Abstract:Background: Dental caries is a prevalent, complex, chronic illness that is avoidable. Better dental health outcomes are achieved as a result of accurate and early caries risk prediction in children, which also helps to avoid additional expenses and repercussions. In recent years, artificial intelligence (AI) has been employed in the medical field to aid in the diagnosis and treatment of medical diseases. This technology is a critical tool for the early prediction of the risk of developing caries. Aim: Through the development of computational models and the use of machine learning classification techniques, we investigated the potential for dental caries factors and lifestyle among children under the age of five. Design: A total of 780 parents and their children under the age of five made up the sample. To build a classification model with high accuracy to predict caries risk in 0–5-year-old children, ten different machine learning modelling techniques (DT, XGBoost, KNN, LR, MLP, RF, SVM (linear, rbf, poly, sigmoid)) and two assessment methods (Leave-One-Out and K-fold) were utilised. The best classification model for caries risk prediction was chosen by analysing each classification model's accuracy, specificity, and sensitivity. Results: Machine learning helped with the creation of computer algorithms that could take a variety of parameters into account, as well as the identification of risk factors for childhood caries. The performance of the classifier is almost unbiased, making it generalizable. Among all applied machine learning algorithms, Multilayer Perceptron and Random Forest had the best accuracy, with 97.4%. Support Vector Machine with RBF Kernel (with an accuracy of 97.4%) was better than Extreme Gradient Boosting (with 94.9% accuracy). Conclusion: The outcomes of this study show the potential of regular screening of children for caries risk by experts and finding the risk scores of dental caries for any individual. Therefore, in order to avoid dental caries, it is possible to concentrate on each individual by utilizing machine learning modelling.

Abstract:Dental caries is an infectious disease that deteriorates the tooth structure, with tooth cavities as the most common result. Classified as one of the most prevalent oral health issues, research on dental caries has been carried out for early detection due to pain and cost of treatment. Medical research in oral healthcare has shown limitations such as considerable funds and time required; therefore, artificial intelligence has been used in recent years to develop models that can predict the risk of dental caries. The data used in our study were collected from a children’s oral health survey conducted in 2018 by the Korean Center for Disease Control and Prevention. Several Machine Learning algorithms were applied to this data, and their performances were evaluated using accuracy, F1-score, precision, and recall. Random forest has achieved the highest performance compared to other machine learnings methods, with an accuracy of 92%, F1-score of 90%, precision of 94%, and recall of 87%. The results of the proposed paper show that ML is highly recommended for dental professionals in assisting them in decision making for the early detection and treatment of dental caries.
Using a Machine Learning Algorithm to Predict the Likelihood of Presence of Dental Caries among Children Aged 2 to 7

Abstract:Background: Dental caries is the most common chronic childhood infectious disease and is a serious public health problem affecting both developing and industrialized countries, yet it is preventable in most cases. This study evaluated the potential of screening for dental caries among children using a machine learning algorithm applied to parent perceptions of their child’s oral health assessed by survey. Methods: The sample consisted of 182 parents/caregivers and their children 2–7 years of age living in Los Angeles County. Random forest (a machine learning algorithm) was used to identify survey items that were predictors of active caries and caries experience. We applied a three-fold cross-validation method. A threshold was determined by maximizing the sum of sensitivity and specificity conditional on the sensitivity of at least 70%. The importance of survey items to classifying active caries and caries experience was measured using mean decreased Gini (MDG) and mean decreased accuracy (MDA) coefficients. Results: Survey items that were strong predictors of active caries included parent’s age (MDG = 0.84; MDA = 1.97), unmet needs (MDG = 0.71; MDA = 2.06) and the child being African American (MDG = 0.38; MDA = 1.92). Survey items that were strong predictors of caries experience included parent’s age (MDG = 2.97; MDA = 4.74), child had an oral health problem in the past 12 months (MDG = 2.20; MDA = 4.04) and child had a tooth that hurt (MDG = 1.65; MDA = 3.84). Conclusion: Our findings demonstrate the potential of screening for active caries and caries experience among children using surveys answered by their parents.

