Abstract:BACKGROUND: Dentigerous cyst (DC) is one of the most common developmental odontogenic cysts. This hospital-based retrospective study evaluated the clinical and histopathological features of 338 DCs in Taiwanese patients. METHODS: In this study, 338 consecutive cases of DCs were collected from January 1995 to December 2009. The demographic data and clinicopathological features of these DC cases were reviewed and analyzed. RESULTS: The 338 DCs were taken from 332 patients (212 men and 120 women, mean age 33.0 years, range 5-83 years). There was a significant difference in the mean age between male and female patients (P < 0.05). Of the 338 DCs, 122 were found in the maxilla and 216 in the mandible. The most common site for DCs was the mandibular molar region (169 cases) and the most frequently involved tooth was the mandibular third molar (153 cases). Forty-six of the 338 DCs were found to be associated with supernumerary teeth. Microscopically, 317 cysts were lined by stratified squamous epithelium, 9 by mucoepidermoid-typed epithelium, and 12 by ciliated pseudostratified columnar epithelium. Hyaline bodies of Rushton, odontogenic epithelial rests, cholesterol clefts, foamy histiocytes, hemosiderin-laden macrophages, and dystrophic calcifications were discovered in 11, 78, 90, 33, 62 and 45 DCs, respectively. CONCLUSION: Our results indicate that DCs of Taiwanese patients have a male predominance and occur more frequently in patients between 10 and 29 years of age. The most commonly affected site for DCs is the mandibular molar region and the most frequently involved tooth is the mandibular third molar.

Abstract:Background: Dentigerous cysts, deemed of developmental origin, are benign odontogenic cysts characterized by a gradual growth rate. Their occurrence is twice as prevalent in men compared to women. These cysts are recognized as the most frequent developmental cysts affecting the jaws, with a typical manifestation in individuals aged 20 to 40, while infrequently identified in young children. Notably, dentigerous cysts have the potential to attain significant dimensions, resulting in painless enlargement of the jaw and subsequent deformation. Objectives: To assess the clinicopathological features and management of ten years of experience with dentigerous cysts. Methods: A challenging cases were reported from reviewed records of the patients who were treated by the surgical intervention of various dentigerous cysts throughout the period of ten years, 2012-2022 and only histologically confirmed cases were selected, at Ramadi Teaching Hospital in addition to Rashid, Razi, Zuhur Private Hospitals and private clinics in Iraq. Results: 76 patients were included in this clinicopathological research. The highest age group affected was ≤ 18 years (68.4%), 54% were male, the mandible was more affected (63.1%) than the maxilla (36.9%). Marsupialization was applied to 30.3% of the cases, while enucleation was carried out in 69.7%. Conclusions: The significance of meticulous examination of radiographs and the consequences associated with undetected and untreated ailments is affirmed by this case study. A comprehensive understanding of oral pathology serves as a valuable resource for dentists, facilitating accurate diagnosis, appropriate referrals, and the provision of anticipatory guidance to patients striving to achieve optimal oral health across various age groups.
Marsupialization of a Large Dentigerous Cyst in the Mandible: A Case Report

Abstract:Dentigerous cysts (DCs) are one of the most common cysts in the oral and maxillofacial region, and they are often discovered by chance in young people. The methods of treatment differ according to the size they reach, but the prognosis and results of the treatment are generally good. DCs are often associated with impacted teeth, especially mandibular third molars and maxillary canines, and they are usually discovered when they reach large sizes or get infected after they have caused great absorption and destruction of the surrounding cortical bone and displacement in adjacent teeth. This case report expresses the importance of conservative treatment of large oral cysts (by marsupialization) in the preservation of jaw bones, in a young female child of 12 years, with a DC associated with an impacted second permanent lower molar. This cyst occupied nearly half of the mandible with the danger of causing more harm to the jaw. In brief, marsupialization is a very effective method of treatment for DCs, especially those that reach very large volumes.
[Imaging Analysis of Ameloblastoma, Odontogenic Keratocyst and Dentigerous Cyst in the Maxilla Using Spiral CT and Cone Beam CT].

