Abstract:We applied dual‐flex coils in a phased‐array configuration to obtain high resolution MR images of the bilateral hips and compared the image quality with non‐phased‐array imaging. Multiple fast spin‐echo images of both hips were obtained in a single acquisition. Two consecutive acquisitions with and without phased‐array capability were performed in 22 hips and a fluid‐filled phantom. The signal‐to‐noise ratio (S/N) enhancement values were calculated. Subjective image analysis was also obtained from three independent reviewers. High quality MR images were obtained using dual‐array coils with a S/N enhancement value of 1.40 for a phantom and 1.83 to 2.22 for subject hips. On subjective analysis, the dual array enhanced delineation of the acetabular labrum, articular cartilage, and retained fluid. The dual array may be of diagnostic value with a higher S/N enhancement and subjective superiority of the images obtained.

Abstract:In an effort to improve dentogingival aesthetics, scientific research has been developing non-surgical techniques and dental restorative materials. A 45-year-old female reported to the hospital with a complaint of an unpleasant aesthetic in the anterior region of the maxilla. She has an irregular contour of the gingiva in her anterior region and no temporomandibular problems. There is a history of trauma, but she did not take any treatment for it, which caused discolouration. Plaque index was retained by the surface roughness. Due to this reason, the patient's appearance, self-esteem, and quality of life have decreased. The purpose of this case is to provide a system-based clinical example of oral rehabilitation for anterior teeth. The patient didn't like the way she looked, and hence aesthetic and functional rehabilitation was planned. Treating the dentogingival asymmetry was the first step in the therapeutic approach. An in-ceram zirconia permanent porcelain metal-free crown was positioned after the temporary crowns were cemented into place. It should be mentioned that in clinical situations, the integration of periodontal and prosthetic treatment is crucial. Additionally, it is stated that the in-ceram zirconia system can produce satisfactory results when applied appropriately. For the main anterior teeth, zirconia crowns have offered an alternate treatment option that addresses aesthetic concerns and facilitates the installation of extra-coronal restorations. This article describes a case involving the aesthetic and functional restoration of severely damaged maxillary incisors using zirconia crowns.
Crafting Confidence: A Comprehensive Case Study of Full-Mouth Reconstruction Utilizing Implant-Supported Computer-Aided Design/Computer-Aided Manufacturing (CAD/CAM) Zirconia Restorations

Abstract:A 67-year-old male patient reported to the department with a chief complaint of tooth mobility. The patient presented with a medical history of diabetes for which he was on medication, and he also reported a history of chronic periodontitis. After a thorough assessment of the patient, a proper treatment plan was designed, which included full-mouth rehabilitation, prior to which the patient was advised full-mouth extraction. Six implants were inserted into the mandibular and maxillary arches as part of the treatment. After the implant was placed, zirconia was the preferred choice for the restoration because of its superior aesthetic results. Polyvinyl siloxane impression material was used to make implant impressions after an eight-week healing period. The case report provides the technique for the current approach to full-mouth rehabilitation with all six concepts of implant placement. In these situations, it is crucial to plan and insert implants correctly and adhere to the entire treatment plan. Restorations were performed using the latest computer-aided design/computer-aided manufacturing (CAD/CAM) technologies for a more aesthetically pleasing result. The case study highlights the significance of meticulous preparation and implementation for the accomplishment of successful full-mouth rehabilitation.

Abstract:Titanium and zirconia custom implant abutments are now commonly used for esthetic implant dentistry. Custom implant abutments allow the clinician to improve an implant's emergence profile, to customize cervical margins in accordance with the anatomy of the natural root, and to compensate for poor implant angulation. All of these are essential for optimum esthetic outcomes. Computer-aided design/computer-aided machining (CAD/CAM) technology allows the clinician to design custom implant abutment configurations and create natural-looking superstructures that are in harmony with the adjacent dentition and soft tissue. The CAD/CAM technique provides precise fit, reduces the cost of the procedure, and eliminates dimensional inaccuracies inherent in the conventional waxing and casting technique. The aim of this report is to describe a simplified technique for reconstructing emergence profiles during implant restoration using milled titanium and zirconia custom implant abutments. The results of 50 consecutive cases are reported.

