Abstract:The tooth-colored inlays in posterior teeth have increased significantly in recent years. Both indirect composite resins and ceramics are being used for esthetic inlay. The indirect composite resin inlays are esthetic,easy to be fabricated,low elastic modulus similar to dentin and lack of abrasion to opposed teeth. The retention and strength are very important in composite resin inlay design.

Abstract:Objective To evaluate marginal quality of five different restorations,including indirect composite inlays,pressed glass ceramic inlays,golden alloy inlays,Ni-Cr alloy inlays and amalgam.Methods 25 extracted human molars were divided into five groups and restored with IPS Empress Ⅱ ceramic inlays,Brilliant composite inlays,Yalian golden alloy inlays,Ni-Cr alloy inlays,or Antai amalgam.Standardized MO or DO cavities were prepared for all groups.After placement of the fillings,the samples were subjected to thermal cycling including 500 cycles,at a minimum temperature of 5℃ and a maximum of 55℃.Specimens were then immersed in 0.5% basic fuchsin dye solution for 24 hours.Each tooth was longitudinally sectioned and observed under a stereomicroscope.The dye penetration along the cavity walls was measured,recorded and analyzed.The results were analyzed by using the Rank Sum test.Results The Rank Sum test showed significant differences between each of ceramic group,composite group,golden group and either of Ni-Cr group,amalgam group,no significant differences among ceramic group,composite group and golden group,no significant differences between Ni-Cr group and amalgam group.Conclusion IPS Empress inlays in combination with Variolink Ⅱ showed perfect adaption,direct amalgam fillings showed the poorest adaption.Marginal adaption among ceramic group,composite group and golden group were similar.Marginal adaption of golden group were better than Ni-Cr group and amalgam group.Marginal adaption between Ni-Cr group and amalgam group were similar.

Abstract:Objectives: Silver amalgam alloy has been used as a dental restorative material since the beginnings of restorative dentistry. It rose as an easily manipulated and low cost material in comparison to other restorative techniques of the time, but it had poor dimensional stability and clinical behavior. Successive research led to the standardization of both its composition and some aspects of its mechanical properties, which have contributed to its widespread acceptance. Nevertheless, the risk of environmental toxicity generated by mercury and its poor esthetics have given rise to the search for alternative and more promising materials. This article endeavors to give a brief historical description of the main events which have led to development of modern silver amalgam alloys. Significance: It is concluded that extensive knowledge of the use of silver amalgam alloy has made it the most widely used restorative material for the posterior oral cavity. However, in recent years its preponderance has been brought into question even though some innovative ideas have been suggested which could help improve this material in the future.

Abstract:With the growing demand for minimally invasive dental restoration, conservative treatment options offer good aesthetic outcomes and durability with minimal tooth reduction. The use of ceramics in dental restoration has been on the rise in the past two decades due to its long-term benefits and capacity to produce a superior bond to tooth structure when used in conjunction with adhesive cement and acid-etch procedures. A ceramic onlay, which covers one or more cusps, also helps to preserve dental tissue. These materials enable the dentist to conservatively prepare the tooth for restoration while also strengthening the patient's own tooth structure. This case report discusses the preparation of a functional and aesthetic ceramic onlay for a patient with a cracked tooth.

Abstract:History of Dentistry starts from the moment the Late Paleolithic Man used a toothpick fashioned from a bone or wood splinter, or the moment our human ancestors began to manipulate the surface of a tooth to remove its retentiveness, so food does not get stuck. That was at least 14,000 years ago, based on available evidence. The current timeline, compiled in this article, is one of many published over the years. However, this timeline incorporates three new strategies. First, it extends to select medical and fundamental dental discoveries, as the History of Dentistry can only be told with the main events within the History of Medicine. Second, it is more detailed (350 entries) than any previous timeline the author has encountered. Third, several critical primary references to support events listed in this timeline characterize this effort. Finally, 130 illustrations are included to improve the visualization of dates. The manuscript also includes a new display of the five main stages of dentistry throughout its history.
Recent advances in ceramics for dentistry

