Abstract:"Stuttering heart attacks" manifest subtly with fluctuating symptoms and may pose diagnostic challenges for healthcare professionals. This case report presents a compelling clinical scenario in which a patient initially presented with a headache as the primary complaint, without any cardiac-related symptoms, but later diagnosed with a triple coronary artery obstruction. The case highlights the intriguing nature of cardiac symptoms that can mimic non-cardiac conditions, potentially leading to an underestimation of cardiac etiologies. The concept of a "stuttering heart attack" is introduced, which represents a unique manifestation of myocardial ischemia characterized by fluctuating symptoms and intermittent coronary blood supply. Diagnostic challenges arise when patients exhibit atypical symptoms and lack evidence of cardiac ischemia on electrocardiography (ECG) or cardiac biomarkers. Differentiating between true myocardial infarction (MI), aborted MI (spontaneously or therapeutically), and masquerading MI is crucial for an accurate diagnosis and appropriate management. This case report emphasizes the importance of a comprehensive evaluation, even in the absence of classic risk factors, and encourages further research to enhance understanding and management of stuttering heart attacks.