Abstract:Objective: To analyze the radiolographic features of ameloblastoma (AM), odontogenic keratocyst (OKC) and dentigerous cyst (DC) in the maxilla by spiral CT (SCT) and cone-beam CT (CBCT), and to provide useful information for the differential diagnosis of benign radiolucent lesions in the maxilla. Methods: Clinical records, histopathological reports and SCT or CBCT imaging of 85 patients with primary maxillary AM, OKC or DC admitted to Peking University School and Hospital of Stomatology from December 2012 to May 2017 were collected. Radiographic characteristics including site, size, shape, cortex expansion, internal structure and effects on neighboring tissue were analyzed. For OKC and DC, the relationship between cysts and enveloped teeth was classified as centripetal, eccentric and adherent. Results: The 85 patients included 56 males and 29 females, aged from 8 to 84 years old. Eighty-three patients had a single lesion, whereas 2 patients had bilateral cysts. In total, 87 lesions were analyzed, comprising 22 AM, 45 OKC and 20 DC. Among the 22 AM, 11 lesions were desmoplastic type, 16 were round-like in shape and 18 presented with buccal expansion. The shapes of the 45 OKC varied as round-like (n=26), oval (n=3), reniform (n=4), sinus-like (n=5), sinus+round (n=5) and irregular (n=2). Furthermore, 30 OKC presented with buccal expansion, 22 nearly filled the maxillary sinus and 26 were 'dentigerous'. The tooth-cyst relationship of the 'dentigerous' OKC was centripetal in 11, eccentric in 4 and adherent in 11. Among the 20 unicystic DC, 8 lesions were centripetal, 6 were eccentric and 6 were adherent type; 16 DC presented with buccal expansion. Conclusions: Demosplastic type is common in maxillary AM. Most AM are round-like in shape and expand buccally. The shape of maxillary OKC varies greatly and maxillary sinus filling is common. More than a half of OKC appear 'dentigerous'. For DC and OKC, tooth-cyst relationship can be centripetal, eccentric and adherent.

Abstract:Dentigerous cysts are known as the second most common type of cyst in the jaws. The cyst is one of the lesions occurred frequently in the posterior body of the mandible and is often related to the unerupted third molar and forms around the crown of the unerupted tooth attaching at the cementoenamel junction. Such characteristic appearances are the diagnostic points differentiating from ameloblastoma or odontogenic keratocyst. However, it would be hard for us to diagnose it as a dentigerous cyst if the lesion does not show its typical appearance. We experienced two cases of dentigerous cysts which did not form around the crown of the unerupted tooth on radiologically. Both cysts were relatively large and resorbed adjacent teeth roots. Therefore, an ameloblastoma or an odontogenic keratocyst was suspected rather than a dentigerous cyst as the imaging diagnosis. The biopsy revealed that the lesion was a "dentigerous cyst" in one of the cases and "developmental cyst with inflammation" in another case. After the excision, the histopathological diagnosis was a dentigerous cyst with inflammation in both cases. This report shows the two cases of dentigerous cysts focusing on panoramic radiography and CT images. Also, we discuss the differential diagnosis by reconsidering those diagnostic points.

Abstract:ObjectivesMicroorganisms in untreated or overlooked root canals can cause endodontic failure or infection and inflammation. Therefore, clinicians should familiarize themselves with patient’s root canal anatomy and morphology. The objective of this retrospective study was to analyze and characterize mandibular root canal morphology using cone beam computed tomography (CBCT) in a Turkish Cypriot population.Materials and methodsThis cross-sectional study consisted of retrospective evaluation of CBCT scans from 272 adult patients (aged 16–80 years). The number of roots and canals and the canal configuration in each root were evaluated using Vertucci’s classification. The incidence of additional roots and of C-shaped canals in the mandibular first and second molars was also noted. Pearson chi-square tests were performed to analyze canal configurations, laterality, and gender (p ≤ 0.05).ResultsFor the female and male patients, 94.5 and 94.4 % of the mandibular first and 96.7 and 97.2 % of the mandibular second molars had two roots, respectively. In females, 4.4 % of mandibular first molars had three roots versus 2.7 % of mandibular first molars in male patients. Type IV was the most prevalent canal configuration regardless of the gender (female 61.3 % and male 64.2 %) in the mesial roots. No statistically significant differences according to gender or laterality were found in the incidence of additional canals for either maxillary first or second molars.ConclusionsType IV was the most prevalent canal configuration of both the mandibular first and second molar teeth. There was a low prevalence of three-rooted mandibular molars in this particular population.Clinical relevanceThe current study is the first of its kind to include a Turkish Cypriot population and, thus, provides baseline data for these patients regarding appropriate root canal treatments.