Abstract:This case report presented a fully digital workflow in esthetic dental restoration. A 51-year-old female patient was referred to BDMS Wellness Clinic due to a fracture of the maxillary left central incisor. An immediate dental implant was planned to restore tooth 21 with esthetic crown lengthening of upper front teeth and new zirconia crowns for teeth 11, 12, 13, 21, 22, and 23. Digital impression was made using a digital scanner (PRIMESCAN®, Dentsply Sirona, Bensheim, Germany); surgical guide (Cerec Guide 3, Dentsply Sirona, Bensheim, Germany) was designed by using a designing software (Galileos Software, Dentsply Sirona, Bensheim, Germany) and was milled by using a milling machine (MCXL milling machine, Dentsply Sirona, Bensheim, Germany) to create a precise surgical guide. 3D smile design was done by using the Digital Smile Design (DSD) program, the crown lengthening guide was designed according to DSD designed by using a designing software (Inlab 19 Software, Dentsply Sirona, Bensheim, Germany), and the guide was fabricated by a 3D printer (FormLabs Form 2, Formlab, MA, USA). Provisional crowns (splinted) for teeth 11, 12, 13, 21, 22, and 23 were milled by using polymer (VIPI BLOCK TRILUX®, VIPI Industria, Pirassununga, SP, Brazil) using a milling machine (MCX5, Dentsply Sirona, Bensheim, Germany). The zirconia crowns were designed by using software (Inlab19 Software, Dentsply Sirona, Bensheim, Germany) and milled using the same milling machine. At the implant position, Ti-base is cemented by using an abutment (Multilink Hybrid Abutment, Ivoclar Vivadent, Schaan, Lichtenstein, Germany) with zirconia coping (Cercon base white shade, Dentsply Sirona, Bensheim, Germany) utilized as the abutment. The zirconia crowns (Cercon Xt, Dentsply Sirona, Bensheim, Germany) were sintered and characterized and then cemented. The patient was satisfied with the esthetic outcome of the treatment.

Recent advances in manufacturing technology have made possible the fabrication of translucent monolithic zirconia ceramics. This case report describes three clinical examples of monolithic zirconia fixed dental prostheses being used in the anterior and posterior regions and exhibiting acceptable esthetic results.
[Complex analysis of the physical metabolic and genetic state in children of mothers with type 1 diabetes].

Abstract:BACKGROUND Complex assessment of the development, clinical and immunological state of children born by mothers with type 1 diabetes, in the years 1998-2000. MATERIALS AND METHODS Material examined were 30 newborns, and subsequently 25 in the first, 30 children at 5-6 years of life. In newborns and in the 5-6 years the antibodies: IA-2A, IAA, ICA, anti GAD, insulin (IRI), peptide C were determined. In the first year HbA(1c), TSH, antigens HLA DQA1 and DQB1 were determined, in the 5th-6th years of life, the level of antibodies anti endomizium. RESULTS Developmental anomalies in newborns were found in 6, in 3 in first and in the 5th-6th years in 4 children. Systemic infections were found in 6, hypoxia and prematurity in 12 newborns. A positive titer of anti GAD in 15, ICA in 18, IA-2A in 8, IAA in 18 newborns. In the first year of life, anti GAD in 2, in the 5-6th years in 4 children. In one child anti GAD was found during three examinations. In the case of this positive titer of antibodies, the allele DQB1*0302 was present. Allel DQB1*0302 was present in 28% of children, allele DQA1*0301 was absent. Mean levels of IRI afterbirth and in the first year of life were: 23.6 and 9.7 uIU/ml and peptide C 1.25 and 0.92 ng/ml. Mean levels of HbA(1c) in the first and 5th yrs of life were: x 6.2%, TSH 2.45 uIU/ml and 3.4 uIU/ml. Pathological values of antibodies anti endomizium were found in 4 children, biopsy of the small intestine confirmed coeliac disease. A normal body mass was seen in 9 newborns in 16 in the first, and 14 children 5-6 years of age. Deficiency in body mass was found in 2 newborns and 3 children in the first and 5-6th yrs. Macrosomia was found in 8 newborns in group I and 3 in group II. Overweight was found in 5 children in the first year and 9 in 5-6th yrs of life. None of the children have developed diabetes. CONCLUSIONS 1. Children of mothers with type 1 diabetes should be monitored by an outpatient clinic for diabetic patients. 2. Children of mothers with diabetes should have determined regularly immunological markers for diabetes and other autoimmunological disease.