Abstract:In this study, we developed machine learning-based prediction models for early childhood caries and compared their performances with the traditional regression model. We analyzed the data of 4195 children aged 1–5 years from the Korea National Health and Nutrition Examination Survey data (2007–2018). Moreover, we developed prediction models using the XGBoost (version 1.3.1), random forest, and LightGBM (version 3.1.1) algorithms in addition to logistic regression. Two different methods were applied for variable selection, including a regression-based backward elimination and a random forest-based permutation importance classifier. We compared the area under the receiver operating characteristic (AUROC) values and misclassification rates of the different models and observed that all four prediction models had AUROC values ranging between 0.774 and 0.785. Furthermore, no significant difference was observed between the AUROC values of the four models. Based on the results, we can confirm that both traditional logistic regression and ML-based models can show favorable performance and can be used to predict early childhood caries, identify ECC high-risk groups, and implement active preventive treatments. However, further research is essential to improving the performance of the prediction model using recent methods, such as deep learning.

Abstract:Background An easily accessible caries risk prediction model (CRPM) based on nonbiological predictors is lacking. Developing a CRPM for community screening is essential for children's dental health promotion by a public health approach. Objective This study aimed to develop and validate a caries risk prediction model (CRPM) for children using a machine learning algorithm based on dental care behavioral factors and other nonbiological factors using a 3-month multicenter cohort. Methods Children aged 12 months to 60 months were recruited at three primary care settings and three kindergartens in Chengdu, China. Dental examination was conducted for all enrolled children by calibrated pediatric dentists at baseline and three months later. All parents of the enrolled children were asked to complete a questionnaire with dental-related information. Machine learning algorithms, including random forest, logistic regression, and adaptive boosting, were used to develop a prediction model. Sensitivity, specificity, accuracy, precision, negative predictive value and F-score were reported to estimate the internal validation of the models. Results A total of 481 out of 745 children without a history of caries experience at baseline remained for analysis. In the total sample population, 236 (49.1%) children were female, and the mean age was 31.2 months. During the follow-up exams, 66 (13.6%) children had new-onset caries. The child's age, height, weight, family caries status, brush teeth two minutes per time, fluoride toothpaste usage, brushing twice per day, parental monitoring brushing teeth, mother delivery method, brushing child's teeth every day, child number counts, and night feeding frequency in the last month were measured and included in a prediction model. Of the prediction models, the highest area under the curve of RF was 0.91 (95% CI: 0.87- 0.94), followed by 0.86 (95% CI: 0.81-0.91) of LR and 0.81 (95% CI: 0.76-0.86) of AdaBoost. Conclusion In this CRPM, new onset of dental caries in three months among children aged < 60 months could be predicted by answering twelve nonbiological questions. A good model performance was shown within the internal validation. Dental home care could be improved by referring the CRPM result before new caries onset.

Abstract:Introduction and objectives: This study investigates key factors influencing dental caries risk in children aged 7 and under using machine learning techniques. By addressing dental caries' prevalence, it aims to enhance early identification and preventative strategies for high-risk individuals. Methods: Data from clinical examinations of 356 children were analyzed using Logistic Regression, Decision Trees, and Random Forests models. These models assessed the influence of dietary habits, fluoride exposure, and socio-economic status on caries risk, emphasizing accuracy, precision, recall, F1 score, and AUC metrics. Results: Poor oral hygiene, high sugary diet, and low fluoride exposure were identified as significant caries risk factors. The Random Forest model demonstrated superior performance, illustrating the potential of machine learning in complex health data analysis. Our SHAP analysis identified poor oral hygiene, high sugary diet, and low fluoride exposure as significant caries risk factors. Conclusion: Machine learning effectively identifies and quantifies dental caries risk factors in children. This approach supports targeted interventions and preventive measures, improving pediatric dental health outcomes. Clinical significance: By leveraging machine learning to pinpoint crucial caries risk factors, this research lays the groundwork for data-driven preventive strategies, potentially reducing caries prevalence and promoting better dental health in children.