Abstract:For the last ten years, the application of high-technology processes to dental ceramics allowed for the development of new materials such as heat-pressed, injection-molded, and slip-cast ceramics and glass-ceramics. The purpose of the present paper is to review advances in new materials and processes available for making all-ceramic dental restorations. Concepts on the structure and strengthening mechanisms of dental ceramics are provided. Major developments in materials for all-ceramic restorations are addressed. These advances include improved processing techniques and greater mechanical properties. An overview of the processing techniques available for all-ceramic materials is given, including sintering, casting, machining, slip-casting, and heat-pressing. The most recent ceramic materials are reviewed with respect to their principal crystalline phases, including leucite, alumina, forsterite, zirconia, mica, hydroxyapatite, lithium disilicate, sanidine, and spinel. Finally, a summary of flexural strength data available for all-ceramic materials is included.

Abstract:The field of dental materials has undergone more of a revolution than an evolution over the past 100 y. The development of new products, especially in the past half century, has occurred at a staggering pace, and their introduction to the market has been equally impressive. The movement has mostly come in the area of improved esthetics, marked by the gradual replacement of dental amalgam with dental composite and all-metal and porcelain-fused-to-metal indirect restorations with reinforced dental ceramics, all made possible by the rapid improvements in dental adhesive materials. This article covers the time course of dental materials development over the past century in which the Journal of Dental Research has been published. While there have been advances in nearly all materials used in the field, this article focuses on several areas, including dental amalgam, dental composites and light curing, dental adhesives and dental cements, ceramics, and new functional repair materials. A few short statements on future advances will be included at the end.

Abstract:This article presents a brief history of dental ceramics and offers perspectives on recent research aimed at the further development of ceramics for clinical use, at their evaluation and selection, and very importantly, their clinical performance. Innovative ceramic materials and ceramics processing strategies that were introduced to restorative dentistry since the early 1980s are discussed. Notable research is highlighted regarding (1) wear of ceramics and opposing enamel, (2) polishability of porcelains, (3) influence of firing history on the thermal expansion of porcelains for metal ceramics, (4) machining and CAD/CAM as fabrication methods for clinical restorations, (5) fit of ceramic restorations, (6) clinical failure mechanisms of all-ceramic prostheses, (7) chemical and thermal strengthening of dental ceramics, (8) intraoral porcelain repair, and (9) criteria for selection of the various ceramics available. It is found that strong scientific and collaborative foundations exist for the continued understanding and improvement of dental ceramic systems.

Abstract:Summary.Dental composite filling materials are improved by incorporating nanofillers. They impart increased hardness and wear resistance to composites. In addition, they produce better polishing results than macrofillers. If the particles are sufficiently small, transparent composite pastes are obtainable, independent of the refractive index of the polymerisable monomers. In this context, organosols, non-agglomerated nanoparticles in organic liquid media, are especially interesting. Some of our own results on organosols are presented in this paper. Their relatively low viscosity enables the preparation of composites with a high filler load, thus reducing the shrinkage of the dental composite during polymerisation and improving the mechanical properties. Inorganic–organic hybrids are an attractive class of materials for dental fillings. The synthesis of different polymerisable ormocers for dental composites is reviewed in the second part. Ormocers can be applied as a polymerisable matrix, improving biocompatibility and wear resistance. Their use as inorganic fillers improves the thermodynamic compatibility of the filler with the matrix and enhances the polishability. Functionalised inorganic clusters used as new additives combine the properties of very small nanoparticulate fillers and well-designed highly functional monomers with high crosslinking capabilities. Xerogel colour pigments are advantageous alternative additives to conventional pigments. 3-D structural colour pigments, which are obtained by the self-assembly of monodisperse spherical particles, produce an opalescent effect resembling that of the natural enamel in highly aesthetic composites.

