Abstract:Dentigerous cysts are the most common developmental odontogenic jaw cysts, associated with crowns of an Unerupted tooth. Usually, they occur in the second and third decade of life with a male predilection. They are frequently asymptomatic and mostly discovered incidentally on routine dental radiographic examination. Its growth is slow, and can reach a considerable size causing bone expansion and displacement of teeth. Mandibular lower third molars are the most frequently effected teeth; dentigerous cysts involving impacted mandibular canines are rarely reported. Surgical enucleation with extraction of the associated tooth is the standard treatment for dentigerous cyst, marsupialization is recommended for large lesions. We report a rare case of a large dentigerous cyst in a 62-year-old male patient, associated with impacted left mandibular canine, extending from the retro-molar area of left side crossing the midline to the 2nd premolar in the other side, that was successfully treated by marsupialization followed by enucleation and removal of the impacted tooth.

Abstract:Four new compounds, RbSiBiS(4), RbGeBiS(4), CsSiBiS(4), and CsGeBiS(4), have been synthesized by means of the reactive flux method. The isostructural compounds RbSiBiS(4), RbGeBiS(4), and CsGeBiS(4) crystallize in space group P2(1)/c of the monoclinic system with four formula units in cells of dimensions at 153 K of a = 6.4714(4) A, b = 6.7999(4) A, c = 17.9058(11) A, and beta = 108.856(1) degrees for RbSiBiS(4), a = 6.5864(4) A, b = 6.8559(4) A, c = 17.9810(12) A, and beta = 109.075(1) degrees for RbGeBiS(4), and a = 6.5474(4) A, b = 6.9282(4) A, c = 18.8875(11) A, and beta = 110.173(1) degrees for CsGeBiS(4). CsSiBiS(4) crystallizes in a different structure type in space group P2(1)/c of the monoclinic system with four formula units in a cell of dimensions at 153 K of a = 9.3351(7) A, b = 6.9313(5) A, c = 12.8115(10) A, and beta = 109.096(1) degrees. The two structure types are closely related and consist of [MBiS(4)(-)] (M = Si, Ge) layers separated by bicapped trigonal-prismatically coordinated alkali-metal atoms. In each, the M atom is coordinated to a tetrahedron of four S atoms and the Bi atom is coordinated to seven S atoms comprising five close S atoms at the corners of a square pyramid with Bi near the center of the basal plane and the sixth and seventh S atoms further away to complete a distorted monocapped trigonal prism. The optical band gaps of 2.23 eV for RbGeBiS(4) and 2.28 eV for CsGeBiS(4) were deduced from their diffuse reflectance spectra. From a band structure calculation, the optical absorption for RbGeBiS(4) originates from the [GeBiS(4)(-)] layer. The Ge 4p orbitals, Bi 6p orbitals, and S 3p orbitals are highly hybridized.
Dentigerous Cyst in a Pediatric Patient: A Case Report

Abstract:One of the most prevalent types of odontogenic cysts is a dentigerous cyst, which is usually connected to the crown of an immature tooth. We report the case of an 11-year-old boy, who had a swelling over his left cheek, which was determined to be a dentigerous cyst by radiological imaging and clinical examination. Over the course of two months, the peanut-sized mass grew to 3x2 cm. A massive, well-defined cystic lesion connected to an unerupted premolar tooth was found on a CT scan of the left maxillary alveolar arch and sinus floor. Under general anesthesia, the patient had a Caldwell-Luc surgery to remove the cyst. In order to avoid difficulties related to cyst formation, which can invade surrounding tissues and even result in cancer if left untreated, early detection using radiological imaging is essential. Complete excision of the cyst is the treatment, particularly for big lesions, in order to limit morbidity and lower the likelihood of aggressive behavior. This case emphasizes the necessity of thorough examination and surgical intervention when necessary, underscoring the significance of early identification and adequate therapy to minimize potential problems related to dentigerous cysts. In cases of dentigerous cysts, early intervention, and appropriate surgical procedures are critical to reducing morbidity and improving patient outcomes.

