Abstract:Tuberculosis cutis orificialis is a rare manifestation of cutaneous tuberculosis which occurs in the oral, perianal and adjacent mucosa. The most frequent orificial lesion location is the tongue. We report a case of a 35-year-old female with tuberculosis cutis orificialis with gingival involvement. She had a six-month history of a moderately painful gingival ulceration. Histopathological examination showed granulomatous infiltrates composed of epithelioid cells, Langhans giant cells and caseating necrosis. A purified protein derivative test was positive. Sputum culture was positive for Mycobacterium tuberculosis. Chest X-ray and high resolution computed tomography showed active pulmonary tuberculosis in both upper lung zones. The gingival specimen was positive for M. tuberculosis polymerase chain reaction. A complete resolution was achieved after six months of anti-tuberculosis therapy. Dental identification of M. tuberculosis may serve as an important aid in the first line of control of this dangerous infectious disease.
Clinical and Pathologic Analyses of Tuberculosis in the Oral Cavity: Report of 11 Cases.

Abstract:OBJECTIVE:The aim of this study was to analyze tuberculosis (TB) in the oral cavity according to clinical appearance, clinical differential diagnosis, treatment, and outcome.STUDY DESIGN:We enrolled 11 patients with TB in the oral cavity between November 2012 and November 2016. Glossal lymphoid TB was excluded. Clinical symptoms, auxiliary examinations, treatments, and outcomes were recorded and analyzed.RESULTS:The study population comprised 6 men and 5 women, with a mean age of 59 years. Five patients presented with ulcer, 5 with a mass, and 1 with osteomyelitis. Excisional biopsy was performed in 3 patients, mass resection in 7, and curettage of mandibular lesion in 1. After pathologic diagnosis of TB in the oral cavity in 8 patients; 6 of them underwent purified protein derivative examination, and 4 of them had positive results and received drug therapy. The mean follow-up period was 24.9 months, and there was no recurrence.CONCLUSIONS:TB in the oral cavity is rare and has no specific clinical features. Pathology, acid-fast staining, polymerase chain reaction, and DNA testing for Mycobacterium tuberculosis are useful for final diagnosis. Surgery is recommended as the treatment of choice to achieve good clinical outcomes.
Primary Tuberculosis of the Oral Cavity Presenting as the Ulcers: A Case Report

Abstract:Tuberculosis is an infectious and chronic granulomatous disease, which involves almost any organ in the body. Here we report a rare presentation of primary oral tuberculosis in a 51-year-old male, who presented to us with ulcers of the tongue, palate and buccal mucosa. This case emphasizes the importance of early diagnosis of primary oral tuberculosis. Biopsy and histopathological examination revealed tuberculous etiology of the lesion and the patient responded well to antitubercular therapy. The underdiagnosis of extrapulmonary tuberculosis lesions, along with an emerging global resistance to antitubercular drugs, warrants an increased awareness of the involvement of mycobacterium tuberculosis in atypical lesions of oral cavity. Clinicians should be therefore aware of the possible occurrence of oral lesions of tuberculosis and consider it while making a differential diagnosis of suspicious oral ulcers.

