Abstract:OBJECTIVE:To analyze the clinical features and treatment of tuberculous otitis media and mastoiditis.METHOD:Thirteen patients with tuberculous otitis media and mastoiditis were retrospectively analyzed and the related literatures were reviewed. One case was treated by surgery only, and anti-tuberculosis treatment was given to another one patient, and surgical removal of disease lesions in combination with anti-tuberculosis treatment were given to 12 patients.RESULT:One patient received surgery only was found to recur after follow-up for seven months. The patient did not recur after seven months' follow-up after antituberculosis therapy,and other patients did not recur.CONCLUSION:Although the regular anti-tuberculosis chemotherapy treatment was the main treatment for tuberculous otitis and mastoiditis, surgical treatment was helpful to achieved more rapid healing of the ear.
Analysis of the clinical features of nine cases of tuberculous otitis media and mastioditis

Abstract:Objective To analyze of the clinical features and treatment outcomes patients of tuberculous otitis media and mastioditis,and increase awareness of the disease and reduce misdiagnosis.Methods The clinical data of 9cases(9ears)with tubercular otitis media and mastoiditis were reviewed.Results Nine patients had otorrhea history.8patients demonstrated hearing loss and 2patients demonstrated a peripheral-type facial palsy.Temporal bone high-resolution CT scans demonstrated the entire tympoanum and mastoid air cells were occupied by soft tissue,six patients demonstrated bone destruction and one sequestra was found.Two cases(2ears)have a history of tuberculosis.Surgical removal of disease lesions in combination with anti-tuberculousis treatment were given to 8patients.One patients acute miliary tuberculosis-line drug therapy alone without surgery.Other than 1cases of tuberculous otitis media and mastoiditis diagnosed by the ear canal secretions microscopy,the remaining 8cases wered diagnosed post-operatively.Nine patients were followed up for 6months-15symptoms disappeared and no recurrence.Conclusion Tuberculous otitis media and mastioditis is a rare disease.The clinical features of tubercular otitis media and mastoiditis was similar easily confused with chronic suppurative otitis media.

Abstract:Tuberculosis cutis orificialis is a rare manifestation of cutaneous tuberculosis which occurs in the oral, perianal and adjacent mucosa. The most frequent orificial lesion location is the tongue. We report a case of a 35-year-old female with tuberculosis cutis orificialis with gingival involvement. She had a six-month history of a moderately painful gingival ulceration. Histopathological examination showed granulomatous infiltrates composed of epithelioid cells, Langhans giant cells and caseating necrosis. A purified protein derivative test was positive. Sputum culture was positive for Mycobacterium tuberculosis. Chest X-ray and high resolution computed tomography showed active pulmonary tuberculosis in both upper lung zones. The gingival specimen was positive for M. tuberculosis polymerase chain reaction. A complete resolution was achieved after six months of anti-tuberculosis therapy. Dental identification of M. tuberculosis may serve as an important aid in the first line of control of this dangerous infectious disease.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Since the introduction of effective chemotherapy, the development of new diagnostic tools, and raising public awareness of the devastating health, social, and economic consequences of tuberculosis (TB), the frequency of this disease has decreased. Pulmonary TB is the most common form of the disease, but TB bacilli can spread to other organs including lymph nodes, and the oral cavity especially the tongue with very unusual features. Oral lesions, although rare, are very important for early diagnosis and interception of primary TB. The bacteriological and histopathological results are important to confirm the diagnosis. Oral TB lesions may be either primary or secondary. The evolution is generally good under a medical treatment. A 35-year-old patient was admitted to our department for an ulcerous tumor in the lateral border of his tongue. The biopsy of the ulceration confirmed the diagnosis of TB. A concomitant pulmonary localization has been found. This case report aims to attract the attention of TB, which can be located in some uncommon regions, and have unusual clinical presentations.

Abstract:Tuberculosis (TB) is an infectious disease affecting humans of all ages in all parts of the world. The dentist plays an important role in the identification and control of this condition by early recognition of oral lesions that may precede the detection of the pulmonary form. Occurrence of increased incidence of mycobacterial infections as a part of the spectrum of AIDS only emphasizes the importance of early diagnosis. A case of a tuberculous ulcer on the tongue along with oral ulcerations, which occurred as a consequence of oral antituberculosis therapy (ATT), is presented. Such complications have rarely been reported in the literature and the management of these is described herein. The tuberculous ulcer healed uneventfully in five weeks after institution of ATT and the other ATT-induced ulcers healed after a week of topical anesthetic application. The clinical presentations, differential diagnoses to be considered, and management of such oral manifestations is discussed. The occupational risk posed by TB to the dentist and appropriate precautions to be observed have been highlighted.