Abstract:Background: Dentigerous cysts, deemed of developmental origin, are benign odontogenic cysts characterized by a gradual growth rate. Their occurrence is twice as prevalent in men compared to women. These cysts are recognized as the most frequent developmental cysts affecting the jaws, with a typical manifestation in individuals aged 20 to 40, while infrequently identified in young children. Notably, dentigerous cysts have the potential to attain significant dimensions, resulting in painless enlargement of the jaw and subsequent deformation. Objectives: To assess the clinicopathological features and management of ten years of experience with dentigerous cysts. Methods: A challenging cases were reported from reviewed records of the patients who were treated by the surgical intervention of various dentigerous cysts throughout the period of ten years, 2012-2022 and only histologically confirmed cases were selected, at Ramadi Teaching Hospital in addition to Rashid, Razi, Zuhur Private Hospitals and private clinics in Iraq. Results: 76 patients were included in this clinicopathological research. The highest age group affected was ≤ 18 years (68.4%), 54% were male, the mandible was more affected (63.1%) than the maxilla (36.9%). Marsupialization was applied to 30.3% of the cases, while enucleation was carried out in 69.7%. Conclusions: The significance of meticulous examination of radiographs and the consequences associated with undetected and untreated ailments is affirmed by this case study. A comprehensive understanding of oral pathology serves as a valuable resource for dentists, facilitating accurate diagnosis, appropriate referrals, and the provision of anticipatory guidance to patients striving to achieve optimal oral health across various age groups.
Pseudocysts of the Jaw: a Retrospective Study of 41 Cases from a Single Institution.

Abstract:Abstract Objective The purpose of this retrospective study was to investigate and compare the clinical, radiographic, pathological, pathogenesis, and therapeutic features of simple bone cysts (SBCs) and aneurysmal bone cysts (ABCs) of the jaw. Methods 35 patients with SBCs and 6 patients with ABCs who received treatment at the Department of Oral and Maxillofacial Surgery, Zhejiang University School of Medicine from 2017 to 2022 were followed up and reviewed retrospectively. Results The study included 41 patients, accounting for 2.14% of all jaw pathologies, with 35 patients having SBCs and 6 patients having ABCs; their average ages were 26.63 ± 13.62 years and 17.83 ± 7.88 years, respectively. The prevalence of SBC and ABC did not differ significantly by sex. The mandible was the most vulnerable area to be involved. Only 5.71% (2/35) of patients with SBCs and 16.7% (1/6) of patients with ABCs reported histories of previous trauma in the same region of the pseudocysts. A total of 42.86% (15/35) of SBC cases and 66.67% (4/6) of ABC cases had malocclusions. The radiographic features of pseudocysts varied in shape, were associated with the root, and unilocular or multilocular. All patients had curettage with or without bone graft or substitute implantation, and recurrences did not occur in 94.29% (33/35) of SBC patients and 100% (6/6) of ABC patients after a mean follow-up time of 26.23 ± 15.47 months and 21.67 ± 19.75 months, respectively. Conclusions Pseudocysts, including SBCs and ABCs, are benign osteolytic lesions without an epithelial lining that occur occasionally in the jaw, mostly in adolescents and young adults, and their incidence did not significantly differ by sex. The most vulnerable site of involvement is the mandible, and they are generally not overtly aggressive. Trauma has a less significant role in pseudocysts, but minor trauma, such as malocclusion, has the potential to influence pseudocyst development. The clinical presentation of pseudocysts lacks specificity, and most patients are asymptomatic and found incidentally during radiographs. Dental panoramic radiographs and CBCT cannot accurately distinguish between SBC and ABC, and the final diagnosis depends on pathological diagnosis. Curettage combined with bone grafting is currently the best treatment for both, with a 5.71% (2/35) recurrence rate for SBC and no recurrence found for ABC.