Abstract:Background and Objectives: Dentigerous cysts are one of the most frequent pathologies associated with unerupted or impacted teeth. Such cysts show a male predilection and a preference for the mandibular region. Also, they commonly occur in the second and third decades of life, with only 9% occurring in the first decade. The aim of this work is to apply and study the therapeutic algorithms developed for dentigerous cysts and their outcomes, from the early diagnostic stage to the complete healing phase of pediatric patients diagnosed with this medical condition. Materials and Methods: The study included 19 pediatric patients diagnosed with dentigerous cysts who underwent the enucleation and extraction or conservative attitude of the associated tooth. The bony healing was also followed-up 9 months after the surgery. Results: A higher incidence in the posterior area of the mandible and maxilla was observed, as well as a higher incidence in boys. The 9 months postoperative radiographic assessment showed that the bony defects were completely healed. Conclusions: A thorough understanding of the nature of the lesion backed by a good clinical history and by state-of-the-art radiographic and radiologic examinations can go a long way in helping the surgeon to choose the correct therapeutic approach and to ameliorate the medical condition in the best long-term interest of the young patient. The considered dentigerous cyst cases demonstrated that an early diagnosis and treatment of this pathology is followed by a responsive treatment.
Three-dimensional Radiographic Features of Ameloblastoma and Cystic Lesions in the Maxilla

Abstract:OBJECTIVES To characterize the radiographic features of maxillary ameloblastoma (AM), odontogenic keratocyst (OKC) and dentigerous cyst (DC) comparatively by using spiral CT and cone beam CT (CBCT). METHODS Clinical records, histopathological reports, and nonenhanced spiral CT or CBCT images of 191 consecutive patients with primary maxillary AMs, OKCs, or DCs were retrospectively acquired, and radiographic features were analyzed. RESULTS The study included 118 males and 73 females (age: 5-84 years). 72.0% of AMs and 84.3% of OKCs originated from the posterior maxilla, while 69.6% of DCs occurred in the anterior maxilla. Among 25 AMs, 44.0% were of desmoplastic type, with honey-combed appearance. 84.0% of AMs were circular or oval in shape, 84.0% expanded buccally, and 36.0% invade the nasal floor. Among 89 OKCs of 88 patients, 61.8% were circular or oval, 58.4% expanded buccally, 49.4% were dentigerous, 41.6% nearly filled the maxillary sinus, and 13.5% invaded the nasal floor. 93.7% (74/79) of DCs enveloped a single tooth, and the tooth-cyst relationship was centripetal in 35, eccentric in 30, and circumferential in 9. Moreover, 98.2% (55/56) of the cysts enveloping a supernumerary tooth were DCs, while 80.9% (38/47) of the cysts enveloping the third molar were OKCs. CONCLUSIONS Maxillary AMs tend to grow with buccal expansion and invade the nasal floor, and DAs with honey-combed lobularity are common. Maxillary OKCs have variant shapes and tend to invaginate the maxillary sinus. The tooth-cyst relationship of dentigerous OKCs and DCs can be centripetal, eccentric, or circumferential.

