Abstract:Those GPs of my generation (I qualified in 1982) will not be surprised by the benefits of a good health visiting service that allows frequent contact and continuity of care.1 I remember it well and valued it hugely, as did my patients. It's not just good for children, though. Health visitors are often the first to recognise post-natal depression and provide essential support and safeguarding for mothers being treated in the community. They also have a unique insight into family dynamics and can help mothers at risk of domestic violence. This is much more likely to happen if health visitors and mothers know each other well.

Abstract:But the future looks bleak for local scrutiny of health organisations The public inquiry into Mid Staffordshire NHS Foundation Trust has left few reputations undamaged. But amid the wreckage two emerge with credit: the group Cure the NHS (www.curethenhs.co.uk), set up by patients, relatives, and friends to draw attention to failings at Stafford Hospital, and a local journalist, Shaun Lintern, who gave them a public voice. The rest—boards, regulators, managers, and most doctors and nurses—failed in their professional duty, some more egregiously than others. In identifying failure it was the outsiders who proved to be the true professionals. This is a sobering conclusion, because the past decade has seen a systematic failure to involve local people effectively in the NHS. Since the abolition of the community health councils in 2002 under Tony Blair’s government, each effort to replace them has proved feebler than the last. The councils were replaced firstly by patient and public involvement forums and then, in 2005, by local involvement networks (LINks). With the unconscious irony for which NHS publications are rightly celebrated, the document setting up LINks was entitled A Stronger Local Voice . Now a new plan is afoot to replace LINks with Healthwatch, but with little evidence that any lessons have been learnt. Abstract:Abstract The National Programme for IT (NPfIT) was launched in 2005 to implement 7 nationwide IT services across the National Health Service (NHS). Despite the success of many of these designated “deliverables,” the establishment of a single nationwide electronic health record (EHR) system never fully materialized. As a result, NHS medical records are now stored using a diverse array of alternate EHR systems, which frequently restricts health care practitioners from accessing extensive portions of their patients’ notes. This not only limits their ability to make well-informed clinical decisions but also impacts the quality of care they are able to provide. This article assesses the medical, economic, and bureaucratic implications of an NHS-wide EHR system. Additionally, it explores how the shortcomings of the NPfIT should be addressed when attempting to introduce such a system in the future.

Abstract:A doctor who left the medical records of about 300 patients unsecured in a rented apartment when she moved out has been struck off the UK medical register.Documents found in the former home of Maryam Mohamed Al Qureshi in Southampton included records of abortions and overdoses and of psychiatric treatment, as well as patients' names, addresses, and birth dates. The documents, from eight different healthcare providers, covered dates from 1996 to 2013.Mohamed Al Qureshi, who was known as Victoria Pickles when she qualified from Leeds University in 2006 but changed her name in 2018, was already suspended after a 2022 medical practitioners tribunal found her culpable of failing to provide good clinical care when a patient called the NHS 111 hotline.That tribunal also found that she had failed to cooperate with a subsequent NHS inquiry and had made a dishonest statement during an appraisal. Street-level bureaucracy and policy implementation in community public health nursing: a qualitative study of the experiences of student and novice health visitors

Abstract:Aim To explore the experiences of student and novice health visitors in implementing health visiting policy reform pre- and post-qualification. Background In England, public health nursing has been subject to major policy reform. The Health Visitor Implementation Plan (2011) set out a plan to recruit increasing numbers of nurses and midwives to the profession to deliver an expanded and refocussed health visiting service. Exploring this policy change from the viewpoint of those new to health visiting offers a unique perspective into how a specific policy vision is translated into nursing practice. Methods A descriptive qualitative study in which participants were enrolled on a one-year post-graduate health visiting course at a University in South West of England. Qualitative data were collected pre- and post-qualification. A total of 16 interviews and a focus group were conducted with nine participants between September 2012 and March 2013. Findings Descriptive data were interpreted using Lipsky’s theoretical framework of street-level bureaucracy. Three themes emerged which relate to this ‘bottom-up’ perspective on policy implementation; readiness to operationalise policy, challenges in delivering the service vision; and using discretion in delivering the vision. Community public health nurses operate as street-level bureaucrats in negotiating the demands of policy and practice, and by this means, attempt to reconcile professional values with institutional constraints. Barriers to policy implementation at a local level mediate the effects of policy reform, ultimately impacting upon outcomes for children and families.