Abstract:The dentigerous cyst is the second most frequent odontogenic cyst after the root, being typically asymptomatic and commonly incidentally diagnosed. The objective of this case was to describe a successful treatment of a dentigerous cyst associated with an unerupted lower third molar, which extended from the condyle to the premolar region, treated by decompression followed by surgical enucleation. That said, a 19-year-old female patient, black, without underlying pathologies, came to the dental office for extraction of the third molars, and after a preoperative orthopantomographic radiography, the displacement of the dental element was noted 38 for the condylar region, unilocular cystic lesion occupying the branch and mandibular body region, root resorption of the dental elements involved in the region and rejection of the mandibular canal. After cytopathological analysis of the material collected through fine needle aspiration, the diagnosis of infected dentigerous cyst was confirmed. Afterwards, extraction and decompression were performed followed by surgical enucleation. Thus, it is concluded that the treatment performed promoted a satisfactory result, given a spontaneous bone neoformation after decompression and surgical enucleation of the lesion.

Abstract:Three rows of six evenly spaced 0.5 mm metal beads were implanted midsagittally into the discs of ten L4–5 human lumbar motion segments. The intradiscal bead displacements in response to compression, flexion, and extension loads were obtained by digitizing the bead positions from sagittal plane radiographs taken before and during the load application. Each disc was denucleated and the loading process was repeated. For the intact discs, in compression, the intradiscal bead displacements were predominantly anterior. In flexion, the beads in the center of the disc moved posteriorly whereas the beads closer to the periphery of the disc moved anteriorly. In extension, the central beads moved anteriorly and the beads closer to the periphery of the disc moved posteriorly. After denucleation, the bead displacements for compression and flexion implied an inward bulging of the inner wall of the annulus, despite outward bulging of the disc surface. We hypothesize that the inward bulging causes radial tensile stresses within the disc, leading to disruption of adjacent layers of annulus.

Abstract:Dentigerous cysts involves erupted or developing teeth in decreasing order of frequency as mandibular third molars, the maxillary canines, the maxillary third molars with rare involvement of the central incisors & are usually asymptomatic but becomes symptomatic with symptoms such as swelling, mild sensitivity, tooth mobility and displacement when it reaches size > 2 cm in diameter. Dentigerous cysts are seen most commonly in 20–30 years age group with relative low frequency in children with proportion of 4–9% of these cysts occur in the first 10 years after birth. Hospital based study conducted in the Department of ENT at Ashwini Rural medical college & Hospital, Solapur in which we have reported a series of three cases of dentigerous cyst in paediatric patients. Dentigerous Cysts in our study were associated with maxillary permanent lateral incisor, mandibular permanent premolar, Mandibular second molar tooth in pediatric age group. Provisional diagnosis of Dentigerous cyst based on clinical, radiological & biochemical study but Histopathological examination confirms the diagnosis. Dental orthopentamogram aids in diagnosing and localizing the Dentigerous cyst and ct face in all views helps to see bony effects and complications due to expansion of Dentigerous cyst. Enucleation of the cystic lesion followed by extraction of the offending teeth is a suitable treatment modality for Dentigerous cyst associated with maxilla and mandible giving adequate balance between complete removal of cyst & giving chance for eruption of unerupted teeth.

Abstract:The most common kind of developing odontogenic cysts are dentigerous cysts. They frequently push impacted teeth into ectopic locations by encircling the crown of the tooth. The mandibular third molar, maxillary canine, mandibular premolar, and maxillary third molar are the teeth that are most commonly afflicted, in decreasing order. Although these cysts are more prevalent in men, they can also occur in children and teenagers, especially in the second and third decades of life. Dentigerous cyst-related teeth in the maxilla frequently cause root dilatation in the mandibular area, permanent tooth bud displacement close to the sinus, and cortical bone enlargement, sometimes, even up to the level of the orbit or floor of the nose. Clinically, these cysts do not cause any symptoms and are frequently found by chance during radiographs done to look into cases of primary teeth that are retained, teeth that are missing, teeth that erupt slowly, or orthodontic issues. Cone-beam computed tomography’s advent has just made it possible to shed fresh, extensively documented light on diagnostic and treatment approaches. Protocol for management: Surgery is typically used to treat dentigerous cysts. In this instance, the affected tooth underwent endodontic therapy, after which the cyst was curettaged, a further apicoectomy was performed, and canine impaction was completed. In the end, a bone graft and platelet-rich fibrin were implanted for rehabilitation.