Abstract:Full-mouth rehabilitation, also known as full-mouth reconstruction or restoration, is the comprehensive process of restoring and rehabilitating the function, aesthetics, and health of all teeth in the upper and lower jaws. This involves a multidisciplinary approach, addressing issues such as occlusion (bite), tooth wear, tooth loss, and periodontal (gum) health to achieve optimal dental function and appearance. This treatment provides a stable and reliable solution for dental problems associated with the complete or partial loss of dental elements. This case report discusses the management of a 72-year-old male patient who came with a chief complaint of decayed, missing, and mobility of his teeth. His medical history included drug-regulated diabetes and chronic periodontitis, which remained a reason behind his deteriorating dental health. The treatment plan encompassed a comprehensive approach, starting with the full-mouth extraction and progressing to the placement of six implants in both the mandibular and maxillary arches for complete restoration. After allowing eight weeks for healing, the impressions for the implants were recorded with the polyvinyl siloxane material. The case demonstrates the application of the all-on-six concept; whereby full arch rehabilitation is possible with the strategic placement of six implants. By employing the latest in advanced computer-aided design/computer-aided manufacturing (CAD/CAM), a zirconia prosthesis that was retained on a milled bar was fabricated-yielding a very high degree of accuracy and excellent aesthetics in the restoration. Restorations with zirconium were done, as it is excellent in aesthetics and functioning. This article considers the importance of precision planning, correct implant placement, and advances in dental science and technology. Most importantly, the conclusion that can be drawn is that the end outcome of total rehabilitation with all-on-six should not only be an affirmation that difficult cases can be managed very well but also the patient's quality of life is improved by using the latest dental procedures.
Single-tooth implant restorations in the esthetic zone with PureForm ceramic crowns: 3 case reports

Abstract:The anterior maxillary jaw is frequently called the "esthetic zone" because of its high visibility and influence on facial appearance. Implant placement and single-tooth restoration in this region can present some esthetic challenges for the clinician. The underlying titanium abutment, for example, can diminish porcelain translucency and result in a darkened prosthesis. Subsequent changes in soft-tissue margins can also result in the visibility of titanium at the gingival crevice. In recent years, all-ceramic restorations have gained popularity in response to increasing patient demands for improved esthetics. This article describes the clinical use of a new, tooth-shaped, ceramic coping system in the restoration of single teeth in the esthetic zones of 3 patients. Four tapered screw implants were placed in the locations of the central incisors (n = 3) and bicuspid (n = 1). Conventional ceramic protocols were used to fabricate all-ceramic crowns that were cemented onto small core abutments attached to the implants. All prostheses restored the biomechanical needs of the patients and achieved excellent esthetic results.

Multilayer Ultratranslucent Monolithic Zirconia Crowns Treated With Multimode Universal Adhesive System Containing Zirconia Primer: A Clinical Report

