Abstract:A conservative approach to restoration assists in preserving the remaining tooth structure of extensively destroyed vital teeth. This case report describes a single-appointment chairside technique for placement of ceramic restorations in posterior teeth. A patient presented for treatment of her mandibular right first molar, which had a fractured resin-based composite restoration. Due to the presence of vital pulp, extent of the restoration, and presence of caries in the tooth, the following treatment plan was proposed: placement of a lithium disilicate glass-ceramic onlay fabricated with a computer-aided design/computer-aided manufacturing workflow. After the dentist removed the restoration and performed selective caries removal, structural analysis guided the reduction of the buccal cusps. Immediate dentin sealing was performed with a 2-step self-etching adhesive system, and a 1-mm-thick layer of flowable resin-based composite was placed as a resin coating. A digital impression was obtained, the onlay restoration was designed, and a lithium disilicate block was milled and subsequently crystallized. When the onlay was completed, the tooth preparation was sandblasted, selectively etched, and coated with a universal adhesive. The intaglio surface of the onlay was cleaned and primed, the onlay was bonded with dual-cure resin cement, and occlusal adjustments were completed. Follow-up examinations at 1 and 4 months revealed the clinical success of the case. From start to finish, it takes approximately 2.5 hours to produce a single-appointment chairside restoration. The technique used in this case offers a fast-paced workflow that is comfortable and practical for the patient and provides a predictable clinical outcome without the need for a temporary restoration.

Abstract:As patient demand for faster, streamlined, and convenient treatments increases, clinicians need to be prepared to provide accurate, esthetic, and predictable restorative solutions that meet these requirements. Among the many options currently available in coronal treatments utilizing a digital workflow, the author's practice has found that the use of a certain lithium-disilicate block in tandem with a self-adhesive resin cement provides such a solution. This case report demonstrates the use of CAD/CAM dentistry with in-office milling to restore a failed No. 14 full-coverage crown.

Abstract:Ceramics can simulate the visual character of the tooth substance successfully and are biocompatible materials. However, a wide range of ceramic materials and systems on the market are available for use in dentistry. Therefore, it is the aim of this article to provide an overview of dental ceramics, their classifications, methods of construction, and clinically relevant aspects that enable the reader to select the most appropriate ceramic for a particular clinical situation.The PubMed (MEDLINE) search engine was used to gather the most recent information on dental ceramics. The search was restricted to a ten-year period (January 1, 2010–December 31, 2019) and only English-language studies. A Boolean search of the PubMed data set was implemented to combine a range of keywords: (ceramics OR all-ceramics OR dental porcelain OR polycrystalline OR porcelain fused to metal OR ceramometal OR procera OR e max OR zirconia OR In-ceram OR Inlays OR Onlays OR Overlays OR Endocrown) AND (survival rate OR success rate OR clinical outcomes OR classification) AND (humans). Studies were also obtained by manual searches and from Google Scholar.By using this process, 2173 articles and studies were obtained. More studies were also obtained by manual searches and from Google Scholar. The most relevant published studies were chosen and used in the current review.All-ceramic restoration use has increased in recent years. This increase has been attributed to patients' demand for good aesthetics and an improvement in the materials' mechanical and aesthetic properties as well as to required minimally invasive tooth preparation and the methods of fabrication. The success of ceramic restorations depends on several factors, such as selection of material, restoration design, occlusion, and cementation media.

Abstract:The introduction of new techniques and materials in all-ceramic systems, together with improvements in resin bonding agents, has enabled the use of resin cements to directly bond ceramic crowns, veneers, and inlays/onlays to conservative tooth preparations. Directly bonded ceramic onlays can eliminate the need for conventional means of retention and allow the restoration of lost tooth structure with minimal tooth preparation. This article presents the use and short-term clinical performance of directly bonded, heat-pressed, leucite-reinforced ceramic onlays in both anterior and posterior areas of the mouth.

Abstract:From the restorative perspective, various methods are available to prevent the progression of non-carious cervical lesions. Direct, semi-direct, and indirect composite resin techniques and indirect ceramic restorations are commonly recommended. In this context, semi-direct and indirect restoration approaches are increasingly favored, particularly as digital dentistry becomes more prevalent. To illustrate this, we present a case report demonstrating the efficacy of hybrid ceramic fragments fabricated using computer-aided design (CAD)/computer-aided manufacturing (CAM) technology and cemented with resin cement in treating non-carious cervical lesions over a 48-month follow-up period. A 24-year-old male patient sought treatment for aesthetic concerns and dentin hypersensitivity in the cervical region of the lower premolar teeth. Clinical examination confirmed the presence of two non-carious cervical lesions in the buccal region of teeth #44 and #45. The treatment plan involved indirect restoration using CAD/CAM-fabricated hybrid ceramic fragments as a restorative material. After 48 months, the hybrid ceramic material exhibited excellent adaptation and durability provided by the CAD/CAM system. This case underscores the effectiveness of hybrid ceramic fragments in restoring non-carious cervical lesions, highlighting their long-term stability and clinical success.