Abstract:Background. Dental caries is one of the major oral health problems and is increasing rapidly among people of every age (children, men, and women). Deep learning, a field of Artificial Intelligence (AI), is a growing field nowadays and is commonly used in dentistry. AI is a reliable platform to make dental care better, smoother, and time-saving for professionals. AI helps the dentistry professionals to fulfil demands of patients and to ensure quality treatment and better oral health care. AI can also help in predicting failures of clinical cases and gives reliable solutions. In this way, it helps in reducing morbidity ratio and increasing quality treatment of dental problem in population. Objectives. The main objective of this study is to conduct a systematic review of studies concerning the association between dental caries and machine learning. The objective of this study is to design according to the PICO criteria. Materials and Methods. A systematic search for randomized trials was conducted under the guidelines of PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). In this study, e-search was conducted from four databases including PubMed, IEEE Xplore, Science Direct, and Google Scholar, and it involved studies from year 2008 to 2022. Result. This study fetched a total of 133 articles, from which twelve are selected for this systematic review. We analyzed different types of machine learning algorithms from which deep learning is widely used with dental caries images dataset. Neural Network Backpropagation algorithm, one of the deep learning algorithms, gives a maximum accuracy of 99%. Conclusion. In this systematic review, we concluded how deep learning has been applied to the images of teeth to diagnose the detection of dental caries with its three types (proximal, occlusal, and root caries). Considering our findings, further well-designed studies are needed to demonstrate the diagnosis of further types of dental caries that are based on progression (chronic, acute, and arrested), which tells us about the severity of caries, virginity of lesion, and extent of caries. Apart from dental caries, AI in the future will emerge as supreme technology to detect other diseases of oral region combinedly and comprehensively because AI will easily analyze big datasets that contain multiple records.

Abstract:Background Machine learning (ML) algorithms play a key role in estimating dental age. In this study, three ML models were used for dental age estimation, based on different preprocessing methods. Aim The seven mandibular teeth on the digital panorama were measured and evaluated according to the Cameriere and the Demirjian method, respectively. Correlation data were used for decision tree (DT), Bayesian ridge regression (BRR), k-nearest neighbors (KNN) models for dental age estimation. An accuracy comparison was made among different methods. Subjects and methods We analyzed 748 orthopantomographs (392 males and 356 females) from eastern China between the age of 5 and 13 years in this retrospective study. Three models, DT, BRR, and KNN, were used to estimate the dental age. The data in ML is obtained according to the Cameriere method and the Demirjian method. Coefficient of determination (R 2 ), mean error (ME), root mean square error (RMSE), mean square error (MSE) and mean absolute error (MAE), the above five metrics were used to evaluate the accuracy of age estimation. Results Our experimental results showed that the prediction accuracy of dental age was affected by ML algorithms. MD, MAD, MSE, RMSE of the dental age predicted by ML were significantly decreased. Among all the methods, the KNN model based on the Cameriere method had the highest accuracy (ME = 0.015, MAE = 0.473, MSE = 0.340, RMSE = 0.583, R 2 = 0.94). Conclusion The results show that the prediction accuracy of dental age is influenced by ML algorithms and preprocessing method. The KNN model based on the Cameriere method was able to infer dental age more accurately in a clinical setting.

Fairness of Machine Learning Algorithms for Predicting Foregone Preventive Dental Care for Adults

Abstract:Importance: Access to routine dental care prevents advanced dental disease and improves oral and overall health. Identifying individuals at risk of foregoing preventive dental care can direct prevention efforts toward high-risk populations. Objective: To predict foregone preventive dental care among adults overall and in sociodemographic subgroups and to assess the algorithmic fairness. Design, setting, and participants: This prognostic study was a secondary analyses of longitudinal data from the US Medical Expenditure Panel Survey (MEPS) from 2016 to 2019, each with 2 years of follow-up. Participants included adults aged 18 years and older. Data analysis was performed from December 2022 to June 2023. Exposure: A total of 50 predictors, including demographic and socioeconomic characteristics, health conditions, behaviors, and health services use, were assessed. Main outcomes and measures: The outcome of interest was foregoing preventive dental care, defined as either cleaning, general examination, or an appointment with the dental hygienist, in the past year. Results: Among 32 234 participants, the mean (SD) age was 48.5 (18.2) years and 17 386 participants (53.9%) were female; 1935 participants (6.0%) were Asian, 5138 participants (15.9%) were Black, 7681 participants (23.8%) were Hispanic, 16 503 participants (51.2%) were White, and 977 participants (3.0%) identified as other (eg, American Indian and Alaska Native) or multiple racial or ethnic groups. There were 21 083 (65.4%) individuals who missed preventive dental care in the past year. The algorithms demonstrated high performance, achieving an area under the receiver operating characteristic curve (AUC) of 0.84 (95% CI, 0.84-0.85) in the overall population. While the full sample model performed similarly when applied to White individuals and older adults (AUC, 0.88; 95% CI, 0.87-0.90), there was a loss of performance for other subgroups. Removing the subgroup-sensitive predictors (ie, race and ethnicity, age, and income) did not impact model performance. Models stratified by race and ethnicity performed similarly or worse than the full model for all groups, with the lowest performance for individuals who identified as other or multiple racial groups (AUC, 0.76; 95% CI, 0.70-0.81). Previous pattern of dental visits, health care utilization, dental benefits, and sociodemographic characteristics were the highest contributing predictors to the models' performance. Conclusions and relevance: Findings of this prognostic study using cohort data suggest that tree-based ensemble machine learning models could accurately predict adults at risk of foregoing preventive dental care and demonstrated bias against underrepresented sociodemographic groups. These results highlight the importance of evaluating model fairness during development and testing to avoid exacerbating existing biases.