Abstract:PURPOSE:To analyze oral tuberculosis in terms of clinical appearance,clinical differential diagnosis,treatnent and prognosis.METHODS:From November 2012 to June 2016,eleven patients with oral tuberculosis were treated.The patients' general condition,clinical symptoms and signs,imaging modalities,pathological files,treatment and prognosis were collected and analyzed.RESULTS:Among 11 patients,there were 6 males and 5 females with a mean age of 59.1 years,ranging from 48 to 78 years old.Five patients presented ulceration (four in gingiva and one in buccal region),four patients presented mass (one in gingival and three in buccal region),one patient presented osteomyelitis;only one patient presented enlargement of cervical lymph nodes.Five patients' chest radiography showed sequelae of pulmonary tuberculosis.Two of ten patients received drug therapy after surgical biopsy,and eight patients received surgical resection of mass in the oral cavity.After being diagnosed as oral tuberculosis by pathological examination,six patients received purified protein derivative examination,and four of them received drug therapy against tuberculosis.The mean follow-up period was 24.9 months ranging from 8 to 48 months,with one patient loss to follow-up,no lesion recurrence was found during the follow-up period.CONCLUSIONS:Oral tuberculosis is rare,with no specific clinical appearance.Pathology and other examinations are necessary for diagnosis of oral tuberculosis.The treatment of oral tuberculosis is suggested as surgical resection or surgical resection combined with drug therapy against tuberculosis.The prognosis of oral tuberculosis is fine.
Gingiva as the primary site of extrapulmonary tuberculosis: A rare case report with brief review of literature

Abstract:Multiple strains of Mycobacteria cause tuberculosis (TB), a chronic, specific infectious granulomatous disease. It mainly occurs with pulmonary involvement when compared to extrapulmonary involvement. Primary oral occurrence is uncommon and oral lesions are usually secondary to pulmonary involvement. When there are no active pulmonary clinical manifestations of TB, the diagnosis of the very rare entity of primary gingival TB poses a great challenge to clinicians. In this case report, we discuss a case of primary gingival TB in a 24-year-old lactating mother. This article briefs the onset and course of the lesion during pregnancy and postpartum, elaborates the pathway to diagnosis, various investigations performed and the regimen of antitubercular therapy for 6 months, followed by complete resolution of the lesion without recurrence. This report also describes the significance of considering TB as a differential diagnosis in oral lesions and the various diagnostic methods available. It also emphasizes the sole importance of histopathology in the early detection of the lesion and its management.
Retrospecting Analysis of 161 Cases of Tuberculosis in Oral and Maxillofacial Region

Abstract:【Objective】To investigate the clinical features of tuberculosis in oral and maxillofacial region,and to provide reference for its diagnosis and treatment.【Methods】A tofal of 161 cases diagnosed as tuberculosis in the oral and maxillofacial region by pathological examination in Tianjin Stomatological Hospital between January 1970 and October 2006 were analyzed.【Results】Of all the 161 cases,118 cases were outpatients,43 cases were inpatients;the male to female ratio was 1∶1.5;the average age was 39.1 years;the incidence in 1990s was the highest;all parts of oral and maxillofacial region could be involved;the group of 16～45 years had higher incidence.Of all the 43 inpatients,30 cases(78%)were misdiagnosed,only 3 cases(7.8%)had systematic symptom,tumor-like presentation was the common local symptom,all the 43 inpatients were treated by surgery and drugs.No recurrence was found 1～3years′ follow-up【Conclusions】Tuberculosis in oral and maxillofacial region have gender and age predisposition.It occurs mostly in the middle age and youth,and in famale.Local sign is atypical,systematic symptom is rare.It should be differentiated from the common diseases in these areas.The principol mears of definite diagnosis is pathological examination.

Abstract:Tuberculosis (TB) is one of the most relevant infectious diseases worldwide, caused by Mycobacterium tuberculosis, generating significant symptoms or death. Early diagnosis and treatment are essential to combat new cases, although there are infections that are multi-resistant to conventional drugs. TB is mainly pulmonary; thus, two new cases are presented emphasizing the diagnostic challenge of unusual intraoral TB lesions.

Oral manifestation of tuberculosis: a case-report

Abstract:The present case-report describes tuberculosis on the oral mucosa, in a rare manifestation of the disease. The importance of appropriate diagnosis and awareness of the clinical manifestations is highlighted. Oral lesions seem to occur as chronic ulcers, nodular or granular areas, and rare, firm leukoplakia regions. Most extra-pulmonary lesions represent secondary infections of a primary lung infectious focus; therefore, early and accurate diagnosis is required for planning of the best treatment and strategies to control the disease.