Abstract:In today's competitive market infection control teams are under increasing pressure from purchasing authorities to provide hard data on quality issues. The initial aim of this study was to determine whether a modified form of the audit system used to monitor infection control standards in Derbyshire could be used effectively across three district health authorities. Infection control nurses in each of the study districts have different approaches to training with only Stafford using a link nurse system. Our additional aim therefore was to examine the relationship between the knowledge base of nursing staff and their ward audit score, and to determine whether wards with link nurses achieved significantly higher scores. To establish this a questionnaire was devised and completed by three different grades of staff on each ward at the time of the audit. These results were fed back to the wards and to managers by the infection control nurse, and an action plan given with a review date where applicable. Results of the study indicated that the audit tool could be used effectively in any district but would need modifying for psychiatric departments. The value of education in improving standards of infection control was clearly demonstrated. An important finding in Stafford was that wards with link nurses obtained significantly higher scores. The value of using such a clinically based audit will be discussed.

Abstract:The Liverpool care pathway for patients needing care at the end of life, though well intentioned and often admirably carried out, should be scrapped in favour of individualised care plans, says an independent review immediately accepted by the UK government. Too often, the review found, the pathway was applied with a lack of expert knowledge and care, its advice turned into a set of rigid instructions by staff who had not been properly trained.1 Its very name suggested that it was a pathway with only one possible destination, “a conveyer belt to death,” it said. Hospitals were rewarded financially for using the pathway, creating the impression that they were hastening death for financial gain. All this must change, the review concludes. The review, chaired by the rabbi and peer Julia Neuberger, makes few direct criticisms of the pathway itself apart from its name. In the right environment, and with the right staff, it had enabled many people to have a better death than might otherwise have been the case. Simplifying Nursing Record

Abstract:Objective: To explore the strategy to simplify nursing record and shorten indirect nursing time through designing and using the nursing record chart of admission and discharge. Methods: We sampled 600 nurses from our hospital and divided them into two groups, 300 nurses in each group. The nurses in experimental group implemented the new nursing record chart method. Meanwhile, the nurses in control group used traditional method. Then we compared the nursing record quality and time to write them. Results: Nurses in experimental group got higher nursing record quality score and spent less time to write them than control group (P 0.01). Conclusion: The application of nursing record chart of admission and discharge could reduce the time in writing nursing record, simplify nursing document and improve nursing care quality.

Abstract:The effect of electron irradiation-induced shrinking on glass nanocapillaries with diameters ranging from 75 to 14 nm was analyzed by measuring the conductance characteristics with and without DNA translocation. We have investigated nanocapillary shrinking with a scanning electron microscope from several perspectives to understand the geometry of the shrunken nanocapillary. On the basis of this observation, the conductance was modeled with respect to the nanocapillary diameter, which allowed reproducing the experimental results. We then translocated DNA through the shrunken nanocapillaries and measured higher conductance drops for smaller diameters, reaching 1.7 nS for the 14 nm diameter nanocapillary. A model taking into account the conical shape of the shrunken nanocapillaries also supported this dependence. Next, we calculated the noise in the form of the standard deviation of the ionic conductance (between 0.04 and 0.15 nS) to calculate a signal-to-noise ratio (SNR) and compared it with nanopores embedded in 20 nm thick silicon nitride membranes. This shows that although nanocapillaries have smaller signal amplitudes due to their conical shape, they benefit from a lower noise. The glass nanocapillaries have a good SNR of about 25 compared with the SNR of 15 for smaller sized nanopores in silicon nitride membranes. The ability to use a modified model of nanopores to mimic the block conductance by DNA translocation provides a theoretical framework to support experimental results from translocating polymers such as DNA.
Child health nurses' perceptions of the Family Community-based Assistance Resourcing and Education program in contemporary practice: a qualitative study

Abstract:The Family Community-based Assistance Resourcing and Education Program (FCP) is a nurse home visiting program that was introduced in Queensland two decades ago to redress health inequalities for infants from families experiencing specific social stressors. Locally adapted versions of this home visiting program are still in use, but have not been evaluated. This study examined child health nurse perceptions of the adapted FCP in one regional Queensland health service. A qualitative descriptive exploratory study using two focus groups (conducted May 2019) with Child Health Nurses who delivered the FCP was conducted. Transcripts of digital recordings were analysed using Braun and Clarke's (2006) six-step framework for guided thematic analysis. A total of 16 Child Health Nurses participated in the study, with a mean of 10 years' experience with the program. Data analysis generated 12 themes organised under three domains: 'Establishing the relationship with families', 'What works in practice' and 'We could do it better'. Participants cited flexibility, expert input and in-home delivery as key program benefits. However, narrow eligibility criteria, poor screening for perinatal anxiety and resourcing constraints were identified as limitations. This study is the first to measure Child Health Nurses' perceptions of an adapted FCP. It sheds light on their 'practice wisdom', including the program's ability to meet the needs of families with social vulnerabilities. The study supports prior calls for home visiting programs to be evaluated against clearly stated program intentions. Participant insights have been shared to inform practice and program implementation both locally and as part of Queensland's First 2000 Days health service delivery reform agenda.