Abstract:Dentigerous cysts involves erupted or developing teeth in decreasing order of frequency as mandibular third molars, the maxillary canines, the maxillary third molars with rare involvement of the central incisors & are usually asymptomatic but becomes symptomatic with symptoms such as swelling, mild sensitivity, tooth mobility and displacement when it reaches size > 2 cm in diameter. Dentigerous cysts are seen most commonly in 20–30 years age group with relative low frequency in children with proportion of 4–9% of these cysts occur in the first 10 years after birth. Hospital based study conducted in the Department of ENT at Ashwini Rural medical college & Hospital, Solapur in which we have reported a series of three cases of dentigerous cyst in paediatric patients. Dentigerous Cysts in our study were associated with maxillary permanent lateral incisor, mandibular permanent premolar, Mandibular second molar tooth in pediatric age group. Provisional diagnosis of Dentigerous cyst based on clinical, radiological & biochemical study but Histopathological examination confirms the diagnosis. Dental orthopentamogram aids in diagnosing and localizing the Dentigerous cyst and ct face in all views helps to see bony effects and complications due to expansion of Dentigerous cyst. Enucleation of the cystic lesion followed by extraction of the offending teeth is a suitable treatment modality for Dentigerous cyst associated with maxilla and mandible giving adequate balance between complete removal of cyst & giving chance for eruption of unerupted teeth.

Abstract: Aim: The aims of this study are to investigate the prevalence of odontogenic cysts and tumors occurred in a sample of Turkish population in the last 10 years and to compare the data with latest reports. Methods and Material: The present retrospective study was carried out at Gazi University, School of Dentistry (GUSD) in Ankara, Turkey. All the data was retrieved from the GUSD archive on patients treated for various oral and maxillofacial lesions between 2008 and 2018. Patients' demographical data, main clinical features of the lesions along with histopathological findings were retrieved from the dental files of the patients. The type of lesions was evaluated according to the latest Head and Neck Tumor (HNT) classification published by the World Health Organization (WHO) in 2017. Data was analyzed by descriptive statistics and Chi-square test using SPSS version 21.0 software. Results: Among a total of 739 biopsy-conducted lesions, 467 (63.19%) were odontogenic cysts, 43 (5.82%) were benign odontogenic tumors, and 7 (0.95%) were malignant tumors. The rest of 222 cases were defined as others. Conclusions: The present study assessed the demographic and clinico-pathological characteristics of odontogenic cysts/tumors in a group of Turkish people, who visited GUSD for therapeutic purpose in the past 10 years. Results showed that male patients in their 5th-6th decades have the highest risk of odontogenic cyst/tumor, especially in the mandibular posterior region.

Abstract:One of the most prevalent types of odontogenic cysts is a dentigerous cyst, which is usually connected to the crown of an immature tooth. We report the case of an 11-year-old boy, who had a swelling over his left cheek, which was determined to be a dentigerous cyst by radiological imaging and clinical examination. Over the course of two months, the peanut-sized mass grew to 3x2 cm. A massive, well-defined cystic lesion connected to an unerupted premolar tooth was found on a CT scan of the left maxillary alveolar arch and sinus floor. Under general anesthesia, the patient had a Caldwell-Luc surgery to remove the cyst. In order to avoid difficulties related to cyst formation, which can invade surrounding tissues and even result in cancer if left untreated, early detection using radiological imaging is essential. Complete excision of the cyst is the treatment, particularly for big lesions, in order to limit morbidity and lower the likelihood of aggressive behavior. This case emphasizes the necessity of thorough examination and surgical intervention when necessary, underscoring the significance of early identification and adequate therapy to minimize potential problems related to dentigerous cysts. In cases of dentigerous cysts, early intervention, and appropriate surgical procedures are critical to reducing morbidity and improving patient outcomes.
A conservative treatment of an involved molar tooth associated with dentigerous cyst: a case report and literature review

Abstract:Background: Dentigerous cysts (DCs) are among the most frequently odontogenic cysts in young and middle-aged individuals. Marsupialization and enucleation are the main treatment options in clinical practice. However, there are few reports on preserving molars severely involved by the cyst. Case presentation: A 35-year-old male patient with a large odontogenic cyst that was successfully treated using a multidisciplinary approach. The patient's chief complaint was discomfort during mastication in the lower left molar region for a month. Clinical examination revealed that teeth 36 and 37 were intact without pain upon percussion, while tooth 38 was unerupted. The radiology examination illustrated a typical well-defined oval radiolucent lesion surrounding the crown of unerupted tooth 38, extending to the distal root of tooth 37. The treatment was divided into two parts: removal of the cyst and the impacted third molar, followed by filling with collagen bone particles; and preserving the tooth 37 via hemisection, root canal treatment and crown restoration. Pathological examination indicated a dentigerous cyst. After 18 months of treatment, the bone defect completely recovered and tooth 37 functioned well following the hemisection. Conclusions: Hemisection effectively preserves the cyst-associated molars and maintains oral function. This article underscores the significance of collaboration among various departments in the treatment of odontogenic cysts, ultimately aiming to achieve minimally invasive and functional surgery.