Abstract:Objective To study the clinical characteristics of tuberculosis in oral and maxillofacial region,and to provide reference in early diagnosis and early treatment. Methods The clinical data of 132 patients were analyzed retrospectively. Results In all cases, male to female ratio of 1∶1.69, the average age of 38.7 years, the group of 15 ～40year-old had higher incidence. Al1 parts of oral and maxillofacial region could be involved. In all 132 cases,107 cases were treated by surgery and drugs,25 cases by drugs only. Conclusion The local sign of tuberculosis in oral and maxillofacial region was atypical ,so it was likely to misdiagnosis. Definitive diagnosis relied on pathological examination. Operations played an important role in the cases of unifocal lymph nodes tuberculosis,oral mucosal tuberculosis, multifocal concentrated lymph nodes tuberculosis and salivary tuberculosis, while the therapy of multifocal sporadic lymph nodes tuberculosis and osseous tuberculosis should be relied on the medicine.
Is the Identification of Caseating Granuloma in the Intestine Indicative of Tuberculosis? a Rare Case of Crohn’s Disease

Abstract:BACKGROUND:Crohn's disease (CD) is a chronic intestinal inflammatory disorder, the etiology of which remains unknown, and is characterized by symptoms such as chronic abdominal pain, diarrhea, obstruction, and perianal lesions. Histopathology is widely regarded as the preferred method for diagnosing CD, although the histological diagnosis may lack specificity. The identification of granulomas is commonly believed to be the most reliable diagnostic indicator for CD, surpassing all other clinical features in significance. Nevertheless, research indicates that the detection rate of granulomas in CD exhibits considerable variability. Furthermore, granulomas can manifest in various specific infections including tuberculosis and Yersinia, as well as in a range of diseases characterized by macrophage reactions such as sarcoidosis and drug-induced enteritis. Granulomas associated with CD typically do not exhibit necrosis. However, the formation of caseous granulomas may occur as a result of secondary infections related to anti-CD drug treatment or perforation of the intestinal wall. CASE PRESENTATION:In this study, we present a case of a 28-year-old female patient diagnosed with CD exhibiting histologic granulomas, including both caseating and non-caseating forms, which demonstrated a positive response to medical treatment. CONCLUSION:In clinical practice, various forms of granulomas may indicate diverse underlying diseases, yet lack specificity. It is suggested that the presence of caseous granulomas should not be considered as a definitive exclusion criterion for the diagnosis when clinical, endoscopic, imaging and other histopathological features are consistent with CD. This study is the first report of caseous granulomas in CD without concomitant tuberculosis infection.
[Primary mucosal tuberculosis of head and neck region: a clinicopathologic analysis of 47 cases]

Abstract:Objective: To study the clinicopathologic features, histologic diagnosis and differential diagnosis of primary mucosal tuberculosis (TB) in the head and neck region. Methods: Forty-seven cases of primary mucosal TB of the head and neck region were studied by hematoxylin-eosin and Ziehl-Neelsen stains. The clinical and pathologic features were analyzed with review of the literature. Results: The patients included 26 male and 21 female, with mean age 47.1 years (range 14-84 years). There were three sinonasal TB, 19 nasopharyngeal TB, two oropharyngeal TB, 18 laryngeal TB, four middle ear TB, one salivary gland TB and one laryngeal TB complicating laryngeal cancer. The initial symptoms were nasal obstruction, mucopurulent rhinorrhea, epistaxis, snoring, hoarseness, dysphagia, odynophagia, serous otitis, hearing loss, tinnitus, and otalgia. Physical examination result was variable, from an apparently normal mucosa, to an evident mass, or a mucosa with an adenotic or swollen appearance, ulcers, leukoplakic areas, and various combinations thereof. CT and MRI findings included diffuse thickening, a soft-tissue mass, calcification within the mass and bone destruction resembling malignancy. Histologic examination showed granulomas with a central necrotic focus surrounded by epithelioid histiocytes and multinucleated Langhan's giant cells. Acid-fast bacilli were difficult to demonstrate but found in 13/45 cases. Follow-up data were available in 42 patients. Conclusions: Primary TB arising in the head and neck mucosa is rare. It may mimic or co-exist with other conditions. The characteristic histopathology is a granuloma with central caseous necrosis and Langhans'giant cells. Identification of acid-fast bacilli and bacteriologic culture confirm the diagnosis of mycobacterial disease.