Abstract:OBJECTIVE:To further understand the clinical features of laryngeal tuberculosis and to reduce the rate of misdiagnosis.METHODS:Sixteen cases of laryngeal tuberculosis from Feb. 1998 to Aug 2006 were retrospectively studied. The clinical features and the causes underlying misdiagnosis were analyzed.RESULTS:The disease was more common in the elderly. The clinical manifestations of the infection were various and atypical. The lesions commonly involved the vocal cord (12/16 cases). Local manifestations were mainly proliferation lesions such as mass and granulation.CONCLUSION:Attention should be paid to the clinical features of laryngeal tuberculosis. Thorough examination should be carried out and early biopsy can avoid misdiagnosis.

Abstract:Tuberculosis (TB) is an infectious disease and major health concern. Head and neck tuberculosis (HNTB) is relatively rare, but can arise in many regions, including the lymph nodes, larynx, oral cavity and pharynx. We retrospectively reviewed the clinical records of 60 patients diagnosed with HNTB in our department between March 2005 and January 2016. A review and summary of previous HNTB articles published in PubMed since 1885 was also performed. The subjects consisted of 17 males and 43 females, and the average age of patients was 45 ± 14.67 years. The major clinical presentation was a lump or swelling, followed by an oral ulcer and skin fistula. The most common site of tuberculosis was in the cervical lymph node. Three patients also suffered from a malignant tumor in the head and neck region. A total of 980 papers involving 5881 patients were included in our literature review. The included subjects ranged in age from 15 months to 100 years with a male-to-female ratio of 1.5:1. The larynx (38.92%), cervical lymph nodes (38.28%) and oral cavity (9.92%) were the three most common development sites. 465 patients were positive according to a HIV test, and 40 patients had comorbidities with different types of tumors. Head and neck tuberculosis should always be considered during a differential diagnosis for lesions in the head and neck region. Early diagnosis and treatment can greatly enhance the therapeutic effect and patients' quality of life.

Abstract:ABSTRACT. Tuberculous gumma (TG) is a rare type of cutaneous tuberculosis thought to occur as a result of the hematogenous spread of Mycobacterium tuberculosis , which is more common in immunosuppressed individuals. An 8-year-old boy presented with a 2-month history of multiple indolent enlarging ulcerated nodules on his left upper extremity. He had a past medical history of bacille Calmette–Guerin vaccine induced lupus vulgaris. Skin biopsy of the nodules showed granulomas and neutrophil-dominated purulent inflammation. Ziehl–Neelsen staining was negative, and the cultures were positive for M. tuberculosis . Furthermore, the M. tuberculosis complex was identified using metagenomic next-generation sequencing. Standard antitubercular therapy was started at full doses, and the skin lesions had significantly improved 3 months later. Here we review the literature since 2000 and describe the clinical and pathological features of TG.

Perianal tuberculous ulcer with active pulmonary, intestinal and orificial tuberculosis: A case report

Abstract:Background: Orificial tuberculosis is a rare type of tuberculosis, which is easy to be misdiagnosed, and can cause great damage to the perianal skin and mucosa. Early diagnosis can avoid further erosion of the perianal muscle tissue by tuberculosis bacteria. Case summary: Here, we report a case of disseminated tuberculosis in a 62-year-old male patient with a perianal tuberculous ulcer and active pulmonary tuberculosis, intestinal tuberculosis and orificial tuberculosis. This is an extremely rare case of cutaneous tuberculosis of the anus, which was misdiagnosed for nearly a year. The patient received conventional treatment in other medical institutions, but specific treatment was delayed. Ultimately, proper diagnosis and treatment with standard anti-tuberculosis drugs for one year led to complete cure. Conclusion: For skin ulcers that do not heal with repeated conventional treatments, consider ulcers caused by rare bacteria, such as Mycobacterium tuberculosis.
Head and neck tuberculosis: Literature review and meta-analysis