Abstract:Background: Low socio-economic status can lead to poor patient outcomes, exacerbated by lack of integration between health and social care and there is a demand for developing new models of working. Aim: To improve connections between patients, local services and their communities to reduce unscheduled admissions. Design and setting: A primary care cluster with areas of high deprivation, consisting of 11 general practices serving over 74,000 people. Method: A multi-disciplinary team with representatives from healthcare, local council and the third sector was formed to provide support for people with complex or social needs. A discharge liaison hub contacted patients following hospital discharge offering support, while cluster pharmacists led medicine reviews. Wellbeing Connectors were commissioned to act as a link to local wellbeing and social resources. Advance Care Planning was implemented to support personalised decision making. Results: Unscheduled admissions in the over 75 age group decreased following the changes, equating to over 800 avoided monthly referrals to assessment units for the cluster. Over 2,500 patients have been reviewed by the MDT since its inception with referrals to social prescribing groups, physiotherapy and mental health teams; these patients are 20% less likely to contact their GP after their case is discussed. An improved sense of wellbeing was reported by 80% of patients supported by wellbeing connectors. Staff feel better able to meet patient needs and reported an increased joy in working. Conclusion: Improved integration between health, social care and third sector has led to a reduction in admissions, improved patient wellbeing and has improved job satisfaction amongst staff.
Nursing Attitudes and Practices in Code Documentation Employing a New Electronic Health Record

Abstract:Abstract Objective Based on feedback from nurses regarding the challenges of code documentation following the implementation of a new electronic health record (EHR), we sought to better understand inpatient nurse attitudes and practices in code documentation and to identify opportunities for improvement. Methods An anonymous electronic survey was distributed to all inpatient nurses working at a single, 999-bed, university-based, and quaternary care hospital. Participation in the study was voluntary and consent was implied by survey completion. Results Overall, 432 (14%) of 3,121 inpatient nurses completed the survey. While nearly 80% of respondents indicated feeling very comfortable using computers for personal use, only 5% felt very comfortable navigating the EHR to document codes in real time. While 53% had documented codes in the new EHR, most admitted to documenting on paper with retroactive entry into the EHR. About 25% reported having participated in a code that was not accurately documented in the new EHR. All respondents provided specific suggestions for improving the EHR interface, and over 90% expressed interest in having opportunities to practice code documentation using simulated code events. Conclusion Despite completion of training modules in code documentation in a new EHR, many inpatient nurses in a single institution feel uncomfortable documenting codes directly into the EHR, and some question the accuracy of this documentation. Improving EHR functionality based on specific recommendations from end-users coupled with more practice documenting simulated codes may ease EHR navigation, leading to nurses' acceptance of the EHR tool, more accurate and efficient documentation, greater nurse satisfaction and more appropriate quality improvement measures.

Abstract:André Spicer, professor of organisational behaviour at London's Bayes Business School, has written recently in the Guardian about how work wellness programmes don't make employees happier.1 And many employees don't seem happy at all, especially in the NHS. Its 2022 staff survey (with over 600 000 responses) showed some of the worst scores since it began collecting data,2 and three in 10 were considering leaving.3 This is mirrored by national data showing a drop in patient and public satisfaction with the NHS.45Almost three quarters of staff survey respondents said that they worked in NHS organisations with "too few colleagues to allow them to do their job properly." Only 27% said that "staffing was sufficient" at their organisation, and only 68% were "happy with the standard of care their organisation provided." Abstract:Continuing education for nurses is an expensive commodity. In order to justify its expense employers must experience tangible benefits to nursing practice. This paper describes a quantitative study carried out in an in-service training organisation in Northern Ireland. The aim of this study was to pilot a tool, which considered factors associated with successful completion of the record of practice documentation following training in peripheral intravenous cannulation. The questionnaire was sent to all course participants for a single year (n=344). The response rate of was 28% (n=97). Seventy two percent of nurses successfully cannulated on at least five occasions with most completing the record of practice form within one month. There were no significant differences found between nursing grades in relation to the time taken to complete the record of practice document (p=0.395, Kruskal-Wallis, chi(2)=1.860, df=2). There was highly statistically significant correlation found in relation to highest academic grade and the time taken to complete the record of practice (p<0.001, r=0.121) as illustrated in Fig. 3. However, the strength of the relationship was weak as the two variables only shared 1.4% of their variation in common. The difference between the grades achieved by hospital and community nurses in time taken to complete the record of practice was statistically significant (p=0.003, Mann Whitney U=470.000, Z=-3.023) with hospital nurses more likely to complete their supervised practice and to do so more quickly.

