Abstract:Burnout is a prolonged response to chronic emotional and interpersonal stressors on the job, and is defined by the three dimensions of exhaustion, cynicism, and inefficacy. The past 25 years of research has established the complexity of the construct, and places the individual stress experience within a larger organizational context of people's relation to their work. Recently, the work on burnout has expanded internationally and has led to new conceptual models. The focus on engagement, the positive antithesis of burnout, promises to yield new perspectives on interventions to alleviate burnout. The social focus of burnout, the solid research basis concerning the syndrome, and its specific ties to the work domain make a distinct and valuable contribution to people's health and well-being.

Understanding the burnout experience: recent research and its implications for psychiatry

Abstract:The experience of burnout has been the focus of much research during the past few decades. Measures have been developed, as have various theoretical models, and research studies from many countries have contributed to a better understanding of the causes and consequences of this occupationally-specific dysphoria. The majority of this work has focused on human service occupations, and particularly health care. Research on the burnout experience for psychiatrists mirrors much of the broader literature, in terms of both sources and outcomes of burnout. But it has also identified some of the unique stressors that mental health professionals face when they are dealing with especially difficult or violent clients. Current issues of particular relevance for psychiatry include the links between burnout and mental illness, the attempts to redefine burnout as simply exhaustion, and the relative dearth of evaluative research on potential interventions to treat and/or prevent burnout. Given that the treatment goal for burnout is usually to enable people to return to their job, and to be successful in their work, psychiatry could make an important contribution by identifying the treatment strategies that would be most effective in achieving that goal.

Abstract:Burnout is a phenomenon that has received considerable research attention in the past 50 years. As such, there is advanced knowledge on its prevalence, conceptualization, predictors, and outcomes. Although the literature has advanced, research on burnout is still topical. Burnout originated in the seventies but remains a contemporary problem because of persistent environmental stressors and challenges for employees and organizations as a whole. The current special section aims to stimulate knowledge on unresolved issues by bringing together contributions related to 1) the role of cognition in burnout research, 2) the development of burnout over time, 3) contextual antecedents of burnout, and 4) the prevention of and recovery from burnout. It is our hope that we will see more high-quality research and evidence-based practice related to burnout.

Abstract:A growing body of empirical evidence shows that occupational health is now more relevant than ever due to the COVID-19 pandemic. This review focuses on burnout, an occupational phenomenon that results from chronic stress in the workplace. After analyzing how burnout occurs and its different dimensions, the following aspects are discussed: (1) Description of the factors that can trigger burnout and the individual factors that have been proposed to modulate it, (2) identification of the effects that burnout generates at both individual and organizational levels, (3) presentation of the main actions that can be used to prevent and/or reduce burnout, and (4) recapitulation of the main tools that have been developed so far to measure burnout, both from a generic perspective or applied to specific occupations. Furthermore, this review summarizes the main contributions of the papers that comprise the Special Issue on “Occupational Stress and Health: Psychological Burden and Burnout”, which represent an advance in the theoretical and practical understanding of burnout.

Abstract:Background: An estimated 180,000 burn deaths occur each year, and the immune system plays a vital role in wound healing and burn complications, including inflammatory reactions and oxidative stress. This paper aims to explore the basic knowledge and dynamic hotspots in burns and immunology research with bibliometric methods. Methods: Through systematic retrieval, we ensured all the documents complied with our retrieval strategy and were included in the Science Citation Index-Expanded of the Web of Science Core Collection. Using bibliometric methodologies, the general information was delineated; and foundational knowledge, as well as dynamic research hotspots, were ascertained through VOSviewer, CiteSpace, and R-bibliometrix. Results: 8758 publications were identified from January 1st, 2000, to June 17th, 2024. The most productive and collaborative country was the USA; Harvard University was the most productive affiliation; and the most productive author was David N. Herndon. According to source analysis, the highest-impact journal is Burns. Historically, "expression" was the most frequently occurring word. "Delivery" was the most frequently occurring word in recent years. Conclusion: The domain of burns and immunology has reached a zenith, with a modest decline in publication output over the past two years, yet it continues to evolve robustly. The focal points of inquiry have evolved from the initial appraisal of immunotherapeutic interventions for critical burn injuries to the elucidation of immune cell mechanisms in burn patients. Future research trajectory is poised to innovate therapeutic modalities, encompassing anti-inflammatory, antioxidant, and targeted drug delivery systems, to enhance precision in immune modulation.