Abstract:Background: Head and neck tuberculosis (HNTB), including cervical lymphadenopathy, is the most common extrapulmonary manifestation of TB. The proposed study investigated the epidemiologic and clinical characteristics of HNTB. Materials and methods: A literature search was conducted via PubMed, Embase, Cochrane Library and Wanfang for keywords (tuberculosis, head and neck, laryngeal, pharyngeal, tongue, oropharyngeal, nasopharyngeal, and oral cavity). Scientific articles published from January 1990 through July 2017 were selected and reviewed to assess the epidemiology, presentation, diagnosis and treatment of HNTB disease. Results: Results from the included 57 studies revealed that the majority of HNTB cases were age<40 years and female. The most common HNTB sites were cervical lymph nodes (87.9%), followed by larynx (8.7%). Involvement of other HN-regions was rare (3.4%). Multidrug resistant TB was not common among the majority of studies. Given the paucibacillary nature of HNTB, sputum tests did not have a good performance on HNTB diagnosis. Most of HNTB cases were diagnosed by fine-needle aspiration, cytology and excision biopsies in combination with clinical presentations. Conclusion: HNTB disease is an important manifestation in the diagnostic process in an otolaryngologist practice. The developments of rapid, ultrasensitive, simple and cost-effective high-throughput methods for early diagnosis of HNTB are urgently needed.
A Clinicopathologic Review of 21 Cases of Head and Neck Primary Tuberculosis

Abstract:Purpose: A diagnosis of tuberculosis (TB) of the head and neck has been a dilemma for clinicians, because the clinical and pathologic features tend to mimic different pathologies. Our study aimed to identify the demographic, clinical, and pathologic features of head and neck TB to help healthcare providers in the early detection of the disease. Materials and methods: We performed a retrospective analysis using the medical archives at the pathology laboratory. Twenty-one patients with a clinical and pathologic diagnosis of head and neck TB were identified from 2010 to 2019. Results: The age distribution was broad, with 28.5% of the patients younger than 15 years old. Seven patients had oral TB, with the most common sites affected the labial vestibule (3 cases) and buccal vestibule (3 cases), followed by 1 case in the tongue. The predominant clinical presentation was ulceration, and the most common entity in the clinical differential diagnosis was squamous cell carcinoma. The most common histopathologic pattern of oral TB was a noncaseating granuloma. Most of the tuberculous lymphadenitis was located in the submandibular and submental areas. Conclusions: TB of the head and neck region is not, as previously thought, a rare condition. Head and neck TB, in general, and oral TB, in particular, are great mimickers of different pathologic entities. In particular, the absence of the typical histopathologic pattern of TB granuloma has made the diagnosis more complicated. Healthcare providers must be vigilant and consider all the demographic, clinical, and histopathologic features to detect the disease earlier, because an early diagnosis can result in better outcomes.
Gingiva as the primary site of extrapulmonary tuberculosis: A rare case report with brief review of literature

Abstract:Multiple strains of Mycobacteria cause tuberculosis (TB), a chronic, specific infectious granulomatous disease. It mainly occurs with pulmonary involvement when compared to extrapulmonary involvement. Primary oral occurrence is uncommon and oral lesions are usually secondary to pulmonary involvement. When there are no active pulmonary clinical manifestations of TB, the diagnosis of the very rare entity of primary gingival TB poses a great challenge to clinicians. In this case report, we discuss a case of primary gingival TB in a 24-year-old lactating mother. This article briefs the onset and course of the lesion during pregnancy and postpartum, elaborates the pathway to diagnosis, various investigations performed and the regimen of antitubercular therapy for 6 months, followed by complete resolution of the lesion without recurrence. This report also describes the significance of considering TB as a differential diagnosis in oral lesions and the various diagnostic methods available. It also emphasizes the sole importance of histopathology in the early detection of the lesion and its management.

Abstract:Tongue diagnosis in traditional Chinese medicine (TCM) assesses the health by investigation of tongue coating. The science and technology of tongue coating analysis have become a significant issue for modernization of TCM. The relationship between microecology of tongue coating and TCM was relevant to the syndrome differentiation in TCM, such as the cold/hot syndrome may exhibit different specific microbiota patterns in the tongue coating. This article provides a review on the microbiota research of tongue coating.
Oral manifestation of tuberculosis: a case-report

Abstract:The present case-report describes tuberculosis on the oral mucosa, in a rare manifestation of the disease. The importance of appropriate diagnosis and awareness of the clinical manifestations is highlighted. Oral lesions seem to occur as chronic ulcers, nodular or granular areas, and rare, firm leukoplakia regions. Most extra-pulmonary lesions represent secondary infections of a primary lung infectious focus; therefore, early and accurate diagnosis is required for planning of the best treatment and strategies to control the disease.
Tuberculosis of middle ear and mastoid