The extent and application of patient diaries in Australian intensive care units: a national survey

Abstract:Background: Critical illness can be a psychological stressor for patients and family members. Scandinavian studies have portrayed patient diaries as a supporting catalyst for patients and their families through a process of reflection and fact presentation. However, the small number of randomisation trials exhibits theoretical and methodological limitations. Given the increased interest amongst Australian Critical Care Nurses, further understanding of nurses' perceptions and use of patient diaries was warranted. Objectives: To identify nurses' perceptions of patient diary use and to describe their application within Australian ICUs. Method: A descriptive, exploratory study design using an online survey of the Australian College of Critical Care Nurses was used. Descriptive statistics illustrated participant responses and textual data were subjected to content analysis for emergent themes. Results: Out of the 194 participants, 19% (n=37) reported patient diary use in their unit, with nine indicating some form of evaluation. Fifty (70%) of the non-diary users, who responded, indicated hesitancy to introduce them. Respondents noted a lack of guiding policies and potential medico-legal ambiguity. Content analysis revealed three themes, considering diaries as: communication tool, patient recovery tool and a family centred care strategy. Conclusion: Patient diary use appears to be in its infancy in Australia with a lack of guiding policy for format and processes including evaluation. Nurses using diaries were enthusiastic about the initiative, holding optimistic perceptions about the potential positive impact of diaries on patient and family outcomes. Further research on their efficacy is required.
A mixed-methods survey to explore views of staff and patients from mental health wards prior to introduction of a digital early warning system for physical deterioration

Abstract:WHAT IS KNOWN ON THE SUBJECT?: The potential benefits of introducing technological innovation into all types of health services are recognized internationally. There are few studies exploring the use of technology in inpatient mental health settings, or the views of staff and patients regarding such developments. "Early warning systems" are increasingly used in inpatient mental health services to detect physical deterioration in patients and prompt staff to take appropriate action. We have identified no peer-reviewed publications concerning this development. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: Using a questionnaire survey, this project gathered views from staff and patients on a planned change from a paper-based "early warning system" to one using handheld electronic devices (tablets). This study adds to the existing evidence by reporting specific staff and patient concerns about a technological development in an inpatient mental health setting. Some patients were concerned about confidentiality of data entered onto tablets. Whilst were either positive or neutral in attitude to the planned change some staff raised concerns that electronic devices could be used as weapons, and also that patients or visitors might misinterpret their use. WHAT ARE THE IMPLICATIONS FOR PRACTICE?: Views of staff and patients must be considered when introducing new technology into clinical practice, in order to make the change successful and sustainable. Inadequate or unclear information about the use and purpose of electronic devices may lead to misunderstandings as to their purpose and the security of their data, particularly in mental health settings. Further exploration across a range of services and countries would be useful in determining whether attitudes towards implementing similar technological change in mental health practice are commonly shared. ABSTRACT: Introduction Technological innovation offers opportunities to improve mental health care; however, little evidence exists regarding attitudes of inpatient staff and patients to such changes. We present a survey of staff and patients prior to introduction of a digital version of the National Early Warning Score (eNEWS) system for identifying physical deterioration. Aim To collate views of staff and inpatients related to prospective use of eNEWS, to inform the plan for implementation. Method Paper questionnaires were distributed to both groups in six wards prior to eNEWS implementation. Two discussion groups were then held. Results Eighty-two staff and 26 inpatients completed questionnaires. Some inpatients expressed concerns about data confidentiality. Most staff were neutral or positive about the planned change, but raised possible safety risks and the risk of electronic recording being misinterpreted by patients. The implementation plan was modified in response to this information, principally by improving communication processes with patients. Discussion This study adds to the existing evidence by reporting specific staff and patient concerns towards a form of information technology. Further evaluations would help determine the transferability of these findings. Implications for practice Listening to patient and staff views about planned technological innovation is essential for effective implementation.
Nurses engaging with referral letters and discharge summaries: A qualitative study