Abstract:Dental caries has high prevalence among kids and adults thus it has become one of the global health concerns. The current modern dentistry focused on the preventives measures to reduce the number of dental caries cases. The employment of machine learning coupled with UV spectroscopy plays a crucial role to detect the early stage of caries. Artificial neural network with hyperparameter tuning was employed to train spectral data for the classification based on the International Caries Detection and Assesment System (ICDAS). Spectra preprocessing namely mean center (MC), autoscale (AS) and Savitzky Golay smoothing (SG) were applied on the data for spectra correction. The best performance of ANN model obtained has accuracy of 0.85 with precision of 1.00. Convolutional neural network (CNN) combined with Savitzky Golay smoothing performed on the spectral data has accuracy, precision, sensitivity and specificity for validation data of 1.00 respectively. The result obtained shows that the application of ANN and CNN capable to produce robust model to be used as an early screening of dental caries.

Abstract:Background: The association between periodontitis and cardiovascular disease is increasingly recognized. In this research, a prediction model utilizing machine learning (ML) was created and verified to evaluate the likelihood of coronary heart disease in individuals affected by periodontitis. Methods: We conducted a comprehensive analysis of data obtained from the National Health and Nutrition Examination Survey (NHANES) database, encompassing the period between 2009 and 2014.This dataset comprised detailed information on a total of 3,245 individuals who had received a confirmed diagnosis of periodontitis. Subsequently, the dataset was randomly partitioned into a training set and a validation set at a ratio of 6:4. As part of this study, we conducted weighted logistic regression analyses, both univariate and multivariate, to identify risk factors that are independent predictors for coronary heart disease in individuals who have periodontitis. Five different machine learning algorithms, namely Logistic Regression (LR), Gradient Boosting Machine (GBM), Support Vector Machine (SVM), K-Nearest Neighbor (KNN), and Classification and Regression Tree (CART), were utilized to develop the model on the training set. The evaluation of the prediction models' performance was conducted on both the training set and validation set, utilizing metrics including AUC (Area under the receiver operating characteristic curve), Brier score, calibration plot, and decision curve analysis (DCA). Additionally, a graphical representation called a nomogram was created using logistic regression to visually depict the predictive model. Results: The factors that were found to independently contribute to the risk, as determined by both univariate and multivariate logistic regression analyses, encompassed age, race, presence of myocardial infarction, chest pain status, utilization of lipid-lowering medications, levels of serum uric acid and serum creatinine. Among the five evaluated machine learning models, the KNN model exhibited exceptional accuracy, achieving an AUC value of 0.977. The calibration plot and brier score illustrated the model's ability to accurately estimate probabilities. Furthermore, the model's clinical applicability was confirmed by DCA. Conclusion: Our research showcases the effectiveness of machine learning algorithms in forecasting the likelihood of coronary heart disease in individuals with periodontitis, thereby aiding healthcare professionals in tailoring treatment plans and making well-informed clinical decisions.