Abstract:The use of all-ceramic crowns offers the potential for improved aesthetic results compared to conventional ceramometal crowns. Silica/glass-based all-ceramic crowns are more translucent than alumina or zirconia-based crowns and therefore have better optical properties. However, they are mechanically weaker and need to be used in conjunction with resin bonding cements. It is accepted that adhesion between ceramics and resin cements is provided by two major mechanisms namely micromechanical attachment and chemical bonding. Due to its resistance to aggressive chemical agents, chemical bonding of zirconia is difficult when compared to silica-based ceramics materials. In this paper, we discuss the luting modalities of both glass ceramics and zirconia based ceramics through a clinical report. It describes a case of rehabilitation of a missing lateral incisor and a peg lateral incisor using zirconia Cantilever Bridge and a ceramic veneer. Depending on material properties, the zirconia based restoration was mechanically bonded meanwhile the ceramic veneer was chemically bonded. The final result was esthetically acceptable and the patient was satisfied.

Abstract:PURPOSE:To investigate the short-term and long-term effects of all-ceramic onlay on restoration of premolars and its influence on dental function.METHODS:Ninety-five premolars receiving root canal treatment in People's Hospital of Peking University from January 2017 to January 2018 were enrolled, and randomly divided into two groups based on different repairing methods. Patients in the control group (n=47) received full crown restoration, while patients in the experimental group(n=48) received all-ceramic onlay restoration. The success, survival and failure rates of the teeth were compared. The United States Public Health Service(USPHS) and occlusal function of the prosthesis were compared 6, 12 and 36 months after treatment. The data were processed using SPSS 19.0 software package.RESULTS:The success and survival rate of the experimental group were higher than those of the control group, but without significant difference (P＞0.05). The morphology, marginal integrity, marginal coloration, surface texture, secondary caries, gingival health and proximal contacts showed no significant difference between the two groups 12 months after treatment(P＞0.05). Thirty-six months after treatment, the marginal integrity, marginal coloration and surface texture showed no significant difference between the two groups (P＞0.05), while the morphology, secondary caries, gingival health and proximal contacts were significantly better in the experimental group than in the control group (P＜0.05). The occlusal function between the affected side and contralateral side of both groups showed no significant difference 6, 12 and 36 months after treatment(P＞0.05).CONCLUSIONS:All-ceramic onlay restoration of premolars has high success and survival rate, and good short-term and long-term restoration effect, which is beneficial to improve the occlusal function of the affected teeth.

Abstract:OBJECTIVE:This study aimed to evaluate the success and survival rate of endodontically treated premolars restored by ceramic onlays by comparing restored by quartz fiber posts and metal ceramic crowns.METHODS:Ninety-four patients with 126 endodontically treated premolars were enrolled in this study and divided into 4 groups according to the remaining axial walls and restorative methods. The observation time was 36 months. The success and survival rates of the restorations, as well as causes of failures, were analyzed.RESULTS:At the final follow-up, the success and survival rates of the mild defect endodontically treated premolars were restored by quartz fiber posts and the crowns were at 96.3% and 98.1%; the success and survival rates of the severe defect premolars restored by quartz fiber posts and crowns were at 88.5% and 96.2%. The success and survival rates of the mild defect premolars restored by cast ceramic onlays were both at 96.6%, the success and survival rates of the severe defect premolars restored by cast ceramic onlays were at 94.1% and 100%, respectively. The success and survival rates of the different groups were no significant difference (P>0.05).CONCLUSION:Based on the results and within the limits of this study, cast ceramic onlays is a very reliable method to restore endodontically treated premolars.

Abstract:Regressive changes of natural dentition may be a physiologic or pathologic process which proceeds to drastic occlusal wear. Loss of incisal or occlusal edge morphology due to functional or parafunctional activity of teeth can be regarded as physiologic whereas frictional action of a foreign body on the teeth or loss of tooth substance due to a chemical process involving bacteria may be termed as pathogenic. These result in loss of occlusal vertical dimension and may elicit occlusal instability compromised function and aesthetics.The treatment modalities for the same are confined to fixed prosthesis for restoration of the lost vertical dimension. But this technique in general is expensive technique sensitive and complex. An Overlay Removable Partial Denture ORPD though rarely reported in the literature can be considered as one of the alternative interim or even a permanent dental prosthesis. The proposed paper is a case report of worn dentition rehabilitated with ORPD. This approach was found to be non-invasive simple and cost effective. The patient on short term follow-up reported with optimal aesthetics and masticatory efficiency. Overall satisfaction and quality of life improved after the treatment.