Abstract:Precious metals have a long history of use for dental applications because of their ease of manipulation and excellent biocompatibility. Presently, most precious alloys are used for restoration of teeth as crowns or bridges by casting and lost-wax investment methods. This chapter reviews common characteristics of dental precious alloys, and common high noble and noble alloy groups are presented. Physical and mechanical properties are described as relative to common dental procedures such as finishing and polishing. Modifications of the precious alloys for metal-ceramic bonding as well as some novel applications are also discussed. Finally, trends for future applications of precious alloys in dentistry are considered.

Abstract:Initially, ceramics - mostly burnt clay - were used to manufacture container pottery. The first porcelain objects reached Europe out of China in the Medieval Ages. The technique of their manufacturing was a mystery for many hundred years, yet Germans succeeded first to produce fine European porcelain at the beginning of the 18th century. Its elegance and hardness woke the dentists’ interest too thus Frenchmen created the first porcelain dentures in the second half of the 18th century. Since then, there has been an increasing demand for esthetic fixed implant dentures instead of removable ones. The development of ceramic materials resulted in better mechanical and optical properties, thus the first fixed porcelain inlays and jacket crowns were introduced already in 1889. The addition of leucite filler crystals to porcelain in the 20th century increased the thermal expansion of the ceramic. It could be fired on common dental casting alloys, so the first porcelain-fused-to-metal (PFM) crown was created in 1962. Several new techniques were developed from the middle of the 1980s to the end of the 1990s to deal with initial shrinkage and achieve better properties. Beyond casting, pressing, and CAD/CAM technology, additive manufacturing opened new perspectives in dentistry several years ago in processing dental ceramics.

Abstract:ABSTRACT:Objective To analyze the stress changes of tooth tissues in the medial-occlusal (Class Ⅱ)cavity restored with different kinds of inlay materials by the three-dimensional (3-D)finite element.Methods The inlay restored 3-D finite element model of medial-occlusal (Class Ⅱ)cavity was established by CBCT scanning method and reverse engineering software Mimics,Geomagic Studio,Pro/E5.0 software,and finite element analysis software. The Von-mises stress and change tendency of tooth tissues and four different kinds of inlay materials were compared after taking vertical loading and tongue 45°to loading.Results There were different stress levels of tooth tissues among different inlays of medial-occlusal (MO)Class Ⅱ cavity.The stress level of the cobalt chromium alloy inlay was the largest,the stress level of the gold alloy inlay was lower,and the stress level of the composite inlay was the lowest.Dental tissues stress distribution was similar for the four different restorative materials,and focused on hole bottom of dentin near the pulp in dentin.Conclusion Compared with ceramics,gold alloy and cobalt chromium inlay restoration,restoration with composite resin inlay can reduce the organization stress of the remaining tooth in the medial-occlusal (Class Ⅱ)cavity type.
A 2-year clinical study of inlay and onlay on posterior teeth with vital pulp

Abstract:Objective To evaluate the clinical effect of inlay/onlay(gold-palladium alloy)and inlay(Empress 2)on seriously defected postior teeth(two or multi-surface cavity)with vital pulp and analyze the possible failure reason.Methods In this study,33 gold-palladium inlay,13 gold-palladium onlay and 14 Empress inlay were made for 56 patients and followed up for 2 years.The survival rates were recorded and the results evaluated with modified USPHS evaluation system.Results The 2 years survival rate was 90.0% for all restorations.The survival rates between premolars and molars and among the three kinds of restorations were not significantly different.When analysed item by item with USPHS evaluation criterion,Empress inlay was similar to gold-palladium inlay/onlay in anatomical form,marginal adaptation,secondary caries and pulp vitality.The causes of failure were mainly crown dropping,pulpitis,ceramic fracture and secondary caries.Conclusion The gold-palladium inlay/onlay can be used for patients with strong occlusal force.The gold-palladium onlay has advantage of protecting dental pulp and recovering function and form.Under proper indicatons and following strict biomechanical principle,tooth-colored Empress inlay can achieve good restoration outcome and esthetic effect.
A Historical Perspective on Dental Composite Restorative Materials