Abstract:The dentigerous cyst is defined as a pathological cavity whose origin is produced by developmental alterations. Most usual treatment is the enucleation of the cyst with the extraction of the tooth. The current approach tends to preserve the tooth. The aim of this study was to determine the total and partial success of the conservative treatment of the dentigerous cyst in terms of eruption of the associated permanent tooth. Pub-Med, Scopus and the Cochrane Central Register of Controlled Trials were searched from January 2012 to December 2023, including patients aged 18 years old or less with a dentigerous cyst associated with a repositionable permanent tooth in the arch. 118 articles were found and 24 were included in full text. 40 cases were reported in the mandible (83 %) and 8 in the maxilla (17 %). 29 cases of marsupialization were included (60 %), 17 of enucleation (36 %) and 2 of decompression (4 %). The percentage of teeth that erupted spontaneously, either completely or partially, after marsupialization, enucleation and decompression was 83 %, 59 % and 100 % respectively. No recurrences have been described in any case. This study highlights that the conservative treatment was a predictable procedure with a total success of 83 % by marsupialization and 100 % after decompression.
Prevalence, Characteristics and Distribution of Odontogenic Cysts amongst the Indian Subpopulation of Southern Rajasthan: A 5-year Retrospective Study of 218 Cysts

Abstract:Background: Odontogenic cysts (OCs) arise from the odontogenic epithelium and occur in the tooth-bearing regions of the jaws. Proliferation and/or degeneration of this epithelium lead to OC development. Aim and objectives: The purpose of this study was to determine the prevalence, characteristics, distribution and clinicopathological features of OCs and compare with other studies. Materials and methods: Records of OCs were retrieved, and patient's data, aetiology, clinical features, histopathological distribution, treatment, complications and recurrence were studied. Results: A total of 218 OCs were studied in 200 patients. The mean age of patients was 35.74 years. The overall sex ratio (male: female) was 1.22:1. Second, third and fourth decades of life were commonly affected. The majority of the patients (85%) reported with a complaint of pain (with or without swelling). Radicular cyst was the most prevalent type (49.55%). The most common site involved was the anterior maxilla (41.75%). Dental complications such as displacement, mobility, discoloration and root resorption were diagnosed in 91 cysts (41.75%). Bony complications such as expansion and perforation were present in 136 cysts (62.38%). Dentition was non-vital in 61.46% of cysts. The most common treatment method performed was combination surgery (enucleation with or without endodontic treatment/extraction and apicoectomy). Recurrence rate was 3.20%. Conclusion: The prevalence, characteristics and distribution features of OCs in India are similar to those of studies conducted in other parts of the world. There is a broad spectrum of OCs, with some cysts having a propensity for certain age, gender and site, whereas a few cysts have distinct properties to recur and have aggressive and invasive behaviour.
Dental Cyst Occupying the Branch Region and Mandibular Body with Displacement from the Third Molar to the Condition Region

Abstract:The dentigerous cyst is the second most frequent odontogenic cyst after the root, being typically asymptomatic and commonly incidentally diagnosed. The objective of this case was to describe a successful treatment of a dentigerous cyst associated with an unerupted lower third molar, which extended from the condyle to the premolar region, treated by decompression followed by surgical enucleation. That said, a 19-year-old female patient, black, without underlying pathologies, came to the dental office for extraction of the third molars, and after a preoperative orthopantomographic radiography, the displacement of the dental element was noted 38 for the condylar region, unilocular cystic lesion occupying the branch and mandibular body region, root resorption of the dental elements involved in the region and rejection of the mandibular canal. After cytopathological analysis of the material collected through fine needle aspiration, the diagnosis of infected dentigerous cyst was confirmed. Afterwards, extraction and decompression were performed followed by surgical enucleation. Thus, it is concluded that the treatment performed promoted a satisfactory result, given a spontaneous bone neoformation after decompression and surgical enucleation of the lesion.