Abstract:Objectives To give a comprehensive review of advancement in dental ceramics, fabrication methods, and the challenges associated with clinical application. Data, sources and study selection Researches on chemical composition, biomechanical behaviors, optical properties, bonding strategies and fabrication methods were included. The search of articles was independently conducted by two authors in the PubMed, Scopus, Medline and Web of Science. Conclusions Dental ceramics have shown significant advancements in terms of esthetics and function. However, improving fracture toughness without compromising optical properties remains a challenge. Repairing fractured zirconia or glass-matrix ceramic prostheses with the same material is difficult due to the sintering process. Developing innovative bonding techniques that provide strong and long-lasting bonding strength between ceramics and tooth structures poses a recurring obstacle. Clinical significance Despite the emergence of dental ceramics and fabrication techniques, certain limitations such as susceptibility to brittleness and fracture still exist. Therefore, the current review provided valuable information around the advanced dental ceramics in tooth repair. The laboratory test data and the clinical outcome are also presented in details, aiming to guide clinicians in making informed decisions regarding ceramic restorations.

Abstract:Cast metal restorations have been a cornerstone in restorative dentistry for decades, providing durable and reliable solutions for restoring damaged teeth. This case report explores the evolution of cast metal restoration techniques, highlighting recent advancements and their implications in modern dental practice involving indirect cast metal onlay restoration as a successful treatment option that involves the replacement of the tooth cusp and reinforcement of the tooth through indirect restoration. Historically, cast metal restorations, commonly fabricated from alloys such as gold, have offered superior mechanical properties, biocompatibility, and longevity compared to other materials. However, concerns regarding esthetics and cost have prompted the development of alternative materials such as ceramics and composite resins. Nonetheless, cast metal restorations remain indispensable for certain clinical scenarios, particularly in cases of extensive damage or high occlusal forces.

Abstract:Abstract Aim The purpose of this case report is to describe the clinical procedure for fabricating fiber-reinforced composite (FRC) onlay composite resin restorations using a FRC as the substructure. Background A variety of therapeutic modalities are available to restore teeth with moderate coronal defects in the posterior region of the mouth. For patients who refuse complete crown restorations or when minimal tooth reduction is preferred, a FRC restoration can be a good alternative to conventional restorative techniques. Case Report A 42-year-old female patient presented with an endodontically treated mandibular right first molar with extensive destruction of the coronal tooth structure. To conserve the remaining tooth structure a FRC resin core substructure was fabricated and veneered with Ceramage dentin and enamel hybrid composite to create the final restoration. Summary FRC restorations using adhesive technology appears to be a promising restorative option. However, further clinical investigation will be required to provide additional information on this technique. Based on the clinical and radiographic findings in the present case, the fabrication of a conventional crown was avoided in order to conserve the remaining tooth structure. Clinical Significance The restoration of badly damaged teeth is a challenge for clinicians when cast crown restorations are not an option for the patient. Abstract:ABSTRACT Tooth wear is a process that is usually a result of tooth to tooth and/or tooth and restoration contact. The process of wear essentially becomes accelerated by the introduction of restorations inside the oral cavity, especially in case of opposing ceramic restorations. The newest materials have vastly contributed toward the interest in esthetic dental restorations and have been extensively studied in laboratories. However, despite the recent technological advancements, there has not been a valid in vivo method of evaluation involving clinical wear caused due to ceramics upon restored teeth and natural dentition. The aim of this paper is to review the latest advancements in all-ceramic materials, and their effect on the wear of opposing dentition. The descriptive review has been written after a thorough MEDLINE/PubMed search by the authors. It is imperative that clinicians are aware of recent advancements and that they should always consider the type of ceramic restorative materials used to maintain a stable occlusal relation. The ceramic restorations should be adequately finished and polished after the chair-side adjustment process of occlusal surfaces.
Occlusal onlays as a modern treatment concept for the reconstruction of severely worn occlusal surfaces