Abstract:Objective: Review clinical manifestations, investigation, and treatment of tuberculosis of the middle ear. Material and method: A retrospective review of six cases diagnosed as tuberculosis of the middle ear at a tertiary referral center was included in the present study. The outcome measures were history, clinical manifestations, investigation, and treatment were reviewed in the medical records. Results: The authors retrospectively reviewed data of our patients diagnosed as tuberculosis of the middle ear. It has been found that the most common manifestation is chronic otorrhea. Investigations are variable such as histopathology, tissue culture, or polymerase chain reaction for identification of tuberculosis. Treatment is mainly medication, however, surgery is considered for histopathology in selected cases. Conclusion: Tuberculosis of the middle ear is a rare disease that requires high index of suspicion for diagnosis due to the variety of clinical manifestations. Identification of tuberculosis is a key for diagnosis and treatment.
Cutaneous tuberculosis: A diagnostic and therapeutic study of 20 cases

Abstract:Cutaneous tuberculosis is showing a resurgence. It continues to be one of the most elusive and difficult diseases to diagnose in dermatological practice. We sought to describe the clinical manifestations, laboratory features, and antituberculous therapy in cases of cutaneous tuberculosis. Twenty cases of cutaneous tuberculosis were observed in this study. Of the 20 cases, seven were in women (35%) and 13 were in men (65%). There were eight (40%) cases of lupus vulgaris, four (20%) cases of orificial tuberculosis, four (20%) cases of tuberculosis verrucosa cutis, two (10%) cases of scrofuloderma, and two (10%) cases of primary inoculation tuberculosis. In total, 20 strains of acid-fast bacilli were isolated by culture in Löwenstein-Jensen medium at 37°C. Mycobacterial species identification by polymerase chain reaction (PCR) assay showed that all strains were Mycobacterium tuberculosis. In vitro susceptibility testing by use of absolute concentration method showed that the 20 strains were sensitive to rifampin, ethambutol, and isoniazid; however, two strains were resistant to streptomycin. All of the patients responded favourably to antituberculous treatment. Cutaneous tuberculosis is a skin disease that should still be thought much of in mainland China, and in this study lupus vulgaris is the most common clinical presentation.

Abstract:Rationale: Tuberculosis of the long tubular bones in children's extremities is infrequent, particularly in the ulna. Early diagnosis poses significant challenges. This report presents a case involving a 2-year-old child with tuberculosis of the ulnar bone, accompanied by a comprehensive review of pertinent literature. The purpose of this study is to share diagnostic and therapeutic experiences and provide potentially valuable insights. Patient concerns: In this case, the patient exhibited complete destruction and expansion of the ulnar bone, resulting in a forearm size considerably greater than normal. Concerns were raised about the irreversible deformation of the ulna, the potential for a malignant bone tumor, and its impact on forearm function, potentially endangering the patient's life. Diagnoses: The diagnosis was confirmed as tuberculosis of the ulnar bone. Interventions: The patient underwent surgery to remove the affected ulnar tissue and received anti-tuberculosis medication. Outcomes: Subsequent to treatment, the destruction and expansion of the ulnar bone resolved, with the return of normal ulnar morphology and bone structure. Lessons: Even in the absence of typical symptoms like fever, weight loss, and loss of appetite, extensive destruction and expansion of a long tubular bone should prompt vigilant consideration of bone tuberculosis.

Abstract:A 78-year-old man with a previous diagnosis of rheumatoid arthritis on prolonged treatment with corticosteroids presented with intense and progressive pain at the cervical level that prevented him from resting his head and walking, in addition to an ulcerative lesion covering 80% of the lingual area that was previously treated as oral candidiasis without improvement. On arrival, with no clinical or serological data of rheumatoid arthritis, immunosuppressive treatment was suspended, and a biopsy of the oral cavity was requested, confirming the diagnosis of lingual tuberculosis, an extremely rare disease, occurring in less than 1% of extrapulmonary cases. MRI of the cervical spine showed a crush fracture of the C6 and C7 bodies associated with spondylitis of probably infectious etiology that required surgical treatment, and histopathological studies confirmed Pott's disease. The patient displayed no evidence of pulmonary tuberculosis from arrival until the end of the follow-up.