Abstract:Introduction Ultratranslucent zirconia combines high mechanical strength and esthetics, making it ideal for monolithic restorations. This case report describes a total rehabilitation of the upper dental arch with single‐unit crowns made of multilayer ultratranslucent zirconia (Prettau 4 Anterior, Zirkonzahn), as well as the use of a simplified adhesive cementation protocol for this type of ceramic. Materials and Methods A 64‐year‐old female patient sought rehabilitative treatment, after planning in a semi‐adjustable articulator, interim prostheses, from tooth 2 to 15, were installed to restore occlusal vertical dimension and esthetics. After all surgical, periodontal, and endodontic procedures, impressions of the upper and lower arches were taken. In the maxilla, 14 zirconia crowns with a color gradient were fabricated and cemented, with their internal surface treated with a multimode adhesive system containing zirconia primer (Single Bond Universal, 3M ESPE). The same adhesive system was applied to the preparations, and the crowns were cemented with dual resin cement (RelyX Ultimate, 3M ESPE). Results After the crowns exhibited excellent esthetic results, with only a minor fracture observed on the palatal cusps of the right first upper premolars. Conclusions Thus, ultratranslucent zirconia appears to be a viable option for oral rehabilitation with single‐unit crowns due to its excellent clinical performance. Clinical Significance Monolithic ultratranslucent zirconia crowns are able to reproduce the natural characteristics of dental elements. Additionally, the application of 10‐methacryloyloxydecyl dihydrogen phosphate‐based primer has proven to be an efficient surface treatment for adhesive cementation of UT zirconia.

Abstract:Porcelain laminate veneers are a popular cosmetic dentistry treatment for correcting discoloured, worn, misaligned, gapped, chipped, or crooked teeth. The restorative material utilized in the indirect method can be processed using CAD/CAM (computer-aided design and computer-aided manufacturing) technology or conventional technique, which is highly sensitive. Due to its multiple benefits, digital technology is growing quickly and has opened up a lot of new opportunities for dental practitioners. These days, CAD/CAM is a helpful technique that enables the creation of monolithic restorations for ceramic materials, which is most recently utilized in the field of ceramic veneers as well as digital impression capturing and digital designing as part of treatment planning. This case study details the methodical process of creating laminate veneers for a patient who wants to enhance the look of their anterior teeth utilizing both traditional and CAD/CAM technology.
CAD/CAM Ceramic Restorative Materials for Natural Teeth

Abstract:Advances in computer-aided design (CAD) / computer-aided manufacturing (CAM) technologies and their ease of application enabled the development of novel treatment concepts for modern prosthodontics. This recent paradigm shift in fixed prosthodontics from traditional to minimally invasive treatment approaches is evidenced by the clinical long-term success of bonded CAD/CAM glass-ceramic restorations. Today, defect-oriented restorations, such as inlays, onlays, and posterior crowns, are predominately fabricated from glass-ceramics in monolithic application. The variety of CAD/CAM ceramic restorative systems is constantly evolving to meet the increased demands for highly aesthetic, biocompatible, and long-lasting restorations. Recently introduced polymer-infiltrated ceramic network CAD/CAM blocks add innovative treatment options in CAD/CAM chairside 1-visit restorations. The material-specific high-edge stability enables the CAD/CAM machinability of thin restoration margins. Full-contour zirconia restorations are constantly gaining market share at the expense of bilayered systems. Advancements in material science and bonding protocols foster the development of novel material combinations or fabrication techniques of proven high-strength zirconia ceramics. CAD/CAM applications offer a standardized manufacturing process resulting in a reliable, predictable, and economic workflow for individual and complex teeth-supported restorations. More evidence from long-term clinical studies is needed to verify the clinical performance of monolithic polymer-infiltrated ceramic network and zirconia teeth-supported minimally invasive and extensive restorations.
In Vivo Cartilage Formation Using Chondrogenic-Differentiated Human Adipose-Derived Mesenchymal Stem Cells Mixed With Fibrin Glue