Abstract:According to the Fifth German Oral Health Study, the caries experience in the German population is declining sharply. The number of teeth still present at an advanced age has also increased significantly in recent decades. This shows a clear trend towards long-term tooth preservation - possibly with fixed dental prostheses - which is further supported by the possibility to place implants to increase the number of abutments. The pronounced decline in caries experience has given Germany a leading international position in terms of dental health. But there is increasing evidence of risks associated with dental hard-tissue damage because of erosion/biocorrosion, attrition, and abrasion. The defect morphology of these wear-related lesions is different from that of caries lesions; occlusal surfaces are more often affected in the posterior region. Against this background, restorative treatment concepts have become significantly more differentiated in recent decades. Predominantly subtractive concepts to provide mechanical retention for the restoration using traditional cements are now replaced by less invasive, primarily defect-oriented procedures wherever feasible. In the case of pronounced dental hard-tissue loss, additive approaches also allow restorations that restore function. In addition, there are modifications of traditional procedures, such as defining the treatment goal in the lead-up to the treatment itself with the aid of a diagnostic wax-up. The wax-up provides orientation for the subsequent tooth preparation and allows a particularly economical approach to the removal of healthy dental hard tissue. Furthermore, the introduction of new preparation designs has contributed significantly to the preservation of dental hard tissue on the teeth to be restored. This article describes the principles of minimally invasive treatment using occlusal onlays for the reconstruction of severely worn occlusal surfaces.
Clinical Approach of the In-ceram All Ceramic Crown in Restoration of Anterior Teeth

Abstract:Objective: To study the clinical esthetic characteristics and application of the In-ceram all ceramic crown to restore anterior teeth.Methods: A regular preparation of anterior teeth related for the In-ceram all ceramic crown.Impression taken, model poured, special die made and the color of teeth being chosen under natural light.Thereafter, Vita In-ceram alumina all ceramic crown had been manufactured for clinical use.A preliminary try-in, adjustment, polishing and cementation.Results: 94% of patients felt greatly satisfied with the In-ceram all ceramic crown and 6% of them received with satisfaction.Dental clinical examination showed that the color of the in-ceram all ceramic crowns had not been changed over 5 year clinical survey.All patients with a good marginal adaptation had no ceramic breakdown and loose teeth.Only 2.6% of patients appeared mild gingivitis.Conclusion: The use of the In-ceram all ceramic crown in restoration of anterior teeth can achieve a satisfied clinical results both in the value of esthetics and in the health of gingivae.

Abstract:Objective This case report illustrates a clinical scenario of a patient with insufficient crowns, gold onlays and localized tooth pain in the posterior teeth, treated with partial and full coverage lithium disilicate restorations. Clinical Considerations A female patient presented in the clinics, complaining about tooth pain and unesthetic metal restorations. After a thorough examination, diagnosis and treatment plan, the existing restorations were removed, and a defect‐oriented preparation was done to preserve as much as possible of the remaining tooth structure. Final partial and full coverage lithium disilicate restorations were delivered. After a 10‐year follow‐up no biological complications were observed. Nevertheless, one major chipping was observed in one of the upper molars. This technical complication was resolved with the fabrication of a new ceramic restoration. Conclusion Lithium disilicate ceramic posterior full‐ or partial‐coverage restorations are a reliable and predictable treatment option not only for the replacement of tooth structure lost due to caries but also for the replacement of insufficient restorations. Clinical Significance The following key factors are to be considered to pursue long‐term success with ceramic restorations: patient selection, minimal invasive tooth preparation, appropriate material selection and cementation. Nevertheless, complications are to be expected, and the patient should be informed accordingly.

Abstract:Advances in computer-aided design (CAD) / computer-aided manufacturing (CAM) technologies and their ease of application enabled the development of novel treatment concepts for modern prosthodontics. This recent paradigm shift in fixed prosthodontics from traditional to minimally invasive treatment approaches is evidenced by the clinical long-term success of bonded CAD/CAM glass-ceramic restorations. Today, defect-oriented restorations, such as inlays, onlays, and posterior crowns, are predominately fabricated from glass-ceramics in monolithic application. The variety of CAD/CAM ceramic restorative systems is constantly evolving to meet the increased demands for highly aesthetic, biocompatible, and long-lasting restorations. Recently introduced polymer-infiltrated ceramic network CAD/CAM blocks add innovative treatment options in CAD/CAM chairside 1-visit restorations. The material-specific high-edge stability enables the CAD/CAM machinability of thin restoration margins. Full-contour zirconia restorations are constantly gaining market share at the expense of bilayered systems. Advancements in material science and bonding protocols foster the development of novel material combinations or fabrication techniques of proven high-strength zirconia ceramics. CAD/CAM applications offer a standardized manufacturing process resulting in a reliable, predictable, and economic workflow for individual and complex teeth-supported restorations. More evidence from long-term clinical studies is needed to verify the clinical performance of monolithic polymer-infiltrated ceramic network and zirconia teeth-supported minimally invasive and extensive restorations.