Abstract:This review article will discuss the origin of resin-based dental composite materials and their adoption as potentially useful adjuncts to the primary material used by most dentists for direct restorations. The evolution of the materials, largely driven by the industry's response to the needs of dentists, has produced materials that are esthetic, strong, and versatile enough to be used in most areas of the oral cavity to replace or restore missing tooth structures. Significant advancements, such as the transition from chemical to light-curing materials, refinements in reinforcing particles to produce optimum polishing and wear resistance, formulating pastes with altered viscosities to create highly flowable and highly stiff materials, and creating materials with enhanced depth of cure to facilitate placement, will be highlighted. Future advancements will likely reflect the movement away from simply being a biocompatible material to one that is designed to produce some type of beneficial effect upon interaction within the oral environment. These new materials have been called "bioactive" by virtue of their potential effects on bacterial biofilms and their ability to promote mineralization of adjacent tooth structures.

Abstract:In 1983 and 1988 a questionnaire was sent to a random sample of Belgian dentists to compare the selection of filling materials such as amalgam and gold with the tooth-coloured filling materials, composites and glass polyalkenoate (ionomer) cements, for the restoration of Class II lesions in premolar and permanent molar teeth. Despite the increasing demand for aesthetics to be considered, the use of composite resins in the posterior region only increased moderately over the assessment period, while amalgam maintained its leading position as the restorative material of choice in premolar and molar teeth. Only in situations where aesthetics played a primary role did composite resins gain over silver-based fillings. There was no change in the use of gold as an indirect restorative material. Finally, the selection rate of glass ionomers increased slightly.

Abstract:Numerous written relicts, belletristic works (poems of Martial, Juvenal, Ovid etc.) indicate that oral hygiene and its tools (toothbrush, toothpick, use of tooth pastes and tooth-powder) were used long before our times. Already ancient people started to remove, file, dye and inlay teeth. The teeth were dyed red, green or black in Egypt, red or brown (with henna or betel) in India, white by Romans. The teeth decoration has a long but forgotten history. The most skillful and artistic work was done by the Maya's between 900 BC and 1500 AD. The modification of contours (more than fifty forms) of the incisors were practiced also in Mesoamerica. Dentistry was surely practiced in ancient Egypt, India, China, Greece and Rome, while odontology and especially suitable dental appliances arose only by Etruscan. Dental prosthesis, including bridges and simple retention bands were invented by the Etruscans 2500 years ago. These Etruscan bridges were worn mostly by females, suggesting that cosmetics was the principal dental concern. Some,--if not all--of the Roman and other prostheses have been purely ornamental. Orthodontic appliances are also Etruscan invention. The holes caused by caries were filled with garlic, incense, caraway seed in Egypt, with wood or lead in Rome, and with "silver-paste" (amalgam) in ancient China. The toothache was cured with poppy-tee, or hashish and nightshade plants (Solanaceae) in Egypt, Greece, Roman Empire while with coca (Erythroxylon coca) in South-America.

Engineering assessments of clinical function are dealt with and literature is reviewed on the clinical behaviour of all-ceramic systems. Practical aspects are presented regarding the choice and use of dental ceramics to maximize aesthetics and durability, emphasizing what we know and how we know it.
Ceramic Inlay Bonded Interfaces in Minimally Invasive Preparations: Damage and Contributing Mechanisms in Sliding Contact