Abstract:Dentigerous cyst is the second most common odontogenic cyst and constitutes around 20-24% of all the odontogenic cysts involving the jaws. Usually, these cysts remain asymptomatic and rarely cause enlargement and displacement of associated tooth. In this paper, we present a case of a displaced mandibular third molar, which was associated with a large dentigerous cyst. A conservative treatment modality was adopted with a 2 year radiographic follow-up of the patient. A brief review of previously reported cases with dentigerous cyst causing displacement of the mandibular impacted third molars is also discussed, highlighting the treatment modality followed in all these cases.

Abstract:Dentigerous cysts are rarely found in children during the first decade of life. A 2-year-old male was referred to the Oral and Maxillofacial Surgery Service due to a complaint of facial swelling persisting for 12 months. His medical and family history indicated hyperactivity disorder and continuous use of phenobarbital. During the oral examination, a swelling was observed in the mandibular left posterior region, which obscured the buccal sulcus. An orthopantomogram revealed a well-circumscribed radiolucency with a developing molar tooth inside the lesion, measuring 5.0 cm.

Abstract:The absence of a tooth in the oral cavity may be the result of some obstruction with the process of tooth eruption. Physical obstruction can result from many different causes, such as supernumerary teeth, mucosal barrier, scar tissue, and cysts or tumors. Their removal will usually permit the affected tooth to erupt. This case report describes the delayed eruption of the permanent mandibular first molar in a 9-year-old boy due to the dentigerous cyst associated with it. The management was done through a multidisciplinary approach. Surgical removal of cystic sac through marsupialization along with the removal of the barrier was done. As a consequence of delayed eruption of the permanent first molar, the opposing tooth had supraerupted, which was intruded using mini-implants. Following this, the tooth erupted satisfactorily into the oral cavity to establish class I molar relation.

Abstract:In children, large odontogenic cysts affect adjacent anatomical structures as well as displace developing permanent teeth. Odontogenic cysts are treated via enucleation or marsupialization. This case reports a 5-year-old boy and a 10-year-old boy who have not only displaced 3 or more permanent teeth but also elevated the maxillary sinus floor due to the large size of the cyst in the maxillary anterior region. In both cases, marsupialization was selected to minimize complications. After marsupialization, a customized acrylic obturator, window opening, and orthodontic traction for eruption guidance were gradually attempted, and it showed a good prognosis, so we report these cases.

Abstract:The dentigerous cyst is defined as a pathological cavity whose origin is produced by developmental alterations. Most usual treatment is the enucleation of the cyst with the extraction of the tooth. The current approach tends to preserve the tooth. The aim of this study was to determine the total and partial success of the conservative treatment of the dentigerous cyst in terms of eruption of the associated permanent tooth. Pub-Med, Scopus and the Cochrane Central Register of Controlled Trials were searched from January 2012 to December 2023, including patients aged 18 years old or less with a dentigerous cyst associated with a repositionable permanent tooth in the arch. 118 articles were found and 24 were included in full text. 40 cases were reported in the mandible (83 %) and 8 in the maxilla (17 %). 29 cases of marsupialization were included (60 %), 17 of enucleation (36 %) and 2 of decompression (4 %). The percentage of teeth that erupted spontaneously, either completely or partially, after marsupialization, enucleation and decompression was 83 %, 59 % and 100 % respectively. No recurrences have been described in any case. This study highlights that the conservative treatment was a predictable procedure with a total success of 83 % by marsupialization and 100 % after decompression.

Abstract:The dentigerous cyst is an odontogenic cyst that typically associates with the crown of an unerupted permanent tooth. A 38-year-old male patient was referred by the orthodontist due to a bone lesion in the region of tooth 38. The patient reported numbness and tenderness in the mandible. Imaging exams revealed tooth 38 in an inverted vertical position and a radiolucent lesion encompassing the crown, starting from the cementoenamel junction and extending to the mesial root of tooth 37 and the base of the mandible.