Novel Machine Learning Algorithms for Prediction of Treatment Decisions in Adult Patients With Class III Malocclusion

Abstract:Background: Management of Class III (Cl III) dentoskeletal phenotype is often expert-driven. Purpose: The aim is to identify critical morphological features in postcircumpubertal Cl III treatment and appraise the predictive ability of innovative machine learning (ML) algorithms for adult Cl III malocclusion treatment planning. Study design: The Orthodontics Department at the University of Illinois Chicago undertook a retrospective cross-sectional study analyzing Cl III malocclusion cases (2003-2020) through dental records and pretreatment lateral cephalograms. Predictor: Forty features were identified through a literature review and gathered from pretreatment records, serving as ML model inputs. Eight ML models were trained to predict the best treatment for adult Cl III malocclusion. Outcome variable: Predictive accuracy, sensitivity, and specificity of the models, along with the highest-contributing features, were evaluated for performance assessment. Covariates: Demographic covariates, including age, gender, race, and ethnicity, were assessed. Inclusion criteria targeted patients with cervical vertebral maturation stage 4 or above. Operative covariates such as tooth extraction and types of orthognathic surgical maneuvers were also analyzed. Analyses: Demographic characteristics of the camouflage and surgical study groups were described statistically. Shapiro-Wilk Normality test was employed to check data distribution. Differences in means between groups were evaluated using parametric and nonparametric independent sample tests, with statistical significance set at <0.05. Results: The study involved 182 participants; 65 underwent camouflage mechanotherapy, and 117 received orthognathic surgery. No statistical differences were found in demographic characteristics between the two groups (P > .05). Extreme values of pretreatment parameters suggested a surgical approach. Artificial neural network algorithms predicted treatment approach with 91% accuracy, while the Extreme Gradient Boosting model achieved 93% accuracy after recursive feature elimination optimization. The Extreme Gradient Boosting model highlighted Wit's appraisal, anterior overjet, and Mx/Md ratio as key predictors. Conclusions: The research identified significant cephalometric differences between Cl III adults requiring orthodontic camouflage or surgery. A 93% accurate artificial intelligence model was formulated based on these insights, highlighting the potential role of artificial intelligence and ML as adjunct tools in orthodontic diagnosis and treatment planning. This may assist in minimizing clinician subjectivity in borderline cases.

There are several Machine learning algorithms that are used to perform predictive analysis in various fields. Predictive analysis in healthcare is a challenging task but ultimately can help practitioners make data informed about a patient's health and treatment. In this project, for experiment purposes, we have taken a dataset which is originally from the National Institute of diabetes and digestive and kidney diseases. All patients here are females at least 21 years old of Pima Indian heritage. By studying the dataset, we must find hidden information, hidden patterns to discover knowledge from the data and predict outcomes accordingly. The objective of this project is to diagnostically predict whether the patient has diabetes or not, based on certain diagnostic measurements included in the dataset. We have proposed a diabetes prediction model for better classification of diabetes by applying some popular machine learning algorithms namely, Logistic Regression, Random Forest Algorithm and KNN Algorithm to predict Diabetes.

Abstract:Resin composites are commonly applied as the material for dental restoration. Wear of these materials is a major issue. In this study specimens made of dental composite materials were subjected to an in-vitro test in a pin-on-disc tribometer. Four different dental composite materials applied in the experiment were soaked in a solution of chewing tobacco for certain days before being removed and put through a wear test. Subsequently, four different machine learning (ML) algorithms (AdaBoost, CatBoost, Gradient Boosting, Random Forest) were implemented for developing models for the prediction of wear of dental materials. AdaBoost, CatBoost, Gradient Boosting and Random Forest model show an MAE of 0.7011, 0.0773, 0.0771 and 0.2199. AdaBoost model performs poorly in comparison to other models.

Abstract:Since dental materials are worn down over time and eventually need to be replaced. Resin composites are frequently employed as dental restorative materials. By employing the in-vitro test findings of the pin-on-disc tribometer [ASTM G99-04], the goal of this study is to evaluate the capability of three different machine learning (ML) models in analyzing the wear of dental composite materials when immersed in chewable tobacco solution. Four distinct dental composite material samples are used in this investigation, and after being dipped in a chewing tobacco solution for a few days, the samples are taken out and subjected to a wear test. Three different ML models (MLP, KNN, XGBoost) have been chosen for predicting the wear of dental composite specimens. XGBoost ML model yields an R 2 value of 0.9996 and it performs noticeably better than the other approaches.