Abstract:One patient with tuberculous ulcer occurring in the left tongue was reported and the literatures regarding to tuberculosis were reviewed.Pathological biopsy was performed,which confirmed the diagnosis of tongue ulcer.The patient received anti-TB treatment without recurrence.The occurrence of TB in the oral cavity is rare,pathologic biopsy is needed to confirm the diagnosis,and anti-TB treatment should be given immediately when the diagnosis is established.

Abstract:Despite global efforts to eradicate tuberculosis (TB), it remains a leading cause of death worldwide. TB primarily affects the lungs but can seed and become extra-pulmonary tuberculosis (EPTB), accounting for 15-20% of cases. Gastrointestinal tuberculosis (GITB) is rare, accounting for 1% to 5% of all EPTB infections, affecting mostly the ileocecal region and presenting with symptoms like abdominal pain, nausea, diarrhea, and weight loss. Diagnosis involves clinical history, lab work, and endoscopies, with biopsies having up to 80% accuracy. AFB presence, positive cultures, or evidence of TB in other organs may aid diagnosis. High clinical suspicion warrants starting antituberculosis therapy, even with negative tests, to prevent life-threatening consequences.

Abstract:Here a rare case of primary splenic tuberculosis is reported. The immunocompetent patient was admitted to our department for fever, without definite pulmonary infection. He was ultimately diagnosed based on pathologic evidence after splenectomy and positive culture for mycobacterium. The clinical manifestation of the disease and diagnostic procedure used in this case will help clinicians recognize this disease.

Abstract:Tuberculosis (TB) is an infectious disease and major health concern. Head and neck tuberculosis (HNTB) is relatively rare, but can arise in many regions, including the lymph nodes, larynx, oral cavity and pharynx. We retrospectively reviewed the clinical records of 60 patients diagnosed with HNTB in our department between March 2005 and January 2016. A review and summary of previous HNTB articles published in PubMed since 1885 was also performed. The subjects consisted of 17 males and 43 females, and the average age of patients was 45 ± 14.67 years. The major clinical presentation was a lump or swelling, followed by an oral ulcer and skin fistula. The most common site of tuberculosis was in the cervical lymph node. Three patients also suffered from a malignant tumor in the head and neck region. A total of 980 papers involving 5881 patients were included in our literature review. The included subjects ranged in age from 15 months to 100 years with a male-to-female ratio of 1.5:1. The larynx (38.92%), cervical lymph nodes (38.28%) and oral cavity (9.92%) were the three most common development sites. 465 patients were positive according to a HIV test, and 40 patients had comorbidities with different types of tumors. Head and neck tuberculosis should always be considered during a differential diagnosis for lesions in the head and neck region. Early diagnosis and treatment can greatly enhance the therapeutic effect and patients' quality of life.

Abstract:BACKGROUND Primary nasal tuberculosis (TB) is a rare form of extrapulmonary TB, particularly in patients receiving anti-tumor necrosis factor (TNF) immunotherapy. As a result, its diagnosis remains challenging. CASE SUMMARY A 58-year-old male patient presented to the ear, nose, and throat department with right-sided nasal obstruction and bloody discharge for 1 month. He was diagnosed with psoriatic arthritis and received anti-TNF immunotherapy for 3 years prior to presentation. Biopsy findings revealed chronic granulomatous inflammation and a few acid-fast bacilli, suggestive of primary nasal TB. He was referred to our TB management department for treatment with oral anti-TB agents. After 9 months, the nasal lesions had disappeared. No recurrence was noted during follow-up. CONCLUSION The diagnosis of primary nasal TB should be considered in patients receiving TNF antagonists who exhibit thickening and crusting of the nasal septum mucosa or inferior turbinate, particularly when pathological findings suggest granulomatous inflammation.