Abstract:Aims To investigate the ways that nurses engage with referral letters and discharge summaries, and the qualities of these documents they find valuable for safe and effective practice. Design This study comprised a qualitative, case‐study design within a constructivist paradigm using convenience sampling. Methods Interviews were conducted with nurses to investigate their practices relating to referral letters and discharge summaries. Data collection also involved nurses' examination and evaluation of a diverse range of 10 referral letters and discharge summaries from medical records at two Australian hospitals through focus‐group sessions. The data were transcribed and analysed inductively. Results In all, 67 nurses participated in interviews or focus groups. Nurses indicated they used referral letters and discharge summaries to inform their work when caring for patients at different times throughout their hospitalisation. These documents assisted them with verbal handovers, to enable them to educate patients about their condition and treatment and to provide a high standard of care. The qualities of referral letters and discharge summaries that they most valued were language and communication, an awareness of audience and clinical knowledge, as well as balancing conciseness with comprehensiveness of information. Conclusion Nurses relied on referral letters and discharge summaries to ensure safe and effective patient care. They used these documents to enhance their verbal handovers, contribute to patient care and to educate the patient about their condition and treatment. They identified several qualities of these documents that assisted them in maintaining patient safety including clarity and conciseness of information. Implications for the Profession and Patient Care It is important that referral letters and discharge summaries are written clearly, concisely and comprehensively because nurses use them as key sources of evidence in planning and delivering care, and in communicating with other health professionals in relaying goals of care and implementing treatment plans. Impact Nurses reported that they regularly used referral letters and discharge summaries as valuable sources of evidence throughout their patients' hospitalisation. The qualities of these documents which they most valued were language and communication styles, awareness of audience and clinical knowledge, as well as balancing conciseness with comprehensiveness of information. This research has important impact on the patient experience in relation to encouraging effective referral letter and discharge summary writing. Abstract:Background and aim Communication is a key factor in intraprofessional collaboration between hospital nurses and homecare nurses in hospital‐to‐home transitions of older patients with complex care needs. Gaining knowledge of the nature of cross‐sectoral communication is crucial for understanding how nurses collaborate to ensure a seamless patient trajectory. This study explores how cross‐sectoral electronic health records communication influences collaboration between hospital nurses and homecare nurses when discharging older patients with complex care needs. Method The study is based on qualitative group interviews with six hospital nurses and 14 homecare nurses working at different hospitals and municipalities across Denmark. Data were analysed using reflexive thematic analysis, as described by Braun and Clark. Findings The themes Collecting pieces for the 'puzzle': Losing the holistic picture of the patient; Working blindfolded: limited provision of and access to critical information; and Bypassing the 'invisible wall': dialogue supports cohesion illustrate the impact of organisational structures within electronic health records have on hospital nurses' and homecare nurses' intraprofessional collaboration across sectors. Challenges with predefined and word‐limited elements in digital communication, and inadequate and limited access to significant medical information were identified. To compensate for the inadequacy of the electronic health records, direct contact and dialogue were emphasised as ways of fostering successful collaboration and overcoming the barriers created by electronic health records. Conclusion Despite hospital nurses' and homecare nurses' desire to conduct holistic patient assessments, their ability to collaborate was hindered by failures in electronic health record communication resulting from restrictive organisational structures across sectors. Thus, it became necessary for hospital nurses and homecare nurses to bypass the electronic health record system and engage in dialogue to provide holistic care when discharging older patients with complex care needs. However, by hospital nurses and homecare nurses compensating for counter‐productive organisational structures, problems brought about by the electronic health record system paradoxically remain invisible.

Abstract:NHS England has announced that two general practice retention schemes will end in March, in a move criticised by the Royal College of General Practitioners.The General Practice Fellowship—a two year programme that provides support, mentorship, and learning opportunities to newly qualified GPs and nurses who are new to general practice—is one of the schemes set to be cut. One of the programme's stated aims is to "increase and maintain high levels of participation in the primary care workforce."1The other scheme being axed is the Supporting Mentors Scheme, which enables integrated care boards to upskill experienced GPs to mentor newer recruits.
