hours after trauma induction, animals were sacrificed. In 16 of 44 (36%) AT animals, one or more macroscopically visible injuries of the intestine were observed. Epithelial damage was detected by histological analysis of jejunum and ileum tissue samples, quantified by the Chiu score and by increased plasma concentrations of the intestinal fatty acid-binding protein, an enterocyte damage marker. Moreover, in the early posttraumatic period, elevated syndecan-1, claudin-5, and mucin-2 plasma levels also indicated alterations in the gut-blood barrier. Increased levels of pro-inflammatory cytokines such as TNF and macrophage inflammatory protein 2 in tissue homogenates and plasma indicate a systemic immune activation after blunt AT. In conclusion, we detected early morphological intestinal damage associated with high, early detectable intestinal fatty acid-binding protein plasma levels, and a considerable time- and dose-dependent impairment of the gut-blood barrier in a newly established mouse model of blunt AT. It appears to be a sufficient model for further studies of the intestinal immunopathophysiological consequences of AT and the evaluation of novel therapeutic approaches.

Abstract:Aim: To investigate the functional, morphological changes of the gut barrier during the restitution process after hemorrhagic shock, and the regional differences of the large intestine and small intestine in response to ischemia/reperfusion injury. Methods: Forty-seven Sprague-Dawley rats with body weight of 250-300 g were divided into two groups: control group (sham shock n = 5) and experimental group (n = 42). Experimental group was further divided into six groups (n = 7 each) according to different time points after the hemorrhagic shock, including 0(th) h group, 1st h group, 3rd h group, 6th h group, 12th h group and 24th h group. All the rats were gavaged with 2 mL of suspension of lactulose (L) (100 mg/2 mL) and mannitol (M) (50 mg/each) at the beginning and then an experimental rat model of hemorrhagic shock was set up. The specimens from jejunum, ileum and colon tissues and the blood samples from the portal vein were taken at 0, 1, 3, 6, 12 and 24 h after shock resuscitation, respectively. The morphological changes of the intestinal mucosa, including the histology of intestinal mucosa, the thickness of mucosa, the height of villi, the index of mucosal damage and the numbers of goblet cells, were determined by light microscope and/or electron microscope. The concentrations of the bacterial endotoxin lipopolysaccharides (LPS) from the portal vein blood, which reflected the gut barrier function, were examined by using Limulus test. At the same time point, to evaluate intestinal permeability, all urine was collected and the concentrations of the metabolically inactive markers such as L and M in urine were measured by using GC-9A gas chromatographic instrument. Results: After the hemorrhagic shock, the mucosal epithelial injury was obvious in small intestine even at the 0(th) h, and it became more serious at the 1st and the 3rd h. The tissue restitution was also found after 3 h, though the injury was still serious. Most of the injured mucosal restitution was established after 6 h and completed in 24 h. Two distinct models of cell death-apoptosis and necrosis-were involved in the destruction of rat intestinal epithelial cells. The number of goblet cells on intestinal mucosa was reduced significantly from 0 to 24 h (the number from 243+/-13 to 157+/-9 for ileum, 310+/-19 to 248+/-18 for colon; r = -0.910 and -0.437 respectively, all P<0.001), which was the same with the large intestine, but the grade of injury was lighter with the values of mucosal damage index in 3 h for jejunum, ileum, and colon being 2.8, 2.6, 1.2, respectively. The mucosal thickness and the height of villi in jejunum and ileum diminished in 1 h (the average height decreased from 309+/-24 to 204+/-23 microm and 271+/-31 to 231+/-28 microm, r = -0.758 and -0.659, all P<0.001; the thickness from 547+/-23 to 418+/-28 microm and 483+/-45 to 364+/-35 microm, r = -0.898 and -0.829, all P<0.001), but there was no statistical difference in the colon (F = 0.296, P = 0.934). Compared with control group, the urine L/M ratio and the blood LPS concentration in the experimental groups raised significantly, reaching the peak in 3-6 h (L/M: control vs 3 h vs 6 h was 0.029+/-0.09 vs 0.063+/-0.012 vs 0.078+/-0.021, r = -0.786, P<0.001; LPS: control vs 3 h vs 6 h was 0.09+/-0.021 vs 0.063+/-0.012 vs 0.25+/-0.023, r = -0.623, P<0.001), and it kept increasing in 24 h. Conclusion: The gut barrier of the rats was seriously damaged at the early phase of ischemic reperfusion injury after hemorrhagic shock, which included the injury and atrophy in intestinal mucosa and the increasing of intestinal permeability. Simultaneously, the intestinal mucosa also showed its great repairing potentiality, such as the improvement of the intestinal permeability and the recovery of the morphology at different phases after ischemic reperfusion injury. The restitution of gut barrier function was obviously slower than that of the morphology and there was no direct correlation between them. Compared with the small intestine, the large intestine had stronger potentiality against injury. The reduction of the amount of intestinal goblet cells by injury did not influence the ability of intestinal mucosal restitution at a certain extent and it appeared to be intimately involved in the restitution of the epithelium.
Gut failure and translocation following burn and sepsis