Abstract:Background: In the preparation of inlay cavities, a choice must be made between conventional standard and minimally invasive preparation designs; in the long run, this choice can affect the integrity of the bonded interface. Purpose: To evaluate the effect of minimally invasive cavity preparation designs on the extent and contributing mechanisms of damage to ceramic inlay bonded interfaces. Methods and materials: Tooth blocks with 90°, 120° and 75° marginal angles were prepared, representing tooth cavities with conventional standard and minimally invasive preparations with large divergence and convergence angles and bonded to monolithic ceramic (IPS e.max CAD). Vickers indentations were placed at various distances from the bonded interface. The indentation morphology and crack length were observed. Reciprocating wear tests were performed on the bonded interface with a 20-newton (N) vertical load. The wear depth and wear-scar morphology were characterized after increments of cyclic sliding contact. Results: The 120° group exhibited longer indentation cracks in the ceramic, whereas the 75° group showed larger indentations in the enamel when compared to the 90° group (p<0.001). Consistent with the weaker edge crack resistance, the 120° group experienced the greatest wear (p=0.008), and the wear depth in the enamel of the 75° group exceeded that of the 90° group (p<0.001) in the early stage (5×102 cycles). However, no significant difference in the wear depth (p>0.147) and morphology were found at the later wear stage among the three groups. Conclusion: Within the limitations of this study, minimally invasive preparations with 120° and 75° marginal angles can result in early sever damage at the ceramic inlay bonded interface but show comparable wear behaviors to the conventional 90° group at the later stage.
Composite Materials Used for Dental Fillings

Abstract:This article explores the properties of composite materials employed in dental fillings. A traditional nano-hybrid composite containing nanofiller particles exceeding 82% by weight served as a benchmark. The remaining samples were fabricated from ormocer resin, maintaining an identical nanofiller content of 84%. In all specimens, the nanoparticles were dispersed randomly within the matrix. This study presents findings from investigations into surface geometry, hardness, wettability, and tribological behavior. The microscopic observations revealed that ormocer-based samples exhibited greater surface roughness than those composed of the traditional composite. Hardness testing indicated that both ceramic addition and sample preparation significantly influenced mechanical properties. Ceramic-enhanced samples demonstrated superior hardness, surpassing the reference composite by 30% and 43%, respectively. Contact angle measurements revealed hydrophilic characteristics in the classic composite, contrasting with the hydrophobic nature of ceramic-containing samples. Tribological evaluations revealed the superiority of the classic composite in terms of friction coefficients and volumetric wear compared to ormocer-based materials.

Marginal gap, internal fit, and fracture load of leucite-reinforced ceramic inlays fabricated by CEREC inLab and hot-pressed techniques

Abstract:Purpose: This in vitro study was designed to evaluate and compare the marginal gap, internal fit, and fracture load of resin-bonded, leucite-reinforced glass ceramic mesio-occlusal-distal (MOD) inlays fabricated by computer-aided design/manufacturing (CAD/CAM) or hot pressing. Materials and methods: Fifty caries-free extracted human molars were prepared for standardized MOD inlays. Impressions of each specimen were made and poured using type IV dental stone. Dies were randomly divided into two equal groups. Twenty-five ceramic inlays were fabricated by the hot-pressed technique using IPS Empress leucite-reinforced glass ceramics, and the other 25 ceramic inlays were produced by CAD/CAM technology using ProCAD leucite-reinforced ceramic blocks and CEREC inLab facilities. Inlays were bonded to the teeth using a dual-cured resin cement. The specimens were stored in distilled water at 37°C for 24 hours and then thermocycled for 5000 cycles. The marginal gap measurements were taken with a stereomicroscope. Specimens in each group of inlay systems were randomly divided into two subgroups of 10 and 15 specimens each. Ten specimens in each subgroup were sectioned mesiodistally for evaluation of the internal fit. The fracture load of specimens in the second subgroup (n = 15) of the two inlay systems was determined under compressive load in a universal testing machine. Data were analyzed using Student's t-test at a significance level of p < 0.05. Results: The mean marginal and internal gap size in both IPS Empress and ProCAD inlays were less than 100 μm; however, the marginal gap for the IPS Empress restorations was significantly higher than that of ProCAD restorations (p < 0.05). There was no significant difference in the mean internal fit or the fracture load between the two glass ceramic inlays (p > 0.05). Conclusions: The leucite-reinforced glass ceramic inlay restorations fabricated by CEREC inLab (CAD/CAM) and the hot-pressed technique provided clinically acceptable marginal and internal fit with comparable fracture loads after luting.