Abstract:This papers examines academic burnout syndrome in students. Referring to the Maslach and Leiter’s burnout concept, it identifies such burnout components as exhaustion, cynicism, and the sense of ineffectiveness, and specifies potential burnout causes linked to the person-organisation fit. In the paper, the author refers to the selected results of the study she developed to investigate academic burnout in Polish students in comparison with students from other European countries in which similar studies were carried out using the Maslach Burnout Inventory (MBI). Other objectives of the study were to verify the factorial structure of the author’s diagnostic tool, the Academic Adjustment Area Scale (ODA Skala Obszarów Dopasowania Akademickiego) and examine relationships between the six areas of academic life and a level of mental stress and academic burnout experienced by the study participants. The main research issue was to determine the extent to which students’ adjustment in the key areas of academic life may explain how they feel about their studies. A total of 866 students from various faculties of the Adam Mickiewicz University, Poznań (Uniwersytet Adama Mickiewicza in Poznań) participated in the study.

Abstract:Purpose Firefighters are daily confronted with adverse, unpredictable and demanding situations. It is a dangerous profession that puts firefighters at risk of developing burnout. Although the literature has already identified personal and work-related factors of burnout, the examination of specific factors explaining burnout among volunteer and career firefighters is still needed. The purpose of this study is to investigate the explaining role of personal and work-related factors on volunteer and career firefighters' burnout. Design/methodology/approach A nonrandom convenience sample of 250 firefighters (67% volunteer; M age = 31.88) completed a sociodemographic questionnaire, the Oldenburg Burnout Inventory, the Proactive Coping Scale and a standard of living subscale item. Hierarchical multiple linear regression models were tested. Fisher's criterion was considered, with p-values lower than 0.05 interpreted as statistically significant. Findings Personal and work-related factors accounted for 18% of volunteer and 31% of career firefighters' variations in burnout. Personal factors offered a greater contribution explaining volunteer and career firefighters' burnout. Still, variations in the role played by age, family responsibilities, proactive coping and satisfaction with standard of living on burnout were found among volunteer and career firefighters. Taking the work-related factors into account, working in rotative shifts constituted a risk factor for career firefighters' burnout. Research limitations/implications This study advances the understanding about the role of personal and work-related factors in volunteer and career firefighters' burnout. Originality/value This study adds information about specific factors explaining burnout among voluntary and career firefighters. It deepens existing knowledge on variations in the role played by age, family responsibilities, work conditions, proactive coping and satisfaction with standard of living on the burnout of volunteer and career firefighters.

Abstract:This study sought to investigate the measurement properties of a "gold standard" instrument for measuring burnout, the Maslach Burnout Inventory – General Survey (MBI-GS), with a systematic literature review and meta-analysis of studies that had as the primary aim its psychometric validation. The search spanned from January 1996 to December 2022 using the following databases: Web of Science, Scopus, PubMed, APA PsychINFO, ScienceDirect, and ProQuest Dissertations and Theses Global. Our search identified 35 eligible studies for inclusion in the systematic review. Of these, 17 were included in the meta-analysis. The meta-analysis of the original 16-item version studies supported a three-dimensional representation of burnout with modest internal consistencies. However, similar to the narrative findings, it also indicated the viability of a two-factor solution. Consequently, the structural validity of the MBI-GS remains unclear, and so does its cross-cultural validity. The criterion validity of the cynicism and personal efficacy scales also raised a few questions. Overall, the findings provided some support for the adequacy of the measurement properties of the MBI-GS as a research tool measuring exhaustion, cynicism, and professional efficacy. However, we also identified challenges and practices of which researchers should be cognisant and that they should consider in future burnout projects.