Abstract:The insulin-binding isotherms and the structural composition of human liver insulin receptors were examined by using plasma membranes that were prepared from liver biopsies of nine non-obese and 10 obese subjects undergoing elective surgery. The insulin-binding characteristics of liver membranes from non-obese subjects were quite similar to those previously described in rat liver membranes. However, when the membranes from obese subjects were compared with the non-obese group, insulin-binding activity was reduced by 50% (P less than 0.01). The reduction in obesity resulted primarily from a decrease in total receptor number, although a small decrease in receptor affinity was also observed. Insulin binding was not correlated with sex or with the fasting plasma insulin level. The insulin-binding sites of liver membranes were affinity-labeled with 125I-insulin and the cross-linking reagent, disuccinimidyl suberate. The liver membranes from both the non-obese and the obese group had heterogenous (nonreduced) insulin-binding species of 300,000, 260,000, and 150,000 mol wt, which were again comparable to the findings reported in rat liver. Sulfhydryl reduction demonstrated a major sub-unit of 125,000 and a minor component of 40,000-45,000 in both groups. These results indicate a close similarity between the hepatic insulin receptor of man and the more intensely studied rat hepatic receptor. Obesity in human subjects is associated with a loss of hepatic insulin receptors. This alteration may contribute to the insulin resistance reported in this organ as well as to obesity-mediated glucose tolerance.

Abstract:Introduction: Nasopalatine duct cyst (NDC) is the most prevalent non-odontogenic cyst emerging from the epithelial remnants in the maxillary incisive canal. A sublabial or transpalatal approach is performed to enucleate NDC completely. More recently, transnasal endoscopic marsupialization has been used gradually. Case presentation: A 24-year-old male patient with a large nasopalatine duct cyst with a diameter of 51 mm was managed by transnasal endoscopic marsupialization under general anesthesia. The presentation involves painless swelling around the left side of the anterior maxilla and bulging of the hard palate. No postoperative complications were observed after a 3-month follow-up. Transnasal endoscopic marsupialization is a minimally invasive surgery for large NDC. Clinical discussion: Approximately 1% of the population has a nasopalatine duct cyst. Surgical treatment was carried out under general anesthesia; the cyst was dissected and removed using a typically transnasal endoscopic marsupialization technique. Conclusion: The cause of the NDC is unclear. Simple surgical resection and clinical and radiological control are recommended to ensure the case is resolved correctly.

Abstract:BACKGROUND: Dentigerous cyst (DC) is one of the most common developmental odontogenic cysts. This hospital-based retrospective study evaluated the clinical and histopathological features of 338 DCs in Taiwanese patients. METHODS: In this study, 338 consecutive cases of DCs were collected from January 1995 to December 2009. The demographic data and clinicopathological features of these DC cases were reviewed and analyzed. RESULTS: The 338 DCs were taken from 332 patients (212 men and 120 women, mean age 33.0 years, range 5-83 years). There was a significant difference in the mean age between male and female patients (P < 0.05). Of the 338 DCs, 122 were found in the maxilla and 216 in the mandible. The most common site for DCs was the mandibular molar region (169 cases) and the most frequently involved tooth was the mandibular third molar (153 cases). Forty-six of the 338 DCs were found to be associated with supernumerary teeth. Microscopically, 317 cysts were lined by stratified squamous epithelium, 9 by mucoepidermoid-typed epithelium, and 12 by ciliated pseudostratified columnar epithelium. Hyaline bodies of Rushton, odontogenic epithelial rests, cholesterol clefts, foamy histiocytes, hemosiderin-laden macrophages, and dystrophic calcifications were discovered in 11, 78, 90, 33, 62 and 45 DCs, respectively. CONCLUSION: Our results indicate that DCs of Taiwanese patients have a male predominance and occur more frequently in patients between 10 and 29 years of age. The most commonly affected site for DCs is the mandibular molar region and the most frequently involved tooth is the mandibular third molar.