Abstract:Objective To improve the diagnosis and treatment of granulocytic sarcoma. Methods Clinical data of one case of oral cavity was retrospectively analyzed and pertinent literatures were reviewed.We also analyzed the clinical characteristics, pathological features, diagnosis, treatments and prognosis. Results Granulocytic sarcoma is a rare extramedullary tumor composed of immature cells of the granulocytic series.Effects of chemotherapy and radiotherapy are good, patient follow-up with non recurrence. Conclusion Oral cavity GS is rare, and may be easily misdiagnosed as lymphoma. Histopathologic examination is the golden standard in GS diagnosis. MPO is the most susceptible marker for diagnosing GS.

Rare Extrapulmonary Tuberculosis: Covert symptoms and Diagnostic Dilemma

Abstract:Background: Pulmonary tuberculosis may result in haematogenous and lymphatic extension in case of failure of early detection, or immunocompromised status, leading to extrapulmonary tuberculosis. Rare sites of extrapulmonary tuberculosis include the gastrointestinal tract, musculoskeletal system, genital tract, middle ear and pericardium. Histopathological findings of macro-confluent granuloma with or without caseous necrosis, along with detection of acid-fast bacilli (AFB) on Ziehl-Neelsen (ZN) staining, and GeneXpert for detection of Mycobacterium tuberculosis DNA, are key in establishing a diagnosis of tuberculosis. Methodology: Biopsy-proven extrapulmonary granulomatous lesions were included in this study. Histopathological evaluation of all extrapulmonary biopsy specimens sent to the Department of Pathology were done for the presence of granuloma and necrosis, and ZN staining for AFB was done in all the cases of granulomatous lesions with or without the presence of necrosis. The same cases, with biopsy specimens sent in normal saline, were re-evaluated in a molecular laboratory with the help of GeneXpert MTB to detect the DNA of Mycobacterium tuberculosis. All biopsy specimens from extrapulmonary sites which were sent to the Department of Pathology were used for DNA extraction. Results: Out of the 10 cases of extrapulmonary granulomatous lesions, 8 showed caseous necrosis on microscopy, and 7 showed the presence of acid-fast bacilli on Ziehl-Neelsen staining. GeneXpert detected DNA of Mycobacterium tuberculosis in 9 cases. Conclusion: Extrapulmonary tuberculosis rarely occurs as primary, and mostly spreads from lung parenchyma via a haematogenous route. Tuberculosis of the gastrointestinal tract, peritoneum, lymph nodes, and solid viscera are together termed abdominal tuberculosis. Entities like tuberculosis of the pericardium and ear are extremely rare. Extrapulmonary tuberculosis should be a differential in cases of chronic non-responding cases with diagnostic dilemmas. To avoid diagnostic delay, in cases of high suspicion, one should go for biopsy along with ZN staining for diagnostic confirmation as this is cost-effective, followed by GeneXpert for Mycobacterium tuberculosis in highly suspected cases with absent caseous necrosis and negative ZN staining.

Abstract:Objective:To report a case with fatal bacteria granuloma after trauma(FBGT).Methods:Clinical profile and response to treatment were described.Results:The patient presented with growing red plaque in face after trauma,and the therapy of antituberculosis showed little effect.The lesions improved significantly after one month's treatment with oral prednisone and triamcinolone acetonide was injected in lesions immediately.One week later,the patient started to show signs of fever and conscious disturbance.Pathological examination and MRI confirmed the diagnosis of FBGT.Conclusion:FBGT is a rare infectious disorder,which often leads a fatal outcome.