Abstract:Burnout is a psychological syndrome characterized by emotional exhaustion, depersonalization, and a reduced sense of personal accomplishment. Due to the demands of treating people with psychological problems, burnout is prevalent among psychotherapists. In this article, we present a comprehensive review of 44 quantitative and qualitative studies from the past decade focusing on both the risk factors for, and effects of, burnout among psychotherapists. Factors influencing burnout include perceived job control, the nature of psychotherapists' caseload, countertransference reactions, supervisory support, and psychotherapists' mental health history. Burnout affects psychotherapists' general well-being, as well as the extent to which clients engage in and benefit from psychotherapy. Implications for psychotherapists and their supervisors for burnout prevention and intervention are discussed, and recommendations for further research in this area are identified. Abstract:Background: Although burnout is not an F‑Diagnosis in the ICD-11, many people are suffering from it. This has many economic and health-related consequences, because burnout is closely tied to an inability to work and significant secondary illnesses. This article provides an umbrella review of therapy approaches for burnout that have already been used and evaluated and shows in which areas there are promising, but still little researched, therapeutic support options for those affected. Methods: A systematic literature search of studies was carried out in PubMed and Google Scholar between 2010 and 2024. The search terms were "burnout," "therapy," and "intervention" combined with "systematic review" and "meta-analysis." Overall, 10 systematic reviews or meta-analyses met the inclusion criteria. Results: In five systematic reviews or meta-analyses, the therapy approaches focused on a specific professional group (nurses, doctors, and teachers); the others related to students or various groups of people. In seven studies, the effects of mindfulness-based stress reduction on burnout were shown. The effects of cognitive behavioral therapy (CBT) were shown in four studies. Acceptance and commitment-therapy (ACT) as well as rational emotive therapy (RET) also proved to be effective. Conclusion: It still needs to be researched whether established thinking and behavioral patterns can be changed through the therapies discussed. The professional context should be given more attention. Some resource-oriented approaches like optimism, a sense of control, self-efficacy, and self-acceptance seem promising in the treatment of burnout.
Burnout and its correlates in emergency physicians: four years' experience with a wellness booth

Abstract:Objective: To measure the degree of burnout among emergency physicians (EPs) and to identify and rank predictive factors. Methods: Using the Maslach Burnout Inventory as well as a 79-item questionnaire, a cross-sectional survey was conducted for physician registrants at the Annual Scientific Assemblies of the American College of Emergency Physicians from 1992 to 1995. Degrees of burnout were stratified into low, moderate, and high ranges. Univariate and stepwise multiple regression analyses were conducted to identify and rank correlates to burnout scores. Results: Of 1,272 registrants taking the inventory, 60% registered in the moderate to high burnout ranges. Twenty-one correlates were identified. These were classified broadly in terms of negative perceptions of self, negative practice habits and attitudes, and unhealthy lifestyles. The most highly ranked correlates were: self-recognition of burnout, lack of job involvement, negative self-assessment of productivity, dissatisfaction with career, sleep disturbances, increased number of shifts per month, dissatisfaction with specialty services, intent to leave the practice within 10 years, higher levels of alcohol consumption, and lower levels of exercise. Age and years of practice were not significant predictors of burnout. Projected attrition rates were 7.5% over 5 years and 25% over 10 years. Conclusions: Elevated levels of burnout exist among a substantial percentage of surveyed EPs. However, there is evidence for a "survivor" category of practitioners for whom burnout either does not develop or is a reversible process. The projected attrition rate over 5 and 10 years appears to be no greater than that of the average medical specialty.

Abstract:Job burnout has long been recognized as a problem that leaves once-enthusiastic professionals feeling drained, cynical, and ineffective. This article proposes two new approaches to the prevention of burnout that focus on the interaction between personal and situational factors. The first approach, based on the Maslach multidimensional model, focuses on the exact opposite of burnout: increasing engagement with work by creating a better “fit” between the individual and the job. The second approach draws from the decision-making literature and reframes burnout in terms of how perceptions of the risk of burnout may lead to suboptimal choices that actually increase the likelihood of burning out. These new approaches provide a more direct strategy for preventing burnout than typical unidimensional “stress” models because these new approaches (1) specify criteria for evaluating outcomes and (2) focus attention on the relationship between the person and the situation rather than one or the other in isolation.