Abstract:Human adipose-derived mesenchymal stem cells (MSCs) were differentiated into chondrogenic MSCs, and fibrin glue was used together to explore the feasibility of whether cartilages can be generated in vivo by injecting the differentiated cells. Mesenchymal stem cells extracted from human adipose were differentiated into chondrogenic MSCs, and such differentiated cells mixed with fibrin glue were injected subcutaneously into the back of the nude mouse. In addition to visual evaluation of the tissues formed after 4, 8, and 12 weeks, hematoxylin-eosin staining, Masson trichrome staining, measurement of glycosaminoglycan concentration using dimethylmethylene blue, agreecan through reverse transcriptase-polymerase chain reaction, type II collagen, and expression of SOX-9 were verified. Moreover, the results were compared with 2 groups of controls: 1 control group that received only injection of chondrogenic-differentiated MSC and the supporting control group that received only fibrin glue injection. For the experimental group, cartilage-like tissues were formed after 4, 8, and 12 weeks. Formation of cartilage tissues was not observed in any of 4, 8, and 12 weeks of the control group. The supporting control group had only a small structure formation after 4 weeks, but the formed structure was completely decomposed by the 8th and 12th weeks. The range of staining dramatically increased with time at 4, 8, and 12 weeks in Masson trichrome staining. The concentration of glycosaminoglycan also increased with time. The increased level was statistically significant with more than 3 times more after 8 weeks compared with 4 weeks and more than 2 times more after 12 weeks compared with 8 weeks. Also, in reverse transcriptase-polymerase chain reaction at 4, 8, and 12 weeks, all results expressed a cartilage-specific gene called aggrecan, type II collagen, and SOX-9. The study verified that the chondrogenic-differentiated MSCs derived from human adipose tissues with fibrin glue can proliferate and form new cartilage. Our findings suggest that formation of cartilages in vivo is possible.
Zirconia for teeth and implants: a case series

Abstract:This case series reports on the use of nonsilica-based high-strength full ceramics for different prosthetic indications. Fifty-two consecutive patients received tooth- or implant-supported zirconia reconstructions during a 2-year period. The observation period for reexamination was 12 to 30 months. The most frequent indications were single crowns and short-span fixed partial dentures. A few implant superstructures were screw-retained, whereas all remaining restorations were cemented on natural teeth or zirconia implant abutments. Clinical examination included biologic (probing depths, bleeding on probing) and esthetic (Papilla Index) parameters, as well as technical complications. No implant was lost or caused any problems, but two teeth were lost after horizontal fracture. Overall, the periodontal parameters were favorable. Fractures of frameworks or implant abutments were not observed. Abutment-screw loosening occurred once for one premolar single crown. Furthermore, five implant crowns in the posterior region exhibited chipping of the porcelain veneering material. With regard to esthetics, no reconstructions were considered unacceptable, but three crowns were remade shortly after delivery. In this short-term study, it was observed that biologic, esthetic, and mechanical properties of zirconia were favorable, and the material could be used in various prosthetic indications on teeth or implants.

Abstract:Monolithic self-glazed zirconia is a new type of biomimetic zirconia that has been successfully used as crown and bridge restorations in the anterior and posterior regions. It exhibits excellent biomechanics and aesthetic properties through completely digital processing technology. This case presents a young patient's aesthetic rehabilitation with self-glazed ultra-thin zirconia veneers by closing the scattered diastema in the aesthetic zone of the anterior teeth with a fully digital workflow. The restorations exhibited excellent aesthetic outcomes with a minimally invasive concept and superior mechanical properties over glass-based ceramic veneers. The application of full-contour monolithic ultra-thin self-glazed zirconia restorations in the anterior aesthetic zone constitutes a reliable, fully digital clinical workflow.

Abstract:Objective To evaluate the clinical behavior of zirconia restorations fabricated by computer aided design and computer aided manufacture (CAD-CAM) system.Methods One hundred and thirteen patients with 167 unit restorations were enrolled in the study.First,the shape of abutment teeth and occlusion record was captured and digitized by Sirona inEos system.Then,each individually shaped framework was created using the cut-back program of inLab V3.85 software to leave a uniform thickness to the ceramic veneer.All restorations were veneered with Vita-VM9 and evaluated at baseline,6,12 months after delivery using the modified U S Public Health Service Criteria (USPHS).Results During the 6 to 12 months' follow-up visit,the success rate of the restorations was 96.1％ ( 147/153),none was replaced due to framework fracture or unrestorable delamination of the ceramic veneer,and all patients were satisfied with the shape and color of their restorations.Conclusions Based on the results of this study,it can be concluded that zirconia frameworks demonstrated sufficient stability for all-ceramic fixed partial denture using individual-design processing technology.