Abstract:Sepsis with multisystem organ failure is a major cause of morbidity and mortality in burns. We studied the anatomic, physiologic, and metabolic changes of gut mucosa as a normal barrier against sepsis and systemic inflammatory response after burn and sepsis in the chronic porcine model. Flow probes were placed on the mesenteric and hepatic arteries and portal vein. Catheters were placed in the pulmonary artery (Swan-Ganz), aorta, superior mesenteric, and hepatic veins. After 5 days, baseline data were collected and studied after a 40%, third degree burn. They were resuscitated with Ringer's lactate solution (Parkland formula). Eighteen hours later, Escherichia coli endotoxin (100 micrograms/kg) was administered. All animals were sacrificed after 30 hr. The data were compared to a group of sham animals. Following thermal injury the cardiovascular status was stable. Endotoxin administration decreased systemic vascular resistance index and mean arterial pressure, but increased cardiac index. Mesenteric blood flow, vascular resistance, and oxygen consumption showed a transient fall after endotoxin infusion with 20, 23, and 40% reduction, respectively. These changes were associated with a rise in plasma levels of conjugated dienes. The intestinal ornithine decarboxylase activity was elevated at the end of the experiment, evidence of gut repair. Gut bacteria translocated into mesenteric lymph nodes, spleen, and burn wounds in 50% of the animals. We concluded that bacterial translocation into mesenteric lymph nodes, spleen, and wound is due to gut mucosal failure after burn trauma and sepsis. These pathophysiologic changes may be the result of mesenteric ischemia and reperfusion injury.
[Enteral access and intestinal function assessment in the critically ill patient]

Abstract:The use of enteral nutrition (EN) in the critically-ill patient makes necessary to evaluate its effectiveness and impact on achieving the target requirements. Gastrically administered EN has a high complication rate, especially increased residue that leads to hyponutrition. The use of the small bowel (jejunum) may achieve greater administered volume, although there are three aspects that directly influence on its use: intestinal access route, motility and absorptive capability, and barrier function. The selection of the access route to the digestive tube has to be done after evaluating the underlying disease and predicted duration of EN. If it is greater than 4-6 weeks a definitive access will be performed through an invasive technique of ostomy (radiologic, endoscopic or surgical jejunostomy) and if it is shorter than 4-6 weeks, an endoscopic, fluoroscopic or ultrasonographic non-invasive or transnasal technique (naso-duodenal, or nasojejunal) will be used. By protocoling procedures and experiences, it has been shown that jejunal nutrition may achieve an increase in the amount of requirements administerd to critically-ill patients with mechanical ventilation as compared to gastric feeding, although the benefits with regards to reducing the number of infectious complications, hospital stay and mortality are not so clear-cut, so that it should be left to those cases in which gastric feeding has been clearly documented. By using the manometrich technique or the acetaminophen absorption tests it has been shown that 50% of critically-ill patients with mechanical ventilation have gastric antral hypomotility with decreased migratory motor complexes and gastric voiding, which considerably hampers nutrition. Under normal circumstances, during fasting, there are regular motor contractions, or an inter-digestive migratory motor complex which pattern prevents nutrient absorption because of being highly propulsive, so that during the nutrient phase, this pattern changes into the postprandial pattern with an irregular and continuous contraction activity, with no activity centers, which is much more adapted to nutrient absorption. In critically-ill patients, this normal propulsive pattern is lost, the postprandial pattern is frequently lost, and the inter-digestive pattern remains, which prevents enteral feeding. There are several factors that have an impact on this change, mainly the underlying disease, sepsis, head trauma, mechanical ventilation, sedation, and muscle relaxation. The use of pro-kinetic agents such as metoclopramide may, at least theoretically, modify motility impainment and facilitate the correct administration of prescribed requirements. Among other functions, the gastrointestinal tract (GIT) has a barrier function between inner and outer media, which prevents bacteria, antigenic agents, and toxicants from entering the blood. Its failure is characterized by decreased nutrient absorption, impaired intestinal immunological response and increased intestinal permeability (IP). Among the hypothesis trying to explain systemic infection and multiorgan failure (MOF), there is precisely anatomical and functional integrity of the intestinal mucosa. Mucosal impairment with increased IP has been shown in burn patients, polytrauma, major surgery, hematopoietic cell transplantation, and sepsis, although its relationship with bacterial translocation has not clearly been established. Before the evidences that link the GIT with MOF, the monitoring methods aimed at early correction of splaenic hypoperfusion focus on the mechanisms implicated in increased IP.