Abstract:BackgroundPractitioners and decision makers in the medical and insurance systems need knowledge on the relationship between work exposures and burnout. Many burnout studies – original as well as reviews - restricted their analyses to emotional exhaustion or did not report results on cynicism, personal accomplishment or global burnout. To meet this need we carried out this review and meta-analyses with the aim to provide systematically graded evidence for associations between working conditions and near-future development of burnout symptoms.MethodsA wide range of work exposure factors was screened. Inclusion criteria were: 1) Study performed in Europe, North America, Australia and New Zealand 1990–2013. 2) Prospective or comparable case control design. 3) Assessments of exposure (work) and outcome at baseline and at least once again during follow up 1–5 years later. Twenty-five articles met the predefined relevance and quality criteria. The GRADE-system with its 4-grade evidence scale was used.ResultsMost of the 25 studies focused emotional exhaustion, fewer cynicism and still fewer personal accomplishment. Moderately strong evidence (grade 3) was concluded for the association between job control and reduced emotional exhaustion and between low workplace support and increased emotional exhaustion. Limited evidence (grade 2) was found for the associations between workplace justice, demands, high work load, low reward, low supervisor support, low co-worker support, job insecurity and change in emotional exhaustion. Cynicism was associated with most of these work factors. Reduced personal accomplishment was only associated with low reward. There were few prospective studies with sufficient quality on adverse chemical, biological and physical factors and burnout.ConclusionWhile high levels of job support and workplace justice were protective for emotional exhaustion, high demands, low job control, high work load, low reward and job insecurity increased the risk for developing exhaustion. Our approach with a wide range of work exposure factors analysed in relation to the separate dimensions of burnout expanded the knowledge of associations, evidence as well as research needs. The potential of organizational interventions is illustrated by the findings that burnout symptoms are strongly influenced by structural factors such as job demands, support and the possibility to exert control.

Abstract:When I started noticing signs of burnout in myself 7 years into my work as a family physician, I was out of luck. Back then, burnout was not talked about, and most studies addressed only incidence, with a rare article mentioning prevention or interventions. Although I loved medicine, my job was exhausting me in an extreme way, and time off didn't help. The typical day felt like an obstacle course as I struggled to meet the needs of every patient with empathy amid soul-crushing productivity and time pressures.

Abstract:The burnout phenomenon, while widely acknowledged as an important issue among people-helping professionals, has to date been treated chiefly in anecdotal or descriptive reports that lack both precise theoretical foundations and substantial empirical support. Einsiedel and Tully criticize burnout research, maintaining that conceptual and operational definitions vary widely and that little effort has been expended to draw on more established literature to explain burnout. This paper proposes a new model of burnout, largely based on the work of Bandura. Burnout is defined as a state in which individuals expect little reward and considerable punishment from work because of a lack of valued reinforcement, controllable outcomes, or personal competence. The burnout literature is briefly reviewed and grouped according to the categories of the proposed burnout model.

Abstract:Objectives: To study the prevalence of burnout among non-health care workers (NHCW), the risk and protective factors and to quantify the risk of burnout. Method: We conducted a cross-sectional study on the 3142 NHCW of the University Hospital of Clermont-Ferrand. They received a self-assessment questionnaire. Results: Four hundred thirty seven (13.9%) NHCW completed the questionnaires. More than three quarter (75.4%) of NHCW was in burnout, with one in five (18.7%) having a severe burnout. Job demand was the main factor explaining the increase in exhaustion and overinvestment was the main factor explaining the increase in cynicism. Effort-reward imbalance (ERI) multiplied the risk of severe burnout by 11.2, job strain by 3.32 and isostrain by 3.74. Conclusion: NHCW from hospital staff are at high risk of burnout. The two major models of stress at work, the job demand-control-support and the ERI, were highly predictive of burnout, with strong dose-response relationships.

Abstract:Much has been published on burnout in the human services. However, despite the extensive literature on job stress in policing, burnout in police officers has rarely been studied. The present study examined stressors in police work, focusing specifically on the lack of reciprocity that officers experience in relations with civilians, colleagues and the police service. It also investigated the relationship between burnout and the attitudes of officers towards violence, as well as to their own use of violence. Dutch police officers (N = 358) completed a self-report questionnaire. The results showed that: (1) organizational stressors were more prevalent than task-related stressors; (2) compared to other service jobs, police officers report a particular profile on the three scales of the Maslach Burnout Inventory (MBI)?a relatively low level of emotional exhaustion,an average level of depersonalization, and a high level of personal accomplishment; (3) burnout is associated with a lack of reciprocity between investments and outcomes in the relations that officers have with citizens, colleagues and their organization; and (4) burnout is positively related to attitudes towards use of violence and the use of violence during the officers' duty.