Abstract:The dentigerous cyst is an odontogenic cyst that typically associates with the crown of an unerupted permanent tooth. A 38-year-old male patient was referred by the orthodontist due to a bone lesion in the region of tooth 38. The patient reported numbness and tenderness in the mandible. Imaging exams revealed tooth 38 in an inverted vertical position and a radiolucent lesion encompassing the crown, starting from the cementoenamel junction and extending to the mesial root of tooth 37 and the base of the mandible.

Abstract:Dentigerous cyst is the second most common odontogenic cyst and constitutes around 20-24% of all the odontogenic cysts involving the jaws. Usually, these cysts remain asymptomatic and rarely cause enlargement and displacement of associated tooth. In this paper, we present a case of a displaced mandibular third molar, which was associated with a large dentigerous cyst. A conservative treatment modality was adopted with a 2 year radiographic follow-up of the patient. A brief review of previously reported cases with dentigerous cyst causing displacement of the mandibular impacted third molars is also discussed, highlighting the treatment modality followed in all these cases.

Abstract:Odontogenic cysts are fluid-filled sacs arising from tooth-developing tissues within the jawbone, often causing swelling, pain, or displacement of teeth. A dentigerous cyst, specifically encapsulating the unerupted tooth's crown, can interfere with dental development by displacing teeth, causing resorption of adjacent teeth, and complicating dental or surgical interventions. This case report represents a long-term follow-up of a 35-year-old male with a large mandibular dentigerous cyst extending to the lower right border of the mandible, associated with a horizontally impacted third molar in the lower right mandibular region. The treatment consisted of cone beam computed tomography(CBCT) evaluation, followed by enucleation of the cyst with the extraction of second and third molars, root canal treatment (RCT) of the first molar, followed by autogenous symphyseal bone graft placement. Eighteen months later, the implant was placed with respect to the missing second molar. After osseointegration, an implant-supported crown was placed.

Abstract:Background: This study was conducted to analyze the morphological variations of maxillary and mandibular molars in the Turkish population, to evaluate the distances between the additional canals vertically and horizontally, to guide clinicians by determining the distance between the canals. Materials and Methods: The maxillary and mandibular first and second molars (a total of 50 teeth per group) were using in the study. The teeth were embedded in an arch-shaped silicone impression material. CBCT images of the teeth were recorded. Two examiners scrutinized the root canal systems and sought out any additional canals. Results: Upper first molars; 16% had three canals, 64% had four canals and 20% had five canals. The average distance between mesiobuccal 1(mb1)-mesiobuccal 2(mb2), mesiobuccal 1(mb1)-mesiobuccal 3(mb3), and distobuccal 1(db1)-distobuccal 2(db2) was 2.35, 2.52 and 1.91 millimeters, respectively. Upper second molars; 32% had three canals, 52% had four canals and 16% had five canals. The distance between mb1-mb2 and db1-db2 canals was 2.14 and 2.01 mm, respectively. In 16%, the mb2 canal was observed at an average depth of 0.72 mm from mb1. No significant difference was found between the number of canals and additional canals in the upper first molars and upper second molars teeth (p=0,275). Lower first molars; Two canals were detected in 8%, three canals in 20%, four canals in 28% and five canals in 44%. While 60% of the midmesial canals merged with the mesiolingual canal, 20% merged with the mesiobuccal, and 20% terminated in separate apices. Mesiobuccal(mb)-midmesial and mesiolingual(ml)-midmesial distances were 1.81 and 1.76 mm, respectively. Lower second molars; 4% had one canal, 8% had two canals, 60% had three canals, 28% had four canals. No midmesal canal was observed in the lower second molars. There was a significant difference between the number of teeth with three canals in the lower first molars and lower second molars teeth (p=0,00). Conclusions: During root canal treatment, all canal access must be accurately identified. Using adequate light sources, checking the pulp floor with canal probes, strengthening the theoretical knowledge about the points where additional canals can be found can reduce the risk of missed canals.