Abstract:Artificial intelligence has been applied to medical diagnosis and decision-making but it has not been used for classification of Class III malocclusions in children. Objective: This study aims to propose an innovative machine learning (ML)-based diagnostic model for automatically classifies dental, skeletal and functional Class III malocclusions. Methods: The collected data related to 46 cephalometric feature measurements from 4-14-year-old children (n = 666). The data set was divided into a training set and a test set in a 7:3 ratio. Initially, we employed the Recursive Feature Elimination (RFE) algorithm to filter the 46 input parameters, selecting 14 significant features. Subsequently, we constructed 10 ML models and trained these models using the 14 significant features from the training set through ten-fold cross-validation, and evaluated the models' average accuracy in test set. Finally, we conducted an interpretability analysis of the optimal model using the ML model interpretability tool SHapley Additive exPlanations (SHAP). Results: The top five models ranked by their area under the curve (AUC) values were: GPR (0.879), RBF SVM (0.876), QDA (0.876), Linear SVM (0.875) and L2 logistic (0.869). The DeLong test showed no statistical difference between GPR and the other models (p > 0.05). Therefore GPR was selected as the optimal model. The SHAP feature importance plot revealed that he top five features were SN-GoMe (the ratio of the length of the anterior skull base SN to that of the mandibular base GoMe), U1-NA (maxillary incisor angulation to NA plane), Overjet (the distance between two lines perpendicular to the functional occlusal plane from U1 and L), ANB (the difference between angles SNA and SNB), and AB-NPo (the angle between the AB and N-Pog line). Conclusions: Our findings suggest that ML models based on cephalometric data could effectively assist dentists to classify dental, functional and skeletal Class III malocclusions in children. In addition, features such as SN_GoMe, U1_NA and Overjet can as important indicators for predicting the severity of Class III malocclusions.
Caries Detection and Classification in Photographs Using an Artificial Intelligence-Based Model—An External Validation Study

Abstract:Objective: This ex vivo diagnostic study aimed to externally validate a freely accessible AI-based model for caries detection, classification, localisation and segmentation using an independent image dataset. It was hypothesised that there would be no difference in diagnostic performance compared to previously published internal validation data. Methods: For the independent dataset, 718 dental images representing different stages of carious (n = 535) and noncarious teeth (n = 183) were retrieved from the internet. All photographs were evaluated by the dental team (reference standard) and the AI-based model (test method). Diagnostic performance was statistically determined using cross-tabulations to calculate accuracy (ACC), sensitivity (SE), specificity (SP) and area under the curve (AUC). Results: An overall ACC of 92.0% was achieved for caries detection, with an ACC of 85.5–95.6%, SE of 42.9–93.3%, SP of 82.1–99.4% and AUC of 0.702–0.909 for the classification of caries. Furthermore, 97.0% of the cases were accurately localised. Fully and partially correct segmentation was achieved in 52.9% and 44.1% of the cases, respectively. Conclusions: The validated AI-based model showed promising diagnostic performance in detecting and classifying caries using an independent image dataset. Future studies are needed to investigate the validity, reliability and practicability of AI-based models using dental photographs from different image sources and/or patient groups.

Abstract:Dental caries is the most frequent dental health issue in the general population. Dental caries can result in extreme pain or infections, lowering people's quality of life. Applying machine learning models to automatically identify dental caries can lead to earlier treatment. However, physicians frequently find the model results unsatisfactory due to a lack of explainability. Our study attempts to address this issue with an explainable deep learning model for detecting dental caries. We tested three prominent pre-trained models, EfficientNet-B0, DenseNet-121, and ResNet-50, to determine which is best for the caries detection task. These models take panoramic images as the input, producing a caries–non-caries classification result and a heat map, which visualizes areas of interest on the tooth. The model performance was evaluated using whole panoramic images of 562 subjects. All three models produced remarkably similar results. However, the ResNet-50 model exhibited a slightly better performance when compared to EfficientNet-B0 and DenseNet-121. This model obtained an accuracy of 92.00%, a sensitivity of 87.33%, and an F1-score of 91.61%. Visual inspection showed us that the heat maps were also located in the areas with caries. The proposed explainable deep learning model diagnosed dental caries with high accuracy and reliability. The heat maps help to explain the classification results by indicating a region of suspected caries on the teeth. Dentists could use these heat maps to validate the classification results and reduce misclassification.
